2. Factors Affecting COVID-19 Vaccine Confidence and Prevalence of Post-COVID Syndrome.

- Abstract
Background/Aims: Determining the attitudes and confidence levels of society towards vaccines is important even after administering vaccinations. The aims of this study are: to determine the prevalence of post-COVID syndrome, the side effects after COVID-19 vaccinations, and the factors affecting COVID-19 vaccine confidence in individuals aged eighteen years old and over. Methods: The population of this cross-sectional study consisted of people aged eighteen years old and above living in the city center of Amasya. In total, 762 people were reached. Data collection was carried out between August 15 – September 15, 2022 by applying the questionnaire form prepared by the researchers face-to-face in rural areas and face-to-face or online in urban areas. Results: The number of people having chronic COVID disease was 55 (20.0%). Three hundred nine people (43.2%) developed side effects after at least one dose of the vaccine. The rate of trusting all the vaccines was higher among those who did not develop side effects after vaccination, who did not use social media as a source of information, who were aged 65 and over, who resided in rural areas, who had secondary school education and below, who did not work and who were housewives (p<0.001). Conclusions: One in five people who have had the disease developed chronic COVID syndrome and almost half of respondents trusted all types of COVID-19 vaccines. For vaccines to be accepted by the society, the ways in which media such as social media reduce the trust in the vaccine should be examined and the reasons for the lack of confidence in the vaccine should be determined, especially in people with a high education level. 3. Depot Versus Oral Antipsychotics in Patients with Schizophrenia: Which is Better on Side Effects, Functional Improvement, and Life Satisfaction.

Objective: Schizophrenia is a chronic mental disorder that reduces quality of life, causes deterioration in social and occupational functioning, and requires continuous care for psychosocial rehabilitation. The aim of this study was to investigate functional improvement, life satisfaction, and drug side effects in patients with schizophrenia and schizoaffective disorder using depot antipsychotics, oral antipsychotics, or both. Methods: The study included 162 patients with clinically stable schizophrenia and schizoaffective disorder who were regularly followed up at the Rize Community Mental Health Center for at least 1 year. The Satisfaction with Life Scale (SLS), Glasgow Antipsychotic Side-Effect Scale (GASS), and Functional Remission of General Schizophrenia (FROGS) scale were administered to patients with available sociodemographic and clinical data. Results: There were significant differences between the groups with respect to age at onset, number of hospitalizations, and gender. The mean GASS score of patients using both depot and oral antipsychotics was significantly higher than that of those using only depot antipsychotics. There was no significant difference between the groups in terms of mean SLS, total FROGS scores, and sub-dimension FROGS scores. Conclusion: It was concluded that it would be appropriate to prefer depot antipsychotics in the presence of poor prognostic factors such as medication non-adherence, frequent hospitalization, and lack of insight. There is a need for multicenter prospective studies with longer follow-up of patients for side effects, life satisfaction, quality of life, and functional improvement to achieve more significant results. 5. Türkiye'de COVID-19 aşı tereddüdü: Bir infodemiyoloji çalışması.

Objective: The aim of this study is to examine online search behaviour related to vaccine adverse effects, side effects, harms, and safety that can be the main basis of vaccine hesitancy and common objections to vaccines in Türkiye during the COVID-19 vaccine development and vaccination efforts. Methods: It was aimed to determine the relative search volumes (RSV) on Google Trends data from January 1, 2020, to December 31, 2022, in order to identify internet searches related to the origins of hesitations and prevalent concerns about COVID-19 vaccines. Within this scope, search query logs were scrutinized, and four primary search categories were established: (1) queries containing general or specific terms related to the adverse effects and safety of COVID-19 vaccines, (2) queries containing the name or commercial identity of the COVID-19 vaccine manufacturer and its safety, (3) queries containing the name or commercial identity of the COVID-19 vaccine manufacturer and its side effects, (4) queries containing the name or commercial identity of the COVID-19 vaccine manufacturer and its harm. Finally, these different search categories were evaluated in terms of RSV by country and region. Results: Throughout the entire pandemic, a consistent and significant increase in search interest regarding a coronavirus vaccine was observed in Türkiye, persisting at a consistently high level. The occurrence of peaks in search queries coincided with significant events including official declarations, prominent increases in the incidence of COVID-19 cases, and the announcement of noteworthy advancements in vaccine development. RSV curves pertaining to adverse events, side effects, and safety of COVID-19 vaccines exhibited distinct fluctuations during study time periods, while the RSV curve reflecting concerns about potential harm associated with COVID-19 vaccines demonstrated an overall upward. The form of RSV curves exhibited a remarkable resemblance to all provinces, excluding Bayburt. Moreover, the search interest in vaccines developed by various countries and pharmaceutical companies demonstrated a similar pattern. Interest in vaccine adverse effects was consistently the highest level (60%), followed by side effects (39%) and safety concerns (20%). Furthermore, in the course of the study period, the interval characterized by the highest magnitude of search queries and the occurrence of peak values was observed in June 2021, coinciding with the lifting of curfews. Conclusion: When considering the peak recorded in search queries in June 2021, it is thought that the dominant factor influencing search behavior could be the societal risk perception. 16. PREVALENCE AND ASSOCIATED FACTORS OF ADVERSE EFFECTS IN CHILDREN AND ADOLESCENTS TREATED WITH SELECTIVE SEROTONIN REUPTAKE INHIBITORS: A CHART REVIEW STUDY.

