1. LINFOMA OCULAR EM PACIENTE ADULTO JOVEM: RELATO DE CASO

Abstract

Introdução: Linfomas orbitários são raros e compreendem aproximadamente 10% de todas as neoplasias orbitais. A maioria dos linfomas não Hodgkin dos anexos oculares são linfomas extranodais de células B da zona marginal. O objetivo é descrever o caso de uma paciente adulta jovem portadora de linfoma ocular de alto grau de progressão e sem manifestações sistêmicas. Resultados: Paciente MRBS, 23anos, feminino, encaminhada ao oftalmologista do Hospital Universitário Getúlio Vargas onde era atendida a cerca de 1 ano, para a FHEMOAM, com tumoração no olho direito suspeita de linfoma. Em 2021, durante a 2ª gravidez, notou aparecimento de tumoração no olho direito no 4° mês de gestação, acompanhamento clínico, com involução da tumoração sem tratamento. Após 6 meses, houve crescimento rápido do tumor, retornou ao serviço de oftalmologia onde realizou TC de seios da face e biopsia da lesão. A tomografia de seios de face mostrou existência de lesão expansiva e sólida infiltrando intra e extra-clonal, evidente no compartimento medial e desvio do globo ocular direito para a lateral, sem erosão da cavidade nasal/etmoide. A lesão mediu aproximadamente 4,6 × 1,6 cm. Ao exame histopatológico apresentou pele com subcutâneo infiltrado por neoplasia maligna de células linfoide de tamanho intermediário e padrão difuso de crescimento. Imuno-histoquímico demonstrou positividade para:Ki-67-Antígeno de Proliferação celular/CD20-Antígenos de linfócitos B/CD10-Antígeno Comum da Leucemia Linfoide Aguda/Proteína Antiapoptótica bcl-2/ Proteína BCL6-Fator de Transcrição (zine finger)/MUM1,Fator Regulador de Interferon,4,IRF4;cone MUM1p. A conclusão diagnóstica foi de linfoma B CD20+ de alto grau de progressão, com sugestão de pesquisa de translocação nos genes BCL2 e MYC pela técnica de FISH. Encaminhada para FHEMOAM. Exame físico: ausência de adenomegalias ou visceromegalias, investigação para estadiamento, tomografia computadorizada de tórax, com discreta osteofitose na coluna dorsal. Não foi possível o exame de PET CT. Achados importantes como a biópsia de medula óssea-BMO mostraram medula óssea normocelular para idade, com leve retardo de maturação da série granulocítica e hiperplasia relativa eritróide. O exame líquido efálio-raquidiano-LCR: normal. Considerando os exames realizados, a paciente recebeu o estadiamento I-A. Tratamento prescrito de ciclofosfamida, doxorrubicina, vincristina e prednisona. Após o 1° ciclo de quimioterapia-Qt apresentou redução clínico de mais 90% da massa tumoral. Evoluiu satisfatoriamente e a ressonância magnética da orbitas evidenciou discreta obliteração da gordura medial e inferomedial direta, adjacente a musculatura orbitária, músculos mediais e reto inferior e no aguardo por um PET Scan de controle pós-tratamento. Conclusão: O linfoma orbitário é raro, mas a quimioterapia mostrou-se eficiente no tratamento de linfoma B CD20+ de alto grau de progressão em região periocular. Exames específicos conseguem trazer respostas seguras e tomadas de decisões mais assertivas. Entre os exames de imagem mais comumente utilizados para o diagnóstico de linfomas estão a tomografia computadorizada ressonância magnética e a tomografia por emissão de pósitrons acoplada à tomografia computadorizada. A oftalmologia é fundamental para o diagnóstico precoce, biopsia da lesão, estudo sistêmico complementar e decisão de tratamento. O acompanhamento sistemático no mínimo 5 anos, é recomendável.

