Abstract

Aphasic syndromes occur in an overwhelming proportion in right-handed individuals as a consequence of injuries in the left hemisphere and very rarely due to right side brain lesions (the so called crossed aphasia). Diagnostic criteria of crossed aphasia include preference for using the right hand without family history o f left handed Individuals, unilateral lesions of the right hemisphere and the structural integrity o f the left hemisphere. We present the case o f a 59 years old male patient, right-handed, without any left handed famify member, w ith sudden onset of left hemiplegia and aphasia mainly expressive. The cerebral CT scan revealed subacute ischemic strokes in the right middle cerebral artery territory and small chronic ischemic lesions In corona radiate and left internal capsule. Data obtained in patients w ith left hemisphere tumors located near the speech area show that it can lead to the reorganization of neural circuits therefore causing partial o r total migration o f the speech area to the non-dominant hemisphere. The discussion required in the case presented is whether the small ischemic lesions in the white m atter o f the left hemisphere, silent until the occurrence of the major stroke in the right hemisphere can cause relocation of the speech area in the non-dominant hemisphere or it is just coincidental in a rare case of crossed aphasia.