Objective: This study aims to examine the prevalence and associated clinical and sociodemographic factors of adverse effects in medication among naïve young subjects who received selective serotonin reuptake inhibitor (SSRI) monotherapy. Material and Methods: The medical records of 85 patients who had received SSRI monotherapy in a university hospital’s child and adolescent psychiatry clinic were reviewed. The subjects who met the inclusion criteria were included in the study. Results: A total of 67 subjects (10.82±3.63 years) were included. More than half (n=39, 58.9%) developed at least one adverse effect possibly associated with SSRI treatment, with psychic (n=25, 37.3%) and autonomic (n=20, 29.9%) adverse effects as well as behavioral activation (n=13, 19.4%) being the most frequently reported. Medication was discontinued in 13 subjects (19.4%) due to adverse effects, with behavioral activation (6 out of 13 subjects) being the most frequent reason for discontinuation. The development of behavioral activation was significantly associated with younger age, diagnosis of obsessive compulsive disorder in the subjects, and psychiatric history in the subjects’ fathers (p value<0.05). Conclusions: Despite the fact that SSRIs are generally safe and well-tolerated in young subjects, adverse effects may be frequent, and medication discontinuation may be required under some conditions. Thus, clinicians treating young subjects should be cautious, particularly about the development of behavioral activation. They should also be mindful of the clinical and sociodemographic factors associated with the adverse effects that may arise during SSRI treatment. [ABSTRACT FROM AUTHOR]

Introduction: Oculogyric crisis (OGC) is a dystonic reaction characterized as conjugate and typically upward deviation of the eyes lasting from seconds to hours which is seen as an uncommon neurologic manifestation. Metoclopramide is a prokinetic agent that is frequently prescribed in the treatment of nausea, vomiting prophylaxis, and vomiting. One of the side effects after metoclopramide treatment is acute dystonic reactions.Case Presentation: We report a case of metoclopramide-induced OGC despite the anticholinergic treatment in patient diagnosed with schizophrenia.Conclusions: Metoclopramide-induced OGC and other acute dystonic reactions may be more frequent than expected. Therefore, clinicians should monitor patients closely during treatment with metoclopramide and necessary precautions should be taken for these reactions.Keywords: Metoclopramide, oculogyric crisis, side effect, schizophrenia

Objective: Vaccination is the most effective way to control the COVID-19 pandemic all over the world. We aimed to evaluate the relationship between antibody titers and vaccine side effects after the BNT162b2 vaccine was administered as a reminder dose in healthcare workers (HCW) who received two doses of inactivated SARS-CoV-2 vaccine name CoronaVac (Sinovac Life Sciences, Beijing, China). Methods: A total of428 HCWs participated in the study. Participants who received the mRNA vaccine as a reminder dose were evaluated with a questionnaire regarding antibody values and vaccine side effects. Three weeks after the first BNT162b2 vaccine, the same questionnaire was applied face-to-face to HCW, and the same questionnaire was applied to those who received a second reminder dose via telephone. Results: Out of428, 373 (87.1%) HCWs preferred one and 55 (12.9%) two doses of the BNT162b2 vaccine as reminder doses after being vaccinated with an inactivated vaccine. It was observed that side effects were more frequent in women aged 18-40 after a single dose of the BNT162b2 vaccine (p<0.001). The most common side effects are redness, swelling, and pain at the injection site, with a rate of 59.6%. Fatigue-weakness was the most common systemic reaction, with a rate of 58.6%. Axillary lymphadenopathy was observed seen in 3 (1.1%) HCWs. The median value of IgG titers in the third week after the reminder dose was found to be higher in HCW with side effects than those without side effects (p<0.001). When the cumulative incidence rate of vaccinated people was evaluated over 389 people, no cases were observed on the 14th and 30th days after the first reminder dose of BNT162b2. However, the first case was observed on the 60th day, and after the second reminder dose, cases were seen on the 14th, 30th, and 60th days. Conclusion: Since the side effects detected after the BNT162b2 reminder dose were mild to moderate and progressed with local symptoms, it was concluded that highly protective mRNA vaccines could be safely preferred for protection from COVID-19. Purpose: Ceftriaxone is used sparingly in newborns due to its side effects. There are limited studies about its use in neonatal infections. In this study, it was aimed to evaluate the side effects that may occur after ceftriaxone therapy in newborns. Materials and Methods: The clinical and laboratory data before (baseline) and after ceftriaxone treatment of newborns hospitalized in the Newborn Intensive Care Unit of Cukurova University Medical Faculty, Balcalı Hospital between 2015 - 2016 were retrospectively analyzed. Results: Thirty cases were included in the study. The mean gestational age of these babies was 37 ± 3.1 (30-41) weeks, and mean duration of ceftriaxone treatment was 7.2 ± 2.2 (2-12) days. Urine cultures were obtained from all patients and microorganism growth was detected in 17 (56.6%) infants. Escherichia coli grew in 11 (36.6%) cultures while other bacteria were identified in 5 (16.7%) cases. Compared to baseline, significant decreases were found in blood urea nitrogen, total bilirubin and hematocrit values after ceftriaxone therapy. No side effects related to the use of ceftriaxone were observed. Conclusion: The fact that no complications were observed after ceftriaxone in our study may be encouraging, both for the use of ceftriaxone in term or near-term newborns and for conducting extensive studies. More research is needed to investigate the side effects of ceftriaxone in newborns and to determine whether it can be an alternative to other antibiotics. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: The use of antiepileptic drugs in childhood may differ in their pharmacokinetic and pharmacodynamic processes compared to adults and therefore, the effect and adverse effect profiles of drugs may change. We aimed to evaluate the knowledge, attitudes and behaviors of parents of children diagnosed with epilepsy towards antiepileptic drugs and possible adverse effects related to the use of antiepileptic drugs in children. Material and Methods: Our study was conducted by applying a questionnaire to parents. August 2019 and March 2020, 110 parents of patients who were diagnosed with epilepsy in pediatric neurology outpatient clinic were included in the study. Results: The majority of the participants were mothers (n=68, 61.8%). The mean age of the participants was 38.4±7.78 years. 70% of cases were using monotherapy, 30% of cases were using polytherapy. The most commonly used drugs from monotherapy; levetiracetam (22.7%), valproic acid (21.8%) and carbamazepine (13.6%). No side effects were observed against antiepileptic drugs in 34.5% of the cases. The most common side effects were irritability (35.5%), weight gain (18.2%), fatigue (14.5%) and behavioral problems (11.8%). The incidence of adverse effects in patients treated with polytherapy (72.7%) was found to be higher (62.3%) than in patients treated with monotherapy, although it was not statistically significant. Conclusion: We hope that our study will contribute to the enlightenment of the adverse effect profile that may be encountered during antiepileptic drug treatment in children and to the education to be given to parents. Amaç: Osteoporoz tedavisi alan hastalarda, zoledronik asit ve denosumab kullanım oranları ile bu iki ilacın yan etkilerini karşılaştırmayı amaçladık. Gereç ve Yöntem: Çalışmamızda Ocak 2016-Aralık 2017 tarihleri arasında başvuran hastaların dosyaları taranarak, osteoporoz tanısıyla zoledronik asit ve denosumab almış olanlar belirlendi. Klinik ve demografik verileri, tedavi başlangıcı, 6. ay ve 1. yıl laboratuvar değerleri ile ilaç yan etkileri kaydedildi. Bulgular: Çalışmaya 171 hasta dâhil edildi. Denosumab alanlar daha yaşlı, üre ve kreatinin değerleri daha yüksek, kreatinin klirensleriyse daha düşüktü. Yan etki açısından değerlendirildiğinde hipokalsemi, denosumab verilenlerin 1’inde (%1,29) görülürken; zoledronik asit verilenlerde rastlanmadı. Denosumab alanların 4’ünde (% 5,19), zoledronik asit alanların ise 2’sinde (% 2,12) 1 aylık takipte akut böbrek yetmezliği gelişti. Zoledronik asit alanların 2’sinde (% 2,12) grip benzeri semptomlar görülürken; denosumab alanlarda gözlenmedi. Ayrıca zoledronik asit alanların 1’inde (% 1,06) tedavi sonrası çene osteonekrozu olmaksızın, 6 dişinde kendiliğinden dökülme; denosumab alanların 1’inde (% 1,29) ise iskium pubiste bilateral spontan kırık tespit edildi. Sonuç: İki ajan karşılaştırıldığında, yan etki açısından iki grup arasında anlamlı fark olmadığı; gelişen yan etkilerin ise literatürde belirtilenden daha az olduğu; kullanılacak ajanın seçiminde komorbidite yanı sıra, çoklu ilaç kullanımı ve nadir yan etkilerin akılda tutulması gerektiği kanısına varıldı.

