Context: The Y-Balance Test Lower Quarter (YBT-LQ) is a widely utilized tool for evaluating dynamic postural control, requiring a combination of mobility and strength. This study aimed to investigate the combined relationship between isometric thigh muscle strength and joint kinematics on YBT-LQ performance. Design: Cross-sectional laboratory study. Methods: Isometric quadriceps and hamstrings strength were measured before the YBT-LQ in 39 healthy participants (27 females and 12 males). The test was performed under 3-dimensional markerless motion capture, where joint kinematics were extracted from the maximum reach position from each direction. Three multivariable linear regression models were then used to determine the strongest combination of predictors for YBT-LQ performance. Results: Greater hamstrings strength and increased knee flexion, ankle dorsiflexion, and trunk ipsilateral-flexion joint angles explained 56.8% (P <.001) of the variance in anterior reach. Hip flexion, knee flexion, and ankle dorsiflexion angles were the strongest predictors for posteromedial reach distance, explaining 73.0% of the variance (P <.001). Last, 43.3% (P <.001) of the variance in posterolateral reach distance was predicted by hamstring strength and knee-flexion angle. Conclusions: These results emphasize the importance of hamstring strength in YBT-LQ performance across different reach directions. Additionally, the kinematics illustrate a potential movement strategy for maximizing reach distance on the YBT-LQ in healthy individuals. Clinicians can utilize this information to guide interventions aimed at improving dynamic postural control, particularly by focusing on increasing hamstring strength and testing for impairments in specific movement patterns. Context: We designed this study to investigate the effects of 2 myofascial release techniques, Instrument-Assisted Soft Tissue Mobilization (IASTM) and Foam Roller (FR), on pain, joint range of motion, and muscle strength in athletes suffering from iliotibial band (ITB) tightness. Design: A total of 39 male soccer players were enrolled in this randomized controlled trial, aged between 18 and 23 years who were divided into 3 groups: Only Exercise, IASTM, and FR. Methods: All participants performed daily strengthening and stretching exercises, while 1 group added IASTM, and the other added FR to the exercise program. We evaluated ITB tightness with the Ober test and an inclinometer, pressure pain threshold, using an algometer, and we evaluated muscle strength with the Cybex Norm Isokinetic device. Results: We found that all 3 groups exhibited an increase in the Ober inclination angle after the interventions (P =.001), but the increase was greater for participants in the IASTM and FR groups, compared with exercise alone. Additionally, both the IASTM and FR groups displayed an increased pressure pain threshold (P =.001), whereas there was no change in the control group. Moreover, while all 3 groups experienced an increase in hip muscle strength (P =.001), the IASTM and FR groups exhibited a greater increase compared with exercise alone (P =.001). Conclusions: Based on these findings, exercise improves pain, range of motion, and muscle strength in athletes with ITB tightness, and IASTM, and FR techniques enhanced exercise effects but did not differ from one another. While our study demonstrated that both IASTM and FR techniques significantly enhance the benefits of exercise for athletes with ITB tightness, further research could delve into the long-term effects of these interventions. Context: Contusion and soft tissue injuries are common in sports. Photobiomodultion, light and laser therapy, is an effective aid to increase healing rates and improve function after various injury mechanisms. However, it is unclear how well photobiomodulation improves function after a contusion soft tissue injury. This study aimed to determine the effects of a pulsed red and blue photobiomodulation light patch on muscle function following a human thigh contusion injury. Design: Single-blinded randomized control trial design. Methods: We enrolled 46 healthy participants. Participants completed 5 visits on consecutive days. On the first visit, participants completed a baseline isokinetic quadriceps strength testing protocol at 60°/s and 180°/s. On the second visit, participants were struck in the rectus femoris of the anterior thigh with a tennis ball from a serving machine. Immediately following, participants were treated for 30 minutes with an active or placebo photobiomodulation patch (CareWear light patch system, CareWear Corp). Following the treatment, participants completed the same isokinetic quadriceps strength testing protocol. Participants completed the treatment and isokinetic quadriceps strength test during the following daily visits. We normalized the data by calculating the percent change from baseline. We used a mixed model analysis of covariance, with sex as a covariate, to determine the difference between treatment groups throughout the acute recovery process. Results: We found the active photobiomodulation treatment significantly increased over the placebo group, quadriceps peak torque during the 180°/s test (P =.030), and average power during both the 60°/s (P =.041) and 180°/s (P ≤.001) assessments. The mean peak torque and average power of 180°/s, at day 4, exceeded the baseline levels by 8.9% and 16.8%, respectively. Background: Identifying distinct mechanisms and biomarkers for painful diabetic peripheral neuropathy (DPN) is required for advancing the treatment of this major global unmet clinical need. We previously provided evidence in calf skin biopsies that disproportion between reduced sensory small nerve fibers and increased blood vessels may distinguish painful from non-painful DPN. We proposed that overexposure of the reduced nerve fibers in DPN to increased hypoxemia-induced vasculature and related algogenic factors, e.g., nerve growth factor (NGF), leads to neuropathic pain. To further investigate this proposed mechanism, we have now studied more proximal thigh skin biopsies, to see if the same disproportion between increased vasculature and decreased nerve fibers generally differentiates painful DPN from painless DPN. Methods: A total of 28 subjects with type 2 diabetes (T2DM) and 13 healthy volunteers (HV) underwent detailed clinical and neurophysiological assessments, based on the neuropathy composite score of the lower limbs [NIS(LL)] plus 7 tests. T2DM subjects were subsequently divided into three groups: painful DPN (n = 15), painless DPN (n = 7), and no DPN (n = 6). All subjects underwent skin punch biopsy from the upper lateral thigh 20 cm below the anterior iliac spine. Results: Skin biopsies showed decreased PGP 9.5-positive intraepidermal nerve fiber (IENF) density in both painful DPN (p < 0.0001) and painless DPN (p = 0.001). Vascular marker von Willebrand Factor (vWF) density was markedly increased in painful DPN vs. other groups, including painless DPN (p = 0.01). There was a resulting significant decrease in the ratio of intraepidermal nerve fiber density to vasculature and PGP9.5 to vWF, in painful DPN vs. painless DPN (p = 0.05). These results were similar in pattern to those observed in these HV and T2DM groups previously in distal calf biopsies; however, the increase in vWF was much higher and nerve fiber density much lower in the calf than thigh for painful DPN. Thigh skin vWF density was significantly correlated with several metabolic (waist/hip ratio, HbA1c), clinical (e.g., pain score), and neurophysiological measures. Conclusion: This study supports our proposal that increased dermal vasculature, and its disproportionate ratio to reduced nociceptors, may help differentiate painful DPN from painless DPN. This disproportion is greater in the distal calf than the proximal thigh skin; hence, neuropathic pain in DPN is length-dependent and first localized to the distal lower limbs, mainly feet. It is observed that the locations of the most functional lymphatic vessels in the lymphedematous limbs can differ significantly from those in healthy limbs. The aim of this study was to elucidate the lymphatic map of lymphedematous limbs. We retrospectively analyzed 59 patients (118 limbs) with lower limb lymphedema. Fifty-five were women and four were men. The mean age and duration of lymphedema was 62.4 and 7.7 years, respectively. For the lateral thigh lymphosome, we injected indocyanine green (ICG) at the lateral knee and measured the distance (Dt) between the anterior superior iliac spine (ASIS) and the point where the lymphatic vessels crossed the reference line (the line connecting the ASIS and the patellar center). For the lateral calf lymphosome, we injected ICG at the lateral ankle and measured the distance (Dc) between the inferior patellar border and the point where the lymphatic vessels crossed the reference line (the anterior border of the tibia). In the lateral thigh, the mean Dt was 30.4 ± 0.6 cm (range, 0–41 cm) and the distribution peaked at approximately 30 cm from the ASIS. In the calf, the mean Dc was 13.1 ± 0.9 cm (range, −11 to 32 cm). The distribution of lymphatic vessel locations was highly variable. We could establish the lymphatic map in the lymphedematous