Background and Objectives: In this study, we aimed to evaluate pain intensity in patients after pilonidal disease surgeries of varying extent using pressure algometry and the visual analog scale and to explore potential correlations between these methods. Materials and Methods: A total of 78 adult patients with symptomatic pilonidal cysts were enrolled in this study. The patients were divided into two groups based on the type of surgery assigned to each patient at the pre-hospital consultation: pit-picking surgery (n = 39) and radical excision (n = 39). The pain levels at the surgical site were assessed and compared using the visual analog scale (VAS) and pressure algometry the morning before surgery and the day after the operation. Results: There was no statistically significant difference (p > 0.05) in VAS measurement results between surgical groups when comparing pain intensity experienced by patients before, during, and after surgery. While several objective gait analysis systems have been developed and validated to quantify lameness severity in horses, methods to evaluate whether peripheral sensitization contributes to the pain experienced are missing. Objectives: To evaluate whether periarticular pressure pain mapping could be proposed as an auxiliary assessment tool in horses. Specific aims were to evaluate the feasibility and intra- and inter-rater reliability of pressure pain thresholds (PPT) determination at sites overlying the distal thoracic limb joints of clinically healthy horses. Study design: Prospective, randomized validation study. Methods: For feasibility assessment, PPT were measured with a hand-held digital algometer at six periarticular landmarks (2 sites per joint, 3 joints) bilaterally on the distal thoracic limb of 40 healthy horses (20 warmblood and 20 Freiberger). The joints tested were the metacarpophalangeal, on the lateropalmar and dorsal aspects (L-MCP and D-MCP), the proximal interphalangeal, on the dorsal and palmar aspect (D-PIP and P-PIP) and the distal interphalangeal, on the dorsal and lateral aspect (D-DIP and L-DIP). A feasibility score, ranging from 0 to 5, was attributed to each testing session. For intra- and inter-rater reliability assessment, L-MCP and D-MCP were selected to be tested again at 2 weeks intervals in 20 out of the 40 horses. Data were analyzed using a mixed-effect linear model to test differences in threshold per site and limb. Intra- and inter-rater correlation was calculated. Bland-Altman plots were performed to evaluate the variability of the measures. Results: The procedure was considered feasible (score <2) in 95% of horses (95% CI 88%--100%). Overall, median [interquartile range (IQR)] PPT was 9.4 (7.5--11.3) N. No significant side differences were found. P-PIP and D-DIP recorded significantly lower PPT (p < 0.001 and p = 0.002, respectively) than L-MCP. Median (IQR) were 9.9 (7.3--12.4) N, 8.4 (6.1--10.5) N and 9.0 (7.4--10.6) N for L-MCP, P-PIP and D-DIP, respectively. The intra-rater agreement was 0.68 (95% CI 0.35--0.86) for L-MCP, and 0.50 (95% CI 0.08--0.76) for D-MCP. Inter-rater agreement was 0.85 (95% CI 0.66--0.94) for L-MCP and 0.81 (0.57, 0.92) for D-MCP. Main limitations: Evaluation of feasibility was performed only for distal thoracic limbs joints; no data are provided for hind limbs or proximal joints. Only warmblood and Freiberger horses were included. Intra- and inter-rater reliability assessments were performed exclusively on data collected at the MCP joint. Conclusion: Pressure pain mapping of distal thoracic limb joints was feasible in horses. Local sensitivity differed among sites and no side differences were noticed. Data collected from the MCP joint suggest highly variable, subject dependent intra-rater reliability, ranging from poor to good, and good to excellent inter-rater reliability. Viscoelastic creep generated in the lumbar spine following sustained spine flexion may affect the relationship between tissue damage and perceived pain. Two processes supporting this altered relationship include altered neural feedback and inflammatory processes. Out purpose was to determine how low back mechanical pain sensitivity changes following seated lumbar spine flexion using pressure algometry in a repeated-measures. cross-sectional laboratory design. Thirty-eight participants underwent a 10-minute sustained seated maximal flexion exposure with a 40-minute standing recovery period. Pressure algometry assessed pressure pain thresholds and the perceived intensity and unpleasantness of fixed pressures. Accelerometers measured spine flexion angles, and electromyography measured muscular activity during flexion. The fiexion exposure produced 4.4° (2.7°) of creep that persisted throughout the entire recovery period. The perception of low back stimulus unpleasantness was elevated immediately following the exposure, 20 minutes before a delayed increase in lumbar erector spinae muscle activity. Women reported the fixed pressures to be more intense than men. Sustained flexion had immediate consequences to the quality of mechanical stimulus perceived but did not alter pressure pain thresholds. Neural feedback and inflammation seemed unlikely mechanisms for this given the time and direction of pain sensitivity changes, leaving a postulated cortical influence. [ABSTRACT FROM AUTHOR]

Pharmacodynamic understanding of the different local anesthetic concentrations allows adapting their use to diverse clinical/surgical procedures, such as intraoperative and/or postoperative analgesia. A crossover study was performed, where 6 calves (5 male and 1 female), weighing 120 ± 28 Kg, were subjected to combined sciatic and femoral nerve block using three ropivacaine concentrations. The treatments were: R0.75, using 0.75% ropivacaine; R0.2, 0.2% ropivacaine; and R0.12%, 0.12% ropivacaine. All treatments were performed with ultrasound and neurostimulation assistance, and a volume of 0.1 mL/kg of the respective local anesthetic solution was administered in each block point. The sites of mechanical nociceptive threshold (MNT) evaluation were based on the calf pelvic limb dermatomes. The proportion between desensitized areas, MNT elevation time and level of ataxia were registered. Elevation of MNT occurred in 100% of the tested areas in the R0.75 and R0.2 treatments, and in 82% of the R0.12 treatment. Mean MNT elevation times were 9.5 ± 0.7 h for R0.75, 6 ± 0.8 for R.02, and 2.4 ± 2.3 for R0.12, differing significantly between all treatments. No difference was observed between MNT elevation time and ataxia duration time, in each treatment. It is concluded that the duration of sensory-motor effects is dose-dependent, but there was not possible to detect block selectivity as the concentrations was reduced. More desensitized areas and extension were obtained with the use of higher concentrations.