27. 65 Yaş Üstü Osteoporoz Hastalarında Denosumab ve Zoledronik Asit'in Yan Etkilerinin Karşılaştırılması.

Objective: We aimed to compare the rates of denosumab and zoledronic acid in patients receiving osteoporosis treatment and the side effects of these drugs. Material and Method: In our study, the files of the applicants were searched between January 2016-December 2017; those with denosumab and zoledronic acid. Clinical and demographic data, laboratory values of beginning of treatment, 6 months and 1 year and side effects were recorded. Results: 171 patients were included. Denosumab group was older, their urea and creatinine levels were higher and GFRs were lower (respectively, p: <0.001, p: <0.001, p: <0.001, p: <0.001). Hypocalcemia was observed only in denosumab group (1 (1.23%) patient). Acut renal failure occured in 4 (% 5,19) patients with denosumab and 2 (%2,12) patients with the other group during 1 month follow up. Flu-like symptomps were observed only in zoledronic asit group (2 (%2,12) patients). Bilateral spontaneous fracture of the ischium pubis was observed in 1 (1.29%) of the former, spontaneous 6 teeth loss in 1 (1.06%) of the latter group. Conclusion: There were no significant differences between two groups in terms of side effects. Side effects were lower than those reported in the literature; in addition to comorbidity, multiple drug use and rare side effects should be kept in mind. [ABSTRACT FROM AUTHOR]

Objective Primary bladder pain syndrome (PBPS) is characterized with suprapubic pain accompanied by at least one lower urinary tract symptoms including frequent urination, urinary urgency and nocturia for more than 6 weeks. While there are many alternative therapies for the treatment of PBPS, the only approved oral medication is PPS (pentosan polysulfate sodium). As it has been associated with retinal toxicity after its widespread use, this study aims to evaluate the relationship between PPS use and maculopathy. Material and Methods The patients diagnosed with PBPS between 2010 and 2020 who may only benefit from PPS use were included into the study after subgroup and phenotype assessment (urinary and non-ulcerative organspecific subgroups). In our study, patients who had history of degenerative maculopathy or diseases predisposing to maculopathy (age-related macular degeneration, diabetes mellitus, hypertension, chronic vascular disorders, central serous chorioretinopathy, retinal dystrophy, epiretinal membrane, and chronic exposure to hydroxychloroquine) were excluded to prevent possible misdirection. Patients underwent best-corrected visual acuity assessment using Snellen chart, anterior segment and fundus examination using slit lamp biomicroscopy, and intraocular pressure measurement. Color vision test (Ishihara test), posterior segment optical coherence examination and 10-2 visual field test were performed, and color images of the fundus and autofluorescence imaging were obtained. Best-corrected visual acuity, color vision results, macular, choroidal and mean retinal nerve fiber thicknesses, mean deviation of the visual field and fundus findings were recorded. Results Out of 15 patients included into the study, 4 (37.5%) were male and 11 (73.3%) were female. The mean age of the patients was 53.3±11.2 years. During the follow-up, the duration of oral PPS use was found to be 33.01±10.59 months, cumulative oral PPS dose to be 216.02±97.63 g and duration of diagnosis to be 66.64±39.37 months. The mean central macular thickness of the patients was measured to be 254.55±33.11 μm, and the mean choroidal thickness to be 261.82±34.22 μm. Mean deviation of the visual field of the patients was found to be -1.89 ±-1.25 dB. The mean retinal nerve fiber thickness was measured to be 98.1±17.62 μm from the fundus autofluorescence images of the patients. Furthermore, in the present study, the ocular findings of the patients who are at below and above the mean cumulative dose and exposure period were compared. Conclusion This study detected no correlation between longterm PPS use and maculopathy. When forming the patient group; it is crucial to exclude patients with comorbidities such as diabetes mellitus and hypertension, and to form a homogeneous group by phenotype and subgroup assessment. 31. Anti-nucleocapsid Antibody Response After Two Doses of CoronaVac® Among Healthcare Workers.

