Background Primary cutaneous gamma/delta T-cell lymphoma is aggressive, frequently presenting as multiple plaques, tumors, and/or subcutaneous nodules. Methods In this study, we conducted a retrospective study in a tertiary center in Taiwan to characterize this rare tumor. Results We identified six patients. Five presented with a solitary lesion, including two with clinical impression of epidermal inclusion cyst or lipoma. Two of four evaluable cases exhibited epidermotropism, with one mimicking Pautrier's microabscess. The neoplastic cells were pleomorphic and mostly medium to large-sized. In all cases, the neoplastic cells expressed TCR-γ and/or TCR-δ, with four coexpressing βF1. Two of these βF1+ cases co-expressed TCR-γ, but not TCR-δ (two different clones). All were negative for EBV, low stage, and treated with radiotherapy alone or combined chemotherapy and radiotherapy. In two patients, lymphoma relapsed in 3 and 7 months, respectively, with one patient died of the disease in 7 months. Four other patients were free of disease for 6-126 months. Conclusion PCGD-TCL cases in Taiwan are more commonly solitary, frequently with indolent courses. The two currently available TCR-δ clones alone might be insufficient to detect all tumors. Inflammatory pseudotumor is a term used to designate inflammation-rich tumefactive lesions. Following the exclusion of specific entities such as IgG4-related disease and other neoplastic entities previously included in this entity, the majority of hepatic pseudotumors show a prominent fibrohistiocytic inflammatory reaction and have been previously categorized as fibrohistiocytic variant of hepatic pseudotumor (FHVHPT). The goal of this study was to examine the clinical, radiologic, histologic, and etiologic aspects of this entity. After excluding neoplastic diseases, we identified 30 patients with FHVHPT from 3 institutions between 2009 and 2019. We extracted demographic and clinical data, liver function tests as well as culture results and radiologic information. Hematoxylin and eosin-stained slides were reviewed for pattern of inflammation as well as its cellular composition. Immunohistochemistry for IgG4 and IgG was performed in all cases. The mean age of the 30 lesions characterized as FHVHPT was 56 years (range: 23 to 79 y). Nineteen patients showed solitary lesions; 11 were multiple. The mean size of the lesion was 3.8 cm (range: 1 to 7.5 cm). On imaging, a neoplastic process or metastasis was the leading diagnostic consideration (n=15, 50%). The most common symptom was abdominal pain (n=14/30); 8 patients presented with symptoms compatible with an infectious process, including fever. The inflammatory infiltrate was dominated by lymphocytes and plasma cells, and in most cases, a prominent histiocytic infiltrate (22/30). Neutrophils were identified in 12 cases, with microabscess noted in 8. Storiform pattern of fibrosis was seen in 14/30 cases; obliterative phlebitis was not identified. Culture identified a microorganism in 4 of 9 cases evaluated. The mean IgG4 count was 9.3 per HPF (range: 0 to 51) with 9 of the 26 (35%) biopsies showing >10 IgG4 positive plasma cells per HPF. The mean IgG4 to IgG ratio was 8% (range: 8% to 46%). A hepatectomy was performed in 4 cases. On broad spectrum antibiotics (n=14) the lesions either resolved or decreased in size. Eight patients did not receive specific therapy, nevertheless, the lesion(s) resolved spontaneously in 6 cases, remained stable or decreased in size in 2 cases. Notably, none of these patients showed evidence of a hepatic recurrence. FHVHPT, a tumefactive lesion that mimics hepatic neoplasia, is histologically characterized by a fibrohistiocytic infiltrate. In the majority of patients FHVHPT represents the organizing phase of hepatic abscess and can be successfully managed with antibiotic therapy.

Idiopathic granulomatous mastitis (IGM) is a rare form of chronic inflammatory breast disease. Although it is a benign breast lesion, it may be sometimes difficult to distinguish from breast cancer. The cause of IGM is unknown, but may be associated with autoimmunity, abnormal hormone levels and infection. While the clinical manifestations of IGM involve various manifestations of inflammation, the diagnosis is principally established by histopathology, characterized by non-caseating granulomas and microabscess formation centered on the breast lobules. Therapeutic options for IGM range from observation to various medical treatments, such as steroids, immunosuppressants, and antibiotics, to surgical intervention, particularly if secondarily infected. Given that the controversy on etiology and treatment choices, we accomplished the present review through reviewing IGM-related literature published in 'Pubmed' and 'Web of science' databases during 1997 to 2020, aiming to provide the basis for rational clinical diagnosis and treatment.

