2. Detección de células tumorales circulantes en mujeres con cáncer de mama temprano.

- Abstract
Background: There are multiple methods and biomarkers with different detection rates to circulating tumor cells (CTCs) in women with breast cancer (BC). However, to the best our knowledge, no study has been carried out to detect CTCs in women with early BC in Mexico. Objective: Calculate the detection rate of CTCs in peripheral blood of Mexican women with early BC by quantification of the relative expression of KRT19, MUC1 and SCGB2A2. Material and methods: Observational, longitudinal, analytical study, which included 62 Mexican women, over 18 years of age, with BC clinical stage 0, I and II. Cells with a density < 1.077 g/ml were isolated from 4 mL of peripheral blood obtained 24 h before surgery, RNAS was extracted and RTqPCR was performed to quantify the relative expression of KRT19, MUC1 and SCGB2A2. Results: The diagnosis of BC was confirmed in the definitive pathological study in 59 women. 1 case (1.7%), expressed KRT19, 18 cases (30.5%) MUC1, 2 cases (3.4%) SCGB2A2, and 19 cases (32.2%) expressed at least one of the three biomarkers. Conclusions: CTCs detection rates were like those reported in other populations when MUC1 was used, but lower when KRT19 and SCGB2A2 were used. Standardization of blood sample collection and processing, as well as biomarker expression analysis, is important to reduce variability in the detection of CTCs in specific populations. There are numerous skin signs and symptoms that may occur underlying the appearance of an internal neoplasia. Skin metastases from a breast neoplasm usually appear late during the progression of the disease; however, although rare, they sometimes precede this situation. The case of a patient diagnosed with cutaneous metastasis of a lobular breast carcinoma is presented, where the skin lesions were the starting point for more conclusive studies and the neoplastic diagnosis. This case is presentation due to the need to highlight the importance of the differential diagnosis that the dermatologist must perform, given the real possibility of metastatic skin lesions, in patients who consult with unusual skin lesions.

During 2020, 3,887 people in Cuba were diagnosed with breast cancer, which represents a rate of 39.6, and 1,714 died, for a rate of 30.4. After a mastectomy, many things change in a woman's life, so she needs a lot of support. The “Wings for Life” project was created to support and provide information. Today the importance of this is a reality. The purpose of this research is to expose the criteria of the mastectomized women who are part of the Mariposas del Guamuhaya group, created in Cumanayagua, Cienfuegos province, as part of the “Alas por la vida” project, as well as to leave a graphic record of the activities they carry out and the contributions that it has made to the lives of these women in the human order.

5. Estrés percibido en pacientes mexicanas recién diagnosticadas con cáncer de mama.

- Abstract
Background: Cancer diagnosis has been described as a significant factor causing psychological stress, which may increase a vulnerability to develop psychological disorders, decrease quality of life levels and affect the response to cancer treatment. Objective: To validate the Newly Diagnosed Breast Cancer Stress Scale (NDBCSS) and to explore the differences concerning clinical and sociodemographic variables. Material and methods: Cross-sectional study. Patients recently diagnosed with breast cancer on the day they received the definitive cancer diagnosis were invited to participate. The Perceived Stress Scale (PSS) and the NDBCSS were completed; the latter previously underwent a validation process into Spanish (reliability, content validity, structure -- EFA-- and convergent validity). Results: 176 patients participated; their mean age was of 52.8 years. The scale obtained a content validity of 0.53 and reliability of 0.791; the EFA showed 2 factors explaining 42.31% of the variance. Stress levels were 23.05 (PSS) and 13.2 (NDBCSS). Concern about side effects, the progression of the illness and the lack of information were reported as the most critical stressors for these patients. Conclusions: The NDBCSS showed acceptable psychometric properties for its application in Mexican patients and probably for patients with similar personal characteristics and linguistic and cultural contexts. Based on the results, it is possible to design precise interventions addressed to the stressors they face, such as the impact of the disease and the treatment. [ABSTRACT FROM AUTHOR]

El objetivo del estudio fue conocer el perfil inmunohistoquímico del cáncer de mama e identificar el subgrupo HER2 low en la macrorregión norte del Perú. Se realizó un estudio transversal con una muestra de 1176 pacientes atendidos en el Instituto Regional de Enfermedades Neoplásicas Norte del Perú desde enero de 2016 a diciembre de 2023. Los datos recolectados (edad, tipo histológico, grado y resultados complementarios), se analizaron con frecuencias y porcentajes. El perfil correspondió a: luminal B (45,6%); luminal A (24,7%); triple negativo (18,2%); y HER2 positivo no luminal (11,5%). Además, HER2 low fueron 215 pacientes (25,1% de los considerados previamente negativos). Este estudio proporciona evidencia que la subtipificación de cáncer de mama ha cambiado, siendo luminal B más frecuente, y es esencial involucrar a políticas de salud para adquirir terapias dirigidas considerando a pacientes HER2 low.

