Diamond-like Carbon (DLC) has been used as a coating material of choice for a variety of technological applications owing to its favorable bio-tribo-thermo-mechanical characteristics. Here, the possibility of bringing DLC into orthopedic joint implants is examined. With ever increasing number of patients suffering from osteoarthritis as well as with the ingress of the osteoarthritic joints malaise into younger and more active demographics, there is a pressing need to augment the performance and integrity of conventional total joint replacements (TJRs). Contemporary joint replacement devices use metal-on-polymer articulations to restore function to worn, damaged or diseased cartilage. The wear of polymeric components has been addressed using crosslinking and antioxidants; however, in the context of the metallic components, complications pertaining to corrosion and metal ion release inside the body still persist. Through this review article, we explore the use of DLC coatings on metallic bearing surfaces and elucidate why this technology might be a viable solution for ongoing electrochemical challenges in orthopedics. The different characteristics of DLC coatings and their feasibility in TJRs are examined through assessment of tribo-material characterization methods. A holistic characterization of the coating-substrate interface and the wear performance of such systems are discussed. As with all biomaterials used in TJRs, we need mindful consideration of potential in-vivo challenges. We present a few caveats for DLC coatings including delamination, hydrophobicity, and other conflicting as well as outdating findings in the literature. We recommend prudently exploring DLC films as potential coatings on metallic TJR components to solve the problems pertaining to wear, metal ion release, and corrosion. Ultimately, we advise bringing DLC into clinical use only after addressing all challenges and concerns outlined in this article.

INTRODUCTION: The incidence of periprosthetic distal femur fractures (PDFF) is increasing as the number of total knee replacements becomes more common. This study compared the demographics, fracture characteristics, treatment, and outcomes of periprosthetic versus native distal femur fractures (NDFF). MATERIALS AND METHODS: This was a retrospective cohort study of patients ≥ 18 who underwent surgical fixation of NDFF or PDFF from 2012 to 2020 at a level-1 trauma center. The main variables collected included demographics, AO/OTA fracture classification, fixation construct, concomitant fractures, polytrauma rates, bone density, and reduction quality. Primary outcomes were unexpected return to the operating room (UROR), hospital length of stay, and quality of reduction. T-tests, Fishers exact tests, and multivariate analyses were used for statistical analysis. RESULTS: 209 patients were identified, including 70 PDFF and 139 NDFF. PDFF patients were elderly females (81%) with isolated (80%) and comminuted (85%) 33 A.3 (71%) fractures. NDFF patients included 53% females, were commonly middle-aged, and displayed comminuted (92%) 33 C.2 fractures. 48% of NDFF patients had concomitant fractures. Intramedullary nailing was the primary fixation for both groups, followed by nail-plate combination (37%) for PDFF and lateral locking plates (21%) for NDFF. PURPOSE: Hip osteoarthritis (OA) is a major cause of pain and disability worldwide. Lack of effective therapies may reflect poor knowledge on its aetiology and risk factors, and result in the management of end-stage hip OA with costly joint replacement. The Worldwide Collaboration on OsteoArthritis prediCtion for the Hip (World COACH) consortium was established to pool and harmonise individual participant data from prospective cohort studies. The consortium aims to better understand determinants and risk factors for the development and progression of hip OA, to optimise and automate methods for (imaging) analysis, and to develop a personalised prediction model for hip OA. PARTICIPANTS: World COACH aimed to include participants of prospective cohort studies with ≥200 participants, that have hip imaging data available from at least 2 time points at least 4 years apart. All individual participant data, including clinical data, imaging (data), biochemical markers, questionnaires and genetic data, were collected and pooled into a single, individual-level database. FINDINGS TO DATE: World COACH currently consists of 9 cohorts, with 38 021 participants aged 18-80 years at baseline. Overall, 71% of the participants were women and mean baseline age was 65.3±8.6 years. Over 34 000 participants had baseline pelvic radiographs available, and over 22 000 had an additional pelvic radiograph after 8-12 years of follow-up. Even longer radiographic follow-up (15-25 years) is available for over 6000 of these participants. FUTURE PLANS: The World COACH consortium offers unique opportunities for studies on the relationship between determinants/risk factors and the development or progression of hip OA, by using harmonised data on clinical findings, imaging, biomarkers, genetics and lifestyle. This provides a unique opportunity to develop a personalised hip OA risk prediction model and to optimise methods for imaging analysis of the hip.

