The article explores the process I underwent while writing a book that integrated my father's Holocaust testimony into a historical monograph. It emphasises the complexities I faced as a historian and a second-generation Holocaust survivor. It discusses how I navigated historiographical challenges such as truth, reliability, authenticity, transference, and representation through the historian's key functions: research, narrative construction, and writing. It examines the ethical considerations, stylistic choices, and emotional resonance involved in engaging with survivor testimonies. The article delves into the advantages and pitfalls of using one subjective testimony as the core of an atypical narrative that blurs the lines between autobiography and historical analysis. Furthermore, it considers the reasons why the book is narrated in an unconventional language style, as I chose to make the survivor the narrator, creating a biography written in the first person. The article portrays how, by blurring the boundaries between the intimate and the academic aspects, a balance is found between the emotional authenticity of personal testimony and the scholarly rigour required in historiography. Despite relative progress, it is an overt fact that women still take on the heavier burden of unpaid work in the household compared to men in the United States. Our study expands the horizons of previous studies by looking at women specifically and analyzes how women’s performance of unpaid labor differentiates based on various contexts such as generational status, household composition, race, and ethnicity. Using the pooled cross-sectional data from the 2011-2021 American Time Use Survey (ATUS), we test hypotheses derived from an absolute resources perspective and a structural perspective to examine migrant generational differences among married, cohabiting, and single women as well as investigate the intra-group differences of racial and ethnic groups. From our descriptive analysis and ordinary least squares (OLS) regression analyses, we find support for the structural perspective for housework, as the generational differences remain even after controlling for socioeconomic factors, but only for first-generation married women. However, socioeconomic factors account for the differences between cohabiting or single first-generation women, which is supported by the absolute resources perspective. Our intra-group analysis by racial and ethnic groups yielded similar findings but found that Hispanic and Asian first-generation women take on a larger proportion of housework, relative to later-generation women. Language proficiency is a crucial skill for immigrants that influences their social integration and their children's development. This study examines the intergenerational effects of limited English proficiency (LEP) on children's health and health care utilisation. We mitigate potential selection issues arising from insurance coverage by examining Australian-born children who are all covered under a universal public health insurance scheme. We use Australian population Census and longitudinal survey data linked to administrative health care records, and variation in parent's language acquisition, based on their age at arrival into Australia. We find that parental LEP has a strong and positive effect on children's use of general practitioners, but no effect on their use of other healthcare services, or on their physical or mental health. This article explores the assimilation of second-generation migrants from the Former Soviet Union (FSU) who grew up in Israel's social and geographic urban periphery. Their upbringing in peripheral towns and neighborhoods exposed them almost exclusively to Mizrahim (Oriental Jews), a stigmatized minority group whose members make up the overwhelming majority in those locations. We argue that their interaction with members of this group resulted in an alternative process of assimilation, which we term Hitmazrehut (Orientalization). Children of migrants, who were unaware of the historical stigmatization of Mizrahim and their culture, conceived of Hitmazrehut as their only viable trajectory of assimilation into what they erroneously perceived as the Israeli mainstream. However, embracing Mizrahi socio-cultural practices, including consumption and production of ethnic culture, religious traditions and romantic partnerships, did not lead to their downward assimilation, as the model predicts. Rather, it allowed second-generation Russian-speaking migrants to fit in and attain social and economic mobility by assimilating sidewards, into the growing Mizrahi middle-class. Robotic-assisted total knee arthroplasty is increasingly used in revision total knee arthroplasty (rTKA), with imageless systems recently receiving Food and Drug Administration (FDA) approval. However, there remains a paucity of literature on the use of robotic assistance in revision total knee arthroplasty (TKA). This paper describes the imageless surgical technique for robotic revision TKA using a second-generation robotic system and details both intraoperative and 90-day outcomes. This was a retrospective review of 115 robotic revision TKAs from March 2021 to May 2023 at 3 tertiary academic centers. Patient demographics, perioperative surgical data, and 90-day outcomes were collected. Pain and Patient-Reported Outcomes Measurement Information System scores preoperatively and postoperatively were recorded. All-cause reoperations at the final follow-up were detailed. The mean patient age was 65 years (range, 43 to 88), and 58% were women. The mean follow-up time was 13 months (range, 3 to 51). The most common indications for rTKA were instability (n = 37, 32%) and aseptic loosening (n = 42, 37%). There were 83 rTKAs to a posterior-stabilized liner, 22 to a varus-valgus constrained liner, and 5 to a hinged construct. The median polyethylene size was 11 (interquartile range, 10 to 13), and 93% of patients had their joint line restored within 5 millimeters of the native contralateral knee. Within the 90-day postoperative window, there were 8 emergency department visits and 2 readmissions. At the final follow-up, there were 5 reoperations and 2 manipulations under anesthesia. There were 4 patients who required irrigation and debridement after superficial wound dehiscence, and one had an arthrotomy disruption after a fall. This review demonstrates favorable intraoperative and 90-day outcomes and suggests that imageless robotic surgery is a promising modality in rTKA. Using data from the American Community Survey for 2014–2018, we provide empirical evidence about the demographic and socioeconomic characteristics of South Asian Americans. Our study investigates not only Indians, but also provides the first multivariate analyses for Bangladeshi, Nepalese, Pakistani, and Sri Lankans. The focus is on second-generation South Asians, but some descriptive statistics are shown for first-generation immigrants. In comparison to Whites, the educational distributions of first-generation immigrants are bimodal to varying degrees across the South Asian groups. However, with the exception of the Nepalese, all of the native-born South Asian groups obtain higher levels of education than Whites. Poverty among South Asian groups tends to reflect their educational levels so that poverty rates decline between the first-generation and the native-born, but second-generation Bangladeshi and Pakistani have somewhat higher than expected poverty due to family size and composition. Second-generation Indians, Pakistanis and Sri Lankans are more likely to be affluent than Whites, and these differences are partly but not fully explained by educational and other demographic characteristics. Introduction: This study evaluated 7-year effectiveness and safety of second-generation