Background: Low back pain is considered one of the most common problems which occurs during pregnancy and may be persistent for months after delivery. It can be disabling and can limit function of mother after delivery. Objectives: The purpose of the study was to evaluate the impact of dry needling on postnatal low back pain. Methods: This study included 50 multiparous women who had experienced low back discomfort (for at least 3 months) after giving birth. Their body mass index did not exceed 30 kg/m2, the parity ranged from 2 to 4 times, while the type of birth was normal labor. Their ages ranged from 25 to 35 years old. After being randomly distributed into two groups of similar size, The study group: which included 25 patients, received dry needling on the lower back for 20 minutes, three times per week for six weeks, Additionally, each patient completed an exercise program consisting of abdominal, back, and pelvic floor strengthening exercises as well as postural correction exercises for 60 minutes, three times weekly for a period of six weeks. The control group: which included 25 patients, only completed an exercise program for 60 minutes, three times weekly for a period of six weeks. The pain levels of all women in both groups were assessed using a pressure algometry device and a visual analogue scale before as well as after treatment program. Results: there was a statistically highly significant decrease in visual analogue scale and significant increase in pressure algometry in the study group when comparing both groups (A&B) together. Summary: Background: Back pain in horses, regardless of the underlying cause, is a common condition that can result in poor performance. The field veterinarian has limited noninvasive treatment options for reduction in back muscle soreness. This study was designed to evaluate effectiveness of a commercially available herbal, nonheating liniment (Sore No More Performance Ultra, Arenus), in reducing pain and inflammation in horses with mild back pain. Objectives: Focus on a standardised area of the lumbar back region to evaluate effects of an herbal liniment over 44 days using pressure algometry to determine pain response and thermographic measurement of surface temperature to evaluate inflammation. Study design: A randomised complete block design was used. Eighteen mature stock horses were evaluated for back pain and randomly assigned to treatment group (herbal liniment or control solution). Methods: All horses maintained similar light work 5 days per week in a university equestrian programme for 44 days. Evaluation of the back was standardised by identifying the lumbar region, and pain and inflammation assessed weekly with force gauge pressure readings and thermal images. Results: All horses displayed a decrease in pain scores over the first 3 weeks of the study, but the control group saw a rise in pain scores at Day 23. Pain scores on the left side of the back were significantly lower in the liniment group than the control group at Day 30. Main limitations: Number of horses available for the study was limited. Daily exercise of the horses used represented typical use of mature lesson horses, however, was not standardised for the entirety of the 44‐day trial. Conclusions: Standardised evaluation procedures using pressure algometry and thermography found that Sore No More Performance Ultra herbal liniment had some effect on thermography (inflammation) and pain response of the back, with improvements shown at 23–30 days. Kinesio taping (KT) is a technique extrapolated from human physiotherapy consisting of the application of an elastic tape to the skin to trigger analgesic, muscular, postural correction and circulatory effects. It is an easily applicable technique that has been developed in the field of equine physiotherapy over the last decade. The objective of this research is to evaluate the analgesic effect of KT applied to spinous processes of the horse measuring mechanical nociceptive threshold (MNT). KT was applied on 5 spinous processes of 15 horses, in two different experiments, comprising KT with 50% tension (KTT) and KT with no tension (KTNT). Measurements were taken before application of the tape (M0), 60 minutes after (M1) and 24 hours after, following its removal (M2). Clinical assessment of sensitivity to palpation was conducted at M0 and M2. Outcomes obtained at M0 were compared to those obtained at M1 and M2, and between both tests (KTT-KTNT). A significant increase in the MNTs at M1 was observed in both tests but not maintained following its removal 24 hours later. Sensitivity to palpation decreased in practically all the spinous processes in both tests. No significant changes were observed in the comparative analysis between both tests. KT applied to spinous processes of the horse with and without tension causes an increase in the MNTs 60 minutes after application. This effect is not sustained following taping removal although there is a clinically significant decrease of the sensitivity to palpation of the spinous processes.

BackgroundA pressure algometer is a valuable tool for assessing the mechanical nociceptive threshold (MNT) in clinical pain studies. Recent research has turned to large animal models of pain because of the closer anatomy and physiology to humans. Although the reliability and usefulness of the MNT have been extensively validated in humans, similar data from large animals is still sparse.ObjectiveTherefore, the aim of the current study was to evaluate the reliability (within- and between-session) of MNT in the forelimb of pigs using a pressure algometer.MethodsNine animals were used (23–40 kg), and MNTs were measured at both the right and left limbs at three different sessions, with three repetitions per session. The intraclass correlation coefficient (ICC) was used as a metric for relative reliability. The standard error of measurement (SEM) and coefficient of variation (CV) was used to assess absolute reliability. Systematic bias was also evaluated.ResultsThe average ICC was found to be 0.71 and 0.45 for the between-session and within-session, respectively. CV ranged from 17.9% to 20.5%, with a grand average of 19.1%. The grand average SEM was 249.5 kPa (16.6%). No systematic differences were found for the MNT between sessions, which suggests that there was no habituation to the stimulus.ConclusionThe reliability indices obtained in this study are comparable to results obtained in other species or anatomical regions and substantiate the use of the pressure algometer as a valuable tool to investigate the nociceptive system in pigs and translation to the human nociceptive withdrawal reflex.

