Facial nerve paralysis is a debilitating condition with multiple etiologies, with aesthetic, functional, psychological and social impact. Given the complex multitude of causes that may generate such condition, a therapeutic algorithm is mandatory when attempting reconstruction. Severity, timing, patient adherence to a rehabilitation program, status of ipsilateral and contralateral facial nerves and particularities of each patient are all criteria which should be accounted when choosing a treatment option. After initial assessment, a variable treatment panel is available based on condition type include medicamentous therapy, rehabilitation program, dynamic and static procedures surgical procedures, having as primary aim functional restoration achieving aesthetic balanced facial features. This paper summarizes current knowledge in facial paralysis reconstruction and presents an algorithmic approach that eases decision making and therapeutic strategy. [ABSTRACT FROM AUTHOR]

Between 1600 and 1800, Beulake was a peat-dredging village of reasonable size. Peat dredging was profitable but caused permanent land loss and flooding. During the first half of the 18th century, peat-dredging seems to have brought some prosperity, but then a decline set in. The village eventually disappeared due to two severe storms in 1775 and 1776. This article examines the spatial structure of the former village and the socioeconomic conditions of its residents. The reconstruction of bone defects still represents a great challenge for surgeons mainly because it is aimed to properly restore the site of the defect, to maintain the motor and sensate function, and, not lastly, to recreate or maintain the aesthetics of the region. The fibular flap is a great choice for bone defect reconstruction, with a large array of clinical utilization, but insufficiently used because of its demanding technique of harvesting. The ability to transfer simultaneously bone, muscle and skin in a one-stage vascular anastomosis makes this flap even more appealing when addressing composite defects. [ABSTRACT FROM AUTHOR]

Pulmonary artery aneurysms are rare entities usually associated with congenital anomalies that increase blood fl ow/pressure in the pulmonary circulation, like left to right shunts and/or severe pulmonary valve pathologies. Clear recommendations about their management are lacking. We report on a case of severe congenital pulmonary valve stenosis associated with a giant pulmonary artery aneurysm, secondary right ventricular outfl ow tract obstruction and right to left atrial shunt. We discuss the diagnostic pathway, therapeutic approach and patient evolution. The case report proves that a complete imaging of the heart and blood vessels is mandatory with severe pulmonary valve stenosis, in order to better describe its repercussions on the right ventricle and pulmonary circulation. They might be under detected by simple transthoracic echocardiography but their presence has important therapeutic implications. [ABSTRACT FROM AUTHOR]

In the first part of this thesis, we aimed to further define and improve on the current practice of free nipple grafting, specifically for NAC positioning and morphology. Firstly, we addressed the challenges of appropriate NAC localization. In Chapter 2, we proposed a two- step algorithm for the most ideal positioning of the nipple-areolar complex based on a large cohort of cisgender men. This methodological approach had to be further validated in an external cohort. A collaboration with Ghent University Hospital, Belgium was established and the algorithm was validated in a Belgian validation cohort. These outcomes are presented in Chapter 3. Secondly, we addressed NAC morphology. In this study, we aimed to establish the morphology of the free grafted nipples during a year follow-up. The results of this study, including the clinical implications and considerations are discussed in Chapter 4. In the second part of this thesis, we focused on surgical and scar outcomes following gender- affirming surgery. In Chapter 5, we addressed the reported risk that prior breast reduction surgery has on mastectomy outcomes. In an effort to decrease postoperative complications, we performed a within-patient randomized controlled trial in transmasculine individuals. To establish a better understanding of the long-term effects that incisional negative pressure wound therapy (iNPWT) has on scarring, a systematic review was performed incorporating all currently known pre-clinical and clinical outcomes. This review is presented in Chapter 6. In Chapter 7, we tested the efficacy of this dressing technique in preventing complications. The secondary outcomes of the within-patient randomized trial were patient-reported outcome measures and scar quality. Patients were followed for up to a year, and the results of these outcomes are presented in Chapter 8. In the last chapter of this thesis, Chapter 9, an overview is presented on techniques using skin substitutes for scar management, as a potential future modality to further improve surgical outcomes.

