Background/Objectives: Inadequate fluid intake is prevalent among older adults living in care settings and can lead to dehydration-related events such as falls and hospitalization. Staff knowledge and confidence using diverse strategies is needed to provide adequate hydration to residents. PROMOTE is a multicomponent intervention designed to support staff to increase resident fluid intake between meals. This study evaluated the educational components of PROMOTE. Methods: Participants (n = 87) working in long-term care or retirement homes completed an online pre-/post-test evaluation of a 7 min educational video. Key informant participants (n = 13) reviewed all educational materials, evaluated their usefulness and feasibility, and were interviewed to identify how to improve the materials. Results: The educational video improved knowledge (e.g., self-rating of knowledge pre-test median 8, standard error of the mean (SEM) 0.18; post-test median 9, SEM 0.13) and confidence. Participants intended to use PROMOTE strategies in their work with residents (1 [very likely] to 10 [very unlikely] median 2.0 SEM 0.27). Key informants rated the hydration of residents as an organizational priority (median 9.0 SEM 0.42) and all indicated that they would use the educational video in their future training. Less feasible educational components as rated by key informants included huddle discussions and email pushes. Posters were seen as feasible (54%) but only somewhat useful (77%). Conclusions: Brief educational videos can improve staff knowledge and confidence regarding providing adequate hydration to residents. Introduction: High exercise adherence is a key factor for effective exercise programmes. However, little is known about predictors of exercise adherence to a multimodal machine‐based training in older retirement home residents. Aims: To assess exercise adherence and potential predictors of adherence. Furthermore, to evaluate user acceptance of the multimodal training and the change in exercise self‐efficacy. Methods: In this sub‐analysis of the bestform‐F study, a total of 77 retirement home residents ≥65 years (mean age: 85.6 ± 6.6 years, 77.9% female) participated in a 6‐month machine‐based resistance, coordination and endurance training. Attendance to the training was documented for each training session. To identify potential predictors a multiple linear regression model was fitted to the data. Analyzed predictors included age, sex, body mass index (BMI), physical function, exercise self‐efficacy, and physical activity history. Different domains of user acceptance (e.g. safety aspects, infrastructure) and exercise self‐efficacy were assessed by a questionnaire and the exercise self‐efficacy scale (ESES), respectively. Results: Mean exercise adherence was 67.2% (median: 74.4%). The regression model (R2 = 0.225, p = 0.033) revealed that the 6‐minute walk test (6‐MWT) at baseline significantly predicted exercise adherence (β: 0.074, 95% confidence interval (CI): 0.006–0.142, p = 0.033). Different user domains were rated at least as good by 83.9%–96.9% of participants, reflecting high acceptance. No statistically significant change was found for exercise self‐efficacy over 6 months (mean change: 0.47 ± 3.08 points, p = 0.156). Conclusion: Retirement home residents attended more than two thirds of offered training sessions and physical function at baseline was the key factor for predicting adherence. Context: Dehydration is a concern amongst older adults residing in retirement (RH) and long-term care (LTC) homes. Objectives: a) work with home team members to develop effective hydration strategies; b) implement these strategies; c) determine the capacity of home team members to provide process evaluation data on implementation, d) determine if administrative data is helpful in tracking dehydration-related events, and e) determine if a short, online education module can improve the hydration knowledge and attitudes of team members providing care. Methods: Wet your Whistle with Water (W3) included: voluntary online education module for team members; hydration reminders; water stations in common areas; and bi-monthly recreation activities providing beverages. Hydration-related administrative data from 56 LTC residents were analyzed for pre-post comparison. Findings: 218 individuals participated in the education and significant improvements in attitudes and knowledge noted. Assisted living is growing in Ontario. Medical services are not regulated in Ontario, resulting in variability of physician involvement. We described medical service provider involvement and practice characteristics in assisted living homes. Descriptive cross-sectional study. A total of 88 assisted living homes in Ontario, Canada, which responded to a survey in 2023. Administrators responded to questions on recruiting various health professionals, their involvement in the retirement home, support available, documentation in the home, and availability of palliative care. We reported absolute and relative proportions for survey items. We used regression analysis to assess if there is a statistically significant difference in the proportion of patients accessing care from the community in homes with and without a recruited medical service provider. Fifty-four (61.4%) of homes had a medical service provider, primarily an attending medical doctor. Attending medical doctors cared for more than 50 patients in 36% of homes, and 46% visited homes weekly. Administrators reported that medical providers spent most of the