Objective: In this study, our objective was to evaluate mean platelet volume (MPV), an indirect marker of platelet activation, in patients with obstructive sleep apnea (OSA), and assess the effect of OSA treatment with continuous positive airway pressure (CPAP) on MPV. Materials and Methods: In this study, records of consecutive patients who underwent polysomnographic evaluation for OSA symptoms in the Sleep Disorders Laboratory during a one-year period were reviewed retrospectively. Patients who had both complete blood count and MPV measurements were included in the study. Results: A total of 158 patients, including 51 females (32.3%) and 107 males (67.7%), were included in the study. The mean age of the patients was 51±13 (min-18, max-82) years. OSA was detected in 74.1% (117/158) of the patients. It was determined that as the severity of OSA increased, hemoglobin and hematocrit values increased significantly. There was no significant difference in platelet count according to the presence and severity of OSA. The MPV was significantly lower in severe OSA cases compared to those without OSA and mild OSA cases. A negative correlation was observed between MPV and the apnea-hypopnea index, desaturation index, and the amount of oxygen saturation below 90% during sleep. There was no significant difference in median erythrocyte and thrombocyte counts, hematocrit percentage and hemoglobin values before and after treatment in OSA patients who used CPAP therapy. However, a significant decrease in MPV was observed after OSA treatment compared to pre-treatment. (p=0.021). Conclusions: The results of the study do not support an increase in MPV and hence platelet activation in severe OSA patients compared with those without OSA. However, the results suggest that one month of CPAP treatment reduces MPV and thus platelet activation in severe OSA patients. Background: Background: In acute pancreatitis (AP), determining the severity of the disease starting from hospitalization is important for planning the treatment. Data obtained in previous studies that examining the clinical importance of RDW (Erythrocyte Distribution Volume) and MPV (Mean Platelet Volume) levels in patients with AP are contradictory. Our aim in this study is to determine whether RDW and MPV levels can predict the severity of pancreatitis and related mortality. Materials and Methods: The data of 262 patients hospitalized due to AP between 2019 and 2022 were retrospectively examined. Patients were divided into groups according to Atlanta criteria.Then, the relationship between RDW and MPV values and the severity and prognosis of pancreatitis was examined statistically. Results: While 83.9% (n=220) of the patients had mild pancreatitis, 16.1% (n=42) had severe pancreatitis. While the mean RDW index was 42.5±6.9 fL in patients with mild AP, it was 42.0±6.6 fL in severe AP patients and there was no significant difference between the two groups (p=0.625). In addition, there was no significant difference between the two groups in terms of mean MPV level (9.7±1.0 fL vs 9.8±1.3 Fl, p=0.687). ROC analysis showed that there was no significant and critical MPV and RDW threshold value to detect the severity of pancreatitis (AUC values: 0.499 and 0.453, respectively). Conclusions: In our study, we found that there was no significant difference in terms of RDW and MPV in patients with mild and severe pancreatitis according to the Atlanta criteria. Therefore, we can say that MPV and RDW levels are not sufficient and strong parameters to predict the severity and prognosis of pancreatitis. [ABSTRACT FROM AUTHOR]

5. Comparison of First and Third Trimester Complete Blood Count Parameters for Prediction of Preterm Birth.

Introduction: The complete blood count parameters and its combinations, such as neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and mean platelet volume (MPV), are inflammatory markers. In this study, we aimed to demonstrate the possible role of complete blood count parameters (NLR, PLR, and MPV) in predicting preterm birth. Material and Methods: The complete blood count parameters in the first and third trimesters were recorded, as well as the corresponding NLR, PLR, and MPV. The study population was categorized as preterm (n = 94) and term births (n = 953). The preterm birth group was further divided into early preterm (n = 11) and late preterm birth (n = 83) groups. The inflammatory markers were compared between the study groups for the first and third trimesters separately. The alterations between the first and third trimester values were also compared. Results: The first trimester values were similar across the study groups. Moreover, the MPV difference between the first and third trimesters was significantly lower in the preterm group than in the term group (0.0 o1.1vs. O.2 0l.1,p = 0.038). Furthermore, NLR values were higher in the early preterm subgroup than in the late preterm and term groups for the first and third trimesters (first trimester; 4.0 D 1.2 vs. 3.1 D 2.0 and 3.1 D 1.3, p = 0.005; third trimester; 5.3 D 1.2 vs. 4.0 D 1.5 and 4.4 D 2.9, p = 0.013). Conclusion: NLR and MPV predict preterm births in the first trimester and before birth. This enables physicians to take some precautions in preventing preterm delivery. [ABSTRACT FROM AUTHOR]

Background: ST-segment elevation myocardial infarction (STEMI) is the most common reason for mortality worldwide. Scoring systems such as Syntax score plays a significant role in risk stratification in these patients. Besides, as a simple and easily applicable marker, white blood cell count to mean platelet volume ratio (WMR), which could play a vital role in identifying risky patients, has emerged as an indicator of inflammation in atherosclerotic disease. In the current study, we aimed to investigate the predictive role of WMR for Syntax score in patients with STEMI. Material and Method: Demographics, comorbidities, and laboratory results on the admission of hospitalized 335 patients with STEMI were analyzed. The study population was divided into two groups according to the syntax score median of the study population (Syntax score median=19). Findings were compared between Syntax score <19 and Syntax score ≥19 groups. Results: The Pearson correlation value between WMR and Syntax score was 0.415 (p<0.001). One hundred and sixty four patients were assigned to the Syntax score <19 group, 171 patients were designated as Syntax score ≥19 group. White blood cell count to mean platelet volume ratio was significantly higher in the Syntax score ≥19 group than the Syntax score <19 group [median (IQR), 1.56 (1.28–1.81) vs 1.32 (1.16–1.5), p<0.001]. White blood cell count to mean platelet volume ratio was an independent predictor according to multivariate analysis [Odds ratio (95% CI), 1.26 (1.08–1.47), p=0.002]. Conclusion: In patients with STEMI, WMR is significantly correlated with high syntax score. It could provide supportive data for early risk stratification and optimized approach. 9. Relationship Between Platelet Indices and Prolonged Hospitalization in Patients with Acute Pancreatitis: A Retrospective Observational Study

