Resumen: Introducción: La tormenta tiroidea se caracteriza por síntomas exagerados de hipertiroidismo, pudiendo ocasionar falla multiorgánica y paro cardiorrespiratorio. Es un fenómeno poco frecuente, pero con elevada mortalidad. Caso clínico: hombre de 35 años, sin antecedentes, con cuadro de 2 semanas de palpitaciones y epigastralgia, que en 24 horas presenta exacerbación importante, agregándose náuseas y diaforesis. En el traslado a urgencias presenta paro cardiorrespiratorio por asistolia, cursando posteriormente con falla multiorgánica durante su hospitalización. En base a antecedentes otorgados por la familia de una clínica hipertiroidea reciente, se plantea posible diagnóstico de tormenta tiroidea. Se ordenan pruebas tiroideas, resultando en T4 libre mayor a 7,7 ng/dl y TSH menor a 0,005 μUI/ml. Se maneja con tiamazol, hidrocortisona y propranolol, evolucionando favorablemente. Discusión y Conclusión: Es necesario un diagnóstico oportuno, pues un manejo y detección precoz disminuye complicaciones y mortalidad. Una vez sospechada, deben iniciarse medidas de reanimación y tratamiento farmacológico que disminuya los efectos de hormonas tiroideas. Abstract: Introduction: A thyroid storm is a rare but highly lethal disease. It is characterized by exaggerated symptoms of hyperthyroidism, and it can lead to multiorgan failure and cardiopulmonary arrest. Case Report: A 35-year-old man with no prior medical history presented with a two-week history of palpitations and epigastric pain, which abruptly worsened over 24 hours, accompanied by nausea and diaphoresis. During transportation to the emergency department, he experienced cardiopulmonary arrest and multiorgan failure during his hospitalization. Family reported recent hyperthyroid symptoms led to the consideration of a thyroid storm diagnosis. Thyroid function tests revealed a free T4 level greater than 7.7 ng/dl and a TSH level less than 0.005 μIU/ml. The patient was treated with thiamazole, hydrocortisone, and propranolol, showing favorable progression. Discussion and Conclusion: Opportune diagnosis of a thyroid storm is crucial, as early management and detection reduce complications and mortality. Once suspected, resuscitation measures and pharmacological treatment should be initiated to mitigate the effects of thyroid hormones.

