Objective: To determine the psychological impact of sexual assaults among males during their childhood. Study Design: Descriptive cross-sectional study. Place and Duration of Study: Combined Military Hospital Okara from 2 August 2022 to 20 March 2023. Materials and Methods: We consecutively sampled 204 subjects who were given proforma to record demographic data including age, socioeconomic status, ethnicity, being victim of sexual assault, relationship to the assailant, numbers of sexual assault, and mode of sexual assault. Post-traumatic stress disorder (PTSD) check list and Trauma symptom check (TSC) list was applied on participants reporting sexual assault and SPSS 23 used for data analysis. Results: Out of 204 study participants 33 (16.1%) males experienced sexual assault while 27 (81.8%) victims experienced PTSD with trauma symptoms of sleep disturbance in 18 (54.5%), sexual dysfunction experienced by 6 (18.1%), anxiety felt by 13 (39.3%), depression presented in 5 (15.1%), while 2 (6.0%) individuals experienced dissociation. Victims of sexual assault, assailant and number of sexual assaults were highly correlated with mode of sexual assault, trauma symptoms (except dissociation), and PTSD (p < 0.001) Conclusion: Sexual assault on males during their childhood is strongly associated with significant psychological distress.

O artigo se propõe a realizar o entrecruzamento de memórias no testemunho de mulheres presas e torturadas no período da Ditadura Militar no Brasil. A partir do livro seminal 68 a geração que queria mudar o mundo: relatos, publicado por Ferrer em 2011, e dos testemunhos presentes no Relatório da Comissão Nacional da Verdade (CNV), de 2014, pretende-se discutir, sob a perspectiva de violência de gênero, como os relatos das mulheres funcionam como elaboração do trauma social e individual. Para esse intento, foram analisados relatos e testemunhos, tendo como aporte teórico os estudos sobre trauma, literatura de testemunho e os estudos de gênero.

Introduction: Effective trauma care requires the rapid management of injuries. Rural and remote areas face inequity in trauma care due to time, distance and resource constraints, and experience higher morbidity and mortality rates than urban settings. A training needs analysis (TNA) conducted with stakeholders across Queensland, Australia, revealed a lack of contextual, accessible and interprofessional trauma education for clinicians. The Clinical Skills Development Service and Jamieson Trauma Institute developed the Queensland Trauma Education (QTE) program to address these concerns. QTE comprises a face-to-face training course and open access to online training resources created and reviewed by trauma experts. QTE also supports local training through a statewide simulation network and free access to simulation training equipment. The aim of this article is to review the QTE program and assess the benefits to clinicians in both the delivery of education and the provision of trauma care. Methods: To evaluate the QTE program, a desktop review was conducted. This included analyses of website data, course and website content, and facilitator, stakeholder, participant and user feedback. The data were evaluated using the RE-AIM (Reach, Effectiveness, Adoption, Implementation, and Maintenance) framework, and the program's alignment with the original TNA outcomes was assessed. Results: The results showed that QTE aligns with the identified training needs. Specifically, QTE provides trauma education that is relevant, sustainable, employs best practice, is locally delivered, provides continuous support, is multidisciplinary, multi-platformed, physically accessible and accredited by the Australasian College for Emergency Medicine. The review also highlights how QTE has effectively been reaching its target population, improves knowledge and skills, has become widely adopted, and been implemented and maintained with relative success. Conclusion: The innovative QTE program addresses the previous deficits in trauma education and meets the needs identified in the TNA. The review also reveals further opportunities for continuous improvement and program sustainability.

Background: Traumatic Spinal Cord Injury (TSCI) is one of the catastrophic events, the rate of which has been growing compared to the past decades. Complications caused by TSCI have a wide spectrum and can range from complete paralysis to numbness of the limbs. Additional to the injury severity and disability of the patient, the recovery rate depends on the treatment strategies. Despite extensive efforts and research in this field, there are still few treatment options for TSCI patients. Controversial results have been reported, however, spinal cord decompression is the only certainty for the treatment of these patients. In the present study, patients with thoracic and lumbar fractures were undergone decompression, less and more than 24 hours, and the recovery rate (RR) was compared after 6 months. Methods: In this study, patients with lumbar and thoracic fractures who were referred to the neurosurgery department of Ahvaz Golestan Hospital during May 2019 to December 2021 were included. Decompression was performed at the fracture level as a total and at the upper and lower levels of the fracture as a partial decompression. To evaluate fine motor movements, picking up a small object with toes and following a rectangular path were used. Also, the gross motor movements, upper and lower proximal and distal muscle forces were measured. Results: 160 patients including 133 men (83.1%) and 16.9% women (27) with 36±12 years mean age were included. The most fracture location was lumbar (53.1%), followed by the thoracic (43.1%) and fractures in both regions (3.8%). The most injured vertebras were L1 (27.5%) and T12 (18.8%). Background: In most patients, the accessory spleen (AS) is small in size. However, in patients who have undergone splenectomy, AS may hypertrophy. This manuscript presents a rare case of spontaneous AS rupture nearly two decades after a prior splenectomy due to trauma. After searching multiple databases, only a few similar cases have been reported to date. In any acute abdominal patient with a history of previous splenectomy who presents with a mass in the anatomical location of the spleen, along with evidence of hematoma and free abdominal fluid on imaging, the possibility of AS rupture should be considered. Case Presentation: In February 2022, a 36-year-old man who had undergone splenectomy due to trauma 16 years ago was referred to the emergency department at Imam Ali Hospital in Bojnord. He complained of sudden onset of severe abdominal pain, preferably in the upper region of the abdomen. The patient denied having any recent issues. Based on the patient's history and examination, at perforated stomach ulcer and pancreatitis were initially suspected. However, except for leukocytosis, no other abnormalities were observed in the laboratory tests. Ultrasound revealed a low-echo mass-like lesion in the anatomical location of the spleen. Another low-echo mass-like lesion, indicating a hematoma with abundant free fluid in the abdomen and pelvis, was also observed near the aforementioned mass. CT scan confirmed these findings. Open surgical and re splenectomy procedures were performed, and the diagnosis of spontaneous AS rupture was made based on clinical examination and imaging findings. Conclusion: In any patient with a history of previous splenectomy who presents to the emergency room with diffuse and acute abdominal pain, even without recent trauma, if a mass-like lesion is observed in the anatomically suspicious location of the spleen in the left subphrenic space, along with other evidence of rupture such as hematoma/laceration and free fluid in the abdominal cavity, accessory spleen rupture should be considered as the main differential diagnosis.

