Telangiectatic osteosarcoma is a rare variant of osteosarcoma and can be easily misdiagnosed as aneurysmal bone cyst. We report an atypical case of iliac telangiectatic osteosarcoma in a young healthy female, who presents with painful slow growing expansile lytic septate lesion in the left hemipelvis, which is initially treated as aneurysmal bone cyst. The diagnosis of aneurysmal bone cyst is made after histopathological examination of core needle biopsy. Her condition became unstable and massive bleeding is noted at the lesion site after sclerosant injection. She undergoes emergency hemipelvectomy and eventually the biopsy turns up to be telangiectatic osteosarcoma. Our case highlights that core needle biopsy is not useful in making diagnosis for iliac telangiectatic carcinoma. Hence, an open biopsy should be carried out in our case. This case also emphasises on careful evaluation for malignancy which is mandatory because bleeding from pelvis after an unsuitable treatment can be grave, to the extent that major amputation hemipelvectomy is an option. Even though telangiectatic osteosarcoma has the same prognosis and treatment with conventional osteosarcoma, the outcome of delayed treatment for telangiectatic osteosarcoma is not good due to the dilemma in establishing an early correct diagnosis.

Herein, we present the case of a 35-year-old man with a previous history of osteosarcoma of his left tibia followed by multiple episodes of bilateral pulmonary metastases. After being clinically free of disease for seven years, the patient developed a 23 cm mass involving his small bowel wall. To our knowledge, this is the largest reported case of metastatic osteosarcoma involving the small intestine. The metastasis was that of a conventional high-grade osteosarcoma with telangiectatic features. The unusual presentation of this metastasis could be secondary to increased long-term survival of patients with osteosarcoma following more advanced modern therapy.

Telangiectatic osteosarcoma (TO) is an uncommon variant of osteosarcoma that primarily afflicts young adults. In this case report, we describe a unique instance of TO occurring in a young child's maxilla. Under microscopic examination, it reveals abundant blood-filled spaces, extensive hemorrhagic regions, alongside atypical pleomorphic tumor cells and osteoid. Purpose To describe MRI features of multiple Focal Nodular Hyperplasia (FNHs). Methods 40 consecutive subjects (37 females, mean age, 38.8 years) were included. All studies were independently reviewed. This was an observational study to define the radiological features of multifocal FNH. Results 130 lesions were evaluated. The majority (88.5%), were peripheral in location. 92.3% lesions were lobulated. Marked enhancement was present in 94.6% lesions. In the portal venous and delayed phase, 46.2% and 47.7% lesions were mildly hyperintense. Central scar was present in 77% lesions. Conclusions Distinctive features included predominant subcapsular location and mild hyperintensity in the delayed phase, seen in nearly 50% of FNHs. [ABSTRACT FROM AUTHOR]

Objectives: To identify, describe, and investigate the clinical, radiologic, and pathologic features of 8 cases of telangiectatic oncocytoma. Methods: Fifty-three consecutive renal oncocytomas were reviewed for the telangiectatic pathologic features that were subsequently correlated with the demographic, clinical, and radiographic findings. Results: Telangiectatic oncocytoma accounted for 15% of the 53 renal oncocytomas collected in the past 7 years in our institution. On radiology, almost all presented as an enhancing mass and were suspicious for or consistent with a renal malignant tumor. Grossly, the tumors ranged from 2.4 to 6.0 cm (mean, 3.5 cm) and macroscopically were hemorrhagic spongy or multicystic masses without a central stellate scar. Microscopically, they were characterized by variably sized blood-distended spaces (<0.1-mm to 2- to 3-mm blood lakes) lined by typical oncocytoma cells and without evidence of degenerative changes. Conclusions: With its unique radiologic and pathologic presentations in comparison with classic renal oncocytoma, it is important to recognize this new variant of renal oncocytoma. Telangiectatic granuloma is primarily a reactive tumor like overgrowth in oral cavity due to irritation, physical trauma or hormonal factors. It predominantly occurs on maxillary facial gingiva and mainly in anterior region, in the second decade of life of young females. Gingiva is the most common site of occurrence accounting about 75% of all cases due to presence of chronic low grade irritation caused by calculus within the gingival crevice. In this report we present a case of telangiectatic granuloma in floor of the mouth which is relatively unusual site of occurrence. 30-year-old female patient had noticed a growth in mouth for 3 months. The surgically removed mass was histologically evaluated. It was composed of parakeratinized stratified squamous epithelium and underlying connective tissue stroma with numerous dilated and engorged blood filled capillaries lined by plump, proliferating endothelial cells. Telangiectatic osteosarcoma (TOS), an uncommon variant of osteosarcoma, involving skull bones is extremely rare. We present clinico-pathological, imageological and treatment outcome of a primary TOS of occipital bone in a 30-year-old woman and review the previously reported skull bone TOS. We suggest that TOS should be included in the differential diagnosis of destructive lytic lesions involving the skull bones. As radical surgical procedures are not applicable to skull bones, the outcome is poor even with adjuvant chemotherapy. [ABSTRACT FROM AUTHOR]