Survival was analyzed in patients with breast cancer who were beneficiaries of the Popular Health Insurance in San Luis Potosí, from 2007-2012. Observational, cross-sectional and retrospective quantitative study included 502 cases of breast cancer. Kaplan-Meier curves, log rank test (Mantel-Cox) and a multivariate model using Cox regression were used. There was greater survival in women with positive estrogen receptor status than in negative ones. Those who were estrogen receptor or progesterone receptor positive, who received a higher dose of radiation therapy, were in a higher decile of socioeconomic status, had a lower risk of dying compared with those who were not; stage IIA had a lower risk of dying than stage IV. Survival is related to: estrogen and progesterone receptor status, cancer stage, educational level, radiotherapy dose, and socioeconomic level. [ABSTRACT FROM AUTHOR]

Introducción: El cáncer de mama es la principal causa de muerte por neoplasias en mujeres entre los 45 y 65 años. Aunque las lesiones papilares de mama son poco frecuentes, este caso presenta una variante inusual, un carcinoma papilar ulcerado, lo que destaca la importancia de la detección temprana para un diagnóstico y tratamiento oportuno. Se explora la rareza de la ulceración en este tipo de cáncer. Objetivo: Describir un caso clínico de un tumor papilar ulcerado de la mama. Caso Clínico: Paciente femenina de 84 años con antecedentes de hipertensión arterial, acudió a la consulta de mastología donde se detectó una masa palpable de 8 cm ulcerada, sangrante y con cambios de coloración de la piel. Estudios de imagen confirmaron un tumor de 7 x 6 cm. Se le realiza una mastectomía radical modificada y disección ganglionar, el informe anatomopatológico reveló un carcinoma papilar con adenopatía infiltrativa axilar. La paciente tuvo una evolución satisfactoria. Conclusiones: Las lesiones papilares de la mama son poco comunes, la detección temprana es clave en el diagnóstico y tratamiento del cáncer mamario.

Shell breast cancer is one of the clinical forms of presentation of cutaneous metastases of breast cancer. It is rare, especially in males. The case of a 74-year-old male patient with black skin color, with a history of diabetes and high blood pressure, who attended a provincial mastology consultation for a few months of mild pain and pruritic lesions on his neck, is presented the right breast. In addition to other examinations, an excisional biopsy of a small fragment of the lesion was performed with a paraffin study, which reported cutaneous metastasis of a shell breast carcinoma according to its clinical presentation. The case is shown because no publication was found in the province of Cienfuegos or in the country of a male case, which is why the publication of this report is considered of interest to medical personnel.

The skin manifestations of malignant tumors include a group of dermatoses that can be the sign of the occult neoplasms presence and allow their timely diagnosis. The objective of this report is to present the acanthosis nigricans’ clinical characteristics associated with breast carcinoma. The clinical case of a 50-years-old black-skinned patient is described, assisted in the Dermatology consultation of the Raúl Sánchez University Polyclinic, for having a single erythematous plaque located in the left breast, accompanied by pain, increased local temperature and hyperchromic macules in the armpits; as well as multiple axillary lymphadenopathy. Complementary examinations showed the presence of malignant acanthosis nigricans associated with an inflammatory carcinoma of the left breast. She underwent surgery, with the consequent disappearance of the skin lesions, but with a reserved prognosis. Most paraneoplastic syndromes are nonspecific; there is an urgent need to suspect a correlation between skin changes and the possibility of an internal neoplasia, so it is of utmost importance to refer these patients for identification and early diagnosis of the underlying disease. This would improve the prognosis and greatly mitigate the consequences.