Background: Chronic low back pain (CLBP) is one of the major health problems and CLBP patients suffer from reduced quality of life and loss or limitation of employment.Objectives: This study was aimed at comparing pinch pressure pain sensibility of the skin of total low back area in patients with nonspecific low back pain.Methods: This observational, case-control study was conducted on patients who had suffered from chronic low back pain (LBP) with an intensity score of more than 25 (using the visual analog scale, VAS; 0–100 mm) for at least constant three months between October 2012 and June 2013. By mechanical pinching the skin of the lower back area, skin hyperalgesia was assessed in 193 patients. Following a double-blind design, skin hyperalgesia was also assessed for the skin of the control group (108 people).Results: Signs of hyperalgesia in the upper lateral quadrant of the gluteal area, around the posterior part of the iliac crest, were found in both groups but the degree of hyperalgesia in chronic nonspecific LBP patients (with & without symptoms) was significantly higher (P

Chronic low back pain is a global socioeconomic crisis and treatments are lacking in part due to inadequate models. Etiological research suggests that the predominant pathology associated with chronic low back pain is intervertebral disc degeneration. Various research teams have created rat models of disc degeneration, but the clinical translatability of these models has been limited by an absence of robust chronic pain‐like behavior. To address this deficit, disc degeneration was induced via an artificial annular tear in female Sprague Dawley rats. The subsequent degeneration, which was allowed to progress for 18‐weeks, caused a drastic reduction in disc volume. Furthermore, from week 10 till study conclusion, injured animals exhibited significant axial hypersensitivity. At study end, intervertebral discs were assessed for important characteristics of human degenerated discs: extracellular matrix breakdown, hypocellularity, inflammation, and nerve sprouting. All these aspects were significantly increased in injured animals compared to sham controls. Also of note, 20 significant correlations were detected between selected outcomes including a moderate and highly significant correlation (R = 0.59, p < 0.0004) between axial hypersensitivity and disc nerve sprouting. Objective To investigate the efficacy of extracorporeal shockwave therapy (ESWT) on cervical myofascial pain following neck dissection in reducing pain and improving cervical range of motion (ROM). Methods Forty-six patients with cervical myofascial pain following neck dissection surgery were recruited and subdivided at random into two equal groups. The ESWT group received ESWT once a week for 4 weeks (0.25 mL/mm2, 1,000 shocks) and a topical non-steroidal anti-inflammatory drug (3 times/day for 4 weeks). The control group received only topical NSAID. The pain assessment was done by using the visual analog scale (VAS) and pressure algometry. A cervical ROM device was used for the assessment of the lateral flexion and rotation of the neck ROM on both sides. All measurements were collected at baseline, 2 weeks, and 4 weeks. Results The ESWT group revealed a significant improvement in all parameters at post I and post II than did the control group (p>0.001), that revealed a statistical decrease only in the VAS score at post I without any statistical difference in the pain threshold and neck ROM. Abstract Background A research gap exists for optimal management of cervical radiculopathy in the first 12 weeks and short term natural history of the condition is somewhat unclear, although thought to be favourable. The primary aim of this assessor blinded, superiority, 2 parallel group randomised controlled trial is to investigate the effects of a 4 week physiotherapy programme (6–8 sessions) of manual therapy, exercise and upper limb neural unloading tape, compared to a control of weekly phone advice; on disability, pain and selected biopsychosocial measures, in acute and sub-acute cervical radiculopathy patients. A secondary aim is to identify whether any baseline variables, symptom duration or group allocation can predict outcome. Methods Participants are recruited from GP referrals in an urban setting, from a neurosurgery non-urgent waiting list and from self-referral through Facebook advertising. Eligible participants (n = 64) are diagnosed with radiculopathy based on a clinical prediction rule and must have symptoms of unilateral, single level, radiculopathy for between 2 and 12 weeks, without having yet received physiotherapy. Random 1:1 group allocation (using variable block sizes), allocation concealment, blinded assessment and intention to treat analysis are being employed. Treatment is provided by clinical specialist physiotherapists in primary and secondary care settings. Outcomes are measured at baseline, 4 (primary endpoint) and 12 weeks. Participants’ report of pain, disability and their rating of recovery is also recorded by telephone interview at 6 months. Statistical analysis of between group differences will be performed with ANOVAs and MANOVAs, and multivariable regression analysis will be undertaken to explore predictor variables. Ethical approval for this study has been received from the Beaumont Hospital and Irish College of General Practitioners Research Ethics Committees. The trial is registered at ClinicalTrials.gov (NCT02449200). Discussion An internal pilot study to test retention and recruitment strategies led to trial expansion and this is now a multi centre trial involving 5 clinical sites. Trial registration NCT02449200. Background Pain assessment is a key measure that accompanies treatments in a wide range of clinical settings. A low-cost valid and reliable pressure algometer would allow objective assessment of pressure pain to assist a variety of health professionals. However, the pressure algometer is often expensive, which limits its daily use in both clinical and research settings. Objectives This study aimed to assess the instrumental validity, and the intra- and inter-rater reliability of an inexpensive digital adapted pressure algometer. Methods A single rater applied 60 random compressions on a force platform. The pressure pain thresholds of 20 volunteers were collected twice (3 days apart) by two raters. The main outcome measurements were as follows: the maximal peak force (in kPa) and the pressure pain threshold (adapted pressure algometer vs. force platform). Cronbach’s α test was used to assess internal consistency. The standard error of measurement provided estimates of measurement error, and the measurement bias was estimated with the Bland–Altman method, with lower and upper limits of agreement. Results No differences were observed when comparing the compression results (P = 0.51). The validity and internal intra-rater consistencies ranged from 0.84 to 0.99, and the standard error of measurement from 0.005 to 0.04 kPa. Very strong (r = 0.73–0.74) to near-perfect (r = 0.99) correlations were found, with a low risk of bias for all measurements. The results demonstrated the validity and intra-rater reliability of the digitally adapted pressure algometer. Inter-rater reliability results were moderate (r = 0.55–0.60; Cronbach’s α = 0.71–0.75). Conclusion The adapted pressure algometer provide valid and reliable measurements of pressure pain threshold. The results support more widespread use of the pressure pain threshold method among clinicians.