time providing appointments, responding to phone calls and faxes, conducting medication reviews, and discussing with residents' families. Nearly two-thirds of homes had nurses accompany physicians on rounds and provided medical service providers with clinic space and equipment. Two-thirds of homes provided residents with palliative care, primarily through community support. Residents of homes with a recruited medical service provider had 76% lower odds of seeking care from their physician in the community than those without a recruited provider (P <.001). Our findings describe high variability in recruiting medical service providers in assisted living homes and their practice characteristics. Residents may benefit from on-site accessible and patient-centered medical care. The COVID-19 pandemic has brought to the forefront profound questions surrounding dying and the concept of a "good death". This qualitative case study, conducted in a health center in Quebec, Canada, severely affected by outbreaks during the pandemic's first wave, explores end-of-life care for older adults in retirement homes. Through thirty interviews with healthcare practitioners, researchers, and managers, we investigate the critical role of a pharmacological device referred to as the "baby bottle" in providing end-of-life care to older adults infected with COVID-19 in their homes. Drawing upon the boundary object framework, we examine the ambiguities surrounding the use of this device and explores its agency. In this unprecedented context, we argue that the device facilitated a form of death that could be described as "better than nothing", embodying practitioners' efforts to provide some dignity to the dying person, by minimally controlling the distress and ensuring a connection between the dying person and the care team. Additionally, it served as a means of coping with the pandemic's intolerable aspects, such as the exclusion of frail older adults for the supposed common good. Abstract Background The prevalence of dizziness increases with age. We aimed to determine the point prevalence of dizziness and, in particular, of benign paroxysmal positional vertigo (BPPV) among retirement home residents. Furthermore, we aimed to evaluate the efficacy of a 2-axis turntable based BPPV treatment. Methods We contacted all large retirement homes in or around the city of Zurich (Switzerland). 10 retirement homes (with a total of 536 residents) agreed to participate in this study. 83 rejected inquiries by residents led to a potential study population of 453 residents. After a structured interview evaluating the presence and characteristics of dizziness, all willing patients were tested for positional vertigo and nystagmus on a portable and manually operated 2-axis turntable that was transported to the retirement home. Testing consisted of the Dix-Hallpike and supine roll maneuvers to both sides. Participants were immediately treated with the appropriate liberation maneuver whenever BPPV was diagnosed. Otherwise, taking the resident’s medical history, a neuro-otological bedside examination, and a review of the available medical documentation was used to identify other causes of dizziness. Results Out of the 453 residents, 75 (16.6%; average age: 87.0 years; 68% female) were suffering from dizziness presently or in the recent past and gave their consent to participate in this study. Among the participants tested on the turntable (n = 71), BPPV was present in 11.3% (point prevalence). Time-related properties, triggering factors and qualitative attributes of vertigo or dizziness were not significantly different between the dizzy participants with and those without BPPV. In all BPPV patients, appropriate liberation maneuvers were successful. Conclusions BPPV could be demonstrated in about one tenth of retirement home residents with dizziness or recent dizziness. Such point prevalence of BPPV translates to a much higher yearly prevalence if one assumes that BPPV is not present on every day. Our finding suggests that retirement home residents suffering from dizziness should be regularly tested for BPPV and treated with appropriate liberation maneuvers, ideally on turntable to reduce strain. La réalité virtuelle renforce l'arsenal thérapeutique en santé : prévention en santé, réadaptation fonctionnelle, douleurs chroniques et aiguës, qualité de vie en soins palliatifs, psychiatrie et santé mentale : troubles anxieux, dépression, addictions et psychoses. Notre objectif est de dégager des applications de la réalité virtuelle immersive auprès des personnes âgées institutionnalisées. La réalité virtuelle montre des propriétés distractives, antalgiques, anxiolytiques et antidépressives qui lui permettrait ainsi de répondre à de nombreux problèmes de santé invalidants chez le sujet âgé. Sa malléabilité, son ubiquité et sa simplicité d'utilisation lui confèrent une place de choix parmi les thérapies non médicamenteuses recommandées chez les personnes âgées. Si les personnes âgées institutionnalisées peuvent en bénéficier en complément ou en substitution des traitements traditionnels, il convient toutefois de l'adapter à ce public fragilisé. Our objective is showing VR using among elderly people in French retirement home. Virtual reality is showing efficacy as distraction, anti-pain, tranquillising, antidepressant methods. It should be an answer to many elderly health problems. VR is easy to use, with low side effects. Non-drug treatments among elderly people are recommended. Poor glucose regulation associated with