15. Risk Factors Associated with Breast Cancer in Women in a Health Area

Foundation: breast cancer is the most common malignant tumor among women around the world. In Cuba, it continues to be the neoplasia most diagnosed in women, which is why it is important to determine the associated risk factors. Objective: to identify the risk factors associated with breast cancer in women from a family doctor's office in Santiago de Cuba. Methods: an analytical-observational, case-control study was carried out in patients belonging to the Ramon Lopez Peña Community Polyclinic in Santiago de Cuba, in the period from January to December 2022. The study population consisted of female patients, over 30 years of age (3,550), from which the cases (patients diagnosed with breast cancer) and controls (patients who did not suffer from the disease) were chosen, 2 controls were chosen for each selected case. Breast cancer was considered as the dependent variable; the following were used as independent variables: age, personal and family pathological history, early menarche, late menopause, nulliparity, age of the first child, exclusive breastfeeding, replacement or replacement hormone therapy, use of contraceptive methods, obesity, sedentary lifestyle, smoking, alcoholism. They were operationalized as follows (No = absence of the risk factor; Yes = presence of the risk factor) The Odd Ratio, the confidence interval and the Chi square test were determined. Results: age over 60 years (OR=3.47), personal pathological history (OR=3.83), early menarche (OR=5.65), late menopause (OR=5.16), abandonment of breastfeeding (OR=6.59), smoking (OR=8.28) and obesity (OR=7.21) were associated with the appearance of breast cancer. Conclusions: risk factors related to lifestyles such as smoking and obesity and abandonment of breastfeeding were the risk factors that had a strong causal association in the appearance of breast cancer in the present investigation.

- Abstract
Background: Malignant melanoma is a skin neoplasm that typically does not invade the breast, making its spread to the breast tissue in a male patient uncommon. When it metastasizes to the breast, it can present as a palpable mass or even be asymptomatic. Ultrasound allows for the detection of round hypoechoic lesions, while mammography detects well-defined nodules with increased density. The aim of this article is to describe the approach to a male patient with a breast mass and suspected melanoma, as well as the importance of immunohistochemistry for proper differentiation. Clinical Case: A 72-year-old male patient presented with a pigmented lesion on the sternum, an axillary tumor, and a right breast mass, which had been evolving for the past two years. He underwent resection, excisional biopsy, and simple mastectomy, respectively. superficial spreading melanoma with right breast metastases was documented. Immunohistochemistry supported the diagnosis, after positivity for HMB-45 and S100 was found in the examined samples. Conclusions: Metastatic melanoma in the male breast is an exceedingly rare finding. Immunohistochemistry plays a pivotal role in both diagnostic assessment and prognosis, as it guides treatment determination, which encompasses local and systemic interventions. For patients presenting as in this case, the optimal approach involves wide tumor resection with axillary lymph node dissection, coupled with adjuvant systemic therapy. Hence, case reporting is imperative to share diagnostic approaches in such patients and provide appropriate management strategies. [ABSTRACT FROM AUTHOR]

Objective: To identify how the breast and cervical cancer prevention and screening practices performed by nurses working in Primary Health Care in the state of Rio Grande do Sul occurs. Methods: This is a qualitative study with an analytical and comprehensive nature. The collaborators were 58 nurses working in Primary Health Care. Results: The results indicate that the practices are developed in a context of increasing professional autonomy and the role of Nursing. Aspects such as proximity and bonding with the community, other procedures and actions offered in the consultations, including clinical/therapeutic management in cases of signs and symptoms of infection, are consistent with a broader attention to women's health and care needs. Conclusion: The study highlights the importance of supporting protocols for the safe performance of Primary Care nurses' practices, aiming at the prevention and screening of breast and cervical cancer. These documents must be easily accessible and constantly updated in order to guarantee the standardization recommended by the Ministry of Health. [ABSTRACT FROM AUTHOR]

Breast cancer is a chronic non-communicable disease with multifactorial etiology. This study aimed to report and describe the epidemiological profile of female breast cancer in the city of Campo Grande/MS in 2022. The type of observational, descriptive and retrospective study was chosen, with a quantitative approach. The study population comprised all cases of breast cancer of all age groups reported by the municipality's primary health care health units. A total of 192 new cases of cancer were reported, of which 57 were breast cancer. In relation to age group, the highest incidence was between 60 and 69 years old, with 23 cases and 32 already had a family history of cancer. We see the importance of strategies for stratifying and monitoring cases, prevention and health promotion actions and strengthening health education actions for workers. [ABSTRACT FROM AUTHOR]