Introduction: The field of reconstructive surgery of the accidental main bile duct lesions involves several major categories, which, in turn, enroll some distinct phases due to their evolutionary characteristics. Each of these stages enforce different therapeutic options that are in close connection with the morphological and pathologic appearance of ductal lesions, which differ substantially depending mainly upon the timing of the diagnosis. Material and method: We bring into attention a collection of 4 cases with immediately recognized intraoperative lesions of the main biliary pathways that occurred during laparoscopic procedures for biliary lithiasis, since 2001. Although all cases fall into the same category of lesions, the circumstances being different so did the solutions adopted, tailored to the reality of the intraoperative findings, however the common ground was represented by the extraperitoneal drainage. Conclusions: Our short suite of clinical case presentation manages to draw attention to some interesting conclusions, such as it is somehow hazardous and not practical to compare clinical results obtained with various surgical techniques, since those are often in close connection with the details and particularities of the case; no single-center clinical experience, even if it is backed up by a large number of cases, can be imposed as a golden standard in resolving these injuries. Background: The lack of specific symptoms causes late presentation for patients with advanced T-stage malignant tumors (T3-T4) of the hypopharynx. This fact restricts surgical options to total laryngectomy with partial pharyngectomy or total laryngectomy with circular pharyngectomy. In most of these cases pharyngeal reconstruction, using flaps, is mandatory. Method:We present the case of a 61 years old male who underwent surgery for a malignant tumor of the right pyriform sinus extended to the base of the tongue and right tonsil. A total laryngectomy associated with partial pharyngectomy, partial resection of the base of tongue and right tonsillectomy were performed. The reconstruction of the lateral pharyngeal wall was achieved using a myocutaneous pediculated sternocleidomastoid flap. Results: The nasogastric tube was removed after 21 days. No pharyngocutaneous fistula was recorded. The follow up at 3, 6, and 9 months showed no signs of recurrence. Discussions: We consider that the sternocleidomastoid myocutaneous flap represents a valuable resource in the reconstruction of the lateral pharyngeal wall defects. [ABSTRACT FROM AUTHOR]

- Abstract
According to Romanisches Etymologisches Wörterbuch (REW), lat. frater was inherited in Romance languages with one (it. frate, rom. frate) or two (fr. frère) vibrant speech sounds and with two meanings "brother" and/or monk". Christianity loaded semantically much of the Latin lexicon, and some Romance words keep only the new meaning. The early Christians, disciples of Jesus Christ, were designated as "Christians" from outside, and as "brothers" from inside, first by Jesus Christ himself, and then between them (rom. fraţi în Hristos, fr. frères en Jésus-Christ). Lat. frater with the meaning "christian" is attested at the end of the second century and the biblical attestations make from this meaning a biblical meaning. During the fourth century AD, with the emergence of the monasticism, lat. frater is used as a title between monks (meaning "monk" attested at the end of the fourth century). Opinions are divided on the casual Latin form from which come the Romance words: nominative frater or accusative fratrem with Latin or Romance dissimilation of vibrant speech sounds. The dissimilate form conserved in Sardinian, the Protoromance and Romance semantic variation in kinship and religious field are just a few reasons for formal and semantic reconstruction of Protorom. */'fratr-e/, using comparative grammar reconstruction developped in the project Dictionnaire Étymologique Roman (DÉRom). Microtia is an uncommon pathology that is defined by a hypoplasia of the pinna. Multiple reconstructive surgeries have been developed. Nowadays, autologous rib cartilage reconstruction and Medpor implant reconstruction are the most widely used techniques. Total ear reconstruction for microtia depends on the degree of severity of hypoplasia. The goal is the reconstruction of an auricle with an appearance as close as possible to that of a normal ear. Even though the auricle is only a small part of the body, it is probably one of the most complex structures. That is why ear reconstruction is one of the most difficult tasks for a plastic surgeon. Darmstadt after Steinecke When Wolfgang Steinecke – the originator of the Darmstadt Ferienkurse – died at the end of 1961, much of the increasingly fragile spirit of collegiality within the Cologne/Darmstadt-centred avant-garde died with him. Boulez and Stockhausen in particular were already fiercely competitive, and when in 1960 Steinecke had assigned direction of the Darmstadt composition course to Boulez, Stockhausen had pointedly stayed away. Cage's work and significance was a constant source of acrimonious debate, and Nono's bitter opposition to him was one reason for the Italian composer being marginalized by the Cologne inner circle as a structuralist reactionary. Other Cologne figures were starting to assert their creative personalities, and look for their place in the sun: Argentinian-born Mauricio Kagel, whose Anagrama (1959) had upstaged the premiere of Stockhausen's Kontakte at the 1960 ISCM Festival in Cologne, was starting to rebel against Stockhausen's assumptions of supremacy; the Hungarian György Ligeti, disenchanted by the incessant conflicts, had left Cologne for Vienna just at the moment where his own distinctive compositional voice was starting to emerge in the orchestral work Apparitions (1960); and Gottfried Michael Koenig (b. 1926), who had been Stockhausen's right-hand man in the electronic studios since the mid-1950s, and whose Klangfiguren II (1956) had been the only work one could think of setting beside Gesang der Jünglinge, would shortly leave for Holland. Zogeheten fillers zijn vulmaterialen die om een medische of schoonheidsreden via een injectie worden aangebracht in het lichaam. Behandelingen met permanente fillers hebben een grotere kans op ernstige bijwerkingen dan behandelingen met niet-permanente fillers, zoals ernstige, terugkerende ontstekingen. Voor esthetische, dus ‘niet-reconstructieve’, toepassingen is het gebruik van permanente fillers sinds 2015 in Nederland verboden. In de praktijk mogen ze nog gebruikt worden om beschadigingen van het lichaam te herstellen die bijvoorbeeld als gevolg van een ongeval zijn ontstaan (reconstructieve toepassing). De Inspectie Gezondheidszorg en Jeugd (IGJ) heeft het RIVM daarom gevraagd om meer duidelijkheid te scheppen welke fillers in welke situatie gebruikt mogen worden. Daarvoor is uitgezocht welke fillers als permanent worden beschouwd en welke toepassingen met een filler als reconstructief moeten worden gezien. Het RIVM heeft de definities opgesteld op basis van literatuuronderzoek en interviews met wetenschappelijke verenigingen van artsen die fillers toepassen. Revitalizing disadvantaged regions by reinventing local poles (Case study: the Fortress of Ciceu Region). The Fortress of Ciceu, the core of an area which used to stretch over the territory of 63 villages during the Middle Ages, is currently in a very advanced state of degradation. The former domain of Ciceu, a possession in Ardeal of Moldavian princes Ştefan cel Mare, Petru Rareş or Alexandru Lăpuşneanu, includes today rural human communities in search of economic and cultural activities able to restore their importance from the 13th to 14th centuries. Therefore, having no industry and considering that a large part of the territory belongs to the disadvantaged agricultural areas, it has to fi nd new types of support, following the successful example of other areas in a similar position. Thus, by restoring the fortress and the surroundings (including the reconstruction of certain landmarks located on the site, landscaping arrangements to highlight the overall design and the introduction of animated routes – vantage point, demonstrative workshops, etc.), the national cultural heritage to which the fortress belongs as an A class historical monument will be properly highlighted, bringing important and direct benefi ts to the local communities.