trabecular micro-bypass implantation (iStent inject) either in combination with cataract surgery or as a standalone procedure (Combined or Standalone subgroups, respectively) in eyes with open-angle glaucoma. Methods: This prospective, non-randomized, unmasked, longitudinal study included 125 consecutive iStent inject cases of a single surgeon at a large German academic hospital. Patients had considerable preoperative disease burden, with mean intraocular pressure (IOP) of 23.5 mmHg, 84.8% of eyes on ≥ 2 medications, and 38.4% of eyes with prior glaucoma surgery. IOP, medications, adverse events, and secondary surgeries were assessed through 7 years in the Overall cohort and in Combined (n = 81) and Standalone (n = 44) subgroups. Results: Over 7-year follow-up, mean IOP decreased by 36.2–40.0% in Overall eyes, 34.1–38.9% in Combined eyes, and 39.5–43.5% in Standalone eyes (p < 0.001 at all timepoints for all groups). Meanwhile, mean medications decreased by 59.3–71.3% in Overall eyes, 57.9–69.0% in Combined eyes, and 62.1–76.2% in Standalone eyes (p < 0.001 at all timepoints in all groups). At last follow-up (mean 77.4 months; 92.8% of patients with last visit at 6 or 7 years), 83.7% of Overall eyes, 82.3% of Combined eyes, and 86.4% of Standalone eyes had achieved ≥ 20% IOP reduction vs preoperative. At last follow-up vs preoperative, 100% of eyes in all groups had the same or lower IOP and 100% had the same or lower medication regimen. Safety outcomes were favorable, with no filtration surgeries and only 4.84% of eyes experiencing clinically significant visual field loss over 7 years of follow-up. Conclusion: iStent inject implantation with or without phacoemulsification produced significant and durable 7-year reductions in IOP (~ 34–44% reduction) and medications (~ 58–76% reduction) while preventing filtering surgery in this cohort of patients with relatively high preoperative disease burden. Second-generation migrant youth studies are under-researched in South Africa. Often their views are marginalised in social science research. Our article draws on the intersectionality approach to understand the daily experiences of this ‘disadvantaged group’. This article explores the complexities of second-generation Mozambican migrant youth’s lived experiences when constructing their identities and developing a sense of belonging in post-apartheid South Africa. The study was conducted in a rural area in Bushbuckridge, South Africa approximately 100 km from the Mozambican border. This case study is informed by semi-structured interviews and narratives from 22 second-generation Mozambican migrant youths aged 18 to 34 years. Views of two key informants are also provided. We found that undocumented migrants struggle to integrate into host communities and have to be creative in navigating social challenges in creating an identity and a sense of belonging in South Africa. They shared feelings of living in limbo since childhood and fear of possibly dying in limbo with no clear sense of belonging and identity. There is a strong association between documentation status, feelings of identity, sense of belonging and levels of social integration. We recommend the development of a multilayer comprehensive model to uphold the lives of vulnerable groups in South Africa. Transdisciplinary Contribution: In exploring the interdependency and interconnectedness of social categories and social systems, the article revealed that participants’ experiences of identity formation and sense of belonging were marginalised in complex, intersectional and precarious ways where they constantly (re)negotiated their experiences, shaped by their paradoxical migrant status in South Africa.

There is a growing body of research examining the prejudice and discrimination experienced by Muslim youth in Canada. This article explores the narratives of feeling excluded and misunderstood articulated by 8 Canadian-born Shia Ismaili Muslim youth (aged 14-17). Drawing on a psycho-social theoretical framework, I speculate that youth utilize a "happiness defense": a term that denotes their portrayal of being a "good immigrant" or happy subject who is grateful to Canada for giving them a home. My analysis looks behind the scenery of the happiness defense to examine deeper dynamics such as discomfort, difference, and anxiety as youth seek to find a place of belonging in Canada. Underneath the happiness defense, I speculate about the anxiety of hybridity that is linked to feelings of not belonging. Based on ethnographic fieldwork and life-story interviews, this article examines parenting and school strategies of the emergent second generation of minority Danish parents and how they are shaped by their generational position. Across socio-economic differences, the participating parents are highly engaged in their children's schooling and well-being, practising an 'intensive parenting' widespread in the contemporary social generation and also dominant among majority Danish parents. Yet they also draw on their first-hand knowledge of being minority children in Danish schools and society to protect their children against discrimination against Muslims and negative influence from both majority and minority Danes. The article suggests that the parents thus engage in 'intensive minority parenting' and employ different school strategies: an 'averting strategy' characterised by fending off negative influences and antagonists in multi-ethnic neighbourhoods; a 'stepping-up strategy' characterised by performing middle-class parenthood in (upper)middle-class neighbourhoods; and a 'shielding strategy' characterised by carving out a safe new cultural space in 'free schools' with other second-generation parents. Differing from the paths of 'segmented assimilation', these strategies do not depend on ethnic background, but rather on the parents' current social environment and interrelated factors of their own experiences with school, further education, their family and broader society. Abstract Background Female genital mutilation (FGM) is a practice that involves the removal of external female genitalia and is widely known as a violation of human rights. The custom is illegal in the United Kingdom (UK) and carries a sentence of up to 14 years in prison. This prohibition, along with the secretive nature of the practice, has led to limited research on the awareness of FGM on young people in the UK. Little is known about the process of involving young people in research about the topic. Methods This paper is based on the findings of a Ph.D. project that used a community-based participatory research approach (CBPR). The research took a two-stage approach: stage one aimed to recruit nine young people aged 15–18, from the Southwest of England, who attended a 10-day training workshop to prepare them for stage two—data collection with young people aged 13–15. This paper focuses on the 10-day creative, collaborative workshops. The data collected from the collaborative workshops were analysed using thematic analysis. Results Undertaking CBPR enhanced the quality and relevance of this research. Engaging young people as co-researchers was vital for the success of this project. By developing a collaborative learning environment, young people were able to build trusting relationships which flourished beyond the research project. In addition, the creative workshops enabled peer learning about FGM and inspired young people to learn new skills that was useful in their daily lives. Conclusion The collaborative environment created in this project enabled an insightful learning experience for young people and researchers alike. Participants and facilitators formed relationships; participants learned new marketable skills and researchers gained new insights about FGM, from a young person’s perspective.

