- Abstract
Prostate cancer (PCa) is the most common cancer among men in the Netherlands. Robot Assisted Radical Prostatectomy (RARP) for localized/locally advanced PCa is one of treatment options. However, choosing the right treatment can be challenging for patients. Decision aids, based on standardized clinical study information, are available. These aids are crucial due to the significant impact of treatment side effects on patients’ quality of life (QoL). RARP can lead to short-term complications like vesico-urethral anastomosis leakage and long-term issues such as urinary incontinence (UI) and erectile dysfunction (ED). Patient-reported outcome measures (PROMs) are essential in shared decision-making (SDM) and post-treatment follow-up. PROMs, utilized in clinical studies and patient communication, help manage RARP side effects and identify other potential issues. This thesis investigates PROMs' clinical roles, develops a predictive tool for UI post-RARP based on PROM responses, evaluates the effect of individualized continence predictions on treatment decisions, explores non-surgical factors in erectile function recovery, and assesses patients' sexual satisfaction post-RARP. Section I: PROMs in Clinical Practice Chapter 1 examines the response rates to digital and paper-based PROMs in localized prostate cancer patients undergoing RARP. Digital questionnaires were complemented by paper-based versions for non-responders. Higher response rates were observed for paper-based reminders, especially among older patients, indicating age as a significant predictor of response rates. Chapter 2 compares QoL recovery patterns post-RARP using interview and questionnaire-based assessments. Interviews tended to overestimate functional recovery compared to questionnaires, with continence scores strongly correlating with physical and overall QoL. Quality of relationship-building in low-threshold addiction and mental health care The goal of this article is to report about a quest to find a measure for evaluation of the quality of relationship-building in health care. First the importance of quality of relationship building in low threshold care is discussed. We search concepts and questionnaires that have value in this context. From this overview a questionnaire is chosen that covers aspects of the quality of relationshipbuilding best. This instrument was adapted to the care context and we collected data among 80 visitors of low-threshold institutions in four different cities. We evaluate to what extent the instrument meets our requests and explore further possibilities for application. Allereerst wordt beargumenteerd dat de kwaliteit van de relatie een belangrijk onderdeel is van de laagdrempelige verslavingszorg. Vervolgens wordt een literatuuroverzicht gepresenteerd waarmee een aantal aspecten van de kwaliteit van de relatie wordt blootgelegd. Omdat kwaliteit van de relatie van belang blijkt binnen patiëntgerichte zorg, worden verschillende instrumenten die patiëntgerichtheid meten vergeleken. De auteurs selecteerden op basis van deze vergelijking de vragenlijst die de meeste aspecten van de kwaliteit van de relatie meet, en bewerkten deze. Daarna is dit instrument getoetst in een onderzoek onder 80 bezoekers van laagdrempelige verslavingscentra in vier verschillende steden. De auteurs concluderen dat de bewerkte vragenlijst onderscheidend vermogen bezit voor verschillende werkers en instellingen. De lijst is behulpzaam bij het meten van de kwaliteit van de relatie tussen cliënt en hulpverlener.

Reducing unnecessary follow-up visits after low-risk basal cell carcinoma. From identification, to exploration by means of quantitative research. After quantification of these results, a personalized letter for patients was determined the best strategy to reduce the unnecessary care. This was tested in practice and resulted in a decrease in the number of control visits.

To get insight into personal meaning of a person involved in a physical therapy intervention.Mrs. A, a 76-year-old woman is referred to a physical therapist (PT) for assessment of functioning and training before total hip arthroplasty (THA). The patient, her daughter, and PT were asked to write a story about their daily life. Stories were analyzed according to the narrative scheme based on a method to find meaning in daily life, which consists of four phases: 1. Motivation; 2. Competences; 3. Performance; and 4.Mrs. A was mainly motivated by her will to do enjoyable social activities and stay independent. Although she tried her best to undertake activities (performance) that made her proud (evaluation), her pain and physical limitations were anti-competences that motivated her to attend healthcare. Although the PT seemed to be aware of personal participation goals, her main motivation was to improve and evaluate functions and activities. The daughter was motivated by good relationships and did not see herself as informal caregiver.The narrative method was a valuable tool to clarify motivations, competences, and values in the process of creating personal meaning related to functioning. This knowledge could help caregivers in applying patient-centered goal-setting and treatment on a participation level. Implications for rehabilitation Personal meaning of people's functioning within their daily context can be clarified from daily life stories. This case report demonstrates that motivations and goals may differ between patient and therapist; the PT seems to focus on improving and evaluating functions and activities, while the patient seems to focus her motivations and personal meaning on participation. This approach may help in patient-centered goal-setting at the level of activities and participation.