Introduction: Driving under the influence is one of the most significant risk factors for road traffic accidents, leading to severe and multiple orthopaedic injuries. The aim of this study was to find out the prevalence of driving under the influence of alcohol among road traffic accident patients presenting to a tertiary care centre. Methods: A descriptive cross-sectional study was conducted on patients involved in road traffic accidents presenting to a tertiary care centre. Data from 10 January 2020 to 9 December 2021 were collected between 22 July 2023 to 22 August 2023 from the hospital records after receiving ethical approval from the Institutional Review Committee. Patients who had road traffic accidents and were diagnosed with fractures were included in the study. Patients with incomplete hospital records were excluded from the study. Convenience sampling method was used. The point estimate was calculated at a 95% Confidence Interval. Results: Among 2,519 patients, 307 (12.19%) (10.91-13.47, 95% Confidence Interval) were driving under the influence of alcohol when involved in a road traffic accident. Out of 307 patients, 305 (99.35%) accidents involved two-wheelers. A total of 118 (38.44%) patients suffered soft tissue injuries, while 47 (15.31%) sustained head injuries, and 28 (9.12%) sustained facial injuries. Conclusions: The prevalence of driving under the influence of alcohol among road traffic accident patients presenting to a tertiary care centre was similar to other studies done in similar settings.

Parents who apply violence towards minors consider that physical punishments are the main means of disciplining the minor to comply with the rules of behavior imposed by the family. In the practice of many states, these punishments necessarily accompany the primary socialization process, being applied even in the first year after the birth of the minor, continuing during the preschool and school period, until the teenage years. These punishments often go beyond the permitted limits, resulting in serious injuries, up to fractures and trauma. Evidence shows that most of the parents who resort to these violent means grew up and were, themselves, educated in a family environment characterized by violence, being subjected to abuse themselves. In turn, an important part of the minors assaulted by these parents will become aggressors themselves, if violence as a means of discipline was supported by the existing beliefs and norms in the family.

Background: Several trauma-focused treatments have been developed to treat post-traumatic stress disorder (PTSD). Yet there are limited studies on how trauma survivors perceive and experience trauma-focused treatments such as prolonged exposure therapy (PE) for PTSD, especially in low- and middle-income countries (LMIC). Aim: The study aimed to explore the perceptions and experiences of trauma survivors receiving prolonged exposure therapy for PTSD and the general acceptability of PE for PTSD in a LMIC. Setting: The study was conducted at a community psychology clinic in the Eastern Cape, South Africa. Method: Using a qualitative method, seven adult trauma survivors who completed six sessions of brief PE for PTSD were interviewed. Thematic analysis was used to identify relevant themes and to understand how participants perceived and experienced PE for PTSD. Results: The analysis yielded five themes, namely structure, obstacles, gender, exposure and experiences of recovery. Conclusion: The findings suggested that participants perceived and experienced PE to be generally beneficial for the treatment of PTSD. Moreover, the study suggested that PE is an acceptable trauma therapy in a contextually diverse setting such as the Eastern Cape, South Africa. Overall, considering the evidence base of PE for PTSD, this study contributed to the literature on the acceptability of PE in a South African setting. Contribution: The findings of the study are in keeping with the extant literature on how persons perceive and experience PE for PTSD. The findings of the study suggests that PE is an acceptable and beneficial trauma therapy for PTSD in a contextually diverse setting such as South Africa. It is recommended that large scale implementation studies be conducted to further investigate the effectiveness, feasibility, and acceptability of PE in South Africa.

Truth and lies share an element of (in)authenticity that is the affect of associated cognition. Such elements can interfere with relationships and assist – or jeopardize – the individual development path. A lie often veils a reality made of horribly empty spaces which are only inhabited by conflicts caused by the imperfect functioning of that very reality. Identity development may be affected by compliance with ancient masks that are not accessible to subjects – as these masks are not the subjects’ own, but are based on the experiences of the people who preceded them in their family history. These mechanisms often have a traumatic motivation, which apparently remains unprocessed because of a lack of mobilization of the necessary psychic resources. In the transgenerational transmission of family histories, the characteristics of unthinkability and untoldness of the historical-narrative foundations that have come to constitute a lie can generate reflections which are impermeable to historical objectivity. This could be the basis of existential possibilities and psychopathological vulnerabilities.

Traumatic injuries are a major health hazard having an important impact in animals’ welfare. The outcome following a traumatic event in the animal’s life, depends on multiple factors, the most common ones being the cause of trauma, the location of the trauma and the amount of kinetic energy and the way it was distributed to the animal, but also human activities and lifestyles, human-pet relationships and pet-management local strategies. Os traumas mandibulares são comuns em pequenos animas e a região mais acometida em gatos é a sínfise mentoniana. Com o impacto traumático o animal apresenta um quadro de lesões com descontinuidade de tecidos anatômicos e celular, levando a fraturas expostas e não expostas. Como opções terapêuticas há métodos cirúrgicos como pino intramedular, fixação esquelética externa, cerclagem e o uso de acrílicos e placas ósseas. Para a região lesionada o indicado é o uso de curativos baratos e flexíveis proporcionando conforto e bem-estar ao paciente, para que no final alcance a consolidação da sínfise e restabeleça funcionalidade oral ao animal. Logo, surge como alternativa de material biológico, o uso da pele de tilápia do Nilo, fornecendo todas as características que o animal necessita para reepitelização, conforto, flexibilidade, excelente aderência e redução de perdas hidroeletrolíticas. Portanto esse trabalho visa relatar o uso da pele de tilápia do Nilo associado à pomada Dersani® como curativo oclusivo temporário em disjunção de sínfise mandibular após necrose epitelial, em um felino, SRD de aproximadamente 4 meses de idade. A pele foi previamente preparada no Centro Universitário do Cerrado (UNICERP), em laboratório estéril. Para isso utilizou-se solução de clorexidina dergemante a 2% e solução fisiológica 0,9%. Repetiu-se a técnica por 3 vezes, sempre removendo todo material da couraça que não seria utilizado, preservando as escamas e o couro do produto. Após a higienização, o material foi embalado, identificado e armazenado na geladeira para refrigeração, utilizando como meio de armazenamento 80% de solução fisiológica e 20% de glicerina. Em seguida, foi mensurado o tamanho das bordas com o uso de um paquímetro e posteriormente foi higienizado o local da lesão e escarificado para colocação do material biológico. A pele de tilápia foi trocada a cada 3-4 dias, sempre verificando a evolução da ferida e a presença de infecções e inflamações, durante aproximadamente 17 dias. Em suma, observamos a evolução da ferida que permanecia sempre limpa e livre de infecções, deixando o animal calmo e com bem-estar. A técnica demonstrou ser efetiva, trazendo agilidade no pós-operatório e monetariamente mais viável ao proprietário.

