To assess the total, gender-related and ageing process-related incidence rates of amyotrophic lateral sclerosis (ALS) in Beijing, China. Determine whether the decreased male to female ratio among postmenopausal age groups.Background: We used the 2-source capture-recapture method to estimate the incidence of ALS in Beijing. The primary and secondary data sources were from diagnostic hospitals and assisted care institutions in the same area from 2010 to 2015.Methods: A total of 562 cases and 283 cases were extracted from 2 data sources, and a total of 962 patients diagnosed with ALS within the 6-year period were estimated (95% CI 883–1041). The average yearly incidence was 0.77/100,000 persons (95% CI 0.71–0.83). The female to male ratio was 1.63. The incidence was associated with age and peaked in the 55–64 year age group. There was no obvious decline in the male:female ratio among postmenopausal age groups.Results: The total incidence of ALS in Beijing is similar to international reports. Ovarian hyperthecosis is the most common cause of hyperandrogenism in women during postmenopausal age. However, its diagnosis is frequently challenging, since several causes must be ruled out, involving both adrenal glands and ovaries. Herein we describe the case of a 62 years old woman addressed to our Unit after the casual detection of an adrenal mass, compatible with an adenoma. Biochemical evaluation revealed gonadotropins in menopausal range, high testosterone and androstenedione, while the patient had been complaining of androgenetic alopecia and hirsutism for some years. Ultrasound imaging revealed only a small increase in ovarian volume, in relationship to the patient’s age. A GnRHa test was performed, demonstrating gonadotropins suppression and testosterone normalization, thus confirming the suspect of ovarian hyperthecosis. The administration of these agonists, together with the slow progression of symptoms over years, play a fundamental role into excluding an androgen-secreting neoplasia, also limiting the use of ovarian veins catheterization as second line test. Besides, they represent a valid therapeutical option, especially when surgery is contraindicated (or cannot be performed).

4. Effects of ultra-low-dose versus standard hormone therapy on fibrinolysis and thrombin generation in postmenopausal women.

Objective: To assess the effects of different doses of oral hormone therapy (HT) on thrombin generation and fibrinolysis.Study Design: One hundred fifty postmenopausal women were assigned in a randomized controlled study in which the effect of standard dose (1mg 17β-estradiol/5mg dydrogesterone), ultra-low-dose HT (0.5mg 17β-estradiol/2.5mg dydrogesterone) on fibrinolysis and coagulation was compared to controls. Factors measured included plasma clot lysis time (CLT), fibrinolysis activators and inhibitors, thrombin generation (prothrombin fragments 1+2 [F1+2], endogenous thrombin potential [ETP]), normalized activated protein C sensitivity ratio (nAPCsr), and factor (F)VIII activity and were determined before and after 24 weeks of HT.Results: In ultra-low-dose group posttreatment CLT tended to be shorter when compared to standard HT (-7%) and control group (-4%) with reduced plasminogen activator inhibitor-1 (PAI-1) antigen (by -16% vs both groups). Higher mean changes from baseline between ultra-low-HT group and standard group in CLT (-7.4 vs 3.05) and PAI-1 antigen concentration (-5.1 vs 0.9) were observed. Standard HT, compared with the ultra-low-dose HT, led to higher F1+2 (+9%) and FVIII activity (+37%).Conclusions: In contrast to the standard HT, ultra-low-dose HT may enhance fibrinolysis through reduced PAI-1 levels. The study aims to determine the impact of multimodal therapeutic approach on self-perceived stress in women with high risk of Metabolic Syndrome (MetS). The study involved 43 women aged 60 years and over (mean 68.6 ± 6.5) participating in a Mental Health Promotion Program. Over the 3-month course of the project, all of the participants attended meetings of a support group (60-min sessions twice a week). During these meetings, they took part in general fitness training (20 min), dancing (20 min), as well as health-promoting education and psychoeducation sessions (20 min). Moreover, the participants were encouraged to modify their diet to reduce their daily fat and sugar intake. Stress levels were assessed using the Perception of Stress Questionnaire (PSQ). Mood was measured with the Geriatric Depression Scale (GDS-30). In all of the subjects, a body composition analysis was performed using a Tanita BC-545N analyzer. Abdomen and hip circumference were measured to determine the waist-hip ratio. Weight and height were measured to determine the BMI score. At the beginning of the project, the intensity of stress correlated with the level of depressive symptoms (GDS), Body Mass Index (BDI), and the amount of visceral fat. Three months of participation in the Mental Health Promotion Program resulted in a significant reduction in stress intensity (p <, 0.01). At the end of the project, all of the participants expressed their willingness to continue their participation in the classes, which is very important as there is a need to conduct long-term health-promoting activities in the age group in question.

