Bolus is commonly used to improve dose distributions in radiotherapy in particular if dose to skin must be optimised such as in breast or head and neck cancer. We are documenting four years of experience with 3D printed bolus at a large cancer centre. In addition to this we review the quality assurance (QA) program developed to support it. More than 2000 boluses were produced between Nov 2018 and Feb 2023 using fused deposition modelling (FDM) printing with polylactic acid (PLA) on up to five Raise 3D printers. Bolus is designed in the radiotherapy treatment planning system (Varian Eclipse), exported to an STL file followed by pre-processing. After checking each bolus with CT scanning initially we now produce standard quality control (QC) wedges every month and whenever a major change in printing processes occurs. A database records every bolus printed and manufacturing details. It takes about 3 days from designing the bolus in the planning system to delivering it to treatment. A 'premium' PLA material (Spidermaker) was found to be best in terms of homogeneity and CT number consistency (80 HU +/- 8HU). Most boluses were produced for photon beams (93.6%) with the rest used for electrons. We process about 120 kg of PLA per year with a typical bolus weighing less than 500 g and the majority of boluses 5 mm thick. Print times are proportional to bolus weight with about 24 h required for 500 g material deposited. 3D printing using FDM produces smooth and reproducible boluses. Background and purpose: This work reports on the results of a survey performed on the use of computed tomography (CT) imaging for motion management, surface guidance devices, and their quality assurance (QA). Additionally, it details the collected user insights regarding professional needs in CT for radiotherapy. The purpose of the survey is to understand current practice, professional needs and future directions in the field of fan-beam CT in radiation therapy (RT). Materials and methods: An online institutional survey was conducted between 1-Sep-2022 and 10-Oct-2022 among medical physics experts at Belgian and Dutch radiotherapy institutions, to assess the current status, challenges, and future directions of motion management and surface image-guided radiotherapy. The survey consisted of a maximum of 143 questions, with the exact number depending on participants’ responses. Results: The response rate was 66 % (31/47). Respiratory management was reported as standard practice in all but one institution; surface imaging during CT-simulation was reported in ten institutions. QA procedures are applied with varying frequencies and methodologies, primarily with commercial anatomy-like phantoms. Surface guidance users report employing commercial static and dynamic phantoms. Four main subjects are considered clinically important by the respondents: surface guidance, CT protocol optimisation, implementing gated imaging (4DCT, breath-hold), and a tattoo-less workflow. Conclusions: The survey highlights the scattered pattern of QA procedures for respiratory motion management, indicating the need for well-defined, unambiguous, and practicable guidelines. Surface guidance is considered one of the most important techniques that should be implemented in the clinical radiotherapy simulation workflow.

Purpose: Intrafractional motion constitutes a significant challenge in SBRT (Stereotactic Body Radiotherapy).The breath-hold (BH) technique is employed to mitigate tumor motion; however, ensuring reproducibility and consistency remains critically important. Surface tracking systems, integrated into the treatment process, facilitate motion tracking through three-dimensional camera technology. Surface guidance has been incorporated with Varian EDGE (Varian Medical Systems, Palo Alto, CA, USA) and has been utilized at multiple treatment sites within our department since 2018. Drawing on four years of experience, this study aims to publish patient experience, assess the feasibility, and evaluate the tolerability of breath-hold during SBRT with surface guided radiotherapy (SGRT), particularly focusing on a specific subgroup: patients with liver metastases. Methods: Prospective evaluation was conducted on patients with liver metastases undergoing breath-hold SBRT with SGRT. A two-step survey consisting of seven questions was administered after CT simulation and treatment. Treatment duration and the number of breath-holds were recorded. Additionally, factors potentially influencing SGRT and treatment time were assessed. Results: Between April 2021 and May 2022, a total of 41 patients underwent 171 fractions of treatment. According to the questionnaire, prior training was found to be beneficial, and breath-holding during the procedure was tolerable. Patients reported experiencing slight stress due to their active participation in the treatment. Factors such as Karnofsky Performance Status (KPS), age, lung volume, conditions affecting lung capacity, previous breath-hold history, and being a native speaker showed no correlation with treatment time. Moreover, these factors did not correlate with the tolerability of breath-hold during SGRT. However, female patients showed better breath-holding performance in SGRT treatments compared to male patients (p: 0.02). Conclusions: The application of breath-hold with SGRT procedures is tolerable and feasible in liver SBRT treatments. There exists no specific subgroup that cannot tolerate this method.

