The gallstone ileus is part of the list of differential diagnoses of obstructive acute abdomen, with the prerequisite of a biliodigestive fistula through which the migration of a large gallstone occurs. The authors describe two cases of intestinal obstruction secondary to impaction of a gallstone in the ileum, highlighting its clinical conditions, radiological findings and surgical interventions.
