Background/Aims: Psoriatic arthritis with peripheral and axial involvement; It is a heterogeneous disease that can cause enthesitis, dactylitis, and nail and skin involvement. The persistence of inflammation in psoriasis leads to comorbidities such as psoriatic arthritis, cardiovascular disease, and metabolic syndrome. Our study aimed to identify subclinical atherosclerosis in psoriatic arthritis patients without a known risk factor and to prevent possible morbidity and mortality in patients. Methods: Thirty-three patients with psoriatic arthritis and twenty-five healthy control groups were included in our study. Diabetes mellitus, hypertension, coronary artery disease, diagnosis of hyperlipidemia, body mass index above 30, and smoking were determined as exclusion criteria. The age, gender, duration of the disease, medical treatment used for the disease, high-density lipoprotein, low-density lipoprotein, triglyceride, and total cholesterol values, monocyte-lymphocyte ratio, carotid intima-media thickness measurement results obtained by carotid doppler ultrasonography imaging were recorded. Results: A comparison was made between the study and control groups in terms of age, triglyceride, high-density lipoprotein, low-density lipoprotein, and monocyte-lymphocyte ratio, and no statistically significant difference was observed. Introduction: Myocardial infarction remains a major cause of morbidity and mortality in the young population. The relationship between carotid intima-media thickness (CIMT) and atherosclerosis has been shown in many studies, however, there is no study investigating the association between carotid imaging and cardiovascular events in young patients. In our study, we evaluated the carotid imaging of young patients who experienced ST-elevated myocardial infarction (STEMI) and individuals at the same age and with normal coronary arteries. Methods: A total of 160 young patients were enrolled in the study. Of them, 115 patients were under the age of 45 years with STEMI and 45 were under the age of 45 years with normal coronary arteries shown in the coronary angiography. Carotid ultrasound was performed for all patients and they were divided into high-risk and low-risk carotid image groups according to CIMT and the presence of carotid plaque. Both groups were compared according to the traditional risk factors and the predictors of STEMI were investigated. Results: Both CIMT (0.87±0.28, vs 0.70±0.16, p<0.001) and the presence of carotid plaque (14.8% vs 2.2%, p=0.024) were found to be significantly higher in young patients with STEMI compared to the control group. Independently from other traditional risk factors, 0.1 mm increase in CIMT was associated with a 42% increase in odds for STEMI. Similarly, being in the high-risk carotid image group had 9.2 times increased odds for STEMI than being in the low-risk carotid image group. Conclusion: CIMT and the presence of carotid plaque have a predictive value for cardiovascular events in young age independently from traditional risk factors. [ABSTRACT FROM AUTHOR]

Background: Diagonal earlobe crease (DEL) is considered a sign of atherosclerosis. Platelets play an important role in the onset and subsequent complications of atherosclerotic lesions. Mean platelet volume (MPV) is an indicator of platelet activation. In this study, we aimed to evaluate MPV in DEL individuals. Material and Methods: Seventy-five patients with DEL (28F; mean age: 59±10 years) and 54 patients without DEL (23F; mean age: 56±11 years) were included in this study. Clinical and demographic characteristics, MPV, leukocyte counts, hemoglobin levels, platelet counts and plateletcrit levels were compared between the two groups. Results: Clinical and demographic characteristics were similar between the two groups. Hemoglobin levels (13.7±1.3 g/dL vs. 13.8±1.40 g/dL; p=0.782), leukocyte counts (7.01±1.57 x103mm-3 vs. 7.26±1.63 x103mm-3; p=0.429), platelet counts (240±57 x103mm-3 vs. 246±59 x103mm-3; p=0.617) and plateletcrit levels (0.207±0.048 % vs. 0.196±0.046%; p=0.258) were comparable between the two groups. MPV was significantly higher in the DEL group compared to non-DEL group (8.69±0.95 fL vs 7.96±0.75 