BACKGROUND: Opioid use after revision total hip arthroplasty (rTHA) has not been well characterized. The purpose of this study was to characterize preoperative, perioperative, and postoperative opioid use during rTHA. METHODS: Patients undergoing revision THA from 2010 to 2018 were screened for opioid use 3 months before revision surgery and tracked 24 months postoperatively. Patients were categorized as naïve or tolerant. Opioid prescriptions and average morphine milligram equivalents (MME) were compared between the two groups. RESULTS: One hundred twenty-four of 247 patients (50%) in the tolerant group averaged a preoperative MME of 23.7 mg/day. Postoperatively, tolerant patients received significantly higher daily MME at all time points, including at 3 months 31.4 versus 18.1 mg/day (P < 0.001), 6 months 19.9 versus 2.95 mg/day (P < 0.001), 12 months 14.3 versus 3.5 mg/day (P < 0.001), and 24 months 10.7 versus 2.17 mg/day (P < 0.001). Tolerant patients were more likely to have a prescription at 6 months (44% versus 22%), 12 months (41.4% versus 24%), and 24 months (38% versus 19.3%) (P < 0.001, P = 0.002, P < 0.001, respectively). DISCUSSION: Opioid-tolerant patients had higher postoperative MME requirements for longer recovery duration. Both groups reduced opioid use at 3 months and plateaued at 6 months. These findings can help the revision surgeon counsel patients and expectations.

BACKGROUND: Postoperative delirium is a common problem affecting admitted patients that decreases patient satisfaction and increases the cost and complexity of care. The purpose of this study was to use the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database to compare rates and risk factors of postoperative delirium for total hip arthroplasty (THA) and hemiarthroplasty patients indicated for osteoarthritis or proximal femur fracture. METHODS: The 2021 NSQIP database was queried for patients using Current Procedural Terminology (CPT) codes for THA and hemiarthroplasty and ICD-10 codes for osteoarthritis or proximal femur fracture. Demographic, past medical history, preoperative labs, and functional status data were recorded. Procedural data were also collected. Finally, postoperative outcomes and complications were reviewed. Availability contracting is a prominent procurement strategy among many defense and other capital equipment suppliers and their customers. Under availability contracting, the revenue generated for a service provider is a function of equipment/system availability over the predetermined contract period. A major challenge in implementing availability contracting is predicting the availability of equipment, defining the target availability, and determining the best contract duration based on the target availability. In this paper, we extend the classical operational availability model for estimating equipment availability under hierarchical maintenance, which is frequently used for complex equipment such as aircraft. Under an availability contract, bounds have been proposed on system-level operational availability. The current study also develops optimization models for finding the optimal availability contract duration under different scenarios: stochastic and non-stochastic contract durations; contract written at parts level, system level; minimizing total maintenance costs with minimum level of target operational availability as a constraint etc. The study also shows that the optimal contract duration is a non-decreasing function of spares on hand, and the inherent availability of system part. In this paper, a new repairable k-out-of-n system model is proposed, in which the deviation cost of the system is taken into account. Firstly, the standard k-out-of-n system is replaced at planned time or at the failure time. Next, replacement policies are studied at planned time or periodic time when k is a constant. Thirdly, replacement policies are studied at planned time or periodic time when k is a random number, respectively. Finally, replacement policies are planned at the completion of random working time. The expected cost rate functions and its optimum replacement policies are derived analytically for each model. Meanwhile, optimum number of units is designed. Background: Tracheoesophageal speech is one of the most effective method used for voice rehabilitation after laryngectomy. The main limitation is the need for periodic voice prothesis (VP) replacements. The process of developing VP usage complications is still unexplored. The aim of this study was to assess the level of cytokines (IL‐1β, IL‐6, IL‐8, IL‐10, TNFα) and pepsin in saliva as potential factors reducing VP longevity. Methods: Prospective double‐blind randomized clinical trial was conducted (NCT04268459). Patients were randomly divided into two groups depending on VP replacement regimen (regular—every 3 months, or irregular—when complications occur). Levels of IL‐1β, IL‐6, IL‐8, IL‐10, TNFα, and pepsin in saliva samples (fasting and after eating) of laryngectomized patients were measured using ELISA tests. Results: Fifty‐two patients (26 in both groups) with control group (7 patients) participated in the study. The level of IL‐1β, IL‐6, IL‐8, IL‐10, TNFα, and pepsin did not differ according to regularity of VP replacements (p = 0.301–0.801). IL‐6 levels were significantly higher when VP complications occurs (p = 0.012). Conclusions: The saliva components were not significantly different depending on the frequency of VP replacements. Introduction: Primary total elbow replacement (TER) services in England are being restructured with the goal of centralising care to specialised centres. It is important to monitor the impact of this service redesign. This protocol outlines an intended analysis to provide detailed descriptions of the patients who are receiving primary TER, where and by whom TER is being performed, and what the current surgical practices for TER are in England before the reconfiguration. Methods: This analysis will use the National Joint Registry (NJR) elbow dataset and link it with NHS England Hospital Episode Statistics-Admitted Patient Care (HES-APC). It will include eligible patients from the start of the NJR elbow dataset in April 2012 to December 2022. The main objective is to determine the incidence of TER in England. Age-sex standardised rates will be calculated for groups including different