2. Early Hospital Discharge Using Remote Monitoring for Patients Hospitalized for COVID-19, Regardless of Need for Home Oxygen Therapy: A Descriptive Study.

Aim: Since beds are unavailable, we prospectively investigated whether early hospital discharge will be safe and useful in patients hospitalized for COVID-19, regardless of their need for home oxygen therapy. Population and Methods: Extending the initial inclusion criteria, 62 patients were included and 51 benefited from home telemonitoring, mainly assessing clinical parameters (blood pressure, heart rate, respiratory rate, dyspnea, temperature) and peripheral saturation (SpO2) at follow-up. Results: 47% of the patients were older than 65 years; 63% needed home oxygen therapy and/or presented with more than one comorbidity. At home, the mean time to dyspnea and tachypnea resolutions ranged from 21 to 24 days. The mean oxygen-weaning duration was 13.3 ± 10.4 days, and the mean SpO2 was 95.7 ± 1.6%. The nurses and/or doctors managed 1238 alerts. Two re-hospitalizations were required, related to transient chest pain or pulmonary embolism, but no death occurred. Patient satisfaction was good, and 743 potential days of hospitalization were saved for other patients. Conclusion: The remote monitoring of vital parameters and symptoms is safe, allowing for early hospital discharge in patients hospitalized for COVID-19, whether or not home oxygen therapy was required. Abstract Background During the COVID-19 first wave in France, the capacity of intensive care unit (ICU) beds almost doubled, mainly because of the opening of temporary ICUs with staff and equipment from anaesthesia. Objectives We aim to investigate if the initial management in temporary ICU is associated with a change in ICU mortality and short-term prognosis. Design Retrospective single-centre cohort study. Setting Surgical ICU of the Bichat Claude Bernard University Hospital during the COVID-19 “first wave” (from 18 March to 10 April 2020). Patients All consecutive patients older than 18 years of age with laboratory-confirmed SARS-CoV-2 infection and/or typical radiological patterns were included during their first stay in the ICU for COVID-19. Intervention Patients were admitted to a temporary ICU if no room was available in the classical ICU and if they needed invasive mechanical ventilation but no renal replacement therapy or Extracorporeal Membrane Oxygenation (ECMO) in the short term. The temporary ICUs were managed by mixed teams (from the ICU and anaesthesiology departments) following a common protocol and staff meetings. Main outcome measure ICU mortality Results Among the 59 patients admitted, 37 (62.7%) patients had initial management in the temporary ICU. They had the same characteristics on admission and the same medical management as patients admitted to the classical ICU. ICU mortality was similar in the 2 groups (32.4% in temporary ICUs versus 40.9% in classical ICUs; p=0.58). SAPS-II and ECMO use were associated with mortality in multivariate analysis but not admission to the temporary ICU. 6. Do Hospital Workers Feel They Are Ready to Manage a Sanitary Crisis in a Pre-Crisis Context?

Objective: The aim of this study was to measure the perception of readiness to manage a sanitary crisis for hospital workers and to study the factors related to this perception. Methods: This study was a cross-sectional study; 408 French hospital workers responded to an online questionnaire. The variables studied concerned the perceived personal preparedness, the perception of colleagues' and hospital's preparedness, perception of the situation, and preparatory behavioral acts. Correlations, partial correlations, and multiple linear regressions were applied. Results: Based on Pearson's correlations, the higher the participants' sense of personal efficacy and control over their behavior, the more ready they feel (r
p = 0.77*** and rp = 0.55***). The more participants perceive their colleagues as ready and their hospital as prepared, the more ready they feel (rp = 0.52*** and rp = 0.46***). Based on Pearson's partial correlations, upon controlling the effect of preparedness perception, declared preparedness is not significantly correlated with personal readiness perception (rp = 0.01). Conclusion: The perception of personal readiness does not depend only on actual preparedness but also on individual and collective variables. Technically, these results confirm the value of relying on psychosocial variables during training. It would be interesting to propose empowerment in training courses. Before the COVID-19 pandemic, the SME sector in Romania was on an upward trend. In this article we set out to analyse the evolution