Introduction: Every year, five million people around the world experience an out-of-hospital cardiac arrest (OHCA) and less than 40 % receive any assistance before the arrival of the Emergency Medical Services (EMS). Ambulance operators (AO) take care of people experiencing an OHCA, stabilize and then transfer them. In Medellín, Colombia, there is a public AO and several private providers, but the information about an OHCA and the operational characteristics during the response to the event are limited. Objective: To estimate the incidence of OHCA and to explore the factors associated with survival after the event in Medellín city. Methods: Retrospective, population-based cohort study. All the medical records of patients experiencing an OHCA who were assessed and treated by ambulance operators, (AO) of the Emergency Medical Services (EMS) and private agencies in Medellin city were reviewed. Descriptive statistics were used for data analysis and the annual incidence of the event standardized for the general population was estimated. Potential survival-associated factors reported as OR with their corresponding 95% CI were explored. Results: A total of 1,447 patient records with OHCA between 2018 and 2019 were analyzed. The event incidence rate for the number of cases assessed was 28.1 (95 % CI 26.0-30.3) and 26.9 (95 % CI 24.9- 29.1) cases per 100,000 inhabitants/year for 2018 and 2019, respectively; the incidence rate of treated OHCA was 2.6 (95 % CI 2.0-3.3) and 3.2 (95 % CI 2.5-4.0) per 100,000 inhabitants/year, for 2018 and 2019. Survival on arrival at hospitals of treated cases was 14.2 % (95 % CI 5.5-22.8) and 15.5 % (95 % CI 7.4-23.5) for 2018 and 2019, respectively. Conclusions: This study portrays the operating and care characteristics of the population experiencing OHCA in Medellín city. La escuela es el lugar óptimo para iniciar la formación en Soporte Vital Básico y los docentes pueden tener un papel muy relevante para este propósito. Sin embargo, no se conoce los conocimientos y la disposición de este colectivo para impartir esta materia a sus alumnos. Tampoco se ha demostrado la actitud de las familias a que sus hijos o tutelados reciban estos conocimientos dentro del currículo escolar. El objetivo de nuestro estudio fue evaluar de forma cuantitativa la sensibilización, actitud y disposición de la inclusión de esta materia en el currículo escolar de tres figuras clave en el entorno educativo: docentes, familias y alumnos de los grados en ciencias de la educación. Métodos: Se realizó un estudio descriptivo transversal mediante un cuestionario ad hoc autoadministrable. Resultados: Participaron en el estudio 484 familias, 137 docentes y 42 alumnos de los grados en ciencias de la educación. Los tres grupos coincidieron en que el aprendizaje del soporte vital básico debería formar parte del currículo escolar, con carácter obligatorio y considerando la educación primaria como la etapa más adecuada para su inicio. Respecto a quién debe impartir esta materia, destaca que únicamente el 30,7% (n=42) de los docentes considera que esta tarea debe ser realizada por los propios docentes de los centros escolares. Tanto docentes en activo como alumnos de grado de ciencias de la educación afirman como estrategia eficiente para difundir estos conocimientos entre los docentes el incluir estos conocimientos en el plan de estudios de los Grados de Educación.Conclusiones: En base a los resultados se concluye la necesidad de sensibilizar y formar de manera prioritaria a los docentes con objetivo de aumentar además de sus conocimientos en la materia, su autoconfianza, hecho imprescindible para incrementar su disposición para poder instruir a sus alumnos. School is the optimal place to begin training in Basic Life Support and teachers can play a very relevant role for this purpose. However, the knowledge and willingness of this group to teach this subject to their students is not known. Nor has the attitude of families towards their children or guardians receiving this knowledge within the school curriculum been demonstrated. The objective of our study was to quantitatively evaluate the awareness, attitude and willingness to include this subject in the school curriculum of three key figures in the educational environment: teachers, families and students of degrees in educational sciences. Methods: A cross-sectional descriptive study was carried out using a self-administered ad hoc questionnaire. Results: 484 families, 137 teachers and 42 students of degrees in educational sciences participated in the study. The three groups agreed that learning basic life support should be part of the school curriculum, on a mandatory basis and considering primary education as the most appropriate stage for its initiation. Regarding who should teach this subject, it stands out that only 30.7% (n=42) of teachers consider that this task should be carried out by the school teachers themselves. Both active teachers and undergraduate students of educational sciences affirm that including this knowledge in the curriculum of the Degrees of Education is an efficient strategy to disseminate this knowledge among teachers. Conclusions: Based on the results, the need is concluded. to raise awareness and train teachers as a priority with the aim of increasing, in addition to their knowledge on the subject, their self-confidence, an essential fact to increase their willingness to be able to instruct their students. Descriptive, quantitative exploratory study, carried out in a philanthropic pediatric hospital, with the objective of understanding the nursing professional competencies in the care of cardiorespiratory arrests in pediatrics, besides investigating. 30 professionals from the nursing team participated and data was collected using a closed questionnaire. The results, analyzed using descriptive statistics, showed that the majority of nursing professionals had knowledge about the care of the investigated emergency. In relation to their perceptions about the necessary technical skills, the majority of participants perceived themselves to be confident in resuscitation maneuvers and the administration of medications during care. However, when approaching the handling of the defibrillator and performing a venipuncture, most of the sample stated that they did not feel complete confidence for these activities. The emergency manuals based on cognitive aids are an essential component within the operating room for timely crisis management, with the advantage of reducing errors due to omission. The purpose of this writing was to resume the current world experience with them and compare it with the ABC Medical Center. The data obtained from the survey were converted into percentages and used to summarize the categorical variables. The results showed that most doctors were aware of this tool in the operating room and more than a half actually used it in critical situations. The most described situations were: bronchospasm, anaphylactic shock and suspicion of malignant hyperthermia. [ABSTRACT FROM AUTHOR]

Background: Children with congenital heart disease present a higher frequency of cardiorespiratory arrest (CRA) than the general pediatric population. The epidemiology of CRA is not exactly known in our setting, nor are the mortality risk or the neurological evolution factors. Objective: To describe the epidemiology and outcomes associated with pediatric cardiopulmonary resuscitation in a cardiovascular recovery unit. The primary endpoint was the survival to discharge and the secondary endpoints were the return to spontaneous circulation, the survival at 24 hours and the remote neurological condition. Methods: Descriptive, prospective, longitudinal cohort study in children under 18 years of age who required cardiopulmonary resuscitation between 2016 and 2019. Demographic variables, characteristics of cardiopulmonary arrest, resuscitation and outcome were analyzed. An uni- and multivariate analysis was performed comparing survivors and deceased. Results: Out of 1,842 hospitalized patients, 4.1% presented CRA. Fifty patients with complete records were analyzed. Seventy-eight percent (39) returned to spontaneous circulation with a high survival rate of 46%. Resuscitation > 6 min and the use of vasoactive drugs were predictors of mortality; 16/23 patients were followed up, 10 of them with normal development for age at 6 months, six had pervasive developmental disorder. Conclusions: 4.1% of patients presented CRA, with a rate of 3.4 CRA per 1,000 patient-days. Survival at hospital discharge (n = 50) was 46%. Resuscitation > 6 min and the use of vasoactive drugs were independent predictors of mortality. At six months, 63% had normal neurological development for age. [ABSTRACT FROM AUTHOR]