Abstract

Dysoxylum is a genus that has a variety of secondary metabolites. Research on various species of this genus is always growing and producing compounds that have interesting structures and activities, until now many compounds of the terpenoid group, chroman alkaloids, limonoids, sesquiterpenes, flavonoids, steroids, protolimonoids and sulfur have been reported. which is very interesting. One of the Dysoxylum species that has a variety of secondary metabolites is D. alliaceum. The purpose of this study was to obtain secondary metabolites of D. alliaceum bark. The bark of D. alliaceum was macerated successively with n-hexane, ethyl acetate and methanol. The ethyl acetate extract was separated and purified by various chromatographic techniques and was characterized using spectroscopic methods including, ultraviolet, infrared, NMR and mass spectroscopy and guided by thin layer chromatographic analysis to obtain the compound 6,7-dimethoxydyhidrocoumarin and the proposed biogenesis. The chemical structure of these compounds has been determined based on the interpretation of spectroscopic data and compared with spectral data from previous studies. Cytotoxic activity test against P-388 MTT leukemia cells obtained an IC50 of 39.210 g/mL and was declared inactive.

Abstract

We present and analyze observations of polarized dust emission at 850 $\mu$m towards the central 1 pc $\times$ 1 pc hub-filament structure of Monoceros R2 (Mon R2). The data are obtained with SCUBA-2/POL-2 on the James Clerk Maxwell Telescope (JCMT) as part of the BISTRO (B-fields in Star-forming Region Observations) survey. The orientations of the magnetic field follow the spiral structure of Mon R2, which are well-described by an axisymmetric magnetic field model. We estimate the turbulent component of the magnetic field using the angle difference between our observations and the best-fit model of the underlying large-scale mean magnetic field. This estimate is used to calculate the magnetic field strength using the Davis-Chandrasekhar-Fermi method, for which we also obtain the distribution of volume density and velocity dispersion using a column density map derived from $Herschel$ data and the C$^{18}$O ($J$ = 3-2) data taken with HARP on the JCMT, respectively. We make maps of magnetic field strengths and mass-to-flux ratios, finding that magnetic field strengths vary from 0.02 to 3.64 mG with a mean value of 1.0 $\pm$ 0.06 mG, and the mean critical mass-to-flux ratio is 0.47 $\pm$ 0.02. Additionally, the mean Alfv\'en Mach number is 0.35 $\pm$ 0.01. This suggests that in Mon R2, magnetic fields provide resistance against large-scale gravitational collapse, and magnetic pressure exceeds turbulent pressure. We also investigate the properties of each filament in Mon R2. Abstract

In the framework of the computational determination of highly-accurate vertical excitation energies in small organic compounds, we explore the possibilities offered by the equation-of-motion formalism relying on the approximate fourth-order coupled-cluster (CC) method, CC4. We demonstrate, using an extended set of more than 200 reference values based on CC including up to quadruples excitations (CCSDTQ), that CC4 is an excellent approximation to CCSDTQ for excited states with a dominant contribution from single excitations with an average deviation as small as $0.003$ eV. We next assess the accuracy of several additive basis set correction schemes, in which vertical excitation energies obtained with a compact basis set and a high-order CC method are corrected with lower-order CC calculations performed in larger basis sets. Such strategies are found to be overall very beneficial, though their accuracy depend significantly on the actual scheme. Finally, CC4 is employed to improve several theoretical best estimates of the QUEST database for molecules containing between four and six (non-hydrogen) atoms, for which previous estimates were computed at the CCSDT level., Comment: 12 pages, 1 figure (supporting information available)

Abstract

We consider a Feller diffusion (Zs, s $\ge$ 0) (with diffusion coefficient $\sqrt$ 2$\beta$ and drift $\theta$ $\in$ R) that we condition on {Zt = at}, where at is a deterministic function, and we study the limit in distribution of the conditioned process and of its genealogical tree as t $\rightarrow$ +$\infty$. When at does not increase too rapidly, we recover the standard size-biased process (and the associated genealogical tree given by the Kesten's tree). When at behaves as $\alpha$$\beta$ 2 t 2 when $\theta$ = 0 or as $\alpha$ e 2$\beta$|$\theta$|t when $\theta$ = 0, we obtain a new process whose distribution is described by a Girsanov transformation and equivalently by a SDE with a Poissonian immigration. Its associated genealogical tree is described by an infinite discrete skeleton (which does not satisfy the branching property) decorated with Brownian continuum random trees given by a Poisson point measure. As a by-product of this study, we introduce several sets of trees endowed with a Gromovtype distance which are of independent interest and which allow here to define in a formal and measurable way the decoration of a backbone with a family of continuum random trees.

