This is the first report on a localized pigmented villo-nodular synovitis (PVNS or TSGCT) occurring in the trochanteric bursa. Bursal involvement in PVNS is extremely rare. Most often PVNS occurs either as a localized or diffuse lesion in a major synovial joint, such as the knee, ankle joint or hip joint. In principle, all synovial structures can be involved. The case reported here is remarkable regarding the long period between the occurrence of the first symptoms and the final diagnosis as well as the age of the female patient (75 yrs).Therapeutically a complete resection was performed in order to avoid recurrence. More then three years later the patient did well and there has been no evidence of recurrence yet.

Objective. To reveal the causes of unsatisfactory results and mistakes in treatment of patients with pigmented villo-nodular synovitis of a knee joint with the objective to improve this disease diagnosis, as well as the results of treatment and rehabilitation/ Materials and methods. Through the period 2010 - 2018 yrs we conducted a retrospective analysis of medical documents (the case histories) in 77 patients with pathohistologically verified diagnosis of pigmented villo-nodular synovitis of a knee joint, who were treated in the clinic of Institute of Traumatology and Orthopedics (ITO), Kyiv and operated on a knee joint. Results. The causes of unsatisfacory results of treatment for pigmented villo-nodular synovitis of a knee joint are the disease severity, and its diffuse form, absence of the diagnosis-treatment algorithm for this process and rehabilitation of the patients, what leads to diagnostic and tactical mistakes on the treatment process stages. Conclusion. The awaiting tactics in diagnosis and treatment of pigmented villo-nodular synovitis constitutes a wide-spread tactical mistake. Insufficient knowledge of traumatologists in a knee joint surgery leads to tactical faults as well. Most treatment mistakes occur as a consequence of unreadiness of orthopedist-traumatologist for possible intraoperative accidental revealing of pigmented villo-nodular synovitis of a knee joint, as well as because of absence of intraoperative tactics and material gears for treatment of this disease and its outcomes.

Pigmented villo-nodular synovitis (PVNS), also known as giant cell tumor of the tendon sheath or benign synovial histiocytoma, is a benign yet locally aggressive tumor of the synovium that affects both the large joints and the tendon sheaths. The most common site for the tumor includes knees, hip, ankle, and wrist joints; shoulder joint being a very unusual site for the occurrence of this tumor. We present here an unusual case of diffuse PVNS of shoulder joint with eroded humerus head treated with surgical synovectomy and adjuvant radiosynovectomy to prevent recurrence. This case report highlights the natural history of this rare disease due to late presentation, difficulty in the diagnosis, and management of recurrence.

Abstract: Pigmented villo-nodular synovitis (PVNS) is a rare, benign and aggressive disorder of synovial joints and tendon sheaths which is capable of eroding articular structures and bone. It represents a spectrum of pathological disorders ranging from the most common giant-cell tumour of tendon sheath to localized or diffuse intra-articular PVNS lesions and may be associated with joint erosions. It is best treated surgically but recurs if inadequately excised. Adjunctive therapies include injection of intra-articular radio-isotopic materials and external beam radiotherapy. Pigmented villo-nodular synovitis (PVNS) is a common disease entity particularly in the knee joint but its incidence in the foot is quite rare. A case of PVNS of the second toe that presented at the outpatient department with an insidious onset and slowly progressive painful swelling of the toe associated with stiffness is presented. The mass was subsequently excised and the diagnosis confirmed by histo-pathology samples. The patient’s symptoms improved significantly after the operation. This case is one of the very few reported cases of PVNS involvement of flexor tendon sheath of the foot. It also signifies the importance of clinical suspicions based on the symptoms and examination in reaching to the presumptive diagnosis of the foot pathology and its confirmation by histo-pathological evaluation.