of SMEs in Romania in the period 2017-2020, in the context of the pandemic. In order to do this, we tried to analyse in parallel the economic situation of European SMEs compared to the Romanian ones. The research begins with an analysis of the economic situation created by the health crisis due to the SarsCov-19 virus. It analyses the evolution of macroeconomic indicators in Romania during the pandemic, a pandemic that caused the global economic crisis. We will show how the most affected sectors of the national economy were: transport, hospitality, agriculture, the medical system, the education system, light industry and non-food domestic trade. The research will show that in all the analysed economic sectors, difficulties related to access to capital for entrepreneurs, supply or sales chains were the most affected by the crisis. Because of the new level of uncertainty arisen new burocratic barriers and a limited capacity for adapting appeared in the new financial-economic context. The research will highlight that in the case of SMEs in Romania, the impact of the pandemic was felt in a significant way, the economic blockade and the social distancing of the population led to major decreases in income and turnover for entrepreneurs / managers. From the collection of diaries according to a specific questionnaire frame filled by volunteers (project called “Récits Confinés”), this article questions the representation of time and space in the particular context of the two confinements of spring and autumn 2020. The March 2020 confinement was characterized by the deceleration of daily routine rhythm with limitation on movements, interruption of transportation to go to work or to school and a multiplicity of social times limited to the domestic sphere: professional time, learning time, domestic time, free and leisure time. Through our anthropological approach and sociology of everyday life, our article shows that confinement time can be perceived either as an appeasement time (far from daily rhythms and movements sometimes endured) but also as a time of tensions, in the household or within the same person, according to the season. Confinement leads to using the temporality of the event differently: going back to the past; contemplating the passing of time; speeding up daily actions; reconfiguring routines in order to “succeed” this novel social experience. Confinement time is also the time of rupture (daily and professional rhythms) which announces a renewal, a contemplated time, a moment to step back and take stock. more...

Background: COVID-19 sanitary crisis is associated with emotional difficulties such as depression, anxiety and reactional post-traumatic symptoms among healthcare workers. Indeed, healthcare workers were particularly exposed to COVID-19 sanitary crisis. This study aimed to investigate the effects of exposure to COVID-19 sanitary crisis on affective symptoms (anxiety, post-traumatic stress, burnout) among French healthcare workers and the mediating role of cognitive emotion regulation strategies (positive re-evaluation and set in perspective) and coping strategies (active coping, planning, instrumental support, emotional support, emotional expression, positive reappraisal, acceptance, denial, blame, humor, religion, distraction, substance use, behavioral disengagement).Method: This cross-sectional survey-based study collected demographic data and mental health measurements from 1,010 volunteers (838 women) who consented online to participate, from March 24 to June 28, 2020, in France. Participants filled out online questionnaires and visual analogic scales that evaluate affective symptoms related to the COVID-19 sanitary crisis, namely symptoms of post-traumatic stress, burnout, emotion regulation abilities, and coping abilities.Results: The majority (57.8%) of the participants presented post-traumatic symptoms. Depending on the sub-dimensions evaluated, a proportion of participants reported moderate (25.9–31.2%) to severe (17.2–40.7%) burnout symptoms. We found a significant effect of the level of exposure to COVID-19 on affective symptoms. Being a woman, having a lower job position and having less experience were associated with higher level of affective symptoms. Moreover, coping strategies had a mediating effect on the relation between stress and burnout, supporting the coping reserve model.Conclusion: Post-traumatic and burnout symptoms were highly prevalent among French healthcare workers at the beginning of the COVID-19 crisis. Exposure to COVID-19 is a determining factor. We can thus promote both coping training and a good environment to limit the emotional consequences of exposure to COVID-19. more...