Background: the correct application of life support for cardiorespiratory arrest (CPR) creates a short gap between the life or death of a person, hence the importance of having the necessary preparation to perform it. Objective: to explore the training and criteria of professionals about the management of basic and advanced life support in the event of cardiorespiratory arrest, at the Canoa Health Center, Ecuador, between July 2022 and February 2023. Methods: a quantitative, descriptive, cross-sectional study was carried out in the health center and during the time period declared. The study population consisted of 15 professionals, and included medical, nursing, obstetric and dental personnel. The information was collected through an anonymous questionnaire. The results were analyzed according to descriptive statistics. Results: almost half of the professionals were doctors (46.7 %), followed by nursing graduates (33.3 %). 66.66 % have taken basic or advanced life support courses, 86.7 % have witnessed CPR, but only 40 % have participated in resuscitation. All respondents replied that they do not have the necessary equipment to act in case of a CPR. For the most part, 66.66 % reflected a high degree of knowledge compared to knowing how to act in CPR cases, declaring confidence in acting. Conclusions: the human talent of the operating unit meets high levels of knowledge regarding basic and advanced life support.

Los sobrevivientes de la reanimación cardiopulmonar posterior a un paro cardiaco pueden tener un amplio rango de desenlaces y van desde recuperación neurológica completa, estado de vigilia sin respuesta, compromiso cognoscitivo diverso o la muerte. La lesión del tejido cerebral se presenta inmediatamente después del paro cardíaco, durante la reanimación y al retornar la circulación espontánea. La severidad y duración de la noxa isquémica determinarán el devenir neurológico. El examen clínico es el punto de partida en el abordaje multimodal del neuropronóstico. Se debe complementar con electroencefalograma, potenciales evocados somatosensoriales, neuroimágenes y biomar-cadores séricos. Entre un 10 a 15% de los pacientes con lesión cerebral posterior al paro cardiaco evolucionan hacia muerte por criterios neurológicos y son potenciales candidatos a la donación de órganos. Un retiro temprano de las terapias de sostenimiento de vida puede malograr la posibilidad de un potencial donante de órganos. Se puede estimar de manera temprana qué pacientes tienen mayor riesgo de evolucionar a muerte por criterios neurológicos. El neurólogo tiene un papel protagónico en el manejo de pacientes con lesión cerebral post paro cardiaco y sus decisiones tienen implicaciones éticas y legales.

15. Compresiones torácicas mecánicas versus manuales en adultos en parada cardiorrespiratoria extrahospitalaria durante su traslado en ambulancia: una revisión sistemática.

Introducción y objetivo: La parada cardiorrespiratoria extrahospitalaria (PCREH) es una de las principales causas de muerte en los países desarrollados y un problema importante de salud pública. El objetivo de esta revisión sistemática ha sido determinar si la reanimación cardiopulmonar (RCP) mecánica es más eficaz que la RCP manual en adultos en PCREH durante su traslado en ambulancia. Metodología: Revisión sistemática en base a las disposiciones PRISMA. Se elaboró un protocolo de búsqueda que se empleó en tres bases de datos (Medline, ScienceDirect y Web of Science). Se incluyeron ensayos clínicos y estudios observacionales publicados entre enero del 2008 y diciembre del 2020. Resultados: Se seleccionaron un total de 9 estudios, 5 llevados a cabo en pacientes, 3 realizados en maniquíes y 1 realizado en animales. La RCP mecánica no aumenta la supervivencia en adultos a los 30 días y disminuye la situación neurológica favorable al año. En cambio, esta última aumenta a los 6 meses, presenta mejor calidad de compresión, es independiente de los movimientos del vehículo y aporta mayor seguridad durante el traslado. Conclusiones: No existe evidencia suficiente para apoyar el uso rutinario de los dispositivos mecánicos puesto que no mejoran la supervivencia de los adultos en PCREH en comparación con la RCP manual durante su traslado en ambulancia. Por último, se destaca la necesidad de investigación acerca de esta cuestión sanitaria.