Background With the hypothesis that equine dorsal lamellar tissue can be desensitized by anesthesia injection into distal interphalangeal joint (DIPJ), the objective was to assess the mechanical nociceptive threshold of hoof dorsal lamellae following intra-articular (IA) administration of lidocaine into this joint. Methods The DIPJ of the forelimbs of six adult healthy horses were injected with either 5 mL of lidocaine, or 5 mL of lactated Ringer’s solution. Treatments were randomly distributed, with each forelimb undergoing a single treatment. The hooves were evaluated pre- and post-injection at pre-selected times over 4 h, using a pressure algometry model. Objective Nociception caused by injuries may sensitize central mechanisms causing expanded pain areas. After recovery, the status of such pain distribution and sensitivity mechanisms is unknown. The present study investigated whether individuals who have fully recovered from a distal radius fracture demonstrate increased pain sensitivity and expanded distribution of pressure-induced pain. Subjects Twenty-three pain-free individuals with a history of painful distal radius fracture and 22 nonfractured, age/gender-matched controls participated in two experimental sessions (day 0, day 1) 24 hours apart. Methods Pressure pain thresholds (PPTs) were recorded bilaterally at the extensor carpi radialis longus (ECRL), infraspinatus, and gastrocnemius muscles. Spatial distribution of pain was assessed following 60-second painful pressure stimulation at the ECRL (bilateral) and the infraspinatus muscles on the fractured or dominant side. Participants drew pain areas on a body map. After day 0 assessments, prolonged pain was induced by eccentric exercise of wrist extensors on the fractured/dominant side. Results Compared with controls, pressure-induced ECRL pain in the fracture group referred more frequently toward the distal forearm (P  <   0.005) on day 0. Both groups showed larger pain areas on day 1 compared with day 0 (P  <   0.005), although the fracture group showed a larger relative change between days (P  <   0.005). The fracture group showed larger pain areas on the fracture side compared with the contralateral side on both days (P  <   0.005). Conclusions Prolonged pain and recovered prior painful injuries like fractures may sensitize pain mechanisms manifested as expanded pain distribution. Objective: To investigate the efficacy of extracorporeal shockwave therapy (ESWT) on cervical myofascial pain following neck dissection in reducing pain and improving cervical range of motion (ROM). Methods Forty-six patients with cervical myofascial pain following neck dissection surgery were recruited and subdivided at random into two equal groups. The ESWT group received ESWT once a week for 4 weeks (0.25 mL/mm2, 1,000 shocks) and a topical non-steroidal anti-inflammatory drug (3 times/day for 4 weeks). The control group received only topical NSAID. The pain assessment was done by using the visual analog scale (VAS) and pressure algometry. A cervical ROM device was used for the assessment of the lateral flexion and rotation of the neck ROM on both sides. All measurements were collected at baseline, 2 weeks, and 4 weeks. Results The ESWT group revealed a significant improvement in all parameters at post I and post II than did the control group (p>0.001), that revealed a statistical decrease only in the VAS score at post I without any statistical difference in the pain threshold and neck ROM. However, there were statistical differences in all parameters at post II compared to those at pre-treatment and post I (p<0.001). Background. With the hypothesis that equine dorsal lamellar tissue can be desensitized by anesthesia injection into distal interphalangeal joint (DIPJ), the objective was to assess the mechanical nociceptive threshold of hoof dorsal lamellae following intra-articular (IA) administration of lidocaine into this joint. Methods. The DIPJ of the forelimbs of six adult healthy horses were injected with either 5 mL of lidocaine, or 5 mL of lactated Ringer's solution. Treatments were randomly distributed, with each forelimb undergoing a single treatment. The hooves were evaluated pre- and post-injection at pre-selected times over 4 h, using a pressure algometry model. Mechanical nociceptive thresholds (MNTs)were recorded for the sole (dorsal, palmarolateral, and palmaromedial regions), coronary band (medial, lateral, and dorsal regions), heel bulbs (medial and lateral), and dorsal lamellar region (2 cm and 4 cmdistal to the coronary band). The MNT means were compared over time using the Friedman test and between treatments using the Wilcoxon signed-rank test, with values of P <0:05 considered statistically significant. Results. There were no differences between treatments for any region of the hoof during the evaluation period. However, MNT values indicating analgesia were recorded in the dorsal lamellar region in 50% of hooves following administration of lidocaine into the DIPJ. Conclusion. Background and aims: This prospective study aimed to assess pressure pain thresholds (PPTs) by pressure algometry and the correlation to postoperative pain in children undergoing orthopaedic surgery. We hypothesized, that the PPTs would decline immediately after elective orthopaedic surgery and return to baseline values at follow-up. Methods: Thirty children aged 6–16 years were included. PPTs and intensity of pain (Numerical Rating Scale, NRS) were assessed 3–6 weeks before surgery (baseline), 1–2 h before surgery (Day 0), the first postoperative day (Day 1) and 6–12 weeks after surgery (Follow-up). Results: A significant difference of PPTs between the four assessments was seen using the Friedman test for detecting differences across multiple tests and Wilcoxon signed-rank test with a Bonferroni adjustment. The changes in PPTs