Abstract

In de gerandomiseerde fase III- CASSIOPEIA-studie werd het effect van het toevoegen van daratumumab aan de standaardbehandeling met bortezomib, thalidomide en dexamethason onderzocht bij 1.074 patiënten met nieuw gediagnosticeerd multipel myeloom (MM). De patiënten ontvingen zes cycli (vier inductiecycli, twee consolidatiecycli) van 28 dagen, bestaande uit subcutaan (SC) bortezomib (1,3 mg/m2 dagen 1, 4, 8, 11), oraal thalidomide (100 mg per dag), dexamethason (20–40 mg) (VTD) en, afhankelijk van de onderzoeksgroep, daratumumab intraveneus (IV). Deze studie is geregistreerd onder EudraCT-nummer NCT02541383. Met resultaten in het voordeel van dara-VTD werd dit regime vervolgens goedgekeurd door EMA voor gebruik bij nieuw gediagnosticeerd MM en zal in de nabije toekomst in Nederland als standaardbehandeling worden geïmplementeerd. Polyneuropathie (PNP) is een veel voorkomende bijwerking tijdens de behandeling van MM. De incidentie van PNP-graad 2–4 (CTCAE-gradering) werd in de CASSIOPEIA-studie geanalyseerd. In totaal ontwikkelden 394/1.074 (37%) patiënten graad 2-PNP en 105/1.074 (10%) graad 3. Significante risicofactoren geassocieerd met PNP-graad 2 waren graad 1-neuropathie in de voorgeschiedenis en overgewicht (BMI >25). In deze studie is de hoge incidentie van PNP-graad 2–4 gedurende MM-behandeling bevestigd. Hiermee wordt de noodzaak van screening op risicofactoren, frequente beoordeling van PNP en strikte naleving van de richtlijnen benadrukt om de incidentie van ernstige PNP te verminderen.

Abstract

Diels-Alder chemistry has been used for the thermoreversible cross-linking of furan-functionalized ethylene/propylene (EPM) and ethylene/vinyl acetate (EVM) rubbers. Both furan-functionalized elastomers were successfully cross-linked with bismaleimide to yield products with a similar cross-link density. NMR relaxometry and SAXS measurements both show that the apolar EPM-g-furan precursor contains phase-separated polar clusters and that cross-linking with polar bismaleimide occurs in these clusters. The heterogeneously cross-linked network of EPM-g-furan contrasts with the homogeneous network in the polar EVM-g-furan. The heterogeneous character of the cross-links in EPM-g-furan results in a relatively high Young's modulus, whereas the more uniform cross-linking in EVM-g-furan results in a higher tensile strength and elongation at break.

Abstract

Genome-wide association studies (GWAS) have transformed our understanding of susceptibility to multiple myeloma (MM), but much of the heritability remains unexplained. We report a new GWAS, a meta-analysis with previous GWAS and a replication series, totalling 9974 MM cases and 247,556 controls of European ancestry. Collectively, these data provide evidence for six new MM risk loci, bringing the total number to 23. Integration of information from gene expression, epigenetic profiling and in situ Hi-C data for the 23 risk loci implicate disruption of developmental transcriptional regulators as a basis of MM susceptibility, compatible with altered B-cell differentiation as a key mechanism. Dysregulation of autophagy/apoptosis and cell cycle signalling feature as recurrently perturbed pathways. Our findings provide further insight into the biological basis of MM.

Abstract

The kinetics of the Diels-Alder (DA) cross-linking and retro-Diels-Alder (rDA) de-cross-linking reactions have been studied for furan-containing EPM rubber thermoversibly cross-linked with a bismaleimide. Cross-linking conversions obtained from spectroscopic methods deviate from rheological methods as the former take into consideration both the intermediate DA adduct and complete cross-links, while the latter only determine the formation and decomposition of complete cross-links. The thermoreversibility of the system was shown by a combination of equilibrium swelling and rheological measurements. Dynamic time sweeps of the rubber at different temperatures were used to determine the Arrhenius activation energies of 7.04 and 57.9 kJ/mol for the DA and the rDA reactions, respectively. Ea,DAis relatively low and suggests a diffusion limitation while Ea,rDAcorrespond to values reported in literature. Both values were used to simulate cross-link density profiles as a function of time and temperature that correspond well with the experimental data.
