Objectives: Antivenoms are important emergency medications to be held within Australia, particularly in regional and remote locations. We audited current antivenom holdings in hospitals and health services across South Australia (SA) and compared to recommendations in the 'Snakebite and Spiderbite Management Guidelines' from the State's Toxinology service. The process also assessed the feasibility of 'real‐time' remote stock monitoring. Methods: Fifty‐three sites listed in the guideline were recommended to hold antivenom, though only 49 are currently operational. Interrogation of antivenom stock for 29 sites was possible using electronic reports generated from the State Pharmacy database. The 20 remaining centres had their stock levels confirmed by calling the centres directly. Data obtained were then compared to recommended levels of antivenom holdings in the guideline with discrepancies and associated costs documented. A separate report verification process was used to determine 'real‐time' accuracy of the electronic reports. Results: Thirty‐seven sites (75%) held more than the recommended number of antivenom vials, totalling 129 vials in excess with an approximate total cost of $110 000. Twelve sites (24%) held inadequate stock to deliver a treatment dose for 19 envenoming events. The report verification revealed variances in the electronic reports. Conclusions: This audit has demonstrated a significant disparity between recommended and actual antivenom holdings across most sites in SA and has also revealed that 'real‐time' remote monitoring of state antivenom holdings is not currently feasible. Background: Snake bite is one of the neglected public health problems in many tropical and subtropical countries. Wrong perception and practice of unproven traditional methods about snakes and snakebites may increase mortality due to snakebite envenoming. Objective: The objective is to assess the knowledge, attitude and practice regarding snakes and snakebites among the rural population in the Kodagu district. Materials and Methods: A community-based cross-sectional study was conducted between August-2021 and October-2021 in the rural field-practice area of Bhagamandala Primary Health Centre. Systematic random sampling was incorporated into the study to select the study participants. Data were collected using a predesigned and pretested questionnaire. Data entry was done in Microsoft Excel and was analyzed using STATA (16.1), StataCorp, College Station, Texas, USA. Results: Out of 1284 participants, the majority 964 (75.07%) of participants identified spectacled cobra correctly and 1142 (88.94%) correctly identified it as venomous. About 1228 (95.64%) participants had correct knowledge regarding Anti-Snake venom being an effective treatment for snake bites and the overall knowledge regarding snake bites was adequate among only 380 (29.60%) of the study population. Whereas, 188 (14.64%) of them even considered visiting a local quack or faith healer after a Snakebite. Conclusions: In our study, the overall knowledge regarding snake bites was adequate among 29.60% of the study population. The knowledge regarding the venemosity of snakes was good, but identification was poor. The participants had correct knowledge about ASV and its availability. The harmful practices such as visiting traditional healers and applying native medicine are still seen in 14.64% of participants.