between baseline (PPTcrus = 248 kPa, PPTthenar = 195 kPa) and day 1 (PPTcrus = 146 kPa, PPTthenar = 161 kPa) showed a decline of PPTs as hypothesized (Zcrus = 2.373, p = 0.018; Zthenar = 0.55, p = 0.581). More surprisingly, a significant decrease in PPTs between baseline and day 0, just before surgery (PPTcrus = 171 kPa, PPTthenar = 179 kPa), was also measured (Zcrus = 2.475, p = 0.013; Zthenar = 2.414, p = 0.016). PPTs were positively correlated to higher age, weight and height; but not to NRS or opioid equivalent use. Conclusions: Children undergoing orthopaedic surgery demonstrate significant changes in PPTs over time. The PPTs decrease significantly between baseline and day 0, further decreases the first day postoperatively and returns to baseline values at follow-up. This suggests that other factors than surgery modulate the threshold for pain. Implications: Awareness of pressure pain thresholds may help identify children with affected pain perception and hence improve future pain management in children undergoing orthopaedic surgery. Factors as for example anticipatory anxiety, psychological habitus, expected pain, catastrophizing, distraction, physical activity, patient education and preoperative pain medication might play a role in the perception of pain and need further investigation. Objective Pain hypersensitivity has been described as one of the most disabling symptoms of fibromyalgia syndrome (FMS). Here we analyzed the relationship between an anti-inflammatory diet profile and the pressure pain thresholds (PPTs) of tender point sites and other fibromyalgia-related symptoms in patients with FMS. Methods This cross-sectional study included 95 women diagnosed with FMS and 98 menopause-status matched controls. The Dietary Inflammatory Index (DII) was calculated by conducting a 24-hour diet recall interview. The PPTs of tender point sites and self-reported global pain levels were evaluated by algometry and the visual analog scale, respectively. Disease severity, fatigue, sleep anxiety, and central sensitization were also evaluated. Results Linear regression analysis revealed that the PPTs of tender point sites including the occiput (β  = 0.234, 95% confidence interval [CI] = 0.016–0.452, P  =   0.036), trapezius (β  = 0.299, 95% CI = 0.083–0.515, P  =   0.007), zygapophyseal joint (β  = 0.291, 95% CI = 0.022–0.559, P  =   0.035), second rib (β  = 0.204, 95% CI = 0.060–0.348, P  =   0.006), gluteus (β  = 0.591, 95% CI = 0.110–1.072, P  =   0.017), greater trochanter (β  = 0.379, 95% CI = 0.016–0.742, P  =   0.041), and knee (β  = 0.482, 95% CI = 0.117–0.850, P  =   0.011) were associated with DII score after adjustments for the age, menopausal status, and global energy levels reported by the patients with FMS. No significant differences were found for the cases or controls between the DII score and the remaining clinical symptoms. Analyses of covariance showed that the PPTs of the aforementioned tender point sites were also significantly associated (P  <   0.05) with the DII score quartiles in patients with FMS, but no significant differences were found between these quartiles and the other clinical symptoms. Aims: Whiplash injury to the neck is often considered a significant risk factor for development of temporomandibular disorders (TMD), and has been proposed to produce internal derangements of the temporomandibular joint (TMJ). Few studies, however, have examined TMD-related pain in acute whiplash patients compared with a matched control group. The aim of the present stud), was to assess pain and sensorimotor function in the craniofacial region in an unselected group of patients sustaining a motor vehicle accident revolving a rear collision. Methods: Prospectively, 19 acute whiplash patients exposed to a motor vehicle accident involving a rear collision participated in a study of TMD. The control group consisted of 20 age- and gender-matched ankle-injury patients. Participants were seen within 4 weeks and again at 6 months post-injury. The masticatory system was examined in accordance with the research diagnostic criteria. Participants underwent structured interviews, filled out the McGill Pain Questionnaire (MPQ), and had their masticatory system examined by a trained dentist, blinded to their diagnosis. Pain detection threshold (PDT) to pressure stimuli, and maximal voluntary occlusal force (MVOF) were obtained at each visit. Results: One whiplash patient and I ankle-injury patient had jaw pain at the first visit. Palpation scores of the TMJ and the summated palpation scores only tended to be higher in patients sustaining a whiplash injury than in ankle-injury controls at the first visit. However, MPQ, TMD symptoms and signs, MVOF and PDT were not significantly different in whiplash-injury and ankle-injury patients after 4 weeks and 6 months. Conclusion: TMD pain after whiplash injury and ankle injury is rare, suggesting that whiplash injury is not a major risk factor for the development of TMD problems. Background: Shoulder girdle pain is a common disabling complaint with a high lifetime prevalence. Interventions aimed at decreasing shoulder pain without stressing shoulder girdle structures have the potential to improve participation in multimodal shoulder rehabilitation programs. Hypothesis/Purpose: The aim of this study was to determine the acute effects of moderate intensity lower extremity exercise on mechanically induced shoulder pain in individuals without shoulder injury. It was hypothesized that participants would exhibit less shoulder pain, as indicated by increased pain thresholds, following lower extremity exercise. Study Design: Repeated measures study. Methods: Thirty (30) healthy participants were recruited to participate in this study. Pain pressure algometry was used to mechanically induce shoulder pain over the infraspinatus muscle belly. This