Introduction: The Coronavirus disease-2019 (COVID-19) pandemic that started over two years ago has led to high mortality and morbidity. Vaccine studies have been initiated worldwide to end the pandemic, and the CoronaVac® vaccine was first administered to healthcare workers at high risk of COVID-19 in Turkey. In our study, we aimed to investigate serum antibody levels after vaccination. Materials and Methods: Volunteer healthcare workers without COVID-19 disease who received two doses of CoronaVac® vaccine 28 days apart and were at least 14 days after the last dose of vaccine were included in this study. Assessment of antibodies against Severe acute respiratory syndrome-Coronavirus-2 (SARS-CoV-2) in blood samples from participants was performed using the Elecsys® anti-SARS-CoV-2 electrochemiluminescence immunoassay. Samples with a cut-off index (COI) (COI; signal sample/cut-off) <1.0 were considered negative; whereas samples with COIâ?¥1.0 were deemed positive. Results: A total of 269 healthcare workers, 168 women (62.5%), were included in our study. The mean age of the participants was 37.7±8.6 (minimum-maximum: 21-62). Antibody levels were positive in 188 (69.9%) of the participants. The median antibody level was 9.2 COI (interquartile ranges=3-34.7). In terms of mean age, the mean age of participants with negative antibodies was higher with a statistically significant difference (p=0.001). The antibody positivity rate of women was higher than that of men (p<0.001). No statistically significant association was found between the time elapsed after vaccination, presence of comorbidities, development of post-vaccine side effects, and antibody levels. It was found that one or more side effects developed in 45.7% of the participants after vaccination. Conclusion: Our study showed that seropositivity developed significantly in healthcare workers after the CoronaVac® vaccine. It emphasizes the importance of maintaining infection prevention and control measures and administering the SARS-CoV-2 vaccine for healthcare workers at high risk. [ABSTRACT FROM AUTHOR]

Giriş: Solid organ malignitesi olan hastalarda kemoterapi, radyasyon tedavisi, kortikosteroid kullanımı gibi tedaviler nedeniyle immün sistem baskılandığı için aşı ile önlenebilen hastalıkların görülme ve ciddi seyretme oranları daha yüksektir. Bağışıklık kazanmış hastalarda bile yoğun anti-kanser tedavisi sonrasında aşı ile önlenebilen hastalıkların antikor titrelerinin azaldığı bilinmektedir. Aşı yanıtı kanserin tipi, hastanın yaşı, kemoterapinin yoğunluğu ile ilişkilidir. Kanser tedavisi gören hastaların yaşına ve endikasyona uygun aşılarının yapılması düşünülmeli ve bu aşıların uygulanmasının sorumluluğu onkolog ile birinci basamak sağlık hizmeti sağlayıcısı arasında paylaştırılmalıdır. Çalışmamızda hastanemiz erişkin aşı polikliniğine başvuran solid organ malignitesi olan hastalarda hepatit A virüsü, hepatit B virüsü, tetanozdifteri, pnömokok, influenza ve meningokok bağışıklama durumunun retrospektif olarak değerlendirmek ve aşı sonrası istenmeyen etkilerin (ASİE) araştırılması amaçlanmıştır. Gereç ve Yöntem: Temmuz 2022-Mart 2024 tarihleri arasında çeşitli kliniklerden erişkin aşı polikliniğine yönlendirilmiş solid organ malignitesi olan hastaların aşılanma durumu ve ASİE durumları retrospektif olarak hastane kayıt sisteminden ve poliklinik defterlerinden elde edilmiştir. Bulgular: Çalışmaya 129 hasta dahil edilmiştir. Hastaların yaş ortalaması 56,7±13,1 (18-85) olarak bulunmuştur. Hastaların 73'ü kadın (%56,6), 56'sı (%43,4) erkektir. Aşı için başvuran hastalarda en sık görülen maligniteler meme kanseri (s: 40, %31), akciğer kanseri (s: 18, %13,9), kolon kanseri (s: 18, %13,9) ve ürogenital kanserlerdir (prostat, over gibi) (s: 18, %13,9) (Şekil 1a). Hastaların 124'üne pnömokok, 90'ına tetanozdifteri, 83'üne hepatit B, 13'üne hepatit A, üçüne influenza ve sekizine ise meningokok aşısını uygulanmıştır (Şekil 1b). Hiçbir hastada hekime başvurmayı gerektirecek ASİE gelişmemiştir. Sonuç: Çalışmamızda hastalarımızın hiçbirinde aşı sonrasında yan etki görülmemiş olması onkoloji hastalarında da aşıyı güvenle uygulanabileceğini göstermektedir. Hekimlerin kanser hastalarında aşıların güvenliği ve etkinliği konusundaki endişeleri, hastaların ASİE nedeniyle aşıya karşı ön yargılı olabilmeleri nedeniyle ne yazık ki bu hasta grubuna aşılama istenilen düzeyde değildir. Bu hastalarda aşılar, enfeksiyon oranlarını ve şiddetini azaltarak ve anti-kanser tedavisinin kesintilerini en aza indirerek, aşıyla önlenebilir hastalıklara bağlı morbidite ve mortalitenin azaltılmasına yardımcı olabilir. 34. Determining the Side Effects of Covid-19 (Sinovac) Vaccination on Nurses; an Independent Descriptive Study.

Objective: It is important to protect nurses fighting against the COVID-19 pandemic through vaccination. This study aims to determine the incidence rate of side effects experienced by nurses after their COVID-19 (Sinovac) vaccination and relevant factors. Methods: This is a descriptive design study. The sample of the study included 355 nurses who received the Sinovac COVID-19 vaccine in Turkey and who agreed to participate in the study. Data were collected using a questionnaire which was written by the researchers and gathered information on the nurses' sociodemographic and professional characteristics, health status, habits, and vaccine complications. Results: The study included 355 nurses, of which 82.3% were female. Their median age was 35.42±9.67, and their mean BMI was 24.87±4.54. The most common local side effect experienced after the vaccination was pain (54.6%) while the most common systemic effects were fatigue (39.2%) and headache (34.1%). Pain, among local side effects, was significantly higher among male nurses (p= 0.001) and those who worked more than 40 hours a week (p= 0.001). The systemic side effect of fatigue was experienced at a higher rate among nurses who were diagnosed with COVID-19 before their vaccination (p=0.004), those who drank alcohol (p=0.028), and those who worked more than 40 hours a week (p=0.012). The systemic side effect of fever was more common among nurses with chronic conditions (p=0.037). Conclusions: The most common systemic side effect experienced after the COVID-19 vaccination was reported as fatigue and the most common local side effect was pain. Considering the relevant factors that affected the incidence rates of side effects, it would be suitable to plan the weekly working hours of nurses no longer than 40 hours. It can be recommended to conduct more randomized controlled studies to determine what else weekly working hours affect among nurses. [ABSTRACT FROM AUTHOR]