Surface-guided radiation therapy (SGRT) has gained wide popularity across radiation oncology community due to its non-radiographic characteristic and real-time motion monitoring capability. Nevertheless, it has not yet gained its full potential in routine clinical practice. Implementing SGRT system into the clinical practice requires not only the definition of steps in clinical workflow, but also establishment of the comprehensive quality assurance (QA) program including commissioning, acceptance and periodic QA test to facilitate a safe, and efficient use of SGRT system in clinical settings. This review focuses on the latest recommendation of American Association of Physicists in Medicine and European Society for Radiotherapy and Oncology guidelines about the implementation of comprehensive QA program for SGRT system. Introduction: Surface guidance (SG) radiotherapy (RT) is now used by many radiotherapy departments globally and has expanded in popularity over the last number of years. A number of commercial systems are available. SG has routinely been used and is well established for cranial stereotactic radiosurgery (SRS) patient set ups and intra-fraction motion monitoring.However, data is limited in relation to its clinical use for extracranial stereotactic body radiotherapy (SBRT), particularly for targets which are impacted by respiratory motion such as the lung and liver. Objective & Information Source: A review of available literature was carried out on 24th October 2021 to assess the clinical feasibility and use of SG in SBRT via PubMed. Methods: Eligibility CriteriaThe search criteria involved identifying articles where SG is used in extracranial SBRT.Risk of BiasTo eliminate the risk of bias, any particular commercial system was not the focus of the review and not included in the search criteria. Numerous clinical terms for similar things were used to reduce the risk of missing papers e.g. SBRT and SABR.Search CriteriaThe PRISMA checklist was used. Searching for “surface guidance and radiotherapy” yielded 3271 results, where as “SGRT” alone returned 72 results, when the search term was narrowed down using different iterations of SG and SBRT, only 6 results were available. Of these, 4 had reviewed clinical data in relation to SG and SBRT for patient set up and intra-fraction motion monitoring. Results: The 4 studies indicate positive results for using SG with sufficient image guidance (IG) for both patient set up and intra-fraction monitoring during SBRT. This was observed both in free breathing and in patients with respiratory motion management being employed such as deep inspiration breath-hold (DIBH) techniques. All used multiple IGRT solutions to verify localisation pre-treatment in conjunction with SG.LimitationsThe number of studies available which report using SG in SBRT is extremely limited. All centres had also installed SG systems therefore this could result in an unconditional bias in using the system positively. Conclusion: SG can be used for SBRT set-ups and intra-fraction motion monitoring once sufficient IG is used to verify target localisation for treatment.

