Ишемический инсульт – тяжелое заболевание, исходом которого часто является нарушение постуральной регуляции. Цель – анализ особенностей постуральной регуляции пациентов с гемипарезом на разных этапах раннего восстановительного периода ишемического инсульта. Материал и методы. Обследовали 14 пациентов в раннем восстановительном периоде ишемического инсульта. Проводили клиническое неврологическое обследование и пробу Ромберга на компьютерном стабилографе. Анализировали: площадь эллипса; длину статокинезиограммы; скорость перемещения и смещение общего центра давления; среднеквадратичное отклонение центра давления; 60 % спектра во фронтальной и сагиттальной плоскостях, коэффициент Ромберга. Результаты. При первичном исследовании наблюдали раскачивание испытуемых, увеличивающееся при закрывании глаз, ротацию плечевого и тазового пояса в противоположных направлениях, наклон головы и туловища вперед. Площадь статокинезиограммы, среднеквадратичное отклонение во фронтальной плоскости, частота колебаний превышали норму. Закрывание глаз сопровождалось увеличением всех перечисленных особенностей. При повторном обследовании сохранялась асимметрия позы. Смещение общего центра давления во фронтальной плоскости увеличилось, остальные параметры стабилограммы приблизились к норме. Закрывание глаз сопровождалось приближением общего центра давления к центру опорного контура во фронтальной плоскости. Заключение. В самом начале раннего восстановительного периода ишемического инсульта площадь статокинезиограммы, среднеквадратичное отклонение во фронтальной плоскости и частота колебаний пациентов увеличены. Депривация зрения сопровождается смещением центра давления во фронтальной плоскости. В дальнейшем, через 1 месяц, пациенты стоят более уверенно, но смещение центра давления во фронтальной плоскости увеличивается. Закрывание глаз сопровождается приближением положения центра давления к середине опорного контура., Ischemic stroke is a serious disease which often leads to a violation of postural regulation. Objective – to analyze the dynamics of postural regulation of patients with hemiparesis at different stages of the early recovery period of ischemic stroke. Material and methods. 14 patients in early and transitional recovery periods following ischemic stroke were examined. A clinical neurological examination and Romberg test using computerized stabilograph were conducted. We analyzed ellipse area; length of statokinesiogram; speed and displacement of common pressure center; standard deviation of pressure center; 60% of spectrum in coronal and sagittal planes; Romberg coefficient. Results. Primary research observations: patients’ bodies swayed, the swaying increased after patients closed eyes; rotation of shoulder and pelvic girdle are in opposite directions; head and torso are tilted forward. Area of the statokinesiogram, standard deviation in coronal plane and frequency of oscillations exceeded the norm. After patients closed their eyes, all of the listed features increased. Secondary research observation: asymmetry of the pose remains; displacement of the total center of pressure in the coronal plane has increased. The rest of the stabilogram parameters were close to the norm. Closing of the eyes was accompanied by approach of the general center of pressure to the center of the reference contour in the coronal plane. Conclusion. At the very beginning of the early recovery period following ischemic stroke (day 8), the area of the statokinesiogram, the RMS deviation in the coronal plane, and the frequency of oscillations of patients are increased. Visual deprivation is accompanied by a displacement of the center of pressure in the coronal plane. In the future, after 1 month, patients stood more confidently, but the displacement of pressure center in coronal plane increased. Closing of the eyes is accompanied by approaching the position of the center of pressure to the middle of the support contour.