gradual insulin resistance is a significant risk factor in several age-related chronic diseases. An eating plan that promotes a lower carbohydrate intake may have a beneficial effect on glucose metabolism. This study aimed to evaluate how a diet reduced in carbohydrate by 32% (RCHO) over a 2 month period would influence the metabolic profile of older individuals ( N  = 24) living in a retirement home (RH). A continuous glucose monitor was used to measure blood glucose during four periods: the standard diet before (baseline) and after (washout) the intervention, during the 4 initial days of the RCHO diet (RCHO-early), and the final days of the 2 month intervention (RCHO-end). The blood metabolic profile was also measured (glucose, ketones, insulin, triglycerides, and cholesterol). RCHO intake decreased average blood glucose compared to the standard diet in hyperglycemic participants: RCHO-early 7.8 ± 1.0 vs 7.5 ± 1.1 mM ( p =  0.012) and RCHO-end 7.8 ± 1.0 vs 7.0 ± 0.9 mM ( p =  0.050). In the hyperglycemic participants, the percentage of time spent in hyperglycemia (>10.0 mM) decreased by 50% during the RCHO-early ( p =  0.012) and by 66% at RCHO-end ( p   =  0.021) compared to baseline. Glycated hemoglobin was significantly lower at RCHO-end in both hyperglycemic and normoglycemic participants compared to baseline ( p  < 0.008). Plasma ketones increased threefold in hyperglycemic participants at RCHO-end compared to baseline ( p  < 0.028). Background: The COVID-19 pandemic caused widespread societal disruption, with governmental stay-at-home orders resulting in people connecting more via technology rather than in person. This shift had major impacts on older adult residents staying in retirement homes and residential care facilities, where they may lack the technology literacy needed to stay connected. The enTECH Computer Club from the University of Waterloo in Ontario, Canada created a knowledge translation toolkit to support organizations interested in starting technology literacy programs (TLPs) by providing guidance and practical tips. Objective: This paper aimed to present a framework for implementing TLPs in retirement homes and residential care facilities through expanding on the knowledge translation toolkit and the framework for person-centered care. Methods: Major concepts relating to the creation of a TLP in retirement homes and residential care facilities were extracted from the enTECH knowledge translation toolkit. The domains from the framework for person-centered care were modified to fit a TLP context. The concepts identified from the toolkit were sorted into the three framework categories: "structure," "process," and "outcome." Information from the knowledge translation toolkit were extracted into the three categories and synthesized to form foundational principles and potential actions. Results: All 13 domains from the framework for person-centered care were redefined to shift the focus on TLP implementation, with 7 domains under "structure," 4 domains under "process," and 2 domains under "outcome." Domains in the "structure" category focus on developing an organizational infrastructure to deliver a successful TLP; 10 foundational principles and 25 potential actions were identified for this category. Domains in the "process" category focus on outlining procedures taken by stakeholders involved to ensure a smooth transition from conceptualization into action; 12 foundational principles and 9 potential actions were identified for this category. Domains in the "outcome" category focus on evaluating the TLP to consider making any improvements to better serve the needs of older adults and staff; 6 foundational principles and 6 potential actions were identified for this category. Conclusions: Several domains and their foundational principles and potential actions from the TLP framework were found to be consistent with existing literatures that encourage taking active steps to increase technology literacy in older adults. Although there may be some limitations to the components of the framework with the current state of the pandemic, starting TLPs in the community can yield positive outcomes that will be beneficial to both older adult participants and the organization in the long term. Background: With aging process, cognitive levels decline which has affected nearly 50 million people worldwide. In view of this, our study aims to estimate the prevalence of cognitive impairment and its correlation with co morbidities among the elderly residing in old age homes in Southern India and determine its various risk factors among them. Methodology: This cross-sectional study was carried among 330 elderly living in old age homes by using multistage sampling method. Study population included Elderly above the age of 60 and residing in old age homes, who were willing to participate. A pretested structured questionnaire was used. Data was analysed using SPSS. Results: Of the 330 participants, nearly 44% had mild to moderate and 36% had severe cognitive impairment. The odds of developing cognitive impairment were 2.5 times more among individuals who had at least one feature of dementia when compared with those who did not have any symptoms. Conclusion: The strategies of National Programmes for Health Care of Elderly and National Mental Health Program do not cater specifically to cognitive impairment. Level of attention given towards Alzheimer’s disease is more when compared to screening for cognitive impairment which is an early precursor for Alzheimer’s disease.