Film language is an important resource for health teaching-learning. Discussions on cancer and its illness process have used several movies to trigger reflections necessary for a more empathic, sensitive and humanized health care performance. Seeking to contribute to this scenario, this study investigates the meanings around cancer mobilized by the movie Aquarius. The narrative portrays cancer as an invasion against the healthy body and as theft of one's life. Sexuality and female empowerment are intertwined in this film that metaphorizes cancer in its multiple social representations, opening up space for a sense of power and breaking with negative stereotypes prevalent in other languages. Despite the challenges imposed by the COVID-19 pandemic, it was possible to develop work that highlights the contribution of educational groups to the emancipation and autonomy of subjects in relation to their health and highlights the importance of Occupational Therapy in health education. Background: HER-2 positive (+) breast cancer (BC) accounts for 20-25% of BC, it is more aggressive, and it has a lower survival rate. Since the approval of trastuzumab in 1998, other HER-2-targeted therapies such as pertuzumab and trastuzumab emtansine (TDM1) have been introduced, improving patient survival. However, cardiotoxicity is an adverse effect of these treatments. Objective: To estimate the incidence of cardiotoxicity with trastuzumab, trastuzumab/pertuzumab, and TDM1 in women with HER-2 + BC treated over a 6-year period at the Hospital de Clínicas and the Hospital Departamental de Soriano. Material and methods: Observational, descriptive, and retrospective study which included patients with HER-2 + BC treated with trastuzumab, trastuzumab/pertuzumab, or TDM1. Results: 81 patients were included, with a cardiotoxicity incidence of 23.4%. Cardiotoxicity was determined by a > 10% decrease in left ventricular ejection fraction (LVEF) (57.9%) and a LVEF < 50% evident during treatment (42.1%). Only 1 patient presented symptomatic heart failure. 63.1% of those who discontinued treatment due to cardiotoxicity managed to resume it. No relationship was evident between cardiovascular history or the administration regimen and the development of cardiotoxicity. Conclusion: The study showed a cardiotoxicity incidence similar to the international one. Background: In Mexico and the world, breast cancer is the cancer type with the highest incidence and mortality for women. Its incidence has increased due to a higher life expectancy and a higher exposure to risk factors. Screening is done by mammography using the BIRADS (Breast Imaging Reporting and Data System) system, the standard for mammography screening report which classifies lesions assigning recommendations for patient follow-up. The system goes from 0 (not conclusive) to 6 (demonstrated malignancy), being of interest for this study the BIRADS 0 category. Objective: To describe patients classified as BIRADS 0 by mammography and their reclassification in a first-level hospital during 2021. Material and methods: Retrospective, descriptive, crosssectional, observational study. Women over 40 years with a BIRADS 0 result were studied. The following databases were used: Institutional Cancer Registry, Family Medicine Information System, Electronic Clinical File, and the mammography and patient list from preventive medicine. Results: Reclassification by ultrasound (US) was achieved in 100% of patients, in all of the BIRADS US categories. In 3.8% of BIRADS 0 patients, ductal adenocarcinoma was found and confirmed by histological testing. Conclusion: All of the reassessed lesions with US were adequately reclassified. 27. Respuesta patológica completa de pacientes con cáncer de mama HER2 positivo tratadas con quimioterapia neoadyuvante en Colombia

Introduction. Breast cancer is the most common type of cancer and the leading cause of death by cancer in women in Colombia. Approximately 15 to 20% of breast cancers overexpress HER2. Objective. To analyze the relationship between multiple clinical and histological variables and pathological complete response in patients with HER2-positive breast cancer undergoing neoadjuvant therapy in a specialized cancer center in Colombia. Materials and methods. We performed a retrospective analysis of non-metastatic HER2-positive breast cancer patients who received neoadjuvant therapy between 2007 and 2020 at the Instituto de Cancerología Las Americas Auna (Medellín, Colombia). Assessed parameters were tumor grade, proliferation index, estrogen receptor, progesterone receptor, HER2 status, type of neoadjuvant therapy, pathologic complete response rates, and overall survival. Results. Variables associated with low pathologic complete response rates were tumor grades 1-2 (OR = 0.55; 95% CI = 0.37-0.81; p = 0.03), estrogen receptor positivity (OR =0.65; 95%; CI = 0.43-0.97; p=0.04), and progesterone receptor positivity (OR = 0.44; 95% CI = 0.29-0.65; p = 0.0001). HER2 strong positivity (score 3+) was associated with high pathological complete response rates (OR = 3.3; 95% CI = 1.3-8.35; p=0.013). Five-year overall survival was 91.5% (95% CI = 82.6-95.9) in patients with pathological complete response and 73.6% (95% CI = 66.4-79.6) in patients who did not achieve pathological complete response (p = 0.001). Additionally, the pathological complete response rate was three times higher in patients receiving combined neoadjuvant chemotherapy with anti-HER2 therapy than in those with chemotherapy alone (48% versus 16%). Conclusion. In patients with HER2-positive breast cancer, tumor grade 3, estrogen receptor negativity, progesterone receptor negativity, strong HER2 positivity (score 3+), and the use of the neoadjuvant trastuzumab are associated with higher pathological complete response rates.