was performed on the dominant shoulder before and immediately after performing 10 minutes of moderate intensity lower extremity exercise using a recumbent exercise machine. Heart rate and rate of perceived exertion were measured following exercise. Repeated measures ANOVA was used to compare pain pressure threshold scores between the baseline and post-exercise time points. Significance was set at p ≤ 0.05 a priori. Effect size (ES) was calculated using Glass's Δ. Results: Moderate intensity lower extremity aerobic exercise led to significantly (F=8.471, p=0.003) decreased evoked shoulder pain in healthy adults with moderate effect sizes (0.30-0.43). Conclusions: Lower extremity aerobic exercise significantly decreased pain of the infraspinatus in this sample of young healthy participants. The objective of this study was verify if there are differences in the pain threshold between men and women, using the pressure algometer. The electronic search strategy was carried out using the PubMed, BIREME, LILACS, MEDLINE, PEDro, Cochrane Library, SCIENCE DIRECT and SciELO databases from August to December 2017. Observational studies were carried out to evaluate the differences in the threshold of pressure through algometry. We included 16 articles, 8 of them (50%) found differences between sexes and 8 (50%) did not. Among these, the justification for not presenting differences between the sexes is due to factors such as pain intensity independent of the application site (11.11%) and psychological factors (44.44%). There is still divergence in the literature about the existence or not of difference in the pain threshold between men and women. Therefore, further studies are needed to determine standardized values of pain thresholds and their sites in both the healthy population and pre-existing pathology. [ABSTRACT FROM AUTHOR]

Hot-iron disbudding in goat kids causes acute pain and tissue damage. However, the duration of healing and wound sensitivity is unknown. We assessed wound healing and pain sensitivity in 18 female dairy goat kids disbudded with a heated iron at 10 d of age (range: 5–15 d). Pressure algometry was carried out twice a week from d 1 after disbudding to determine the mechanical nociceptive threshold (MNT) in 4 locations on each bud (front, lateral, caudal, and middle). At the same time, digital and infrared images of the wounds were used to visually and thermally describe the healing process. Wounds were visually scored daily for the presence of 7 tissue types: attached necrotic tissue, detached necrotic tissue, burns outside the necrotic ring, exudate, granulation, crust, and epithelium. All data were taken until epithelium was present for 4 consecutive days. Necrotic tissue detached completely from the scalp 26 ± 5 d after the procedure (mean ± SD; range: 17–43 d), and wounds took 50 ± 8 d (35–63 d) to re-epithelialize. Wounds were more sensitive at all stages of the healing process compared with re-epithelialized tissue. The caudal and middle locations were the most- and least-sensitive test sites (1.24 ± 0.10 and 1.90 ± 0.10 N, respectively; mean ± SE). Goats became less responsive to stimulation as their wounds decreased in size. Sensitivity did not differ between left and right sides of the head. Maximum surface temperature of necrotic tissue, when present, tended to be higher than that of epithelium (38.8 ± 0.2 vs. 38.1 ± 0.2°C); temperature did not differ among other tissue types. Our results indicate that hot-iron disbudding wounds in goats take, on average 7, wk to re-epithelialize (35–63 d) and are painful throughout this time, raising additional welfare concerns about this procedure. To assess the clinical course of abdominal wall sensitivity after ventral midline coeliotomy in horses by determining mechanical nociceptive thresholds (MNTs) during hospitalization, and to determine the inter-observer reliability of pressure algometry on the abdominal wall. Observational, cohort study. A total of 13 horses presenting with signs of abdominal pain/colic undergoing ventral midline coeliotomy and 10 healthy horses without an abdominal incision. Measurements were performed on days 1, 3, 5, 7 and 9 postoperatively using a pressure algometer. Measurement sites were marked left and right, abaxial to the abdominal incision. Cranial to the incision, two control points were marked. Measurements were made by one observer, blinded to the recorded MNT values. To determine inter-observer reliability, five horses (surgical group n = 2; nonsurgical group n = 3) were measured by two observers in a randomized order. Mean MNT values on days 5 and 7 were 9.61 Ncm−2 and 10.14 Ncm−2 in the operated group (p = 0.009 ; p = 0.005) respectively versus 13.00 Ncm−2 on day 1. Wound-associated points showed lower values than control points (p = 0.002). The nonsurgical group did not show a difference between control points and wound-associated points (p = 0.06). No significant differences were found between the surgical and the nonsurgical groups at the wound-associated points on any days measured. The inter-observer reliability was low (intraclass correlation coefficient of 0.26; Cronbach's alpha of 0.27). Operated animals showed a reduction in MNT values on days 5 and 7 when compared with day 1 and lower values for the wound-associated points when compared with the control points. Inter-observer reliability was low. Purpose: Anterior knee pain is a frequent symptom in the osteoarthritic knee. Its intensity has been classically evaluated by the visual analogic score (VAS). The main objective of the study was to assess whether pressure algometry (PA) can be used as a reliable tool to quantify anterior pain in the osteoarthritic knee. Methods: A prospective study was designed, including 160 patients scheduled for unilateral total knee arthroplasty. Patellar pain and function