Introduction: clinical ultrasonography has undergone a significant development in recent years. Its use has spread to most medical specialties and has ceased to be a complementary examination and has been integrated into the clinical approach. Objective: to describe the usefulness of ultrasonography in cerebral- cardiopulmonary resuscitation. Method: a search for information was carried out in the databases PubMed/Medline, Scopus and SciELO, using a combination of terms. Twenty-nine references were chosen. Development: during cerebral-cardiopulmonary arrest, ultrasonography allows differentiation of cardiac rhythm, finding reversible causes of arrest, performing ultrasound-guided procedures during resuscitation, displaying real-time physiological data reflecting dynamic changes in response to medical treatments, and providing prognostic information. The identification of causes impacts on the management and prognosis, and is also applicable to the evaluation of the quality of chest compressions. There are different protocols, each with its own features. Ultrasound has a positive influence on the return of spontaneous circulation, survival to admission and survival to hospital discharge. Conclusions: the use of ultrasound during cerebral cardiopulmonary resuscitation is based on the diagnosis of these acute cardiorespiratory events, the identification of reversible causes, evaluation of the technique, selection of therapy, guiding cardiorespiratory resuscitation, avoiding a new event and assessing the functional status after resuscitation. The mastery of clinical echocardiography by all health care providers in the event of cardiopulmonary arrest is a necessity.

- Abstract
The COVID-19 pandemic is having a large impact on the general population, but it has taken a specially high toll on healthcare personnel. Resuscitation efforts require potential modifications of the present Cardiopulmonary Resuscitation (CPR) international guidelines because of the transmissibility rate of the new SARS-CoV 2 virus. It has been seen that up to 15% of COVID-19 patients have a severe disease, 5% have a critical form of infection and the mean death rate is 3%, although there are significant differences according to the country that reports it and patients' baseline conditions that include age, presence of arterial hypertension, cardiovascular disease, diabetes or obesity. In these high risk subjects, mortality might go up to 24%. There are also reports of a recent increase in out-of-hospital cardiopulmonary arrest (OHCA) victims. Cardiac arrest (CA) in these subjects might be related to many causes, but apparently, that phenomenon is related to respiratory diseases rather than cardiac issues. In this context, the decision to start or continue CPR maneuvers has to be carefully assessed, because of the low survival rate reported so far and the high contagion risk among healthcare personnel. Patients with critical illness develop physiological dysfunction which provokes multi-organ system failure and triggers fatal outcomes and irreversible injuries or even death. Inside Internal Medicine services of Mexican hospitals, especially in public hospitals, there is a considerable number of critically ill patients with multiple pathologies who require surveillance to reach clinical goals of their treatment. Introduction: the course of basic brain-cardiopulmonary resuscitation is included in the curricular design of the undergraduate degree in Medicine, confirming the need to strengthen the learning process in students through the establishment of didactic materials. Objective: to create a complementary leaflet for the study of a basic course for brain-cardiopulmonary resuscitation in Medicine Studies. Method: a pedagogical research was carried out for the basic brain-cardiopulmonary resuscitation during 2018-2019 academic year at Pedro Borrás Astorga University Polyclinic in Pinar del Río city, Cuba, mainly applying dialectical materialistic method; which allowed the use of theoretical and empirical methods. Development: a complementary leaflet was created as a teaching tool for medical students in the first-academic year who are taught the basic course of brain-cardiopulmonary resuscitation; in consequence it guarantees a scientific and professional training, aimed at developing knowledge and skills that are easy to be understood. Conclusions: a complementary leaflet was available for the basic course of brain-cardiopulmonary resuscitation, as well as a valuable educational tool for medicine-students at first-academic year in order to train them professionally into the province or outside it.

Introduction: a system of questions for a particular subject must be able to constantly investigate the relevance of work, promote the acquisition of skills and strengthen a learning capacity. Objective: to develop a system of questions for the study of the basic cardiopulmocerebral resuscitation course itself. Method: a pedagogical investigation was carried out related to the study program of the basic cardiopulmocerebral resuscitation course during the 2018-2019 academic period at the “Pedro Borrás Astorga” University Polyclinic in the city of Pinar del Río, governed by the dialectical materialist method and the use of theoretical and empirical methods. The research was approved by the scientific council and institutional ethics committee. Development: a question system was developed as a tool of the teaching-learning process for students in the first year of the Medicine career who received their own basic cardiopulmocerebral resuscitation course; thereby guaranteeing comprehensive scientific training, with acquisition of knowledge and skills that are easy to understand. Conclusions: a question system was developed for the study of the basic cardiopulmocerebral resuscitation course that represents a valuable educational instrument for first year students of the Medicine career.