Introdução: No mundo, um bilhão de pessoas tem algum tipo de deficiência, dentre elas a deficiência auditiva e mais de 190 milhões são mulheres surdas. É relevante tecnologias para educação inclusiva e acessível sobre o câncer de mama a surdas. Objetivo: Construir e validar vídeo educativo para mulheres surdas sobre prevenção e rastreamento do câncer de mama. Materiais e Método: Estudo metodológico com construção de vídeo educativo e validação por 11 especialistas de conteúdo e sete especialistas técnicos. Conteúdo fundamentado no Ministério da Saúde e no Instituto Nacional do Câncer. Na análise de conteúdo foi utilizado o Instrumento de Validação de Conteúdo Educativo em Saúde. A validade foi atestada pelo Índice de Validade de Conteúdo e o teste binomial. Resultados: O vídeo tem 17 minutos e 12 segundos. Na validação de conteúdo todos os itens tiveram concordância acima de 90,9% entre os participantes. O Índice de Validade de Conteúdo global foi superior a 0,90 entre os especialistas de conteúdo. Consideraram o material excelente, muito bom ou bom, 85,8% dos especialistas técnicos. Discussão: O vídeo contribui para o ensino e aprendizagem sobre o tema, ações de autocuidado e promoção da saúde às surdas. Conclusões: Vídeo construído com narração em LIBRAS, foi considerado válido quanto ao conteúdo e aspectos técnicos e poderá ser utilizado em futuras pesquisas e na educação em saúde de surdas.

29. Disponibilidad de mastógrafos en México: ¿Es posible ampliar la cobertura?

Background: Breast cancer screening coverage in Mexico is limited. It is necessary to configure the Mexican health system to make it capable of producing more mammograms, without neglecting quality and diagnostic opportunity. Objective: To project the coverage of breast cancer screening programs according to the mammographs availability in the Mexican Health System Material and methods: An exploratory study was conducted correlating the number mammographs available in the Mexican Health System with women between 40 and 69 who are the target of screening programs in Mexico. The analysis consisted of calculating the coverage of breast cancer screening programs if each mammograph could perform 6336 screening mammograms per year. All the calculations were made with the information of the year 2020 as the most current data in terms of population density. Results: It was determined that the number of mammographs is enough to raise the offer of screening studies to ≈50% at the national level. There are even states whose coverage capacity is projected above 70%. Some of them are Mexico City, Colima, Durango, Quintana Roo, Zacatecas, Baja California Sur and Coahuila. Also, it was observed that the distribution of mammographs is not equitable in the national territory, suggesting investment both in equipment and in screening strategies for breast cancer. Conclusions: Our data show that it is possible to advance in preventing breast cancer in Mexico, although the improvement of the management capacities of the Mexican Health System is required to improve both the coverage and quality of the breast cancer screening programs. - Abstract
Introduction: Gastric outlet obstruction or pyloric syndrome can occur secondary to neoplastic involvement, and metastasis as an etiology is unusual. Breast neoplasms generally cause bone, liver, and lung metastases, rarely involving the gastrointestinal tract. Case presentation: A 69-year-old female patient with infiltrating lobular carcinoma of the right breast consulted for abdominal pain and postprandial emetic episodes with oral intolerance and dyspnea. Bilateral neoplastic breast involvement and dilation of the gastric chamber with thickening of the pylorus were recorded. She required antiemetic management and placement of a nasogastric tube. She was taken to an upper digestive tract endoscopy, which found an ulcerated lesion with an infiltrative appearance at the postpyloric level that circumferentially compromised the duodenal lumen. Then, a biopsy was taken, which was compatible with a breast carcinoma of a lobular type. This entity, called pyloric syndrome due to neoplasia, can be managed with gastrojejunostomy or an enteral prosthesis that improves the quality of life of patients with an ominous short-term prognosis. The patient in our case expressed advance directives not to receive invasive procedures, for which an uncovered metal prosthesis was placed for palliative purposes, achieving the re-establishment of the feeding route and resolution of dyspnea due to restriction. Conclusions: The metastatic involvement of neoplasms of the breast to the gastrointestinal tract is rare; however, it should be suspected in elderly patients with previously documented neoplasms. Introducción: El objetivo del presente estudio fue evaluar las características clínicas, patológicas e histológicas tumorales y su asociación con la recurrencia, metástasis y pronóstico en términos de supervivencia global y libre de enfermedad, de las pacientes que padecen sobrepeso u obesidad al momento del diagnóstico de cáncer de mama. Materiales y métodos: Se condujo un estudio descriptivo, longitudinal, retrospectivo, en un centro oncológico de referencia de Medellín. Se recolectó información de pacientes mayores de 18 años, con cáncer de mama infiltrante temprano y avanzado, entre los años 2012 – 2017, quienes presentaran IMC > 25 kg/m 2 al momento del diagnóstico. Las medianas de supervivencia se calcularon a través de curvas de Kaplan Meier y las diferencias mediante Log Rank Test. Resultados: Se analizó información de 1.349 pacientes. La mortalidad por todas las causas fue de 13.6% y aumentó proporcionalmente con el IMC (HR = 1.03, IC 1.0-1.05). Se identificaron 12.6% de recurrencias y el riesgo con el aumento de IMC no fue estadísticamente significativo (HR =1.02, IC 0.99 - 1.05). Características como mala diferenciación tumoral, invasión linfovascular y estadio tumoral se asociaron de forma univariada con mayor mortalidad. Conclusión: Se demostró una asociación positiva e independiente entre el IMC elevado, la mortalidad y el riesgo de recurrencia en pacientes con cáncer de mama. Así como una asociación con fenotipos tumorales agresivos y características de peor pronóstico. Se sugiere considerar modificaciones en el estilo de vida y un manejo multidisciplinario, como estrategias que posiblemente impacten en estos desenlaces.