In the present study, E. coli isolates representing four different phylogroups viz. A, B1, D, and F were used in rat model to study their pathogenic potential for the development of endometritis. Animal were classified into four infected groups and one group served as negative control. Isolates representing respective phylogroups were inoculated by intrauterine route in twenty diestrus rats. Histopathological changes noted in all the experimental groups with varying intensity of lesions of endometritis characterized by invasion of neutrophils in endometrial superficial epithelium, stratum compactum, uterine cavity, and microabscess formation. Experimental group 4 (phylogroup D) showed highest mean histopathological score, followed by group 2 (phylogroup A), group 3 (phylogroup B1) and group 5 (phylogroup F). However, no statistical significant difference could be observed between the infected groups. The present study concluded that, different phylogroups of E. coli do not differ significantly to induce the lesions of endometritis in diestrus rats.

Intermittent inflammation of the vulval pilosebaceous units is common and usually self-limiting, but some patients experience recurrent and more troublesome symptoms. There is a scarcity of information on this problem. We describe the clinical and histological features in these patients and the response to treatment. A retrospective, observational study of 16 patients with this phenomenon of recurrent, protracted folliculocentric inflammation of the vulval pilosebaceous unit was performed. Details on the clinical features, histology and response to treatment were collected. Mean age at presentation was 32 years (range 21-45). All patients reported recurrent painful papules and pustules on the labia majora and labia minora. Nine patients reported a cyclical pattern to the development of lesions, with premenstrual exacerbation being most common. Histological examination of these lesions showed a folliculocentric microabscess formation surrounded by an acute and chronic inflammatory cell infiltrate, with a focal foreign-body granulomatous reaction. All our patients responded well to tetracycline, antiandrogenic or retinoid therapy. We propose the term 'vulval acne' for this condition and propose a stepwise approach to its management. We hope to highlight this as a common but underreported entity.

Aims Types 1 and 2 autoimmune pancreatitis (AIP) can mimic pancreatic neoplasia. Due to the small quantity of tissue in mass-targeted pancreas biopsies, inflammatory features may raise the differential of AIP. However, the frequency of AIP-like histology in neoplastic pancreas is not well characterised. Therefore, the specificity of inflammatory lesions on biopsy with respect to the diagnosis of AIP is uncertain. Methods and results Neoplastic pancreas resections performed at our institution between 2008 and 2019 were retrospectively reviewed. Features of AIP types 1 and 2 were assessed in the non-neoplastic areas. If features of immunoglobulin (Ig)G4-associated AIP were seen, IgG4 immunohistochemistry was performed. We identified 163 neoplastic pancreas resections. Of these, 34 had one or more types of inflammatory lesions in non-neoplastic pancreatic tissue. Dense lymphoplasmacytic inflammation mimicking type 1 AIP was found in six cases with mild to moderately increased IgG4-positive plasma cells. Neutrophilic infiltrates in small intralobular ducts were found in 20 cases. Mild extralobular ductitis or duct microabscess was found in 10 specimens. Marked neutrophilic duct destruction that resembled granulocytic epithelial lesions was found in 12 cases. Some cases showed multiple features. Conclusion Approximately 20% of neoplastic pancreas resections showed focal areas that could raise the differential of AIP. More cases showed neutrophilic predominant inflammation as seen in type 2 autoimmune pancreatitis, compared to dense lymphoplasmacytic infiltrates seen in type 1 AIP. Pathologists must be cautious when making a diagnosis of AIP on biopsy tissue based on histological findings alone.

Background Psoriasis is an immune-mediated, chronic inflammatory disease with diverse phenotypes. However, its biological diversity has not been well-characterized in Chinese psoriasis population. Objectives To characterize psoriasis biological heterogenicity using gene expression profiles of lesional skin biopsy specimens in a Chinese psoriasis population. Methods Lesional tissues and blood samples from Chinese psoriasis patients (n = 40), atopic dermatitis (AD) patients (n = 25) and age-matched healthy controls (n = 19) were investigated by using real-time PCR array, histological evaluation and flow cytometry. Unsupervised hierarchical clustering was performed using gene expression profiles of patients with psoriasis. Results Two distinct psoriasis clusters were identified. Both clusters indicated high TH 17 activation. One cluster (n = 6 of 40 consecutive psoriasis patients) indicated a strong TH 2 component in skin lesions, with early onset and low peripheral blood eosinophil level. Significantly higher IL-4, IL-13, IL-25, IL-31 and TSLP gene induction typified this cluster of psoriasis patients, even compared with AD patients. Both psoriasis clusters were characterized by neutrophilic microabscess formation. Histologically, the TH 2 high psoriasis cluster indicated a low percentage of perivascular eosinophils. Conclusions Two distinct psoriasis clusters were identified. One presented early onset and a low eosinophil level, indicating TH 17 polarization and a strong TH 2 component. These results laid the foundation for further demonstrating the pathogenesis of psoriasis in Chinese population.