With the complexity of the vital structures, it necessitates a surgical team to respect each anatomical boundary. In the past, there was an exceptionally high number of cases with surgical errors. These errors were not because of flaws in the surgeon’s skills or techniques but owing to lack of resources. Visualisation is one of the key factors that determines the precision of any surgical outcome. Advances in surgical planning have led to the introduction of a “Navigation” system that helps surgeons to see more, know more and ultimately do more for their patients. The usefulness of the navigation system in oral surgeries has been indicated by its surgical applications in craniomaxillofacial trauma, orthognathic surgeries, head and neck pathological resections, complex skull base surgeries and surgery involving temporomandibular joint. A vast majority of research literature has suggested remarkable improvement in surgical outcomes under the guidance of 3d planning and navigation. However, with such an inordinate advancement, financial expenses and a gradual learning curve are always a constraining factor in surgical navigation. In Smile (2017), Roddy Doyle represents a society that is still heavily influenced by the moral authority of the Catholic Church and that, therefore, avoids any open discussion about sexuality in any of its manifestations. In the midst of this climate, Victor Forde, the working-class protagonist of the novel, tries to disclose the sexual abuse he suffered as a child in a Christian Brotherhood School. In my article, I argue that the silences and taboos that permeate the society as represented in the novel, and the protagonist’s awareness that his social position made him the perfect target for abuse, condition Victor’s only opportunity for disclosure; this – and the absolute failure of his attempts at divulgence – ultimately frustrates his chances of healing from trauma and of leading an ordinary life.

Introduction: Traumatic spinal injury is a major source of morbidity and mortality throughout the world. The number of spinal injuries is growing annually but epidemiological and demographic features may be different in different regions. This study aims to find out the prevalence of traumatic spinal injury among patients with spinal injuries admitted to the spine unit of a tertiary care centre. Methods: This was a descriptive cross-sectional study was done on a total of 102 traumatic spinal injury patients admitted to the spine unit of a tertiary care centre from 1st June, 2019 to 31st May, 2021 after receiving ethical approval from the Institutional Review Committee (Reference number: 077/78-09). Demographic details, mode of injury, morphology, patterns of fractures, neurological level, and management methods in the hospital were recorded. Convenience sampling was done. Data were analysed using the Statistical Package for the Social Science version 24.0. Point estimate at 95% Confidence Interval was calculated along with frequency and percentages for binary data. Results: Among 130 spinal injury patients, the prevalence of traumatic spinal injury was found to be 102 (78.46%) (71.39-85.53 at 95% Confidence Interval). The most common mode of spinal injury was due to falls in 80 (78.43%) cases. Conclusions: The prevalence of traumatic spinal injury was higher when compared to the other studies done in similar settings.

Penetrating neck injuries causing rupture of sternocleidomastoid muscle along with transection of major vessels of the neck have significant morbidity and mortality due to the risk of severe hemorrhage and cerebral infarction. However, there are no universal guidelines for the management of penetrating neck injuries. Here, we report a case of a 67-year-old female with a lacerated wound on the left side of the neck with a complete transection of the left sternocleidomastoid muscle along with transection of internal jugular vein and two superficial branches of internal carotid artery following penetrating injury to the neck by a bamboo stick. It was managed by emergency wound exploration with ligation of the injured vessels with repair of sternocleidomastoid muscle. Post-operatively the hemorrhage was controlled and the patient was discharged on the fourth postoperative day. Thus, in a case of penetrating injury to the neck, prompt surgical wound exploration is beneficial.

O presente artigo discute a coalescência das intempéries naturais e sanitárias (tais como secas, tempestades e pandemias) com a má condução política das crises para a produção de catástrofes coletivas na tradição histórica brasileira. Aborda-se como os traumas sociais geralmente são denegados pela narrativa oficial, retornando, por sua vez, pela via da tradição oral e da literatura, que transmitem o testemunho de atores que, à margem do apoio do Estado, constroem caminhos para o enfrentamento das urgências, articulando soluções e mobilizando laços sociais. Rodolfo Teófilo, escritor e farmacêutico cearense que viveu no final do século XIX e início do século XX, período marcado pela fome e longas secas, é um exemplo desses atores responsáveis por tais atos de resistência popular. Seus livros e sua vida encarnam um posicionamento político diante dos acontecimentos de seu tempo. Além de se empenhar na defesa da dignidade dos flagelados da seca, que eram confinados em campos de concentração fora do perímetro urbano de Fortaleza, ele executou campanhas de vacinação da varíola que atingiram sobretudo a população mais desfavorecida. A sua obra, que ainda não recebeu a merecida atenção da crítica, mostra-se hoje mais instigante e pertinente do que nunca, haja vista que as mesmas atitudes de desrespeito e descaso com os mais vulneráveis contra as quais ele lutou pauta hoje a ação dos governantes responsáveis por criar soluções de enfrentamento da pandemia de Covid-19. A partir da psicanálise, com os conceitos de denegação e desautorização, problematiza-se os livros de Teófilo, tratando-os como a materialização de uma estética da melancolia e do testemunho de uma memória traumática. Toma-se o sertanejo como paradigma da alteridade e a fome e a seca, como uma forma de catástrofe subjetiva coletiva. Repercute-se daí o legado do escritor para o cenário político, cultural e sanitário atual.

