For patients with chronic obstructive pulmonary disease (COPD), the assessment of the treatment efficacy during hospitalization is of importance to the optimization of clinical treatments. Conventional spirometry might not be sensitive enough to capture the regional lung function development. The study aimed to evaluate the feasibility of using electrical impedance tomography (EIT) as an objective bedside evaluation tool for the treatment of acute exacerbation of COPD (AECOPD). Consecutive patients who required hospitalization due to AECOPD were included prospectively. EIT measurements were conducted at the time of admission and before the discharge simultaneously when a forced vital capacity maneuver was conducted. EIT-based heterogeneity measures of regional lung function were calculated based on the impedance changes over time. Surveys for attending doctors and patients were designed to evaluate the ease of use, feasibility, and overall satisfaction level to understand the acceptability of EIT measurements. Patient-reported outcome assessments were conducted. User's acceptance of EIT technology was investigated with a five-dimension survey. A total of 32 patients were included, and 8 patients were excluded due to the FVC maneuver not meeting the ATS criteria. Spirometry-based lung function was improved during hospitalization but not significantly different (FEV1 %pred.: 35.8% ± 6.7% vs. 45.3% ± 8.8% at admission vs. discharge; p = 0.11. FVC % pred.: 67.8% ± 0.4% vs. 82.6% ± 5.0%; p = 0.15. FEV1/FVC: 0.41 ± 0.09 vs. 0.42 ± 0.07, p = 0.71). The symptoms of COPD were significantly improved, but the correlations between the improvement of symptoms and spirometry FEV1 and FEV1/FVC were low (R = 0.1 and -0.01, respectively). The differences in blood gasses and blood tests were insignificant. All but one EIT-based regional lung function parameter were significantly improved after hospitalization. The results highly correlated with the patient-reported outcome assessment (R > 0.6, p < 0.001). The overall acceptability score of EIT measurement for both attending physicians and patients was high (4.1 ± 0.8 for physicians, 4.5 ± 0.5 for patients out of 5). These results demonstrated that it was feasible and acceptable to use EIT as an objective bedside evaluation tool for COPD treatment efficacy. [ABSTRACT FROM AUTHOR]

The effectiveness of HFNC treatment and the methods for monitoring its efficacy in the general ward remain unclear. This prospective observational study enrolled 42 patients who had acute hypoxemic respiratory failure requiring HFNC oxygen therapy in the general adult respiratory ward. The primary outcome was the all-cause in-hospital mortality. Secondary outcomes included the association between initial blood test results and HFNC outcomes. Regional ventilation distributions were monitored in 24 patients using electrical impedance tomography (EIT) after HFNC initiation. Patients with successful HFNC treatment had better in-hospital survival (94%) compared to those with failed HFNC treatment (0%, p < 0.001). Neutrophil-to-lymphocyte ratios of ≥9 were more common in patients with failed HFNC (70%) compared to those with successful HFNC (52%, p = 0.070), and these patients had shorter hospital survival rates after HFNC treatment (p = 0.046, Tarone-Ware test). Patients with successful HFNC treatment had a more central ventilation distribution compared to those with failed HFNC treatment (p < 0.05). Similarly, patients who survived HFNC treatment had a more central distribution compared to those who did not survive (p < 0.001). We concluded that HFNC in the general respiratory ward may be a potential rescue therapy for patients with respiratory failure. EIT can potentially monitor patients receiving HFNC therapy. Electrical impedance tomography (EIT), a noninvasive and radiation-free imaging technique can be used in pulmonary function monitoring for determining regional ventilation distribution within the lung. Gold standard in pulmonary function monitoring is spirometry/body plethysmography, a method using forced breathing maneuvers to obtain global lung function parameters. However, this method is heavily dependent on the cooperation of the patients. Within this observational study, a method under normal breathing was tested with 5 healthy volunteers, which provides regional information about ventilation distribution. The occlusion method Rocc, a method for determining airway resistance, was used to create a short-term airway closure. Regional ventilation during the airway closure was examined with EIT. Simultaneously four different artificial airway resistances were used to simulate airway obstructions. Results show that EIT in combination with the ROcc method is suitable for the detection of regional differences in ventilation during airway closure for all four artificial airway resistances. Although the sum of relative impedances at the end of the shutter maneuver are smaller (nearly -0.100 AU) for the airway resistances Ø 12.5 mm, Ø 10.5 mm and Ø 9.5 mm than for the smallest one with Ø 30.0 mm (~ -0.070 AU), the changes in impedance from the start to the end of the shutter maneuver differs only slightly between the four artificial airway resistances. All impedance changes are in the range of 0.100 to 0.130 AU. The combination of EIT and the ROcc method provides not only global parameters such as airway resistance under normal breathing conditions, but also results of regional ventilation, which could enable the identification of areas affected by airway obstructions. However, the obtained results indicate that EIT might be a useful tool in the diagnosis and follow-up of obstructive lung diseases.

