Neighborhood context plays an important role in producing and reproducing current patterns of health disparity. In particular, neighborhood disorganization affects how people engage in health care. We examined the effect of living in highly disorganized neighborhoods on care engagement, using data from the Coordinated Healthcare for Complex Kids (CHECK) program, which is a care delivery model for children with chronic conditions on Medicaid in Chicago. We retrieved demographic data from the US Census Bureau and crime data from the Chicago Police Department to estimate neighborhood-level social disorganization for the CHECK enrollees. A total of 6458 children enrolled in the CHECK between 2014 and 2017 were included in the analysis. Families living in the most disorganized neighborhoods, compared with areas with lower levels of disorganization, were less likely to engage in CHECK. Black families were less likely than Hispanic families to be engaged in the CHECK program. We discuss potential mechanisms through which disorganization affects care engagement. Objective: This article describes the demographic distribution of, and association between, neighborhood disorganization and mental health diagnosis by race in a large cohort of urban children with chronic medical conditions. Methods: Data for this study were from Coordinated Healthcare for Complex Kids (CHECK), a health care demonstration project funded by the Center for Medicare and Medicaid Innovation. We conducted regression analyses to examine the relationship between neighborhood disorganization and mental health diagnosis among 6,458 children enrolled in CHECK. Results: The most common mental health diagnoses were mood disorders (8.6%), Attention-Deficit/Hyperactivity Disorder (7.4%), conduct disorders (6.1%), and anxiety disorders (4.8%). Black children had the highest neighborhood disorganization scores compared with other racial/ethnic categories. However, Black children had the lowest proportion of mental health diagnoses. Lower neighborhood disorganization was associated with having a mental health diagnosis; however, when adding race/ethnicity to the model, neighborhood disorganization no longer was significant. Conclusions: Level of neighborhood disorganization was highly correlated with racial/ethnic composition of the neighborhoods, and Black children disproportionately resided in highly disorganized neighborhoods compared with other groups. Neighborhood disorganization may not have sufficient variability within the racial/ethnic categories, which may explain the absence of an interaction between race/ethnicity and mental health diagnosis.

This study applies indicators of social disorganization theory (i.e., concentrated disadvantage, residential instability, and concentrated immigration) to predict officers' use of coercive action during street stops of citizens suspected of criminal activity. It also investigates whether concentrated disadvantage moderates suspects' likelihood of receiving greater levels of police coercive actions when stopped for reasons listed in the New York City Police Department's Unified Form 250 (UF-250). Results from multilevel analyses of stop incidents nested within neighborhoods confirm that certain indicators of social disorganization are associated with officer use of coercive action. Further, suspects stopped in areas marked by concentrated disadvantage are less likely to receive higher levels of police coercive action. Previous studies have indicated that neighborhood disorganization affects child-rearing beliefs in the United States, but few studies have focused on such influences among Asian American parents. Largely due to Asian American parents' immigration experiences, neighborhood disorganization factors inevitably intersect with their traditional cultures, which may lead to different patterns in their parental beliefs. Using structural equation modeling, this study found that neighborhood disorganization factors directly influenced Asian American parents' beliefs toward physical punishment and parenting stress mediated this relationship. Approximately, 2.4 billion people, globally, lack access to safe toilets. The burden of this deficiency falls disproportionately on women living in informal settlements in developing countries. While there is a growing body of literature looking at the factors influencing women's sanitation behavior, little is known about the role of the neighborhood environment. This study sought to explore this issue using data from a 2016 mixed-methods study focused on women in Mathare Valley informal settlement in Nairobi, Kenya. Qualitative and quantitative data were used to analyze the role of neighborhood disorganization in the relationship between women's sanitation-related stressors and their sanitation behaviors (utilization). Findings suggest that neighborhood disorganization is inextricably linked to women's daily sanitation behaviors and the stressors that are associated with those behaviors. These results suggest that the health and safety of neighborhood environments may be critical factors in the future success of sanitation-related interventions and policies. [ABSTRACT FROM AUTHOR]

Competing theories have been posed regarding the causes of animal cruelty leading to conflicting findings and little direction for public policies to combat it. Objective: To assess the applicability of extant theories of the causes of animal cruelty: domestic violence; deviance; perpetrator traits; and social disorganization. Methods: Data are drawn from police department reports of animal cruelty in the City of Detroit from 2007 to 2015; 302 incidences of animal cruelty were reported. Multiple regression is used to determine the theory which best appears to account for animal cruelty. Results: Common types of animal cruelty in Detroit are shooting; blunt force trauma; neglect; and dogfighting. While most incidents involve unknown persons; cruelty by owners; neighbors; and domestic partners is also common. Neighborhood conditions in terms of economic stress; vacancy and blight; and crime appear to have the greatest impact on animal cruelty. Conclusions: The findings from Detroit support deviance and social disorganization theories of animal cruelty. Neighborhood conditions in terms of economic stress, vacancy and blight, and crime appear to have the greatest impact on animal cruelty in this urban area.