Reticulated telangiectatic erythema is a rare entity; it has been reported to occur following the placement of implanted cardiac devices and drug delivery systems. Histologically, reticulated telangiectatic erythema of the pacemaker is characterized by slight spongiosis and increased dermal telangiectasias. We describe a patient that developed reticulated telangiectatic nonpruritic patches on the left chest after the placement of a pacemaker. Background: Parosteal osteosarcoma (POS) is an uncommon, low-grade malignancy that carries a 15% risk of dedifferentiation. MDM2 gene amplification has been described in more than 85% of POS, as well as most intramedullary low-grade central osteosarcomas. The amplification might also be retained in the dedifferentiated component of POS. Results: We describe a 51-year-old woman who presented with a slow growing mass in the distal right femur. Radiographs demonstrated a parosteal heavily mineralized lesion with expansile lytic intraosseous component. MRI showed multiple fluid/fluid levels in the metaphysis and cortical breaching posterolaterally. Core biopsy was obtained from the parosteal component and microscopic sections demonstrated fascicles of bland spindle cells infiltrating skeletal muscle with no bone component seen. In spite of the bland morphologic features, additional molecular study was helpful to make a diagnosis of POS in the context of the radiographic presentation, with MDM2 gene amplification demonstrated by fluorescent in situ hybridization (FISH). Microscopic sections of the consequent resection specimen demonstrated features of POS within the juxtacortical component and telangiectatic osteosarcoma within the dedifferentiated intraosseous component. Conclusion: POS is a rare, low-grade malignant tumour that is characterized by the presence of ring chromosomes and MDM2 gene amplification. Due to bland morphology on core biopsy, microscopic findings can mimic benign lesions. Therefore, the MDM2 gene amplification is a useful diagnostic marker in differentiating POS from benign lesions. The MDM2 gene acts as an oncogene by inhibiting the cell cycle regulator p53. Targeting MDM2 and restoring p53 activity is a potential pharmaceutical strategy. [ABSTRACT FROM AUTHOR]

Background: Reticular telangiectatic erythema is a benign cutaneous reaction that may occur in patients who have received a subcutaneous implantable cardioverter-defibrillator. Reticular telangiectatic erythema is characterized by asymptomatic telangiectasias, blanchable erythematous patches, or both overlying and/or adjacent to the subcutaneous implantable cardioverter-defibrillator., Purpose: We describe a man who developed reticular telangiectatic erythema after receiving a subcutaneous implantable cardioverter-defibrillator and review the salient features of this condition. We also summarize the conditions that can mimic reticular telangiectatic erythema., Materials and Methods: The features of a man with reticular telangiectatic erythema are presented and the literature on reticular telangiectatic erythema is reviewed., Results: Our patient developed reticular telangiectatic erythema within one month of subcutaneous implantable cardioverter-defibrillator insertion. The subcutaneous manifestations were asymptomatic. The patient concurred to have periodic clinical follow up and his condition will be monitored for any changes., Conclusion: Reticular telangiectatic erythema is a benign condition characterized by the development of erythema, telangiectasia, or both following insertion of a subcutaneous implantable cardioverter-defibrillator. Other subcutaneous implantable cardioverter-defibrillator-related side effects, such as pressure dermatitis and contact dermatitis, can mimic the condition. Reticular telangiectatic erythema can also be observed following insertion of other devices or, rarely, in the absence of inserted devices. Local microcirculatory changes and subcutaneous implantable cardioverter-defibrillator-related obstruction of blood flow have been suggested as possible mechanisms of pathogenesis. The diagnosis can usually be established by clinical presentation. Therefore, patch testing can usually be omitted. Reticular telangiectatic erythema is typically asymptomatic and thus removal of the device is not required.

Background: Although microcalcifications of hydroxyapatite can be found in both benign and malignant osteotropic tumors, they are mostly seen in proliferative lesions, including carcinoma. The aim of this present study is to develop a molecular imaging contrast agent for selective identification of hydroxyapatite calcification in human osteotropic tumor tissues ex vivo and in human osteosarcoma cells in vitro., Methods: A bioinspired biomarker, hydroxyapatite binding peptide (HABP), was designed to mimic natural protein osteocalcin property in vivo. A fluorescein isothiocyanate dye conjugated HABP (HABP-19) was utilized to characterize hydroxyapatite on human osteotropic tumor tissue sections ex vivo and to selectively image hydroxyapatite calcifications in human osteosarcoma cells in vitro., Results: Using a HABP-19 molecular imaging probe, we have shown that it is possible to selectively image hydroxyapatite calcifications in osteotropic cancers ex vivo and in human SaOS-2 osteosarcoma cells in vitro., Conclusion: Hydroxyapatite calcifications were selectively detected in osteotropic tissues ex vivo and in the early stage of the calcification process of SaOS-2 human osteosarcoma in vitro using our HABP-19 molecular imaging probe.
