This paper analyzes the role of families facing the disruption caused by the shutdown of school facilities due to the health crisis caused by the SARS - C o V -2 virus, based on the premise that the reflection of conflicts and the debate about how to deal with them allows us to attain and build institutional learning. It shares the findings of the research “Reception, experiences and practice of the 'Learn at Home strategy' during the emergency of COVID -19. A study from heterogeneity”, whose purpose was to describe the actions carried out in adapting schools to teaching in an emergency, from the point of view of the actors and intended to build knowledge from the analysis of this crisis, thinking about its implications and acknowledging potential learnings for the daily life of the schools. 12. Effectiveness of a hybrid closed-loop system for children and adolescents with type 1 diabetes during physical exercise: A cross-sectional study in real life

Objective: The aim of the study was to describe how physical exercise affects metabolic control, insulin requirements and carbohydrate intake in children who use hybrid closed-loop systems. Methods: Cross-sectional study design. The sample included 21 children and adolescents diagnosed with type 1 diabetes. During the study, participants were monitored for a period of 7 days to gather comprehensive data on these factors. Results: Nine participants (42.9%) had switched to exercise mode to raise the target glucose temporarily to 150 mg/dL. The HbA1c values ranged from 5.5% to 7.9% (median, 6.5%; IQR, 0.75). The percentage of time within the target range of 70−180 mg/dL was similar; however, there was an increased duration of hyperglycaemia and more autocorrections on exercise days. The time spent in severe hyperglycaemia (>250 mg/dL) increased by 2.7% in exercise compared to non-exercise days (P = .02). It is worth noting that hypoglycaemic episodes did not increase during the exercise days compared with non-exercise days. Conclusion: The hybrid closed-loop system was effective and safe in children and adolescents with type 1 diabetes during the performance of competitive sports in real life. Resumen: Objetivo: El objetivo de este estudio es describir cómo el ejercicio físico afecta el control metabólico, las necesidades de insulina y el consumo de carbohidratos en niños usuarios de un sistema híbrido de circuito cerrado. Métodos: Diseño de estudio transversal. Se reclutaron veintiún niños y adolescentes diagnosticados con diabetes tipo 1. Durante el estudio, los participantes fueron monitoreados durante un período de 7 días para recopilar datos completos sobre estos factores. Resultados: Nueve participantes (42,9%) habían activado el modo de ejercicio, para elevar temporalmente la glucosa objetivo a 150 mg/dL. La HbA1c osciló entre 5,5% y 7,9% (mediana 6,5%, IQR 0,75). El porcentaje de tiempo dentro del rango de 70−180 mg/dL fue similar; sin embargo, hubo una mayor duración de la hiperglucemia y más autocorrecciones en los días en que se realizó ejercicio. El porcentaje de tiempo en hiperglucemia grave (>250 mg/dL) fue 2,7 mayor durante el ejercicio versus los días sin ejercicio (p = 0,02). En particular, los episodios de hipoglucemia no aumentaron durante los días de práctica de ejercicio, en comparación con los días sin ejercicio. Conclusiones: El sistema híbrido de circuito cerrado fue efectivo y seguro en niños y adolescentes diagnosticados con diabetes tipo 1 durante ejercicios físicos competitivos en la vida real.