ethnicities, and socioeconomic backgrounds, using the mid-year population data provided by the Office for National Statistics. This planned analysis will summarise patient characteristics such as age, sex, body mass index (BMI), hand dominance, American Society of Anaesthesiologists (ASA) grade, indication for TER, socioeconomic status, and patient co-morbidities. It will also examine implant fixation type, classification, brand/type, and changes over time in implant types used in England. Additionally, it will explore the characteristics and volume of the surgeons and hospitals providing primary TER services, including the grade of the primary surgeons, funding source for surgery, and admission type. The analysis will cover the number of procedures performed by surgeons and hospitals annually in England and in each region of England. Finally, the planned analysis will summarise the elective wait time, postoperative length of stay, and any serious adverse events or re-admissions within 30 and 90 days after the TER. Discussion: This protocol describes the first deep dive analysis into the NJR elbow dataset to describe the incidence of TER surgery in England and the characteristics of patients who are receiving it. This analysis will summarise current primary TER practices in England before service reconfigurations. AbstractIn this paper, the issue of determining an optimal age replacement is explored by incorporating minimal repair, preventive replacement, and corrective replacement into a k-out-of-n system subject to shocks. The k-out-of-n system in question consists of n identical components, and functions if at least k components function. Each shock causes the failure of a random number of components; if at least n-k + 1 components fail, the system fails and implements corrective replacement, otherwise the system is minimally repaired. The system is scheduled to implement preventive replacement before failure at age T or at the complement of a random working cycle, whichever occurs first or last. In order to balance the bias time between preventive replacement and corrective replacement, a replacement bias cost is also taken into account for planning replacement policies. The present paper develops a two-phase maintenance methodology and determines the optimal number of components and preventive replacement age that minimize the expected cost rate functions. For each model, the expected cost rate function is formulated analytically and optimized theoretically, and a numerical example is given to illustrate the applicability of the proposed methodology. Microorganisms are important members of seagrass bed ecosystems and play a crucial role in maintaining the health of seagrasses and the ecological functions of the ecosystem. In this study, we systematically quantified the assembly processes of microbial communities in fragmented seagrass beds and examined their correlation with environmental factors. Concurrently, we explored the relative contributions of species replacement and richness differences to the taxonomic and functional β-diversity of microbial communities, investigated the potential interrelation between these components, and assessed the explanatory power of environmental factors. The results suggest that stochastic processes dominate community assembly. Taxonomic β-diversity differences are governed by species replacement, while for functional β-diversity, the contribution of richness differences slightly outweighs that of replacement processes. A weak but significant correlation (p < 0.05) exists between the two components of bdiversity in taxonomy and functionality, with almost no observed significant correlation with environmental factors. This implies significant differences in taxonomy, but functional convergence and redundancy within microbial communities. Environmental factors are insufficient to explain the β-diversity differences. In conclusion, the assembly of microbial communities in fragmented seagrass beds is governed by stochastic processes. The patterns of taxonomic and functional β-diversity provide new insights and evidence for a better understanding of these stochastic assembly rules. This study investigated whether differences in survival rates and clinical outcomes exist in patients undergoing TKA by insurance type: National Health Insurance (NHI) vs. Medical Aid Program (MAP). This study conducted a retrospective analysis of 762 TKAs (NHI, n = 505; MAP, n = 257) with a mean follow-up of 8.4 ± 1.8 years. Patient-reported outcomes (PROMs) were evaluated using the American Knee Society's (AKS) score at the final follow-up. The survival rate of each group was analyzed using Kaplan–Meier survival analysis. Any postoperative complications and readmissions within 90 days of discharge were recorded and compared between the groups. There were no between-group differences in pre- to postoperative improvement in AKS scores. The estimated 10-year survival rates were 98.5% in the NHI group and 96.9% in the MAP group, respectively, with no significant differences (p = 0.48). However, the length of hospital stay (LOS) was significantly longer in the MAP group than in the NHI group (13.4 days vs. 13.1 days, p = 0.03), and the transfer rate to other departments was significantly higher in the MAP group than in the NHI group (3.9% vs. 1.4%, p = 0.04). Readmission rates for orthopedic complications for 90 days were 3.0% in the NHI group and 3.5% in the MAP group, respectively (p = 0.67). Patients' insurance type showed similar survival rates and clinical outcomes to those of primary TKA at a mean follow-up of 8.4 years, but the LOS and rate of transfer to other departments during hospitalization were influenced by insurance type. Prior research has demonstrated that incorporating solid waste from various sources, such as plastics, civil engineering waste, and industrial and mining waste, into pavement and civil works mixes has notable environmental benefits. This includes reducing the over-exploitation of aggregate banks and preventing waste materials from being deposited in open-pit landfills for extended periods. This review focuses on sustainable aggregates produced from solid waste with rheological or mechanical properties suitable for replacing conventional aggregates in asphalt or hydraulic concrete mixtures. The aim is to determine the optimal replacement percentage in the