• Setup of right sided breast cancer patients using surface guidance yields higher accuracy than setup using skin marks/tattoos and lasers. • There is no difference in setup time between a surface guided procedure and a skin marker/tattoo based procedure. • CBCT should still be used for setup verification. To prospectively investigate whether surface guided setup of right sided breast cancer patients can increase efficiency and accuracy compared to traditional skin marker/tattoo based setup. Twenty-five patients were included in this study. Each patient was positioned using skin marks and tattoos (procedure A) for half of the fractions and surface guidance using AlignRT (procedure B) for the other half of the fractions. The order of the two procedures was randomized. Pretreatment CBCT was acquired at every fraction for both setup procedures. A total of ten time points were recorded during every treatment session. Applied couch shifts after CBCT match were recorded and used for potential error calculations if no CBCT had been used. In the vertical direction procedure B showed significant smaller population based systematic (Ʃ) and random (σ) errors. However, a significant larger systematic error on the individual patient level (M) was also shown. This was found to be due to patient relaxation between setup and CBCT matching. Procedure B also showed a significant smaller random error in the lateral direction, while no significant differences were seen in the longitudinal direction. No significant difference in setup time was found between the two procedures. Setup of right sided breast cancer patients using surface guidance yields higher accuracy than setup using skin marks/tattoos and lasers with the same setup time. Patient alignment for this patient group can safely be done without the use of permanent tattoos and skin marks when utilizing surface-guided patient positioning. • Swallow motion may compromise the successful delivery of glottic larynx SBRT. • We developed a motion management workflow using surface image guidance. • It was found that swallow motion was common during treatment. • Our method was feasible and straightforward to implement in clinic. Involuntary motion due to swallowing cause inaccurate dose delivery during larynx radiotherapy, a deviation that may be particularly problematic during stereotactic body radiation therapy (SBRT). The goal of this study was to develop a motion management solution for larynx SBRT using surface imaging. Ten patients were recently treated on a phase II study of larynx SBRT on a LINAC equipped with a surface guidance system. A small region of the immobilization mask was manually cut open to allow surface tracking. Pre-treatment and intra-fractional CBCTs were acquired to verify internal anatomy. Patients were verbally instructed not to swallow during treatment. During treatment delivery, beam hold was initiated by the Motion Management Interface if surface motion exceeded a patient-specific threshold. Patient motion was recorded in log files and analyzed. We also performed phantom studies to assess the theoretical impact of gating on dose delivery. The frequency (6.5 ± 5.2 times per fraction) and duration (3.9 ± 2.5 seconds per swallow) of swallowing varied both between patients and fractions. The magnitude of each swallow showed mean peak amplitude at 5.8 ± 3.8 mm above baseline, mostly in the longitudinal direction. Beam duty cycle was 95.0% ± 7.0% (absolute range: 76–100%), with inefficiency most prominent in the early fractions. The 95th percentile residual motion was reduced from 3.4 mm to 2.3 mm with both verbal instruction and gating. Phantom studies confirmed dose delivery accuracy represented by gamma pass rate was improved by 5% using this approach. Laryngeal motion management using surface imaging is feasible and efficacious. Uncontrolled movement of the larynx was not uncommon during treatment, with gating reducing potential for unplanned dose deviations. • AlingRT InBore™ is a novel SGRT solution for bore-based Linacs. • Cameras caused no adverse effects on beam delivery or image quality. • Sub-millimetric accuracy is achieved for inter and intrafraction motion management. • Systems' stability and performance are not affected by Halcyon™ rotations. • AlignRT InBore™ shall allow contactless DIBH treatments on Halcyon™. To commission and assess the performance of AlignRT InBore™, a Halcyon™ and Ethos™-dedicated Surface Guided Radiation Therapy (SGRT) platform which combines ceiling-mounted cameras for patient setup and bore-mounted cameras for in-bore tracking. To check the potential impact of InBore™ cameras on dose delivery, 16 SRS, H&N, breast and pelvis patients' quality assurance (QA) treatment plans were measured with/without AlignRT InBore™ and using ArcCHECK® and SRS MapCHECK®. Impact on image quality was determined using Catphan® 540 phantom and considering all available MV and CBCT protocols (head, breast, chest and pelvis). The stability, accuracy and overall performance of AlignRT InBore™ was assessed using an MV Cube and anthropomorphic phantoms. Comparison of 2D dose distributions with/without AlignRT InBore™ showed no impact on treatment delivery for all 16 QA checks (p-value > 0.25). 