The objectives of this study were to describe poisonings occurring in older or disabled patients residing in structured living facilities, identify risk factors, and propose preventive measures. This was a prospective and observational study, covering all cases of poisoning occurring in structured living facilities in the Brittany region of France. All calls were received at the Grand Ouest Poison Control Centre (PCC) of Angers and were recorded from 1 February 2019 to 31 January 2020. The clinical severity of the poisonings was assessed using the Poisoning Severity Score (PSS). Clinical severity was compared by univariate and multivariate analyses using the following dependent variables: PSS score < 2 and PSS ≥ 2. This study included 158 residents with a median age of 83 years (5–116 years old). The average number of residents supervised by a member of the supervisory staff was 11 (+/−10.7). The substance ingested was a personal hygiene product or a cleaning product in 48% and 25% of cases, respectively. The most frequently ingested product was a bar of soap (n = 20). All moderate to severe cases (4.6%, n = 8 including one death) occurred in residents with dementia living in nursing homes for elderly patients, and 9% of residents required hospital treatment. In more than 50% of cases (n = 83), the product was provided by the facility and in 23% (n = 40), it was brought in by the family. Ingestion of a bar of soap or a product brought in by the family was significantly associated with higher poisoning severity (PSS ≥ 2). After adjustment for age, sex and the number of residents per supervisor, the severity of poisoning was significantly greater after ingestion of a bar of soap (OR = 12.33, CI95 [2.12, 71.63], p = 0.005). Older adults residing in medical facilities who have a history of dementia and/or cognitive impairment are more at risk of non-medicinal product poisoning. During the COVID-19 pandemic, individuals working in therapeutic recreation (TR) roles in retirement and long-term care homes had to modify their practice in response to rapidly changing infection prevention and control regulations. The experiences of TR staff working in these settings in Ontario, Canada during the second wave of COVID-19 were investigated to better understand their challenges and share learnings for the future. A 20-item questionnaire was completed by 261 individuals (M = 35 years, range = 19-71, 95% female). Participants reported many common modifications to their roles, including a move to innovative one-on-one and small groups programming, and increased use of technology to facilitate activities and social interactions. Responses emphasize the resilience, creativity, and innovation of staff working within this space, and highlight the benefits of recreation for residents living in these homes. In an 80-bed fee-based retirement home with nursing care, the dispatched-pharmacist has provided prescription recommendations to visiting physicians based on pathogen identification using Gram staining as part of an antimicrobial stewardship program. Thus, we evaluated the effects of pharmacist-supported antimicrobial stewardship. We calculated the total number of all antimicrobials and macrolides, fluoroquinolones, and cephalosporins prescriptions per 100 residents per month at the retirement home from January 2013 to December 2017. Using log-transformed monthly resident numbers with an offset before and after the intervention, we performed Poisson regression analyses that adjusted for monthly mean age. Interrupted time series analyses (ITSA) were conducted to examine the changes in the incidence rate ratios for the baseline and slope before and after the intervention. The total number of all antimicrobial prescriptions per 100 residents per month from 2013 to 2017 was 14.10, 18.51, 10.59, 5.41, and 3.90, respectively. Although there was a significant pre-intervention increase in the total number of all antimicrobial prescriptions, the intervention was followed by a significant decrease. There was also a significant reduction in the slope. ITSA of the changes in the prescription of macrolides and fluoroquinolones showed that there were significant pre-intervention increase and followed by a significant post-intervention decrease in the slope. There was no significant change in cephalosporin prescriptions by the intervention. Our study shows that pharmacist-supported AS can reduce antimicrobial prescriptions in a retirement home. Handbook of Polish, Czech and Slovak Holocaust Fiction is a work in progress aiming at becoming a standard reference work addressed to universities and public libraries and the broader public. It