Ao longo das últimas décadas, a Shoah vem sendo rememorada em obras de diversos autores ao redor de todo o mundo. Na contemporaneidade, destaca-se a abordagem do tema do ponto de vista das histórias individuais. Neste trabalho, analisamos como Michel Laub e Noemi Jaffe articulam trauma, escrita e silenciamento na construção de suas obras, a ficção Diário da queda e o diário comentado O que os cegos estão sonhando?, respectivamente. Investigamos como ambos escritores ressignificam a memória de um trauma histórico coletivo – o genocídio hitlerista – à luz da experiência individual.

The 2010 remake of the horror film A Nightmare on Elm Street focuses on the earthly abuses committed by the now confirmed pedophile Freddy Krueger. The fact that the victims of Krueger’s sexual assaults start get- ting murdered in their sleep is relevant to the trauma studies, as nightmares are one of the most common symptoms observed in people diagnosed with Post Traumatic Stress Disorder. Therefore, one of the goals of this paper is to analyze the ways in which trauma is portrayed in the movie. Considering that the teenagers can only overcome Krueger — and therefore, their trauma — once they are able to witness the terrible events of their past, this work also aims at observing and discussing the trajectory that allows the protagonists to survive. In order to meet these goals, some central concepts to trauma theory are presented and discussed in relation to their portrayal in the film. The analysis shows that the main character Nancy can only defeat Krueger once she knows everything that has happened in the past and is able to have agency in the story.

Background and Objectives: Love failure can cause intense emotional damage in a person. Therefore, it is necessary to identify and strengthen the factors that improve the experience of an individual with love failure. With this background in mind, the present study aimed to evaluate the effect of spiritual-religious psychotherapy on love trauma syndrome and acceptance of the problem in female students with love failure. Methods: This experimental pretest-posttest study was performed on 30 female undergraduate students with an experience of love failure in Bandar Abbas, Iran, in 2019, using a control group design. The study population was selected through snowball sampling method, and then assigned randomly into two groups of intervention and control. The participants of the intervention group received spiritual-religious psychotherapy, and the final analysis was performed on 27 subjects. Data collection tools entailed Love Trauma Inventory, in addition to Acceptance and Action Questionnaire-II. All data were analyzed by multivariate analysis of variance using SPSS software (version 22). Secondary hypertension accounts for 5% to 10% of all hypertensive cases, and renal artery stenosis is one of the most common causes of secondary hypertension. Although atherosclerotic vascular disease and fibromuscular dysplasia are the leading causes of renal artery stenosis, there are other, rare etiologies, such as vasculitis and trauma. A partial nephrectomy is the standard of care treatment option for early stage renal carcinoma patients. Traumatic renal artery stenosis can occur during this surgical intervention, though it is a very rare adverse event, and only a few case reports have been reported in the literature. This report is the description of successful percutaneous treatment of accelerated hypertension secondary to traumatic renal artery stenosis after a partial nephrectomy.

Introduction: The Accident is an event that causes damage to be detected. Increasing in traffic accidents threaten the community health promotion. Physical, financial and psychological disadvantages of accident are essential to invest a public health priority. This study evaluated the epidemiology of accidents and injuries of motorcycles and bicycles riders in Jahrom city. Method: In this cross-sectional study, 14111 accidents accidents motorcycles and bicycles Jahrom emergency census registration statistics were extracted from 89-93 years. The data gathered by a check lists of 115 emergency care. Data were analyzed with descriptive statistics and logistic regression by software version 14. Results: 62.7 % were facing urban accidents. 57.9 % and 61.4 % of injury of accidents on the day in children under 30 years old. 1.3% of fatal accident and others were injury. Most accidents occurred in the 12 to 2pm and 18 to 20pm. Most prevalent trauma, a blow to the organs (41.2%) and head trauma (28.9 %). Car-motorcycles (50.3%) and motor reversal (32.9%) were the most vehicle accidents involved. Occupants died at the crash area (OR= 2.77), season (OR= 1.15), age (OR= 0.624) and sex (OR= 2.065) were associated. Conclusion: Fatal accident motorcycle riders, are mostly outside the city and daylight, those occurred in early age and men and most commonly injuries are a blow to the organs and head trauma. Therefore, educational planning necessary to inform people about accident risks and use of helmets. On the other hand, emergency activities focus in suburban accidents and timely on scene presence.

Introduction: Trauma patients residing in rural areas face increased challenges to accessing timely and appropriate health services as a result of large geographic distances and limited resource availability. Virtual trauma supports, coined 'teletrauma', are one solution offered to address gaps in rural trauma care. Teletrauma represents a new and innovative solution to addressing health system gaps and optimizing patient care within rural settings. Here, the authors synthesize the empirical evidence on teletrauma research. Methods: A review of literature, with no date limiters, was guided by Arksey and O'Malley's (2005) scoping review methodology. The aim of the review was to provide an overview of the current landscape of teletrauma research while identifying factors associated with utilization. Results: Following a systematic search of key health databases, 1484 articles were initially identified, of which 28 met the inclusion criteria and were included for final analysis. From the review of the literature, the benefits of teletrauma for rural and remote areas were well-recognized. Several factors were found to be significantly associated with teletrauma utilization, including younger patient age, penetrating injury, and higher injury or illness severity. Lack of access to resources and clinician characteristics were also identified as reasons that sites adopted teletrauma services. Conclusion: By identifying factors associated with teletrauma utilization, teletrauma programs may be used more judiciously and effectively in rural areas as a means of enhancing access to definitive trauma care in rural areas. Gaps in current knowledge were also identified, along with recommendations for future research.

