Abstract Background While advancements in cancer treatments have improved survival rates, they also lead to adverse effects such as insomnia, which significantly impacts survivors' sleep quality. Objective This study explores the influence of cancer‐related fatigue (CRF), Fear of Cancer Recurrence (FCR), and psychological distress, with rumination serving as a mediating factor, on the insomnia experienced by cancer survivors. Methods The study involved 220 cancer survivors attending Shohada‐e‐Tajrish Hospital's oncology center in Tehran, Iran. Participants were selected through convenience sampling and completed several questionnaires: the Insomnia Severity Index, Fear of Cancer Recurrence Inventory, Cancer Fatigue Scale, Kessler Psychological Distress Scale, and Rumination Response Scale. Abstract Background Studies have indicated that apart from enhancing patient survival, chemotherapy has adverse side effects on the psychological, social, and cognitive functions of breast cancer survivors. Aims This study was conducted to understand chemotherapy's impact on breast cancer survivors' cognitive functions. Methods and Results Our study is a systematic review based on the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) statement. We searched English databases, including PubMed/MEDLINE, PsycINFO, and Web of Science, and Persian databases, such as Irandoc and Elmnet, using Persian keywords of cancer, breast cancer, chemotherapy, cognitive functions, executive functions, and neuropsychological functions. Two reviewers independently evaluated the full text of the articles according to predefined criteria. Among the 937 available studies, 26 were selected based on the inclusion and exclusion criteria, of which 17 (65%) were longitudinal and 9 (35%) were cross‐sectional. The findings indicated a significant relationship between the use of chemotherapy and cognitive impairments, most notably attention, working and short‐term memory, and executive functions. However, the studies differed in their findings regarding the long‐term persistence of cancer‐related cognitive impairment (CRCI), which could be due to the wide range of tools used, different methods to measure cognitive functions, and the difference in the sample size of the studies. Conclusion Chemotherapy, affecting cortical and subcortical brain structures, causes a set of cognitive impairments that can lead to impairments in social responsibility acceptance, daily functioning, and quality of life of women. Therefore, rigorous and extensive research design is required to understand the causes and consequences of CRCI using standardized and sensitive measures of cognitive functions. Specifically, studies comparing the effects of different chemotherapy regimens on cognition and potential mechanisms and/or moderators of CRCI would be instrumental in designing more effective therapy regimens and evaluating the efficacy and cost‐effectiveness of cognitive rehabilitation and supportive care programs.

Background: Femininity is a complicated term because it does not define a single phenomenon. Different perceptions of femininity are very dependent on one's background and knowledge of everyday actions that the meaning of femininity understood by them. In addition, the concept of femininity is close to the concept of gender, which in all societies is one of the basic structures of human identity and experiences, and the importance and role of it varies from society to society. Aims: Since the definition of femininity varies depending on the culture and standards of each country, examining the perception of women in different countries can clarify various aspects of femininity. Methods: In the present study, using the qualitative metasynthetic method of Noblit and Hare (1988), qualitative studies related to the subject of research in three databases Scopus, Sciencedirect and PubMed in the period 2000 to 2020 were examined. The search retrieved 377 articles and based on PRISMA method and inclusion and exclusion criteria, 19 articles selected to enter the research . Results: Six different factors were found in the perception of femininity: sexual agency, motherhood, appearance and body, caregiver or self-care, independence and respect, and cultural factor. Conclusion: Identifying different aspects of femininity can help researchers to explain and solve various problems and concerns of women

Medical digital twins, which represent medical assets, play a crucial role in connecting the physical world to the metaverse, enabling patients to access virtual medical services and experience immersive interactions with the real world. One serious disease that can be diagnosed and treated using this technology is cancer. However, the digitalization of such diseases for use in the metaverse is a highly complex process. To address this, this study aims to use machine learning (ML) techniques to create real-time and reliable digital twins of cancer for diagnostic and therapeutic purposes. The study focuses on four classical ML techniques that are simple and fast for medical specialists without extensive Artificial Intelligence (AI) knowledge, and meet the requirements of the Internet of Medical Things (IoMT) in terms of latency and cost. The case study focuses on breast cancer (BC), the second most prevalent form of cancer worldwide. The study also presents a comprehensive conceptual framework to illustrate the process of creating digital twins of cancer, and demonstrates the feasibility and reliability of these digital twins in monitoring, diagnosing, and predicting medical parameters.

PURPOSE: To provide resource-stratified, evidence-based recommendations on the early detection of colorectal cancer in four tiers to clinicians, patients, and caregivers. METHODS: American Society of Clinical Oncology convened a multidisciplinary, multinational panel of medical oncology, surgical oncology, surgery, gastroenterology, health technology assessment, cancer epidemiology, pathology, radiology, radiation oncology, and patient advocacy experts. The Expert Panel reviewed existing guidelines and conducted a modified ADAPTE process and a formal consensus-based process with additional experts (Consensus Ratings Group) for two round(s) of formal ratings. RESULTS: Existing sets of guidelines from eight guideline developers were identified and reviewed; adapted recommendations form the evidence base. These guidelines, along with cost-effectiveness analyses, provided evidence to inform the formal consensus process, which resulted in agreement of 75% or more. CONCLUSION: In nonmaximal settings, for people who are asymptomatic, are ages 50 to 75 years, have no family history of colorectal cancer, are at average risk, and are in settings with high incidences of colorectal cancer, the following screening options are recommended: guaiac fecal occult blood test and fecal immunochemical testing (basic), flexible sigmoidoscopy (add option in limited), and colonoscopy (add option in enhanced). Optimal reflex testing strategy for persons with positive screens is as follows: endoscopy; if not available, barium enema (basic or limited). Management of polyps in enhanced is as follows: colonoscopy, polypectomy; if not suitable, then surgical resection. For workup and diagnosis of people with symptoms, physical exam with digital rectal examination, double contrast barium enema (only in basic and limited); colonoscopy; flexible sigmoidoscopy with biopsy (if contraindication to latter) or computed tomography colonography if contraindications to two endoscopies (enhanced only).

Treatments for IIB-IIC: in basic and limited, open en bloc resection following standard oncologic principles, if not possible, transfer to higher-level facility; in emergency, limit to life-saving procedures; in enhanced, laparoscopic en bloc resection, if not possible, then open. Treatments for obstructing, IIB-IIC: in basic, resection and/or diversion; in limited or enhanced, emergency surgical resection. Treatment for IIB-IIC with left-sided: in enhanced, may place colonic stent. Treatment for T4N0/T3N0 high-risk features or stage II high-risk obstructing: in enhanced, may offer adjuvant chemotherapy. Treatment for rectal cancer cT1N0 and cT2n0: in basic, limited, or enhanced, total mesorectal excision principles. Treatment for cT3n0: in basic and limited, total mesorectal excision, if not, diversion. Treatment for high-risk patients who did not receive neoadjuvant chemotherapy: in basic, limited, or enhanced, may offer adjuvant therapy. Treatment for resectable cT3N0 rectal cancer: in enhanced, base neoadjuvant chemotherapy on preoperative factors. For post-treatment surveillance, a combination of medical history, physical examination, carcinoembryonic antigen testing, imaging, and endoscopy is performed. Frequency depends on setting.
