The purpose of this finite element analysis (FEA) was to evaluate the stress distribution within the prosthetic components and bone in relation to varying cement thicknesses (from 20 to 60 μm) utilized to attach a zirconia crown on a conometric cap. The study focused on two types of implants (Cyroth and TAC, AoN Implants, Grisignano di Zocco, Italy) featuring a Morse cone connection. Detailed three-dimensional (3D) models were developed to represent the bone structure (cortical and trabecular) and the prosthetic components, including the crown, cement, cap, abutment, and the implant. Both implants were placed 1.5 mm subcrestally and subjected to a 200 N load at a 45° inclination on the crown. The results indicated that an increase in cement thickness led to a reduction in von Mises stress on the cortical bone for both Cyroth and TAC implants, while the decrease in stress on the trabecular bone (apical zone) was relatively less pronounced. However, the TAC implant exhibited a higher stress field in the apical area compared to the Cyroth implant. In summary, this study investigated the influence of cement thickness on stress transmission across prosthetic components and peri-implant tissues through FEA analysis, emphasizing that the 60 μm cement layer demonstrated higher stress values approaching the material strength limit. Background and objectives: wear and corrosion can lead to the gross failure of the Morse taper junction with the consequent fracture of the true neck of the prosthetic stem in hip arthroplasty. Materials and Methods: 58-year-old male patient, with a BMI of 38 kg/m2. Because of avascular necrosis, in 2007, a metal-on-metal total hip arthroplasty was implanted in him, with a TMZF stem and a Co-Cr head. In December 2020, he complained of acute left hip pain associated with the deterioration of his left leg and total functional impairment, preceded by the crunching of the hip. X-rays and CT scan showed a fracture of the prosthetic neck that necessitated prosthetic revision surgery. A Scanning Electron Microscope (SEM) analysis of the retrieved prosthetic components was conducted. Results: Macroscopically, the trunnion showed a typical bird beak appearance, due to a massive material loss of about half of its volume. The gross material loss apparently due to abrasion extended beyond the trunnion to the point of failure on the true neck about half a centimeter distal from the taper. SEM analysis demonstrated fatigue rupture modes, and the crack began close to the neck's surface. On the lateral surface, several scratches were found, suggesting an intense wear that could be due to abrasion. Conclusions: The analysis we conducted on the explanted THA showed a ductile rupture, began close to the upper surface of the prosthetic neck where the presence of many scratches had concentrated stresses and led to a fatigue fracture. Purpose: The use of the conical coupling connection to support definitive restorations was evaluated in cases involving full acrylic resin or hybrid acrylic resin-composite prostheses. The aim of this study was to evaluate the performance of definitive fixed partial prostheses made with lithium disilicate and supported by coupling connection abutments and healed implants. Materials and Methods: The prostheses were placed in the posterior regions of partially edentulous patients; 65 patients received fixed lithium disilicate restorations splinted with conical coupling connections to two implants. Implants were placed into healed sites and fresh extraction sockets. The prostheses were placed after healing periods of 3 months: restorations were followed yearly for 3 years. At each follow-up visit, peri-implant bone levels and pocket depths were recorded. Esthetic, functional, and biologic United States Public Health Services (USPHS) parameters modified by the FDI World Dental Federation study design were assessed at the last follow-up appointments. Results: Two prostheses (3.07%) fractured: one was related to framework design error; one was caused by a car accident. Three patients reported small chips: these restorations were immediately polished. No significant difference involving periimplant bone and probing levels between the experimental times were found. None of the prostheses became loose. Conclusions: The results of this research indicated that abutment-prostheses coupling connections were successful within the timeframe of this study. The present study aimed to evaluate the influence of manual torque (10 Ncm) versus clinical torque (30 Ncm), which is recommended by the manufacturer, on the total length of morse cone implant abutments. Twenty specimens were prepared and distributed into two groups: group 1 with ten analogs for morse cone type implant, and group 2 with ten morse type implants, size 4.3 × 15 cm. In each group, the distance between the implant platform to the top of the prosthetic abutment (abutment height) was measured and subjected to a torque of 10 Ncm. Then, the 30 Ncm torque was applied to the same abutment, and abutment height was measured. The distance between the top of the abutment and the implant/analog base was measured. In order to verify the clinical reproducibility of the experiment, comparisons between the abutment height of the analog at 10 Ncm and the implant at 30 Ncm were performed, showing a greater discrepancy in torque for the 10 Ncm analog (p < 0.05). In order to verify if the change in the laboratory protocol from 10 to 30 Ncm could minimize the differences in the height of the prosthetic abutments, the abutment height in groups 1 and 2 was compared with 30 Ncm, and no significant difference was observed (p > 0.05). The data indicated that the manual torque and the torque recommended by the manufacturer influence the total length of the prosthetic abutments of morse cone implants. [ABSTRACT FROM AUTHOR]