Introduction The 'second-generation' (i.e. the children of migrants) represent one of the fastest growing subpopulations of the child and young adult populations in Europe today. The research so far appears to indicate that their mortality risk is elevated relative to people with non-migrant backgrounds. Sources of data Peer-reviewed publications. Areas of agreement Second-generation status is a clear marker of elevated mortality risk in Europe in early life (including stillbirth, perinatal, neonatal and infant mortality) and adulthood, particularly if the parent(s) were born outside of Europe. Socioeconomic inequality plays an important, albeit rarely defining, role in these elevated risks. Areas of controversy It remains unclear what causes-of-death are driving these elevated mortality risks. The exact influence of (non-socioeconomic) explanatory factors (e.g. health care, racism & discrimination, and factors related to integration) on the elevated mortality risks of the second-generation also remains unclear. Growing points The second-generation will continue to grow and diversify in Europe; we must intervene to address these inequalities now. Utilising semi-structured interviews, this study aims to explore how second-generation Arab migrants perceive and negotiate their belonging in Qatar. It is argued that second-generation Arab migrants construct their belonging to Qatar beyond the legal definition (Citizen/resident) imposed on them as temporary migrants. However, the same legal definition is key to understand how their sense of belonging to Qatar is perceived and negotiated. Participants' reflections on their sense of belonging were analyzed based on two analytical levels: the place-belonging level, and the politics of belonging level. Three general strategies for negotiating the gap between the sense of place-belonging and being exposed to the boundaries of belonging in the Qatari context were identified. The study concludes that while second-generation Arab migrants construct their positions on the spectrum of belonging – unbelonging beyond the legal status imposed on them, such status is still the rigid criteria on which inclusion and exclusion are based. It can sometimes be hard for young people to get involved with research involving a sensitive topic, such as female genital mutilation. This may be due to several reasons, one of which is the hidden nature of the practice, which may make it harder for researchers to engage and involve the youth. This project involved a group of second-generation youth from FGM-affected communities as co-researchers in a community based participatory research (CBPR) approach, which examined how second-generation youth, living in the United Kingdom (UK)—whether directly or indirectly affected by FGM—interpreted and understood the practice. We followed a two-stage process. Stage one involved recruiting and training young people to become co-researchers, and stage two involved the co-researchers conducting focus groups and interviews with their peers. The focus of this paper is stage one. Nine young people aged 15–18, attended a 10-day creative workshop training programme [7] and worked with the researcher to develop participatory methods that would be used with young people aged 13–15 at stage two of the project. The workshops employed team-building approaches and interactive learning techniques, including drawing and writing. Participants and facilitators formed relationships; participants learned new marketable skills and researchers gained new insights about FGM, from a young person's perspective. Pacific youth growing up in rural horticultural towns in Victoria, Australia, frequently experience the stigmatising labels of 'fruit picker' and 'farmworker'. Drawing on extensive fieldwork in the Sunraysia region, we focus on schools as early sites of racialisation and racism, and compare the perspectives of students and teachers. While teachers often blame 'culture' for poor academic performance, students complain of teachers' racism while also experiencing low confidence and poor self-esteem that hinder their academic progress. Underpinning these issues is the racial hierarchy in the towns, shaped by their particular histories of colonisation, migration and intercultural relationships. Current theories of rural multiculturalism tend to focus on everyday interactions and strategies to encourage social cohesion, with the inherent danger of neglecting the deeper structural issues. We argue that Pacific youth seeking to move beyond the casual and precarious farmwork undertaken by their migrant parents encounter structural barriers that begin in schools and profoundly shape their future opportunities. An issue that has preoccupied young researchers and educators is the interaction of the fields of special and intercultural education as well as the factors that lead to the coexistences of them. The purpose of this study is to examine the factors that hinder the learning process of second-generation immigrant students in Greek primary schools. The ages of the children we will refer to are 7-8 years old due to the school achievements in reading and writing that are mastered up to this age point. This study is part of a larger study, which consists of qualitative and quantitative data from two research tools: interviews and questionnaires from teachers and guardians. The results were divided into categories. One of the main ones that stand in the way of children's learning is psychological and social factors, according to 91.8% of the participants in the questionnaire. In addition, it is worth noting that 82.6% argued that the reason students have learning difficulties is due to biological reasons. In conclusion, there is a correlation between learning difficulties and intercultural education