2D and CBCT images showed no artefacts or change in the contrast-to-noise ratio, resolution and noise values measured with Catphan® 540. Anti-collision sensors were unaffected by the bore-mounted cameras. Additionally, AlignRT InBore™ cameras allowed for motion detection with sub-0.5 mm accuracy and sub-0.4 mm stability with surface coverage of >50 × 60 × 35 cc. Accurate transition (sub-0.3 mm) from virtual to treatment isocentres was achieved. Finally, Halcyon™ rotations during CBCT and beam delivery resulted in limited camera vibrations with translation uncertainty <0.5 mm in left-right and anterior-posterior directions and <0.1 mm in head-feet direction. AlignRT InBore™ provides SGRT setup and intrafraction monitoring capabilities with a performance comparable to standard SGRT solutions while having no adverse effect on Halcyon™. Purpose: Surface guided radiotherapy (SGRT) is reported as a feasible setup technique for whole‐breast radiotherapy in deep inspiration breath hold (DIBH), but position errors of bony structures related to deeper parts of the target are not fully known. The aim of this study was to estimate patient setup accuracy and margins obtained with two different SGRT workflows with and without daily kV‐ and/or MV‐based image guidance (IGRT). Methods: A total of 50 breast cancer patients were treated in DIBH, using SGRT for the patient setup, and IGRT for isocenter corrections. The patients were treated at two different departments, one using AlignRT® (25 patients) and the other using Catalyst™ (25 patients). Inter‐fractional position errors were analyzed retrospectively in orthogonal and tangential setup images, and analyzed with and without IGRT. Results: In the orthogonal kV‐kV images, the systematic residual errors of the bony structures were ≤ 3 mm in both groups with SGRT‐only. When fine‐adjusted by daily IGRT, the errors decreased to ≤ 2 mm; except for the shoulder joint. The residual errors of the ribs in tangential images were between 1 and 2 mm with both workflows. The heart planning margins were between 3 and 7 mm. Conclusions: The frequency of IGRT may be considerably reduced with a well‐planned SGRT‐workflow for whole‐breast DIBH with residual errors ≤ 3 mm. Purpose: We report our initial clinical experience using IDENTIFY, an optical surface guidance system, for monitoring intra-fraction motion during stereotactic radiosurgery with HyperArcTM. Methods: The IDENTIFY system consists of three ceiling mounted camera pods. For each treatment fraction, a reference surface was captured after radiographic image guidance prior to the first beam on. Surface guidance and linear accelerator trajectory logs were used to evaluate the reported offsets relative to the reference surface. Offsets were obtained at the start of treatment (after radiographic image alignment), end of treatment, and during treatment at 4 gantry angles: 140 and 220 degrees, when all three camera pods had a clear view, and 50 and 310 degrees, when the view of one of the camera pods was obstructed. Results: 733 fractions of 295 treatment plans were evaluated. The average treatment time was 3.35 min (range 1.87 to 7.45 min). The mean (x,y,z) translation offset at the end of treatment was (0.00, -0.01, -0.07) mm, with median magnitude 0.27 mm and 95% of magnitudes were less than 0.97 mm. The mean reported offset during treatment was (-0.40, 0.23, -0.08), (-0.25, 0.02, -0.16), and (-0.16, 0.21, -0.05) mm at table angles 45, 90, and 315 degrees, respectively. The mean change in translation when a camera pod was blocked by the gantry was (0.01, 0.14, 0.02), (0.10, 0.23, 0.07), and (-0.02, 0.15, -0.00) mm at table angles 45, 90, and 315 degrees, respectively. The fraction of offsets exceeding 1 mm was 4.4%, 6.9%, and 2.5% when all three cameras had an unobstructed view, and was 13.4%, 1.9%, and 10.6% when a camera pod was blocked at table angles 45, 90, and 315 degrees, respectively. Conclusion: The IDENTIFY system can be used to monitor for intra-fraction motion with submillimeter accuracy; however, the accuracy is reduced when a camera is obstructed by the gantry and/or the couch is rotated.