16. From commission to publication : a study of Mario Castelnuovo-Tedesco's guitar repertoire informed by his correspondence with Andrés Segovia

Abstract

Graham Wade, in his article on Andrés Segovia for the Grove music Online, describes his impact on the progress and status of the guitar as a recital instrument as 'immeasurable'. Supporting the adjective with the statement: 'He revitalized traditional playing techniques and expanded the repertory by editing many transcriptions and by his massive work in inspiring composers to write new music for the instrument.' Indeed, not only did he established the guitar as a classical instrument by performing in the most renowned concert hall, but he also worked actively to develop its repertoire. Segovia's 36 years collaboration with the Italian/American composer Mario Castelnuovo-Tedesco resulted in 35 works featuring the guitar. For the first time, both sides of the correspondences between Andrés Segovia and a composer are gathered. They offer a significant insight on the role of Andres Segovia in the development of the guitar repertoire through his collaboration with Castelnuovo-Tedesco. The letters reveal previously unknow biographical information which, in several cases, correct misinformation in the Segovia literature. Furthermore, the collection of letters represents an unprecedented source of information for performers. The numerous details about Castelnuovo's guitar works revealed in the letters contribute to a new understanding of the repertoire, including the inspiration behind numerous composition elements. The editing and publication of Castelnuovo's repertoire is, also, discussed in detail within the letters. Editions and publication of the 'Segovia repertoire' has been and is still one of the main topics of debate in the classical guitar world. This thesis answers numerous questions on the role of Segovia in the composition, editing, and publishing process of Castelnuovo's guitar repertoire. It is, therefore, an important read for any researcher on 20th century guitar repertoire as well as for every guitarist who want to perform Mario Castelnuovo-Tedesco's music.

Abstract

In this contribution we are interested in proving that a given observation-driven model is identifiable. In the case of a GARCH(p, q) model, a simple sufficient condition has been established in [1] for showing the consistency of the quasi-maximum likelihood estimator. It turns out that this condition applies for a much larger class of observation-driven models, that we call the class of linearly observation-driven models. This class includes standard integer valued observation-driven time series, such as the log-linear Poisson GARCH or the NBIN-GARCH models.

L'ARTICLE 23 DE la Charte canadienne des droits et libertés garantit le droit à l'instruction dans la langue des minorités de langues officielles. Avec son objet préventif, réparateur et unificateur, l'article 23 contribue à assurer la sécurité linguistique des communautés de langues officielles minoritaires. Pourtant, à l'heure actuelle, la pénurie d'enseignantes et d'enseignants francophones au Canada met en péril la capacité des provinces de réaliser l'objectif constitutionnel de l'article 23. La reconnaissance d'un droit à l'éducation postsecondaire en vertu de l'article 23 de la Charte constitue une avenue réparatrice face à cette réalité. Les auteurs proposent certains prolégomènes à la reconnaissance de droits constitutionnels en lien avec l'éducation et la gestion postsecondaires découlant de l'article 23 de la Charte. Ils mettent en exergue certains principes jurisprudentiels qui permettraient de défendre une telle extension du libellé de l'article 23 de la Charte. La première partie de cet article soutient qu'il est possible de conclure à l'existence de tels droits au regard de canons ordinaires d'interprétation législative et constitutionnelle. La deuxième partie présente des pistes de réflexion quant au droit de gestion et de contrôle implicite dans le contexte postsecondaire qui pourrait découler d'une telle reconnaissance. SECTION 23 OF the Canadian Charter of Rights and Freedoms guarantees the right to minority language education. With its preventative, remedial, and unifying purposes, section 23 contributes to the language security of official language minority communities. However, at present there is a shortage of Francophone teachers in Canada that jeopardizes the ability of provinces to fulfill the constitutional goal of section 23. The recognition of a right to post-secondary education under section 23 of the Charter constitutes a remedial approach to this reality. The authors propose some prolegomena to the recognition of constitutional rights in relation to post-secondary education and management under section 23 of the Charter. They highlight some jurisprudential principles that would potentially support such an extension of the text of section 23 of the Charter. The first part of this article argues that it is possible to conclude that such rights exist using the ordinary canons of statutory and constitutional interpretation. Abstract