35. Bupropion-induced leukopenia: A case report.

Bupropion hydrochloride, a norepinephrine/dopamine reuptake inhibitor, is administered for the treatment of depression and smoking cessation. Common side effects of bupropion are dry mouth, nausea and insomnia, also it may lower the seizure threshold. The normal range of total white blood cell (WBC) count is 4000 -11000/pl for adults. The values below 4000/pl, are defined as leukopenia. A 33-year-old woman admitted to the psychiatry outpatient clinic with the complaints of mild depression, also wanted to quit smoking. Bupropion hydrochloride (extended release-XL) 150 mg/day was initiated to the patient. The leukocyte count of her treatment was 3890/pl at the third month and 3730/pl at the fourth month. The leukocyte count was at normal value before initiation of bupropion hydrochloride (7220/pl) and after stopping the treatment (7290/pl). She did not have any chronic disease, medication and drug or alcohol abuse. According to this case, it is probable that there was a relationship between bupropion hydrochloride and leukopenia as an adverse event. Many idiosyncratic drug reactions involve blood dyscrasias. Some of the psychotropic drugs have been associated with leukopenia and sometimes agranulocytosis. Although some rare studies and case reports related to leukopenia have been reported with some of the antidepressants but to our knowledge bupropion induced leukopenia is not a common side effect. Therefore, reporting this adverse event due to bupropion is important to make a contribution to literature. [ABSTRACT FROM AUTHOR]

37. Case Report: Acute Unilateral Uveitis Induced by Infusion of Zoledronic Acid.

Bisphosphonates are a group of drugs that inhibit osteoclast-mediated bone resorption, used for treating osteoporosis, Paget's disease, metastatic bone disease, and hypercalcemia caused by malignancy. Zoledronic acid treatment, which is the most potent member of the group and is administered annually, is frequently preferred due to high patient compliance. The most common side effect in the first 3 days after administration is transient flu-like syndrome, which has also been reported to cause serious ocular adverse events. Although the most common ocular side effect is nonspecific conjunctivitis, it can also cause serious symptoms such as uveitis and scleritis. A limited number of cases diagnosed as uveitis triggered by zoledronic acid have been reported in the literature. In this article, we presented the occurrence of unilateral anterior uveitis 24 h after the application in a 62-year-old female patient who was under oral letrozole therapy for breast cancer diagnosed previously and was treated with zoledronic acid for osteoporosis. A detailed ophthalmologic medical history should be taken for patients who will be prescribed zoledronic acid. Additionally, recent bisphosphonate use should be questioned in patients presenting with symptoms of uveitis. Clinicians should warn patients about symptoms that may develop related to uveitis, which is a very rare but serious side effect of bisphosphonates and should promptly evaluate patients by an ophthalmologist when any symptoms develop. Amblyseius swirskii Athias-Henriot (Acari:Phytoseiidae) and Euseius scutalis Athias-Henriot (Acari: Phytoseiidae) are important predatory mites for citrus orchards in Turkey. The side effects of insecticides on these predatory mites have been rarely studied. In this study was investigated to the effect of tau-fluvalinate on A. swirskii and E. scutalis. The studies have conducted both laboratory and field. Field studies were carried out detecting of the side effects of tau-fluvalinate on the mixed stages of A.swirskii. and E scutalis. at Seyhan and Kozan districts in Adana, while laboratory studies were evaluated the side effects of tau-fluvalinate on the nymph stages of A. swirskii The studies were carried out following the standard guidelines which were adopted and issued by the IOBC-WPRS Working group ‘Pesticedes and Benefical Organism’. In the field studies, the effects of the pesticides were calculated according to the Henderson-Tilton formula. As a result of study, tau-fluvalinate was determined as toxic on predatory mites in the laboratory studies. Side effects of abamectin, one of the standard toxic acaricides, on predatory mites were found to be “T” (Toxic) according to IOBC class value. Side effects of tau-fluvalinate were found to be ’M’’ (Moderately Harmful) on 7th days while were detected to be “N” (Harmless) on 28th days according to IOBC class value in field condition. The fields studies at Kozan and Seyhan districts showed similar results.

40. 65 Yaş Üstü Osteoporoz Hastalarında Denosumab ve Zoledronik Asit'in Yan Etkilerinin Karşılaştırılması.

Objective: We aimed to compare the rates of denosumab and zoledronic acid in patients receiving osteoporosis treatment and the side effects of these drugs. Material and Method: In our study, the fles of the applicants were searched between January 2016-December 2017; those with denosumab and zoledronic acid. Clinical and demographic data, laboratory values of beginning of treatment, 6 months and 1 year and side effects were recorded. Results: 171 patients were included. Denosumab group was older, their urea and creatinine levels were higher and GFRs were lower (respectively, p: <0.001, p: <0.001, p: <0.001, p: <0.001). Hypocalcemia was observed only in denosumab group (1 (1.23%) patient). Acut renal failure occured in 4 (% 5,19) patients with denosumab and 2 (%2,12) patients with the other group during 1 month follow up. Flu-like symptomps were observed only in zoledronic asit group (2 (%2,12) patients). Bilateral spontaneous fracture of the ischium pubis was observed in 1 (1.29%) of the former, spontaneous 6 teeth loss in 1 (1.06%) of the latter group. Conclusion: There were no signifcant differences between two groups in terms of side effects. Side effects were lower than those reported in the literature; in addition to comorbidity, multiple drug use and rare side effects should be kept in mind. [ABSTRACT FROM AUTHOR]