Les tensions récurrentes sur la disponibilité des extraits de venins ainsi que la pandémie COVID-19 ont amené le Groupe de Travail Insectes Piqueurs de la SFA/Anaforcal à établir des recommandations sur l'immunothérapie allergénique (ITA) aux venins d'hyménoptères en accord avec les recommandations internationales, pour pouvoir proposer et maintenir l'ITA aux patients avec risque vital. Les modalités du diagnostic d'allergie aux venins d'hyménoptères, les indications d'ITA et de sa durée, la fréquence des rappels, le choix des doses, l'interchangeabilité des extraits de venins et le suivi des patients en ITA ont été adaptées à ce temps de crise, permettant aux allergologues de soigner au mieux leurs patients avec un minimum de risque et de contrainte. Ces recommandations sont temporaires et seront à revoir après retour à une situation normale. Recurrent venom extracts shortages as well as the COVID-19 pandemic have prompted the SFA/Anaforcal Stinging Insects Task Force to develop recommendations on venom immunotherapy (VIT), in accordance with international guidelines, to be able to offer and maintain VIT to patients with life-threatening risks. How to diagnose allergy to hymenoptera venoms, indications and duration of VIT, maintenance intervals and doses, interchangeability of venom extracts and monitoring of patients on VIT have been adapted to this time of crisis, allowing clinicians to treat their patients with the most benefit and lowest constraint. The global sanitary crisis generated by the Covid-19 pandemic (officially declared on March 11, 2020) has revealed the incapacity of the international governance, starting with the WHO, the insufficiency and inadequacy of the interstate regulations, and the displacement of the efforts in managing the sanitary response towards the states. The provisions of the IHR (especially regarding the mandatory preemptive alarm and notification) have not been totally respected, and the international solidarity leaves much to be desired. Amongst the progresses registered in practice and the official declarations, the consolidation of the significations of the concepts of global health, one health, international sanitary security, or access to vaccination, as a fundamental aspect of the right to health, hold an important place. The impact on fundamental human rights has favored special developments and resonances. The idea of a special development and affirmation of an international law of health, as a branch of public international law and original scientific field, has been consolidated by the jurisprudential codification of the international law of pandemics made by the IDI, and the WHA initiative to elaborate and adopt an international treaty on prevention and response to pandemics. The analysis of impact of sanitary crisis for European countries is nowadays one of the most discussed subjects. In this research, 19 European countries are analyzed in order to identify and compare the effect of pandemic measures for the economy. Principal components analysis is used to reduce the number of variables from 19 indicators (about labor, trade, GDP, consumption and others) to 7 new variables, while K-means algorithm is used to group the countries in 3 major classes. The seaside village of Mahahual (located in the South of the Yucatan peninsula in Mexico) has been impacted by the economic and social consequences of the COVID-19 crisis. This sanitary crisis has been having some repercussions on the economy of the tertiary sector; this village on the Caribbean coast works in majority thanks to tourism, in particularly with the cruise-ship industry. The 14th of mars 2020, the Costa Maya port announced that not a single boat would moor anymore; two weeks after the transmission of this information, the mayor proceeded to the temporary closure of the entrance of the village with the aim that no tourists and no one from outside the community may enter.Some adaptation strategies have been implemented by the tertiary workers and by the owners of tourism establishments; this unprecedented situation of “no tourism” builds some new exchanges spaces and solidarity – inside the community – that we will report in this article. more...

Este artigo tem como objetivo compreender a expansão do segmento público e privado de saúde para a gestão da população de territórios periféricos da cidade de São Paulo (Zona Sul e Zona Leste). Esta gestão é baseada nas margens do Estado periférico capitalista, vendo a crise sanitária, a partir de 2020, como acentuação dos elementos genéricos presentemente em ação num contexto de individualização das políticas sociais, revestindo-se, então, de um caráter social e estrutural. Além dos usuários entrevistados, participaram da pesquisa, dirigentes sociais e agentes do programa de atenção primária de saúde, Estratégia Saúde da Família (ESF). Assim, procura-se privilegiar a ascensão de uma triagem sistêmica, caracterizada por políticas neoliberais e uma tecnocracia em saúde que há muito negligencia a saúde pública. A crise do SARS-CoV-2 é, portanto, muito mais do que uma crise de saúde – por meio de diferentes ações e projetos governamentais, organizações e instituições sociais e mercado – mantém, pois, um vínculo estreito com a questão do acesso às clínicas médicas populares. This article aims to understand the expansion of the public and private health segment for the management of the population of peripheral territories of the city of São Paulo (South Zone and East Zone). This management