Introducción: El cáncer de mama triple negativo (TNBC) se caracteriza por la ausencia de receptores hormonales estrogénicos y progesterona; así como, del receptor 2 del factor de crecimiento epidérmico humano (HER2). Los TNBC se asocian con altas tasas de recurrencia, metástasis rápidas, supervivencia deficiente y mayor mortalidad en comparación con otros subtipos histológicos de cáncer de mama. El objetivo de este estudio fue establecer la prevalencia del TNBC; Así también, las características clínicas, en pacientes atendidas en un centro oncológico de referencia en Bogotá. Metodología: Estudio retrospectivo descriptivo transversal observacional, donde se evaluó la frecuencia del cáncer de mama subtipo triple negativo, Asimismo, las variables clínicas. En mujeres atendidas en la Organización Clínica Bonnadona Prevenir S.A.S. en Barranquilla, Colombia en el periodo 2021-2022. Resultados: Se estudiaron 350 pacientes, de los cuales 61 pacientes (17.4%) presentaban el inmunofenotipo triple negativo. La edad promedio fue de 54.5 años, 74% eran multíparas, 85% brindaron lactancia materna, 70% eran postmenopáusicas y el estadio clínico más frecuente fue el IIIB. Conclusión: En el presente estudio el 57.35% de la población exhibió un estadio clínico avanzado en el momento del diagnóstico; así mismo, las características clínicas son congruentes con los reportes en la literatura.

Introducción: El factor de riesgo más importante para el desarrollo del síndrome metabólico (SM) es la obesidad, un estado pro inflamatorio con alteraciones sistémicas y locales vinculadas al crecimiento tumoral, siendo un determinante en el desarrollo del cáncer de mama en la posmenopausia, en su pronóstico y en la eficacia del tratamiento. El objetivo del presente estudio fue determinar la asociación entre el SM y el Cáncer de Mama en mujeres posmenopáusicas comparradas con un grupo control. Materiales y métodos: El estudio fue de diseño observacional, analítico; se llevó a cabo en el Hospital de SOLCA-Guayaquil entre enero a diciembre del 2019. El muestreo fue no probabilístico. Se incluyeron casos de mujeres de 50 – 70 años con patología de mama. Las variables fueron presencia de síndrome metabólico y diagnóstico de cáncer de mama en mujeres posmenopaúsicas. Se utilizó Chi cuadrado, Odds Ratio, el intervalo de confianza al 95 % y valor P < 0.05 para revisar la significancia estadística. Resultados: Ingresaron al estudio 157 casos, 107 en el grupo con cáncer de mama (G1) y 50 mujeres (31 %) sin patología maligna (G2). La edad en G1 fue de 59.9±9.9 años en G2 55.5±5.5 (P=0.001). Introduction: Currently, there is a global problem related to access to essential medicines by populations, which is why international organizations such as the Panamerican Health Organization and the World Health Organization have expressed their concern to guarantee the right to health for all, especially for those who suffer pathologies with high prevalence and mortality such as breast cancer, so it is of great interest to explore access barriers. Aim: To conduct a narrative synthesis to identify elements that may have an impact on access and use of medicines for the treatment of breast cancer. Methodology: a narrative review was carried out, it consisted of five phases: I) bibliographic search, II) review of titles and abstracts, III) full text review, IV) selection of references, and V) descriptive analysis. Results: Individual barriers to access were identified, such as low purchasing power in patients, low educational level, long travel requirements to obtain medicines, and elements of social discrimination. On the other hand, at the institutional and health system level, difficulties were detected in the procedures to access treatments, lack of coordination between institutions, barriers in price regulation, and little availability of medicines. Conclusions: The findings offer an approach to proposing solutions and alternatives for the structuring of public health policies that guarantee access to breast cancer medicines to patients who require it. Desde el comienzo de la pandemia de Covid-19 los servicios de prevención de cáncer de mama han sido aplazados gravemente. Esta acción provocó la reducción en el número acumulado de mastografías de detección a nivel mundial e incrementó las disparidades en salud, sobre todo entre las mujeres vulnerables. Si bien la evidencia que respalda las estrategias para rescatar el tamizaje con mastografía en la fase de resolución de la pandemia no es suficiente, hay algunas consideraciones pragmáticas que pueden guiar su recuperación y garantizar su continuidad. Una de estas estrategias es prever los retrasos en el tamizaje y abordarlos a través de programas seguros que se alineen con la situación epidemiológica actual. Las acciones planteadas en el presente ensayo están en consonancia con las iniciativas internacionales de garantizar la continuidad de los programas de tamizaje de cáncer de mama en atención a que el cuidado de las mujeres sea una acción prioritaria, continua y permanente.