The key elements in order to understand the interrelationships between the vital bone and different implant devices are represented by the acknowledging the bone formation process and structural components of the osseous tissue, of the evolution dynamics and bone reactivity towards different favourable and unfavourable influences, of the determining factors for the bone structure and reconstruction. Factors that disadvantage the osteogenetic potential are: age, nutritional diseases, endocrinopathies, ionizing radiation treatment, alcoholism and tabacosis. In stimulating and favouring osteogenesis and osteo reconstruction are used: tissue grafts, osteo substituents, growth hormones stimulating osteoblast function, materials guiding osteogenesis, hydroxyapatite and phosphorus-calcium materials as well as tissue growth factors specific to the bony tissue. For the process of implantation on some different devices in orthopedic practice, examination of the structure and bone density offers important information regarding osseointegration perspectives of future implants. Treatment with permanent fillers present a higher probability of serious adverse effects compared to treatments with non-permanent fillers, such as severe recurring inflammations. Since 2015 non-reconstructive applications with permanent fillers are not allowed in the Netherlands. In practise, permanent fillers may still be used, for instance to treat injuries that have resulted from an accident (reconstructive use). The Health and Youth Care Inspectorate therefore requested the RIVM to create clarity on which fillers should be used in which situation. Consequently, it was investigated which fillers should be considered as being permanent and which applications of permanent fillers should be considered as being reconstructive. RIVM has addressed these questions based on literature research and interviews with scientific associations of physicians who apply fillers. RIVM defines permanent fillers as filling agents that are not fully degradable and therefore remain present in the human body. According to all interviewed scientific associations permanent fillers have disadvantages compared to non-permanent fillers and should therefore not be applied anymore, not even in reconstructive applications.

