Abstract

Introduction: The kidney transplant patient should daily administer a polypharmacy that prevents rejection to the graft, infection by opportunistic pathogens and for the treatment of chronic diseases. Thus, the interaction of the clinical pharmacist with the multiprofessional team in the care of this patient is important. Objectives: The aim was d to describe and analyze the Pharmacotherapeutic follow-up (PF) of transplanted patients at a university hospital in Ceará. Method: The pharmacotherapy of transplanted patients and records of clinical pharmacy service were analyzed, Records of immunosuppressive therapies, prophylaxis, treatment of comorbidities and accompanying laboratory tests were analyzed in PF. Results: 581 drug related problems (DRP) were identified and 581 pharmaceutical interventions (PI) were conducted in 170 PM. Health problems were prevented in 76.8% of cases and in 13.1% of cases the patients improved the health problem. Conclusion: PF in transplanted patients was effective to detect DRP, perform PI, minimize the occurrence of negative results related to drugs and evaluate clinical outcomes, as well a useful tool for promoting the safety in transplanted patients.