legs. Background: Limb-lengthening surgeries via nail distraction osteogenesis (DO) have become more popular lately. This provides an opportunity to study human bone that has grown longer. Case details: We present a case of a 22-year-old male who underwent internal upper and lower leg lengthening by 12 cm and 6 cm, respectively, under full weight bearing. He requested bilateral femoral shortening by 4 cm using a shortening nail, 24 months after the index surgery. The regenerated bones were harvested and analyzed. Results: Good bone quality and well-organized structure were observed in the regenerated bones compared with the native human adult bony architecture. Conclusions: This case demonstrates that bilateral bone regeneration during DO with a nail can result in a bone morphology that is comparable to that of native adult human bony macro- and micro-anatomy. This supports the effectiveness and potential of this surgical approach for limb lengthening and shortening procedures, although more investigations are necessary in this regard. Background/Objectives: There are many aspects that may influence clinical outcomes in a total hip arthroplasty (THA). The influence of femoral stem on the clinical outcome of THA is probably underestimated in the literature. Our work aims to analyze how uncemented stem geometry (straight or anatomical) in THA might affect outcomes in clinical and radiographic terms. Methods: Over a period of 36 months, in a prospective-observational manner, we collected the results of THA secondary to osteoarthritis (OA) that met the inclusion criteria with the only variable being the straight or anatomical stem design in a single manufacturer. A total of 84 patients were selected and divided into two groups: group A, treated with straight stem (44 patients), and group B, treated with anatomical stem (40 patients). The assessment clinical tools were Harris Hip Score (HHS), Visual Analogue Scale (VAS), and Short Form Health Survey-36 (SF-36). Follow-up controls were at 6 months (T0), 12 months (T1), 24 months (T2), and 36 months (T3). Results: No statistically significant differences emerged between the two groups under analysis with VAS, SF-36, and HHS. At follow-up controls, eight patients (group A) and four patients (group B) showed anterior thigh pain. At T1, there were radiographic signs of aseptic loosening in two cases (group A) and one case (group B). In group A there were two cases of iatrogenic fracture, two cases of dislocation, one case of infection, and two cases of heterotopic ossification. Conclusions: The anatomical stem compared to the straight stem showed lower complication rates outcomes; the anatomical uncemented stem could be considered as a preferred first choice in THA compared to the straight stem. Objective: To determine the association between joint structure and gait in patients with knee osteoarthritis (OA). Methods: IMI-APPROACH recruited 297 clinical knee OA patients. Gait data was collected (GaitSmart®) and OA-related joint measures determined from knee radiographs (KIDA) and MRIs (qMRI/MOAKS). Patients were divided into those with/without radiographic OA (ROA). Principal component analyses (PCA) were performed on gait parameters; linear regression models were used to evaluate whether image-based structural and demographic parameters were associated with gait principal components. Results: Two hundred seventy-one patients (age median 68.0, BMI 27.0, 77% female) could be analyzed; 149 (55%) had ROA. PCA identified two components: upper leg (primarily walking speed, stride duration, hip range of motion [ROM], thigh ROM) and lower leg (calf ROM, knee ROM in swing and stance phases). Increased age, BMI, and radiographic subchondral bone density (sclerosis), decreased radiographic varus angle deviation, and female sex were statistically significantly associated with worse lower leg gait (i.e. reduced ROM) in patients without ROA (R2 = 0.24); in ROA patients, increased BMI, radiographic osteophytes, MRI meniscal extrusion and female sex showed significantly worse lower leg gait (R2 = 0.18). Higher BMI was significantly associated with reduced upper leg function for non-ROA patients (R2 = 0.05); ROA patients with male sex, higher BMI and less MRI synovitis showed significantly worse upper leg gait (R2 = 0.12). Conclusion: Structural OA pathology was significantly associated with gait in patients with clinical knee OA, though BMI may be more important. Introduction: Surgical site infections (SSI) are one of the most common complications after extensive sarcoma resections and represent a daily challenge. SSI occur in up to 50% of cases particularly in the peripelvic area. One possible approach to reduce infection rate is perioperative antibiotic prophylaxis. The aim of this study therefore was to investigate the influence of perioperative antibiotic prophylaxis on the infection rate and the possible influence of location-specific antibiotic prophylaxis with ampicillin/sulbactam. Methods: This monocentric retrospective study included 366 patients who underwent sarcoma resections in the groin, proximal thigh, or gluteal region. All patients were operated on by 2 surgeons after neoadjuvant pretreatment if necessary. 3 groups of patients were defined. Group 1: In 60.4% of all cases, antibiotic prophylaxis was administered with cephalosporins (also clindamycin in case of penicillin allergy). Group2: In 9.8% of cases, ampicillin/sulbactam was used. Group 3: 29.8% of patients did not receive any antibiotic prophylaxis. Results: In 31.1% of treated cases, antibiotic therapy was prolonged due to extended tumor resections. Postoperative infections occurred in 23.2% (85 cases), in 77 cases within the first 90 days (on average after 20 days). The median operating time, blood loss was higher, and tumor size were significantly larger in cases with infections, compared to patients without infection. In group 1 and 2 with perioperative single-shot prophylaxis, infection occurred in 24.1% of cases, compared to 13.5% of cases without prophylaxis (group 3) (p= 0.032). In the patients with prolonged antibiotic therapy, infection occurred in 31.6% of cases, compared to 16.3% of cases without prolongation (p< 0.001). In the group 2, infection occurred in 19.4% of cases compared to 24.9% of cases in the group 1 (p= 0.479). In the multivariate analysis, surgery time longer 80 min, blood substitution, neoadjuvant radio- and chemotherapy proved to be a risk factor for SSI. Discussion: Region adapted perioperative antibiotic prophylaxis may reduce the risk of infection after extended sarcoma resection in the peripelvic area. Background: Basketball-related fractures involving the lower extremities frequently present to emergency departments (ED) in the United States (US). This study aimed to identify the primary mechanisms, distribution, and trends of these injuries. Hypothesis: We hypothesize that (1) lower extremity fracture frequency will decrease from 2013 to 2022, (2) the ankle will be the most common fracture site, and (3) noncontact twisting will be the most common injury mechanism. Study Design: Descriptive epidemiological. Level of Evidence: Level 3. Methods: The National Electronic Injury Surveillance System (NEISS) was queried for lower extremity fractures from basketball presenting to US EDs from January 1, 2013 to December 31, 2022. Patient demographics, injury location, and disposition were recorded. The injury mechanism was characterized using the provided narrative. National estimates (NEs) were calculated using the NEISS statistical sample weight. Injury trends were evaluated by linear regression. Results: There were 6259 cases (NE: 185,836) of basketball-related lower extremity fractures. Linear regression analysis of annual trends demonstrated a significant decrease in lower extremity fractures over the study period (2013-2022: P = 0.01; R 2 = 0.64). The most common injury mechanism was a noncontact twisting motion (NE: 49,897, 26.9%) followed by jumping (NE: 39,613, 21.3%). The ankle was the most common fracture site (NE: 69,936, 37.6%) followed by the foot (NE: 49,229, 26.49%). While ankle and foot fractures decreased significantly (P < 0.05), fractures of the lower leg, knee, toe, and upper leg showed no significant trends (P = 0.09, 0.75, 0.07, and 0.85, respectively). Conclusion: Basketball-related lower extremity fractures decreased from 2013 to 2022, with the ankle being the most common fracture site and most fractures arising from a noncontact twist. Increasing utilization of outpatient clinics may have contributed to the decline, particularly for ankle and foot fractures. Background: The repair of diabetic foot defects (DFD) is a major challenge in clinical practice. The purpose of this study was to investigate the difference in clinical efficacy of different flap techniques in repairing DFD wounds, and to compare the difference in clinical efficacy of anterolateral thigh perforator flap (ALTP) in repairing DFD and non-DFD. Methods: This study is a retrospective clinical study of different types of flap reconstruction in patients with DFD admitted to our hospital from January 2010 to December 2021. A total of 40 patients with DFD and 43 patients with non-DFD were included in this study. Detailed