Abstract

The addition of hyperthermic intraperitoneal chemotherapy (HIPEC) to interval cytoreductive surgery (CRS) improves recurrence-free (RFS) and overall survival (OS) in patients with FIGO stage III ovarian cancer. We evaluated the effect of HIPEC on patient's health-related quality of life (HRQoL) in the OVHIPEC trial. OVHIPEC was a multicentre, open-label, randomized phase III trial for patients with stage III ovarian cancer. Patients were randomly assigned (1:1) to receive interval CRS with or without HIPEC with cisplatin. HRQoL was assessed using the EORTC QLQ-C30, and the ovarian (QLQ-OV28) and colorectal cancer (QLQ-CR38) modules. HRQoL questionnaires were administered at baseline, after surgery, after end of treatment, and every three months thereafter. HRQoL was a secondary endpoint, with the prespecified focus on the QLQ-C30 summary score and symptom scores on fatigue, neuropathy and gastro-intestinal symptoms. HRQoL was analysed using linear and non-linear mixed effect models. In total, 245 patients were randomized. One-hundred-ninety-seven patients (80%) completed at least one questionnaire. No significant difference over time in the QLQ-C30 summary scores was observed between the study arms (p-values for linear and non-linear growth: p > 0.133). The pattern over time for fatigue, neuropathy and gastro-intestinal symptoms did not significantly differ between treatment arms. The addition of HIPEC to interval CRS does not negatively impact HRQoL in patients with stage III ovarian cancer who are treated with interval CRS due to the extent of disease. 44. HIPEC for ovarian cancer

Abstract

Eierstokkanker staat in de top 10 van kankerdiagnoses bij vrouwen wereldwijd. Ongeveer 70-80% van de patiënten met eierstokkanker, heeft deze ziekte in stadium III of IV, waarbij de ziekte is uitgezaaid naar het buikvlies of buiten de buikholte. De behandeling van eierstokkanker bestaat uit chirurgie, chemotherapie en doelgerichte onderhoudstherapie. Hypertherme intraperitoneale chemotherapie (HIPEC) is een techniek waarbij verwarmde chemotherapie in de buikholte wordt toegediend aan het einde van de operatie, gericht op het behandelen van microscopische ziekte op het buikvlies. In dit proefschrift hebben we de resultaten van de OVHIPEC-1 studie beschreven. OVHIPEC-1 was de eerste fase III-studie die het effect van HIPEC bij primaire eierstokkanker evalueerde. 245 patiënten werden gerandomiseerd om interval cytoreductieve chirurgie met of zonder HIPEC te ondergaan. In dit proefschrift laten we zien dat de toevoeging van HIPEC aan interval chirurgie de ziektevrije- en algehele overleving bij patiënten met stadium III eierstokkanker verbetert. HIPEC voegt weinig tot geen bijwerkingen toe, en gerapporteerde kwaliteit van leven bleef in verloop van tijd vergelijkbaar in vergelijking met patiënten die alleen met chirurgie en chemotherapie werden behandeld. In een kosteneffectiviteitsanalyse tonen we aan dat behandeling met HIPEC duurder (~€15.500 per patiënt) maar kosteneffectief is, gezien de gewonnen kwalitatieve levensjaren. Als gevolg van dit onderzoek wordt HIPEC volledig vergoed door de Nederlandse zorgverzekering, en is het opgenomen in nationale en internationale richtlijnen. Toekomstig onderzoek is gericht op het analyseren van het directe werkingsmechanisme, en het verbeteren van de patiëntenselectie om uiteindelijk de uitkomsten voor deze patiënten verder te verbeteren.