is based on the margins of the capitalist peripheral state, seeing the health crisis, from 2020, as an accentuation of the generic elements currently in action in a context of individualization of social policies, thus taking on a social and structural character. In addition to the users interviewed, social leaders and agents of the primary health care program, the Family Health Strategy (ESF) participated in the research. Thus, it seeks to privilege the rise of a systemic triage, characterized by neoliberal policies and a health technocracy that has long neglected public health. The SARS-CoV-2 crisis is therefore much more than a health crisis – through different governmental actions and projects, social organizations and institutions and the market – it therefore maintains a close link with the issue of access to popular medical clinics. Este artículo tiene como objetivo comprender la expansión del segmento de salud pública y privada para la gestión de la población de los territorios periféricos de la ciudad de São Paulo (Zona Sur y Zona Este). Esta gestión se asienta en los márgenes del estado periférico capitalista, viendo la crisis sanitaria, a partir de 2020, como una acentuación de los elementos genéricos actualmente en acción en un contexto de individualización de las políticas sociales, adquiriendo así un carácter social y estructural. Además de los usuarios entrevistados, líderes sociales y agentes del programa de atención primaria de salud, la Estrategia Salud de la Familia (ESF) participó de la investigación. Así, buscar privilegiar el surgimiento de un triaje sistémico, caracterizado por políticas neoliberales y una tecnocracia sanitaria que durante mucho tiempo ha descuidado la salud pública. La crisis del SARS-CoV-2 es entonces mucho más que una crisis sanitaria – a través de diferentes acciones y proyectos gubernamentales, organizaciones e instituciones sociales y el mercado –, por lo que mantiene un estrecho vínculo con el tema del acceso a las clínicas médicas populares. Cet article vise à comprendre l'expansion du segment de la santé publique et privée pour la gestion de la population des territoires périphériques de la ville de São Paulo (Zone Sud et Zone Est). Cette gestion s'appuie sur les marges de l'État périphérique capitaliste, voyant la crise sanitaire, à partir de 2020, comme une accentuation des éléments génériques actuellement en action dans un contexte d'individualisation des politiques sociales, prenant ainsi un caractère social et structurel. En plus des usagers interrogés, des leaders sociaux et des agents du programme de soins de santé primaires, la Stratégie de santé familiale (ESF) ont participé à la recherche. Ainsi, nous cherchons à privilégier la montée d'un triage systémique, caractérisé par des politiques néolibérales et une technocratie de la santé qui a longtemps négligé la santé publique. 35. Por el amor a la libertad de expresión y a la humanidad: el periodista francés Lucien Bousquet-Deschamps en el Trienio Liberal (1820-1823)

This book presents the biography of a French journalist, famous during the years 1820-1823 both in France and Spain, and unjustly forgotten today in both countries: Lucien Bousquet-Deschamps. After establishing the date and place of his birth, family circumstances and studies, the first part introduces his early compromise with liberalism, his collaboration in the newspaper L’Aristarque and his effort, in close collaboration with the bookseller Corréard, to preserve a free press in the face of the measures taken by the French government to silence the newspapers after the assassination of the Duke of Berry, with the consequent judicial persecutions that finally forced him to flee to Spain. Next, we examine the way in which Bousquet-Deschamps was assimilated into the world of Madrid journalists and how, when this project failed, he published in Madrid a French-language newspaper, L’Écho de l’Europe. The content and the reasons for the failure of this newspaper are thus analysed. The third part is dedicated to the self-sacrifice he demonstrated during the yellow fever epidemic that devastated Barcelona in 1821, endangering his own life, and earning him the admiration of the doctors of the sanitary mission sent by the French government, who managed to get permission for him to return to his homeland. There, he was met with (as we show in the fourth chapter) the unfortunate surprise of being granted only a partial pardon, enduring a prison sentence of one year in hard conditions, which made him a victim of an absolutism incapable of true «forgiveness and oblivion». The epilogue refers to his expatriation to Alexandria and Smyrna where he maintained an important journalistic work, although we do not know how, when and where his existence ended, En este trabajo, se presenta la biografía de un periodista francés, celebérrimo du-rante los años 1820-1823 tanto en Francia como en España, y hoy total e injustamente ol-vidado en ambos países: Lucien Bousquet-Deschamps. Después de aclarar la fecha y lu-gar de su nacimiento, condiciones familiares y estudios, se presenta en una primera parte su temprano compromiso con el liberalismo, su colaboración en el periódico L’Aristarque y su lucha, en estrecha colaboración con el librero Corréard, por mantener una prensa de libre expresión pese a las medidas de amordazamiento de los periódicos tomadas por el gobierno francés después del asesinato del duque de Berry, con las consiguientes perse-cuciones judiciales que finalmente le obligaron a huir a refugiarse en España. Se examina luego cómo se integró Bousquet-Deschamps en el mundo de los periodistas madrileños en el que estuvo a punto de dirigir una nueva versión de El Constitucional, y cómo, fra-casado este proyecto, publicó en Madrid un periódico en francés L’Écho de l’Europe, del que se analiza el contenido y los motivos de su fracaso. 36. Feasibility of Ceilometers Data to Estimate Radiative Forcing Values: Application to Different Conditions around the COVID-19 Lockdown Period