13. Anterior approach for pediatric supracondylar humerus fractures: A systematic review

Background and purpose: Open reduction is rarely performed in pediatric supracondylar humerus fractures. However, clear evidence is lacking regarding the optimal open approach to achieve satisfactory results. The anterior approach provides direct visualization of the fracture and excellent exposure to neurovascular structures, although its utilization is less common. The objective of this study was to review the indications, outcomes, and complications associated with the anterior approach for open reduction of these fractures. Methods: Our protocol was registered at PROSPERO: CRD42023446923. MEDLINE/PubMed, Embase, Web of Science, Clinicaltrials.gov, and Cochrane Library were searched from database inception to search date (December 2023) and screened in duplicate for relevant studies. Data were collected regarding patient demographics, indications for open reduction, Flynn's functional and cosmetic outcomes, and complications. Study quality was assessed using the Methodological Index for Non-Randomized Studies Criteria. Results: A total of 19 studies involving 483 patients were included. One study was classified as Level 2 evidence, ten as Level 3, and eight as Level 4. The mean MINORS score was 13.05 ± 3.47. The primary indication for open reduction was failed closed reduction, observed in 46% of patients. 97.7% and 98.6% of patients achieved Flynn's functional and cosmetic satisfactory results, respectively. The postsurgical neurovascular injury rate was 1.4%. One patient required reintervention. Conclusion: The anterior approach is safe and effective for managing pediatric supracondylar humerus fractures requiring open reduction. Level of evidence: Systematic review of Level 2–4 evidence studies. Resumen: Antecedentes y objetivo: La reducción abierta rara vez es necesaria en las fracturas supracondíleas de húmero pediátrica. Sin embargo, falta evidencia sobre cuál es el abordaje óptimo para obtener resultados satisfactorios. El abordaje anterior facilita una visión directa de la fractura y una excelente exposición de las estructuras neurovasculares, aunque su uso es poco común. El objetivo de este estudio fue revisar las indicaciones, resultados y complicaciones asociadas con el abordaje anterior para la reducción abierta de estas fracturas. Métodos: Nuestro protocolo se registró en PROSPERO: CRD42023446923. La búsqueda se realizó en MEDLINE/PubMed, Embase, Web of Science, Clinicaltrials.gov y Cochrane Library, desde inicios hasta diciembre de 2023. Se realizó screening por duplicado. Se recogieron datos demográficos, indicaciones para el abordaje abierto, puntuación de Flynn funcional y estética y complicaciones. La calidad de los estudios se evaluó con Methodological Index for Non-Randomized Studies Criteria (MINORS). Resultados: Se incluyeron 19 estudios que agrupaban a 483 pacientes. Un estudio se clasificó con nivel de evidencia 2, 10 con nivel 3, y 8 con nivel 4. La puntuación media MINORS fue 13,05 ± 3,47. La indicación principal para el abordaje abierto fue el fracaso de la reducción cerrada, reportada en 46% de pacientes. El 97,7% y 98,6% de los pacientes lograron un Flynn funcional y cosmético satisfactorio, respectivamente. La tasa de lesiones neurovasculares postquirúrgicas fue 1,4%. Un paciente fue reintervenido. Conclusiones: El abordaje anterior es seguro y efectivo en el maneco de fracturas supracondíleas de húmero pediátricas cuando se requiera un abordaje abierto. Nivel de evidencia: Revisión sistemática de estudios con nivel de evidencia 2-4.

The management of urinary tract infection (UTI) in infants and children has changed significantly over the past few decades based on scientific evidence that questioned the efficacy of strategies used to prevent kidney injury and subsequent progression to chronic kidney disease, which is very unlikely in most paediatric cases. However, there is still substantial heterogeneity in its management and uncertainty regarding the diagnosis, indication of imaging tests, treatment or follow-up in these patients. The Spanish clinical practice guideline has been updated through the review of the literature published since 2009 and a rigorous evaluation of current clinical practice aspects, taking into account the evidence on the benefits of each intervention in addition to its risks and drawbacks to attempt to provide more precise recommendations. Resumen: En las últimas décadas, se han producido profundos cambios en el manejo de la infección urinaria en pediatría, a partir de estudios que cuestionaban la eficacia de las estrategias para prevenir el daño renal y la evolución posterior a enfermedad renal crónica, muy poco probable en la mayoría de casos. No obstante, todavía existe una gran variabilidad clínica en el manejo e incertidumbre en cuestiones relativas al diagnóstico, la indicación de una exploración radiológica, el tratamiento o el seguimiento de estos niños. La actualización de la guía de práctica clínica española ha examinado en la literatura publicada desde el año 2009 y de modo riguroso aspectos de la práctica actual, considerando la evidencia de los beneficios de cada intervención, junto con los riesgos asociados y desventajas, para intentar delimitar las pautas de actuación más precisas.