We revisit the problem of integer factorization with number-theoretic oracles, including a well-known problem: can we factor an integer $N$ unconditionally, in deterministic polynomial time, given the value of the Euler totient $$\Phi$(N)$? We show that this can be done, under certain size conditions on the prime factors of N. The key technique is lattice basis reduction using the LLL algorithm. Among our results, we show for example that if $N$ is a squarefree integer with a prime factor $p > $\sqrt$ N$ , then we can recover p in deterministic polynomial time given $$\Phi$(N)$. We also shed some light on the analogous problems for Carmichael's function, and the order oracle that is used in Shor's quantum factoring algorithm.

Abstract

Dark current degradation, origins, and annealing behavior after x-ray irradiation are studied in a P-type, hole collecting, backside-illuminated image sensor currently being developed at STMicroelectronics and based on deep-trenched photo-MOS pixels. Different biasing conditions during irradiation, i.e. grounded or biased and sequenced, are compared. The dark current increase with total ionizing dose (TID) and the dark current annealing behavior seem to be driven by the backside interface between the P-epitaxy of the pixels and the ONO stack. Despite still being under development, this pixel architecture already exhibits both very good electro-optical performance and a better radiation hardness than pinned photodiode-based CMOS Image sensors that benefit from the same advanced CIS processing technologies. At high total dose range, the photogate challenges custom Radiation-Hardened-by-Design photodiodes by exhibiting a comparable radiation tolerance while bringing new features such as high-resolution or Correlated Double Sampling.

Abstract

In this paper the Hartree equation is derived from the $N$-body Schr\''odinger equation in the mean-field limit, with convergence rate estimates that are uniform in the Planck constant $\hbar$. Specifically, we consider the two following cases:(a) T\''oplitz initial data and Lipschitz interaction forces, and (b) analytic initial data and interaction potential, over short time intervals independent of $\hbar$. The convergence rates in these two cases are $1/\sqrt{\log\log N}$ and $1/N$ respectively. The treatment of the second case is entirely self-contained and all the constants appearing in the final estimate are explicit. It provides a derivation of the Vlasov equation from the $N$-body classical dynamics using BBGKY hierarchies instead of empirical measures.

Participants were randomized 2:1 to MK-5475 or placebo via dry-powder inhaler once daily for 7 days in Part 1 (360 μg) or 28 days in Part 2 (380 μg). Safety was assessed by adverse events (AEs) and arterial blood oxygenation. Part-2 participants had pulmonary vascular resistance (PVR; primary PD endpoint) and pulmonary blood volume (PBV; secondary PD endpoint) measured at baseline and Day 28. A non-informative prior was used to calculate posterior probability (PP) that the between-group difference (MK-5475 – placebo) in mean percent reduction from baseline in PVR was less than − 15%.Results: Nine participants were randomized in Part 1, and 14 participants in Part 2. Median age of participants (86.4% male) was 68.5 years (41– 77 years); 95.5% had moderate-to-severe COPD. Abstract

A weighted automaton is functional if any two accepting runs on the same finite word have the same value. In this paper, we investigate functional weighted automata for four different measures: the sum, the mean, the discounted sum of weights along edges and the ratio between rewards and costs. On the positive side, we show that functionality is decidable for the four measures. Furthermore, the existential and universal threshold problems, the language inclusion problem and the equivalence problem are all decidable when the weighted automata are functional. On the negative side, we also study the quantitative extension of the realizability problem and show that it is undecidable for sum, mean and ratio. We finally show how to decide whether the language associated with a given functional automaton can be defined with a deterministic one, for sum, mean and discounted sum. The results on functionality and determinizability are expressed for the more general class of functional group automata. This allows one to formulate within the same framework new results related to discounted sum automata and known results on sum and mean automata.