27. Factores de riesgo asociados a mortalidad por paro cardiaco intraoperatorio en el Hospital Nacional Guillermo Almenara Irigoyen, 2011-2015

Introducción. La incidencia y mortalidad de paro cardiaco intraoperatorio (PCI) son indicadores de la calidad de atención y eficiencia en centros quirúrgicos. Objetivo. Determinar los factores de riesgo asociados a mortalidad por PCI en el Hospital Nacional Guillermo Almenara Irigoyen, Lima, Perú, durante los años 2011 a 2015. Métodos. Se realizó un estudio observacional, descriptivo y retrospectivo de PCI ocurridos en el periodo de estudio, donde se determinó el riesgo relativo. Resultados. Entre los años 2011 a 2015 ocurrieron 88 PCI de un total de 74 096 cirugías realizadas; la incidencia de PCI fue de 11,87 x 10 000 cirugías. De los 88 PCI, 39 pacientes fallecieron (44,3%); la mortalidad intraoperatoria fue 5,26 x 10 000 cirugías. Los factores de riesgo asociados a mortalidad por PCI fueron: RCP mayor de 20 minutos (RR: 3,9), clasificación ASA (RR: 3,4), cirugía cardiovascular (RR: 2,9), causas quirúrgicas (RR:2,9), etapa de mantenimiento anestésico (RR:2,0). Conclusiones. En el periodo de estudio la incidencia de PCI fue de 11,87 x 10 000 cirugías; los factores de riesgo asociados a mortalidad por PCI fueron la RCP mayor de 20 minutos, alto riesgo ASA, cirugía por enfermedad cardiovascular, eventos adversos relacionados con cirugías y mantenimiento anestésico.

Paro cardiaco en salas de recuperacion posanestesica, entidad poco frecuente en esterilizacion tubaria. Factores descencadenantes como sobresedación, bloqueo espinal alto,uso de opiodes, toxicidad sistemica por anestesicos locales. La ligadura de trompas como metodo anticonceptivo definitivo que se realiza de preferencia bajo anestesia raquidea. Paciente de 35 años en su puerperio de 12 horas, se somete a salpingoclasia bilateral bajo anestesia raquidea, con dosis de fentanil de 20 mcg y bupivacaina pesada de 12 mg.Nivel de dermatoma alcanzado de T6 (ideal para el procedimiento quirurgico), sin ninguna otra administracion medicamentosa. En la unidad de recuperacion postanestesica presenta paro cardiaco con inicio de manera inmediata de soporte vital avanzado con buenos resultados posteriores. Se le pracrican los examenes complementarios e interconsultas correspondiente. Paciente es dada de alta hospitalaria sin secuelas neurologicas a los 4 dias posteriores al evento. Prevenir y tratar la causa correspondiente del paro cardiaco.