that should be studied. This article examines 'de-stigmatisation strategies' of Turkish-Dutch youth. Our in-depth interviews and observations revealed three strategies to negotiate belonging in the Netherlands, particularly to resist dominant Dutch characterisations of Turks and Muslims as backwards, disloyal and unintegrated: (1) confronting by asserting their right to cultural distinctiveness, (2) convincing by relocating cultural achievements in their heritage, and (3) contextualising: embracing ideological and political positions calibrated to country-specific contexts. We found that students' de-stigmatisation strategies – which are learnt, contested and first performed within secure in-group settings – mobilise multiple, context-dependent identifications. Although students are often critical of the assumptions embedded in Dutch nativist discourse, their strategies also partly reproduce them, showing the pervasiveness of nativism within current political debates on culture, identity, belonging and nationality. Aims: Previous studies on major depressive disorder (MDD) among immigrants have reported mixed results. Using data from primary healthcare settings in Sweden, we compared the incidence of MDD among first- and second-generation immigrants aged 15–39 years with natives. Methods: This was a retrospective nationwide open cohort study. Eligible individuals were born 1965–1983, aged 15–39 years at baseline, and resided in Sweden for at least one year during the study period 2000–2015. We identified MDD cases through the Primary Care Registry (PCR). The follow-up for each individual started when they met the inclusion criteria and were registered in the PCR and ended at MDD diagnosis, death, emigration, moving to a county without PCR coverage, or the end of the study period, whichever came first. Results: The final sample included 1,341,676 natives and 785,860 immigrants. The MDD incidence rate per 1000 person-years ranged from 6.1 (95% confidence intervals: 6.1, 6.2) to 16.6 (95% confidence intervals: 16.2, 17.0) in native males and second-generation female immigrants with a foreign-born father, respectively. After adjusting for income, the MDD risk did not differ substantially between first-generation male and female immigrants and natives. However, male and female second-generation immigrants had a 16–29% higher adjusted risk of MDD than natives. Conclusions: This cohort study using primary healthcare data in Sweden, albeit incomplete, indicated that second-generation immigrants seem to be at a particularly high risk of MDDs. Objectives: We assessed the efficacy of cognitive behavioral therapy and bupropion compared to cognitive behavioral therapy alone for methamphetamine use disorder. Methods: The selection criteria for this systematic review study with meta-analysis were randomized clinical trials on the efficacy of cognitive behavioral therapy and bupropion in the treatment for methamphetamine use disorder (assessed by urine metabolites). The search was conducted in PubMed, PubMed Central, LILACS, SciELO, Cochrane Library, SCOPUS, Google Scholar, Ovid Medline, Clinicaltrials.gov, and the International Clinical Trials Registry Platform. The primary outcome was relapse. Risk of bias was assessed with the RoB 2 tool. The results of each clinical trial were input into an Excel spreadsheet. We performed a meta-analysis using relative risk and a 95%CI. Results: Of the 597 initial articles (498 after removing duplicate records), five were included in the meta-analysis, with an aggregate sample of 539 patients. An overall relative risk of 0.91 (95%CI 0.78- 1.05) was estimated for relapse. Conclusion: Our study limitations included publication bias and heterogeneous populations. We found no evidence that cognitive behavioral therapy and bupropion reduced the risk of relapse compared to cognitive behavioral therapy and placebo. [ABSTRACT FROM AUTHOR]

The phenomenon of majority-minority cities refers to increasingly ethnically diverse neighbourhoods where everybody belongs to a minority group.Simultaneously, another phenomenon parallels this majority-minority formation and that is gentrification, leading to an inflow of white middle-class residents into neighbourhoods which were previously predominantly occupied by residents with a migration background. Neighbourhoods that 30 years ago experienced a 'white-flight' and were subsequently almost exclusively inhabited by migrants and their descendants are now experiencing a 'comeback' of young urban professionals and middle-class young families who purchase (un)affordable housing in these gentrifying neighbourhoods. This creates an interesting condition for not only the migrants, but also their descendants who were often born and raised in these neighbourhoods and have lived there for over 30 years. Using Elias and Scotson's established–outsiders model, this article shows that, while claiming the neighbourhood as their own, descendants of migrants are experiencing a loss of their established status and feeling like an outsider in the face of the changes brought about by gentrification. Using semi-structured interviews with residents with and without a migration background, this article shows how gentrification has different implications for people living in majority-minority settings at the intersections of social class and ethnicity. Asian Americans are generally understood as a success story: scholars point out the incredible educational leaps in the second-generation. As a result, research on Asians is often devoted to explaining why mobility occurs. A recent explanation points to immigrant hyper-selectivity: post-1965 Asian immigrants come to the U.S. highly educated. Yet, research has shown divergent patterns among two of the largest hyper-selected Asian groups, Chinese