Abstract
- Practitioners need to work collaboratively to enable people experiencing homelessness to achieve a good standard of care whenever they have a ‘transition of care’ between hospital and community services. The Queen’s Nursing Institute, in partnership with other organisations, is sharing best practice with a network of more than 850 health professionals who specialise in working with homeless and vulnerable people. This article outlines the role primary health professionals should play in preventing homelessness and supporting people with ongoing care. [ABSTRACT FROM AUTHOR]