Обследованы 13 пациентов в остром периоде ишемического инсульта. Цель – анализ особенностей постуральной регуляции пациентов в остром периоде ишемического инсульта. Методы. Проведение пробы Ромберга на компьютерном стабилографе с анализом показателей: площадь эллипса; длина статокинезиограммы в сагиттальной и фронтальной плоскостях; отношение длины статокинезиограммы к площади; скорость перемещения общего центра давления; смещение общего центра давления; среднеквадратичное отклонение центра давления; 60 % спектра во фронтальной и сагиттальной плоскостях; коэффициент Ромберга. Результаты. Площадь статокинезиограммы пациентов при стоянии с открытыми глазами превышала норму почти в 2 раза, величины среднеквадратичного отклонения центра давления во фронтальной и сагиттальной плоскости были увеличены, скорость перемещения центра давления по опорной площадке – ниже, чем у здоровых людей. Центр давления в сагиттальной плоскости смещен вперед, смещение во фронтальной плоскости не превышало норму. Закрывание глаз приводило к увеличению площади статокинезиограммы. Коэффициент Ромберга в среднем составил 309, что выше, чем при стоянии здоровых людей. Увеличивалась асимметрия позы во фронтальной плоскости, центр давления смещался вперед. Скорость миграции общего центра давления, частота колебаний в сагиттальной и фронтальной плоскостях также возросли. Заключение. У пациентов в остром периоде инсульта длина, площадь статокинезиограммы, среднее квадратичное отклонение больше, чем у здоровых. Скорость перемещения общего центра давления снижена; спектр колебаний смещен в полосу низких частот. Закрывание глаз приводит к увеличению длины, площади статокинезиограммы, более значительному, чем при стоянии здоровых. Увеличение скорости перемещения общего центра давления и частоты спектра выражено менее. Отчетливого смещения общего центра давления во фронтальной плоскости при стоянии пациентов с открытыми глазами не выявлено. Results. The area of the patients’ statokinesiogram exceeded the norm by almost 2 times, the values of the center of pressure’s standard deviation in the frontal and sagittal planes were increased, the speed of center’s of pressure movement was lower than that in healthy people.The center of pressure in sagittal plane is displaced forward, the displacement in the frontal plane did not exceed the norm. Closing eyes led to increasing the statokinesiogram’s area. Romberg's coefficient averaged 309, that is higher than in healthy people standing. The asymmetry of the posture in frontal plane increased, the center of pressure shifted forward. The speed of common center of pressure’smigration, the frequency of oscillations in the sagittal and frontal planes increased. Conclusions. The stabilogram of patients in the acute period of stroke has the following features: length, area, standard deviationsare greater than that of healthy people. The speed of general center of pressure’s movement is lower; the vibration spectrum is shifted to the low frequency band. Closing the eyes leads to increasing in the length, area of the statokinesiogram, more than when healthy personsstanding. There was no clear displacement of the general center of pressure in the frontal plane when patients stood with their eyes open. Vision deprivation leads to a shift in the center of pressure in the frontal plane.

International audience; The aim of the present study was to evaluate the effects of mental fatigue (MF) induced by a 90-min continuous demanding cognitive task on balance control. Twenty healthy young participants were recruited. They had to perform three postural tasks (on a stable support with eyes open, with eyes closed and on a wobble board) while standing on a force platform before and after watching a documentary in a control condition or carrying out a prolonged continuous demanding cognitive task (AX-continuous performance test—AX-CPT) in a MF condition. Results showed that performing the AX-CPT generated. MF since participants felt a higher subjective workload from the NASA Task Load Index after the AX-CPT than after viewing the documentary. Both the AX-CPT and the viewing of the documentary impaired balance control, mainly by affecting postural regulatory mechanisms which evolved towards a less automatic and less complex regulation mode with an increased participation of cognitive resources. MF generated by the AX-CPT affected balance control by compromising the attentional processing, while the deleterious influence of watching a documentary on postural control could stem from an adverse effect of prolonged sitting on balance control during subsequent standing.