mixture to directly impact pavement performance. This review also delves into the impact on fatigue resistance and permanent deformation based on the type of waste material used in construction. Additionally, using sustainable aggregates presents added benefits for pavement binders, such as inhibiting reflection cracks, reducing traffic noise, and prolonging the service life of the pavement. However, it is crucial to study the percentage of replacement of sustainable aggregates in conjunction with other materials using mathematical models and simulations to ensure a substantial contribution to the sustainability of the construction industry. Ré Island Bridge connects Île de Ré to the mainland on the west coast of France and crosses four navigable sea passes, with a maximum air clearance of 30 m above sea level. This bridge is 2926 m long and is famous for its rapidity of construction and the first use of high performance concrete for such a long bridge. The bridge deck consists of a prestressed concrete box girder divided into six independent viaducts. It was commissioned in 1988, and 30 years later, the rupture of an external prestressing tendon occurred inside the box girder. At first, the article introduces the emergency measures taken by the strategic bridge's owner. Then it presents an analysis of the causes of the failure and the strategy developed by the owner to deal with the possible rupture of other tendons. Safety measures against the whiplash of tendons by the installation of straps around the tendons are described and the design of the strapping is discussed. The studies for the replacement of deficient tendons are then presented, with the difficulty of replacing tendons anchored in the half-joints between viaducts. This comprehensive review was conducted to assess the effects of tranexamic acid (TXA) on blood loss, venous thromboembolism (VTE) risk, and wound complications following total hip arthroplasty (THA). Additionally, it sought to evaluate the effects of various TXA dosages, modes of administration, and combinations with other antifibrinolytic drug. In search of systematic reviews and meta-analyses on the use of TXA in THA patients, we searched extensively through databases including Scopus, the Cochrane Library, Embase, Medline, the Web of Science, PubMed, and Google Scholar. We discovered 23 meta-analyses covering 32,442 patients overall that fulfilled our study criteria, spanning the period from the creation of these databases until May 2023. This comprehensive review's meta-analyses, which together examined over 35,000 patients, repeatedly demonstrated how TXA administration during THA successfully lowers perioperative blood loss and the need for transfusions. TXA reduced total blood loss by an average of 151–370 ml, postoperative hemoglobin levels by 0.5–1.1 g/dL, and transfusion rates by 19–26% on average when compared to control groups. The information gathered did not indicate that using TXA significantly increased the risk of VTE or wound complications. When comparing different TXA doses, administration techniques, or its use in conjunction with other anti-fibrinolytic therapies, no discernible differences were found in terms of efficacy or safety outcomes. The comprehensive review clearly indicates that TXA improves THA outcomes without increasing the risk of adverse events by lowering blood loss and the requirement for transfusions. We consider an operator of machinery with deteriorating efficiency, facing the problem of optimally timing of either a minor (maintenance) investment or a major (replacement) investment under price uncertainty. If a maintenance investment is chosen, the efficiency of the machinery will deteriorate more slowly, and replacing later is still possible. The optimal decision rule is expressed in the form of thresholds for long-run prices, indicating that it may be rational to wait to see which of the large and small investment is the better choice. We relate the setting to repowering of green energy facilities, such as hydropower plants and wind farms. Our analysis provides several managerial insights. We characterize the conditions that govern whether the smaller investment should be considered at all, and we quantify the effect of having a replacement option embedded in a maintenance option. Background: Chief Executive Officer's (CEO) are integral leaders of health care organisation. Over the last two decades in United States (US) hospitals, it has been noted that CEO turnover rates are increasing, and it was reported that the CEO turnover rates have augmented from 14% in 2008 to 18% in 2017 in the private sector. In Australia, it was discovered that during two years, 41 executives had 18 distinct positions. It has been highlighted that the increasing CEO turnover is a major issue for Australian and international health care organisations. Some of the negative consequences of CEO turnover include organisational instability, high financial costs, loss of human capital and adverse effects on staff morale and patient care. Objective: Our scoping review aimed to map and summarise the evidence associated with CEO turnovers in both health and non-health setting, and answer the following questions: 1. What are the reasons for CEO departure?, 2. What are the strategies to minimise CEO turnover? Results: A protocol explaining the objectives, inclusion criteria and methods for this scoping review were specified in advance and published. This scoping review included 17 studies (13 health and 4 non-health setting) published over a 31-year period that investigated and described the increasing CEO turnover rates. All 17 studies identified causes of CEO turnover along with certain studies identifying facilitators of CEO retention. We classified CEO's departure reasons into three major themes: organizational, performance, and personal. Organisational factors include CEO departures due to issues within the organisation, performance factors include issues with CEO's work and personal factors captures personal reasons for CEO's leaving their job. Conclusion: CEOs are under immense pressure to deliver good results and drive growth while satisfying the interests of internal and external stakeholders. There are various reasons for CEO's departure however the most common factor identified is organisational. Aims: Understanding the spatio‐temporal patterns of