Aim: To investigate relationship between platelet count, platelet mass index, mean platelet volume, platelet distribution width and plateletcrit and prolonged hospitalization in patients with acute pancreatitis. Material and Method: This study was conducted as a retrospective cohort study of all patients with acute pancreatitis from a tertiary level, academic emergency department between June 2017 and July 2021. Demographics, comorbidities, laboratory parameters, length of stay in the hospital and 30-day mortality information of the patients were recorded using computer-based data system of the hospital. Hospitalizations lasting longer than 7 days were considered as prolonged hospitalization. Results: 752 patients with a median of age of 58 years (25th-75th percentiles: 43.5-75) were included in the study. The median length of hospital stay of the enrolled patients was 4 days (25th-75th percentiles: 3-7). The hospitalization of 166 patients was prolonged, and the prolonged hospitalization rate was 22.1%. The univariate analysis for platelet indices showed that there was no statistically significant difference [Platelet count (p=0.543), mean platelet volume (p=0.656), plateletcrit (p=0.427), platelet distribution width (p=0.497), and platelet mass index (p=0.484)]. Conclusion: There is no clear relationship between platelet indices and prolonged hospitalization and they could not be predictors of prolonged hospitalization in patients with acute pancreatitis.

11. The Use of Mean Platelet Volume, Plateletcrit, and N-terminal Brain Natriuretic Peptide as Biomarkers of Coronary Artery Involvement in Atypical Kawasaki Disease.

Objective: Coronary artery aneurysm and ectasia develop in approximately 15% to 25% of children with untreated Kawasaki disease (KD). Atypical KD has a higher incidence of coronary artery involvement compared to typical KD. Our aim in this study was to identify new markers to support early diagnosis and prevent complications associated with delayed treatment in atypical KD. Method: The patients’ demographic characteristics, presenting complaints, clinical findings, mean platelet volume (MPV), plateletcrit (PCT), and N-terminal brain natriuretic peptide (NT-proBNP) levels were analyzed. Coronary artery abnormalities were evaluated using two-dimensional echocardiography. Results are expressed as mean (± standard deviation). Results: Sixty children between the ages of 3 and 96 months who were diagnosed with atypical KD were included. Forty consecutive normal children were included as a control group. NT-proBNP, MPV, and PCT values were 381.7 (±272.7) pg/mL, 5.8 (±0.93) fL, and 0.266% (±0.92%) in the patient group and 48.5 (±28.5) pg/mL, 8.29 (±1.12) fL, and 0.227% (±0.78%) in the control group, respectively (p<0.001 for all). In the comparison of atypical KD patients with coronary artery involvement (CAI subgroup) and without (non-CAI subgroup), the NT-proBNP values in these subgroups were 542.9 (±226.8) and 171 (±161.7) pg/mL (p<0.001), MPV values were 5.8 (±0.77) and 6.54 (±0.95) fL (p<0.005), and PCT values were 0.264% (±0.1%) and 0.269% (±0.08%), respectively. Conclusion: The simultaneous evaluation of MPV, PCT, and NT-proBNP was useful for the diagnosis of atypical KD. NT-proBNP and MPV can be used as markers of CAI in atypical KD. [ABSTRACT FROM AUTHOR]

12. Risk Factors to Pre-Clinical Atherosclerosis Evaluated by Carotid Intima-Media Thickness in Juvenile Idiopathic Arthritis: Descriptive Research.

Objective: Juvenile idiopathic arthritis (JIA) is the most common chronic rheumatic disease in childhood. Cardiovascular morbidity and mortality are becoming important health problems for children with inflammatory rheumatic diseases. The aim of this study was to evaluate carotid intima-media thickness (CIMT) in children with JIA and to examine its association with JIA subtype, markers of inflammation, and early atherosclerosis. Material and Methods: We included 112 (50 boys and 62 girls) patients who have been followed by a diagnosis of JIA for at least 6 months and 54 healthy control subjects (32 girls and 22 males) who were matched with the patients age and gender. All children underwent a carotid artery. Inflammation markers were evaluated in the patient group. Cumulative drug doses, total disease duration (TDD), active disease duration (ADD) were calculated. All CIMT values were compared in patients, subgroups, and control groups, and their relationship with inflammation markers was investigated. Results: CIMT values in found that children with JIA were significantly higher than in healthy children. There was no difference between the disease subgroups in terms of CIMT. No relationship was found between CIMT measurements and atherosclerosis risk factors, drugs, erythrocyte sedimentation rate, white blood cell and C-reactive protein. A negative correlation was found between mean platelet volume (MPV) and right CIMT. A statistically significant positive correlation was detected between the right CIMT and TDD (r=0.221, p=0.022), and ADD (r=0.248, p=0.010). Conclusion: The study showed that the patients with JIA had more risks than healthy controls for cardiovascular disease (CVD) regardless of subgroup. We concluded early and aggressive therapies may be protective for CVD. Negative correlation was found between MPV and CIMT that is consistent with recently published literature, but there is need for further studies with a larger patient population. 13. The Relationship Between Mean Platelet Volume/Platelet Count (MPV/PLT) Ratio and Peripheral Artery Disease.