In this study, the feasibility of using ceilometer signals to retrieve radiative forcing values is evaluated. The Global Atmospheric Model (GAME) radiative transfer model is used to estimate the shortwave and longwave radiative forcing using an aerosol parameterization based on AERONET data and vertical profiles from a Lufft CHM-15k Nimbus ceilometer. First, eight cases confirmed as dusty days are analyzed to check the feasibility of using ceilometer profiles to feed GAME. The obtained radiative forcing estimates are in good agreement with the literature showing negative values in the short wave (SW) (cooling effect) and positive values in the long wave (LW) (heating effect), both at all levels. As in the literature, radiative forcing estimates show a strong dependence on variations in the aerosol optical depth (AOD), solar zenith angle (θz), surface temperature (ST), and single scattering albedo at 440 nm (SSA440). Thus, GAME can be fed using ceilometer measurements obtaining reliable results. Then, as the temporal evolution of the AOD440 between 27 January and 15 June compared to the 6-year weekly AERONET AOD440 average (from 2014 to 2019) shows a decrease because of the lockdown imposed in Spain due to the COVID-19, a total of 37 radiative forcing calculations without African dust, divided into 8 scenarios, are performed in order to check the effect of the lockdown measures in the radiative forcing. It is shown that the decrease in the AOD, during the lockdown, caused a decrease in the cooling effect in the SW spectral range at all levels. Besides, the increase in the ST increased the heating effect of the aerosols in the LW at the top of the atmosphere and the presence of pollution and absorbing particles (SSA440 < 0.90) caused an increase of the heating effect in the LW at the surface. Therefore, the observed variations in the radiative forcing estimates before and during the lockdown are directly related with the decrease in emissions of aerosols related to human activities. more...

- Abstract
The notion of trials and supports, inscribed in the sociology of the individual, allows us to link complex situations of historical and structural character that affect the lives of individuals (trials) with the responses they deploy to face them (supports). Through a qualitative and exploratory study with Chileans and migrants residing in Santiago, Chile, we sought to identify, from the perception of those affected, the main problems produced and/or reinforced by the COVID-19 pandemic during 2020 and 2021. Also, the impact of these problems on their lives and how they responded to them. The results indicate that the pandemic impacted family structure and roles, the conception of work, the modes and mechanisms of interpersonal relationships, and the view of the future. These aspects vary depending on life cycle, gender, national or migrant status, employment status, and intersections between these dimensions. The main supports that emerged are the strength of character, family, and social ties, as well as the development of creativity and individual talents. In the case of the migrant population, the personal migratory project emerges as the primary support., A noção de provas e suportes, inscrita na sociologia do indivíduo, permite vincular situações complexas de caráter histórico e estrutural que afetam a vida dos indivíduos (provas) às respostas que estes desenvolvem para enfrentá-las (suportes). Mediante um estudo qualitativo e exploratório com chilenos e migrantes residentes em Santiago, no Chile, buscamos identificar, a partir da percepção dos indivíduos afetados, os principais problemas produzidos e/ou reforçados pela Pandemia do COVID-19 entre os anos 2020 e 2021, os modos em que eles hierarquizavam seu impacto e as respostas mobilizadas para responder aos mesmos. Os resultados obtidos indicam que a pandemia incidiu na estrutura e nas funções familiares, na concepção do trabalho, nas formas e mecanismos de relação interpessoal e nas projeções para o futuro, tudo isso mediado pelo ciclo de vida, pelo gênero, pela condição de nacional ou migrante, pela situação laboral e por interseções entre estas dimensões. Quanto aos suportes, aparecem a força de caráter, os vínculos familiares e sociais, o desenvolvimento da criatividade e os talentos individuais. No caso da população migrante, o projeto migratório emerge como suporteprincipal., La noción de pruebas y soportes, inscritas en la sociología del individuo, permite vincular situaciones complejas de carácter histórico y estructural que afectan la vida de los individuos (pruebas), con las respuestas que despliegan para enfrentarlas (soportes). Mediante un estudio cualitativo y exploratorio con chilenos y migrantes residentes en Santiago, Chile, buscamos identificar, desde la percepción de los afectados, los principales problemas producidos y/o reforzados por la pandemia del COVID-19 entre los años 2020 y 2021, los modos en que jerarquizaban su impacto y las respuestas movilizadas