includes novels, short stories, poems and plays written in Polish, Czech and Slovak within the scope of 650 standard pages. The table of contents consists of 53 articles focused on Polish, 45 articles on Czech, and 23 articles on Slovak literature. The editors provide an introduction about the main developments of Holocaust literature in the broader context of three lands: crucial topics, situations, characters, motifs and places, periodization due to political changes, reception processes in the national and transnational context. The Handbook aims primarily at the researchers and readers in Western Europe and the U.S. where the Polish, Czech and Slovak Holocaust fiction remains largely unknown. The project results from the cooperation among researchers from Polish, Czech, German and Slovak universities. This article presents two entries from Polish and Czech literature. We report a therapy cat in a nursing home in Germany infected with severe acute respiratory syndrome coronavirus 2 during a cluster outbreak in the home residents. Although we confirmed prolonged presence of virus RNA in the asymptomatic cat, genome sequencing showed no further role of the cat in human infections on site. [ABSTRACT FROM AUTHOR]

The Sociocultural Animation Project “Active Ageing in a Retirement Home” designs Animation as an integral part the daily life of the institution and its residents. Using this approach we hope to improve quality of life for residents, promoting an environment that helps elderly people live their closing years in an active and satisfactory way. We conclude that Sociocultural Animation is a mode of intervention in all aspects of the life of the elderly, and it is a permanent stimulus for mental, physical and emotional capacities. It acts in facilitating access to a more active and creative life, improves communication in relationships with others, encouraging better preparation for life in a community and it develops personal autonomy.

Background: Long-term care facilities have been widely affected by the COVID-19 pandemic. Empirical evidence demonstrated that older people are the most impacted and are at higher risk of mortality after being infected. Regularly testing care facility residents is a practical approach to detecting infections proactively. In many cases, the care staff must perform the tests on the residents while also providing essential care, which in turn causes imbalances in their working time. Once an outbreak occurs, suppressing the spread of the virus in retirement homes (RHs) is challenging because the residents are in contact with each other, and isolation measures cannot be widely enforced. Regular testing strategies, on the other hand, have been shown to effectively prevent outbreaks in RHs. However, high-frequency testing may consume substantial staff working time, which results in a trade-off between the time invested in testing and the time spent providing essential care to residents., Objective: We developed a web application (Retirement Home Testing Optimizer) to assist RH managers in identifying effective testing schedules for residents. The outcome of the app, called the "testing strategy," is based on dividing facility residents into groups and then testing no more than 1 group per day., Methods: We created the web application by incorporating influential factors such as the number of residents and staff, the average rate of contacts, the amount of time spent to test, and constraints on the test interval and size of groups. We developed mixed integer nonlinear programming models for balancing staff workload in long-term care facilities while minimizing the expected detection time of a probable infection inside the facility. Additionally, by leveraging symmetries in the problem, we proposed a fast and efficient local search method to find the optimal solution., Results: Considering the number of residents and staff and other practical constraints of the facilities, the proposed application computes the optimal trade-off testing strategy and suggests the corresponding grouping and testing schedule for residents. The current version of the application is deployed on the server of the Where2Test project and is accessible on their website. The application is open source, and all contents are offered in English and German. We provide comprehensive instructions and guidelines for easy use and understanding of the application's functionalities. The application was launched in July 2022, and it is currently being tested in RHs in Saxony, Germany., Conclusions: Recommended testing strategies by our application are tailored to each RH and the goals set by the managers.