Предмет исследования. Постуральная регуляция пациентов с травматической болезнью головного мозга. Цель – оценить влияние восстановительного лечения на динамику постуральной регуляции пациентов с травматической болезнью головного мозга. Методы. Наблюдали 82 пациентов в позднем восстановительном периоде травматической болезни головного мозга. Всем им проведены клиническое неврологическое обследование и компьютерная стабилометрия в начале и в конце курса восстановительного лечения. Результаты. Площадь статокинезиограммы, девиации во фронтальной плоскости и работа пациентов больше, чем при стоянии здоровых людей, коэффициент стабильности ниже. Закрывание глаз делает вертикальную позу менее устойчивой. Коэффициент Ромберга – 266. После курса восстановительного лечения наблюдали уменьшение площади статокинезиограммы, девиаций во фронтальной плоскости и работы. Наиболее значительные изменения произошли при стоянии с закрытыми глазами. Коэффициент Ромберга уменьшился до 141 и стал сопоставим с таковым при стоянии здоровых людей. Postural regulation of patients with traumatic brain injury. Objective – to evaluate the influence of restorative treatment on the dynamics of postural regulation of patients with traumatic brain injury. Methods. Altogether, 82 patients were observed in the late recovery period of traumatic brain injury. All patients underwent a clinical neurological examination and computer stabilometrics at the beginning and end of the course of restorative treatment. Results. The area of the statokinesiogram, deviations in the frontal plane and the work of patients is larger than when standing healthy people, the stability factor is lower. Closing the eyes makes the vertical position less stable. The Romberg Coefficient is 266. After a course of restorative treatment, a decrease in the area of the statokinesiogram, deviations in the frontal plane, and work were observed. These changes are typical for all patients, but they are more manifested in individuals with tetraplegia. The parameters of the stabilogram of patients with left-sided hemiparesis are closest to those of healthy people. As a result of a course of restorative treatment, the stability of the vertical posture increases, and the energy cost decreases when standing with open eyes, and, to a greater extent, when standing with closed eyes.

Ehlers-Danlos syndrome (EDS) is the clinical manifestation of hereditary connective tissue disorders, comprising several clinical forms. The EDS hypermobility type (EDSh) is characterized by generalized joint hypermobility and variable skin hyperextensibility, which both generate somatosensory impairment. Somatosensory system is, together with visual and vestibular systems, crucially involved in sensorimotor system functioning. The aim of this work was to understand the impact of impaired proprioception on perceptive and sensorimotor mechanisms underlying postural control in EDSh patients. Evaluation of postural control was structured around two approaches. The first one was indirect, and evaluated the sensory mechanism underlying vertical perception. The second one was direct, and used detailed stabilometric analyses to investigate postural control.The first objective of this work was to evaluate how somatosensory impairment affects the contribution of spatial frame of reference (allocentric, egocentric, and geocentric) to visual vertical perception. Two types of tests were conducted to assess the vertical perception with and without visual information (Rod and Frame Test, RFT; Subjective visual vertical, SVV). These two studies showed that somatosensory impairment reduces the contribution of egocentric frame of reference (body axis) to vertical perception. In response, patients increase their visual field dependence, and thus, use preferentially allocentric frame of reference. The second axis aimed to identify sensory strategies adopted by these patients and their repercussion on postural regulation mechanisms. To investigate this question, a thorough postural assessment was conducted, using sensory perturbation and dual-task paradigm, and linear and non-linear analyses. We observed that somatosensory impairment impacts muscular proprioceptive contribution to automatic regulation mechanism involved in postural control. These modifications in postural regulation induce an increase of active monitoring of postural sway. In response, EDSh patients develop a visual dependence, and produce adaptive strategies based on stiffening of corrective mechanisms acting in long term. Finally, two pilot studies were conducted to test the impact of proprioceptive remediation, somatosensory