Hereditary factor VII (FVII) deficiency is an uncommon autosomal recessive disorder associated with mutations in the F7 gene, and laboratory investigations usually reveal isolated prolongation in prothrombin time (PT)/international normalized ratio (INR). Venom-induced consumptive coagulopathy (VICC) is distinguished by the activation of the coagulation pathway, which is triggered by procoagulant toxins in snake venom. Diagnosing snakebites in patients with hereditary FVII deficiency presents a challenge because prolonged time PT/INR is considered the most valuable diagnostic method for VICC. Therefore, it is possible that certain patients may not promptly receive an accurate diagnosis of hereditary FVII deficiency. Background: Coagulopathies can manifest on a spectrum, from minor mucosal bleeding to life-threatening hemorrhage. Minor cases can be discovered in the setting of known risk factors, such as malignancy, old age, immunosuppression. However, acquired hemophilia A diagnosed after a snake bite is of lesser-known incidence and can present in a more acute, potentially life- or limb-threatening fashion. To properly diagnose this coagulopathy, one must be familiar with the related signs, symptoms, and laboratory findings so that swift diagnosis can follow. Diagnosis is key for early proper management, as displayed in the following case. Case Report: Our case report details a male patient presenting with diffuse bruising after a snake bite. Initially, on presentation to outside facilities, the diagnosis of acquired hemophilia A was not found. However, upon worsening of bruising in the setting of previous treatments initiated for the patient, he presented to our facility, where he subsequently received a diagnosis with acquired hemophilia A. He developed compartment syndrome due to excessive bleeding, requiring surgical intervention. With proper diagnosis, his bleeding diathesis was corrected with multiple rounds of repletion of factors and immunosuppression. His follow-up laboratory test results and examinations have shown continued resolution of his symptoms. Conclusions: As acquired hemophilia A is less often linked with snake bites, this case highlights the importance of considering this disease process as a differential in patients with bleeding diathesis after a snake bite. Snakebites result from snake bites, causing a major impact on public health in Brazil. Snakes of the genus Bothrops, popularly known as "jararacas", are responsible for the vast majority of snakebites reported in Central and South America, requiring immediate medical treatment due to the actions of their venom, which can affect the circulatory system and lead to tissue damage. and hemorrhages. Antivenom serum is effective and essential for neutralizing the effects of venom in humans, reducing the severity of symptoms and helping to save lives. The purpose of the present study was to present a literature review on the predominant clinical manifestations in cases of snakebites with the genus Bothrops, thus increasing the population's knowledge regarding the main signs and symptoms that should be given more attention in the face of such accidents. According to the results obtained, it was found that the main and most recurrent clinical manifestations involving accidents with "jararaca" are: pain, edema, erythema, ecchymosis and serious complications such as blister formation, abscess and necrosis. Además se ofrecen algunas ideas para desarrollar programas educativos sobre estos reptiles y para entrenar a los niños sobre qué hacer si se encuentran con una serpiente en su escuela o en otro lugar. This case study examines the clinical course of a Bothrops snakebite poisoning that occurred in Jarinu, São Paulo, Brazil, in 2021. The patient required a fasciotomy in the upper limb due to acute compartment syndrome, followed by a second surgical procedure involving dermo-epidermal grafting. In the late postoperative period, the patient experienced retraction, leading to a subsequent surgical intervention to correct the retraction in the left upper limb. The severity of the snakebite accident, the effects of venom on tissues, complications, acute compartment syndrome, as well as the indications and techniques for decompressive fasciotomy, are discussed based on the available literature.

Introduction: Snakebites are a neglected tropical disease. In many areas, envenoming incidence and antivenom administration rates are unknown. This study compared antivenom (AV) availability to rates of envenoming and recommendations to treat (RTT) in South Africa. Methods: This retrospective study identified, extracted, and reviewed all cases of envenoming (snake bites and spits) reported to the Poisons Information Helpline of the Western Cape of South Africa (PIHWC) from June 1, 2015 to May 31, 2020 by public hospitals in the Western Cape. A standardized interview was administered to the pharmacies of the 40 hospitals in winter and summer to determine how many vials of monovalent and polyvalent AV they had on hand at the time of the call and their expiration dates. Descriptive analysis was used to compare rates of envenoming and recommendations to treat to antivenom stock in winter and summer and by hospital type and location. Results: Public hospitals reported 300 envenomings, 122 from snakes. The PIHWC recommended antivenom administration in 26% of cases (N = 32). All hospital pharmacies queried answered our questions. Our study demonstrates urban district hospitals have higher ratios of AV vials compared to mean annual rates of envenoming and RTT than rural district hospitals at both the winter and summer timepoints. Conclusion: This study evaluates antivenom supply and demand in a province of South Africa. The findings suggest South African urban hospitals have a relative excess of antivenom, and thus more capacity to meet demand, than their rural counterparts. It supports consideration of a redistribution of antivenom supply chains to match seasonal and local rates of envenoming. It indicates a need for higher quality, prospective data characterizing envenoming incidence and treatment.