restoration outcomes is crucial to improve predictability of restoration. High beta diversity of species‐rich communities is sought because it increases overall biodiversity and improves ecosystem stability and multifunctionality. For predictive restoration, it is important to identify the significance of drivers like site characteristics but also uncontrolled factors such as spatial effects, historical factors, and year effects. Location: Dikes at river Danube, SE Germany. Methods: We studied dike grasslands 4–19 years after restoration over five years (2017–2021, 41 plots in 12 sites). We calculated beta diversity indices to describe spatial variation and temporal turnover, including their additive components 'replacement' and 'nestedness', or 'gains' and 'losses'. We analysed the main drivers of beta diversity like local site characteristics, landscape, and historical factors. Results: Spatial variation of the restored dike grasslands was dominated by the replacement component and showed no homogenisation despite a significant temporal turnover. The replacement drivers changed over time, although replacement was mainly affected by slope aspect and landscape factors. Historical factors were inconsistent over time, and no statistically clear drivers of nestedness were found. The dike grasslands exhibited a year‐to‐year turnover in species composition of 37 ± 11%. Gains and losses were balanced over time, although the ratio changed and was most pronounced on south‐facing slopes. Conclusions: The restored grasslands exhibited spatial variation by site characteristics but also by spatial factors which were not controlled by restorations. Moreover, high non‐directional temporal turnover occurred, caused most likely by weather fluctuations, slightly varying management, and stochastic biotic dynamics. Thus, flexible targets are recommended for restoration monitoring, by defining a set of desired states within a certain range. Furthermore, the dominance of the replacement component of spatial variation should move the focus from defining one precise restoration approach to defining a set of possible methods which together would foster beta diversity. Psychopharmacological treatment may be an independent risk factor for increased length of stay and readmission after hip and knee replacement. Thus, temporary perioperative discontinuation may be beneficial. However, little is known regarding the treatments, and not all are feasible to discontinue. Therefore, the aim of this study was to describe the treatments in terms of type, dose, duration, indication and initiating physician to assess the feasibility of temporary perioperative discontinuation. We included 482 patients planned for hip or knee replacement in psychopharmacological treatment for psychiatric disorders from 2021 to 2023 at five orthopaedic departments in Denmark. Most patients were treated with antidepressants (89%); most frequently, either selective serotonin reuptake inhibitors (SSRIs; 48%) or serotonin‐norepinephrine reuptake inhibitors (SNRIs; 21%). The majority received monotherapy (70%); most frequently, an SSRI (36%) or an SNRI (12%). Most antidepressants were initiated by general practitioners (71%), and the treatments had lasted for more than a year (87%). The doses of SSRIs/SNRIs were moderate, and the most frequent indication for antidepressants was depression (77%). These results imply that temporary perioperative SSRI/SNRI discontinuation may be feasible in hip and knee replacement patients and support a future randomized controlled trial investigating the potential benefits of temporary discontinuation. Successful total hip arthroplasty relies on accurate preoperative planning. However, the conventional preoperative planning, a two-dimensional method using X-ray template, has shown poor reliability of predicting component size. To our knowledge, artificial intelligence technology assisted three-dimensional preoperative planning is promising to improve the accuracy of preoperative planning but there is a dearth of clinical evidence. Therefore, in this study we compared the prediction accuracy of these two maneuvers. We conducted a prospective study consisting of 117 consecutive patients who underwent a primary cementless total hip arthroplasty to compare the prediction accuracy of these two methods. The two-dimensional and artificial intelligence assisted three-dimensional planning results of the same patient were compared with the definitive implant size respectively. The prediction accuracy of artificial intelligence assisted three-dimensional planning for cup and the stem sizes were 66.67% (78/117) and 65.81% (77/117), two-dimensional planning was 30.77% (36/117) and 37.61% (44/117) (p < 0.05). There were poor prediction results of two-dimensional planning in patients with hip dysplasia (p = 0.004, OR = 7.143) and excessive femoral anteversion (p = 0.012, OR = 1.052), meanwhile the failure risk of stem side two-dimensional planning increased as patients got older (p = 0.003, OR = 1.118). The accuracy of artificial intelligence assisted three-dimensional planning cannot be affected by above factors. BackgroundThe purpose of this study was to investigate the relationship between preoperative aspartate aminotransferase-to-platelet ratio index (APRI) and postoperative complications following total hip arthroplasty (THA).MethodsAll THA for osteoarthritis patients from 2007 to 2020 within the American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) database were included in this study. Subjects were subsequently divided into cohorts based on APRI. Four groups, including normal range, some liver damage, significant fibrosis, and cirrhosis groups, were created. PURPOSE: To evaluate the 30-day complication rates after arthroscopic meniscus repair and meniscectomy using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database, with subgroup analysis of patients aged > 40 years. METHODS: NSQIP registries between 2006 and 2019 were queried using Current Procedural Terminology codes to identify patients undergoing arthroscopic meniscus repair (CPT 29882, 29883) and meniscectomy (29880, 29881). The following 30-day complications were assessed: pulmonary embolism (PE), venous thromboembolism (VTE), surgical site infection (SSI), reoperation, and readmission. Complications rates between treatment groups were compared using