1. Seroprevalence of COVID-19 infection in a rural district of Tamil Nadu: A population-based seroepidemiological study

Abstract

Background: Coronavirus disease 2019 (COVID-19) was a recent global pandemic of the era which posed a great challenge for the health care in terms of preventive, diagnostic and treatment dimensions. The seroprevalence rate of COVID IgG antibodies is very crucial in estimating the susceptibility of a particular area to the viral disease. In our study, we estimated the seroprevalence of COVID-19 in a rural area. Aims and Objectives: We aimed to estimate the seroprevalence of COVID-19 in a rural district of Tamil Nadu, 6 months after the index case. Materials and Methods: We conducted a cross-sectional study of 509 adults aged more than 18 years. From all the seven Taluks, two gram panchayats (administrative cluster of 8–10 villages) were randomly selected followed by one village through convenience. The participants were invited for the study to the community-based study kiosk set up in all the eight villages through village health committees. We collected sociodemographic characteristics and symptoms using a mobile application-based questionnaire, and we tested samples for the presence of IgG antibodies for severe acute respiratory syndrome coronavirus 2 using an electro chemiluminescent immunoassay. We calculated age-gender adjusted and test performance adjusted seroprevalence. Results: The age-and gender-adjusted seroprevalence was 8.5% (95% confidence interval [CI] 6.9–10.8%). The unadjusted seroprevalence among participants with hypertension and diabetes was 16.3% (95% CI: 9.2–25.8) and 10.7% (95% CI: 5.5–18.3), respectively. When we adjusted for the test performance, the seroprevalence was 6.1% (95% CI 4.02–8.17). The study estimated 7 (95% CI 1:4.5–1:9) undetected infected individuals for every reverse transcription polymerase chain reaction confirmed case. Abstract

Background: Long-term exposure to silica dust makes people prone for silicosis which raises the risk of the individual in developing pulmonary tuberculosis (PTB). Silicosis is a progressive lung disease characterize with shortness of breath, cough, fever, and bluish skin, caused due to inhalation of crystalline silica dust found in abundance in sand, rock, and quartz. Aims and Objectives: The disease is known to effect the functioning of macrophages making it unable to defend mycobacterium species, thus allowing the bacterium to invade and develop tuberculosis. Materials and Methods: A retrospective cross-sectional study is carried out to estimate the prevalence of PTB in newly diagnosed silicosis patients at Madurai Medical College, Madurai, Tamil Nadu. All the patients who got registered in Occupational Lung Diseases Registry from January 2016 to December 2021 are included in the study. Silicosis patients tested positive on sputum CBNAAT are considered as silicotubercular patients, whose prevalence is calculated. Results: Our study included a total of 54 subjects (90.8% males and 8.2% female) with mean SD age of 38±6.7 years ranging from 26 to 53 years. Of which 22 patients, 40.7% tested positive for Mycobacterium tuberculosis (MTB) on sputum CBNAAT. The prevalence of MTB in among silicosis patients was found to be 40.7%. Conclusions: Association of TB and silicosis is very strong with the prevalence of TB among silicosis patients as 40.7%. Our study also observed the male sex and elder age as elevated risk of developing silicotuberculosis. All the people getting exposed to silica dust need to be educated in the usage and usefulness of protective equipment.

3. AI Based Solar Panel Cleaning Robot

Abstract

Among the various renewable energies, Solar energy is one of the most important sources. Solar panel is used to collect the solar energy and convert it into useful electrical energy. The dust accumulated on the solar panel reduce its efficiency to a certain degree. To overcome this problem, efficient techniques to clean the solar panel must be implemented. The proposed model is to clean the dust and bird droppings that has accumulated on the solar panel. An AI-based solar panel cleaning robot is designed which performs dry or wet cleaning based on the dirt or bird droppings, thus reducing water usage. The robot utilizes a Convolutional Neural Network model based on the Visual Geometry Group 16 architecture to detect dirt and bird droppings on solar panels. The robot is designed to perform dry cleaning using a brush and wet cleaning using a water pump, depending on the type of dirt detected. The experimental readings show that the power output of the solar panel increases significantly after cleaning, and the prototype model demonstrates the effectiveness of the robot in detecting and cleaning dirt and bird droppings. The development of such a robot has the potential to improve the efficiency of solar panels and reduce water usage in the cleaning process, making it a more sustainable and eco-friendly solution for maintaining solar panels.

