Abstract

We demonstrate that a simple silver coated ball lens can be used to accurately measure the entire distribution of radiative transition rates of quantum dot nanocrystals. This simple and cost-effective implementation of Drexhage's method that uses nanometer-controlled optical mode density variations near a mirror, not only allows to extract calibrated ensemble-averaged rates, but for the first time also to quantify the full inhomogeneous dispersion of radiative and non radiative decay rates across thousands of nanocrystals. We apply the technique to novel ultra-stable CdSe/CdS dot-in-rod emitters. The emitters are of large current interest due to their improved stability and reduced blinking. We retrieve a room-temperature ensemble average quantum efficiency of 0.87+-0.08 at a mean lifetime around 20 ns. We confirm a log-normal distribution of decay rates as often assumed in literature and we show that the rate distribution-width, that amounts to about 30% of the mean decay rate, is strongly dependent on the local density of optical states.

Abstract

Objective: The aim of this Meta‐analysis is to evaluate the impact of different treatment strategies for early postoperative hypoparathyroidism on hypocalcemia‐related complications and long‐term hypoparathyroidism. Data Sources: Embase.com, MEDLINE, Web of Science Core Collection, Cochrane Central Register of Controlled Trials, and the top 100 references of Google Scholar were searched to September 20, 2022. Review Methods: Articles reporting on adult patients who underwent total thyroidectomy which specified a treatment strategy for postthyroidectomy hypoparathyroidism were included. Random effect models were applied to obtain pooled proportions and 95% confidence intervals. Primary outcome was the occurrence of major hypocalcemia‐related complications. Secondary outcome was long‐term hypoparathyroidism. Results: Sixty‐six studies comprising 67 treatment protocols and 51,096 patients were included in this Meta‐analysis. In 8 protocols (3806 patients), routine calcium and/or active vitamin D medication was given to all patients directly after thyroidectomy. In 49 protocols (44,012 patients), calcium and/or active vitamin D medication was only given to patients with biochemically proven postthyroidectomy hypoparathyroidism. In 10 protocols (3278 patients), calcium and/or active vitamin D supplementation was only initiated in case of clinical symptoms of hypocalcemia. No patient had a major complication due to postoperative hypocalcemia. The pooled proportion of long‐term hypoparathyroidism was 2.4% (95% confidence interval, 1.9‐3.0). There was no significant difference in the incidence of long‐term hypoparathyroidism between the 3 supplementation groups. Conclusions: All treatment strategies for postoperative hypocalcemia prevent major complications of hypocalcemia. The early postoperative treatment protocol for postthyroidectomy hypoparathyroidism does not seem to influence recovery of parathyroid function in the long term. [ABSTRACT FROM AUTHOR]

Abstract

Radiofrequente ablatie (RFA) is een techniek waarbij men met behulp van een naald selectief weefsel kan verhitten en doen afsterven. Deze minimaal invasieve techniek wordt ingezet bij euthyreote patiënten met een solide – dat wil zeggen: > 25% van het volume is solide –, symptomatische, benigne schildkliernodus, zodat chirurgische resectie achterwege gelaten kan worden. Inmiddels is er ruimschoots wetenschappelijk bewijs dat RFA kan zorgen voor volumereductie van de schildkliernodus en daardoor bijdraagt aan significante vermindering van de klachten. RFA is naast chirurgie opgenomen in internationale richtlijnen als alternatieve behandeloptie voor patiënten met een symptomatische benigne schildkliernodus.

Abstract

Aim Although cardiopulmonary exercise testing (CPET) is considered the gold standard, a preoperative abdominal CT scan might also provide information concerning preoperative aerobic fitness for risk assessment. This study aimed to investigate the association between preoperative CT-scan-derived body composition variables and preoperative CPET variables of aerobic fitness in colorectal surgery. Method In this retrospective cohort study, CT images at level L3 were analysed for skeletal muscle mass, skeletal muscle radiation attenuation, visceral adipose tissue (VAT) mass and subcutaneous adipose tissue mass. Regression analyses were performed to investigate the relation between CT-scan-derived body composition variables, CPET-derived aerobic fitness and other preoperative patient-related variables. Logistic regression analysis was performed to predict a preoperative anaerobic threshold (AT) ≤ 11.1 ml/kg/min as cut-off for having a high risk for postoperative complications. Results Data from 78 patients (45 men; mean [SD] age 74.5 [6.4 years]) were analysed. A correlation coefficient of 0.55 was observed between absolute AT and skeletal muscle mass index. Absolute AT (R2 of 51.1%) was lower in patients with a lower skeletal muscle mass index, together with higher age, lower body mass and higher American Society of Anesthesiologists (ASA) score. Higher ASA score (odds ratio 5.64; P = 0.033) and higher VAT mass (odds ratio 1.02; P = 0.036) were associated with an increased risk of an AT ≤ 11.1 ml/kg/min. Conclusion Body composition variables from the preoperative CT scan were moderately associated with preoperative CPET-derived aerobic fitness. Higher ASA score and higher VAT mass were associated with an increased risk of an AT ≤ 11.1 ml/kg/min.