Abstract Background Mamushi bites are the most common venomous snake bites in Japan, with known complications including rhabdomyolysis and acute kidney injury; however, adrenal insufficiency as a result of snake bites has not been previously reported. We report a case of empty sella with transient adrenal insufficiency during hospitalization for a Mamushi bite. Case presentation An 84-year-old man was admitted to our hospital with a Mamushi bite on the right fifth finger. Serum sodium (Na) level remained in the normal range. On the ninth day of admission, he developed hyponatremia, with a serum Na level of 114 mEq/L and serum cortisol level of 4.0 μg/dL (reference value 4.5–21.1 μg/dL). His serum Na level was restored within the normal range after administration of corticosteroids with 3% NaCl solution. Both rapid adrenocorticotrophin and corticotropin-releasing hormone loading tests showed low cortisol response. Based on the results of the hormone loading tests, a diagnosis of pituitary adrenal insufficiency was made. Contrast-enhanced pituitary magnetic resonance imaging (MRI) showed primary empty sella. After discontinuation of corticosteroids, the hyponatremia did not recur, and the patient was discharged on the 24th day of hospitalization. After discharge, the patient visited an outpatient clinic, but hyponatremia recurrence was not observed. Conclusions This is the first report of hyponatremia due to pituitary adrenal insufficiency during hospitalization for a Mamushi bite in a patient with empty sella. When hyponatremia occurs during hospitalization for a Mamushi bite, cortisol measurement, hormone loading test, and head MRI should be performed to search for pituitary lesions because of the possibility of adrenal insufficiency caused by snake venom.

Rattlesnake envenomation is an important cause of morbidity and mortality in humans and animals in the southwestern United States and elsewhere. Two Visayan warty pigs (Sus cebifrons) from a regional zoo were submitted for autopsy after being found dead close to a southern Pacific rattlesnake (Crotalus helleri) in their enclosure. Both pigs had severe regionally extensive cutaneous, subcutaneous, and muscle hemorrhage and edema with myonecrosis. Additionally, both pigs had lesions consistent with puncture wounds within the oral cavity, and one pig had a similar wound on a forelimb. The history, and gross and histologic findings, were consistent with envenomation by rattlesnake bite. There are few documented cases of snakebite envenomation in pigs, and it had been suggested that pigs may have some degree of resistance to envenomation. Our results indicate that warty pigs are susceptible to the action of rattlesnake venom.

In this respect, tests such as prothrombin time (PT), international normalized ratio (INR), and activated partial thromboplastin time (aPTT) are commonly used. The present study aimed to determine the value of coagulation parameters in the prognostication of snakebite patients. Methods: This prospective descriptive observational study examined the coagulation parameters, including PT, INR, and aPTT, in consecutive cases of snake envenomation admitted to a tertiary health care hospital from October 2019 to August 2021. The correlation of coagulation parameters with each other and with prognostic indicators (ie, the number of days spent in the intensive care unit [ICU], the requirement of anti-snake venom [ASV], fresh frozen plasma [FFP], and the total number of blood products) was studied. Results: A total of 58 patients with snakebite were studied. The majority were males (69%). The highest proportion of patients belonged to the age group of 16-40 years (60%). The correlation of coagulation parameters with each other was statistically significant. The correlation of coagulation parameters with prognostic indicators (ie, the total number of ASV vials administered, the total number of days in the ICU, the total number of FFP units administered, and the total number of blood components administered) was statistically significant. Conclusion: Coagulation parameters (PT, INR, and aPTT) are essential laboratory investigations for treatment monitoring and can be used effectively for the prognostication of snakebite patients. In cases where the coagulation profile of snakebite patients shows abnormalities, conducting repeated coagulation studies at intervals of 12 hours can contribute to improved prognostication.

IntroductionFew of the 5000-8000 snakebites reported to poison control centers annually in the USA are attributed to coral snakes. This study describes Texas coral snake envenomations reported to the North American Snakebite Registry.MethodsAll Texas coral snake envenomation cases reported to the registry were identified for the period from January 1, 2015, through December 31, 2019. Data reviewed for this study included details regarding the snake encounter, patient demographics, signs and symptoms, treatment, and outcomes. Descriptive statistics were used to report results.ResultsTen men and four nonpregnant women reported coral snake bites. The median patient age was 15.5 (range 5-72 years). There were 12 upper extremity bites and two bites to the lower extremity. The most common symptoms reported were paresthesias and pain. All subjects had paresthesias, often described as an "electric" sensation. Seven patients described them as painful. The most common clinical findings were erythema and swelling. No patient developed tissue damage, hematotoxicity, rhabdomyolysis, hypotension, weakness, or respiratory symptoms. Thirteen subjects were treated with opioids. Six patients were treated with antiemetics: three prophylactically and two for opioid-induced nausea. One patient developed nausea and non-bloody, nonbilious emesis within 1 hour of the bite, prior to receiving opioids. No patients were treated with antivenom. Antibiotics were not administered to any patient, and no infections were reported.ConclusionsEnvenomations from M. tener in Southeast Texas are characterized by painful paresthesias. Mild swelling and erythema are common. Neurotoxicity necessitating antivenom or mechanical ventilation did not occur.