The quality of Cardiopulmonary resuscitation (CPR) depends on the systematization of actions, which are apply on cardiac arrest (PCR); such as the Basic Life Support (BLS). Therefore, the development of training in this field, aiming the spread of standardization of BLS, is especially important to achieve quality in CPR. This study aimed to describe the experience of Master's students of the Graduate Program in Nursing and Health of a Public University of the State of Bahia to develop a BLS workshop, which was administrated to health students. The workshop proposal on BLS was to promote a theoretical-practical approach based on reality. In this sense, a dialogued lecture was held, using audiovisual resources, containing the following theoretical contents: Epidemiology of diseases of the circulatory system; Symptomatology of an individual in PCR; Steps for recognizing a PCR situation; Evaluation sequence and conduct at each moment; Use of External Automated Defibrillator (AED) and Heimlich Maneuver in adults. It is of fundamental importance that the whole society has knowledge about the Basic Life Support protocols, since anyone can deal with an urgent or emergency situations, thus previous experience can be effective to help the victim. Introduction: Community health workers are considered a professional health category in Brazil who are in direct daily contact with community users and, therefore, more likely to face emergency or first aid situations. Objective: To assess the level of knowledge and confidence of community health workers on first aid and cardiopulmonary resuscitation. Materials and Methods: A descriptive quantitative study was conducted between June and October 2018 in 22 Family Health Centers in a city in Ceará, Brazil with 186 community agents grouped as follows: a group that has already received training on these matters and another group that has not had such training. For data collection, a questionnaire containing the following items was administered: 1) sample characterization; 2) questions on cardiopulmonary resuscitation; 4) questions on first aid; 4) confidence scale. Results: A significant difference among both groups was found in questions related to the knowledge of the four items, especially for explaining CPR, checking for responsiveness of an unconscious person, the sequence for placing hands and arms in cardiopulmonary resuscitation (CPR) and rotating resuscitators. Regarding first aid, a statistically significant difference was also found in four items related to first aid questions when attending a victim in an emergency such as convulsions, drowning and choking in children. Both groups showed low confidence to intervene in cardiac, neurological and respiratory emergencies. Conclusions: Research results indicate that there is a lack of knowledge among community health workers to respond effectively to situations involving cardiopulmonary resuscitation and first aid, which is also associated with low confidence to perform interventions in emergencies. Objective: To evaluate the effect of an educational intervention, in the context of high school, on the theme cardiorespiratory arrest in adults. Methods: Quasi-experimental study, before and after, with quantitative approach developed between February and November 2019 in a public school in the city of Fortaleza, Ceará. Results: Initially, 30 students were recruited, but 10 met the inclusion criteria, which was the final sample. To carry out the study, a structured questionnaire was used, authored by the researchers, composed of 10 objective questions. After the educational intervention and data analysis, an average of 7,225 correct answers were obtained in the post-test, proving through the numbers the efficiency of health education performed, since initially the average presented was 4,375. Conclusion: It was found that the students had knowledge related to insufficient basic life support, but with great learning capacity, and the school was a favorable environment for educational practices on the subject. To map the scientific evidence on the skills developed in undergraduate nursing students using clinical simulation in teaching cardiac arrest assistance. Scoping review following the recommendations of The Joanna Briggs Institute, in the MEDLINE, CINAHL, SCOPUS, Web of Science, LILACS and EMBASE databases. 13 studies published between 2015 and 2020 were selected, with low levels of evidence. The evidence pointed to clinical simulation as an effective strategy in the development of skills related to cardiopulmonary arrest, including: high-quality cardiopulmonary resuscitation, development of students, self-confidence and critical thinking, improves teamwork and effective communication. The mapped evidence demonstrated a consensus regarding the effectiveness of clinical simulation on the development of motor, attitudinal and cognitive skills aimed at teaching cardiorespiratory arrest in undergraduate nursing. The objective of the present study was to evaluate the knowledge of nursing students in situations of cardiorespiratory arrest (CRA), according to the Brazilian literature. For this, an integrative descriptive and exploratory review was carried out at the Virtual Health Library. Articles in Portuguese, available in full, published between 2012 and 2017 were included in the research. In total, ten articles made up the sample. Thus, it was possible to identify nursing students who have heterogeneous levels of knowledge about CRA, since they present important changes in the percentage of correct and incorrect answers in the discussion points of the analyzed articles. In addition, feelings of inability to act appear as an aspect highlighted by some students in these studies. These reports may point to the need for more effective training for these professionals, in view of the importance of nursing performance in the face of patients in CRA. [ABSTRACT FROM AUTHOR]

Many cases of cardiopulmonary arrest can be reversed with immediate and accurate cardiopulmonary resuscitation (CPR). Successful cardiopulmonary resuscitation is an important criterion for evaluating Emergency Medical Services (EMS) personnel's performance. The present study aims to use EMS personnel's experiences to determine the barriers that affect the quality of cardiopulmonary resuscitation. The present study is a qualitative work with a conventional content analysis approach. The subjects were 20 EMS personnel who were selected through purposeful sampling. Data were collected via semi-structured individual, in-depth interviews with each of the 20 subjects on a face-to-face basis. The data were analyzed using Graneheim and Lundman's approach. The collected data analysis yielded 3 main themes: personal and occupational factors, organizational factors (ineffective management), and socio-cultural factors. For high-quality cardiopulmonary resuscitation, there are cultural, professional, and organizational requirements that must be met. Prehospital emergency administrators can use the present study's findings to plan and take measures toward eliminating barriers and enhancing the effectiveness and quality of cardiopulmonary resuscitation. Cell suffer structural and metabolic changes in stress situations,which allow them to maintain an adequate homeostasis and avoid death . This bibliographic review had the objective of describing the main morph-physiological changes which occur in cardiac failure and cardiac-pulmonary-cerebral resuscitation. The method was documentary reviewing (database Regional SciELO, PubMed, Cochrane and Infomed), developed during the first semester of 2018. Twenty eight references were selected. It was concluded that there are cellular changes during circulatory stop, the procedures of resuscitation and re-perfusion. In cardiac failure, cellular changes are expressed in all the organelles. And may cause death due to necroses. During re-perfusion new structural changes occur, for calcium entrance, sodium disturbances, production of free radicals and swelling. Morph.physiological changes depend on previous metabolic condition, time of cardiac failure and the successful establishment of resuscitation measures.