were evaluated according to a specific patellofemoral pain questionnaire and the Knee Society Score. Patients were asked about their pain at rest, while walking and in the stairs with the VAS score. PA by an algometer was used in the anterior part of the patella to determine the pressure pain threshold (PPT): the minimal pressure when the patient feels a painful sensation. Results: The mean values of PPT were 385.1 kPa in the knees going to be operated and 403.4 kPa in the contralateral knees (p < 0.05). The anterior knee pain in the patellar score averaged 6.9, and the total patellar score averaged 15.2. PPT values were correlated with the anterior pain in the patellar score (ρ = 0.31, p < 0.001) and also with the total patellar score (ρ = 0.33; p < 0.001). Conclusions: PA demonstrated to be suitable to measure anterior knee pain in the osteoarthritic knee. PA can specifically assess the anterior knee pain better than other methods as VAS score. Background: A research gap exists for optimal management of cervical radiculopathy in the first 12 weeks and short term natural history of the condition is somewhat unclear, although thought to be favourable. The primary aim of this assessor blinded, superiority, 2 parallel group randomised controlled trial is to investigate the effects of a 4 week physiotherapy programme (6-8 sessions) of manual therapy, exercise and upper limb neural unloading tape, compared to a control of weekly phone advice; on disability, pain and selected biopsychosocial measures, in acute and sub-acute cervical radiculopathy patients. A secondary aim is to identify whether any baseline variables, symptom duration or group allocation can predict outcome.Methods: Participants are recruited from GP referrals in an urban setting, from a neurosurgery non-urgent waiting list and from self-referral through Facebook advertising. Eligible participants (n = 64) are diagnosed with radiculopathy based on a clinical prediction rule and must have symptoms of unilateral, single level, radiculopathy for between 2 and 12 weeks, without having yet received physiotherapy. Random 1:1 group allocation (using variable block sizes), allocation concealment, blinded assessment and intention to treat analysis are being employed. Treatment is provided by clinical specialist physiotherapists in primary and secondary care settings. Outcomes are measured at baseline, 4 (primary endpoint) and 12 weeks. Participants' report of pain, disability and their rating of recovery is also recorded by telephone interview at 6 months. Statistical analysis of between group differences will be performed with ANOVAs and MANOVAs, and multivariable regression analysis will be undertaken to explore predictor variables. To investigate the association of pain sensitivity and endogenous analgesia capacity, and training volume in a group of competitive swimmers. An observational multi-center study. Multiple competitive swimming clubs. 102 healthy competitive swimmers. Training volume was estimated using self-reported information. Static and dynamic measures of pain were assessed using pressure pain thresholds (PPTs) and conditioned pain modulation (CPM), the latter as a measure of endogenous pain inhibition. Selected demographic and psychosocial measures were considered as possible confounding factors. Moderate positive correlations (0.38 < r < 0.44; p < 0.01) exist between self-reported training volume and PPTs at widespread body areas in competitive swimmers. These results were maintained during linear regression analysis while addressing possible confounding factors such as age and selected psychosocial factors. No associations were found between self-reported training volume and conditioned pain modulation (−0.08 < r < 0.06; p > 0.05). Self-reported swim training volume is associated with pain sensitivity in competitive swimmers. Swimmers who train more show higher pressure pain thresholds, indicating lower pain sensitivity. The aim of this study was to determine the association between metformin therapy and pressure pain threshold (PPT) in lean women with polycystic ovary syndrome (PCOS). We hypothesized that metformin therapy in lean PCOS women increases PPT. Materials and methods: Twenty-seven lean PCOS women with free androgen index phenotype >5 and 18 lean healthy controls were enrolled in the study. Fifteen of the PCOS women were randomly assigned to be treated with metformin 1,500 mg daily for 6 months. PPT and plasma β-endorphin levels were measured in all women at the beginning of the study and after 6 months of observation. The clinical assessment of pain is subjective; therefore, variations exist between practitioners in their ability to identify and localize pain. Due to differing interpretations of the signs or severity of pain equine practitioners may assign varying levels of clinical significance and treatment options. There is a critical need to develop better tools to qualify and quantify pain in horses. Palpation is the most common method to detect local tenderness or sensitivity. To quantify this applied pressure, pressure algometry has been used to gradually apply pressure over specified landmarks until an avoidance response is noted, which is defined as the mechanical nociceptive threshold (MNT). Numerous studies have used pressure algometry in different applications to measure MNTs in horses. There is an acute need to establish normative values within different body regions and to develop standardized methods of testing MNTs to better guide practitioners in the diagnosis and treatment of pain. The aim of this systematic review was to summarize the evidence for the use of pressure algometry in horses. There is good evidence that pressure algometry is a repeatable, semi-objective method that can be used in a wide array of clinical and research applications to assess MNTs in horses.