The history of research and preparation of prescriptions for snake bites in Fujian Province was reviewed, and the process of incorporating snake doctors into the system and the gradual establishment of snake bite treatment system in Fujian were explored. The composition of several main snake bite preparations in Fujian was preliminarily compared and analyzed to provide reference for the research and development of similar new preparations at present. BackgroundRattlesnake envenomations are a significant cause of morbidity in the USA. While pediatric rattlesnake envenomations are relatively common, data comparing adult and pediatric patients with rattlesnake envenomations remain limited.MethodsThis multi-center retrospective study used the North American Snakebite Registry (NASBR), a sub-registry of the Toxicology Investigator's Consortium (ToxIC). All cases of rattlesnake envenomations between January 1, 2013, and December 31, 2017, which were entered into the NASBR, were reviewed. Clinical and laboratory parameters, as well as treatment and outcome measurements, were compared between adult and pediatric patients.ResultsA total of 420 unique cases were identified, including 94 pediatric patients. Adult patients were more likely to be male (76% vs. 62%; OR 1.98) and sustain upper extremity envenomations (57% vs. 25%; OR 4.4). After adjusting for bite location, adults were more likely to exhibit edema compared with pediatric patients. After controlling for envenomation location, there was no difference in rates of necrosis between adult and pediatric patients. Adults exhibited early hematologic toxicity less frequently than pediatric patients, but there was no difference in the rates of late hematologic toxicity. There were no differences in the rates of hypotension or intubation.ConclusionWhile adult and pediatric patients have some differences in envenomation characteristics and laboratory parameters, adults and pediatric patients had similar rates of systemic toxicity, severity, length of stay, and late hematologic toxicity.

Abstract Aim: YouTube has become an increasingly used platform for obtaining health information such as first aid knowledge of snake bites rescuers in recent years. Aim of the study is to evaluate the quality of existing YouTube videos on first aid interventions for snake bites and whether they are of an educational nature. Materials and Methods: Descriptive study. On February 20, 2021, determined search terms such as; “Snake bite emergency aid, Snake bite treatment medicine, first aid in snake bite“ were searched separately on YouTube (https://youtube.com). A total of 360 YouTube videos were listed. It was analyzed according to the Global Quality Scale (GQS) developed to evaluate internet-based resources and the DISCERN scale developed to determine reliability. Results: A total of 72 videos meeting the criteria were identified. The median duration of the videos were 292 seconds. The median number of views were 13.8, the number of comments were 0.01, likes were 0.11, and the number of dislikes were 0.01. When the DISCERN scores and uploaded sources were compared, significant differences were found (p = 0.031). Botropic accidents are the most common among snake accidents in Brazil. The species Bothrops jararaca, the popular jararaca, predominantly inhabits the south and southeast regions, and thus is the most relevant venomous snake in the region, being responsible for most accidents. Its poison causes important clinical repercussions, with proteolytic, coagulant and hemorrhagic actions, which cause an exuberant clinical picture. In addition to the pain, local manifestations and coagulation disorders, the jararaca sting can present complications such as infections and tissue necrosis. Thus, this case report aims to describe the approach and evolution of a botropic accident involving a previously healthy 5-year-old pediatric patient.

