Purpose There has been increasing awareness of perinatal health and organisation of maternal and child health care in the Netherlands as a result of poor perinatal outcomes. Vulnerable women have a higher risk of these poor perinatal outcomes and also have a higher chance of receiving less adequate care. Therefore, within a consortium, embracing 100 organisations among professionals, educators, researchers, and policymakers, a joint aim was defined to support maternal and child health care professionals and social care professionals in providing adequate, integrated care for vulnerable pregnant women. Description Within the consortium, vulnerability is defined as the presence of psychopathology, psychosocial problems, and/or substance use, combined with a lack of individual and/or social resources. Three studies focussing on population characteristics, organisation of care and knowledge, skills, and attitudes of professionals regarding vulnerable pregnant women, were carried out. Outcomes were discussed in three field consultations. Assessment The outcomes of the studies, followed by the field consultations, resulted in a blueprint that was subsequently adapted to local operational care pathways in seven obstetric collaborations (organisational structures that consist of obstetricians of a single hospital and collaborating midwifery practices) and their collaborative partners. We conducted 12 interviews to evaluate the adaptation of the blueprint to local operational care pathways and its' embedding into the obstetric collaborations. Conclusion Practice-based research resulted in a blueprint tailored to the needs of maternal and child health care professionals and social care professionals and providing structure and uniformity to integrated care provision for vulnerable pregnant women., Author(s): H. Introduction: Suboptimal circumstances during the early life course, ranging from 100 days before conception to 1000 days following birth, significantly impact a child's future health and well-being. To optimize these circumstances, collaboration is needed which includes professionals working in medical, social and public domains, as well as parents. This action research protocol aims to improve care for (future) parents facing suboptimal circumstances during the early life course by enhancing inter-professional, cross-domain collaboration and (future) parents-professional collaboration. By employing iterative action research cycles, we seek to foster integrated care pathways and improve continuity of care across the medical, social and public domains. The research will be conducted in Rotterdam, the Netherlands. Methods and analysis: Four action research cycles incorporating descriptive qualitative and quantitative studies, including focus groups, questionnaires and observations with (future) parents facing suboptimal circumstances, professionals and policymakers. This intervention study will not only foster improved, integrated care around identification of the need of additional support, referral and care, but also foster the necessary conditions for a self-supporting neighbourhood care learning network of (future) parents, professionals and policymakers to encourage bidirectional feedback and enable reflection beyond a single organisation. These interventions will also be evaluated. Dissemination: The results will be disseminated through peer-reviewed publications, layman summaries, regional and national knowledge platforms and presentations and factsheets relevant to all involved actors. - Abstract
Objective: The term ‘vulnerable’ is often used to describe women facing psychosocial adversity during pregnancy, implying a heightened risk of experiencing suboptimal pregnancy outcomes. While this label might facilitate the pathway to appropriate care, it can be perceived as stigmatizing by the women it intends to help, which could deter their interaction with healthcare services. This study explores how women facing psychosocial adversity before, during and after pregnancy perceive the concept of vulnerability and experience being labeled as such. Methods: We conducted a thematic analysis of semi-structured, in-depth interviews. Through purposive sampling targeting maximum variation, ten women of diverse backgrounds were included. Results: Three central themes emerged: defining vulnerability, embracing vulnerability and the feeling of being stigmatized. Women perceived vulnerability as an inability to adequately care for themselves or their children, necessitating additional support alongside routine antenatal care. Acceptance of the ’vulnerable’ label came when it also acknowledged their proactive efforts and strengths to improve their situation. Conversely, if discussions surrounding vulnerability failed to recognize women's agency – specifically, their personal journeys and the courage needed to seek support – the label was perceived as stigmatizing. Conclusions: Addressing vulnerability effectively in maternity care requires a nuanced, patient-centered approach, acknowledging both the challenges and strengths of women facing psychosocial adversities. Emphasizing personal narratives and their courage in seeking support can mitigate the stigmatizing effects of the ’vulnerable’ label. Integrating these narratives into maternal healthcare practices can foster deeper connections with the women involved, enhancing the overall quality of care.