There are a variety of techniques described in the literature, using both the autologous graft and the alloplastic materials. In the Democratic Republic of Congo (DRC), the mining sector has the potential to play a pivotal role in post-conflict reconstruction (World Bank, 2008), and artisanal mining sustains the livelihoods of millions people in the country (PACT, 2010). However, in the last 15 years, minerals from this artisanal mining have been ill-reputed. Eastern DRC has often been characterised by chronic instability and violent conflicts (Autesserre, 2010; Stearns, 2011) because it is widely believed that minerals in this region have attracted the greed of national and foreign armed groups, who benefit from the mining business. Although this ‘greed hypothesis’ has been criticised for its inconsistent performance in explaining resource-related conflicts (Le Billon, 2010; Ross, 2006), various national and international reform initiatives have gained momentum (Verbruggen et al., 2011). These initiatives aim to make the Congolese artisanal mining sector more transparent and to prevent ‘conflict minerals’ from entering the international market. In 2014, 13 reform initiatives—10 focusing on 3T (tantalum, tin and tungsten) and three on gold—were operational in eastern DRC (Cuvelier et al. 2014: 5). The implicit assumptions are that mining reforms will fully ‘clean’ artisanal mining of violence and corruption and that this will contribute to sustaining people’s livelihoods (Garrett and Mitchell, 2009: 12). This study investigated initiatives intended to ‘formalise’ artisanal mining in DRC—in other words, they aimed to bring mining under state control. The study especially focuses on the effects of one among these initiatives—the ITRI Tin Supply Chain Initiative (iTSCi)—on two groups of actors: miners (creuseurs) and middlemen (négociants). This thesis thus presents a fine-grained case study of the iTSCi. Designed by the International Tin Research Institute in 2009, iTSCi provides a means of determining the origin of 3T and documenting the trading chain for these minerals by ‘tagging and bagging’ the loads of 3T near miners’ shafts (at postes d’achat/selling points or buying stations), at counting offices (comptoirs) and in mineral depots, before the minerals are exported through the international market. This is a qualitative study undertaken at three coltan mining sites of northern Katanga: Kahendwa, Kisengo and Mai-Baridi. Coltan has been extracted at these sites since 2007. From March 2013 to September 2014, data were collected using participant observation of people’s practices (extraction/sale of coltan and various types of interactions between trading houses, cooperatives, mineworkers (creuseurs) and middlemen (negociants), as well as detailed in-depth interviews with creuseurs, negociants and their households. Data were also collected from the staff of mining cooperatives, trading houses, state authorities and civil servants—predominantly of the Service d’Assistance et d’Encadrement du Small-Scale Mining (SAESSCAM) and the Division des Mines. The last group of informants were a group of clandestine coltan negociants (known as hiboux—literally, ‘owls’), who were followed in the study. The purpose of this research is to study the micro-dynamics of changes after the reforms following the implementation of iTSCi. The study thus provides insights into how iTSCi is concretely implemented and how it has altered the organisation of mining and the trade of coltan. The study also aims to examine how this organisation affected creuseurs and negociants. The main research question of this study is as follows: How have initiatives to reform artisanal mining (iTSCi in particular) affected institutional change, how does this relate to changes in patterns of coltan production and trade, how were creuseurs and negociants affected by these changes, and how did these groups respond in the coltan mining areas of Kahendwa, Kisengo and Mai Baridi (northern Katanga) from 2009 to 2014? Analytically, the study adopted three main theoretical perspectives. First, an actor-oriented approach was taken, building on the premise that individual actors have the agency, knowledge and experience to reflect upon their situation and to respond to changes in their surrounding context (Giddens, 1984). Although the examined mining reforms consist predominantly of ‘ready-made’ techniques such as iTSCi’s ‘tagging and bagging’, analysing reforms with an actor orientation helps to highlight people’s reactions and responses. This includes how reform policies are applied in institutions (e.g. mining cooperatives), how they interact, how they are assigned meaning and how they are negotiated by social actors (Christoplos and Hilhorst, 2009). Second, the study builds on the sociology of economic life, which holds that economic action is a form of social action that is socially ‘embedded’, meaning that it is linked with or dependent on actions and institutions (such as social networks) that are noneconomic in content, goals and processes (Granovetter, 2005). This perspective facilitates the analysis of the livelihoods of negociants, including mechanisms of smuggling minerals into and beyond the mining areas where iTSCi is in force. Third, this thesis introduced the original concept of ‘enclaves of regulations’. These enclaves refer to the mining areas where iTSCi or other reforms are in force. This thesis has shown that, although these ‘enclaves’ appear to be ‘closed’ and insulated from the environment in terms of the locally applied rules for the mining and trading of minerals (e.g. ‘tagging and bagging’), in reality, such closure is not complete. This thesis has demonstrated that it would therefore be more appropriate to consider these ‘enclaves’ as semi-autonomous fields with porous boundaries. Apart from the introduction and the concluding chapters, this thesis is composed of five chapters. Chapter 2 explores the evolution of the mineral sector in the Katanga province. It analyses the history of mining, the initiation of artisanal mining and how the ongoing reforms have been informed by this history. In this chapter, it is shown that there is a long history of the organisation of mining in the Katangese province. The reforms therefore did not enter into a stage of anarchy, or an institutional void, but they added a layer to already existing forms of organisation. Chapter 3 focuses on mining cooperatives as newly introduced institutions aimed at governing the artisanal mining sites. Through a single case study, the chapter analyses how these cooperatives —especially the Cooperative des Artisanaux Miniers du Congo, CDMC—were introduced into the mining areas and how they interacted and blended with pre-existing miners’ organisations. This chapter demonstrates that cooperatives have been an emergent—rather than durable—solution in terms of representing the interests of artisanal miners. In Chapter 4, I provide a different perspective on ‘conflict minerals’. I thus introduce the notion of ‘reform conflicts’ to emphasise that, although ongoing reforms aim to sever the supposed linkages between the artisanal mining business and violent conflicts, these reforms have become a driving force behind the emergence of new conflicts over property rights and access to minerals. Chapter 5 is about livelihoods. It analyses how the reforms have influenced the livelihoods and socioeconomic position of negociants. This chapter also explores what kind of opportunities the reforms have offered to this group of mineral brokers often considered powerful in the mineral supply chain and explains what kind of constraints the negociants have confronted and why they have opted to diversify their livelihood portfolios. The chapter has shown that the reforms have affected this group of mineral brokers in different ways. Some negociants were well off, whereas others have been excluded from the mineral commodity chain. These findings contradict the widespread opinion that negociants are always abusive brokers in the mineral production and commodity chain. Chapter 6 analyses the responses of creuseurs and negociants to iTSCi. Although the mining sites where iTSCi is in force appear to be ‘enclaves of regulations’, I explore the strategies of creuseurs and negociants to bypass iTSCi and the reforms, especially around the coltan trade. This chapter demonstrates that coltan smuggling is a deeply rooted practice. Despite the reforms, smuggling continues in different forms. All of the elements highlighted above suggest that mining reforms have undergone a major shift, from addressing the initial problems associated with ‘conflict minerals’ to creating or reinforcing various types of problems, such as the influence of ‘big men’ in the mining business, coltan smuggling and the emergence of new conflicts over accessing minerals. This means that reform initiatives such as iTSCi should be based on knowledge about the actual situation. Thus, understanding and addressing these new types of problems calls for a comprehensive approach at both local and broader levels.