Background/Aim: In our study, we aimed to evaluate the effect of mean platelet volume/platelet count ratio on the development of peripheral artery disease (PAD) by comparing it with the control group. Material and Methods: A total of 115 patients with peripheral artery disease as a result of lower extremity color Doppler ultrasonography examination in our hospital were included in the study as the patient group and 100 individuals without peripheral artery disease as the control group. Results: History of coronary artery disease (p<0.001), diabetes mellitus (p=0.009), hypertension (p<0.001) and smoking history (p<0.001) were significantly higher in the PAD group than in the control group. Serum glucose (121.27±25.66 vs. 104.81±34.77; p<0.001), MPV levels (9.99±0.79 vs. 9.04±0.7; p<0.001) and MPV/PLT ratio (0.041±0.07 vs. 0.037±0.006, p< 0.001) were found to significantly higher in the PAD group than in the control group. Serum HDL levels (45.03±11.5 vs. 48.67±12.52 p=0.026) were significantly lower in the PAD group than in the control group. In multivariate analysis, age, MPV and MPV/PLT ratio were determined as independent predictors for peripheral arterial disease. Conclusion: In our study, we found that age, MPV and MPV/PLT ratio were independent predictors of peripheral artery disease. [ABSTRACT FROM AUTHOR]

15. Metabolik Sendromda Ortalama Trombosit Hacmi ve Trombosit Düzeylerinin Değerlendirilmesi.

Objective: In the study, the relationship between metabolic syndrome and average thrombocyte volume and thrombocytes values was evaluated. The difference between waist circumference, arterial hypertension, HDL, triglyceride levels, thrombocyte volume and thrombocytes levels between patients with metabolic syndrome and patients in control group was investigated. Methods: The study is a retrospective case-control study. Adult patients registered to Family Medicine Unit were retrospectively scanned between January 1, 2017 and December 1, 2020. The family medicine information system (FMIS) was utilized to assess which patients had metabolic syndrome and which patients did not. A simple random sample procedure was used to enroll 382 persons from the research population. From the data, the patients' sociodemographic parameters, waist circumference, arterial blood pressure, HDL, triglyceride, thrombocytes and average thrombocyte volume values, documented in the research form we developed. The IBM SPSS 21.0 program was used for statistical analysis, and the statistical significance threshold was set at p<0.05. Results: The patients in the research were 48.2 years old on average, with 61 percent of them being female. In individuals there is a weak, negative, statistically significant correlation between thrombocytes and average thrombocyte volume. It was found that the platelet value was higher in females than males. It was shown that as the level of education increases metabolic syndrome reduced. Abdominal obesity was found to be the most common metabolic syndrome component in the population with metabolic syndrome. It was determined that body mass index increased the risk of metabolic syndrome by 27.5%, triglyceride increased by 2%, fasting blood sugar by 14.9%, and platelet value increased by 1.1%. Conclusion: Patients with low educational status and obesity should be given priority in the diagnosis of patients with metabolic syndrome who are at high risk of cardiovascular problems in primary care. However, no effect of average thrombocyte volume in predicting metabolic syndrome was found. In our study, thrombocyte value was interpreted as metabolic syndrome positively. [ABSTRACT FROM AUTHOR]

OBJECTIVE: Platelet activation is thought to play a role in the angiogenesis process involved in the pathophysiology of retinopathy of prematurity (ROP). We planned this study to investigate whether the mean platelet volume (MPV), used to assess platelet activation, could be used as a biomarker in the diagnosis and treatment of ROP. STUDY DESIGN: In our study, we evaluated infants who underwent ROP examination among infants born at 32 weeks or less and/or 1500 grams or less as well as infants who had more than these values but experienced a bad neonatal period. These patients were divided into two groups as those with and without ROP, and in those with ROP group, as requiring treatment and not requiring treatment. We recorded the patients’ identity, maternal characteristics, antenatal/natal/postnatal features, and complications during follow-up, ROP control times, and complete blood count parameters (platelet count, MPV, platelet count/MPV). Primarily, we evaluated the differences of platelet parameters, especially the MPV values, between treatment requiring ROP and non-treatment requiring ROP groups and secondarily, we evaluated the correlation between scanned parameters and ROP development. RESULTS: Of the 144 patients included in the study, 49 patients (34%) had ROP and 25 patients (16.6%) had ROP requiring treatment. There were statistically significant differences between patients who were diagnosed with ROP and who were not, in terms of gestational age, birth weight, respiratory distress syndrome, surfactant use, duration of oxygen use, intraventricular hemorrhage, patent ductus arteriosus, neonatal sepsis, number of blood transfusions, necrotizing enterocolitis, bronchopulmonary dysplasia and time to catch up birth weight. However, there was no significant difference in terms of MPV, platelet count and platelet/MPV ratio. The gestational week, invasive ventilation day and duration of oxygen use, BPD and time to catch up birth weight were found to be statistically high in treatment-requiring rather than non-treatment-requiring ROP groups. There was no significant difference in MPV, platelet count, platelet/MPV ratio among these groups. A statistically significant difference was found in the most advanced stage ROP and plus existence in the treatment requiring ROP group. CONCLUSION: As a result of our study, we could not find enough data to say that MPV is a marker that can be used in the diagnosis of ROP and in measuring the severity of retinopathy. We suggest that more studies should be done on this subject.