orthoses and sensorimotor rehabilitation program, on postural control of these patients. Both of these two therapeutic solutions seem to induce a beneficial effect on postural control. This effect is reflected by an improvement of postural stability when patients wore somatosensory orthoses, and an enhancement of postural efficiency in response to sensorimotor rehabilitation. However, results also indicate that the immediate effect induced by orthotic device of somatosensory substitution is limited, because it did not help to decrease visual dependency. Hence, these observations allowed us to identify the postural regulation specificities in EDSh patients, and, in a preliminary way, to observe how they change in response to therapeutic solutions based on sensory remediation., Le syndrome d’Ehlers-Danlos (SED) est un groupe mixte de maladies héréditaires dont la caractéristique commune est une altération d’origine génétique du tissu conjonctif. Sa forme hypermobile (SEDh) se caractérise par une hypermobilité articulaire généralisée, associée à une hyperélasticité cutanée, générant toutes deux un déficit somesthésique. Or, le système somesthésique est, avec les systèmes visuel et vestibulaire, crucialement impliqué dans le fonctionnement du système sensorimoteur. Ce travail de thèse visait donc à comprendre comment le déficit somesthésique propre au SEDh modifie les mécanismes perceptifs et sensorimoteurs sous-tendant le contrôle postural. L’appréhension de ces mécanismes s’est articulée autour de deux types d’approches : l’une indirecte, au travers de l’étude des mécanismes sensoriels sous-tendant la perception de la verticale, et l’autre directe, au travers d’analyses posturographiques approfondies.Le premier objectif de travail visait à déterminer comment le déficit somesthésique affecte l’utilisation des cadres de référence spatiale (allocentré, égocentré et géocentré) pour la perception de la verticale visuelle. Pour ce faire, nous avons conduit deux études évaluant respectivement la perception de la verticale avec ou sans indices visuels (test de la baguette et du cadre, RFT ; test de verticale visuelle subjective, VVS). Ces deux études ont montré que le déficit somesthésique diminuait la contribution du référentiel égocentré (axe corporel) à la perception de la verticale. En réponse, les patients présentent une plus grande dépendance au champ visuel et s’appuient donc préférentiellement sur le référentiel allocentré. Le second axe de travail visait à identifier les stratégies sensorielles adoptées par ces patients, et à évaluer les répercussions de celles-ci sur les mécanismes de régulation posturale. Ces questions ont été investiguées au moyen d’évaluations posturographiques approfondies, utilisant à la fois des paradigmes de perturbation sensorielle et de double tâche, ainsi que des analyses linéaires et non-linéaires. Nous avons ainsi observé que le déficit somesthésique des patients altérait la contribution de la proprioception musculaire aux mécanismes en charge de la régulation automatique de l’équilibre postural. Ces modifications dans la régulation posturale se répercutent par une augmentation du monitoring actif des oscillations posturales par le système nerveux central. En retour, les patients SEDh développent une visuodépendance, et mettent en place des stratégies adaptatives basées sur une rigidification des mécanismes correctifs à long terme. Enfin, deux études pilotes ont été conduites afin de tester l’effet de stratégies de remédiation proprioceptive, à savoir les orthèses somesthésiques et la reprogrammation sensorimotrice, sur le contrôle postural de ces patients. Chacune de ces prises en charge semble exercer un effet bénéfique sur leur contrôle postural, qui se traduit par une augmentation de la stabilité posturale lors du port des orthèses somesthésiques, et une amélioration de l’efficacité du contrôle postural suite à la reprogrammation sensorimotrice. Néanmoins, les résultats indiquent également que l’effet immédiat opéré par les dispositifs orthétiques de suppléance somesthésique est limité puisqu’il ne permet pas de diminuer la visuodépendance des patients. Cet effet est, en revanche, induit par l’action de la reprogrammation sensorimotrice, qui, par le renforcement du système proprioceptif, libère les patients SEDh de leur visuodépendance. Ainsi, ces observations nous ont permis de caractériser les spécificités de la régulation posturale chez les patients SEDh, et, de façon préliminaire, d’observer la façon dont celles-ci évoluent en réponse à des prises en charge spécifiquement orientées sur la remédiation sensorielle.