- Abstract
Coastal flood managers seek to anticipate future flood risk and as a result consider the adaptation of flood defences. Instead of crest heightening, dikes can be adapted to include hydrodynamic reducing vegetated foreshores to form a nature-based hybrid flood defence, for instance; at managed realignments. In this study we investigated the potential of vegetated revetments as a natural continuous connection between the realigned dike and restored foreshore. We applied the historic grass sod transplantation technique with the aim to improve our understanding of the strength of a transplanted sod revetment. In Living Lab Hedwige-Prosperpolder, dikes were available for in-situ experiments during managed realignment preparations. We transplanted grass sods and studied erosion resistance after one growth season. Our results show transplanted sod vegetation continued to grow and started to attach to the clay layer. While erosion occurred under extreme wave impact and overflow, the sod pulling method revealed individual sod strength. In conclusion, sod transplantation is a good technique to source local material for green realigned dike revetments. A vegetated dike revetment can hereby create a natural continuous connection between the realigned dike and foreshore, which benefits flood protection as well as flora and fauna., Hydraulic Engineering

19. Simultaneous Chandra and HST observations of the quiescent neutron star low-mass X-ray binaries in 47 Tucanae.

We present simultaneous Chandra X-ray Observatory and Hubble Space Telescope observations of three certain (X5, X7, W37) and two likely (X4, W17) quiescent neutron star low-mass X-ray binaries (qLMXBs) in the globular cluster 47 Tuc. We study these systems in the X-ray, optical, and near-ultraviolet (NUV) using the simultaneous data and additional non-contemporaneous HST data. We have discovered a blue and variable NUV counterpart to W17. We have not securely identified the eclipsing qLMXB W37 in the optical or NUV. Deeper high-resolution imaging is needed to further investigate the faint NUV excess near the centre of the W37 error circle. We suggest that a previously identified optical astrometric match to X7 is likely the true counterpart. The H α emission and the location of the counterpart in the colour–magnitude diagram, indicate that the secondary is probably a non-degenerate, H-rich star. This is consistent with previous results from fitting X7's X-ray spectrum. In X4, the simultaneous X-ray and optical behaviour supports the earlier suggestion that the X-ray variability is driven by changes in accretion rate. The X-ray eclipses in X5 coincide with minima in the optical/NUV light curves. Comparison of the 47 Tuc qLMXBs with the cataclysmic variables (CVs) in the cluster confirms that overall the qLMXBs have larger X-ray to optical flux ratios. Based on their optical/NUV colours, we conclude that the accretion discs in the qLMXBs are less prominent than in CVs. This makes the ratio of X-ray flux to excess blue-optical flux a powerful discriminator between CVs and qLMXBs. [ABSTRACT FROM AUTHOR]

42. Factors Contributing to the Negation of Therapeutic Services by Emerging Adults at a South African University

- Abstract
From a practice point of view it seems as if there are certain factors that might contribute to the fact that emerging adults tend to negate therapeutic help and services. It also seems to be specifically true with regard to emerging adults at university. Help negation seems to occur albeit the fact that therapeutic intervention is seen as an effective tool in managing distress. The aim of the study therefore was to explore which factors contribute to help negation behaviour in emerging adults at a specific university in South Africa. A qualitative case study design was employed where participants who complied with the inclusion criteria set out for the study, were selected by means of non-probability target and snowball sampling. Fifteen students residing in campus residences respectively participated in one of three focus group discussions. Creswell's spiral of data analysis was used to analyse the transcribed data. The data crystallised into four themes, which contribute to help negation behaviour in emerging adults. Emerging adults have a fear of being judged, stigmatised, recognised, of not being treated confidentially and a fear of being vulnerable and hurt. They prefer to seek help and support from their family and peers and to put their trust in God and their religion; they have internalised beliefs about themselves and therapy and a need for independence; student interns work at the therapeutic centres on campus which specifically contributes to help negation for emerging adults at the university. It is crucial that the professionals revisit their strategies and approaches in order to overcome negation of formal help and create a more understandable, approachable and effective therapeutic service to emerging adults, especially at university therapeutic centres.

- Abstract
The "common sense" intervention by toxicology journal editors regarding proposed European Union endocrine disrupter regulations ignores scientific evidence and well-established principles of chemical risk assessment. In this commentary, endocrine disrupter experts express their concerns about a recently published, and is in our considered opinion inaccurate and factually incorrect, editorial that has appeared in several journals in toxicology. Some of the shortcomings of the editorial are discussed in detail. We call for a better founded scientific debate which may help to overcome a polarisation of views detrimental to reaching a consensus about scientific foundations for endocrine disrupter regulation in the EU. © 2013 Bergman et al.; licensee BioMed Central Ltd.