4. Assessment of the Management of Ascites in Pediatrics in Jafar Ibn Auf Pediatrics Hospital: A Descriptive Retrospective Study

Abstract

Background: Ascites are pathological conditions characterized by the accumulation of fluids in the peritoneal cavity. Ascites are among the most common complications of different diseases in pediatrics, such as renal diseases, cardiac diseases, and hepatic diseases. The pediatric group is considered a vulnerable category needing special care, so assessing the management of ascites in pediatrics needs to be addressed. Aim: The study was conducted to assess ascites' management in pediatrics in Jafar Ibn Auf pediatric hospital. Methods: A descriptive retrospective study was conducted at Jafar Ibn Auf pediatric hospital, Khartoum, Sudan. All medical files of pediatrics with ascites from 1 month to 18 years old during the period between 2017-2020 were included. Data were collected from file records using a predesigned data collection sheet and analyzed using the statistical package for the social sciences (SPSS) version 23. Results: A total of 143 medical records were included in this study. The majority of the group were males. The most affected patients were within the age group of ˂ 6 years old. Grade I was the most common presentation. Hepatic diseases and gastrointestinal diseases were the main etiological factors for ascites and represented in 62.2% and 15.4%, respectively. Regarding ascites management, the most frequent method of managing ascites used was medications with liver support. Moreover, 63.2% of the patients were treated by combining spironolactone with furosemide. Out of the 143 patients, 92.3% of patients were discharged from the hospital, while the remaining were referred to other hospitals. Statistically, there was a strong association between the grade of ascites, management of ascites, and the patient's discharge (P = 0.001). Conclusion: The results showed that liver diseases were the most common etiological cause of ascites in pediatrics. The majority of patients were treated properly managed in term of dose, and the combination of spironolactone and furosemide were the most frequently used medications in this study. The proper diagnosis and appropriate management of ascites is critical to prevent related morbidity and mortality in pediatrics.

Abstract

Stevens–Johnson syndrome (SJS) is a rare immune-mediated severe cutaneous adverse reaction with an incidence rate of 0.05–2 persons/ million population/month. Drugs are the most commonly implicated in 95% of cases. In our report, a 52-year-old male patient presented with chief complaints of skin rashes over the body and was having a history of using a tab. ofloxacin for gastroenteritis. The severity of SJS was assessed using SCORTEN (=1). The drug can be considered as a probable/likely cause of adverse drug reaction as per causality assessment of the suspected adverse drug reactions. Early diagnosis helps the clinician to elude secondary infection and subsequent complications. It highlights the mandatory reporting of the offending drug and the necessity of pharmacovigilance in different countries.

Abstract

A new classification framework for a Clinical Decision Support System, utilizing a Genetic algorithm and an Artificial Flora Optimized Neural Network is presented in this paper. GAFON is an artificial neural network whose topology is optimized with Genetic Algorithm and the learnable parameters are optimized with Artificial Flora Optimization algorithm. Drop out technique is used in the topology optimization phase and weight regularization is used in the parameter optimization phase. The proposed method minimizes the co-adaptation problem, reduces over-fitting of training data and improves the generalization of a feed forward neural network. The classification framework developed has been tested for classifying both multi class and binary class clinical datasets. The proposed method attained accuracy values of 86.82% for Hepatitis C Virus (HCV) for Egyptian patients, 84.91% for Vertebral Column 95.65% for Statlog Heart Disease (SHD), SHD and 93.79% for Early Stage Diabetes Risk Prediction (ESDRP), all datasets obtained from UCI repository

7. Reliability of Typhidot-M in diagnosis of typhoid fever in children

Abstract

Background: Typhoid fever continues to be endemic and a major cause of mortality and morbidity in the developing countries. Many times, it presents a diagnostic challenge to treating physicians due to overlapping clinical features with other common causes of acute fever. A rapid and reliable laboratory test would be of great help to clinicians for early diagnosis and appropriate treatment of typhoid fever. Aims and Objectives: The present study was conducted to assess the reliability of Typhidot-M in diagnosis of typhoid fever. Materials and Methods: A total of 203 children with clinical features consistent with typhoid fever were enrolled in the study. Blood culture, Typhidot-M, and Widal test were performed. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated for Typhidot-M and Widal test in comparison with blood culture as reference standard. Results: Out of 203 patients, 30 (14.8%) were blood culture positive. One hundred and six patients were Typhidot-M positive and 97 were negative. Typhidot-M had a sensitivity of 93.3%, specificity of 54.9%, and PPV and NPV of 26.4% and 97.9%, respectively. Conclusion: Typhidot-M is a sensitive test for early diagnosis of typhoid fever. However due to low specificity, positive results should be correlated with clinical picture and other possible diagnoses.