Introduction: It was aimed to investigate the effects of vitamin B12 deficiency on blood tests, mean platelet volume (MPV), and neutrophil-lymphocyte ratio (NLR), which is one of the inflammation markers.Methods: Our study is a case-control study of 375 patients. 170 patients with low B12 ( lt;130 pg/dL) were accepted as the experimental group, and 205 individuals with normal B12 levels were accepted as the control group. Between these two groups, white blood cells (WBC), platelet count (Plt), hemoglobin (Hgb), mean erythrocyte volume (Mean Corpuscular Volume-MCV), mean platelet volume (Mean Platelet Volume-MPV), neutrophil, lymphocyte counts, and neutrophil -lymphocyte ratios (NLR) were examined.Results: The mean age was 48.45 ±17.497 years in the B12 deficient group and 51.93 ±16.175 years in the control group. The mean of vitamin B12 in the deficient group was 101.41 ± 20.50 pg/mL (min-max 37-130), while it was 257.24 ± 88.13 pg/mL (131–498) in the control group. It was observed that there was no statistically significant difference between WBC, Hgb, Plt, neutrophil, lymphocyte, MVC, MPV and NLR values.Conclusion: In our study, we could not find a significant relationship between B12 deficiency and blood tests. In previous studies, there has been information that MPV values are affected by B12 deficiency. In our study, we found that there was no significant relationship between B12 deficiency and MPV or NLR (p gt; 0.05). There is a need for new studies on how B12 deficiency will change as the degree and duration of B12 deficiency increases, especially its effect on atherogenic events.

Background: Coagulation abnormalities has been reported in patients with impaired metabolism of thyroid hormones. Platelets play an important role in coagulation and Mean Platelet Volume has been considered as an inflammatory biomarker in multiple diseases. Objectives: The aim of this study was to investigate whether any relationship exists between the values of thyroid-stimulating hormone, free thyroxine, anti-thyroid peroxidase, anti-thyroglobulin and those of the MPV. Methods: Patients who were admitted to the Endocrinology outpatient clinic between October 2013 and July 2019 with a pre-diagnosis of thyroid disease were included in the study. The data were analyzed with IBM SPSS V23. Compatibility with normal distribution was examined with the Shapiro Wilk test. The relation between the variables was evaluated with Spearman rank correlation. Results: Records of 1098 patients were examined. There is a very weak positive relationship between TSH and MPV (r: 0,075), there is no significant relationship between ST4, Anti TPO, Anti TG and MPV. Conclusion: Patients have high TSH values display a increased MPV should hence be acknowledged in risk prediction of thrombotic events.

Objective: Mean platelet volume (MPV) has been associated with autoimmune diseases and is considered a potentially useful parameter to assess platelet activity and subclinical inflammation. In the present study, we aimed to investigate the effects of thyroid autoimmunity on MPV. Methods: 84 patients with newly diagnosed Hashimoto's thyroiditis (HT), including 44 euthyroid (EHT, mean age 41.4±10.8 years) and 40 hypothyroid (HHT, mean age 44.6±13.2 years) patients, 61 patients with newly diagnosed Graves' disease (GD, mean age 42.8±12.4 years) and 57 euthyroid control subjects (age-matched 46.4±11.6 years) were included in the study. Patients were not taking antithyroid medication or levothyroxine. Thyroid hormones and platelet parameters were determined in all study participants. Results: The MPV of the patients was significantly higher than that of the control group (p:0.008). There was no association between MPV and age, sex, or TSH (p:0.06, p:0.4, and p:0.9, respectively). MPV increased in chronic autoimmune thyroid disease regardless of age, sex, and TSH. Conclusion: Our results suggest that autoimmunity, rather than thyroid hormone levels, affects MPV in GD and HT. - Abstract
Aim: Transient tachypnea of the newborn (TTN) is the most common respiratory problem encountered by physicians working in the neonatal intensive care unit, and its incidence is higher in late preterm infants compared to term infants. Platelets prevent pulmonary fluid collection and pulmonary edema by an unknown mechanism. Platelet function can be more accurately assessed by platelet mass index (PMI) rather than mean platelet volume (MPV) or platelet count alone. In our study, it was planned to investigate the duration of need for respiratory support and the relationship between thrombocyte markers, especially PMI, and other respiratory outcomes in late preterm infants with the diagnosis of TTN. Materials and Methods: In this retrospective study, data of all late preterm newborns whose tachypnea lasted for at least 12 hours and who were hospitalized in the neonatal intensive care unit of Buca Seyfi Demirsoy Training and Research Hospital with the diagnosis of TTN between August 1, 2020-July 31, 2021 were analyzed retrospectively from medical records. The thrombocyte markers, leukocyte and neutrophil values of the infants and respiratory support durations were compared. In addition, these parameters were evaluated in terms of possible differences according to gestational weeks. Results: 52 infants were included in the study. The median gestational week was 35 weeks (34-366/7), and the mean birth weight was 2647+474. There was no correlation between total respiratory support duration or >48 hours of respiratory support in infants and platelet count, MPV, PMI, leukocyte and absolute neutrophil count (ANS). However, it was observed that the PMI values were lower in babies who received respiratory support for more than 72 hours, and this was statistically significant. In addition, it was observed that MPV value was significantly higher in babies born at 35th gestational week and ANS value was significantly higher in babies born at 36th gestational week. Conclusion: In our study, it was seen that PMI, one of the platelet markers, was associated with the need for respiratory support for more than 72 hours in late preterm infants with TTN, as the mean PMI values of these infants being lower. Background: It was aimed to investigate the effects of neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR) and mean platelet volume (MPV) on mortality in cases of acute type A aortic dissection in this study. Materials and Methods: The files of patients who were operated on with the diagnosis of acute type A aortic dissection in Adıyaman Training and Research Hospital between January 2013 and November 2020 were reviewed retrospectively. The patients were divided into those who were discharged (Group I) and those who resulted in death (Group II). Demographic data, preoperative blood tests, operation times and mortality status of the patients were recorded. MPV, NLR and PLR were calculated and recorded in blood samples taken preoperatively. The obtained data were compared between groups and the relationship of the data with mortality was examined. Results: 23 of the patients in the study were male and 9 were female. While 22 of the patients were discharged with cure, 10 patients resulted in mortality. A significant difference was found in the exit group in terms of EF, CRP and cardiopulmonary bypass times (p<0.05). MPV, NLR and PLR were found to be higher in patients who resulted in death. However, this difference was not statistically significant (p>0.05). Conclusions: MPV, NLR and PLR could not be shown to have an effect on predicting mortality in acute type A aortic dissection. Objective: The relationship between chronic inflammation and psychiatric disorders has been evaluated in previous studies. Neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), and mean platelet volume (MPV) appear to be new markers for the evaluation of systemic inflammatory response. In this study, it was aimed to evaluate the levels of NLR, PLR and MPV biomarkers in patients with major depressive disorder (MDD) by comparing them with healthy controls, and also investigate the relationship between biomarkers and MDD. Material and Method: This cross-sectional study was carried out from July 15, 2020 to May 15, 2021. The study was conducted with 92 patients with diagnosed MDD and 72 healthy controls who agreed to participate. The sociodemographic data form and SCID-5/CV form were filled out for each participant through face-to-face interviews with all participants. Additionally, Hamilton Depression Scale was applied to the patient group to determine the severity of MDB. Results: Patients with MDD and healthy controls were compared in terms of the mean scores of NLR, PLR and MPV, and no statistically significant difference was found between the groups (p>0.05). Additionally, in the patient group, there was no statistically significant difference between the levels of depression severity (mild, moderate, severe) in terms of the mean scores of NLR, PLR, and MPV (p>0.05). Conclusion: This is the first study to evaluate NLR, PLR and MPV biomarkers simultaneously in MDD patients by comparing them with healthy controls. In this study, it was found that there was no significant relationship between NLR, PLR and MPV levels and MDD. Additionally, it was determined that there was no relationship between the severity of depression and the aforementioned markers. Further studies investigating these and another biomarkers with prospective and larger studies will provide more insight into the inflammatory process in MDD. [ABSTRACT FROM AUTHOR]