para responder a ellos. Los resultados obtenidos indican que la pandemia incidió en la estructura y funciones familiares, en la concepción del trabajo, en las formas y mecanismos de relación interpersonal y en las proyecciones hacia el futuro. Lo anterior, mediado por el ciclo de vida, el género, la condición de nacional o migrante, la situación laboral e intersecciones entre estas dimensiones. En cuanto a los soportes, aparecen la fuerza de carácter, los vínculos familiares y sociales, el desarrollo de la creatividad y los talentos individuales. The SARS-CoV-2 virus changed social reality worldwide, affecting people’s health and work life, particularly their anxiety levels. The purpose of this study is to verify the situation of women in terms of anxiety and social determinants in Spain during the pandemic. The sample consisted of 4686 people (3500 women and 1186 men). The tools used were the State-Trait Anxiety Inventory (STAI) and an ad hoc questionnaire to assess the work and mental situation of the participants. The results indicate a higher rate of anxiety among women than among men and reveal a relationship between higher levels of anxiety and more vulnerable work situations in terms of higher unemployment rates, contract changes, etc. Furthermore, there was a higher percentage of women than men in the sectors where the health crisis has had a greater impact and presence, with repercussions on the physical, mental, and social health of the entire population and especially on women. It is necessary to take into account the social determinants of health, not only at the structural level, in terms of the socio-economic and political contexts, to avoid and limit the axes of inequality such as gender. more...

The year 2020 marked the largest human pandemic in the world since the first century. Covid19 crossed the borders of China and found habitat in the whole world, paralyzing the most powerful countries of the globe, causing hundreds of thousands of victims, postponing thousands of businesses, cancelling millions of flights, keeping enclosed at home millions of people and giving the Nature the freedom to breathe, like never before. The aim of this paper is to explore how the news media and political agendas were shaped during this unprecedented crisis. Did the news media agenda reflect the overall worries of the citizens about the sources of this pandemic? Did it offer a larger view of the reality, of the ramifications and of the solutions to the locked down citizens? Does it have an impact to the political agendas? The overall findings of this research showed us that during the Covid lockdown, the news media agenda was full of information about the pandemic situation and all its economic, political and social consequences but it was also full of information about the welfares of this situation to the Nature. The visual, print and online news media offered to the public not only statistics and emotional videos from hospitals but it also offered extraordinary pictures of the Earth in pause and pictures of the Nature in peace. The news media discourse put the spotlight on the urgent need to stop the human violence towards the Nature. The political discourse shifted from elections to the pandemic situation. National audience; The 1st phase of 2020 confinement seems to have lead French consumers to reflect on their way of life and their daily practices. Through the lens of the immersion process, this research aims to study the impact of the lock-down on sustainable food practices. Three sets of data were collected over the period, allowing to analyze the profiles of change in food habits. Results show that confinement has led to more sustainable food consumption for 30% of the respondents, mainly due to an increase of local and seasonal product purchases.; La première phase de confinement de 2020 semble avoir été l’occasion d’une réflexion sur les modes de vie et de changement dans les pratiques quotidiennes. A travers les processus d’immersion, il s’agit ici d’étudier l’impact du confinement sur les pratiques alimentaires durables. International audience; Objective: The measures put in place to contain the coronavirus epidemic in France have led to fears of a massive increase in the consumption of alcohol, tobacco, and other drugs.Method: Taking advantage of an ongoing weekly Internet survey monitoring living conditions of the French population during the containment, we introduced questions on alcohol and tobacco use 1 month after the start of the first containment. Analyses were conducted with multivariate Poisson regressions.Results: Tobacco consumption remained almost unchanged. By contrast, more than a third of French people did change their weekly alcohol intake: 13% increased it, whereas 21% decreased it, indicating that the containment measures were often taken as an opportunity to reduce alcohol consumption, especially among those who drank less than 4 days per week before. Men were more likely to decrease their alcohol intake and less likely to increase it than women, probably because of a differential impact of the reduction of social activities on gendered social roles. The people most affected were smokers and those living in a dwelling in which someone had been infected, including themselves.Conclusions: The crisis tended to polarize alcohol consumption toward extremes. If the new drinking habits were to continue, the consequences could be beneficial for occasional drinkers but harmful for heavier drinkers. more...

