Abstract

The REVHEPAT network designed an outpatient therapeutic education program (TEP) for patients with chronic hepatitis B and or C to optimize their care pathway. This program is part of the 3 stages of the life of the patient and his relatives before, during and after the treatment, on the basis of collective workshop. This is an innovative approach to chronic liver disease with 562 patients. The collective workshop is a space to understand the subjective experience of illness. It allows resorting to listening, what is seen and felt to overcome the difficulties in communicating about the disease and combining the complex and the intimate. The therapeutic alliance is improved thanks to a newposture of caregivers increasing their knowledge (do/be), for better prevention. The coordination of actions between therapeutic hepatologist and the general practitioner or care structure is reinforced by the TEP team. The collective workshop brings added value to patients. It is a real level that patients can seize to take care of their health, learning how to act on comorbidities and associated risk factors. Thereby, 69 collective workshops have been conducted in 2015. It is a key and a supportive 'tool' with a collective dimension at the person's service (not only focused on the liver).