Snakebite is considered a major health threat in Iran, mostly caused by pit vipers (Crotalidae species). A bite by a viper snake causes the release of plasma and blood to the tissue around the bite inflammation and tissue damage. The occurrence of epididymitis in the absence of a bite in the scrotal area is an unusual phenomenon. In this case report study, we reported epididymitis in a 16-year-old patient who was bitten by a viper on the right leg. This patient initially experienced local bleeding, reversible coagulopathy (thrombocytopenia and increased prothrombin time and international normalized ratio), leukocytosis, and neutrophilia. Due to the occurrence of a new organic complication, re-administration of five vials of snake antidote polyvalent serum along with corticosteroid, pain reliever, and antihistamine alleviated epididymitis and the patient was discharged consequently with full recovery. Deaths occurring due to rattlesnake envenomization are extremely rare and must be thoroughly investigated in the same manner as any other type of death. Our research presents the case of an adult white male who suffered a fatal timber rattlesnake (Crotalus horridus) envenomation in northwest Florida in 2018. Blood samples were taken from the decedent's heart and vasculature of the chest and sent for serum proteomic analysis. Serum proteomic analysis was utilized in order to identify proteins from timber rattlesnake (C. horridus) found within the victim's blood. The confirmation of the presence of timber rattlesnake venom within the victim's blood allows the forensic pathologist to determine the cause of death most accurately and likewise, assists with the manner of death determination. Blood samples were separated into two groups: one with the abundant endogenous proteins depleted to facilitate detection of lower abundant proteins and one undepleted. In the depleted sample, a total of 712 proteins were identified, with 47 of the proteins (6.6%) occurring originating from timber rattlesnake (C. horridus). Likewise, a total of 742 proteins were identified in the undepleted sample, with 52 of the proteins (7.0%) occurring in timber rattlesnake (C. horridus). Objective: to identify the knowledge of the nursing staff in dealing with bothropic accident victims in the emergency service. Methods: descriptive, exploratory study with a qualitative approach. Developed in 2019, with professionals from the nursing staff of the adult emergency sector of a University Hospital. A semi-structured interview was used, transcribed and analyzed using simple descriptive statistics and content analysis. Results: 34 professionals from the nursing team participated in the study, being 82% female, with an average age of 37 years. The content analysis revealed three categories: knowledge about accidents caused by poisonous animals, academic and professional trajectory in the context of poisonous animals and management of the nursing team to the patient victim of bothropic accidents. Abstract Background Snake envenomation is a public health problem that has caused many deaths, disabilities and significant physical, psychological and socio-economic losses worldwide. In many rural communities, some plants have been utilized for preventing snake envenomation. The present study aims to document the knowledge of traditional medicine specialists on the plants used to prevent snakebites or repel snakes in Benin. A survey was conducted between January 2020 and September 2020 to this end. A total of 238 traditional healers and 56 hunters of different socio-cultural groups were interviewed using snowball sampling technique. Survey data were analyzed using Relative Citation Frequency. The diversity of antisnake bite plants was assessed using the Shannon diversity index, Piélou equitability, the Bray–Curtis similarity index and a Factorial Correspondence Analysis. Finally, qualitative phytochemical screening was performed on aqueous extracts following standard methods based on staining and/or precipitation reactions. Results A total of 74 plant species belonging to 36 families and 70 genera were identified. The most frequently encountered families were Leguminosae (27.3%), Euphorbiaceae (8.1%), Arecaceae, Asteraceae and Cucurbitaceae (4.5% each). Annona senegalensis, Securidaca longipedunculata, Piliostigma thonningii, Chenopodium ambrosioides and Cymbopogon citratus were the most cited species. The analysis of the intra-community diversity of antisnake bite plants from socio-cultural and sociolinguistic groups showed a high diversity and strong equi-representativeness of the plants used in the prevention of snakebites in Benin. Preliminary qualitative analysis of the bioactive compounds in extracts of the most cited antisnake bite plants revealed the presence of alkaloids, flavonoids, tannins, saponosides, mucilages, reducing compounds and triterpenoids. Conclusions In-depth pharmacological and phytochemical studies would make it possible to rationalize on the effectiveness of the identified medicinal plants as well as provide insights of the compounds responsible for their antisnake bite activity.

Objective: To define the microbiologic characteristics of animal bites in tropical Australia and the appropriateness of current Australian antimicrobial guidelines for their management. Methods: This retrospective audit examined hospitalizations in tropical Australia after an animal bite or animal-associated penetrating injury between 2013 and 2020. The primary outcome was a composite of death, intensive care unit admission, amputation, quaternary center transfer, or unplanned rehospitalization. Results: A wide variety of animals were implicated, but snakes (734/1745, 42%), dogs (508/1745, 29%), and cats (153/1745, 9%) were the most common. Hospital presentation after 24 hours (odds ratio (OR) (95% confidence interval (CI)): 68.67 (42.10-112.01)) and a cat-related injury (OR (95% CI): 22.20 (11.18-44.08)) were independently associated with an increased risk of infection. A pathogen not covered by the relevant antimicrobial regimen recommended in Australian guidelines was identified in only 12/1745 (0.7%) cases.