29. The Role of Platelet Indices in Predicting Short-Term Mortality in Elderly Patients with Pulmonary Embolism

Objective: This study aimed to investigate the role of platelet count, mean platelet volume (MPV), plateletcrit, platelet distribution width (PDW), platelet mass index (PMI), and MPV-to-platelet count ratio (MPR) in predicting short-term mortality in patients aged 65 and over with pulmonary embolism. Methods: This retrospective, observational, cohort study included corrected pulmonary embolism patients. Demographics, clinical characteristics, and platelet indices and the data of all-cause mortality within 30 days after admission were noted. The receiver operating characteristic curve analysis and multivariate analysis was performed to determine the discriminative ability of the scores. Results: A final analysis of 128 patients was performed. The mortality rate was 21.8%. There was no significant relationship in the multivariate analysis between mortality and platelet indices (Mann-Whitney U test). Area under curves of neutrophil-to-lymphocyte ratio, PMI, MPR, plateletcrit, platelet count, and MPV are 0.501, 0.640, 0.626, 0.642, 0.633, and 0.532, respectively. Conclusion As a conclusion based on results of our simple, platelet indices could not predict short-term mortality in elderly patients with pulmonary embolism.

Aim: Potent combined and long-term antithrombotic therapies that predispose to gastric injury are the mainstay of treatment in acute coronary syndromes (ACS). Severe chronic gastric mucosal inflammation due to the Helicobacter Pylori (H. pylori) infection was shown to be associated with higher peripheral blood lymphocytes and lower blood mean platelet volume (MPV) levels. We aimed to investigate the discriminative usefulness of blood lymphocyte to MPV ratio as a simple premise marker for CagA positive H. Pylori infection before the required advanced diagnostic tests in patients with coronary arterial disease (CAD). Materials and Methods: A total of 293 patients’ who had undergone elective and urgent coronary angiography due to CAD were included in the study. Serologic H. pylori infection status and hematological parameters were determined. Two groups were compared according to CagA serology status. Confounding factors were adjusted by propensity score matching and multivariate logistic regression analysis. Results: Rates of ACS, male gender, diabetes mellitus, family history of CAD, current smoking and lymphocyte to MPV ratio were higher in seropositive patients according to seronegative patients (p lt; 0.05). The ROC curve analysis showed that the lymphocyte to MPV ratio at a cut-off point of 165 had 71% sensitivity and 60% specificity for discriminating patients with positive H. pylori serology (AUC = 0.71, p lt; 0.0001). Lymphocyte to MPV ratio was independently associated with positive H. Pylori serology. Conclusion: Lymphocyte to MPV ratio can be helpful for discriminating CagA positive H pylori infected CAD patients requiring advanced confirmatory tests.

Objective: Thrombocytes and mean platelet volume (MPV) are related to atherosclerosis. The purpose of the present study was to investigate the relationship between MPV and subclavian arterial stenosis (SAS). Materials and Methods: Patients who were diagnosed with SAS by an angiography performed in the catheter laboratory between September 2010 and September 2015 were included in this study. A control group with similar clinical and demographic characteristics as the patients with SAS in angiographic terms was formed. The MPV and other hemogram values for SAS patients (35 patients) and the patients in the control group (38 patients) were determined retrospectively from the registry system and then compared. Results: The two groups were similar to each other in terms of demographic and clinical characteristics. When the hemogram parameters were compared between the groups, the MPV was detected as being significantly higher in patients with SAS (8.07 ± 1.41 vs 7.19 ± 0.52; p = 0.001). Conclusion: In conclusion, this study was the first to find a higher MPV in patients with SAS. Considering both the effect of platelets on atherosclerosis and their close association with other risk factors, MPV level may be an important factor in pathogenesis of SAS.