preoperative basic information, intraoperative details, postoperative complications and long-term follow-up results were collected. Result: The comorbidities, wound infection and wound duration of DFD group were more serious than those of non-DFD group. In addition, the incidence of complications was higher in DFD group, the wound healing time was longer, the aesthetic evaluation, the functional recovery rate of ankle joint and the sensory recovery effect of flaps were worse. Conclusion: In this study, it is concluded that different flap techniques can obtain better clinical efficacy in repairing DFD wounds. Compared with non-DFD wounds, the postoperative risk of DFD wounds using free ALTP flaps is higher, but the risk can be reduced by reasonable preoperative vascular examination. Facial reconstruction is challenging for plastic surgeons, as it can be difficult to decide the best approach. One technique that has been widely used is the anterolateral thigh (ALT) flap due to its numerous benefits. However, its thickness can be a drawback, especially regarding facial reconstruction. The thinning technique is not a new novel, but how to apply it to the ALT flap to get the best result hasn't been reported yet. Our study involved 117 patients, and we used 73 thinned ALT flaps to determine the best method to increase the flap's safety. After thinning, we significantly reduced the flap's thickness from an average of 22.5 mm to 5.9 mm, making it more suitable for contouring purposes. We apply a thinned ALT flap for coverage, contouring, and recreating the facial 3D structure. The 12/45 flap has the chance to make the multiple-paddle ALT flap, which helps to reconstruct difficult positions even more flexibly. The key to successfully thinning the ALT flap is understanding the perforator's structure and pathway through the fascia. With the thinning technique, we have overcome the limitations of the flap's thickness, making it suitable for use in whole-body reconstruction. Background The sartorius is a muscle of lower limb thigh with variant morphological distribution of blood supply. The detailed knowledge vascular patterns of the muscle and overlying skin is an absolute guide in reconstructive surgery. In this study the vascular pattern is analyzed by dissection method both in sartorius and overlying skin. Methods 50 sartorius muscles and the circumferential skin of the thigh were dissected from 25 cadavers. Anatomical morphology and Morphometrical parameters, such as pedicle number, location, diameter, and length, are recorded using vernier callipers. Thread method was used to measure the length of muscle supplied by a single pedicle which is defined as a major pedicle. Lastly, the area of cutaneous territory of each major pedicle was calculated. Results The sartorius muscle is supplied by 5 to 8 vascular pedicles. The major pedicles are proximal 20 to 28 cms from ASIS size around 2cms Distal major pedicle 32 to 40 cms from ASIS size around 2mm. Conclusion This vascular anatomical pattern study is a guideline in freeflap surgeries of sartorius muscle during transposition with the pedical of the sartorius muscle. [ABSTRACT FROM AUTHOR]

Falls represent a significant risk factor, necessitating accurate classification methods. This study aims to identify the optimal placement of wearable sensors—specifically accelerometers, gyroscopes, and magnetometers—for effective fall-direction classification. Although previous research identified optimal sensor locations for distinguishing falls from non-falls, limited attention has been given to the classification of fall direction across different body regions. This study assesses inertial measurement unit (IMU) sensors placed at 12 distinct body locations to determine the most effective positions for capturing fall-related data. The research was conducted in three phases: first, comparing classifiers across all sensor locations to identify the most effective; second, evaluating performance differences between sensors placed on the left and right sides of the body; and third, exploring the efficacy of combining sensors from the upper and lower body regions. Statistical analyses of the results for the most effective classifier model demonstrate that the support vector machine (SVM) is more effective than other classifiers across all sensor locations, with statistically significant differences in performance. At the same time, the comparison between the left and right sensor locations shows no significant performance differences within the same anatomical areas. Regarding optimal sensor placement, the findings indicate that sensors positioned on the pelvis and upper legs in the lower body, as well as on the shoulder and head in the upper body, were the most effective results for accurate fall-direction classification. The study concludes that the optimal sensor configuration for fall-direction classification involves strategically combining sensors placed on the pelvis, upper legs, and lower legs. Wearable gait analysis systems using inertial sensors offer the potential for easy-to-use gait assessment in lab and free-living environments. This can enable objective long-term monitoring and decision making for individuals with gait disabilities. This study explores a novel approach that applies a hidden Markov model-based similarity measure (HMM-SM) to assess changes in gait patterns based on the gyroscope and accelerometer signals from just one or two inertial sensors. Eleven able-bodied individuals were equipped with a system which perturbed gait patterns by manipulating stance-time symmetry. Inertial sensor data were collected from various locations on the lower body to train hidden Markov models. The HMM-SM was evaluated to determine whether it corresponded to changes in gait as individuals deviated from their baseline, and whether it could provide a reliable measure of gait similarity. The HMM-SM showed consistent changes in accordance with stance-time symmetry in the following sensor configurations: pelvis, combined upper leg signals, and combined lower leg signals. Additionally, the HMM-SM demonstrated good reliability for the combined upper leg signals (ICC = 0.803) and lower leg signals (ICC = 0.795). These findings provide preliminary evidence that the HMM-SM could be useful in assessing changes in overall gait patterns. This could enable the development of compact, wearable systems for unsupervised gait assessment, without the requirement to pre-identify and measure a set of gait parameters. [ABSTRACT FROM AUTHOR]

It is known that microtrauma exists in the thigh muscles after long‐distance running such as the half‐marathon. Moreover, training characteristics of long‐distance runners may influence the specificity of the distribution of muscle fiber types in the thigh and affect muscle responses to lipid metabolism. However, the specific changes in microtrauma and intramuscular lipid in thigh muscles after a half‐marathon are unknown. A cohort of 20 healthy recreational marathon runners was recruited to complete a half‐marathon. MRI T2 mapping and 6‐echo q‐Dixon sequences were employed at baseline (P1), 2–3 h after running (P2), and 1 day after running (P3). Inflammatory markers (the T2 values) and intramuscular fat fraction (the proton density fat fraction, PDFF) were measured in thigh muscles to detect microtrauma and intramuscular lipid changes, respectively. One‐way analysis of variance showed significant time effects for T2 values and PDFF. Post hoc analysis of the 14 datasets collected at three time points revealed significantly higher T2 values in all thigh muscles after running (all p < 0.05). Significant differences in T2 values persisted for all thigh muscles at P3 compared to P1 (all p < 0.05). The PDFF of the vastus lateralis and vastus medialis was significantly decreased at P2 compared to P1 (p < 0.05). No significant differences in PDFF were observed for the thigh muscles at P3 compared to P1. The manifestations of inflammation edema and intramuscular lipid investigated through MRI may offer valuable insights for recreational marathon runners regarding the lower limb movement characteristics during half‐marathon running. The aim of this study was to assess the agreement of three different automated methods of identifying force-onset (40 N, 5 SDs, and 3 SDs) with manual identification, during the isometric mid-thigh pull (IMTP). Fourteen resistance-trained participants with >6 months experience training with the power clean volunteered to take part. After three familiarisation sessions, the participants performed five maximal IMTPs separated by 1 min of rest. Fixed bias was found between 40 N and manual identification for time at force-onset. No proportional bias was present between manual identification and any automated threshold. Fixed bias between manual identification and automated was present for force at onset and F150. Proportional but not fixed bias was found for F50 between manual identification and all automated thresholds. Small to moderate differences (Hedges g = −0.487- −0.692) were found for F90 between all automated thresholds and manual identification, while trivial to small differences (Hedges g = −0.122—−0.279) were found between methods for F200 and F250. Background: Excess body adiposity and skin irregularities represent a major problem in today's society. Currently, radiofrequency‐based devices constitute an increasingly popular medical‐aesthetic application and a powerful non‐invasive method to combat this problem. Objective: This study aims to determine the efficacy and safety of the high‐frequency device Zionic™ (Termosalud, Inc., Gijón, Spain) in reducing body contouring and improving skin appearance in the abdomen/flanks or thighs. Materials and Methods: Thirty nine individuals were treated with the Zionic™ device in the abdomen/flanks or thighs. A total of 8 sessions of 40–50 min each, 72/96 h apart were performed. At baseline, body measurements were taken including photographs, body contours, and ultrasound scans to assess dermal thickness, dermal echogenicity, and subcutaneous fat thickness. The measurements were repeated after the eight sessions for comparison and analysis. Quantitative data was complemented with a customized survey to evaluate participants satisfaction level. Results: At the follow‐up visit, a significant average reduction of 3% in abdomen/flanks circumferences and 2% in thighs contours was noticed. Abdomen/flanks and thighs subcutaneous fat layer thickness was significantly reduced by 8% and 6%, respectively. Dermal echogenicity changes, related to skin collagen content and organization, showed non‐significant increasing tendencies of 7% for abdomen/flanks and 8% for thighs. Thighs dermal thickness was significantly increased by 6%. Results were associated to a high satisfaction level (80%) and no severe adverse events. Conclusion: Zionic™ treatment is a safe, effective, and well‐tolerated noninvasive procedure for body contouring and improvement of skin properties in abdomen, flanks, and thighs. Background: The straight and thin design of the CLS Spotorno stem makes it prone for coronal plane mal-alignment, with potential for cortical impingement; reactive osteogenesis and thigh pain have been documented in this situation with some stems. The literature is scarce about the effect of distal mal-alignment with this particular stem, and its relationship with thigh pain, functional outcomes and stem survival. We assessed functional outcomes with incidence of varus/valgus CLS stem alignment, and correlated stem subsidence of these thin stems with hip scores. Methodology: Hip arthroplasty cases with CLS Spotorno stems, operated between 2015 and 2022, with a minimum follow up of 18 months were evaluated. Radiology included 100% pelvis x-rays with bilateral hips (anteroposterior with 15° internal rotation plus lateral views); parameters documented were coronal mal-alignment, stem subsidence, any pedestal formation and heterotrophic ossification (HO); special note was made of any thigh pain, and functional outcomes were recorded using mHHS and SF-36 scores. Results: We evaluated 65 patients with 79 operated hips at an average follow up of 5.26 years. 31.6% (25/79) stems were neutrally aligned, 64.6% (51/79) were in varus alignment and 3.8% (3/79) were in valgus alignment. The mean subsidence was 3.15 mm + 2.26 and 3 cases had Pedestal formation at the stem tip. There was no correlation between varus/valgus stem positions with the mHHS (p = 0.271) and SF-36 score (p = 0.553), which also did not correlate with the stem subsidence. HO formation was seen in 16 hips (20.3%), but no cases in our series needed revision during follow up. Conclusion: Significant distal malalignment with some subsidence were noted in the hips studied; despite these radiological issues, there were excellent mid-term outcomes and good stems survival, implying that distal malposition may have no bearing on functional outcomes. The accurate segmentation of individual muscles is essential for quantitative MRI analysis of thigh images. Deep learning methods have achieved state‐of‐the‐art results in segmentation, but they require large numbers of labeled data to perform well. However, labeling individual thigh muscles slice by slice for numerous volumes is a laborious and time‐consuming task, which limits the availability of annotated datasets. To address this challenge, self‐supervised learning (SSL) emerges as a promising technique to enhance model performance by pretraining the model on unlabeled data. A recent approach, called positional contrastive learning, exploits the information given by the axial position of the slices to learn features transferable on the segmentation task. The aim of this work was to propose positional contrastive SSL for the segmentation of individual thigh muscles from MRI acquisitions in a population of elderly healthy subjects and to evaluate it on different levels of limited annotated data. An unlabeled dataset of 72 T1w MRI thigh acquisitions was available for SSL pretraining, while a labeled dataset of 52 volumes was employed for the final segmentation task, split into training and test sets. The effectiveness of SSL pretraining to fine‐tune a U‐Net architecture for thigh muscle segmentation was compared with that of a randomly initialized model (RND), considering an increasing number of annotated volumes (S = 1, 2, 5, 10, 20, 30, 40). Our results demonstrated that SSL yields substantial improvements in Dice similarity coefficient (DSC) when using a very limited number of labeled volumes (e.g., for S = 1, DSC 0.631 versus 0.530 for SSL and RND, respectively). Moreover, enhancements are achievable even when utilizing the full number of labeled subjects, with DSC = 0.927 for SSL and 0.924 for RND. In conclusion, positional contrastive SSL was effective in obtaining more accurate thigh muscle segmentation, even with a very low number of labeled data, with a potential impact of speeding up the annotation process in clinics. Objective Electromyography (EMG) biofeedback has been suggested as a relatively new approach to help resolve muscle disorders. This research aimed to evaluate the influence of 8 weeks of proximal pelvic muscle training and biofeedback on pain, muscle strength, and their EMG activity in individuals suffering from patellofemoral pain syndrome (PFPS). Materials & Methods This randomized clinical trial was performed on 30 young people aged 22 to 45 with PFPS. People in the intervention group (15 people) received EMG biofeedback of the abductor (gluteus medius) and external rotator (gluteus maximus) muscles along with routine physiotherapy treatments, including stretching exercises, isometric exercises of gluteal and quadriceps muscles, and conventional transcutaneous electrical nerve stimulation. In the control group (15 people), routine physiotherapy treatments were only performed on the patients. The participants of two groups were treated for 8 weeks, 3 sessions every week. Pain by a visual analog scale (VAS), maximal voluntary isometric strength (MVIC) of the abductor and external rotator muscles of the thigh with a dynamometer, and activity of these muscles, including maximum contraction, integrated EMG and onset EMG using EMG, were measured before and after the interventions. Results MVIC and EMG of the gluteus medius muscle demonstrated that the intervention group had more maximum EMG activity (P=0.043) and MVIC (P=0.003) than the control group. MVIC and the maximum EMG activity of the gluteus maximus muscle showed no significant difference between the two groups. Still, the increase in the integrated EMG of this muscle in the intervention group indicated a greater increase than in the control group (P=0.0001). Moreover, people in the intervention group had significantly less pain than the group of routine exercises alone after 8 weeks of treatment (P=0.016). Conclusion Adding 8 week biofeedback training of abductor and external rotator muscles to the routine therapeutic exercise in PFPS can lead to pain reduction, improvement of muscle strength and better performance of these muscles. Background: The number of hip arthroscopies performed in the United States has grown significantly over the past several decades, with evolving indications and emerging techniques. Purposes: To (1) examine the evolution of hip arthroscopy at 3 tertiary referral centers between 1988 and 2022 and (2) quantify trends in patient demographics and procedures performed. Study Design: Case series; Level of evidence, 4. Methods: A retrospective analysis was performed of all patients undergoing hip arthroscopy at 3 academic centers between 1988 and 2022. Demographic data were collected using standardized forms and operative notes, and intraoperative images were manually reviewed for each patient to determine the specific procedures performed at the time of the hip arthroscopy. Surgical procedures were plotted over time to evaluate trends. Patients were divided into 3 time periods for comparison: early hip arthroscopy from 1988 to 2008, 2009 (the time of the first labral repair in our cohort) to 2015, and 2016 to 2022. Results: A total of 3000 patients (age, 35.7 ± 13.8 years; age range, 10-89 years; female sex, 2109 (70.3%); body mass index, 27.4 ± 6.3 kg/m2) underwent arthroscopic hip procedures between 1988 and 2022. The mean number of cases increased from a mean of 3.2 per year in 1988-2008 to 285.9 per year in 2016-2022 (P <.001). Labral treatment at the time of primary hip