Introduction: Orthopedic conditions include a range of condition varying from traumatic injuries, congenital anomalies, chronic back-pain, arthritis, rheumatologic conditions, and other. In Nepal, approximately 2.35 million people are living with musculoskeletal conditions in which 175,000 people are suffering with MSD from non-traumatic causes. The aim of the study is to observe the clinical and demographic pattern of orthopedic problems presenting in the hospital. Methods: A descriptive cross-sectional study was done in All Nepal Hospital Private Limited in the month of November. The data from the medical record section was retrospectively collected. Sample size of 384 was included and convenience sampling technique was used. The descriptive statistical analysis was done. Results: Eight hundred forty four cases were included in the study. Implant removal was the main condition for admission in the hospital 105 (12.44%) followed by Forearm Bone Fracture 84 (9.95%) and Cut Injury 64 (7.58%). Most cases presented during the time 6 PM to 12 AM 312 (36.96%). Conclusions: Most of the orthopedic cases presented in the evening and night time. Implant removal, forearm bone fracture and cut injury form the major bulk of the cases.

Surviving near-lethal insults, such as sepsis, trauma, and major surgery is more common due to advances in medical care. The decline in mortality has unmasked a population of chronic critically ill patients, many with the pathological immunophenotype known as Persistent inflammation, Immunosuppression, and Catabolism Syndrome (PICS). Though initially described in adults, many critically ill children exhibit the hallmarks of PICS, including lymphopenia, hyperinflammation, and evidence of ongoing somatic protein catabolism. These patients are plagued with recurrent infections and suffer worse outcomes. There remains a need to understand the pathophysiology underlying this condition to elucidate potential therapies and develop interventions. Globally, 90% of road crash deaths occur in the developing world. Children in Africa bear the major part of this burden, with the highest unintentional injury rates in the world. Our study aims to better understand injury patterns among children living in Kampala, Uganda and provide evidence that injuries are significant in child health. Trauma registry records of injured children seen at Mulago Hospital in Kampala were analysed. Data were collected when patients were seen initially and included patient condition, demographics, clinical variables, cause, severity, as measured by the Kampala trauma score, and location of injury. Outcomes were captured on discharge from the casualty department and at two weeks for admitted patients. From August 2004 to August 2005, 872 injury visits for children

Introduction: Maxillofacial injury is one of the commonest causes of surgery performed by an oral and maxillofacial surgeon. Socioeconomic conditions, cultural variation, age, and gender affect the etiology of the injury. The study is aimed to find the prevalence of facial injury that is operated by the oral and maxillofacial surgeons in the College of Medical Sciences and Teaching Hospital, Bharatpur, Chitwan, a tertiary hospital. Methods: A descriptive cross-sectional study was performed using the chart from the hospital registry for the patient being operated under general anesthesia from April 1, 2017, to March 2019. Simple random sampling was done using computer-generated random numbers. Ethical approval was received from the Institutional Review Committee of the hospital. The Data for the reason for surgery, age, age groups etiology, and tissue involvement were analyzed using Statistical Package for the Social Sciences version 20. Results: Facial injury occupies 378 (71.59%) of the total operation performed by Oral and Maxillofacial surgeon in a tertiary hospital. Soft tissue 196 (52.85%) and facial bone fracture 182 (48.15%) is distributed among the facial injuries. Young adults are commonly affected, and the road traffic accident is the major cause of facial trauma. Conclusions: Facial injury-related surgeries are more prevalent in the tertiary hospital of Bharatpur.

The purpose of this study was to examine the relationships between traumatic experiences, insecure attachment styles and psychiatric symptoms in adulthood. Self-report measures concerning trauma, attachment styles and psychopathology were administered to 59 adults (33.9% males, 66.1% females) ranging in age from 18 to 61 years old. Results showed that traumatic experiences were associated with insecure attachment styles and psychopathology, in particular concerning aggressive tendencies. Findings of the study suggest that the exploration of past experiences and current attachment relationships, in terms of interpersonal functioning, may be crucial for understanding aggressiveness.

Danielle Knafo's "The Sexual Illusionist" is examined from the perspective of working unobtrusively with unsymbolized and unrepresented states in the register of enactment. Knafo's paper offers a unique rendition of an extremely challenging treatment involving great intimacy, love, violation and rampant and provocative sexual fantasies. In this commentary I suggest that the treatment exemplifies the challenge of working in the realm of psychic excess, too muchness and chronic developmental failure. I suggest that much of the highly sexualized material can be best understood as attempts by the patient, Cain, to create a transformational object such that he can manage his immersion in excess and come into being as a subject. I suggest that the therapeutic action takes place in the realm of unsymbolized enactment where love and limit can grow and take hold. INTRODUCTION[|]The aim of the present study was to report the outcomes of pars plana lensectomy (PPL) combined with pars plana vitrectomy (PPV) in traumatic ectopia lentis performed at a tertiary eye hospital.[¤]METHODS[|]Patients who underwent PPL combined with PPV for surgical management of traumatic lens subluxation/dislocation in the clinic were retrospectively evaluated. Only patients with at least 6 months follow-up were included in the study.[¤]RESULTS[|]Sixteen eyes of 16 patients were included in the study. Of the total, 94% of the patients were male and 6% were female. The mean age of the patients was 41+-17 years. The mean length of follow-up was 9+-5 months. At presentation, 19% of the patients had intravitreal hemorrhage and 38% had retinal detachment. Preoperatively, 19% of the patients had 5/200 or better visual acuity. At the final visit, visual acuity was 'hand motion' or better in all patients, and 38% of the patients had 5/200 or better visual acuity. Visual acuity was stable or improved in all patients. Anatomical success was achieved in all patients who had retinal detachment preoperatively.[¤]DISCUSSION AND CONCLUSION[|]PPL combined with PPV is a safe and effective method of surgical management of traumatic ectopia lentis; however, the associated pathologies determine the final visual outcome and preoperative visual acuity is the most important prognostic factor.[¤]

This article intends to show the possibility of change that allows Art Therapy with its purely non-verbal language. It is well known that Art Therapy has a particular quality that is not typical of conventional treatments. These qualities will be discussed showing a practical example as to what distinguishes us, looking to motivate other professionals of this discipline to deepen and study them. The interaction in an Art Therapy space and materials are presented thru the case of a four year old girl who’s presented hyperactivity behavior derived from the experience of highly stressful situations. Through the interaction with materials like tempera colors this girl lowers the body activation and start a journey from pre-representation to representation.

