Zusammenfassung: Hintergrund: Geschlechtsunterschiede in Diagnostik und Therapie verschiedenster Erkrankungen werden zunehmend erforscht mit dem Ziel, Therapiestrategien zu optimieren und den individuellen Behandlungserfolg zu verbessern. Method: ik: In dieser Arbeit wird die bestehende Literatur zu geschlechtsspezifischen Unterschieden bei entzündlich-rheumatischen Erkrankungen zusammengefasst. Ergebnisse: Viele, aber nicht alle entzündlich-rheumatischen Erkrankungen treten häufiger bei Frauen als bei Männern auf. Frauen haben oft eine längere Beschwerdedauer bis zur Diagnosestellung als Männer, was durch unterschiedliche klinische und radiologische Erscheinungsbilder verursacht sein kann. Frauen haben krankheitsübergreifend häufiger niedrigere Remissions- und Therapieansprechraten in Bezug auf die antirheumatische Medikation als Männer, auch Therapieabbrüche sind bei Frauen häufiger als bei Männern. Ob Frauen vermehrt Anti-drug-Antikörper gegen biologische Antirheumatika entwickeln, ist noch unklar. Bei Januskinaseinhibitoren gibt es bislang keine Hinweise auf ein unterschiedliches Therapieansprechen. Schlussfolgerung: Ob auch in der Rheumatologie individuelle Dosierungsschemata und geschlechtsangepasste Therapieziele nötig sind, kann aus der bisherigen Evidenz nicht abgeleitet werden. Autoimmunerkrankungen haben spezifische pathophysiologische Mechanismen, die zu einem erhöhten Risiko für arterielle und venöse Thrombosen führen. Das Risiko einer venösen Thromboembolie (VTE) variiert je nach Art und Stadium der Erkrankung sowie der begleitenden Therapie. In dieser Übersichtsarbeit werden die häufigsten Autoimmunerkrankungen wie Anti-Phospholipid-Syndrom, entzündliche Myositis, Polymyositis und Dermatomyositis, rheumatoide Arthritis, Sarkoidose, Sjögren-Syndrom, autoimmunhämolytische Anämie, systemischer Lupus erythematodes, systemische Sklerose, Vaskulitis und chronisch-entzündliche Darmerkrankungen behandelt. Darüber hinaus geben wir einen Überblick über die Pathophysiologie, die für das VTE-Risiko bei jeder Autoimmunerkrankung verantwortlich ist, und berichten über die aktuellen Indikationen für eine gerinnungshemmende Therapie zur Primär- und Sekundärprävention von VTE. Hintergrund: Die Verfügbarkeit von Januskinase (JAK)-Inhibitoren hat die Behandlung der rheumatoiden Arthritis (RA) verändert und Patienten zu einer klinischen Remission verholfen. Es wurde jedoch auch über das Auftreten opportunistischer Infektionen (OI) im Zusammenhang mit der Anwendung von JAK-Inhibitoren berichtet. Diese strukturierte Literaturübersicht wurde erstellt, um Berichte über OI im Zusammenhang mit der Behandlung von RA mit JAK-Inhibitoren in klinischen Studien zusammenzufassen. Methoden: Eine strukturierte Suche nach relevanten klinischen Studiendaten bis März 2021 wurde in MEDLINE® und Embase® durchgeführt. Es wurden auch bibliografische Recherchen in neueren Übersichtsarbeiten durchgeführt und in grauer Literatur recherchiert, um wichtige Lücken zu schließen. Die Publikationen wurden gescreent, extrahiert und hinsichtlich ihrer Qualität bewertet. Die Daten wurden in narrativer Form zusammengefasst. Ergebnisse: Nach dem Screening wurden 105 Publikationen über 62 spezifische klinische Studien eingeschlossen, die über die Inzidenz von OI bei RA-Patienten, die mit JAK-Inhibitoren behandelt wurden, berichteten. Insgesamt wurde die höchste expositionsbereinigte Inzidenzrate für Herpes-zoster-Infektionen (alle Formen) berichtet, gefolgt von OI (alle Formen) und Tuberkulose, basierend auf begrenzten Daten aus klinischen Studien mit zugelassenen Dosen von JAK-Inhibitoren. Das Fehlen von direkten Vergleichsstudien und Unterschiede im Studiendesign lassen einen direkten Vergleich der JAK-Inhibitoren nicht zu. In der asiatischen und australischen Bevölkerung wurden im Vergleich zur Weltbevölkerung höhere OI-Raten festgestellt. Dieser Artikel fasst den Fall einer Patientin zusammen, die wegen einer Methotrexat-induzierten Kolitis im Zusammenhang mit einer Panzytopenie ins Krankenhaus eingeliefert wurde. Obwohl Methotrexat üblicherweise bei einer Vielzahl von Krankheiten eingesetzt wird, kann dieses Medikament bei einer Untergruppe von Patienten Nebenwirkungen hervorrufen. Dies ist ein atypischer Fall von chronischer Methotrexat-Einnahme in niedriger Dosierung, die zu Kolitis und Panzytopenie führte. The hip is a frequently affected joint in rheumatoid arthritis. Although the hip is rarely the site of primary manifestation, it is almost always affected during the course of the disease and, if left untreated, leads to progressive joint destruction with the development of secondary osteoarthritis. Despite greatly improved drug therapy options in recent years, especially with the introduction of biologic agents, total hip replacement continues to be a frequently performed procedure in rheumatoid patients. Due to the specific pathology of rheumatoid joint destruction, hip arthroplasty presents special challenges to the surgeon. This concerns the preoperative preparation, the surgical procedure as well as the postoperative treatment. However, taking these special features into account, the results of total hip replacement in rheumatic diseases are good and often lead to a significant improvement in mobility and quality of life. We present a case with undifferentiated, unclassified polyarthritis and with peripheral diffuse idiopathic skeletal hyperostosis (DISH).