Summary: Owing to the gradual modification of breast tissue in postmenopausal women, there can be differential effects on local oestrogen receptor (ER) expression, with potential impingement on the biological behaviour of cancer cells in the ageing. A series of 45 ductal carcinoma (DC) cases were selected in postmenopausal women who were not being treated with HRT. Immunohistochemical analyses were performed for hormone receptors and Ki67 expression. Fluorescence in situ hybridisation (FISH) analysis was carried out to study CCND1 amplification. The selected population was subdivided into three groups by age and was subjected to statistical studies: linear model analysis, estimation of relative incidence (RI), multivariate analysis, and nonparametric tests were performed to investigate whether there were any links between age and molecular variables in DCs. The results show a low rate of proliferation and high ER expression in the oldest age group. In the same group a close correlation was found between high ER expression and CCN in the older age group D1 amplification (P=0.000), as was a more advanced phenotype in terms of tumour size and presence of positive lymph nodes than in the other age groups considered. The results suggest that ductal breast cancer has a favourable molecular prognosis, especially in extreme old age. In particular, there is an inverse correlation between ageing and proliferation rate despite the presence of an accentuated proliferation stimulus (high ER with CCD1 amplifications) in the oldest group relative to the other groups considered. Abstract: Objective: To assess chances of adequate cyto-colposcopic follow-up after laser conization performed in postmenopausal screening population by the evaluation of postoperative cervical stenosis and unsatisfactory colposcopy rates. Patients and methods: One thousand two hundred eighteen patients were submitted to laser CO2 conization for cervical intraepithelial neoplasia (CIN). Incidence and risk factors for cervical stenosis and unsatisfactory follow-up were retrospectively evaluated comparing fertile with postmenopausal patients. Results: Global incidences of postoperative unsatisfactory colposcopy and cervical stenosis were 46.2% and 7.1%. These rates were higher in postmenopausal patients compared with fertile ones, revealing a statistical correlation between unsatisfactory follow-up and postmenopausal status at the time of conization. Association between CIN relapse and cervical stenosis was significantly higher in postmenopausal group (66.7% vs. 8.6%; P < 0.05), where two cases of recurrence were detected only after hysterectomy, because of an insurmountable cervical stenosis. Univariate and multivariate analysis revealed HRT use as the only significant factor in influencing postoperative cervical stenosis. Conclusion: Conization is still considered as the standard treatment for CIN at any woman''s age, when excisional management is indicated, but all postmenopausal patients should be counseled about the possibility of postoperative stenosis that could exclude an adequate follow-up, configuring a failure of the conservative treatment. HRT use is associated with a low risk of stenotic complications; therefore, if possible, users should be encouraged to continue therapy at least 1 year after laser conization. - Abstract
Mature cystic teratoma of the ovary (MCT) is rare in pre and postmenopausal age patients. Among various types of malignant transformation in MCT, adenocarcinoma is a rare subtype. Dual type tumors arising from ovarian MCT have been described in the literature very rarely. A 47-year-old postmenopausal female patient presented with abdominal mass for ten years. The radiological opinion was a dermoid cyst. Grossly, a 22 × 20 × 10 cm, unilocular cystic left ovarian mass with intact capsular surface and focal thickened wall measured 3.0 cm. Microscopically, it showed components of all three germ cell layers. In addition, features of colonic type adenocarcinoma and well-differentiated neuroendocrine tumor (carcinoid) were noted and confirmed by immunohistochemistry (IHC). We report this rare case of synchronous malignancy arising from an ovarian MCT with a clinicopathological review., Competing Interests: None

13. Prevencija patoloških promjena oralne sluznice u postmenopauzi uzrokovanih kserostomijom

Ispitano je 30 žena u postmenopauzi koje su patile od osjećaja suhoće u ustima s ciljem da se ustanovi korisnost preventivne terapije upalnih promjena kod kserostomije. Kod svih je kliničkim i oralnim testiranjem ustanovljena kserostomija. Kod 15 žena provedena je funkcijska i medikamentozna simp tomatska terapija kserostomije kao i prevencija komplikacija uzrokovanih nedostatkom sline. Preostalih 15 žena odgovarajuće dobi i jednakog intenziteta kserostomije kao i prethodna skupina ispitanika predstavljalo je kontrolnu skupinu. Usporedba stanja obiju skupina ispitanika ukazala ie na djelotvornost provedene preventivne terapije i potrebu za njenim provođenjem kod žena u postmenopauzi koje pate od kserostomije., In 30 postmenopausal women suffering from dryness of the mouth the efficacy of preventive therapy against inflammatory lesions due to xerostomia has been studied. In all examinees xerostomia was diagnosed by clinical and oral testing. Fifteen women were extended functional and symptomatic medicinal therapy against xerostomia and preventive therapy against complications due to the lack of secretion. The remaining 15 women of corresponding age and equal intensity of xerostomia served as the control group. A comparison of the state of health of the women in these two groups has confirmed the efficacy of preventive antiinflammatory therapy and the usefulness of such therapy in postmenopausal period suffering from xerostomia.
