北海道文教大学人間科学部看護学科老年看護学領域は，2018 年度に恵庭市地域と連携をとり，学生の認知症高齢者の理解を深めるために，認知症高齢者とかかわることができる企画を考案し実施した．本資料では「RUN 伴in 北海道文教大学」「認知症カフェin オープンキャンパス」「看護師国家試験応援出張認知症カフェ」の実践報告をする．結果，学生は実際に認知症高齢者とかかわり共に過ごすことで，認知症高齢者をイメージすることができた．また，認知症高齢者にも笑顔がみられ，脳への刺激となったといえる．今後は，グループホーム（以下：GH）に出向いて認知症高齢者の方とコミュニケーションをとり，どのように生活をしているのか学習する実習を考えていきたい．さらに，学生プロジェクトを立ち上げ，学生が地域に出ていき認知症の理解を深めることができるような，企画にも発展させたいと考える．

本研究の目的は「認知症高齢者の気になる行動」を中心に、生活者としての認知症高齢者と家族の抱える問題について分析し、認知症高齢者と介護者の生活支援の方法を検討することである。インタビューから得られたデータを抜粋し、KJ法により分類した結果、認知症高齢者の気になる行動は「もの忘れ」「怒りっぽい」「不安になる」「徘徊・うろうろする」「痛みがある・病気・身体の不調」「状況がわからない」「排泄の問題」「問題行動」といった8つのカテゴリーに分類された。生活上の最も困難な事象は認知症高齢者の問題行動であると考えられる。「徘徊・うろうろする」「認知症高齢者の不安から生じるさまざまな気になる行動」などは、いわゆるノーマルなものではなく、それなりの生活問題として重要であるが、最も困難な事象は弄便（ろうべん）であると考えられた。これらの問題行動は認知症という疾患が原因となるものであり、疾患を理解することで介護者の精神的な負担は軽減することが先行研究や本研究からも推察される。介護者は認知症高齢者の奇異な行動にも意味があることの理解をすることが、介護者の精神的な介護負担軽減に有用であると考えられた。これらの結果より、介護者への認知症高齢者の疾患の理解を促す支援が重要であることが示唆された。

This study investigates the effects of the rehabilitations that would promote the Quality of Life in the elderly with dementia. The salivary amylase activity and communications activity indicated that music therapy significantly diminished the stress level and increased verbal communication of the participant. Although dementia had kept developing, she seemed to enjoy therapy sessions. The results suggested that music therapy could decrease the stress and promote the Quality of Life in the elderly with dementia.

本研究の目的は,認知症高齢者の自発性を引き出す集団音楽療法における介入スキルについて明確化することである.認知症高齢者5人に対して8回の集団音楽療法を実施し,セラピストによる介入を帰納的に分析した結果,以下のことが示された.1)セッション中にみられた認知症高齢者の自発性の変化として,「覚醒時間が増加する」「演奏の始まりがスムーズになる」「演奏中のテンポやリズムの乱れを修正していく」「演奏後に満足感を表出する」「場を創造していく」の5つが見出された.2)セラピストが用いていた介入タイプは,「音楽への同調を促す」「注意・関心をひきつける」「巻き込む」「ほめる・支持する」「場の発展・拡張を期待して仕掛ける」の5つに分類された.3)認知症高齢者の自発性を引き出す集団音楽療法における介入スキルには,セラピストが楽曲ごとに(1)演奏に向かうまで,(2)演奏の始まり,(3)演奏中,(4)演奏の終わり,(5)演奏直後の5つの時点において5つの介入タイプを使い分け,参加者が音楽に同調していこうとする自発性を助けること,参加者自身が自分の演奏に満足し自尊心や自信が回復していく体験を積み重ねることで,自ら場を創造していく力を発揮できる機会を仕掛けていくことが見いだされた., The purpose of this study was to clarify therapeutic intervention skills for eliciting spontaneity of the elderly with dementia in a group of music therapy session. Subjects were five elderly persons with dementia guided by one music therapist. Group music therapy sessions focused on rhythm synchronization were held once a week during 8 weeks. The intervention types for eliciting the spontaneity of the elderly with dementia were extracted by the inductive method form description of spontaneous responses of the elderly and the therapist's intervention. The following results were obtained. 1) The change of spontaneous responses among the elderly with dementia in the group music therapy session included (1) increasing time of awaking, (2) smooth participation to singing songs and playing instruments, (3) correcting the tempo of singing and playing, (4) expressing the gratification of playing music, and (5) creating a scene spontaneously in the session. 2) The intervention that the therapist used included (1) inducing synchronization to the rhythm and melody, (2) attracting to pay attention and interest, (3) involvement, (4) admiration and supporting, and (5) prompt to anticipation of evolvement in the group. 3) It is suggested that a music therapist should have a skill to select appropriate intervention for aiming 5 different timings of intervention when a therapist plays a tune, which is (1) before the execution, (2) at the beginning of the execution, (3) in the middle of the execution, (4) at the end of execution, (5) after the execution. It is also found that a skill to organize the combination of 5 types of interventions while focusing on these 5 points is important for a music therapist working for the elderly with dementia.