Introduction: Esophageal pH-impedance monitoring is a tool for diagnosing gastroesophageal reflux in children. The position of the pH catheter is essential for a reliable reading and the current formulas for calculating catheter insertion length are not completely accurate. The aim of the present study was to develop a new formula for adequate insertion of the pH catheter. Material and methods: A cross-sectional study was conducted on children that underwent pH-impedance monitoring and later radiographic control, to calculate the correct catheter insertion length. The documented variables were age, sex, weight, height, naris to tragus distance, tragus to sternal notch distance, sternal notch to xiphoid process distance, and initial insertion length determined by the Strobel and height interval formulas. A multivariate regression analysis was carried out to predict the final insertion length. Regression ANOVA and Pearson’s adjusted R-squared tests were performed. Results: Forty-five pH-impedance studies were carried out, 53% of which were in males. The age and weight variables were not normally distributed. In the initial regression model, the variables that did not significantly correlate with the final insertion length were: sex (P 0.124), length determined by the Strobel or height interval formulas (P 0.078), naris to tragus distance (P 0.905), and tragus to sternal notch distance (P 0.404). The final equation: 5.6 + (height in cm * 0.12) + (sternal notch to xiphoid process distance * 0.57) produced an R2 of 0.93 (P 0.000). Conclusions: This formula can be considered a valid option for placement of the pH-impedance monitoring catheter in pediatrics. Resumen: Introducción: La pH-impedanciometría esofágica es una herramienta para el diagnóstico de reflujo gastroesofágico en niños, la posición del sensor de pH es crucial para una lectura confiable, las fórmulas existentes para el cálculo de la inserción del catéter no son tan precisas; el objetivo de este estudio fue desarrollar una nueva fórmula para la adecuada inserción de dicho catéter. Material y métodos: Estudio transversal, de niños a los que se realizó pH-impedanciometría y control radiográfico posterior para estimar la longitud de distancia de inserción correcta del catéter; se registraron las variables de: edad, sexo, peso, talla, distancia narina-tragus, tragus-horquilla esternal y horquilla-xifoides así como la longitud inicial de inserción determinada por fórmulas de Strobel e Intervalos de talla. Se realizó un análisis de regresión multivariada para predecir la longitud de inserción final y se obtuvieron ANOVA de la regresión y R cuadrado ajustado de Pearson. Resultados: Se realizaron 45 pH-impedanciometrías, 53% fueron masculinos. Las variables edad y peso no mostraron distribución normal. En el modelo de regresión inicial las variables sin correlación significativa con la longitud de inserción final fueron: sexo (p 0,124), longitud determinada por Strobel o intervalos de talla (p 0,078), distancia narina-tragus (p 0,905) y distancia tragus-horquilla (p 0,404). La ecuación final: 5,6 + (talla en cm * 0,12) + (distancia horquilla-xifoides * 0,57) alcanza un R2 de 0,93 (p 0,000). Conclusiones: Esta fórmula puede ser considerada como una opción válida para la colocación del catéter de pH-impedanciometría en pediatría.

El uso de tecnologías de la información y comunicación, se convierten en apoyo del aprendizaje de los niños con trastorno del espectro autista, siendo una herramienta poderosa y versátil; estas tecnologías ofrecen una amplia gama de recursos que pueden adaptarse a las necesidades individuales de cada niño, facilitando así un enfoque más personalizado y centrado en el proceso educativo. El objetivo de esta investigación fue analizar la gestión del aprendizaje con tecnología para niños en el espectro autista. Se llevó a cabo utilizando una metodología de Revisión Sistemática de Literatura (PRISMA-ScR) a través de una recopilación rigurosa y sistemática de información relevante. Los principales hallazgos dan cuenta del uso de las tecnologías de información y comunicación como soporte en el aprendizaje de niños con trastorno del espectro autista; igualmente, se produce mejoras en la comunicación y el lenguaje a través de aplicaciones de comunicación, facilitando el desarrollo de habilidades sociales con juegos interactivos y aplicaciones de simulación, que promueven la inclusión y participación en entornos educativos y sociales. Se concluye que la integración de tecnología en la gestión del aprendizaje para niños en el espectro autista, ofrece un vasto potencial para personalizar la educación y mejorar su experiencia de aprendizaje