and Filipinos: while the former achieves tremendous educational gains in the second-generation, the latter experiences stagnation. Drawing on data from the National Longitudinal Study of Adolescent Health, the following study examines why current hyper-selectivity literature fails to explain the two groups' disparate outcomes. Findings point to unequal levels of hyper-selectivity between Chinese and Filipinos, largely due to their opposing gendered migrations. Addressing the absence of second‐generation exiles from Southern Cone post‐dictatorship memory scholarship, this paper compares two documentaries: Hora Chilena, about the British‐Chilean community and Tus padres volverán, depicting Uruguayan exiles across Europe – both made by and/or about the no retornadxs (those who did not return to their countries of origin after dictatorship). The paper deploys documentary to offer a nuanced depiction of the hijxs del exilio (children of exile), finding them to be distinct to both the protagonist generation and their second‐generation peers in the Southern Cone. This article explores how diaspora youth is impacted by and deals with the legacies of the violent conflict in their parents' homeland of Bosnia. Based on the analysis of narrative-biographic interviews with second-generation Bosnians in Switzerland and ethnographic observations in diaspora spaces, this paper highlights the repercussions that Yugoslav disintegration wars and, particularly, the war in Bosnia (1992–1995) have for young people who were raised in Switzerland. First, it demonstrates that instead of a coherent picture about the past, young people receive fragments and pieces of personal memories and experiences that their families went through. Second, the history of conflict has repercussions for the ways young people establish and articulate their sense of attachment and belonging to the homeland of their parents. Third, while second-generation Bosnians rarely reproduce the conflict dynamic themselves, they move in spaces in which the legacies of conflict continue to be an important discursive and structural force. BackgroundMental disorders are a leading cause of global disability, driven primarily by depression and anxiety. Most of the disease burden is in Low and Middle Income Countries (LMICs), where 75% of adults with mental disorders have no service access. Our research team has worked in western Kenya for nearly ten years. Primary care populations in Kenya have high prevalence of Major Depressive Disorder (MDD) and Posttraumatic Stress Disorder (PTSD). To address these treatment needs with a sustainable, scalable mental health care strategy, we are partnering with local and national mental health stakeholders in Kenya and Uganda to identify 1) evidence-based strategies for first-line and second-line treatment delivered by non-specialists integrated with primary care, 2) investigate presumed mediators of treatment outcome and 3) determine patient-level moderators of treatment effect to inform personalized, resource-efficient, non-specialist treatments and sequencing, with costing analyses. Our implementation approach is guided by the Exploration, Preparation, Implementation, Sustainment (EPIS) framework.Methods/designWe will use a Sequential, Multiple Assignment Randomized Trial (SMART) to randomize 2710 patients from the outpatient clinics at Kisumu County Hospital (KCH) who have MDD, PTSD or both to either 12 weekly sessions of non-specialist-delivered Interpersonal Psychotherapy (IPT) or to 6 months of fluoxetine prescribed by a nurse or clinical officer. Participants who are not in remission at the conclusion of treatment will be re-randomized to receive the other treatment (IPT receives fluoxetine and vice versa) or to combination treatment (IPT and fluoxetine). The SMART-DAPPER Implementation Resource Team, (IRT) will drive the application of the EPIS model and adaptations during the course of the study to optimize the relevance of the data for generalizability and scale -up.DiscussionThe results of this research will be significant in three ways: 1) they will determine the effectiveness of non-specialist delivered first- and second-line treatment for MDD and/or PTSD, 2) they will investigate key mechanisms of action for each treatment and 3) they will produce tailored adaptive treatment strategies essential for optimal sequencing of treatment for MDD and/or PTSD in low resource settings with associated cost information - a critical gap for addressing a leading global cause of disability.Trial registrationClinicalTrials.gov NCT03466346, registered March 15, 2018.

The efficacy of antidepressant treatment for depression is controversial due to the only modest superiority demonstrated over placebo. However, neurobiological heterogeneity within depression may limit overall antidepressant efficacy. We sought to identify a neurobiological phenotype responsive to antidepressant treatment by testing pretreatment brain activation during response to, and regulation of, emotional conflict as a moderator of the clinical benefit of the antidepressant sertraline versus placebo. Using neuroimaging data from a large randomized controlled trial, we found widespread moderation of clinical benefits by brain activity during regulation of emotional conflict, in which greater downregulation of conflict-responsive regions predicted better sertraline outcomes. Treatment-predictive machine learning using brain metrics outperformed a model trained on clinical and demographic variables. Our findings demonstrate that antidepressant response is predicted by brain activity underlying a key self-regulatory emotional capacity. Leveraging brain-based measures in psychiatry will forge a path toward better treatment personalization, refined mechanistic insights and improved outcomes.