本研究では、介護満足感やその他関連すると思われる要因と在宅で介護する家族の介護継続意向の関連を調査し、その結果を考慮した支援内容を検討することを目的とした。対象は、A県内で認知症高齢者を介護する家族介護者で、通所介護、通所リハビリテーションおよび重度認知症デイケアを利用し協力の得られた320人である。介護継続意向と関連のあると思われる要因を検討した。その結果、家族介護者の介護継続意向に有意な関連があった要因は、介護満足感、認知症高齢者との関係性、行動・心理症状の有無、家族外支援者の有無であった。在宅での家族介護者の介護継続を支援するためには、介護負担度を軽減する働きかけだけでなく、介護満足感を高める支援方法を実施することで介護継続意向を高める可能性が考えられる。また、在宅での介護支援を進めるためには、家族介護者のおかれている在宅介護時期を考慮する必要性も示唆された。(著者抄録), This study aims to discuss support services, based on the result of examining the relationships between care-giving satisfaction and related factors, and care continuity intention of family caregivers. The study was performed with 320 family caregivers of the elderly with dementia who utilized daycare or rehabilitation services for the elderly with severe dementia in A Prefecture, Japan. As a result, the factors significantly related to care continuity intention of family caregivers are care-giving satisfaction, relationships with the elderly, and presence of behavioral disorder and supporters outside the family. The result suggested that the care continuity of the family could be enhanced not only by the reduction of the caregivers' burden but also by the promotion of caregivers' satisfaction. 回想法で用いるプロンプトが認知症高齢者におよぼす影響を分析した.グループ回想法34回の場面の参加者のプロンプトへの反応記録169シートの中から,特徴的な発言を取りだし,思考活動の高まりと対人交流の視点で分析した.音の刺激をもたらすプロンプトへの反応は,音を聞くことでさまざまな発言が得られた.食べ物のプロンプトへの反応は,食材を触って,その場で調理することで,日頃,食欲のあまりない参加者もじっくり味わうことを楽しんだ.ものを作る工程を回想するプロンプトへの反応は,梅干し作り,草鞋作りの工程を回想法の中で参加者が共に手を使って実体験し,そのもの作りにまつわる歌を口ずさむ参加者につられて,発言の少ない参加者も歌い始めることが何度もあった.回想法開始時にMMSEが15点以上の参加者はプロンプトを手がかりに自己の体験を話すなど,広がりのある回想が聞かれた.MMSEが14点以下の参加者は視覚情報だけでは回想にまつわる発言は少なかった.ただし,音を楽しむ,食べ物を味わう,香りを楽しむなど,五感刺激を手がかりとしたプロンプトへの感想を聞くことはできた.(著者抄録)

本研究の目的は認知症高齢者をケアする上で,看護師がどのような感情を抱きながら関わっているのかを明らかにし,サポートの視点を見出すことである。介護老人保健施設,老人認知症専門病棟,特別養護老人ホームで重度の認知症高齢者をケアする看護師9名を対象に,研究の同意を得た上で,半構成質問紙を用いた面接調査を行った。その結果看護師の感情として,1)「なんでそんなことするの」という怒り,2)認知症高齢者の言動への困惑,3)言動に対する対応への困惑,4)身体症状の判断への自信のなさ,5)自分のケアを評価できない不安,6)自分の思うケアができないジレンマ,7)責任の重さへの不安,8)ケアへの達成感のなさの8つが明らかになった。, The aim of this paper is to report the results of a qualitative study which explored the emotions experienced by nurses caring for the elderly with dementia with a view to providing proper support for such nurses. Data were collected using semi-structured interviews from nine nurses working in Health Service Facilities for Elderly, Dementia Units and Nursing Homes. The nurses' emotions caring for the elderly with dementia were as follows: 1) Anger toward the patients' behaviors, 2) Being at a loss how to cope with the behavior of the dementia patients, 3) Being puzzled over ways to deal with the behavior of the patients, 4) Lack of confidence in judging the symptoms of the patients, 5) Inability to evaluate their own care, 6) Dilemma about providing proper care, 7) Worries about too much responsibility, 8) Lack of sense of achievment.