arthroscopy evolved from 100% debridement and 0% repair in 1988-2008 to 5.0% debridement, 94.0% repair, and 1.0% labral reconstruction in 2016-2022 (P <.001). Cam resection increased from 4.1% in 1988-2008 to 86.9% in 2016-2022 (P <.001). By 2022, 45 out of 325 cases (13.8%) were revisions. The rate of capsular repair at the time of primary hip arthroscopy increased from 0.0% in 1988-2008 up to 81.0% in 2016-2022. Conclusion: There has been a significant growth of hip arthroscopy volumes as well as a significant transition from use as a tool for diagnosis and labral debridement to procedures restoring native anatomy including labral repair, cam resection, capsular repair, periacetabular osteotomy, and gluteal repair. Background: Surgical treatment of patients with proximal hamstring avulsions provides good results; however, less is known about the outcome in patients who are offered conservative treatment. Purpose: To investigate the effect of surgical or conservative treatment (decided by a shared decision strategy) of proximal hamstring avulsions. Study Design: Cohort study; Level of evidence, 2. Methods: A total of 24 patients with magnetic resonance imaging–verified proximal hamstring avulsion were included and had either surgical treatment (11 patients, 45% women; mean age, 50 ± 16 years) or conservative treatment (13 patients, 46% women; mean age, 50 ± 17 years). At baseline, 6 months and 12 months, all patients answered the Perth Hamstring Assessment Tool (PHAT) (0-100 scale) and Hip Sports Activity Scale (0-8 scale). Patients had their maximal hip extension strength and maximal strength at 30° and 90° of knee flexion measured in newton meters per kilogram using a handheld dynamometer. A minimal important change in PHAT was considered >7 points and a minimal important change in strength was considered >0.15 N·m/kg, respectively. Results: The surgical group had a shorter time from injury to initiation of treatment compared with the conservative group (median: 15 vs 64 days; P =.02). The surgical group had a greater amount of retraction of the tendons compared with the conservative group (3 vs 2 cm; P =.04). From baseline to 12-month follow-up, the surgical and conservative groups improved their mean PHAT scores (35 points [95 CI, 24-45 points] and 20 points [95% CI, 9-31 points], respectively) reaching a median of 79 points (interquartile range [IQR], 66-95 points) in the surgical group and 75 points (IQR, 66-85 points) in the conservative group at the 12-month follow-up. Their Hip Sports Activity Scale levels at 12 months were 3 points (95% CI, 1-4 points) and 1 point (95% CI, 0-3 points) (not significant). Furthermore, the surgical and conservative groups improved their maximal hip extension strength by 0.61 N·m/kg (IQR, 0.42-0.80 N·m/kg) and 0.62 N·m/kg (IQR, 0.13-1.10 N·m/kg), respectively. Their maximal knee flexion strength at 30° improved by 0.52 N·m/kg (IQR, 0.29-0.74 N·m/kg) and 0.32 N·m/kg (IQR, 0.12-0.52 N·m/kg) and their maximal knee flexion strength at 90° improved by 0.28 N·m/kg (IQR, 0.19-0.37 N·m/kg) and 0.22 N·m/kg (IQR, 0.02-0.41 N·m/kg). At the 12-month follow-up, the side-to-side difference in maximal muscle strength was 6% and 7%, respectively, during hip extension and 19% to 25% and 16% to 17%, respectively, during knee flexion. Background: The sedentary aspects of many U.S. occupations and the amount of time American workers spend in the workplace make it an ideal location to implement health promotion programs. Methods: This study assessed the effectiveness of a free smartphone app with a goal-setting feature to increase physical activity (PA) and impact anthropometric, body mass (BM), and body composition (BC) changes among overweight and obese women within a community health workforce. Eighteen overweight and obese (body mass index [BMI] = 32.18 ± 4.48 kg/m2), adult (50.73 ± 8.76 years), female volunteers, tracked daily steps with a free smartphone app (StridekickTM) over an 8-week period. Pre- and post-program body composition (BC) measurements included: relative (%) body fat (BF), fat mass (FM), fat-free mass (FFM), and lean mass (LM), using dual X-ray absorptiometry (DEXA scan), and five anthropometric measurements (biceps, waist, abdomen, hips, and thigh). Findings: Pre- to post-program average daily steps resulted in significant anthropometric changes for biceps, hips, and thigh measures, with encouraging changes in FFM, LM, and relative (%) BF. The goal-setting feature of the app did not result in significant differences between the experimental and control groups. No differences were noted in FM, BMI, waist, and abdomen or step goals compared with steps completed. Conclusions: An occupational PA health promotion intervention program that tracked daily steps through the StridekickTM smartphone app resulted in anthropometric, BM, and BC changes. Introduction Aims Methods Results Conclusion Limb dominancy has been suggested, by some, to influence arterial occlusion pressure (AOP). However, we hypothesized that the differences in AOP between the dominant and nondominant legs were more likely explained by differences in cuff position.To determine the impact of limb dominance, composition, and cuff position on AOP in the context of error associated with measuring AOP twice on the same leg.Fifty‐eight adults (30 males) volunteered to have AOP measured on their dominant legs with the cuff bladder covering their inner thighs and on their nondominant legs with the bladder covering their inner and outer thighs (in random order). Thigh circumference and muscle and fat thicknesses were also measured on each leg.We found evidence for differences in AOP between legs [median δ of −0.222, 95% credible interval: (−0.429, −0.016)] when the cuff position was matched. The mean difference was −2.8 mmHg, and the 95% limit of agreement in a Bland–Altman plot was −24.8 to 19.0 mmHg. When plotting this alongside an error range (i.e., 95% limits of agreement) of taking the same measurement twice from our previous study (Spitz et al., 2020), 52 out of 58 measurements were within the error range. This difference was not due to the cuff position. Additionally, there was no evidence that thigh circumference or composition (muscle/fat thickness) moderated any difference between limbs.The difference in AOP between limbs is small and is mostly indistinguishable from the difference observed from taking the measurement twice on the same limb. The study presents a new approach for assessing plantarflexor muscles' function using a smartphone. The test involves performing repeated heel raises for 60 s while seated. The seated heel-rise test offers a simple method for assessing plantarflexor muscles' function in those with severe balance impairment who are unable to complete tests performed while standing. The study aimed to showcase how gyroscopic data from a smartphone placed on the lower limb can be used to assess the test. Eight participants performed the seated heel-rise test with each limb. Gyroscope and 2D video analysis data (60 Hz) of limb motion were used to determine the number of cycles, the average rise (T-rise), lowering (T-lower), and cycle (T-total) times. The number of cycles detected matched exactly when the gyroscope and kinematic data were compared. There was good time domain agreement between gyroscopic and video data (T-rise = 0.0005 s, T-lower = 0.0013 s, and T-total = 0.0017 s). The 95% CI limits of agreement were small (T-total −0.1118, 0.1127 s, T-lower −0.1152, 0.1179 s, and T-total −0.0763, 0.0797 s). Results indicate that a smartphone placed on the thigh can successfully assess the seated heel-rise test. Background: Accurately measuring energy expenditure during physical activity outside of the laboratory is challenging, especially on a large scale. Thigh-worn accelerometers have gained popularity due to the possibility to accurately detect physical activity types. The use of machine learning techniques for activity classification and energy expenditure prediction may improve accuracy over current methods. Here, we developed a novel composite energy expenditure estimation model by combining an activity classification model with a stride specific energy expenditure model for walking, running, and cycling. Methods: We first trained a supervised deep learning activity classification model using pooled data from available adult accelerometer datasets. The composite energy expenditure model was then developed and validated using additional data based on a sample of 69 healthy adult participants (49% female; age = 25.2 ± 5.8 years) who completed a standardised activity protocol with indirect calorimetry as the reference measure. Results: The activity classification model showed an overall accuracy of 99.7% across all five activity types during validation. The composite model for estimating energy expenditure achieved a mean absolute percentage error of 10.9%. For running, walking, and cycling, the composite model achieved a mean absolute percentage error of 6.6%, 7.9% and 16.1%, respectively. Conclusions: The integration of thigh-worn accelerometers with machine learning models provides a highly accurate method for classifying physical activity types and estimating energy expenditure. Musculoskeletal ultrasound has