Abstract

Practitioners need to work collaboratively to enable people experiencing homelessness to achieve a good standard of care whenever they have a ‘transition of care’ between hospital and community services. The Queen’s Nursing Institute, in partnership with other organisations, is sharing best practice with a network of more than 850 health professionals who specialise in working with homeless and vulnerable people.