認知症高齢者に尊厳ある個別看護を提供するには,高齢者の人生暦を理解することが重要である。認知症高齢者の人生暦に対する看護師の認識が所属施設により相違があるかを明らかにするために,近畿地方の介護老人保健施設100施設,介護老人福祉施設200施設,グループホーム125施設(1施設につき1～3名)の看護師を対象に,独自に作成した質問紙を用いた構成的質問紙調査を行った。調査内容は「人生暦を理解する意義」「看護における人生暦の有用性」「看護における人生暦の必要性」「人生暦と情報収集の関係」である。その結果,看護師169名から回答が得られ,認知症高齢者の人生暦に対する看護師の認識は高く施設間に差がないことが明らかになった。また,人生暦が理解できる情報収集用紙の必要性,記録する必要性,家族と共有する必要性のいずれにおいても8割以上が肯定しており,施設間の差は認められなかった。, Understanding the life history of the elderly with dementia is highly needed to provide them with appropriate individual nursing care with respect to their dignity. In order to clarify the difference in recognition of the life history of the elderly with dementia by the nurses working in different setting, a structural questionnaire survey was conducted by using the original form of questions to the subject nurses working at 100 long term care health facilities for the elderly, 200 long term care welfare facilities for the elderly and 125 group homes (1～3 nurses in each facility). Contents of the survey are "the significance of understanding the life history", "the effect of life history in nursing", "the necessity of life history in nursing", and "the relation between life history and information gathering". As the result, answers were collected from 169 nurses and it was found that the recognition of the importance of understanding the life history of the elderly with dementia by the nurses was quite high with not much difference among care providing facilities. Also over 80% of the nurses gave affirmative answers to the necessity of information gathering form to get knowledge of life history, the necessity to keep record and to be shared with the family, and there was not significant difference among the facilities.

痴呆対応型共同生活介護事業(以下、グループホームとする)は、介護保険法施行後急速に増加している.グループホームの利用者は、介護保険施行当初は軽度から中等度の痴呆性高齢者が中心と考えられてきたが、年数を重ねるにつれて痴呆が進行し、重度化する状況にある.そのため、グループホームにおけるターミナルケアについて考えていくことが重要となっている.そこで岩手県におけるグループホームでのターミナルケアの取り組みについて現状を把握し、課題を明らかにすることを目的として調査を行った.調査協力の得られた23ホームのホーム長に対して半構成的面接法による調査を行った結果、グループホームでターミナルケアを行うにあたって、以下の4つの課題が見出された.1.スタッフの質的問題 2.医療面での問題 3.建物の構造上の問題 4.家族の理解の問題また、グループホームにおけるターミナルケアの必然性について、議論が不足し論点が整理されないままの現状においては、今回の調査で見出された課題への対応を含め、ホーム利用者のターミナル期について、個々のグループホームにおける考えにより決定されていた.グループホームにおけるターミナルケアについて、グループホーム全体でもっと真剣に論議され、全体的な合意がなされるような方策が早急にとられなければならないことが示唆された., The number of group homes for dementia patients has increased rapidly after the enactment of the nursing-care insurance law. At first, most group home users were elderly patients with mild to moderate dementia. However, over time, the severity of dementia in group home users has increased. Terminal care in group homes is thus starting to become a relevant issue. The present study was conducted to ascertain the current state of terminal care in group homes in Iwate Prefecture, and to clarify problems associated with terminal care in group homes. Semi-structured interviews were held at 23 group homes, and the following 4 problems with terminal care in group homes were identified : 1. Problems with substance of care staff 2. Problems with medical care 3. Problems with building structures 4. Problems with family understanding Currently, little discussion about the necessity of terminal care in group homes has been initiated, and related issues have not been identified and organized. As a result, terminal care for group home users, including countermeasures for problems identified by the present study, is determined based on the individual approaches of each group home. Group homes need to actively initiate wider discussion of terminal care and promptly establish consistent and uniform measures that can be adopted by all group homes.

Data were collected using semi-structured interviews with eight third-year students of Osaka Prefecture College of Nursing. The results indicated six categories in the process of understanding of the elderly with dementia as follows: 1) Mental preparation for care; 2) Rejection, 3) Being at a loss how to cope with the behavior of dementia, and 4) Groping for ways to deal with the behavior of dementia (appearing in Week 1); 5) Getting a feel for ways to cope (appearing in Week 2) ; 6) Understanding of care effectiveness (appearing in Week 3). It is one of the important problems of the elderly with dementia that consolidate living environment. We need to clarity the principle of housing in order to realize that family take care of the elderly with dementia in the home. As a foundation study, this study aims to understand the life style of the elderly with dementia and their family. The action of the elderly with dementia at home is large different from former action when they had the symptom of dementia. Especially, vascular dementia is taking a passive action because of a paralysis and intellectual disorder. Therefore, the bedroom of the elderly is moved near the living room. On the other hand, Alzheimer disease is taking a passive action as vascular dementia, and often they move around in the home.