Abstract

Background: Drug utilization studies helps in the understanding of drug usage among patients, as well as the socioeconomic background and rational use of antiepileptics in children with epilepsy. Objective: The primary aim of this article is to examine the justification for utilizing sodium valproate and Levetiracetam as monotherapy in pediatric epilepsy patients, as well as drug use and prescribing patterns for various kinds of epilepsy. Materials & Methods: Pediatric epilepsy patients under the age of 18 who visited the outpatient department, patients who were on sodium valproate, carbamazepine, or Levetiracetam monotherapy for at least 3 months after starting treatment. Demographic details, disease characteristics like type of epilepsy, onset and duration of episode, details of first attack, last attack, frequency and family history related to epilepsy comorbid conditions. Details on the treatment, such as the drug prescribed, the dose, the duration of the treatment, and the dosage, were noted. Magnetic resonance image (MRI) Brain, CT Brain, Electroencephalogram (EEG) was done in selected patients and noted. Results: In this study we have total of 91 patients younger than 18 years of age confirmed with epilepsy receiving Sodium valproate, Levetiracetam as Monotherapy. Among them 70.3% were male and 29.6% were females. As per the recent Kuppuswamy scale 64.8% of the patients were from an urban background, whereas 35.1% were from a rural background. On analyzing socioeconomic status we found that 38.5% belong to upper middle class family. Nearly 26 % of the patients were diagnosed with focal seizures and 25% with GTCS. Lesser percentage observed with Rolandic, myoclonic, complex partial, refractory epilepsy. Majority 53.8 % were on Sodium valproate among older AEDs. Among newer AEDs 29.6% of patients received Levetiracetam as monotherapy. Key words: Epilepsy, Sodium valproate, Levetiracetam, Monotherapy, Rational use

The main objective of this project is to determine the microbes present in the hand and intimate to the public and make awareness in the prevention of covid-19 disease by encouraging proper sanitizing of their hands. In our project, we are determining the presence of microbes in hand by using a temperature sensor, gas sensor, and color sensor. All the above-mentioned sensors are connected to the microcontroller unit and the signals obtained from the sensors are further processed and decoded to determine the microbes’ intensity present in the hand. Once our system identifies the microbes present in the hand, it will automatically dispense the sanitizer spray and it will intimate the presence of microbes in the hand to the public through a 16x2 LCD display

Abstract

Artificial neural networks (ANN) are widely used for classification, and the training algorithm commonly used is the backpropagation (BP) algorithm. The major bottleneck faced in the backpropagation neural network training is in fixing the appropriate values for network parameters. The network parameters are initial weights, biases, activation function, number of hidden layers and the number of neurons per hidden layer, number of training epochs, learning rate, minimum error, and momentum term for the classification task. The objective of this work is to investigate the performance of 12 different BP algorithms with the impact of variations in network parameter values for the neural network training. The algorithms were evaluated with different training and testing samples taken from the three benchmark clinical datasets, namely, Pima Indian Diabetes (PID), Hepatitis, and Wisconsin Breast Cancer (WBC) dataset obtained from the University of California Irvine (UCI) machine learning repository.

Abstract

This project aims to develop a smart energy meter which has many advantages like monitoring and controlling the energy meter at anywhere and at any place. Our project also called as pay first and then use it because here the user first has to pay the bill after that only they can consume the electricity. We know that each user has different kind of load demands and based on it we can recharge in number of units (KWh). The energy we are consuming and based on those pulses from the energy meter, microcontroller decrements the number of units and displays on LCD the remaining units we can use. The load gets disconnected when the units become zero and it is reconnected when the user recharges it. Our project will be very useful for the electricity board and also the user because this project gives the live update about the energy usage of the user through the message. GSM (Global system for mobile communication) will help to send and receive this information to the user. This system helps us to identify the energy losses of the electricity board to a large extent.

Abstract

In this modern era, parents are busy building their lifestyle, carrier etc. As we know it has now become hard because parents have to take care of their children simultaneously, which paves a lot of work pressure and family pressure especially for women. Health of the child is affected and better care has also reduced. So, in order to handle such situation, we use temperature, Humidity, ultra-sonic Sensor. The conditions of the external atmosphere help to detect increased body temperature, babies voice while crying and their movements while they are continuously moving and also indicates the time for the diaper to be changed. If there are any abnormal activities are observed in baby’s atmosphere. An alert message is sent to the parents. In this system a video camera is attached and operated under the microcontroller’s instruction and it records a video when the motion sensor detects any continuous movements. The recorded video is broadcasted in a display to the parents which helps them to monitor baby in live. And in addition, this system detects and displays the status of the infant and alert the respective guardian by collecting values from sensor like temperature sensor, ultrasonic sensor, and also the location value from raspberry pi.