Foundation: assisting emergencies and cardio respiratory cerebral failure is currently a priority; when it is performed by non- professionals from the very first minutes present at the moment at the place of event, results are favorable for the patient and the society.Objective: to describe the results of a training proposal for skill development in assisting emergencies and cardio-respiratory failure, for eyewitness non-health professionals.Methods: it is a quasi-experimental study with previous and after design, developed in the Holguin City, between January and July 2018. It was designed a training proposal of 5 hours face to face course (theoretical and practical course) for teaching resuscitation. It was worked with 108 persons, to whom a diagnostic test was applied before and after the educative intervention.Results: the group was composed mainly by teenagers and adults with high school level, who did not know about the topic since they had never been trained nor had information about the topic. The intervention improved theoretical and practical knowledge about resuscitation, skills for the diagnosis and the conduct before main cardio-respiratory disturbances, which require life support.Conclusion: the training proposal for developing skills for assisting emergencies and cardio-respiratory failure was accepted by the participating people in the course. It favored the acquisition of abilities and knowledge about resuscitation.

Fundamento: La atención a las emergencias y la parada cardiorrespiratoria cerebral constituye una prioridad en los momentos actuales; cuando esta se realiza desde los primeros minutos, por personas no profesionales, presentes en el lugar del suceso, los resultados son favorables para el paciente y la sociedad.Objetivo: describir los resultados de una propuesta de entrenamiento para la formación de habilidades en la atención a las emergencias y la parada cardiorrespiratoria, para testigos presenciales no profesionales de la salud.Métodos: estudio cuasiexperimental, con diseño de antes y después, realizado en la ciudad de Holguín, entre enero y julio del 2018. Fue diseñada una propuesta de capacitación de cinco horas presenciales (curso teórico –práctico) para la enseñanza de la reanimación. Se trabajó con 108 personas, a las cuales se aplicó un diagnóstico antes y después de la intervención educativa.Resultados: el grupo estuvo compuesto principalmente por adolescentes y adultos de nivel medio superior, que desconocían del tema pues no habían recibido capacitación ni información al respecto. La intervención mejoró los conocimientos teóricos y prácticos sobre reanimación, las habilidades para el diagnóstico y la conducta ante las principales alteraciones cardiorrespiratorias que precisan apoyo vital básico.Conclusión: la propuesta de capacitación para la formación de habilidades en la atención a las emergencias y la parada cardiorrespiratoria fue aceptada por los participantes en el curso. El entrenamiento favoreció la adquisición de habilidades y conocimientos sobre resucitación.

Las células realizan transformaciones estructurales y metabólicas ante situaciones de estrés, lo que les permite mantener una adecuada homeostasis y evitar la muerte. La presente revisión bibliográfica tuvo como objetivo describir los principales cambios morfofisiológicos celulares que acontecen en la parada cardiaca y reanimación cardiopulmocerebral. El método incluyó una revisión documental (bases de datos SciELO Regional, PubMed, Cochrane e Infomed), realizada durante el primer semestre del 2018. Fueron seleccionadas 28 referencias. Se concluye que existen cambios celulares durante el cese circulatorio, las maniobras de resucitación y en la reperfusión. En la parada cardiaca, los cambios celulares se expresan en todos los organelos y puede llevar a muerte por necrosis. Durante la reperfusión se producen nuevos cambios estructurales, por entrada de calcio, alteraciones en sodio, producción de radicales libres e inflamación. Los cambios morfofisiológicos dependerán del estado metabólico previo, el tiempo de parada cardiaca y la instauración eficaz de medidas de resucitación.

Cardiac arrest in post-anesthetic recovery period, a rare entity in sterilization tubal. Descending factors such as over-sedation, high spinal block, use of opioids, systemic toxicity from local anesthetics. Tubal ligation as prevention of definitive pregnancy, is preferably performed under spinal anesthesia. A 35-year-old patient in his 12-hour puerperium, undergoes Sterrilization Tubal under spinal anesthesia, with fentanyl doses of 20 mcg and hiperbaric bupivacaine of 12 mg. Dermatome level reached of T6 (ideal for the surgical procedure), without any other drug administration. In the post-anesthesia recovery period, she presented cardiac arrest with immediate initiation of advanced life support with good later results. Complementary exams and corresponding consultations are given. Patient released from hospital with no neurological sequelae 4 days post the incident. It is recommended to prevent and treat the cause of all cardiac events. [ABSTRACT FROM AUTHOR]