Background Stimulation of lower limbs’ cutaneous receptors and mechanoreceptors through compression garments could potentially increase somatosensory system efficiency and aid postural regulation in elderly individuals. Research question This study examined immediate effects of wearing knee length socks (KLS) of various compression levels on somatosensory function in community-dwelling healthy elderly men and women during a double-limb standing, balancing task. Methods A total of forty-six elderly participants (Male = 23), aged between 65 and 84 years old, randomly selected from the Singapore community-dwelling, healthy population. Three treatment interventions (wearing clinical compression socks; wearing non-clinical compression socks; wearing commercial socks) and one control condition (barefoot), in a counterbalanced order, were administered to participants while they performed a 30-s Romberg test, with four levels of performance difficulty: (1) standing on a stable surface with vision (SO); (2) a stable surface without vision (SC); (3) a foam surface with vision (FO); and (4), a foam surface without vision (FC). Results Results showed that immediate effects of applying KLS of various compression levels significantly reduced sway area, trace length, velocity, and anterior-posterior (AP) sway as compared to barefoot condition (control) during the FO task condition. Significance This finding indicates the positive immediate effects of garments on somatosensory system function and postural regulation in the elderly men and women, especially when standing on the unstable surface. Wearing compression KLS could be included as a viable intervention on top of other forms of balance training to reduce risk of falling in elderly people. peerReviewed

Systematic reviews of balance control have tended to only focus on the effects of single lower-limb stimulation strategies, and a current limitation is the lack of comparison between different relevant stimulation strategies. The aim of this systematic review and meta-analysis was to examine evidence of effects of different lower-limb sensory stimulation strategies on postural regulation and stability. Moderate- to high- pooled effect sizes (Unbiased (Hedges’ g) standardized mean differences (SMD) = 0.31–0.66) were observed with the addition of noise in a Stochastic Resonance Stimulation Strategy (SRSS), in three populations (i.e., healthy young adults, older adults, and individuals with lower-limb injuries), and under different task constraints (i.e., unipedal, bipedal, and eyes open). A Textured Material Stimulation Strategy (TMSS) enhanced postural control in the most challenging condition— eyes-closed on a stable surface (SMD = 0.61), and in older adults (SMD = 0.30). The Wearable Garments Stimulation Strategy (WGSS) showed no or adverse effects (SMD = -0.68– 0.05) under all task constraints and in all populations, except in individuals with lower-limb injuries (SMD = 0.20). Results of our systematic review and meta-analysis revealed that future research could consider combining two or more stimulation strategies in intervention treatments for postural regulation and balance problems, depending on individual needs. peerReviewed

The postural regulation (control) and the achievement of a sensori-tonic balance allow the individual to interact with his or her physical and social environment. The child's development typically follows a functional alternation between centripetal and centrifugal phases. The child's activity is regulated by a mechanism balancing these two orientations : autocentration and exocentration. We advance the hypothesis that autism is caracterized by a disorder of postural regulation. The present study has been carried out with four children diagnosed according to ADI and cim 10. The children were between 5 and 7 years old with a level of cognitive development between 14 and 30 months (Brunet-Lézine). Each child was seen twice : 1 /for a sensori-motor assessment : vigilance and attention, use the human milieu, processing of the sensory flows and representative control ; and 2 / for a manipulation task under the observation of an adult and then in interaction with him (blocks and sticks were used). The results show that : 1 / The postural regulation of these children shows particularities that distinguish it from the postural regulation of four controls. 2 / Each child may be characterized by a « global formula » (quality of the sensori-tonic balance and modalities of postural regulation). 3 / This formula is related to the degree of autocentration and the complexity of the constructions that the child builds alone or with the adult. 4 / The four children examined differ in their ability to use the adult's interventions for the purpose of decentration., La régulation posturale et la réalisation d'un équilibre sensori-tonique rendent possibles les interactions d'un individu avec son milieu physique et social. Le développement de l'enfant obéit à une loi d'alternance fonctionnelle (phases centripète et centrifuge). Son activité est réglée par une « balance » qui équilibre ces deux orientations : autocentration et exocentration. Nous faisons l'hypothèse que l'autisme se caractérise par un trouble de la régulation posturale. La recherche a porté sur 4 enfants autistes diagnostiqués sur la base de I'adi et de la cim 10. Ils sont âgés de 5 à 7 ans et leur niveau de développement cognitif se situe entre 14 et 30 mois ( Brunet-Lézine ) .