become a practical and accessible diagnostic tool for musculoskeletal diseases. It is used to examine joints, tendons, vessels, and nerves due to its wide availability in rheumatology practice. Ultrasound has also been applied for years in other areas such as muscular injuries in sports activities and rheumatic diseases with inflammation such as myositis. The knowledge among rheumatologists about muscle ultrasound is increasingly growing taking into account it is not the main target of their ultrasound activity but mainly based on the evaluation of joint, synovitis, tenosynovitis, vasculitis in giant cell arteritis, and parotid gland evaluation in Sjögren's syndrome. Thus, the present review describes anatomical and ultrasound findings including all muscles of the thigh (anterior, posterior, medial aspects) and leg (anterior, lateral, posterior superficial, deep posterior compartments) of lower limb structures to ease a comprehensive clinical and sonographic evaluation. Background: Patients undergoing treatment for prostate cancer may develop lymphoedema of the midline region. This has a substantial impact on a patient's quality of life and its diagnosis is often delayed or missed. Therefore, the purpose of this study is to compare the characteristics of patients with leg and midline lymphoedema to patients with only leg lymphoedema. Methods: We retrospectively collected patient-, cancer-, lymphoedema- and lymphoedema treatment-related data of 109 men with lymphoedema after treatment for prostate cancer. First, 42 characteristics were compared between both groups. Second, factors predicting presence of midline lymphoedema were explored by multivariable analyses. Results: The mean age of the patients with lymphoedema was 68 (±7) years and mean BMI is 28 (±4) kg/m2. Median duration of lymphoedema before the first consultation was 27 (9;55) months. Based on univariable analyses, patients with leg and midline lymphoedema had more frequently upper leg lymphoedema (89% (31/35) vs. 69% (51/74), p = 0.026), skin fibrosis (34% (12/35) vs. 16% (12/74), p = 0.034) and lymphatic reconstructive surgery (9% (3/35) vs. 0% (0/71), p = 0.020) than patients with only leg lymphoedema. Additionally, patients with leg and midline lymphoedema reported less frequently lower leg lymphoedema (77% (27/35) vs. 95% (70/74), p = 0.007). Based on the multivariable analysis, not having lower leg lymphoedema, skin fibrosis, performing self-bandaging and self-manual lymphatic drainage appear to be predictors for having midline lymphoedema. Conclusions: If patients with lymphoedema after prostate cancer do not have lower leg lymphoedema, have skin fibrosis, perform self-bandaging or self-manual lymphatic drainage, they possibly have midline lymphoedema. The aim of this study was to determine the effects 12 weeks Step aerobics program on anthropometric characteristics and body composition in young females. A total of 94 female participants were randomly divided in two groups. The E group subsample consisted of 44 participants (20±0.4 years), who had three Step aerobics program trainings per week and the C group subsample consisted of 50 participants (20±0.6 years), who did not take part in any organized exercise program and only conducted their regular daily activities. The following measurements were used: Body height, body weight, waist size, abdomen skinfold, back skinfold, upper arm skinfold, upper leg skinfold, body fat percentage and muscle mass percentage. Upon the Step aerobics program implementation, there was a body fat reduction by 4.9 % and waist size by 7.7 %, while the muscle mass was increased by 9.4 %. Above mentioned skinfolds showed a reduction (p>0.05) at the final measurement compared to the initial one - abdomen by 25 %, upper leg by 23.7 %, back by 15.7 % and upper arm by 3.5 %. Likewise, there was a body fat percentage reduction by 2.1 % and BMI by 6.4 %. Based on the obtained results, we can conclude that the Step aerobics training is efficient in reducing subcutaneous fat tissue and viscelar fat, as well as in increasing muscle mass. BACKGROUND: Unilateral osteoarthritis (OA) affects single knees and presents a unique scenario where individuals experience varying degrees of symptoms between their affected and unaffected knees. OBJECTIVE: This study aims to investigate differences in muscle tightness between symptomatic and asymptomatic knees in individuals with unilateral knee OA while exploring the interplay among pain, functionality, and muscle tightness. METHODS: In this cross-sectional study, thirty knee OA patients underwent assessments for hamstring (Active Knee Extension, Straight Leg Raise), iliotibial band (Ober Test), and quadriceps tightness (Modified Thomas Test). Pain intensity was measured using the Visual Analog Scale (VAS), and functional limitations were evaluated via the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). RESULTS: A negative correlation was observed between participants' pain and AKE (p = 0.004, r = - 0.515), ASLR (p = 0.27, r = - 0.403), Ober (p = 0.010, r = - 0.461) values. However, no significant correlation was found with the Modified Thomas value (p = 0.204, r = - 0.239). There was also a negative correlation between participants' WOMAC scores and AKE (p = 0.019, r = - 0.427), OBER (p = 0.004, r = - 0.510), and Modified Thomas (p = 0.022, r = - 0.416) values, while ASLR (p = 0.286, r = - 0.202) values showed no significant correlation. Comparisons between AKE, Ober, and Modified Thomas values showed higher values in asymptomatic extremities (AKE: p = 0.025, Ober: p = 0.021, Modified Thomas: p = 0.030). CONCLUSION: This study emphasizes the significance of muscle tightness in the symptomatic extremities of individuals with unilateral knee OA. The results indicate that increased muscle tightness makes pain worse and limits movement. Insects exhibit remarkable adaptability in their locomotive strategies in diverse environments, a crucial trait for foraging, survival, and predator avoidance. Microvelia americana , tiny 2–3 mm insects that adeptly walk on water surfaces, exemplify this adaptability by using the alternating tripod gait in both aquatic and terrestrial terrains. These insects commonly inhabit low-flow ponds and streams cluttered with natural debris like leaves, twigs, and duckweed. Using high-speed imaging and pose-estimation software, we analyze M. americana movement on water, sandpaper (simulating land), and varying duckweed densities (10%, 25%, and 50% coverage). Our results reveal M. americana maintain consistent joint angles and strides of their upper and hind legs across all duckweed coverages, mirroring those seen on sandpaper. Microvelia americana adjust the stride length of their middle legs based on the amount of duckweed present, decreasing with increased duckweed coverage and at 50% duckweed coverage, their middle legs' strides closely mimic their strides on sandpaper. Notably, M. americana achieve speeds up to 56 body lengths per second on the deformable surface of water, nearly double those observed on sandpaper and duckweed, which are rough, heterogeneous surfaces. This study highlights M. americana 's ecological adaptability, setting the stage for advancements in amphibious robotics that emulate their unique tripod gait for navigating complex terrains. Carbohydrates are critical for high‐intensity exercise performance. However, the effects of carbohydrate supplementation on muscle metabolism and performance during short‐duration high‐intensity intermittent exercise remain inadequately explored. Our aim was to address this aspect in a randomized, counterbalanced, double‐blinded crossover design. Eleven moderately‐to‐well‐trained males performed high‐intensity intermittent cycling receiving carbohydrate (CHO, ~55 g/h) or placebo (PLA) fluid supplementation. Three exercise periods (EX1‐EX3) were completed comprising 10 × 45 s at ~105% Wmax interspersed with 135 s rest between bouts and ~20 min between periods. Repeated sprint ability (5 × 6 s sprints with 24 s recovery) was assessed at baseline and after each period. Thigh muscle biopsies were obtained at baseline and before and after EX3 to determine whole‐muscle and fiber‐type‐specific glycogen depletion. No differences were found in muscle glycogen degradation at the whole‐muscle (p = 0.683) or fiber‐type‐specific level (p = 0.763–0.854) with similar post‐exercise whole‐muscle glycogen concentrations (146 ± 20 and 122 ± 15 mmol·kg−1 dw in CHO and PLA, respectively). Repeated sprint ability declined by ~9% after EX3 with no between‐condition differences (p = 0.971) and no overall differences in ratings of perceived exertion (p = 0.550). This was despite distinctions in blood glucose concentrations throughout exercise, reaching post‐exercise levels of 5.3 ± 0.2 and 4.1 ± 0.2 mmol·L−1 (p < 0.001) in CHO and PLA, respectively, accompanied by fivefold higher plasma insulin levels in CHO (p < 0.001). In conclusion, we observed no effects of carbohydrate ingestion on net muscle glycogen breakdown or sprint performance during short‐duration high‐intensity intermittent exercise despite elevated blood glucose and insulin levels. Relationships between force-generating capabilities and weightlifting performance (e.g., snatch, clean and