- Abstract
Amaç: Serviks kanseri tanısıyla definitif (kemo) radyoterapi +/- brakiterapi uygulanan 65 yaş ve üzeri hastaların ulusal çok merkezli tedavi sonuçları ve tolerabilitenin retrospektif değerlendirilmesi. Gereç ve Yöntem: Ocak 2007 - Aralık 2021 arasında tedavi edilen ve on yedi ulusal merkezden en az 6 ay izlemi olan 458 hasta retrospektif değerlendirildi. Ortanca yaş 71 (65-91) olan hastaların FIGO evresine göre 74'ü (%16,2) evre IB-IIA, 384'ü (%83,8) evre IIB-IVA idi. Eşzamanlı kemoradyoterapi (KRT) alan 346 (%75,5) iken sadece RT uygulanan hasta 112 (%24,5) idi. Ortanca 50,40 Gy (45-64) tedavi dozu, 215 (%46,9) hastada üç boyutlu konformal RT (3BKRT), 243 (%53,1)'ünde yoğunluk ayarlı RT (YART) / volumetrik ark tedavi (VMAT) ile uygulandı. Eksternal RT'yi takiben brakiterapi (BR) hastaların 415'inde (%90,6) uygulanırken 43'ünde (%9.4) eksternal RT ile boost yapıldı. Genel sağkalım (GS), kansere spesifik sağkalım (KSS), hastalıksız sağkalım (HS), metastazsız sağkalım (MS), lokal yinelemesiz sağkalım (LYS) Kaplan-Meier, log-rank ve Cox regresyon analizleri kullanılarak değerlendirildi; p<0,05 istatistiksel olarak anlamlı kabul edildi. Bulgular: Ortanca izlem 48 ay (6 -243); 4 yıllık GS, KSS, HS, MS, LYS sırasıyla %65,7, %79, %76, %82, %92,1 idi. 4 yıllık GS, tek başına RT uygulananlarda %51,8 iken eşzamanlı KRT için %70,5 olarak saptandı. GS, KSS, HS, MS etkileyen faktörler tek değişkenli analizde performans durumu (PS) (p<0,0001), tümör çapı (=4cm, >4cm) (p=0,004, p=0,008, p<0,0001) histopatoloji (skuamöz, adeno ve diğerleri) (p=0,001, p<0,0001), lenf nodu varlığı (LN) (p<0,0001), tedaviye yanıt oranı (TY) (tam, kısmi, yanıtsız) (p<0,0001) istatistiksel anlamlı bulundu. GS için ayrıca yaş (65-70, 71-79, =80) (p<0,0001), tedavi şekli (KRT, RT) (p<0,0001), BR uygulanması (p<0,0001); KSS ve MS için BR uygulaması (p=0,025, p=0,003) ve LYS için ise TY (p<0,0001) anlamlı parametrelerdi. Çok değişkenli analizde GS, KSS, HS, MS için bağımsız prognostik faktörler TY (p<0,0001), LN (p<0,0001, p=0,007, p=0,003, p=0,002) ve GS, HS için PS (p= 0,003, p= 0,004), GS için yaş (p=0,045), TŞ (p<0,0001) idi. MS için histopatoloji (p=0.026) ve BR uygulanması (p=0.049) anlamlı olarak bulundu. Derece 3-4 erken yan etki oranları bulantı kusma %0,4, gastrointestinal %1,3, genitoüriner %0,2, hemotolojik %3,5 iken geç yan genitoüriner %0,7 ve gastrointestinal %1,3 olarak gözlendi. Sonuç: Bu çalışmada serviks kanseri tanılı 65 yaş ve üzeri hastalarda eşzamanlı KRT'nin genel sağkalıma olumlu katkısı gösterilmiştir. Lenf nodu pozitifliği ve radyoterapiye kısmi yanıt veya yanıtsızlık durumu tüm sağkalımlar için olumsuz bulunmuştur. Ayrıca tanıda düşük PS varlığı GS, KSS, HS'de, skuamöz dışı histoloji ve BR yapılmaması ise MS için bağımsız kötü prognostik faktörlerdir. Geriatrik hastaların düşük yan etki oranlarıyla küratif tedavileri tamamladıkları çalışmamızda gösterilmiş olup özellikle lokal ileri evre serviks kanserinde sağkalımlara olumlu yansıdığı bilinen definitif KRT geriatrik yaşta da önerilir. 43. Hipofrafsiyone Radyoterapi Alan Meme Kanserli Hastaların Bildirdiği Yan Etkiler ve Tedavi Faktörleri Arasındaki İlişki.