Introduction and Objective: Hearing loss is the most common sensory deficiency at birth, and its association with vestibular dysfunction in children has been well-established. The primary aim of our study is to investigate the relationship between these two sensory deficits, their overall impact on children's development, and to establish a simple and effective screening protocol for clinical practice. Method: We conducted a comprehensive narrative literature review. Results: Our review includes a total of 41 publications focused on neurosensorial hearing loss and vestibular dysfunction in the paediatric population. Discussion: Our findings indicate that children with more significant hearing loss are at a higher risk of experiencing vestibular dysfunction. Certain specific conditions, such as congenital cytomegalovirus, inner ear malformations, auditory neuropathy spectrum disorders, sudden hearing loss, ototoxicity, and cochlear implants, are more frequently associated with both hearing loss and balance alterations. To ensure accurate diagnosis and referral to the diagnostic protocol, the evaluation of vestibular dysfunction must consider the patient's age and include assessments of motor development and balance. For young children, vestibular-evoked myogenic potentials and the head impulse test are the most well-tolerated tests. Conclusions: Early recognition of signs indicating vestibular dysfunction in children with neurosensorial hearing loss is crucial. Introduction: Vestibular rehabilitation has proven to be an effective therapeutic approach for addressing vestibular system disorders in adults. However, concerning pediatric cases there is little research compared to the extensive studies conducted with adults. Objective: To update the most relevant medical literature in relation to vestibular rehabilitation in children. Material and method: We have conducted a narrative revision of the most recent and relevant bibliography regarding this topic through the main search engines (OVID, EBSCO, JBI and PubMed). Results: Based mainly on the work of Rose Marie Rine, one of the physiotherapists who has done more research in this area, as well as other authors, the most significant results and their practical implications are described. Discussion: The distinctive aspects of VR in children in relation to adults and the diagnostic tools available for its assessment are described. 32. Nighttime Fears and Coping Responses in School-Aged Children: A Latent Profile Analysis.

Background: Nighttime fears are common among school-aged children and may be linked to psychological difficulties, while coping strategies may vary and affect their emotional well-being. This study aimed to identify patterns of nighttime fears and coping styles in school-aged children using Latent Profile Analysis. Subsequently, possible predictors of the latent profiles were tested through multinomial logistic regression analysis. Method: A sample of 786 Spanish-speaking children (aged 8 to 12; Mage = 9.66, SD = 1.20; 51.3% girls) participated and completed self-report measures of nighttime fears and coping responses. Results: The results revealed four latent profiles: (1)"low fears, low self-control" (24.2%), (2)"high fears (except for imaginary fears), maladaptive coping" (22.8%), (3)"moderate fears, adaptive coping" (44.9%), and (4)"high fears, maladaptive coping" (8.1%). The regression analysis revealed that sociodemographic characteristics and children's perceived helpfulness of coping strategies were predictors of the latent profiles. Conclusions: These findings suggest the presence of distinct patterns and heterogeneous subgroups, which may emerge as higher or lower risk profiles. Objective. To analyze the percentage trends of acute diarrheal disease (ADD) and acute respiratory infections (ARI) and their associated factors in children under five years old in Mexico. Materials and methods. The ADD and ARI percentages were estimated in the two weeks prior to the interview, based on the Encuesta Nacional de Salud y Nutrición Continua 2021-2023 (Ensanut Continua 2021-2023), and compared with those reported in the Ensanut 2006, 2012 and 2018. There were identified, by multiple logistic regression, factors associated with ADD and ARI in 2021-2023. Results. Percentage of 7.8% of ADD and 21.3% of ARI were estimated in 2021-2023, lower than previous years. Presenting ADD was associated with being one or two years old vs. four years; residing in the North Pacific, Central Pacific, North Central, Mexico City, State of Mexico, South Pacific and Peninsula regions vs. Border; and that a woman is the head of the home. Presenting ARI was associated with households with a low level of well-being vs. high level; in the North Pacific, Border, North Central, South Pacific and Peninsula regions vs. the State of Mexico. Conclusions. It is recommended to strengthen the capacities of health authorities at different levels, with civil society, to prevent ADD and ARI, and ensure access to drinking water, with a gender perspective. Objective. To compare vaccination coverage and associated factors in children under five years in Mexico, according to data from the 2021-2023 Continuous National Health and Nutrition Survey (2021-2023 Continuous Ensanut) and the 2012 National Health and Nutrition Survey (2012 Ensanut). Materials and methods. Prevalence analysis of the vaccination status of Mexican children was estimated from data recorded in the vaccination record or any other probatory document obtained from either survey. If the administration of the immunogen complied with the recommendations of time intervals between doses and minimum and maximum age of administration recommended by the National Center for Child and Adolescent Health, the doses were considered valid, and the participant was included in the analyses. Results. In children under five, 2021-2023, Continuous Ensanut vaccination coverage of BCG, hepatitis-B, polio/DPT, antipneumococcal, anti-rotavirus, and triple viral vaccines was 86.5, 63.7, 68.9, 86.6, 81.3, and 71.3%, respectively. At one and two years of age, 45.6 and 32.4% had received the complete scheme, respectively. From 2012 to 2021-2023, vaccination coverage with BCG, hepatitis B, and polio/DPT decreased, and those of pneumococcus and rotavirus increased. Conclusion. In 2021-2023, the vaccination coverage indicator (95%) was not reached for any vaccine investigated. [ABSTRACT FROM AUTHOR]