BACKGROUND:Seasonal affective disorder (SAD) is a seasonal pattern of recurrent major depressive episodes that most commonly occurs during autumn or winter and remits in spring. The prevalence of SAD ranges from 1.5% to 9%, depending on latitude. The predictable seasonal aspect of SAD provides a promising opportunity for prevention. This review - one of four reviews on efficacy and safety of interventions to prevent SAD - focuses on second-generation antidepressants (SGAs). OBJECTIVES:To assess the efficacy and safety of SGAs (in comparison with other SGAs, placebo, light therapy, melatonin or agomelatine, psychological therapies or lifestyle interventions) in preventing SAD and improving patient-centred outcomes among adults with a history of SAD. SEARCH METHODS:We searched Ovid MEDLINE (1950- ), Embase (1974- ), PsycINFO (1967- ) and the Cochrane Central Register of Controlled Trials (CENTRAL) to 19 June 2018. An earlier search of these databases was conducted via the Cochrane Common Mental Disorders Controlled Trial Register (CCMD-CTR) (all years to 11 August 2015). Furthermore, we searched the Cumulative Index to Nursing and Allied Health Literature, Web of Science, the Cochrane Library, the Allied and Complementary Medicine Database and international trial registers (to 19 June 2018). We also conducted a grey literature search and handsearched the reference lists of included studies and pertinent review articles. SELECTION CRITERIA:For efficacy, we included randomised controlled trials (RCTs) on adults with a history of winter-type SAD who were free of symptoms at the beginning of the study. For adverse events, we planned to include non-randomised studies. Eligible studies compared a SGA versus another SGA, placebo, light therapy, psychological therapy, melatonin, agomelatine or lifestyle changes. We also intended to compare SGAs in combination with any of the comparator interventions versus placebo or the same comparator intervention as monotherapy. DATA COLLECTION AND ANALYSIS:Two review authors independently screened abstracts and full-text publications, extracted data and assessed risk of bias of included studies. When data were sufficient, we conducted random-effects (Mantel-Haenszel) meta-analyses. We assessed statistical heterogeneity by calculating the Chi2 statistic and the Cochran Q. We used the I2 statistic to estimate the magnitude of heterogeneity. We assessed publication bias by using funnel plots.We rated the strength of the evidence using the system developed by the GRADE Working Group. MAIN RESULTS:We identified 3745 citations after de-duplication of search results and excluded 3619 records during title and abstract reviews. We assessed 126 full-text papers for inclusion in the review, of which four publications (on three RCTs) providing data from 1100 people met eligibility criteria for this review. All three RCTs had methodological limitations due to high attrition rates.Overall, moderate-quality evidence indicates that bupropion XL is an efficacious intervention for prevention of recurrence of depressive episodes in people with a history of SAD (risk ratio (RR) 0.56, 95% confidence interval (CI) 0.44 to 0.72; 3 RCTs, 1100 participants). However, bupropion XL leads to greater risk of headaches (moderate-quality evidence), insomnia and nausea (both low-quality evidence) when compared with placebo. Numbers needed to treat for additional beneficial outcomes (NNTBs) vary by baseline risks. For a population with a yearly recurrence rate of 30%, the NNTB is 8 (95% CI 6 to 12). For populations with yearly recurrence rates of 50% and 60%, NNTBs are 5 (95% CI 4 to 7) and 4 (95% CI 3 to 6), respectively.We could find no studies on other SGAs and no studies comparing SGAs with other interventions of interest, such as light therapy, psychological therapies, melatonin or agomelatine. AUTHORS' CONCLUSIONS:Available evidence indicates that bupropion XL is an effective intervention for prevention of recurrence of SAD. Nevertheless, even in a high-risk population, three out of four people will not benefit from preventive treatment with bupropion XL and will be at risk for harm. Clinicians need to discuss with patients advantages and disadvantages of preventive SGA treatment, and might want to consider offering other potentially efficacious interventions, which might confer a lower risk of adverse events. Given the lack of comparative evidence, the decision for or against initiating preventive treatment of SAD and the treatment selected should be strongly based on patient preferences.Future researchers need to assess the effectiveness and risk of harms of SGAs other than bupropion for prevention of SAD. Investigators also need to compare benefits and harms of pharmacological and non-pharmacological interventions.

BACKGROUND:Recent studies reveal an association between slow-wave sleep (SWS), amyloid-β aggregation, and cognition. OBJECTIVE:This retrospective study examines whether long-term use of trazodone, an SWS enhancer, is associated with delayed cognitive decline. METHODS:We identified 25 regular trazodone users (mean age 75.4±7.5; 9 women, 16 men) who carried a diagnosis of Alzheimer's dementia, mild cognitive impairment, or normal cognition, and 25 propensity-matched trazodone non-users (mean age 74.5±8.0; 13 women, 12 men), accounting for age, sex, education, type of sleep deficit (hypersomnia, insomnia, parasomnia), diagnosis, and baseline Mini-Mental State Examination (MMSE). Longitudinal group differences in cognitive testing were evaluated through repeated measures tests over an average inter-evaluation interval of four years. RESULTS:Trazodone non-users had 2.6-fold faster decline MMSE (primary outcome) compared to trazodone users, 0.27 (95% confidence interval [CI]: 0.07-0.48) versus 0.70 (95% CI: 0.50-0.90) points per year (p = 0.023). The observed effects were especially associated with subjective improvement of sleep complaints in post-hoc analyses (p = 0.0006). Secondary outcomes of other cognitive and functional scores had variable worsening in non-users and varied in significance when accounting for co-administered medications and multiple comparisons. Trazodone effects on MMSE remained significant within participants with AD-predicted pathology, with 2.4-fold faster decline in non-users (p = 0.038). CONCLUSIONS:These results suggest an association between trazodone use and delayed cognitive decline, adding support for a potentially attractive and cost-effective intervention in dementia. Whether the observed relationship of trazodone to cognitive function is causal or an indirect marker of other effects, such as treated sleep disruption, and if such effects are mediated through SWS enhancement requires confirmation through prospective studies.