The current study explored association of neighborhood elements to children's health and related outcomes. Nationally representative data (N = 49,513,974, ages 6-17, 51.1% Male) was used to empirically define classes of neighborhoods based on presence or absence of various neighborhood elements. Analyses resulted in a three-class model: 1) "High Assets, Low Disorganization" (64.57%), 2) "High Assets, High Disorganization" (13.51%), and 3) "Few Assets, Low Disorganization" (21.91%). Class Membership was differentially associated with health, flourishing, and neighborhood cohesion. Results suggest health interventions should focus on increasing neighborhood assets, decreasing levels of neighborhood violence and poverty, and improving social dynamics of neighborhoods. This study employed fixed effects regression that controls for selection bias, omitted variables bias, and all time-invariant aspects of parent and child characteristics to examine the simultaneous associations between neighborhood disorganization, maternal spanking, and aggressive behavior in early childhood using data from the Fragile Families and Child Wellbeing Study (FFCWS). Analysis was based on 2,472 children and their mothers who participated in Wave 3 (2001–2003; child age 3) and Wave 4 (2003–2006; child age 5) of the FFCWS. Results indicated that higher rates of neighborhood crime and violence predicted higher levels of child aggression. Maternal spanking in the past year, whether frequent or infrequent, was also associated with increases in aggressive behavior. This study contributes statistically rigorous evidence that exposure to violence in the neighborhood as well as the family context are predictors of child aggression. We conclude with a discussion for the need for multilevel prevention and intervention approaches that target both community and parenting factors. [ABSTRACT FROM AUTHOR]

Living in disorganized neighborhoods characterized by high levels of poverty, crime, violence, and deteriorating buildings has been associated with increased alcohol consumption and mental health problems. Data drawn from the Seattle Social Development Project ( N = 790), a theory-driven longitudinal study originating in Seattle, WA, were used to estimate trajectories of Alcohol Use Disorder ( AUD) symptoms from age 21 to 39. Time-varying measures of neighborhood disorganization, psychological distress, and sociodemographic factors were associated with deviations from average AUD symptoms at each wave. Results indicated that, on average, AUD symptoms decreased as individuals got older. Living in more disorganized neighborhoods and experiencing psychological distress was associated with increased AUD symptoms after accounting for average reductions from AUD symptoms over time and time-varying measures of relevant sociodemographic factors. Results of mediation analysis suggested that psychological distress is a mechanism by which disorganized neighborhoods increased risk of AUD from age 21 to 39. [ABSTRACT FROM AUTHOR]

Using agency-generated data collected from the Houston Police Department (HPD) and the 2000 census statistics, this article examines the relationship between police performance and neighborhood disorganization through an analysis of the distribution of police response to in-progress assault calls across different Houston neighborhoods. The results of multilevel analyses suggest that concentrated disadvantage, immigrant concentration, and residential stability are significantly related to the distribution of the HPD’s response time patterns. More specifically, police responses were quicker to in-progress assault calls in disorganized neighborhoods. The implications drawn from the current study’s results on police response time patterns can be useful in improving police service levels, the police–community relationship, and patrol strategies. We investigated the role of social support in the stress process by focusing on African-American women caring for young children in a high-poverty socio-economic context. Data came from 736 women living in Detroit who completed face-to-face interviews in 2002-2003 and 2004-2005. Regression analyses illustrated that the influence of social support varied according to instrumental and emotional support types as well as the stressor examined. Transportation and childcare support partially buffered the negative effects of food insufficiency on psychological distress. Financial support buffered the influence of neighborhood disorganization on psychological distress. Abstract: Aims: This research examines adolescent perceptions of neighborhood disorganization and social capital to determine if they are associated with adolescent alcohol or drug (AOD) use, AOD dependence, and access to AOD treatment. Design: This is a secondary analysis of data from the 1999 and 2000 National Survey on Drug Use and Health (NSDUH). The NSDUH is a cross-sectional survey of a random sample of the non-institutionalized United States population and is conducted in respondents’ homes. Participants: Youth between the ages of 12 and 17, yielding a sample size of 38,115 respondents. Measurements: Neighborhood disorganization was self-reported by youth in response to eight items; 10 items measured social capital. AOD use was also self-reported. AOD dependence was assessed by a series of questions regarding symptoms and impairment that is consistent with the criteria specified in the DSM-IV. Results: A little more than half of the youth reported never using alcohol or drugs (54.3%), 41.1% reported lifetime AOD use, and 4.6% were AOD dependent. Two percent reported receiving AOD treatment. Medium and high levels of social capital were negatively associated with AOD use and dependence. Social capital was unrelated to access to AOD treatment. Neighborhood disorganization was positively associated with AOD use, dependence, and access to treatment. Conclusions: After controlling for individual- and family-level characteristics, neighborhood disorganization and social capital were associated with AOD use and dependence. The findings suggest that subjective measures of social context may be an important component of the complex biopsychosocial model of adolescent AOD addiction and treatment utilization. Neighborhood environments are increasingly recognized as a contextual determinant of health, behaviors, and disease; however, the pathways through which neighborhood characteristics impact health behaviors are poorly understood. This article examines pathways to elucidate how neighborhood social disorder may lead to HIV transmission. Data are from a baseline survey of 701 injection drug users from the Self-Help in Eliminating Lethal Diseases Study, an HIV prevention intervention in Baltimore. Structural equation modeling was used to examine the pathways among social disorder, psychological distress, and drug injection behaviors. The relationship between disorder and injection behaviors in the models tested suggests that psychological distress is higher in more socially disordered neighborhoods, that distress leads to greater injection frequency and equipment sharing, and that injection frequency predicts equipment sharing. Substance use and delinquency in Thai adolescents are growing public health concerns. Research has linked neighborhood characteristics to these outcomes, with explanations focused on neighborhood disorganization, social cohesion, and social control. This study examines the independent associations of these neighborhood constructs with Thai adolescents' substance use and delinquency, through peer deviance, to determine which neighborhood aspects are particularly important. Families (N=420) with adolescents aged 13-14 were randomly selected from 7 districts in Bangkok, Thailand. Structural equation modeling showed that adolescents', but not parents', perceptions of greater disorganization were related to increased rates of both minor and serious delinquency. Surprisingly, greater neighborhood cohesion was related to greater minor delinquency. Peer deviance was unrelated to neighborhood variables. Findings can inform prevention strategies for Thai adolescents, as results suggest that neighborhoods are important for adolescent behaviors regardless of culture. Further work should help communities make use of social cohesion to benefit residents.