Objetivos. Documentar cuantitativamente el grado de concordancia con las recomendaciones del Instituto Nacional del Cáncer (INC) de los mensajes institucionales emitidos en redes sociales de Argentina durante octubre de 2019, en el contexto de las campañas de prevención del cáncer mamario, y analizar cualitativamente los elementos icónicos y textuales que conforman sus piezas de difusión. Materiales y métodos. Análisis cuantitativo y cualitativo de 171 piezas de difusión emitidas durante octubre de 2019 por 54 instituciones, a partir de la evaluación de su concordancia con las recomendaciones del INC, la descripción de las principales recomendaciones discordantes (análisis cuantitativo) y el análisis cualitativo de 30 piezas. Resultados. Ninguno de los mensajes emitidos mencionó potenciales daños del tamizaje. Solamente los del Ministerio de Salud de la Nación fueron totalmente concordantes con las recomendaciones del INC, mientras que los restantes recomendaban realizar mamografías a edades más tempranas o a intervalos más breves. El autoexamen mamario fue la recomendación más frecuente entre las discordantes. Predominaron las imágenes de cuerpos femeninos vinculadas con los estereotipos predominantes de género y belleza, y los discursos paternalistas que apelan al miedo y a la culpa. Conclusiones. Los mensajes emitidos en las piezas de difusión analizadas no fueron concordantes con las recomendaciones del INC, a pesar de que estas últimas están respaldadas por evidencia científica. Por otro lado, los mensajes refuerzan los estereotipos de género y belleza, la culpa y el modelo médico-hegemónico.

Antecedentes: Los pacientes con cáncer de mama pueden experimentar dificultades cognitivas por recibir quimioterapia. Dicha alteración es denominada Deterioro Cognitivo Quimioinducido, también es conocida como Chemobrain o Chemofog. Objetivo: Identificar el perfil cognitivo y las características de la evaluación neuropsicológica de esta población. Método: Se consultaron las bases de datos PubMed, SpringerLink y SciELO. Se seleccionaron artículos de 1994 hasta septiembre de 2021. Fueron utilizadas palabras claves relacionadas con el tema de estudio. Resultados: La quimioterapia puede producir un deterioro cognitivo entre el 15 y el 50% de las mujeres. Dicha alteración es de etiología múltiple y puede asociarse a factores biológicos y a cambios funcionales y/o estructurales del SNC. Se deben de considerar los factores sociodemográficos, clínicos y psicológicos como variables moduladoras. Se manifiesta principalmente en problemas de memoria, función ejecutiva, atención y velocidad de procesamiento. Esta alteración puede ser mensurada a través de los instrumentos de evaluación neuropsicológica disponibles. Discusión y conclusión: Se sugiere que la afectación cognitiva quimioinducida sea incorporada en el consentimiento informado. Se recomienda un mayor desarrollo de estudios longitudinales que se complementen con técnicas de neuroimágenes que permitan avanzar en el conocimiento de esta problemática. Se propone un protocolo de evaluación neuropsicológica, que incluye pruebas de screening, escalas clínicas, test neuropsicológicos específicos y de calidad de vida, según las recomendaciones del International Cognition and Cancer Task Force.