Endonasal or transnasal procedures are sometimes necessary in patients with head trauma. Before these procedures, the integrity of the skull base must be considered to avoid penetration of the cranial vault. A 54-year-old man was taken to a district hospital following a car accident. After the initial assessment and emergency treatment, he was transferred to our emergency clinic for further examination. The patient had massive nasal bleeding, though a Foley catheter had been inserted to control posterior bleeding. Computed tomography (CT) revealed that the catheter was not positioned correctly and was in the cranial vault. Several fractures of the maxillofacial and cranial bones and cervical vertebrae were detected on CT. He also had right hemothorax and bilateral brain contusions. Endonasal insertion of catheters or tubes in trauma patients without a complete assessment of the skull base can cause serious complications.

Solitary Plasmacytoma is characterized by a mass of neoplastic monoclonal plasma cells in either bone or soft tissue without evidence of systemic disease attributing to myeloma. Solitary bony plasmacytoma commonly presents in the axial skeleton. The occurrence of solitary bony plasmacytoma in young individuals is exceedingly rare but has been reported sporadically. We report a case of Solitary bony plasmacytoma involving the calcaneum (appendicular skeleton) in a 26-year-old male with a prior history of trauma at the same site. To our knowledge, this is one of the few cases of solitary bony plasmacytoma in the calcaneum being reported. Our patient presents with some unusual features like the unusual site of the lesion (calcaneum), presentation at a young age and the disease being symptomatic following trauma.

O artigo analisa o conto “Dodora”, de Bernardo Kucinski, publicado em Você vai voltar pra mim e outros contos (2014), obra de teor testemunhal que tematiza o contexto da ditadura civil-militar brasileira. Apesar do livro ter sido publicado após o processo de redemocratização, nota-se a necessidade de rememorar o passado no tempo presente, na perspectiva de evitar a perpetuação do autoritarismo de outrora. Nesse ínterim, debatem-se os impactos traumáticos da ditadura, sobretudo a “síndrome da tortura”, medo constante provocado pelo clima de opressão da época. A leitura que se propõe do conto de Kucinski, portanto, está orientada a entender de que maneira a literatura produzida no contexto pós ditadura civil-militar elabora a repressão política e suas sequelas. Para tanto, esse estudo, eminentemente bibliográfico, recorre a estudiosos de diversas áreas do conhecimento, dentre eles, Ginzburg, Sarlo, Seligmann-Silva e Viñar.

O presente trabalho tem por objetivo investigar como os membros da irmandade de Narcóticos Anônimos lidam com a questão da memória, do trauma e do tratamento. O trauma é ressignificado, proporcionando a reconstrução das identidades sociais dos membros, através de um processo onde a chave para a recuperação, segundo essa visão, passa pela narrativa e pelo sentimento de pertencimento e identificação com o grupo.

Post-traumatic stress disorder (PTSD) is a mental health condition that can occur following exposure to a traumatic experience. An estimated 12 million U.S. adults are presently affected by this disorder. Current treatments include psychological therapies (e.g., exposure-based interventions) and pharmacological treatments (e.g., selective serotonin reuptake inhibitors (SSRIs)). However, a significant proportion of patients receiving standard-of-care therapies for PTSD remain symptomatic, and new approaches for this and other trauma-related mental health conditions are greatly needed. Psychedelic compounds that alter cognition, perception, and mood are currently being examined for their efficacy in treating PTSD despite their current status as Drug Enforcement Administration (DEA)- scheduled substances. Initial clinical trials have demonstrated the potential value of psychedelicassisted therapy to treat PTSD and other psychiatric disorders. In this comprehensive review, we summarize the state of the science of PTSD clinical care, including current treatments and their shortcomings. We review clinical studies of psychedelic interventions to treat PTSD, trauma-related disorders, and common comorbidities. The classic psychedelics psilocybin, lysergic acid diethylamide (LSD), and N,N-dimethyltryptamine (DMT) and DMT-containing ayahuasca, as well as the entactogen 3,4-methylenedioxymethamphetamine (MDMA) and the dissociative anesthetic ketamine, are reviewed. For each drug, we present the history of use, psychological and somatic effects, pharmacology, and safety profile. The rationale and proposed mechanisms for use in treating PTSD and traumarelated disorders are discussed. This paper is inspired by Anzieu's concept of the "skin-ego" as a psychic agency distinguished by a set of ego functions that are symbolic representations of the three main physiological functions of the skin: a primary mode of communication with others, a protective barrier from the external world, and a container of internal organs. The author suggests skin-ego configurations along three qualitative dimensions: sensitivity, permeability, and elasticity. As the imaginary space in which fantasies, dreams, thoughts, and psychopathological formations are established, the skin-ego affects how one negotiates subjectivity and intersubjectivity. The author presents two patients whose skin-ego functions were compromised by early trauma to illustrate how their skin-ego characteristics shape the way they negotiate their contradictory, coexisting needs to connect with others and to protect against the other's potential impingements. The implications of skin-ego functions to the therapeutic process, as well as the therapist's skin-ego functions, are also discussed. [ABSTRACT FROM AUTHOR]

