Alt Ekstremite Vasküler Cerrahisinde Bupivakain ile Levobupivakainin Etkilerinin Karşılaştırılması.

Abstract

Objective: The aim of the study was to compare the effects of intrathecal injections of levobupivacaine with bupivacaine on hemodynamic parameters, sensorial and motor block duration and side effects in adult patients undergoing lower extremity vascular surgery. Methods: After approval of our local ethics committee and the consent of the patients who will undergo lower extremity vascular surgery were obtained, 30 ASA I-III patients aged 25-75 years were randomly selected and included in two intrathecal injection study groups as follows: Group I (n=15) 17.5 mg (3.5 mL) isobaric 0.5% bupivacaine, Group II (n=15) 17.5 mg (3.5 mL) isobaric 0.5% levobupivacaine. All patients were administered 10 ml/kg lactate Ringer infusion and baseline values (T0) were recorded. After spinal anesthesia hemodynamic parameters were recorded at 2. (T1), 4. (T2), 10. (T3), 20. (T4), 30. (T5), 60. (T6) and 90. minutes (T7). In addition, side effects such as nausea, vomiting, itching, respiratory depression, hypotension, bradycardia, arrhythmia, convulsions, and tremors were recorded. Results: No significant difference was found between groups in terms of heart rate (HR), mean arterial pressure (MAP), and cardiac index (KI) parameters. The goups were similar in terms of sensorial block onset times and time elapsed till sensory block involved dermatomes, two-segment regression times and onset time of motor block. However, time to regression, and recovery from motor block was found to be shorter in Group II (p<0.05). None of the patients had ECG changes or side effects (p>0.05). Conclusion: Levobupivacaine, affects hemodinamic status in a similar way to bupicaine, it has been concluded with these findings that this drug may be used safely as local anesthetic in patients carrying cardiovascular disease potential.