19. Optimization and characterization of green synthesized silver nanoparticles and its inhibitory activity against biofilm forming bacterial pathogens

Abstract

In the present study, antibacterial activity of silver nanoparticles synthesized from entophytic fungi extracts were analysed against biofilm forming bacteria include the Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Pseudomonas aeruginosa isolated from clinical specimens. Therefore, we tried to establish a combination of medicinal and nanotechnology possibly with the field of medicine for the development of antibacterial agents against these strains. The nanoparticles were characterized by UV–visible spectroscopy, the presented and capped molecules of proteins with nanoparticles was confirmed by Fourier transform infrared spectroscopy (FTIR), X-ray diffraction (XRD). UV–visible studies display an absorption band at 420 nm due to surface Plasmon resonance (SPR) of the silver nanoparticles. The intra cellular synthesized silver nanoparticles (Ag NPs) size 15-35 nm was determination of X-ray diffraction (XRD) crystal calculation, antimicrobial activity higher than the standard antibiotic. The nanoparticle treated with isolated bacteria as result macromolecule of proteins denatures and oozing out the cells and measure the Bradford method. The nanoparticle treated bacterial cells were compared to negative control more amount of protein released. The nano size establishing the silver nanoparticles directly bind with DNA of the pathogenic bacterial strains most important to higher antimicrobial activity.

20. Synthesis and application of new composite terpolymer for the removal of lead from aqueous solution

Abstract

In this study, a new composite GO-4-HBDTOF was prepared by coating 4-HBDTOF terpolymer on graphene oxide. The terpolymer and composite were characterized by Fourier Transform Infrared Spectroscopy and Scanning Electron Microscopy analysis. Batch experiments were carried out to evaluate efficiency of the sorbents 4-HBDTOF and GO-4-HBDTOF composite for the removal of Pb(II) ion from aqueous solutions. The effect of contact time, pH and initial concentration of Pb(II) on the adsorption process were investigated. The equilibrium data were fitted to Langmuir and Freundlich isotherm model. The Langmuir isotherm gave a better representation of data than Freundlich isotherm. From Langmuir adsorption isotherm model the calculated value of monolayer adsorption capacity for composite was found to be higher than that of terpolymer. Thus the newly synthesized terpolymer composite can be used as better adsorbent for the removal of Pb(II) from aqueous solution.

22. Snoring and Upper Airway Resistance Syndrome

Abstract

Snoring is a common phenomenon and results from vibration of the soft tissue structures in the upper airway. Snoring may occur alone or as a sign of upper airway resistance syndrome (UARS) or obstructive sleep apnea syndrome (OSA). Snoring carries social consequences and has been linked to increased risk for cardiovascular disease, although this relationship may be confounded by undiagnosed OSA. UARS is characterized by daytime dysfunction and repetitive respiratory effort-related arousals, episodes of increasingly intense breathing efforts that terminate with an arousal from sleep before a hypopnea (episode of partial airflow reduction) or apnea (episode of complete airflow reduction) occur. There is controversy whether UARS is a distinct clinical entity or part of the OSA spectrum. Long-term cardiovascular consequences of UARS are unclear. Risk factors for snoring and UARS are similar and include obesity, nasal congestion, ingestion of pharyngeal relaxing substances before bed such as alcohol, hypnotics, and skeletal muscle relaxants, and abnormalities of the bony and soft tissue structures of the head and neck. Treatment options for snoring and UARS overlap and include risk factor modifications, oral appliances to advance the mandible during sleep, continuous positive airway pressure, and upper airway surgeries.

23. Primary Snoring and Upper Airway Resistance Syndrome

Abstract

Snoring is the sound of soft tissues vibrating within the upper throat during sleep. The vibration is a result of turbulent air flow through the throat as it narrows during sleep. Snoring is very common in adults and may occur alone or be a feature of other conditions characterized by greater throat narrowing during sleep, namely upper airway resistance syndrome (UARS) and obstructive sleep apnea syndrome (OSA). UARS is marked by daytime sleepiness and/or fatigue in the context of frequent respiratory-effort related arousals (RERAs). RERAs are episodic arousals that follow periods of more intense breathing effort as resistance to airflow in the throat increases. Once the throat is very narrow, hypopneas (partial reduction in airflow) and apneas (complete cessation of airflow) occur, and these events comprise OSA. Whether UARS is an entity distinct from OSA remains still debated. Risk factors for snoring and UARS are similar and include obesity, nasal congestion, alcohol and drug consumption before bed, and abnormalities of the bony and soft tissue structures of the head and neck. Snoring can be a nuisance, but generally does not produce daytime sequelae, while UARS may lead to daytime fatigue and sleepiness. Treatment options for snoring and UARS include risk factor modifications, nasal-directed therapies, oral appliances, upper airway surgery, while continuous positive airway pressure (CPAP) is an additional option for UARS.

We prove that every topologically transitive map f on the real line must satisfy the following properties: (1) The set C of critical points is unbounded. (2)The set f(C) of critical values is also unbounded. (3)Apart from the empty set and the whole set, there can be at most one open invariant set. (4)With a single possible exception, for every element x the backward orbit {y∈R:fn(y)=x for some n in N} is dense in R.