Objective: to evaluate the knowledge of nursing students about the care of patients in cardiorespiratory arrest. Method: cross-sectional study with quantitative approach, conducted with 136 nursing students from a higher education institution. Data were collected using a semi-structured instrument. The analysis was performed through descriptive and inferential statistics. Results: the students who positively evaluated themselves showed a good level of knowledge about the theme (r=0.29; p=0.001), as well as those who had attended the first aid course in previous periods (p=0.012). The questions they made mistakes were those regarding the routes of administration of the drug and ventilatory support in non-intubated patients. Conclusion: the nursing students demonstrated an adequate level of understanding of the protocols on the care of patients in cardiorespiratory arrest and had a theoretical framework obtained in the discipline of first aid, as well as in improvement courses. Trauma is still one of the main causes of morbidity and mortality worldwide. Traumatic cardiac arrest (TCA) stands out among the causes of death of these patients. Multiple authors claim that, despite medical advances, resuscitation efforts in these patients may lead to poor outcomes, since those suffering from TCA have high mortality rates and poor neurological prognosis. This article compiles the available evidence, which despite being limited, points to recent advances in the management and approach of these patients. BACKGROUND: The incidence of post-surgical cardiac arrest, in patients who have undergone heart surgery, is 0.7-8%. The prognosis for advanced life support is relatively good in cardiovascular surgical units, which as of 2009 is recommended by the European Association for Cardio Thoracic Surgery and the Society of Thoracic Surgeons in 2017. CLINICAL CASE: Patient with a diagnosis of yuxtaductal aortic coarctation, with aortic arch hypoplasia, who was admitted to pediatric intensive care after surgical correction with interposition of a prosthetic graft of Woven Dacron® in the affected site that, after 72 hours, went into cardiac arrest; The advanced life support protocol was initiated in cardiovascular surgical units which, unlike the universal advanced pediatric life support protocol recommended by the American Heart Association, interrupts all infusions, including inotropes, does not administer doses of adrenaline, and interrupts chest compressions to allow for re-esterotomy, thus avoiding wasting time on ineffective compressions. CONCLUSIONS: The CSU-ALS protocol provides guidance specifically aimed at patients with post-surgical cardiac arrest, unlike the PALS protocol, which is recommended by the American Heart Association in a standardized way for all patients, especially when faced with undetected, unmonitored cardiac arrest. Introduction: In-hospital cardiac arrest is a rare but life-threatening event. However, the epidemiology of this event in intensive care units is not clear. Objective: To determine the clinical characteristics of cardiac arrest in adult patients hospitalized in several intensive care units (ICU) of the Metropolitan Area of the Aburrá Valley, Colombia, over a period of one year. Methods: Observational study for a limited period of one year for adult patients with cardiac arrest in the ICU of the hospitals of the Metropolitan Area of the Aburrá Valley, Colombia: San Vicente Foundation University Hospital, IPS University, and Manuel Uribe Ángel Hospital. Results: Of 3710 eligible patients who were treated in 91 beds, 646 cardiac arrest events occurred during this period, of which 151 were candidates for resuscitation maneuvers. The overall incidence of cardiac arrest in the ICU was 17.1%, without differences between the three hospitals included and the incidence of resuscitable cardiac arrest was 39.9 cases per 1000 admissions to the ICU. The most common cardiac arrest rhythm was asystole 54.3% and the overall survival at hospital discharge was 3.3%. Conclusion: In the analyzed ICU, cardiac arrest was an infrequent event, but it presents a high mortality at discharge from the ICU and hospital. Introduction: Cardiac arrest is a critical health condition whose survival rate is associated with the speed with which cardiopulmonary resuscitation is performed. Objective: To assess self-confidence, knowledge and skills of nursing interns about cardiopulmonary resuscitation. Materials and methods: A quantitative descriptive study was conducted between April and July 2018 with 80 nursing interns at a public university in northeastern Brazil. A knowledge test, a self-confidence scale for emergency intervention and an assessment of CPR practical skills using a mannequin were applied. Kruskal-Wallis and Pearson’s Chi-square tests were also used. Results: Nursing interns showed low self-confidence before an emergency. The theoretical questions about the location to check the victim’s pulse and position were correctly answered by most of the nursing interns. The main gaps in knowledge were related to the initiation of compressions (p = 0.245) and the rate of compressions/breaths (p = 0.034). In terms of skills, the gaps in knowledge were related to the position of the rescuer’s hands and arms (p = 0.058), trunk movement (p = 0.062) and the right depth of compressions (p = 0.086). Discussion: Regular training and continuing education are necessary for rapid, safe and effective care which will be provided by future nurses. Conclusions: Gaps in knowledge and skills were evident, highlighting the importance of new methodologies to intensify and ensure the effectiveness of the teaching-learning process. In this review, the international recommendations on the subject are consulted, high-risk moments are defined and recommendations are established on compressions, ventilation, electrical therapy and even pharmacology in patients with cardiac arrest and with a diagnosis of COVID-19, seeking not only the well-being of the patient, but also the safety of health personnel.