Objectives This study aims to evaluate the presence of myofascial trigger points (TrPs), widespread pressure pain sensitivity, and mechanosensitivity of neural tissue in women with chronic pelvic pain. Design Case-control study. Setting Faculty of Health Sciences. Subjects Forty women with chronic pelvic pain between age 18 and 60 years and 40 matched healthy controls were included in the study. Methods TrPs were bilaterally explored in gluteus maximus, gluteus medius, gluteus minimus, quadratus lumborum, and adductor magnus muscles. The referred pain reproduced lumbopelvic symptoms. Pressure pain thresholds (PPTs) were also bilaterally assessed over the Pfannenstiel incision point on the abdominal, C5-C6 zygapophyseal joint, second metacarpal, and tibialis anterior muscle. Mechanosensitivity of neural tissue was assessed with the neurodynamics tests of slump and the straight-leg raising. Results Significant between-group differences were found in TrP presence in patients with chronic pelvic pain (P  < 0.001) compared with those included in the control group. Widespread pressure pain hyperalgesia was also found, with PPTs significantly reduced in the points assessed. Neurodynamics show a significantly decreased value in women with CPP. Conclusions Patients with chronic pelvic pain presented a high percentage of TrPs that reproduce their symptoms. Musculoskeletal trauma and pain can sensitize central pain mechanisms, but whether these normalize on recovery is unknown. This study compared the extent of pain referral in individuals recovered from a musculoskeletal trauma and healthy controls. Twenty pain-free participants recovered from a shoulder fracture and 20 age-/sex-matched controls participated in 2 experimental sessions (day-0 and day-1) separated by 24 hours. On both days, pressure pain thresholds were measured bilaterally at infraspinatus, supraspinatus, trapezius, and gastrocnemius muscles. Referred pain towards the shoulder region was induced by a 60-second pressure stimulation (pressure pain threshold + 20%) at the infraspinatus muscle and recorded on an electronic body chart. After day-0 assessments, delayed onset muscle soreness (DOMS) was induced to challenge the pain systems by exercising the external rotators of the recovered/dominant shoulder. The size of pressure-induced pain referral on day-0 did not differ between groups, although there was a tendency for a smaller referred pain area in recovered group. Pressure pain thresholds at the infraspinatus muscle on the DOMS side were reduced on day-1 in both groups (P = 0.03). An expansion of pressure-induced pain referral was found in both groups following the DOMS protocol on day-1 (P = 0.05) with a relatively larger expansion (P = 0.05) and higher frequency of pain in the shoulder (P = 0.04) in the recovered pain group. After complete recovery and absence of pain symptoms after a fracture, central pain mechanisms seem to normalize in the region of the trauma after recovery but when sensitized a heightened response can emerge. Abstract: Background: Pressure algometry is a commonly employed technique in the assessment of both regional and widespread musculoskeletal pain. Despite its acceptance amongst clinicians and scientists, the relationship between rate of pressure application (RoA) and pain pressure threshold (PPT) remains poorly understood. We set out to test the hypothesis that a strong, positive, linear relationship exists between the RoA and the PPT within the infraspinatus of young healthy subjects. Methods: Thirty‐three participants were randomly recruited from the local university community. PPT measures were recorded from a clinically identified myofascial trigger point within the right infraspinatus muscle during pressure algometry. A total of 2 PPT measures were recorded using each of 3 different RoAs, including low (15 N/s), medium (35 N/s), and high (55 N/s). Three baseline trials were also conducted at 30 N/s. The Pearson's correlation coefficient between RoA and PPT was calculated for each subject and averaged across participants. Results: The mean(SD) correlation between subjects was 0.77 (0.19), and the mean (SD) slope of the linear regression was 0.13 (0.09). Conclusion: Our results demonstrate that there is a strong, linear relationship between the RoA and PPT when using the pressure algometry technique. The low slope between RoA and PPT suggests clinicians can rely on PPT assessments despite small RoA fluctuations. Future research should explore this relationship further in a clinical population and in other muscles affected by chronic myofascial pain. The assessment of nociceptive thresholds is employed in animals and humans to evaluate changes in sensitivity potentially arising from tissue damage. Its application on the intact pig tail might represent a suitable method to assess changes in nociceptive thresholds arising from tail injury, such as tail docking or tail biting. The Pressure Application Measurement (PAM) device is used here for the first time on the tail of pigs to determine the reliability of the methods and to provide novel data on mechanical nociceptive thresholds (MNT) associated with four different age groups (9, 17, 24 and 32 weeks) and with proximity of the target region to the body of the animal. We recorded an overall acceptable level of intra-individual reliability, with mean values of CV ranging between 30.1 and 32.6%. Across all age groups, the first single measurement of MNT recorded at region 1 (proximal) was significantly higher ( P < 0.05) than the following two. This was not observed at tail regions 2 and 3 (more distal). Age had a significant effect ( P < 0.05) on the mean thresholds of nociception with increasing age corresponding to higher thresholds. Furthermore, a significant effect of proximity of tail region to the body was observed ( P < 0.05), with MNT being higher in the proximal tail region in pigs of 9, 17 and 24 weeks of age. There was also a significant positive correlation ( P < 0.05) between mechanical nociceptive thresholds and age/body size of the animals. To the best of our knowledge, no other investigation of tail nociceptive thresholds has been performed with the PAM device or alternative methods to obtain mechanical nociceptive thresholds in intact tails of pigs of different age/body size. The reliability of the data obtained with the PAM device support its use in the measurement of mechanical nociceptive threshold in pig tails. We aimed to study the relationship between pain perception and cytokine release during systemic inflammation. We present a randomized crossover trial in healthy volunteers (n = 17) in 37 individual trials. Systemic inflammation was induced by an i.v. bolus of Escherichia coli LPS (2 ng/kg) on two separate trial days, with or without a nicotine patch applied 10 h previously. Pain perception at baseline, and 2 and 6 h after LPS was assessed by pressure algometry and tonic heat stimulation at an increasing temperature (45–48℃) during both trials. Compared with baseline, pain pressure threshold was reduced 2 and 6 h after LPS, while heat pain perception was accentuated at all testing temperatures after 2 but not 6 h. The magnitude of changes in pain perception did not correlate to cytokine release. No effect of transdermal nicotine or training status was observed. In conclusion, LPS administration in healthy human volunteers leads to reduction in pain pressure threshold and an increase in pain perception to heat stimuli, supporting a relationship between acute systemic inflammation and pain perception.