multivariate logistic regression analyses adjusted for sex, age, steroid use, and smoking/dyspnoea/COPD. A subgroup analysis was performed for patients aged > 40 years. RESULTS: A total 6354 meniscus repairs and 99,372 meniscectomies were identified. Complication rates were  40 years, complication rates were  40 years undergoing meniscus repair versus meniscectomy: PE (0.38% vs 0.12%, p  40 years. These findings support meniscus repair whenever feasible in the setting of preserved articular cartilage. Understanding of the short-term complication rates after arthroscopic meniscus repair and meniscectomy can aid surgeons in providing comprehensive preoperative counselling to patients considering such treatments, specifically when discussing the risks and benefits of meniscus repair. Abstract Introduction Primary total elbow replacement (TER) services in England are being restructured with the goal of centralising care to specialised centres. It is important to monitor the impact of this service redesign. This protocol outlines an intended analysis to provide detailed descriptions of the patients who are receiving primary TER, where and by whom TER is being performed, and what the current surgical practices for TER are in England before the reconfiguration. Methods This analysis will use the National Joint Registry (NJR) elbow dataset and link it with NHS England Hospital Episode Statistics-Admitted Patient Care (HES-APC). It will include eligible patients from the start of the NJR elbow dataset in April 2012 to December 2022. The main objective is to determine the incidence of TER in England. Age-sex standardised rates will be calculated for groups including different ethnicities, and socioeconomic backgrounds, using the mid-year population data provided by the Office for National Statistics. This planned analysis will summarise patient characteristics such as age, sex, body mass index (BMI), hand dominance, American Society of Anaesthesiologists (ASA) grade, indication for TER, socioeconomic status, and patient co-morbidities. It will also examine implant fixation type, classification, brand/type, and changes over time in implant types used in England. Additionally, it will explore the characteristics and volume of the surgeons and hospitals providing primary TER services, including the grade of the primary surgeons, funding source for surgery, and admission type. The analysis will cover the number of procedures performed by surgeons and hospitals annually in England and in each region of England. Finally, the planned analysis will summarise the elective wait time, postoperative length of stay, and any serious adverse events or re-admissions within 30 and 90 days after the TER. Discussion This protocol describes the first deep dive analysis into the NJR elbow dataset to describe the incidence of TER surgery in England and the characteristics of patients who are receiving it. This analysis will summarise current primary TER practices in England before service reconfigurations. Abstract Background Chief Executive Officer’s (CEO) are integral leaders of health care organisation. Over the last two decades in United States (US) hospitals, it has been noted that CEO turnover rates are increasing, and it was reported that the CEO turnover rates have augmented from 14% in 2008 to 18% in 2017 in the private sector. In Australia, it was discovered that during two years, 41 executives had 18 distinct positions. It has been highlighted that the increasing CEO turnover is a major issue for Australian and international health care organisations. Some of the negative consequences of CEO turnover include organisational instability, high financial costs, loss of human capital and adverse effects on staff morale and patient care. Objective Our scoping review aimed to map and summarise the evidence associated with CEO turnovers in both health and non-health setting, and answer the following questions: 1. What are the reasons for CEO departure?, 2. What are the strategies to minimise CEO turnover? Results A protocol explaining the objectives, inclusion criteria and methods for this scoping review were specified in advance and published. This scoping review included 17 studies (13 health and 4 non-health setting) published over a 31-year period that investigated and described the increasing CEO turnover rates. All 17 studies identified causes of CEO turnover along with certain studies identifying facilitators of CEO retention. We classified CEO’s departure reasons into three major themes: organizational, performance, and personal. Organisational factors include CEO departures due to issues within the organisation, performance factors include issues with CEO’s work and personal factors captures personal reasons for CEO’s leaving their job. Conclusion CEOs are under immense pressure to deliver good results and drive growth while satisfying the interests of internal and external stakeholders. There are various reasons for CEO’s departure however the most common factor identified is organisational.

Poly-ether-ether-ketone (PEEK) and PEEK composites are outstanding candidates for biomedical applications, such as orthopedic devices, where biocompatibility and modulus match with surrounding tissue are requisite for long-term success. The mechanical properties can be optimized by incorporating fillers such as continuous and chopped carbon fibers. While much is known about the mechanical and tribological behavior of PEEK composites, there are few articles that summarize the viability of using PEEK reinforced with carbon fibers in orthopedic implants. This paper reviews biocompatibility, tribological, and mechanical studies on PEEK and their composites with carbon fibers, notably PEEK reinforced with polyacrylonitrile (PAN)-based carbon fibers and PEEK reinforced with pitch-based carbon fibers, for application in orthopedics and total joint replacements (TJRs). The main objectives of this review are two-fold. Firstly, this paper aims to assist designers in making informed decisions on the suitability of using PEEK and PEEK composites in orthopedic applications; as it is not well understood how these materials perform on the whole in orthopedics and TJRs. Secondly, this paper aims to serve as a centralized paper in which researchers can gain information on the tribological and mechanical advancements of PEEK and PEEK composites.