jerk [C&J], and combined total) have previously been explored in adult weightlifters; however, associations in young athletes are unknown. The purpose of this study was twofold: (a) to examine the relationships between countermovement jump (CMJ) and isometric midthigh pull (IMTP) kinetics and weightlifting performance in young weightlifters and (b) to determine the proportion of weightlifting performance that could be accounted for by CMJ and IMTP kinetics using principal component analysis (PCA). Thirty-seven young weightlifters (12-18 years of age) completed 3 trials of CMJ and 2 trials of the IMTP assessments on dual force plates. Recent competition maximum loads, obtained within 2 weeks of the data collection, for the C&J and snatch were also recorded. Pearson's zero-order correlation coefficients demonstrated moderate to very high correlations (r = 0.359-0.801; all p ≤ Benjamini-Hochberg critical values [B-Hα]) between CMJ kinetic variables, including jump height, average braking force, average braking velocity, absolute and relative braking net impulse, absolute and relative force at minimum displacement, absolute and relative propulsive force, absolute and relative propulsive net impulse and average propulsive velocity, and weightlifting performance. High to very high correlations were evident between IMTP peak force and relative peak force and all weightlifting performance metrics (snatch, C&J, combined total, combined total [kg]/body mass [kg-0.67], and Sinclair's total; r = 0.538-0.796; p ≤ B-Hα). Components from the PCA were used to conduct stepwise, multiple, linear regression analyses. Moderate (45.8-52.9%) and large percentages (79.1-81.3%) of variance in absolute measures of weightlifting performance were explained by IMTP peak force variables and CMJ propulsive variables, respectively. These novel findings indicate that IMTP peak force variables and CMJ propulsive variables could provide valuable insight for talent identification and long-term athletic development in junior weightlifting pathways. This study aimed to verify the relationship between changes in thigh muscle‐localized bioelectrical impedance analysis (ML‐BIA) parameters and performance in a multiple‐set exercise. The sample consisted of 30 female university students (22.1 ± 3.2 years). The ML‐BIA parameters, including localized muscle resistance (ML‐R), reactance (ML‐Xc), and phase angle (ML‐AngF), were evaluated using a tetrapolar bioelectric impedance device operating at a frequency of 50 KHz. The multiple sets protocol was performed with an isokinetic dynamometer. For body composition, total and leg lean soft tissue (LST) were evaluated using dual X‐ray absortiometry. Student's t‐test for paired samples was used to compare the ML‐BIA parameters and thigh circumference pre and postexercise. Linear regression analysis was performed to verify the ∆ML‐PhA as a predictor of peak torque for the three sets alone while controlling for total and leg LST. There were differences in the ML‐R (∆ = 0.02 ± 1.45 Ω; p = 0.001; and E.S = 0.19), ML‐Xc (∆ = 2.90 ± 4.12 Ω; p = 0.043; and E.S = 0.36), and thigh circumference (∆ = 0.82 ± 0.60 cm; p < 0.001; and E.S = 0.16) pre‐ and post‐multiple sets. ΔML‐PhA was a predictor of performance in the first set (p = 0.002), regardless of total and leg LST. However, the ΔML‐PhA lost its explanatory power in the other sets (second and third), and the variables that best explained performance were total and leg LST. The ML‐BIA (ML‐R and ML‐Xc) parameters were sensitive and changed after the multiple sets protocol, and the ΔML‐PhA was a predictor of performance in the first set regardless of the total and leg LST. Highlights: Muscle‐localized bioimpedance analysis (ML‐BIA) is a noninvasive and inexpensive method used to assess specific muscle groups. It is already known that raw parameters, such as Xc, R, and PhA, can reflect cell hydration, volume, and membrane integrity. We demonstrate that these parameters can also reflect acute changes resulting from exercise.The muscle resistance and ML‐Xc values decreased after the exercise protocol, accompanied by increases in thigh circumference. These findings contribute to explaining the phenomenon of local muscle swelling.The ∆AngF accounted for 39%, 28%, and 24% of the variation in the first, second, and third sets of the multiple series exercise. Numerous cross‐sectional studies have attempted to identify the muscle morphology required to achieve high sprint velocity. Our longitudinal study addressed an unanswered question of cross‐sectional studies: whether hypertrophy of the individual trunk and thigh muscles induced by daily training (e.g., sprint, jump, and resistance training) is linked to an improvement in sprint performance within well‐trained sprinters. Twenty‐three collegiate male sprinters (100‐m best time of 11.36 ± 0.44 s) completed their daily training for 1 year without our intervention. Before and after the observation period, the sprint velocities at 0–100 m, 0–10 m, and 50–60 m intervals were measured using timing gates. The volumes of 14 trunk and thigh muscles were measured using magnetic resonance imaging. Muscle volumes were normalized to the participants' body mass at each time point. Sprint velocities increased at the 0–100 m (p < 0.001), 0–10 m (p = 0.019), and 50–60 m (p = 0.018) intervals after the observation period. The relative volumes of the tensor fasciae latae, sartorius, biceps femoris long head, biceps femoris short head, semitendinosus, and iliacus were increased (all p < 0.050). Among the hypertrophied muscles, only the change in the relative volume of the semitendinosus was positively correlated with the change in sprint velocity at the 50–60 m interval (p = 0.018 and ρ = 0.591). These findings suggest that semitendinosus hypertrophy seems to be associated with sprint performance improvement within well‐trained sprinters during the maximal velocity phase. We revise the systematics of the ranid frogs of the genus Hylarana occurring on Halmahera Island, Northern Moluccas of eastern Indonesia based on molecular and morphological data. Our results show that two distinct species each being nested within two distinct clades (hereafter papua clade and celebensis clade) exist on the island. One corresponds to H. moluccana (celebensis clade) and the other one to an unnamed species (papua clade) that we describe herein. The new species is genetically distinct from all congeners of the papua clade by p distances ranging from 6.9% to 11.5% on the 16S rRNA gene. Morphologically, the new species can be distinguished from all congeners by having the following combination of characteristics: A large species with adult males reaching a maximum snout-to-vent length (SVL) 67.3 mm; vomerine teeth in two oblique rows with narrow interodontophore distance; snout rounded dorsally; dorsum with few scattered cone-shaped tubercles that are black with white tips; distinct skin folds (ridges) on the dorsal side of the thigh coinciding with dark brown cross bars; a marbled pattern on the ventral side of thigh and yellowish groin. Here we provide a redescription for H. moluccana based on its lectotype from Ternate. We also provide new occurrence records for H. daemeli, H. volkerjane, and H. arfaki from the western part of mainland Papua. Furthermore, we demonstrated that H. celebensis harbors multiple mtDNA lineages suggestive of cryptic diversity within the celebensis clade. Background: Amongst primary patella tumor Campanacci grade 3 tumor of Giant cell tumor of bone (GCTB) and malignant tumors requires patellectomy. We had a patient with huge recurrent GCTB of patella with involvement of skin. We reconstructed extensor mechanism of Knee with long iliotibial band (ITB) tendon graft. Material and Methods: After patellectomy, we harvested long ITB graft (length 22 cm; width proximally 6 cm, distally 1.5 cm) through patellectomy wound and small "L" shaped incision proximally. We passed ITB graft from patella tendon and quadriceps tendon in figure of 8 manner and sutured it back to itself and host tendon with fiber Wire and closed both the wounds primarily. We started ROM and quadriceps strengthening in graded manner. We measured functional outcome with Musculoskeletal Tumour Society (MSTS) scoring system. Results: There were no post-operative complications. At 10 weeks follow up, patient had no extension lag and knee ROM was 90°. At final follow up of 7 months patient regained pre-operative functional status without knee instability. Her MSTS score was 30 and she was disease free. Conclusion: Reconstruction of extensor mechanism of knee with ITB graft doesn't have donor site morbidity and gives excellent function with graded physiotherapy protocol. [ABSTRACT FROM AUTHOR]