Introducción: dentro de las principales causas de mortalidad en sexo femenino se encuentra el cáncer de mama, es de las neoplasias maligna más prevalentes con 1,7 millones de casos diagnosticados cada año, constituye casi un cuarto de los tumores malignos de la mujer, y un 15 % de las muertes por cáncer en este sexo a partir de los 20 años de edad. La histología más común del cáncer de seno es el carcinoma ductal invasivo (50-75 % de los pacientes). Los datos justifican la importancia de comprender y caracterizar esta presentación de cáncer de mama. Objetivo: describir un caso de carcinoma ductal infiltrante de células claras moderadamente diferenciado. Presentación del caso: paciente femenina, blanca de 43 años de edad, de origen caucásico, ama de casa, casada. Con antecedentes patológicos personales de hipertensión arterial, diagnosticada desde hace tres años y que trata con Captopril de 50 mg una tableta cada ocho horas e Hidroclorotiazida de 25 mg una tableta al día; acude a la consulta de mama referida por el especialista en Medicina General Integral de su Consultorio Médico Familiar por presentar aumento de volumen y dolor en la mama izquierda. se decide ingreso para estudio y tratamiento quirúrgico. Conclusiones: la importancia del chequeo sistemático de las mamas para prevenir la aparición de enfermedades mamarias, la detección precoz mediante los medios diagnósticos adecuados y el tratamiento oportuno del carcinoma ductal infiltrante de células claras son claves para lograr mejores resultados en los pacientes intervenidos.

Objective: to evaluate the association of religious-spiritual coping with the health-related quality of life of women with breast cancer undergoing cancer treatment. Method: cross-sectional, exploratory, quantitative, descriptive and analytical research, conducted from March to October 2020, in an oncological hospital. Three assessment instruments were used (Sociodemographic and Clinical Characterization, Functional Assessment of Cancer Therapy-Breast plus Arm Morbidity and Religious-Spiritual Coping Scale), descriptive and statistical analyzes were performed. Results: sample size composed of 39 women, mostly adults (79.5%), married/stable union (48.7%), submitted to mastectomy along with lymphadenectomy (53.8%), Catholic (53.8%), among which 94% believed that spirituality/religiosity helped in coping with cancer. There was a positive correlation between the domain Social/family wellbeing and health-related quality of life with levels of positive and total religious-spiritual coping. Conclusion: Spirituality/religiosity and health-related quality of life positively influenced women during cancer treatment, with greater use of these strategies. [ABSTRACT FROM AUTHOR]

46. Cáncer de mama y mutación del gen ataxia telangiectasia: reporte de caso.

- Abstract
Among women, breast cancer is the most common cancer in the world, accounting for 25.7 % of all cancers and thus making it a topic of interest in public health. Breast cancer is a complex, heterogeneous genetic disease and, in the vast majority of cases, of unknown etiology. Around 7 % of breast cancer cases in the general population present a susceptibility gene mutation of Mendelian inheritance. The ataxia telangiectasia mutated (ATM) gene mutation is found in less than 1 % of the general population, and association studies conducted with controls have shown that these alleles are characterized by a moderate risk (RR: 2 - 4) for hereditary cancer. Mutations in ATM incompletely cosegregate the disease, with an estimated 15 % of mutation carriers in this gene developing cancer. The incomplete penetrance of ATM, as well as other moderate-risk genes, supports that they follow a polygenic model of cancer susceptibility. We report the case of a 35-year-old woman diagnosed with metachronous bilateral breast carcinoma--T4b N1 M0 stage IIIB left breast and T1c N0 stage I right breast, intrinsic luminal B subtype, HER2 overexpression and pathogenic variant of ATM gene c.7913 G>A, p. Trp2638*--who was treated with neoadjuvant chemotherapy, followed by mastectomy, sentinel node biopsy and radiotherapy. The objective of this case report is to describe the clinicopathological characteristics of breast cancer and its association with moderate-risk gene mutations. [ABSTRACT FROM AUTHOR]

The study focuses on the importance of cervical and breast cancer as public health issues for working women, highlighting the Woman Healthy Extension Project in a pediatric hospital in Curitiba-PR. The project conducts screenings and educational activities to prevent these cancers, raising the central question: "Do educational activities in the workplace impact gynecological health?" Using an integrative literature review, the study analyzed seven articles from November 2022 to March 2023. It emphasizes that extension projects contribute to academic development, promoting a critical perspective. The lack of preventive practices in gynecological health is related to various factors, underscoring the importance of discussing exams such as Pap smears and mammography. The study concludes that integrating extension projects into the curriculum promotes health education, benefiting female workers through the prevention and early diagnosis of gynecological cancers.