As ameaças, riscos e desafios gerados ou agravados pela atual crise pandémica do coronavírus e a forma como temos vindo a geri-la permitem-nos retirar várias lições para melhor enfrentarmos futuras crises, sanitárias ou outras, seja a nível nacional, regional ou global. Uma crise deve ser encarada como uma oportunidade de mudança e de melhoria da eficiência e eficácia dos nossos mecanismos de gestão e resposta. Com a apresentação, neste artigo, das dez lições aprendidas com a atual crise e das soluções postas em prática nos últimos dois anos, pretendemos tão-só contribuir para esse objetivo de melhorar a nossa preparação e resiliência para enfrentar futuras crises semelhantes. As lições que elencamos visam abarcar de forma integrada o mais vasto espetro e as relações de causa-efeito entre as questões sanitárias, geopolíticas, económicofinanceiras, sociais e securitárias inerentes a uma crise de amplitude planetária. The threats, risks and challenges generated or aggravated by the current coronavirus crisis and the way we have been managing it allow us to draw several lessons to better address future crises, sanitary or others, whether at national, regional or global level. A crisis should be seen as an opportunity for change and to improve the efficiency and effectiveness of our management and response mechanisms. With the presentation in this article of the ten lessons learned from the current crisis and the solutions put in place in the last two years, we intend to contribute solely to this goal of improving our preparedness and resilience to face future similar crises. The lessons we have listed aim to include in an integrated way the broadest spectrum and the cause-and-effect relationships between the health, geopolitical, economic-financial, social and security issues inherent in a crisis of planetary magnitude. : Las amenazas, riesgos y retos generados o agravados por la actual crisis del coronavirus y la forma en que la hemos venido gestionando nos permiten extraer varias lecciones para abordar mejor las crisis futuras, sanitarias u otras, ya sea a nivel nacional, regional o global. In addition to the sanitary constrains implemented due to the pandemic, frontline physicians have faced increased workloads with insufficient resources, and the responsibility to make extraordinary clinical decisions. In 108 physicians who were at the forefront of care of patients with COVID-19 during the first two years of the pandemic, mental health, moral distress, and moral injury were assessed twice, in between two late waves of COVID-19 contagions, according to their adverse psychological reactions, in-hospital experience, sick leave due to COVID-19, quality of sleep, moral sensitivity, clinical empathy, resilience, and sense of coherence. Three months after the wave of contagions, the adverse emotional reactions and moral distress decreased, while moral injury persisted. Moral distress was related to clinical empathy, with influence from burnout and sick leave due to COVID-19, and moral injury was related to the sense of coherence, while recovery from moral distress was related to resilience. The results suggest that measures to prevent physician infection, as well as strengthening resilience and a sense of coherence, may be helpful to prevent persistent mental damage after exposure to a sanitary crisis. more...

We are actually living an endless pandemic due to a coronavirus more precisely « SRAS-COV-2 ». The current period shows us that covid-19 pandemic is endless; therefore it’s necessary for us to learn from the past with the aim to be more efficient in the future. Our main purpose is to analyze the medical residents’ experience of Grenoble during COVID-19 pandemic, during their internship; either in office or in hospital. The reporting period included november 2019 to may 2020. A qualitative study was performed with medical residents in the university of medicine of Grenoble. Six qualitative semi-structured interviews were made during january 2021 to october 2021. These interviews were analyzed according to interpretative phenomenological analysis by two investigators. It results four mains themes:the confrontation with the unknown, the crisis management wich leads to main contradictions and confrontation, humans fundamental needs during the crisis and the contribution of the crisis. Conclusion : This study shows what medical residents felt during their internship (november 2019 to may 2020). They had feelings, emotions and these depend of their kind of internship. L’objectif de cette étude était d’analyser le vécu des internes de médecine générale lors de la crise du COVID-19 dans leurs stages durant la période de novembre 2019 à mai 2020. Une étude qualitative a été menée auprès d’internes de médecine générale de la faculté de Grenoble, volontaires et intéressés ; ils ont été recrutés soit par relation en les contactant par téléphone, SMS ou par effet boule de neige en nous déplaçant dans les internats de Sallanches, Chambéry, Annecy et Grenoble. Six entretiens individuels semi-dirigés, réalisés de janvier 2021 à octobre 2021, ont fait l’objet d’une analyse interprétative phénoménologique.