- Abstract
Amaç: Meme kanseri adjuvan radyoterapisinde geleneksel 1,8-2 Gy fraksiyon dozları yerine hipofraksiyone rejimlerin etkinliğinin yapılan çalışmalarda gösterilmesi ardından günlük pratiğe yoğun olarak girmiştir. Bu çalışmanın amacı kliğimizde covid pandemisi sonrasında yoğun olarak uyguladığımız hipofraksiyone radyoterapi ile tedavi edilen meme kanserli hastalarda kendisinin belirttiği genel ve cilt yan etkilerini değerlendirmek; yan etkilere yol açabilecek etkenlerin analizini yapmaktır. Gereç ve Yöntem: 2021-2022 yılları arasında kliniğimize başvuran ve meme kanseri nedeniyle radyoterapi endikasyonu koyduğumuz hastalar arasından hipofraksiyone radyoterapi uyguladığımız vakalar çalışmaya dahil edilmiştir. Tüm hastalara 40 Gy/15fx radyoterapi uygulanmıştır. Toplam 41 hastanın analizi yapılmıştır. Ortalama yaş 53.2'dir. 41 hastanın 27'si (%69,5) neoadjuvan veya adjuvan kemoterapi almıştır (Tablo 1). Cerrahi yöntem olarak 33 (%80,5) hastaya Meme Koruyucu Cerrahi ve 8 hastaya (%19,5) Mastektomi uygulanmıştır. Meme Koruyucu Cerrahi yapılan 33 hastanın 10'una; Mastektomi uygulanan 8 hastanın 7'sine radyoterapi alanına lenf nodları dahil edilmiştir (Tablo 1). Bulgular: Tedavi sonunda hastalardan genel sağlık durumları ve cilt yan etkilerini bildirmeleri istenmiştir. Yan etki olarak: Halsizlik, iştahsızlık, memede hassasiyet/ağrı, kaşıntı ve yanma 0-3 arasında derecelendirilmiştir. Analiz sırasında az veya hiç yan etki (0), orta veya çok yan etki (1) olarak sınıflandırıldı. Hastaya ait faktörler olarak hasta yaşı, vücut kitle indeksi, meme hacmi ve tedavi ile ilgili olarak öncesinde kemoterapi alıp almaması, tedavi volümü (meme/göğüs duvarı ±periferik lenf nodları) ve cilt üzerinde maksimum dozlar değerlendirildi. Ayrıca tedavi öncesi hemogram tetkiklerinden nötrofil/lökosit oranları (N/L ratio) hesaplandı. Lojistik regresyon analizi ile univaryant ve multivaryant analizler yapıldı. İkili yanıtlanan yan etkiler için point biserial korelasyon, diğerleri için Spearman korelasyon hesaplanmıştır. Sonuç: Hasta özellikleri Tablo 1'de belirtilmiştir. Yapılan tek değişkenli ve çok değişkenli analizlerde anlamlı bir fark bulunmamıştır. Hastaların belirttiği yan etkiler Tablo 2'de özetlenmiştir. Hastaların ileri yaş olmasının halsizlik (-0.1834), iştahsızlık (-0.0815), hassasiyet-ağrı(-0.24604), yanma (-0.259) ile negatif korele olduğu görüldü. Dmax cilt dozunun artması ile halsizlik (0.45282), hassasiyet-ağrı, kaşıntı şikayetleri arasında pozitif korelasyon tespit edildi. N/L oranı ile sadece halsizlik şikayeti arasında pozitif korelasyon (0.33151) tespit edildi. VKI kaşınma ile negatif korele (-0.1280) iken yanma şikayeti ile pozitif korele (0.8259) bulundu. Hastanın belirttiği yan etkiler tedavinin etkinliğinin değerlendirilmesinde önemlidir. Bu çalışmada hasta ve tedaviye ait faktörler arasında univaryant ve multivaryant analizlerde anlamlılık bulunmamakla birlikte ciltte maksimum dozun artması ile memede şikayetlerin arttığı görülmüştür. 44. Simülasyon Tomografisinde Rektum Çapının Değişimi, Vaka Raporu.

- Abstract
Amaç: Prostat bezi, mesane ve rektum gibi pozisyonu ve hacmi oldukça değişken organların komşuluğundadır. Bu sebeple prostat kanseri radyoterapisi zorlayıcı olabilmektedir. Bu nedenle hastaya özel görüntüleme protokolleri oluşturulması ve fraksiyon içi görüntüleme/takip sistemlerinin kullanılmasını öneren çalışmalar mevcuttur. Sunulan vaka radyoterapi planlaması için çekilen simülasyon bilgisayarlı tomografisi sırasında kısa sürede rektum çapı ve hacminin nasıl değişebileceğini göstermektedir. Olgu: 67 Yaşında erkek özgeçmişinde özelliği olmayan hasta 2 yıldır idrara sık çıkma şikayetiyle üroloji polikliniğine başvuruyor. Tanısal tetkikler sırasında 11.11.2020 tarihinde bakılan total prostat spesifik antijen (PSA) değeri 20.67 µg/L olarak saptanıyor. İleri değerlendirme amaçlı 08.02.2021 tarihinde prostat bezinden 12 kadran iğne biyopsi yapılıyor. Patoloji sonucu adenokarsinom, Gleason skoru 5+5, tümör biyopsinin %90'ını kaplamakta, perinöral invazyon mevcut olarak raporlanıyor. 23.02.2021'de çekilen 68Ga-PSMA tüm vücut pozitron emisyon tomografisinde sağ seminal vezikülde artmış 68Ga-PSMA tutulumu (SUVmax: 15,40) saptanıyor. Hasta cT3bN0M0 evre IIIC kabul edilip definitif radyoterapi planlanıyor. Tedavi planı amacıyla çekilen simülasyon bilgisayarlı tomografide (BT) mesane doluluğunun değerlendirildiği dar alan tomografi çekimi (Şekil 1, 2) ile tedavi planının yapılacağı BT çekimi (Şekil 3,4) arasındaki 7 dakikalık sürede symphisis pubis hizasında rektum çapının 27,9 mm'den 46,8 mm'ye çıktığı saptanıyor. Sonuç: Prostat kanseri radyoterapisinde üriner, gastrointestinal, seksüel yan etkiler çalışmalarda farklı oranlarda da olsa görülmektedir [1]. Olası yan etkileri azaltabilmek amacıyla görüntü kılavuzluğunda radyoterapi (IGRT) kullanılması önerilmektedir[2]. Bunun yanında ultrason, manyetik rezonans görüntüleme ile fraksiyon içi görüntüleme öneren çalışmalar da mevcuttur[3]. Hastamızda simülasyon çekimi sırasında saptanan değişim tedavi sırasında da benzer değişimlerin olabileceğini düşündürmektedir. Fraksiyon içi görüntüleme sistemlerinin kullanımının artması tedavi güvenilirliğini ve olası yan etkilerden korunma açısından faydalı olabilir. [ABSTRACT FROM AUTHOR]