The terrorist attacks of January 2015 in Paris prompted intense public debates concerning the relationship between Muslims and core elements of French republican values such as free speech and laïcité. The voices of Muslims themselves were heard relatively little in French media coverage of these matters. Drawing on a sociological study of eight second-generation members of an Algerian immigrant family, this article shows how their first-hand testimony illustrates the diversity of attitudes among French Muslims. Female members of the family who had built successful careers participated in the huge January 11 rally condemning the terrorist attacks and supporting free speech. Brothers with poorer employment records expressed more cynical attitudes, including in some cases supporting the claim that Charlie Hebdo journalists had been "asking for it" by publishing highly provocative cartoons which many Muslims considered to be Islamophobic. A sister working as an employment counsellor in a disadvantaged multi-ethnic banlieue with high levels of unemployment recounted how the disaffected and in some cases increasingly unhinged attitudes displayed in recent years by those she was charged with helping resembled those of the young men who had perpetrated the jihadist attacks of January 2015. This article is a conversation between Judy Goldhill and Fay Ballard, two second-generation artists, respectively Jewish and non-Jewish. Goldhill's parents escaped from Nazi Germany, whilst Ballard's father, the novelist J.G. Ballard, was interned in a Japanese prison camp. Goldhill works on photography, film and artists' books and Ballard draws. They have collaborated extensively, following a conversation in 2016 about a shared experience of generational trauma and early parental loss. Both artists have been using recovered objects, photographs and letters in their artwork and this collaboration has led to several joint exhibitions including Breathe (Freud Museum, 2018), Travelling Companions (Alison Richard Building (ARB), University of Cambridge, 2020–21) and Mending the Psyche (Peltz Gallery, Birkbeck, University of London, 2022). This article explores the experience of 1.5 and second-generation Central American women growing up and living in Canada. Specifically, the study explores the gendered expectations of caring and the impact of culture and social context in shaping the roles of Central American daughters within their families and community. Based on twelve one-on-one interviews with female participants, the findings revealed that Central American women’s upbringings emphasized the importance of family and that mothers played a significant role in their daughters’ lives. Further, participants expressed that they were often confronted with complex family histories of intergenerational trauma and a disproportionate expectation for caregiving. Participants shared that they were often expected to take on a caregiver role to all family members with implications for their personal lives. While Canadianraised daughters were aware of the gendered practice and emotional impact of caretaking, they remained committed to their familial obligation. [ABSTRACT FROM AUTHOR]

Belonging is a human need, one that is particularly difficult for children of immigrants to achieve. The Second-generation youth hangs in a balance between the culture of their parents and that of their friends and peers. Often these youth feel that they are not heard or seen by those around them, enhancing the feeling of not belonging in their communities, or even in their country. Therapeutic interventions could potentially create a "third space" in which they feel a sense of belonging. This paper explores the complexity in the experience of belonging in second-generation Latinx youth through a review of the literature, psychodynamic theories, a composite clinical case example and the reflections and implications to and from Second-generation Latina therapist. OBJECTIVE:The aim of the study was to identify associations between improvement in genitourinary symptoms of menopause (GSM) and vaginal microbiota, vaginal glycogen, and serum estrogen. METHODS:Thirty postmenopausal women enrolled in a hot flash treatment trial (oral estradiol vs venlafaxine vs placebo) who reported GSM and provided vaginal swabs at 0, 4, and 8 weeks were studied. Bacterial communities were characterized using deep sequencing targeting the 16S rRNA gene V3-V4 region. Participants selected a most bothersome genitourinary symptom (dryness, discharge, pain, itch/burn, or inability to have sex) and rated severity on a 10-point scale at baseline and 8 weeks. Vaginal glycogen and serum estradiol and estrone were measured at enrollment and 8 weeks. Comparisons according to improvement in most bothersome symptom (MBS) were made using χ, Wilcoxon signed-rank test, or Hotelling's t test. RESULTS:Of 30 participants, 21 (70%) had improvement in MBS over the 8-week study and 9 (30%) had no improvement or worsening of MBS. A higher proportion of women receiving estradiol or venlafaxine reported improvement in MBS (88%, 78%) compared with placebo (54%; P = 0.28). MBS improvement was associated with Lactobacillus-dominant vaginal microbiota at enrollment (57% vs 22%, P = 0.08). Vaginal glycogen, serum estradiol, and estrone significantly increased in women whose MBS improved. CONCLUSIONS:A larger proportion of women whose MBS improved had a Lactobacillus dominant microbiota at enrollment than those who had no improvement during the trial, though this difference was not statistically significant. Larger trials are needed to determine whether vaginal microbiota modify or mediate treatment responses in women with GSM.