fL; p<0.0001). Correlation analysis showed a significant but moderate correlation between DEL and MPV (p<0.0001; r = 0.356). Conclusion: MPV is elevated in patients with DEL. Considering the effects of platelets on atherosclerosis as well as their close relation to other risk factors, MPV may be an important predisposing factor for atherosclerosis in patients with DEL. Objective: The aim of our study was to determine the usefulness of the aortic knob width (AKW) in the assessment of subclinical atherosclerosis in hypertensive patients. Methods: In this study on diagnostic accuracy, 374 consecutive hypertensive patients with at least one cardiovascular risk factor were enrolled. AKW was measured on chest X-ray. Cardio-ankle vascular index (CAVI) was measured by VaSera-1000 CAVI instrument. The diagnostic value of AKW was assessed using ROC analysis. Results: There was a significant correlation between aortic knob width and CAVI (r=0.45, p<0.001), age (0.39, p<0.001), systolic (r=0.17, p<0.001), diastolic (r=0.23, p<0.001) and mean (r=0.2, p<0.001) blood pressures. In linear regression analysis CAVI (β=0.3, 95% CI 0.33-0.98, p<0.001), age (β=0.3, 95% CI 0.09 - 0.21, p<0.001) and diastolic blood pressure β=0.2, 95% CI 0.08-1.9, p<0.001) were independently associated with AKW. It was significantly higher in patients with subclinical atherosclerosis (CAVI ≥9) than borderline (8 ≤CAVI <9) (41.4±5.5 versus 36.7±5.3 mm, p<0.001) and healthy (CAVI <8) subjects (41.4±5.5 versus 35.5±4.3 mm, p<0.001). Analysis using the ROC curve has demonstrated that aortic knob of 41 mm constitutes the cut-off value for the presence of subclinical atherosclerosis with 71% sensitivity and 77% specificity (AUC-0.67, 95% CI 0.51-0.82). Amaç: Prinıer Sjögrcn sendronıu (pSS) kronik, inflamatuvar ve otoimnıün bir hastalıktır. İnflamasyon artmış kardiyovasküler risk ve ateroskleroz ile yüksek derecede ilişkilidir. Karotis inti-ma-media kalınlığı (KIMK) ve arteryel stiffiıess ölçümleri subk-linik aterosklerozun tespit edilmesinde sıklıkla kullanılan yöntemlerdir. Çalışmamızın amacı; pSS tamlı hastalarda, Ultrason (US) Radyofrekans (RK) Data Teknolojisi kullanılarak saptanan bu ölçümlerle subklinik aterosklerozun noninvaziv bir şekilde ortaya konmasıdır. Yöntem: Çalışmamıza 30 pSS tamlı hasa ve 30 yaş-cinsiyet uyumlu sağlıklı kontrol olgusu alındı. Her iki grupta da katılımcıların diyabetes mellitus, hipertansiyon, hiperlipidemi ve obe-zite gibi herhangi bir prinıer kardiyovasküler risk faktörü yoktu ve 18-60 yaş arasındaydılar. Çalışma ve kontrol grubu olguları Doppler US ile değerlendirildiler. Arteryel stiffiıess ve KIMK ölçümleri bilateral common karotid arterlerden US RF dam teknolojisi kullanılarak tespit edildi. US RF data teknolojisi başta subklinik ateroskleroz olmak üzere vasküler hastalıkların tespitinde kullanılan yeni bir sonografik metottur. Bu metot ile ölçümler otomatik ve doğru bir şekilde yapılırken, ölçümün kalitesi ile ilgili de geri bildirimler vermektedir. Diğer yöntemlerle ölçüm sonuçlan milimetre düzeyinde verilirken bu metot ile mikrometre düzeyinde verilir. Bulgular: Her iki grup arasında sağ, sol ve ortalama KIMK, sağ taraf distensibility cocfficient (DC) ve conıpliance coefficient (CC), sağ ve sol taraf α ve β stiffncss indeksi değerleri açısından bir fark tespit edilmedi (P>0.05). Hasta grubunda sağ ve sol pul-sed wave velority (PWV), sağ ve sol taraf ortalama. stiffncss indeks, , stiffncss indeks ve PWV değerleri anlamlı olarak daha yüksekti (P<0.05). Sol taraf DC ve CC değerleri ile ortalama DC ve CC değerleri hasta grubunda anlamlı olarak daha düşüktü (P<0.05). Tablo PS-llO'da bahsedilen bu ölçüm değerleri görülmektedir. Sonuç: Çalışmamızda pSS tamlı hastalarda subklinik ateroskle-rozun kanıtlan tespit edilmiştir. Literatürde pSS tandı hastalarda lıaşta arteriyel stiflfness olmak üzere subklinik ateroskleroz konusunda sınırlı sayıda çalışma vardır. Subklinik aterosklerozun pSS hastalannda tespit edilebilmesi için KIMK ve PWV ölçümleri bizlere kılavuz olabilir. US RF data teknolojisi KLMK artışı ve vasküler el asti sitede ki azalmanın kantitatif ve doğru bir şekilde ölçümü için noninvaziv bir yaklaşım sağlar. [ABSTRACT FROM AUTHOR]