Article 913 of the Civil Procedure Code regulates two situations in which the court executor will establish the existence of an impediment to enforcement: when the minor himself categorically refuses to leave the debtor, respectively when the minor has aversion to the creditor. Faced with either of the two hypotheses, the court executor will not put pressure on the minor (he will not use force, he will not abuse him), but he will draw up an official report, which he will communicate to the parties and to the representative of the General Directorate of Social Assistance and Child Protection, which will notify the competent court (the guardianship and family court) to order a psychological counselling programme appropriate to the child’s age as a protection measure necessary to avoid the child’s subsequent refusal on enforcement. The child who has reached the age of 10 will be mandatorily heard. According to the law, the psychological counselling programme can not exceed 3 months. After an initial psychological assessment of the child, the psychologist appointed by the court will determine the duration and content of the psychological counselling programme. Upon the completion of the psychological counselling programme ordered by the guardianship court, the psychologist draws up a report which he communicates to the court, to the court executor and to the General Directorate of Social Assistance and Child Protection. Whereas the starting point for the new regulation was represented by the presumption that the minor’s going through the counselling programme will lead to the change of attitude towards the creditor of the obligation, after receiving the report drawn up by the psychologist, the court executor will resume the procedure of enforcement, under the terms of Article 911 of the Civil Procedure Code, therefore in the presence of a representative of the General Directorate of Social Assistance and Child Protection and, if the latter considers it necessary, of a psychologist, and he may benefit, when necessary, from the assistance of the public force agents. Starting from the purpose of adopting the new enforcement procedure for minors, we analyze the extent to which the current regulation of the psychological counselling within the enforcement respects the principle of the higher interest of the minor. The doctrine1 has emphasized the added value of the new regulation, reminding that, in the case law developed in the system of the Civil Procedure Code of 1865, before the introduction of the periodical cominatory fines, it was noted that the judgment which provided the surrender of the minor to the applicant is susceptible to be subject to enforcement and that the failure to comply with the judgment was not attributable to the defendant, who had not opposed the enforcement, the impossibility of carrying on with the enforcement resulting from the opposition of the minor. Psychological counselling is an activity aimed at shaping the behavioural actions of the counselled person in a given situation or, generally speaking, in life and daily activity. Undoubtedly, in the subject matter under review, the psychological counselling is a precious remedy, even without a rival, aspect which also results from ECHR case law. Family therapy, where the therapist focuses on the child’s experience, while observing the entire family, responds best to the child’s needs in context, giving due importance to the specific situations presented by the people involved. Whatever the origin of a child’s problems, the approach is aimed at: bringing together the distinct descriptions offered by the different family members; explaining the appearance of some blockings of the inner resources, which lead to the inability of the family to solve its own problems; the use of individual or family inner resources in order to reach more complex and advantageous awareness, interactions and conducts to solve the difficulties2. The conclusions of the psychological counselling report ordered within the enforcement should allow the court to reassess the measures contained in the enforceable title, in relation to the higher interest of the child and to the current state of facts. Woda, kajaki i reguły gry zespołowej dostarczają wiele emocji i wymagają od zawodnika pełnego zaangażowania fizycznego. Założono, że ze względu na duże obciążenia treningowe i specyfikę gry w kończynach górnych może dojść do przeciążeń tkanek miękkich w obrębie obręczy barkowej. Cel pracy: Celem pracy była ocena ruchomości w obrębie stawu ramiennego z uwzględnieniem długości trenowania. Postawiono następujące pytania badawcze: 1. Czy staż treningowy w dyscyplinie sportowej kajak polo wpływa na ruchomość stawu ramiennego? 2. Czy stały trening ogranicza ruchomość we wszystkich płaszczyznach w obrębie stawu ramiennego Materiały i metody: W badaniach wzięło udział 26 osób trenujących kajak polo: 8 kajakarek, 18 kajakarzy. Wiek (M ± SD) 15,57 ± 4.63; staż treningowy 5,35 ± 2,80. Ze względu na wiek i czas specjalistycznego trenowania dokonano podziału zebranego materiału badawczego na 2 grupy badawcze. W pierwszej grupie jest siedem osób (n=7); drugiej dziewiętnaście (n=19). Średnia wieku I grupy (n=7) wynosi (M ± SD) 12,71 ± 3,37; staż treningowy 1,43 ± 0,86. Średnia wieku II grupy (n= 19) wynosi 16,63 ± 4,58; staż treningowy 6,79 ± 1,64. Do badań wykorzystano przyrząd mierniczy goniometr a uzyskane wyniki zapisano w systemie SFTR. Uzyskane wyniki opracowano statystycznie obliczając średnie arytmetyczne zakresów ruchów obu grup oraz określenie istotności zmian. Wyniki Analiza wykazała, że ruchomość w stawie ramiennym zmienia się wraz z dłuższym stażem treningowym. Grupa I z krótszym stażem treningowym w porównaniu do grupy II z dłuższym stażem treningowym wykazuje większą mobilność stawu. U grupy II zauważono w płaszczyźnie strzałkowej ograniczenie ruchu zgięcia, natomiast wyprost względem grupy I zwiększył się. Przy ruchu odwiedzenia w płaszczyźnie czołowej u grupy II obserwuje się zmniejszenie ruchomości. W płaszczyźnie horyzontalnej (u trenujących powyżej 3 lat) ograniczony jest równocześnie ruch zgięcia i wyprostu horyzontalnego. Ruchomość stawu u grupy II przy rotacji zewnętrznej wzrosła, natomiast przy rotacji wewnętrznej zmniejszyła się. Wnioski 1. Staż treningowy w dyscyplinie sportowej kajak polo wpływa na ruchomość stawu ramiennego. 2. Stały trening ma różnorodne oddziaływanie, ogranicza ruchy w płaszczyźnie poprzecznej, zgięcia w płaszczyźnie strzałkowej oraz zgięcia w płaszczyźnie horyzontalnej. Players trying to put the ball in the opponent's net, they are playing in short and moveable canoes. The canoe polo ball you can throw with your hands or paddle. Water, canoes and game rules provide a lot of excitement and require the full commitment of the physical player. It was assumed that due to the overloading training and specifics of the game, in the upper extremities can lead to overload the soft tissues around the shoulder girdle. The aim of the study: The aim of the study was to assess mobility within the shoulder joint, taking into account the length train. Presented the following research questions: 1. Is long-term training in sport canoe polo affects the mobility of the shoulder joint? 2. Is constant training limited mobility in all directions of the shoulder joint? Material and methods: The study involved 26 people practicing canoe polo: 8 female canoeists, 18 male canoeists. Age (M ± SD) 15,57 ± 4.63; apprenticeship training 5,35 ± 2,80. Due to the age and time specialist training division has been collected research material on 2 research groups. In the first group there are seven people (n=7), second nineteen (n=19). The average age group I is (M ± SD) 12,71 ± 3,37; apprenticeship training 1,43 ± 0,86. The averge age group II is 16,63 ± 4,58; apprenticeship training 6,79 ± 1,64. The study used goniometer measuring instrument and the results stored in the system SFTR. The results were statistically analyzed by calculating the arithmetic mean of range movement of the two groups and determine the significance of the changes. Results: The analysis showed that the mobility of the shoulder joint changes with longer training experience. Group I with a shorter training experience compared to the group II of the long training experience has greater mobility of the joint. In Group II noted in the sagittal plane flexion is limited, while extension in compare to the group I grew. In the motion of abduction in the frontal plane of group II have reduced mobility. In the horizontal plane the flexion and extension are simultaneously limited. Joint mobility in Group II of external rotation has increased, while at the internal rotation decreased. Conclusions: 1. The apprenticeship training in their sport canoe polo affects the mobility of the shoulder joint. 2. Constant training has various impacts on shoulder joint, restricts movements in the transverse plane, flexion in sagittal plane and flexion in horizontal plane. A literatura científica mundial adota atualmente a denominação de negação não psicótica da gravidez para se referir à situação na qual uma mulher não sabe que está grávida até o momento do parto, ou pelo menos durante boa parte do período gestacional. Pode ser definida, também, como a falta de consciência subjetiva por parte da mulher em relação a estar grávida. Esse fenômeno é explorado neste artigo mediante a abordagem de conflitos e mecanismos psíquicos que sustentam sua dinâmica psíquica. Para tal, discorrese sobre o mecanismo Verleugnung, a partir de proposições do psicanalista Luis Cláudio Figueiredo, no sentido de traduzi-lo como “desautorização”. Entende-se que na situação da negação não psicótica da gravidez, explorada sob o viés da desautorização, se encontram ampliadas as condições de compreensão a respeito do estabelecimento de uma condição psíquica singular a qual resulta na situação em que uma mulher não toma conhecimento de sua condição de estar gerando outro ser. Também são apresentadas contribuições de autores psicanalíticos contemporâneos que fornecem pertinentes ferramentas de compreensão no que diz respeito à situação da negação não psicótica da gravidez. Tal fenômeno, ao desautorizar a percepção e gerar um impedimento de estabelecer conexões psíquicas e atribuir sentido a esta experiência, resulta no predomínio de intensidades traumáticas no psiquismo.