Introduction: Intracerebral Spontaneous Hemorrhage (ISH) is a sudden hemorrhage into the brain parenchyma as a result of a rupture of the cerebral vessels that are not related to traumas. The purpose of the present study was to evaluate the relation of mortality with Platelet Volume Index (PVI) scoring systems, which have been reported in a small number of studies in terms of blood RDW, MPV, platelet and intracranial hemorrhages in patients diagnosed with intraparenchymal hemorrhage in Emergency Departments. Methods: This study was performed retrospectively by examining patient files of patients who came to Emergency Medicine Clinic between 01.01.2019-31.12.2019. As a control group for comparison of blood parameters, blood parameters of 72 patients suitable for the same age population, without intraparenchymal haemorrhage.Results: A total of 54.10% (n=85) intraparenchymal bleeding patients (IPC) and 45.90% (n=72) healthy control groups (HCG) were included in the study. There was no statistically significant difference between the clinical results of IPC groups in terms of mortality of RDW parameter and MPV parameter (p=0.930; p=0.118). When PVI ratio was evaluated in IPC group and HCG; the mean PVI (MPV/Platelet ratio) in the IPC group was 4.37±1.66, and the PVI (MPV/Platelet ratio) in the HCG was 3.89±1.02.A statistically significant difference was found between the PVI in the patient group and the HCG.A statistically significant difference was found between the PVI ratio clinical results of the patients with IPC (p=0.043).Conclusion: Spontaneous Intraparenchymal hemorrhage are among the leading causes of stroke-induced mortality and disability. The Hemphill Score and hemorrhage volume are important factors in mortality evaluation in intraparenchymal hemorrhage. There are not many studied conducted on Platelet Volume Index, and it is an important marker in predicting mortality, especially in these patients.

Giriş: Bu çalışma trombosit parametreleri ve diğer biyokimyasal değerlerin otoimmün hepatit (OH) hastalarında karaciğer hasarının değerlendirilmesinde prediktif bir parametre olarak kullanılabilirliğini değerlendirmek amacıyla yapıldı. Gereç ve Yöntemler: Gastroenteroloji kliniğimizde OH hastalarının histolojik bulguları laboratuvar ve klinik parametreleri üzerinden retrospektif kesitsel tipte tanımlayıcı ve metodolojik bir çalışma planladık. Skorlama sınıflamasıyla yapılan değerlendirmeye göre, 0 ile 12 arasında histolojik aktivite indeksi (HAI) olan hastalar düşük aktivite, 13 ile 18 arasında HAI olan hastalar yüksek aktivite olarak tanımlandı. Fibrozis skoru 0-2 olan hastalar erken fibrozis, fibrozis skoru 3 ile 6 arasında olanlar ileri fibrozis olarak tanımlanmış ve buna göre karşılaştırmalar yapılmıştır. Bulgular: Otoimmün hepatitli hastalarda HAI skoru yüksek aktivite gösteren grupta ortalama trombosit hacmi (MPV) düzeyleri düşük aktivite gösteren gruba göre daha yüksek saptanmıştır (p=0,013). Otoimmün hepatitli hastalarda fibrozis sınıfları arasında MPV düzeyleri açısından istatistiksel olarak anlamlı bir fark saptanmıştır (p=0,034). Otoimmün hepatitli hastalarda HAI sınıflamasında yüksek aktivite ve fibrozis sınıflamasına göre ileri fibrozis düzeyinde olan hastaları ayırt etmede MPV ölçümlerinin ROC eğrisi altında kalan alan değerleri, sensitivite ve spesifitesi incelendiğinde istatistiksel olarak anlamlı olduğu saptanmıştır. Tartışma: Otoimmün hepatitli hastalarda MPV değerlerinin HAI sınıflamasına göre yüksek aktivite gösteren grup ile karşılaştırıldığında pozitif korelasyon saptanmıştır. Amaç: Bu çalışmada akut iskemik inme tanısı alan ve orta serebral arter (OSA) enfarktı saptanan hastalarda nötrofil/lenfosit oranı (NLR) ve ortalama trombosit hacminin (MPV) kısa dönem prognoz üzerine etkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Bir üniversite hastanesi Nöroloji Yoğun Bakım ve İnme Ünitesine akut iskemik inme tanısıyla yatan ve OSA enfarktı saptanan hastalar alınmıştır. Kontrol grubu olarak Nöroloji polikliniğine başağrısı yakınmasıyla başvurmuş, inme öyküsü olmayan, bilinen sistemik hastalığı olmayan yaş ve cinsiyet dağılımı benzer hastalar bu çalışmaya alınmıştır. Hasta ve kontrol grubunun NLR ve MPV değerleri kaydedildi. Ayrıca hastaların yatış sırasında ve taburculuk sırasındaki NIHSS (National Institute of Health Stroke Scale) skoru, taburculuk sırasındaki modifiye Rankin skoru (mRS) kaydedildi. Bulgular: Çalışmaya dahil edilen 72 hastanın % 58,3’ü (n=42) erkekti. Hasta ve kontrol grubu NLR ve MPV ortalamaları açısından karşılaştırılmış ve her iki parametre de hasta grubunda kontrol grubuna göre daha yüksek bulunmuştur. NLR kötü prognoz grubunda iyi prognoz grubuna göre ve mortalite grubunda sağ kalım grubuna göre daha yüksek saptanmıştır. Sonuç: Klinisyenler OSA enfarktı olan hastalarda NLR ile kısa dönem prognoz tayininde kullanabilirler ve bu kolayca belirlenebilir ölçüm, objektif, basit ve kolay ulaşılabilirdir, aynı zamanda herhangi bir ek maliyete de yol açmaz.