The current compounded economic crisis in Lebanon has increased poverty rates and widened the gap between families of different socioeconomic classes. Little is known about the impact of advertising, particularly smartphone advertising, on the self‐esteem and materialism levels of children in developing countries like Lebanon. This study aimed to measure the differences among children coming from lower and upper socioeconomic classes in the following: smartphone advertising exposure, materialism and self‐esteem, vulnerability to the effects of smartphone advertising exposure on materialism, and vulnerability to the effects of materialism on self‐esteem. It was found that advertising affected negatively the self‐esteem of the children coming from lower social classes, including the "newly poor," because in the current economic situation, children who are more exposed to advertising are more likely to believe in advertising, have lower self‐satisfaction, and believe themselves to be inferior for not owning the various products that are advertised, which lead them to become more materialistic than children from wealthier families. On the other hand, advertising had a reinforcing effect on materialism among more affluent children. In addition, and contrary to the expected, children from higher social classes, and not children from deprived households, were more susceptible to the impact of advertising on materialism. These findings enlighten us on the significant role that today's advertising plays in societies with wide socioeconomic disparities. [ABSTRACT FROM AUTHOR]

37. Limitations and complications of robotic urological surgery in younger children: debunking old beliefs.

Introduction. Even though certain technical limitations associated with the small size of the patients were taken for granted in the advent of pediatric robotic surgery, we could now be facing a paradigm shift challenging these old beliefs. Materials and methods. A retrospective study of patients undergoing Da-Vinci-Xi(IS4000)-assisted urological surgery from May 2022 to October 2023 was carried out. Patients were divided into two groups –Group A < 15 kg and Group B ≥ 15 kg. Operating times, hospital stay, and intra- and postoperative complications were compared. Results. 17 patients (9 in Group A, 8 in Group B) underwent surgery. Median age was 29 months (A) and 109 months (B) (p< 0.001). Median weight was 12.0 kg (A) and 31.5 kg (p< 0.001). Operating time was 162 min (A) and 130 min (p= 0.203). Console time was 99 min (A) and 70 min (B) (p= 0.065). Mean hospital stay was 2 days (A) and 3 days (B) (p= 0.41). No differences were found in terms of intraoperative (p= 0.453) or postoperative (p= 0.485) complications. Conclusion. Even though operating on younger children seemed more complicated than on older ones in the advent of robotic surgery, the results in our series were similar. El diseño de personajes como embajadores de mensaje y la creación de escenografías atractivas en los videojuegos educativos, son esenciales para garantizar su efectividad de uso. Este estudio se enfoca en la educación musical infantil apoyada en la implementación de una serie de videojuegos llamada MIDI-Musical. Metodologías cualitativas como Design Thinking, consideradas como aspectos claves en investigación aplicada al diseño, respaldan la creación de sus personajes, arquetipos y escenarios, garantizando producciones bien informadas. Con base en los resultados, se busca fortalecer la conexión entre aprendizaje y diversión dentro del contexto educativo musical, potenciando nuevas investigaciones relacionadas con videojuegos serios infantiles.

Introducción: La patología eruptiva puede alterar la cronología de la erupción encontrándonos con erupción precoz y tardía. Estas patologías de erupción pueden asociarse a factores locales o sistémicos. Objetivos: Identificar las alteraciones en la cronología de erupción y los factores asociados, analizando los diferentes grupos dentarios. Material y métodos: Los datos fueron recogidos de 218 pacientes entre 6 y 14 años vistos en la facultad de Odontología de la Universidad de Sevilla desde 2019 hasta 2024. Se evaluaron historias clínicas, ortopantomografías de manera digital y los datos fueron analizados estadísticamente. Resultados: No se encontró asociación significativa entre las alteraciones de la erupción con los grupos de edad ni sexo.