PurposeBecause different targets are used for internal-external rotation, an asymmetric baseplate designed for mechanical alignment may lead to under-coverage and concomitant posterior rim loading in the lateral compartment following unrestricted kinematic alignment (KA) TKA. Recognizing that such loading can lead to premature wear and/or subsidence, our aim was to determine the cause(s) so that occurrence could be remedied. Our hypothesis was that baseplate design features such as asymmetric shape when aligned in KA would consistently contribute to posterior rim loading in the lateral compartment.MethodsBased on analysis of fluoroscopic images of 50 patients performing dynamic, weight bearing deep knee bend and step up and of postoperative CT images, five possible causes were investigated. Causes included internal rotation of the baseplate when positioned in KA; posterior position of the lateral femoral condyle at extension; internal tibial rotation with flexion; internal rotational deviation of the baseplate from the KA rotation target; and posterior slope.ResultsThe incidence of posterior rim loading was 18% (9 of 50 patients). When positioned in KA, the asymmetric baseplate left 15% versus 10% of the AP depth of the lateral compartment uncovered posteriorly for posterior rim loading and non-posterior rim loading groups, respectively (p = 0.009). The lateral femoral condyle at extension was more posterior by 4 mm for the posterior rim loading group (p = 0.003).ConclusionsPosterior rim loading in the lateral compartment was caused in part by the asymmetric design of the tibial baseplate designed for mechanical alignment which was internally rotated when positioned in KA thus under-covering a substantial percentage of the posterior lateral tibia. This highlights the need for new, asymmetric baseplates designed to maximize coverage when used in KA.Level of evidenceIII.

This study develops warranty cost models for repairable products subject to Lemon Laws, encompassing Critical and Non-Critical components forming a multi-component system. Failures can arise naturally or be induced by other components (i.e., failure interaction), defining a lemon if recurrent failures reach a threshold (k) during the warranty period. A lemon declaration triggers a refund or replacement by the manufacturer. Four warranty cost models are proposed from the manufacturer's standpoint, considering failure mechanisms. Increasing failure thresholds in the warranty scheme substantially decreases warranty cost rates. For instance, a threshold (k) of 5 in refund and replacement schemes yields the lowest cost rates of 33.7159 and 25.8249, respectively. Failure interactions escalate total warranty costs; for instance, in a refund scheme (k = 5), costs with failure interaction reach 31.0169 compared to 28.7603 without. Similar trends apply to replacement schemes. Moreover, a lower warranty cost rate will extend the period, indicating regulation fulfillment due to a closer warranty period to the Lemon period. Sensitivity analysis also underscores the role of higher reliability in reducing warranty costs and complying with Lemon Laws. Finally, maintenance strategies and product reliability are emphasized to fulfill Lemon Laws with minimal costs, i.e., fewer warranty claims.

Background: Multimodal regimens are the mainstay of postoperative analgesia. Analgesic effectiveness of programmed intermittent bolus (PIB) versus continuous infusion (CI) in ultrasound-guided adductor canal block (ACB) for unilateral knee arthroplasty, were compared. Materials and Methods: Following Ethical and Clinical Trials Registry approval, eligible patients were administered pericapsular infiltration, intraoperatively and ACB with catheter, postoperatively. As per randomization, the PIB group received 10 ml 0.2% ropivacaine every 3 hourly while the CI group received 6mL/h. In addition, both groups received patient controlled boluses of 5mL with lockout interval of 30 minutes. The numerical rating score (NRS) score, plasma concentration of ropivacaine, use of rescue analgesics, quadricep strength by Straight Leg Rising Test (SLRT), Medical Research Council (MRC) scale for motor power, were monitored at 0, 1, 4, 24, 48 h, and Likert Scale for patient satisfaction, was measured at 72 h. To be clinically meaningful, sample size calculation, was according to a difference in NRS, for a power of 0.80, and σ‐standard deviation of two points, thus requiring at least 17 patients in each group. Results: In PIB group, patients experienced better analgesia in the first 24 h (rest - 95% CI - 0.089–0.669, P = 0.010 and movement - 95% CI - 0.82–0.884, P = 0.018) and motor power, in the first (P = 0.051) and 4th h (P = 0.034), after recovery, along with improved patient satisfaction (P = 0.001). Ropivacaine plasma concentration, at regular intervals, was independent of the pain scores with rest and movement. Rescue analgesia was inconclusive in both groups. Conclusion: The PIB option provided better analgesia in the postoperative period.

