Abstract

Objective To systematically evaluate the efficacy of intracoronary injection of nicorandil on patients with acute myocardial infarction (AMI) after first percutaneous coronary intervention (PCI) without reflow phenomenon (NR). Methods Computer searched PubMed, Cochrane Library, Wily Online Library, Embase, CNKI, Wanfang Data Knowledge Service Platform, Weipu and other databases, and collected data from the establishment of the database until July 1, 2019 on intracoronary injection of nicorandil. A randomized controlled trial (RCT) study for the treatment of NR after the first PCI in patients with AMI. RevMan5.3 software was used for meta-analysis. Results A total of 7 RCTs were included, involving 739 patients. The results of meta-analysis showed that the incidence of NRP or slow blood flow, corrected TIMI frame number (cTFC), left ventricular ejection fraction (LVEF), and incidence of malignant arrhythmia in the nicorandil group were significantly better than those in the control group, with differences Statistically significant (P <0.05). Conclusion Intracoronary injection of nicorandil before PCI can reduce the incidence of postoperative NRP and cTFC in patients with AMI, increase the level of postoperative LVEF, and reduce the incidence of postoperative malignant arrhythmias.