Transformative processes generally occur singly and jointly involving the whole body. Rarely, they appear simultaneously as distinct transformative phenomena. The case study relates to a corpse found inside a storage tank in a rather unusual position, during the winter months. On external examination carried out at the crime scene, both legs and feet were out of the well bending over the storage tank and showing skeletonisation and tissue defects due to biting inflicted by the environmental macrofauna. The thighs were also skeletonised, inside the well but not immersed in the water, as was the torso which, however, was entirely corified. The colliquated shoulders, head and upper limbs were fully immersed into the water as well as the macerated hands. The corpse was exposed simultaneously to three different environmental conditions: the external setting with changes in temperature, rainfall and the action of the macro fauna, the unventilated and humid setting inside the tank, and lastly the stored water. The corpse, lying in a specific position and being exposed to different atmospheric conditions, underwent four post-mortem changes at the same time, making it challenging to estimate the time of death, based only on the available data and macroscopic findings. Purpose: This study aims to evaluate how the structure of medical compression stockings, including three compression levels and five cross-sections from the ankle to the thigh part, will be changed after washing in different conditions and further investigate the effect of the washing parameters on the medical compression stockings. Design/methodology/approach: By washing medical compression stockings in different conditions and measuring the structures (including the density, the girth, the transversal and lengthwise dimension, the weight per unit area and the thickness) of medical compression stockings from the knee to the thigh part. Findings: It was concluded that the density, the weight per unit and the thickness increase and the girth, the transversal and lengthwise dimension, the weight per unit and the thickness decrease. The change degree of Class one and Class two is greater than Class 3. Moreover, the washing temperature is the most significant factor affecting all the structures of medical compression stockings. Meanwhile, the mechanical actions of the washing machine, like drum speed and washing time, also influence different medical compression stockings structures to different degrees. Research limitations/implications: The washing parameter not only includes the temperature and washing cycles but also has other factors, such as the drum speed and washing time. In addition, different kinds of factors will be influenced by each other. Myxoid liposarcomas are malignant soft-tissue sarcomas whose treatment represents a challenge, even for the most experienced surgeon. In this study, we report on our experience with the treatment of myxoid liposarcomas of the thigh. Our retrospective analysis included myxoid liposarcomas of the thigh treated with surgical resection between 2016 and 2022. Resection margins, complications, local recurrences, and metastases were recorded. The oncological outcome of each case was evaluated at their latest follow-up. Adjuvant therapies were administered according to the ESMO guidelines. Functionality was assessed with the MSTS score before surgery and at the patients' latest follow-up. Thirty cases (ten high-grade and 20 low-grade) were included. The mean diameter was 11.8 cm. Twenty-four cases had wide margins (80%) and six (20%) were marginal. Five cases (60% marginal) had local recurrences (17%). Marginal resection was associated with a higher risk of local recurrence (p = 0.041). Three cases with high-grade tumors (10%) developed metastases. At the patients' latest follow-up, their mean MSTS score had risen from 22.9 to 27.3. While tumor grade influences the risk of metastases, the quality of resection margins can determine the local recurrence rate. An adequate surgery can lead to good post-operative functional outcomes. The impacts of cumulative Chlorella vulgaris intake (proportion of microalga in the diet multiplied by the total feed consumed by each bird) on broiler carcass traits, meat quality and oxidative stability were reviewed to identify the optimal intake levels for maximising benefits. Our findings indicate that a cumulative intake of 8.73 g/bird significantly enhances thigh yield, while levels ranging from 8.73 to 401 g/bird optimise carcass weight and overall meat quality. However, higher cumulative levels may reduce carcass dressing percentage due to metabolic inefficiencies. Furthermore, C. vulgaris intake improves the oxidative stability of broiler meat by increasing antioxidant levels and balancing pro- and antioxidants. Including C. vulgaris in broiler diets boosts total carotenoid content, and antioxidant assays confirm that it enhances meat oxidative stability, with low to moderate cumulative intake levels (8.73 to 401 g/bird) providing the best balance of benefits. Optimal oxidative stability and antioxidant properties were observed at a cumulative intake level of 401 g/bird, showing significant improvements in meat antioxidant capacity. Higher levels may lead to diminishing returns or potential negative effects due to the digestibility issues of the microalga. Future research should refine intake models, understand the bioavailability of C. vulgaris nutrients and explore cost-effective methods to enhance its digestibility, to ensure its viability and sustainability as a feed additive. Context: Excessive hip adduction and internal rotation are abnormal movements that may lead to the onset and progression of patellofemoral pain. Previous studies have reported that lower dynamic frontal plane hip joint stiffness in the gait of women is associated with the magnitude of hip adduction and internal rotation angles. However, the factors contributing to the lack of dynamic frontal plane hip joint stiffness in the gait of young women are unclear. This study aims to investigate the factors affecting dynamic frontal plane hip joint stiffness during the weight-acceptance phase of the gait of healthy young women. Design: Cross-sectional study. Methods: This study included 30 healthy women between the ages of 18 and 30 years. The pelvic width/femur length ratio was calculated by dividing the pelvic width by the femur length. Data on hip kinematics and kinetics and activation of the gluteus maximus and medius, tensor fasciae latae, and adductor longus muscles during gait were collected using a motion capture system, force plates, and surface electromyography. Stepwise multiple regression analysis was conducted to determine the extent to which each independent factor affected dynamic frontal plane hip joint stiffness. Results: In healthy young women, decreased dynamic frontal plane hip joint stiffness was associated with decreased muscle activity of the gluteus maximus during the gait, as well as greater pelvic width/femur length ratio. Conclusions: Women with a relatively great pelvic width relative to femur length may have more difficulty in producing dynamic frontal plane hip joint stiffness. Image muscle segmentation is useful to quantitatively assess musculoskeletal diseases by extracting biomarkers such as shape, texture and water diffusivity metrics. Although volumetric manual segmentation is time consuming and a bottleneck in practice, fully automatic approaches are still in progress to reach an acceptable accuracy. In this paper, we provide a robust semi-automated tool to segment two musculoskeletal systems, i.e. thigh and shoulder in MRI and CT modalities, respectively. The tool only needs a few manually labelled cross-sections to build a directed graph-structure of corresponding points between the successive spaced slices. The boundaries of each muscle are obtained by performing a spline interpolation based on the directed graph-structure. Each muscle label and its corresponding 3D mesh are deduced using post-processing techniques. We evaluated the tool on 26 MRI thighs and 16 CT shoulders. Three metrics along with inter-muscle overlapping were employed to evaluate the tool by comparison to an expert manual segmentation and a publicly available tools (ITK-SNAP, 3D Slicer). The results showed a mean Dice $0.988 \pm 0.003$ 0.988 ± 0.003 , and Hausdorff Distance $4.86 \pm 1.67$ 4.86 ± 1.67 mm in comparison to the manual reference for thigh muscle segmentation, and a mean Dice $0.961 \pm 0.005$ 0.961 ± 0.005 and Hausdorff Distance $2.42 \pm 0.79$ 2.42 ± 0.79 mm for shoulder muscle segmentation, outperformed the other methods. Case: A 22-year-old man with a type IIIA open AO/OTA 61C2.2b pelvis fracture and hypotension received exploratory laparotomy, temporary open ligation of the bilateral internal iliac arteries, and retroperitoneal packing. After prompt fracture debridement, a pelvic binder was positioned over the thighs as a reduction aid and maintained for six hours during pelvis open reduction internal fixation. Bilateral anterior thigh compartment syndrome was diagnosed three hours after packing and binder removal. Conclusion: Prolonged application of a pelvic binder to the thighs as an intraoperative reduction tool, shortly after temporary internal iliac artery ligation, may be associated with reperfusion injury and thigh compartment syndrome. Hibernoma is a relatively uncommon benign neoplasm of brown adipose tissue which usually affects head and neck region, thigh and rarely breast. There are different subtypes of hibernoma with the spindle cell-type is the least common one. Herein, we are reporting a case of this rare spindle cell hibernoma in a location which has not been previously reported: perinephric adipose tissue.