Abstract Objective: Coronary artery ectasia (CAE) are defined as the dilation of a segment of a coronary artery at least 1.5 times the adjacent segment. Several studies have shown the association between mean platelet volume (MPV) and cardiovascular diseases. But the results are different. For this reason, we aimed to investigate the association of MPV with CAE. Materials and Methods: 90 patients with normal coronary arteries and 70 patients with CAE were included. Routine blood and biochemical parameters were measured before the arteriography. Differences between groups for continuous variables were analyzed with t- test or Mann-Whitney U test. P values lt; 0.05 were considered significant. Results: Baseline patient demographics, including age and clinical risk factors, were similar between the groups. Compared to the control group, MPV levels were significantly lower in the CAE group (8.84 ± 1.71fL vs. 10.43 ± 1.34, p lt; 0.001). MPV level of lt; 9,5 fL showed sensitivity , specificity value of 95%, 80%; area under the curve= 0.943, 95% CI, 0.906–0.980) for the prediction of CAE. Conclusion: The present study suggests that MPV may decrease in patients with CAE.

Amaç: Kolay elde edilebilirlik ve yaygın kullanım imkanı akut karın ağrılı hastaların erken tanısında biyokimyasal belirteçleri oldukça önemli hale getirmektedir. Bu çalışmada akut karın ağrısı ile acil servise başvurup yatışı yapılan hastalarda ortalama trombosit hacmi MPV ’nin tanısal değerinin araştırılması planlandı. Gereç ve Yöntem: Ocak 2012 -Mart 2013 tarihleri arasında acil servise başvuran 812 hasta ile 45 sağlıklı kişinin kayıtları retrospektif olarak incelendi. Kayıtlardan olguların; yaş, cinsiyet, fizik muayene bulguları, 5.gün MPV, platelet, nötrofil lenfosit oranı, lökosit, C-reaktif protein CRP ile MPV değerleri aralarındaki ilişki incelendi.Bulgular: Çalışmaya alınan hastaların 467’sini %57,5 erkekler, 345’ini %42,5 kadınlar oluşturmuştur. Hastaların yaş ortalamaları 53,09±0,75’dir . Hastaların kesin tanıları incelendiğinde en sık safra kesesi ve koledok patolojileri %18,7 , ikinci sırada nonspesifik karın ağrısı %17,3 , üçüncü ise ileus-volvulus %13,8 idi. Çalışmaya alınan 812 hastanın 5. gün MPV ortalama değeri 8,358±1,046 fL, platelet değerleri ortalama 246,82±91,3 u/L, nötrofil lenfosit oranı ortalama değeri 10,025±12,75 u/L olarak bulundu. Çalışmadaki akut karın ağrılı hasta grubunda giriş MPV değerleri ortalama 7,869±1,071 fL, sağlıklı kontrol grubu MPV değerleri ortalama 8,47±1,058 fL olup, hasta grubunun ortalama MPV düzeyi, sağlıklı bireylerden daha düşüktü. Sonuç: Pankreatit ve ileus-volvulus tanısı alan hastalarda MPV’nin tanıya yardımcı bir belirteç olabileceğini düşünmekteyiz

42. The Importance of High Mean Platelet Volume in Patients with Gastric Cancer.

Objective: The aim of this study was to investigate the use of mean platelet volume (MPV) as a marker in the diagnosis of gastric cancer and the prognostic importance of follow-up after treatment. Methods: A total of 296 individuals (148 healthy and 148 patients having gastric cancer) between January 2010 and July 2018 were included the study. The possible importance of MPV elevation in diagnosis and prognosis of this cancer type was evaluated. Results: The mean MPV of the healthy participants was 8.45±0.94 fL, while the mean preoperative MPV value of the patients with gastric cancer was 10.04±0.97 fL (p<0.001). The follow-up of MPV level in the gastric cancer patients revealed a significant change (p<0.001). No crucial difference was observed in MPV level according to tumor characteristics or cancer stage (p>0.05). A significant difference in the MPV level was seen with respect to overall survival, while no significant difference was found for disease free survival (p=0.009 and p=0.353). Conclusion: High MPV level may be warning when evaluated together with the patient's clinical findings for the prognosis and diagnosis of gastric cancer. It is a noninvasive, simple hematology parameter that requires no additional cost and it may have even greater significance when used with other markers for gastric cancer. [ABSTRACT FROM AUTHOR]