Background: Although electrical impedance tomography (EIT) is widely used for monitoring regional ventilation distribution, reference values have yet to be established for clinical use. The present study aimed to evaluate the feasibility of creating reference values for standard EIT parameters for potential clinical application., Methods: A total of 75 participants with healthy lungs were included in this prospective study (male:female, 48:27; age, 34±14 years; height, 172±7 cm; weight, 73±12 kg). The subjects were examined during spontaneous breathing in the supine position. EIT measurements were performed at the level of the 4 th intercostal space. Commonly used EIT-based parameters, including the center of ventilation (CoV), dorsal and most dorsal fractions of ventilation distribution (TV D and TV ROI4 respectively), global inhomogeneity (GI) index, and standard deviation of regional ventilation delay index (RVD SD ) were calculated., Results: Following outlier detection, EIT data from 71 subjects were finally evaluated. The values of the evaluated parameters were: CoV, 48.7%±1.7%; TVD, 48.1%±5.4%; TV ROI4 , 7.1%±1.8%; GI, 0.49±0.04; and RVD SD , 7.0±2.0. The coefficients of variation for CoV and GI were low (0.03 and 0.07, respectively), but those for TV ROI4 and RVD SD were comparatively high (0.26 and 0.28, respectively). None of the evaluated parameters showed a significant correlation with age. The GI index showed a weak but significant correlation with body mass index (R=0.29, P=0.01). The RVD SD was slightly higher in males than in females., Conclusions: Our study indicated that CoV and GI were stable parameters with small coefficients of variation in participants with healthy lungs. Background: Evaluating the effect of airflow rate on amplitude of lung sound energy and regional distribution of lung sounds may assist in the interpretation of computerized acoustic measurements., Objectives: The aim of this study was to assess the effect of airflow rate on Vibration Response Imaging (VRI) measurement in healthy lungs., Methods: Lung sounds were recorded from 20 healthy adults in the frequency range of 150-250 Hz using 40 piezoelectric sensors positioned on the posterior chest wall. During the recordings, subjects were breathing at airflow rates ranging between 0.3 and 1.7L/s. Online visual feedback was provided using a pneumotach mouthpiece., Results: Amplitude of lung sound energy significantly increased with increasing airflow rate (p<0.00001, Friedman test). A strong relationship (R2=0.95) was obtained between amplitude of lung sound energy at peak inspiration and airflow rate raised to the third power. This correlation did not significantly affect normalized lung sound distribution maps at peak inspiration, especially when airflow was higher than 1.0L/s. Acoustic maps obtained at airflow rates below 0.7L/s differed from those recorded above 1.0L/s (p<0.05, Wilcoxon matched-paired signed-ranks test)., Conclusion: These findings may be of importance when comparing healthy and diseased lungs or when monitoring changes in lung sounds during treatment follow-up.