Objective: Although antimicrobial medications are responsible for the well being of many cases in the world through the history; today it is perceived as a group of medicine which is becoming less effective due to its widespread use by both in-patients and out-patients. Inappropriate use of these medication increases antimicrobial resistance of the human body as well as causing many medical, economical and public health related issues. The purpose of this study is to evaluate inappropriate use of antibiotics for in-patients and to diagnose adverse effects related to such use in addition to determining the most common causes of these adverse effects. Methods: A prospective study was conducted during the discharge of 400 in-patients who used various antibiotics between 30th October and 3rd March 2007 at Akdeniz University Hospital. Underlying reasons of misappropriate antibiotics use as well as adverse effects of inappropriate antibiotic usage has been studied. Results: TDistribution of the patients are as follows: 107 (%27) internal medicine, 239 (%73) surgery department. Infection Department has been involved with the treatment of 98 (%24.5) patients. 84% of the treatments conducted in surgery departments were prophylactic and %95.5 of these treatments were found inappropriate. Studying the underlying reasons of inappropriate treatment, antibiotics use for a term longer than needed was found to constitute 49% of the cases. The most common causes of starting antibiotics in the internal medicine department, with 89%, was empirical. The treatment was found to be effective for %89.5 of patients. It has been understood that 64.5% of the in-patients in the internal medicine and 40% of the patients in the surgery departments knew they were prescribed antibiotics. Nonetheless, 56% of the patients in the Internal Medicine Department and 79% of the patients in the Surgery Department did not know why they were being prescribed antibiotics. Allergic history surveys were conducted only on 22.4% of the patients in the internal department and 20% of the patients in surgical departments. Only %4.7 of the patients in the Internal Medicine Department and %0.7 patients in the Surgery Department have been informed about the side effects of antibiotics before they were being given these medications. Side effects were observed in %9.3 of the patients in the Internal Medicine Department and %3 of the patients in surgery departments. On patients who were being treated by the Infection Department, the rate of inappropriate antibiotics use was %12. Majority of these inappropriate uses were due to the fact that doctors did not follow the instructions given by the attending doctors of the Infection Department. The highest rate of misusage was observed with prophylactic antibiotic use (70%). Patients were not properly informed about the antibiotic treatment. Only %64 of the patients initially regulated by the Infection Department knew that they were taking antibiotics. 57% of these patients were explained why they were being prescribed with antibiotics. Antibiotic allergy survey was conducted on 80% of the patients and %11 of the patients faced with side effects. Analyzing all three sections, it has been understood that antibiotics usage rate is high if the patients were initially evaluated by the Infection department, who follows the guidelines of drug use. During the study, it has been found out that TUFAM has not been informed by the university about events of adverse medicine effects. Conclusion: As a result, we strongly believe that adverse effects of inappropriate antibiotics use shall be prevented by employing sufficient amount of qualified infectious disease specialists in hospitals. Objective: Lenalidomide is an effective immunomodulatory derivative drug used in the treatment of multiple myeloma (MM). It is available in original and generic forms in Turkey, but there is no clinical study that has compared the effectiveness and adverse events (AEs) of the generic and original forms of lenalidomide. We compared the effectivity and AEs of generic and original lenalidomide in patients with relapsed/refractory MM (RRMM). Materials and Methods: Patients with RRMM using original or generic lenalidomide were evaluated retrospectively. Overall response (OR), complete response (CR), very good partial response (VGPR), partial response (PR), stable disease, and progressive disease rates and hematologic and nonhematologic AEs were evaluated in these RRMM patients. The results were described as numbers, frequencies, and percentages and were analyzed using PASW 19.0 for Windows with chi-square and Fisher exact tests. Results: The number of patients using original lenalidomide was 55 and the number of patients using generic lenalidomide was 43. The OR rate was 67.2% for patients using original lenalidomide and 60.4% for those on generic lenalidomide. CR and VGPR rates were 14.5% and 45.4% in the original group while the CR and VGPR rates were 20.9% and 18.6%, respectively, in patients using generic lenalidomide. Hematologic AEs were similar in the two groups while some nonhematologic AEs were less common in the original lenalidomide group than the generic group. Only pyrexia as a grade 3-4 AE was more common in the original lenalidomide than the generic lenalidomide group. Conclusion: This study showed that the generic form of lenalidomide has similar efficacy with the original form of lenalidomide in the treatment of RRMM. The AEs of original lenalidomide were generally fewer than those of generic lenalidomide. Further studies involving a larger number of patients with RRMM would be useful for comparing the efficacy and AEs of original and generic lenalidomide. [ABSTRACT FROM AUTHOR]

Kanser tedavisinde cerrahi ve radyoterapi lokal tedaviler olup sistemik bir tedavi olan kemoterapinin yeri ve önemi büyüktür. Kemoterapinin, kullanılan tedavi rejimine bağlı olarak birçok yan etkisi vardır. Yorgunluk, bulantı ve kusma, nötropeni, anemi, periferal nöropati, uyku bozuklukları kemoterapinin en sık görülen yan etkilerindendir ve hastaların yaşam kalitesini önemli ölçüde etkilemektedir. Kemoterapiye bağlı yan etkilerin yönetiminde önerilen ancak etkinliği tartışmalı olan yöntemlerden biri de Progresif Gevşeme Egzersizleridir (PGE). Progresif Gevşeme Egzersizleri’nin kanser hastalarında ağrı, anksiyete, depresyon, uyku bozuklukları, kemoterapiye bağlı bulantı, kusma, yorgunluk gibi sorunlar üzerinde etkinliğini değerlendiren az sayıda çalışma vardır ve yeterli kanıt yoktur. Yapılan çalışmaların da metadolojik açıdan zayıf olduğu bildirilmiştir. Kanser hastaları gerek hastalığın etkisi veya gerek tedavi yöntemlerinin yan etkileri nedeniyle zorlu bir süreçten geçmektedir. Bu hastalara standart farmakolojik tedavinin yanında progresif gevşeme eğitiminin verilmesi gibi ileri düzeyde, basit, pahalı olmayan ve güvenilir hemşirelik uygulamalarının semptom kontrolünü artıracağı ve yaşam kalitesini geliştireceği düşünülmektedir.

Objective Colorectal cancer (CRC) is an important cause of cancer-related deaths. The aim of this study was to evaluate the efficacy and toxicity profile of regorafenib treatment in metastatic CRC patients. Materials and Methods This was a retrospective study of 25 mCRC patients from a single center. All patients had previously progressed fluorouracil, irinotecan, and oxaliplatin with or without biologic agents such as epidermal growth factor receptor (anti-EGFR) or vascular endothelial growth factor receptor (anti-VEGF). Results The median age was 58 years (range, 27 to 84 years), and there were 14 males and 11 females. Patients had received a median of 3 lines of systemic therapy before regorafenib initiation. The most common grade 3 or 4 toxicities were fatigue 20%, daire 16% and mucositis 16%. Median PFS was 2.07 months (0.43-5.13) and median OS was 4.14 months (0.62-19.88). No factors were significantly associated with PFS and OS in the univariate analysis. Conclusion Regorafenib shows a small but significant survival benefit in patients with metastatic CRC who do not have any further treatment options after the failure over standard therapies. Its toxicity profile along with the absence of predictive factors suggest a careful evaluation before its use in clinical practice. [ABSTRACT FROM AUTHOR]