Background and Design: Hidradenitis suppurativa (HS) is a chronic inflammatory condition of the apocrine glands. The mean platelet volume (MPV) is a risk factor for many inflammatory diseases and can be used as a marker of inflammatory response. This study aimed to evaluate the MPV and platelet count in patients with HS and compare them with those of healthy controls. Materials and Methods: Between September 2014 and October 2019, 97 patients diagnosed with HS and 78 healthy controls were included. Their MPVs and platelet counts were compared retrospectively. Results: The MPV was 10.17±0.94 in the patient group and 9.53±0.63 in the control group, with the difference being significant (p<0.05). No significant difference was noted between the groups in terms of platelet values. Conclusion: The MPV was significantly higher in patients with HS than in healthy controls. Objective: It has been shown that mean platelet volume (MPV) can be used as a marker in many chronic diseases, including inflammatory diseases. In our study, we aimed to examine the relationship between disease severity, genetic mutation, attack/no-attack episodes with MPV levels in children with Familial Mediterranean Fever (FMF). Material and Methods: 109 children diagnosed with FMF according to Tel-Hashomer criteria were included in the study. The patients were classified according to the Pras scoring system. Patients demographic characteristics, genetic screening results, admission complaints and complete blood count parameters, MPV levels, biochemical tests, proteinuria amounts, fibrinogen, erythrocyte sedimentation rate, C-reactive protein values were recorded separately according to their attack/without attack periods. Results: A total of 109 (female/male: 58/51) patients were included in our study. The mean age of the patients was 9.22±4.28 (minimum-maximum: 1.1-17.9) years. A significant relationship between the genetic results of the patients and the disease score could not be shown. Symptom severity scores of patients with arthritis were significantly higher than those without arthritis (p<0.001). MPV was significantly lower in the attack period compared to the attack-free period (7.92±0.19; 8.19±0.27 fL, p=0.001). Those with the highest disease severity score showed the lowest MPV level (p=0.048). MPV levels of patients with proteinuria during the attack period were lower than those without (p=0.03). Conclusion: We think that MPV measurement, which is an easy, inexpensive and fast method, can be a supportive parameter in determining the severity of attacks and disease of FMF patients. Objective: Inflammation markers increase during Familial Mediterranean Fever (FMF) attacks and often return to normal within a few days. It is known that a subclinical inflammation continues in FMF patients in the inter-episode period, which increases the risk of developing amyloidosis. In recent years, several works have been conducted showing the relationship between platelet indices and inflammation, such as platelet count (PLT), platelet distribution width (PDW), mean platelet volume (MPV), plateletrite (PCT). In this study, we aimed to investigate the relationship between platelet indices and disease severity in FMF patients without attacks. Method: The records of all FMF patients followed by us were reviewed retrospectively. Patients with a hematological or oncological disease, patients with data deficiency for the study, and patients under 18 years of age were excluded. While the laboratory data were recorded, the condition of patients to be at least 3 months without attack was required. As the control group, the automation records of all healthcare professionals working in the internal medicine clinic of our hospital were examined. Our clinical staff, who did not have chronic diseases and regular drug use, were taken as a healthy control group. 173 patients and 20 control groups who met the conditions mentioned above were included in the study. Platelet indices were compared between patient and control groups. Results: It was determined that the value of MPV, one of the platelet indices, in the patient group was statistically significantly higher than the value in the healthy control group (p = 0.029). MPV was also found to correlate with disease severity (p <0.001 r = 0.329). Discussion-Conclusion: In our study, it was determined that MPV was higher in FMF patients in the subclinical period compared to healthy controls and also correlated with disease severity. Studies investigating platelet indices and the risk of developing amyloidosis are needed in FMF patients. [ABSTRACT FROM AUTHOR]

48. EVALUATION OF THE RELATIONSHIP BETWEEN MPV AND SEVERITY OF CORONARY ARTERY DISEASE IN TYPE 2 DIABETIC PATIENTS WITHOUT ACUTE CORONARY SYNDROME.

- Abstract
Objective Since many studies focused on the relationship between mean platelet volume (MPV) and severity of type 2 diabetes mellitus (T2DM) or investigated MPV and coronary artery disease (CAD) mostly on acute coronary syndrome (ACS), this study primarily focused to investigate if MPV values have any relation with different levels of CAD specifically in T2DM patients, who had no previous coronary artery disease, and no ACS history. Materials And Methods T2DM patients who had first coronary angiography (CAG) between November 2019 and February 2020, and whose after night fasting blood tests were performed before CAG were included. Two cardiologists, blinded to the patients, evaluated angiographies. Patients have divided into groups accordingly; normal coronary artery (Group 1), non-critic stenosis (Group II), and severe CAD (Group III) according to the Gensini score. Patients' demographics, heart rate, risk factors of CAD, and blood test including MPV were also collected. Results A total of 180 patients (88 female, 92 male) were involved in the study. There was no significant relation between MPV and the severity of CAD in T2DM patients (p=0.228). The mean age of the Group I group was significantly lower than in other groups (p=0.228), and there were significantly lower males in Group I than other groups (p=0.0001). The triglycerides levels were significantly low in Group I than in other groups (p=0.021 and p=0.006). Conclusion MPV values were not associated with the severity of CAD in stable patients. Older age, male gender, higher levels of triglycerides, and initial glucose levels were significantly related to the severity of CAD in T2DM patients. [ABSTRACT FROM AUTHOR]

49. AKNE VULGARİS HASTALARINDA İSOTRETİNOİN TEDAVİSİNİN SERUM ÜRİK ASİT VE ORTALAMA TROMBOSİT HACMİ (MPV) ÜZERİNE OLAN ETKİLERİNİN ARAŞTIRILMASI.

- Abstract
Objective Oral isotretinoin is an effective treatment used for years in the treatment of acne vulgaris. Many side effects of it have been reported, and new ones appear day by day. In this study, it was aimed to investigate whether isotretinoin causes changes in serum uric acid and MPV levels, which are markers that can be evaluated in many diseases. Material and Methods 78 patients who were admitted to Dermatology Outpatient Clinic of our hospital and were diagnosed with mild to moderate or severe acne vulgaris and to whom isotretinoin treatment was started at least three months ago, were included in this study. Hemogram parameters, lipid levels, monocyte / HDL ratio, MPV and uric acid levels were examined before treatment and at the third month of treatment in patients who started isotretinoin at a dose of approximately 0.5 mg / kg / day as a standard. Results While there was no significant change between the hemogram parameters, MPV and uric acid levels of the patients before and in the third month of treatment (p> 0.005); there were significant differences between lipid levels (triglyceride, total cholesterol, LDL) (p <0.005). When the patients were grouped as mild-moderate and severe acne according to the global acne scoring, no significant difference was found between the MPV and uric acid levels measured before the treatment and in the third month of treatment (p=0.43, p=0.23, p=0.31, p=0.14, respectively). Conclusion There are studies showing that uric acid and MPV may be associated with atherosclerosis in the literature. There are contradictory studies and case reports in terms of the effects of isotretinoin treatment on cardiovascular system, also on MPV or uric acid levels. There had been no previous study evaluating both parameters. In this study, we found that systemic isotretinoin treatment did not affect hematological parameters, MPV and serum uric acid parameters, which were shown to be associated with many inflammatory conditions including atherosclerosis. We think that isotretinoin treatment used in the treatment of acne vulgaris can be used safely in individuals without any underlying cardiovascular system or hematological disease.
