Abstract
- The article discusses personalized care planning in medicine which deals with collaborations between clinicians and patients in the creation of longitudinal treatment plans, and it mentions the authors' views about whether personalized care planning can help improve primary medical care in America as of 2017. Patient-centered care and self-care are examined, along with chronic care management services (CNS), patients' goals, and the optimization of the quality of chronic disease management.