A fundação da psicanálise deu-se a partir da formulação do conceito de inconsciente, contudo é possível observar na teoria psicanalítica passagens em que à discussão sobre o tema ultrapassa o registro do universo individual para envolver as relações entre sujeito e cultura, herança filogenética e transmissão psíquica. Dentro dessa perspectiva, este trabalho tem o objetivo de introduzir a discussão sobre o conceito de inconsciente social rastreando seus antecedentes na psicanálise freudiana e na grupanálise inglesa. Tendo como metodologia a pesquisa teórica, o artigo apresenta investigações teóricas atuais lideradas pelos psicanalistas e grupanalistas Earl Hopper e Haim Weinberg. Suas investigações valorizam as interdependências entre indivíduo e sociedade, a coconstrução dos fatos sociais, como também as coerções e restrições de natureza inconsciente presentes nessas relações. O artigo utiliza-se de autores clássicos como Freud, Burrow, Fromm, Foulkes, Elias e Castoriadis e de conceitualizações dos contemporâneos Hopper, Weinberg, Dalal e Volkan. Na sequência aponta para investigações psicanalíticas e grupanalíticas sobre trauma e transmissão psíquica em grandes grupos traumatizados. O estudo sobre o inconsciente social ainda está em seu início, mas promete conferir novas dimensões às investigações sobre as relações entre psicanálise e cultura.

Objectives: To assess the blood transfusion support requirements in mass disaster and trauma situations. Study Design: Cross-sectional observational study. Place and Duration of Study: This study was conducted at Combined Military Hospital Quetta from, Jan 2013 to Dec 2015. Material and Methods: Nature of injuries, triage details, details of surgical procedures and duration of hospital stay were noted. Data was analyzed with respect to cross match to transfusion ratio and the number of units of each component transfused. Patients requiring massive transfusion and any associated complications were also studied. Results: A total of 2228 casualties were received during the study period, of these, males were 18 (75%) and 6 (25%) were females. Mean age was 29.7 years. 1636 (73.4%) casualties had sustained major injuries. Mean hospital stay was 6.31 days. Only 199 (12.2%) patients required blood transfusion with a mean of 2.9 units of RCC, 8.7 bags of FFP and 4.6 bags of platelets. Fifteen (7.5%) patients received massive transfusion. Following massive transfusion, one case of metabolic acidosis and two cases of coagulopathy were reported. Conclusion: Mass disasters and trauma casualties pose a serious challenge to any healthcare facility in general and the blood transfusion services in particular. Only a well-organized blood transfusion center and blood transfusion emergency preparedness can result in better patient care and outcome. This article argues that El tren de la memoria, through its emphasis on female witnesses, confronts and "works through" personal and collective memories of Spanish economic migration to West Germany during the Franco regime. El tren de la memoria functions as a mode of witnessing, and mediates between subjective and collective experiences of the migration. The documentary invites us to re-assess the cinematic archive and to rethink the historical past, so we can foreground the political ontology of the documentary image in terms of its historiographical function. I depart from an exclusive sociological focus in order to explore how the migratory is articulated in, and transmitted through, the documentary form. In addition, I ask whether El tren de la memoria enables us to reflect on whether elements of past totalitarian violence could still seep into the sovereign power of capitalist democracies. While El tren de la memoria may seem to be essentially an historical documentary, this article argues that it is far more; it demands a rethinking of migrant subjectivity, which reveals an irreducible alterity always already constitutive of the self. Thus the documentary does not simply reflect reality, but contributes to the imagination or transformation of reality and its structures of discrimination and exclusion of migrants. El tren de la memoria encourages us to imagine a community based on singularities, which share a world inhabited by plurality, a community in which our relation to alterity is based on our common finitude.