ObjectiveNonspecific factors, such as treatment outcome expectancy and working alliance, can influence treatment outcome. No studies to date have examined the role of expectancy and alliance on pharmacotherapy outcomes in individuals with complicated grief (CG).MethodThis secondary analysis of a larger randomized, control trial (RCT) examined the relationship between pharmacotherapy expectancy and alliance on treatment outcome in adults with CG who were participating in a multisite, double-blind, RCT examining the efficacy of citalopram and complicated grief treatment (CGT). Participants (n = 202) were randomized to one of four treatment conditions: citalopram (CIT), placebo (PBO), CGT + citalopram (CGT + CIT), or CGT + placebo (CGT + PBO).ResultsPharmacotherapy outcome expectancy and working alliance were higher among individuals randomized to CGT + CIT and CGT + PBO compared with CIT or PBO without CGT. Pharmacotherapy outcome expectancy was higher at Week 2 among individuals who ultimately responded to treatment compared with those who did not and among those who remained in treatment compared with those who dropped out. In contrast, working alliance did not correlate with dropout or treatment outcomes in pharmacotherapy.ConclusionsExpectancy for medication was higher among individuals randomized to receive CGT. Application of metabolic magnetic resonance imaging measures such as cerebral blood flow in translational medicine is limited by the unknown link of observed alterations to specific neurophysiological processes. In particular, the sensitivity of cerebral blood flow to activity changes in specific neurotransmitter systems remains unclear. We address this question by probing cerebral blood flow in healthy volunteers using seven established drugs with known dopaminergic, serotonergic, glutamatergic and GABAergic mechanisms of action. We use a novel framework aimed at disentangling the observed effects to contribution from underlying neurotransmitter systems. We find for all evaluated compounds a reliable spatial link of respective cerebral blood flow changes with underlying neurotransmitter receptor densities corresponding to their primary mechanisms of action. The strength of these associations with receptor density is mediated by respective drug affinities. These findings suggest that cerebral blood flow is a sensitive brain-wide in-vivo assay of metabolic demands across a variety of neurotransmitter systems in humans.

Through a close reading of the works by the young Austrian author Milena Michiko Flašar, this article explores the complex role of given, assumed, and socially imposed names for transnational subjects. For the first- and second-generation migrants in Flašar’s texts, migration and translation are both liberating and encumbering: In her first text [Ich bin], translation across linguistic and ethnic borders serves as the perfect defamiliarizing impetus for aesthetic production, while in her novel Okaasan: Meine unbekannte Mutter the plurality of names for the migrant mother inhibits the second-generation daughter’s search for a true or authentic name for herself. Nevertheless, instead of an economy of translation where one name replaces another, Flašar shows her figures gradually developing the capacity to maintain multiple names in different languages. I argue that this replaces the conventional model of individual authenticity as self-congruence with one that does not demand hypermastery of self and that bears witness to the true complexity of transnational experience. A final analysis of Flašar’s breakthrough novel Ich nannte ihn Krawatte builds on these themes to show that even non-migrants and non-minority subjects have access to this same complexity, one which in Flašar’s words infuses a wohltuende Unsicherheit [beneficial uncertainty] into even the most hegemonic society.

Study objectivesThe Menopause Strategies: Finding Lasting Answers for Symptoms and Health network conducted three randomized clinical trials (RCTs) testing six interventions treating vasomotor symptoms (VMS), and also collected self-reported sleep outcomes. A fourth RCT assessed an intervention for insomnia symptoms among women with VMS. We describe these seven interventions' effects relative to control in women with comparably severe insomnia symptoms and VMS.MethodsWe analyzed pooled individual-level data from 546 peri- and postmenopausal women with Insomnia Severity Index (ISI) ≥ 12, and ≥14 bothersome VMS/week across the four RCTs. Interventions included the following: escitalopram 10-20 mg/day; yoga; aerobic exercise; 1.8 g/day omega-3 fatty acids; oral 17-beta-estradiol 0.5-mg/day; venlafaxine XR 75-mg/day; and cognitive behavioral therapy for insomnia (CBT-I). Outcome measures were ISI and Pittsburgh Sleep Quality Index (PSQI) over 8-12 weeks of treatment.ResultsCBT-I produced the greatest reduction in ISI from baseline relative to control at -5.2 points (95% CI -7.0 to -3.4). Effects on ISI were similar for exercise at -2.1 and venlafaxine at -2.3 points. Comparably small decreases in ISI were observed with escitalopram, yoga, and estradiol. The largest reduction in PSQI from baseline was with CBT-I at -2.7 points (-3.9 to -1.5), although PSQI decreases of 1.2 to 1.6 points were significantly better than control with escitalopram, exercise, yoga, estradiol, and venlafaxine. Omega-3 supplements did not improve insomnia symptoms.ConclusionsThis study's findings support current recommendations for CBT-I as a first line treatment in healthy midlife women with insomnia symptoms and moderately bothersome VMS.

Addressing the patient's anxieties about the meaning of life and death as integral phenomena of the life cycle is not commonly discussed in the analytic literature. A central dilemma of analytic work is the effort to facilitate change in the life of a suffering patient while also bearing in mind the inescapable human destiny, i.e., the certainty of death. All too often, these factors remain in the background while dealing with other conflicts or misfortunes. The analyst, like the patient, is aware of the brevity of life and is susceptible to the same fate, and thus may feel defenseless, helpless, and limited in what can be offered. This article incorporates the existentialist thinking of Camus on the subject and illuminates it with analytic work with unspoken trauma of a second-generation Holocaust survivor. Transnational organizations are primarily studied from the perspective of first-generation migrants and how cross-border interactions shape their identities. This study focusses on transnational activism among the second-generation, the children of refugees, asking how refugee status affects second-generation transnationalism. By analysing the case study of Vietnamese refugees in the United States, the largest refugee re-settlement effort in American history, the article analyses an originally curated database of transnational organizations (n = 632), as well as a survey of 80 organizational leaders. Findings show that participation in transnational organizations offers new ways of understanding one's identity and social mobility in the United States and in Vietnam. This transnational point of reference includes transnational labour market opportunities and the transforming of identities from learning about silenced histories in refugee families. Transnational activism creates an alternative reference category for integration outside the common reference groups including intergenerational mobility, native co-ethnics, and the general US population.
