Food protein induced enterocolitis syndrome (FPIES) is a non immunoglobulin (Ig) E mediated gastrointestinal food hypersensitivity. Diagnosis is based on the history, clinical features, exclusion of other etiologies and food challenge. Management relies on the avoidance of any offending food. Herein we represent a twelve month-old boy who was believed to have had an IgE mediated food hypersensitivity after the ingestion of cow's milk based formula but who was diagnosed as FPIES after food challenge.