ACL rupture is in the most cases a sporting accident, due to the patient feels pain in knee joint, accompanied by regional deformation, enlarging joint volume and experiencing partial functional impotence. In our case, both tests for the instability of the knee joint were positive. After laboratory investigations consisting in sagittal radiography of the knee joint to rule out any bone lesions, MRI is done, putting it accurate diagnosis of ACL injury, more precisely an ACL rupture. For therapeutic purpose is decided arthroscopy, ACL reconstruction was done using semitendinosus muscle tendon. Functional recovery takes between 6-8 months, mainly consisting of physiotherapy. Het voordeel van de huidige mervoudige crisis kan evenwel zijn dat er eindelijk ens schot komt in de omvorming van de traditionele kwantiteitslandbouw naar een duurzame kwaliteitslandbouw en van het productielandschap naar een consumptielandschap. De reconstructiewet heeft daarin gefaald. De ons inziens noodzakelijke accentverschijving van landbouw naar landschap zou binnen kort wel eens door Brussel afgedwongen kunnen worden.

In juli 2012 hadden Gerard en Ina Scheffer uit Vorden hoge verwachtingen van hun nieuwe vleesvarkensstal. De eerste weken verliepen probleemloos. Toen sloeg de Wet van Murphy toe: alles wat mis kan gaan, ging ook mis. „Het waren geen kinderziektes meer, maar grotemensenproblemen.” Nu, bijna drie jaar verder, hopen ze dat de problemen zijn overwonnen.