Abstract Background Several studies have compared the pros and cons of simultaneous bilateral versus staged bilateral hip and knee replacement but the outcomes of these two surgical options remains a matter of controversy. This study aimed to evaluate demographic features, incidence and hospitalization rates of bilateral one stage total hip and knee arthroplasty in Italy. Methods The Italian Ministry of Health’s National Hospital Discharge Reports (SDO) were used to gather data. This study referred to the adult population (+ 20 years of age) from 2001 to 2015 for hip arthroplasty and from 2001 to 2016 for knee arthroplasty. Results Overall, 1,544 bilateral simultaneous hip replacement were carried out. The incidence rate was 0.21 cases per 100,000 adult Italian residents. Male/female ratio was 1.1. The average days of hospital stay was 11.7 ± 11.8 days. The main primary codified diagnosis was: osteoarthrosis, localized, primary, pelvic region and thigh (ICD code: 715.15). 2,851 bilateral simultaneous knee replacement were carried out. The incidence rate was 0.37 cases per 100,000 adult Italian residents. Male/female ratio was 0.6. The average days of hospital stay was 7.7 ± 5.8 days. The main primary codified diagnosis was: osteoarthrosis, localized, primary, lower leg (ICD code: 715.16). Conclusions The burden of hip and knee osteoarthrosis as a leading cause of bilateral joint replacement is significant in Italy. The national registers’ longitudinal analysis may provide data for establishing international guidelines regarding the appropriate indications for one stage bilateral simultaneous hip or knee replacement versus two stage.

Background: Cervical cancer is one of the most common gynecological malignancies worldwide. The incidence of cervical cancer ranks second for female malignancies in China. However, human papillomavirus (HPV) prevalence and genotype replacement for a long time in Beijing were not yet evaluated. Methods: Women patients who visited clinical departments, especially the gynecology department, were included in this study from 2014 to 2020. They suffered from different kinds of cervical abnormalities even cervical cancer. Thirteen types of HPV were detected in cervical samples by using multiple fluorescence polymerase chain reactions. All patients were divided into four groups according to age (15?30, 31?45, 46?60, and >60 years old). Results: The study included data from four certain years (2014, 2016, 2018, and 2020). Overall HPV prevalence was 18.0%, 16.7%, 21.9%, and 19.1%, respectively. Of the 13 genotypes, the top one infection type was HPV52, followed by HPV58 and HPV16 or HPV16 and HPV58 in different years. HPV56 prevalence raised from 2018, which replaced HPV39 into the top five list. Only HPV prevalence of 46?60 years age group declined, mainly resulting from the reduced prevalence of HPV39 and HPV58 (p < 0.001 and p = 0.020). The proportions of HPV dual-infection across the four years varied significantly in statistics (p = 0.001), whereas the most common dual-infection HPV39/68 disappeared after 2018. Conclusion: The prevalence of two HPV genotypes (HPV39 and HPV58) and dual-infection HPV39/68 showed a declining trend, especially in the 46?60 years age group. The trends need to be observed continuously.

Aims: The aims of this study were to identify and evaluate the current literature examining the prognostic factors which are associated with failure of total elbow arthroplasty (TEA). Methods: Electronic literature searches were conducted using MEDLINE, Embase, PubMed, and Cochrane. All studies reporting prognostic estimates for factors associated with the revision of a primary TEA were included. The risk of bias was assessed using the Quality In Prognosis Studies (QUIPS) tool, and the quality of evidence was assessed using the modified Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) framework. Due to low quality of the evidence and the heterogeneous nature of the studies, a narrative synthesis was used. Results: A total of 19 studies met the inclusion criteria, investigating 28 possible prognostic factors. Most QUIPS domains (84%) were rated as moderate to high risk of bias. The quality of the evidence was low or very low for all prognostic factors. In low-quality evidence, prognostic factors with consistent associations with failure of TEA in more than one study were: the sequelae of trauma leading to TEA, either independently or combined with acute trauma, and male sex. Several other studies investigating sex reported no association. The evidence for other factors was of very low quality and mostly involved exploratory studies. Conclusion: The current evidence investigating the prognostic factors associated with failure of TEA is of low or very low quality, and studies generally have a moderate to high risk of bias. Prognostic factors are subject to uncertainty, should be interpreted with caution, and are of little clinical value. Effective pain management during hip and knee replacement ensures the efficient and fast recovery of patients. Joint replacement surgery is typically accompanied by the use of general anesthesia or spinal anesthesia. Spinal anesthesia, despite certain risks, demonstrates better effectiveness compared to general anesthesia in hip and knee replacements. In other words, spinal anesthesia entails fewer perioperative complications. The addition of sedation and anesthesia monitoring during joint replacements using spinal anesthesia is of particular importance. After a successful surgical procedure of joint replacement, achieving adequate pain relief in the postoperative period is crucial. This is achieved through intravenous patient-controlled analgesia or epidural analgesia. A multimodal analgesia strategy combines analgesics with different mechanisms of action to enhance pain management. Intraoperative periarticular administration of multimodal agents is one of the most important procedures in the perioperative pain control during total hip arthroplasty. Peripheral nerve blocks have emerged recently as an alternative analgesic approach. During hip and knee replacements, various nerve blocks are utilized, including lumbar plexus block, fascia iliaca block, femoral and sciatic nerve block, adductor canal block, quadratus lumborum block, erector spinae plane block. Given the entire perioperative process, the use of preventive and multimodal therapy along with peripheral nerve blocks can help reduce the intensity of postoperative pain, thereby improving the functional status and quality of life for patients.
