Zusammenfassung Die Popularität von CrossFit-Training ist bei inzwischen über 11.000 Studios weltweit offensichtlich. Obwohl Vorteile für die Gesundheit durch CrossFit allgemein anerkannt sind, birgt das Training ein gewisses Verletzungsrisiko. Akute und chronische Verletzungen sind in der Literatur bereits beschrieben worden. Der Schulterkomplex zählt zu den am häufigsten durch Verletzungen betroffenen Regionen. Das Ziel dieser Arbeit ist es, einen Überblick über die Häufigkeit, Genese und Morphologie von Schulterverletzungen im CrossFit und verwandten Sportarten darzustellen. Summary The popularity of CrossFit training is continuously increasing. Currently, there are more than 11,000 CrossFit training centers registered worldwide. CrossFit training is beneficial for general health. Common training routine, however, bears the risk of musculoskeletal injury. Both acute and chronic injuries have previously been described in the literature. These injuries most commonly involve the shoulder region. Background: After conventional treatment of in-stent restenosis, the incidence of recurrent clinical restenosis may approach 40%. We report the first multicenter, blinded, and randomized trial of intracoronary radiation with the use of a 90Sr/90Y beta-source for the treatment of in-stent restenosis., Methods and Results: After successful catheter-based treatment of in-stent restenosis, 476 patients were randomly assigned to receive an intracoronary catheter containing either 90Sr/90Y (n=244) or placebo (n=232) sources. The prescribed dose 2 mm from the center of the source was 18.4 Gy for vessels between 2.70 and 3.35 mm in diameter and 23.0 Gy for vessels between 3.36 and 4.0 mm. The primary end point, ie, clinically driven target-vessel revascularization by 8 months, was observed in 56 (26.8%) of the patients assigned to placebo and 39 (17.0%) of the patients assigned to radiation (P=0.015). The incidence of the composite including death, myocardial infarction, and target-vessel revascularization was observed in 60 (28.7%) of the patients assigned to placebo and 44 (19.1%) of the patients assigned to radiation (P=0.024). Binary 8-month angiographic restenosis (> or =50% diameter stenosis) within the entire segment treated with radiation was reduced from 45.2% in the placebo-treated patients to 28.8% in the 90Sr/90Y-treated patients (P=0.001). Stent thromboses occurred in 1 patient assigned to placebo <24 hours after the procedure and in 1 patient assigned to 90Sr/90Y at day 244., Conclusions: The results of this study demonstrated that beta-radiation using 90Sr/90Y is both safe and effective for preventing recurrence in patients with in-stent restenosis.

In case of radically operable rectal and recto-sigmoidal carcinomas, the local (and probably also the general) tumor control can be markedly improved by adjuvant radiotherapy. The authors recommend a combination of a unique preoperative irradiation with 5 Gy for all patients and a postoperative irradiation with 40 to 45 Gy in 20 to 25 fractions for risk patients (infestation of the serosa and/or lymph nodes).

A patient suffering from pulmonary metastases of a seminoma of the testicle was treated by subtotal-body irradiation with single doses of 15 cGy and a total dose of 1155 cGy. A complete remission lasting more than one year was achieved. This therapy method is recommended for the treatment of the advanced seminoma (hematogenic metastases, extension beyond the diaphragm, or regional lymph node metastases with a size of more than 5 cm) because of its lower acute toxicity.

A patient with an inoperable endodermal sinus tumor of the mediastinum was treated by combined radiotherapy and chemotherapy. There is no sign of tumor progression 50 months after the end of therapy and 55 months after the establishment of diagnosis.

The remission rate in the radiotherapy of esophageal carcinoma might probably be increased by accelerated fractionation. A 3 and a half year during complete remission of a 15 cm long tumor is reported.

Accelerated fractionation (total dose 66 Gy, single dose 1.8 to 2 Gy, 1 to 2 fractions daily, total treatment period 4 to 5.5 weeks) brings about more than 40% of complete remissions and more than 90% of objective remissions (assessed by radiology) of non-small cell bronchogenic carcinomas. Tumor-induced symptoms are improved in more than 90%. A treatment period of less than four weeks causes considerable acute (and persistent) oesophageal reactions. In case of a treatment period of more than five weeks there is no substantial increase of toxicity.

25 target volumes in 14 patients were treated with accelerated fractionation. Eleven of twelve evaluable target volumes showed a complete remission. Three local recurrences occurred in 25 target volumes. A 35 cm long tumor came in a one and a half year during complete remission.

41 patients were submitted to telecobalt irradiations of the prostate and the adjoining lymph drain regions. The absence of remote metastases was verified by X-ray examinations of the skeleton and bone scintigrams, and stades were divided by means of lymphography. The overall 3,5-year survival rate was 79.9%, the 3,5-year survival rate of the patients without lymph node metastases was 100% and that of the group with lymph node metastases was 25%. 69,6% of all patients had no recurrences after 3.5 years, the corresponding rate for the patients without lymph node metastases was 84% and that for the patients with lymph node metastases was 24.6%. The difference between patients with and patients without lymph node metastases demonstrated by lymphography was statistically significant and decisive for the prognosis. The clinically judged local tumor control was 100% for those patients who were observed during a period of more than one year after the radiotherapy. Lymph nodes which were also irradiated underwent an involution in all cases. Different differentiation degrees responded in the same way to radiotherapy. A dependence of rectal complications on the dose could be demonstrated. The results were not ameliorated by additional hormonal therapy.