Objective To elucidate the spectrum of findings on neuroimaging in leprosy with special reference to magnetic resonance imaging (MRI). Methods We retrospectively reviewed the neuroimaging findings on MRI in thirty-two patients with leprosy where dedicated plexus, brain and spine imaging had been performed. Results Twenty-two patients had positive findings on imaging. Six patients showed involvement of the peripheral nerves as thickening and formation of micro or macroabscesses. The brachial plexus showed thickening and hyperintensity in ten patients, with associated involvement of the lumbosacral plexus in one patient. Four patients showed spinal cord involvement as discrete T2 hyperintense lesions with postcontrast enhancement. Intracranial lesions involving the nucleus ambiguus and facial colliculus were seen in two patients. Conclusion Neuroimaging in Hansen's disease has brought several new findings to the fore front. Besides peripheral nerves, involvement of the centrally located plexuses, spinal cord and brain has also been seen which thus expands the imaging spectrum of the disease.

Background Neonatal suppurative parotitis is a rare disease, characterized mainly by unilateral parotid swelling with erythema and tenderness, and often purulent discharge from the Stensen's duct into the oral cavity. Only 44 cases were reported in the English literature between 1970 and 2013. Methods A MEDLINE search was conducted using the terms acute, neonatal, newborn, suppurative, bacterial, purulent, parotitis, parotid swelling, and parotid abscess, limited to the English-language literature starting from 2011. We reviewed all reported cases, together with two more managed cases in our hospital. We also describe the magnetic resonance imaging (MRI) findings of the early stage of this disease. Results We identified 26 new cases since 2011. The total number of patients reviewed was 72, including our patients. The infection was unilateral in 83% of patients, and 67% of the affected patients were males. The serum amylase levels were generally not elevated despite marked parotid swelling. Of the causative agents of this disease, 65% were Staphylococcus aureus, of which 19% were methicillin-resistant Staphylococcus aureus. As the rate of cesarean section was high in patients with this disease, it was considered a risk factor. The diffusion-weighted MRI images showed multiple punctate hyperintensity regions with reduced apparent diffusion coefficient, suggesting microabscess formation in the affected gland. Conclusions Acute suppurative parotitis should be considered in cases of swelling and tenderness in the parotid gland during the neonatal period. Multiple punctate hyperintensities in the parotid gland on the diffusion-weighted images may indicate a retrograde bacterial infection from the Stensen's duct.

INTRODUCTION Bullous pemphigoid is an autoimmune bullous disease characterized by subepidermal separation. We encountered cases of bullous pemphigoid confirmed by direct immunofluorescence study but demonstrating spongiotic dermatitis without subepidermal clefting. Many of them occurred in volar sites, mimicking dyshidrotic dermatitis. METHODS We retrospectively collected patients who were pathologically and/or immunopathologically diagnosed with bullous pemphigoid from 2002 to 2017. Patients who presented with prominent spongiosis without subepidermal clefting were included and compared with patients who were diagnosed with dyshidrotic dermatitis. RESULTS A total of nine cases of spongiform pemphigoid out of 385 bullous pemphigoid patients (2.3%) were identified and compared with 15 patients with dyshidrotic dermatitis. Granulomatous lobular mastitis (GLM) and mammary duct ectasia (MDE) are inflammatory diseases. However, only a limited number of studies have focused on characterizing their clinicopathological features. The aim of the present study was to investigate the etiology, clinicopathological characteristics and diagnosis of GLM and MDE. The clinical information and treatment of 118 female patients with pathologically-proven GLM or MDE were retrospectively analyzed in the present study. Munro's Microabscess (MM) is the diagnostic hallmark of psoriasis. Neutrophil detection in the Stratum Corneum (SC) of the skin epidermis is an integral part of MM detection in skin biopsy. The microscopic inspection of skin biopsy is a tedious task and staining variations in skin histopathology often hinder human performance to differentiate neutrophils from skin keratinocytes. Motivated from this, we propose a computational framework that can assist human experts and reduce potential errors in diagnosis. The framework first segments the SC layer, and multiple patches are sampled from the segmented regions which are classified to detect neutrophils. Both UNet and CapsNet are used for segmentation and classification. Experiments show that of the two choices, CapsNet, owing to its robustness towards better hierarchical object representation and localisation ability, appears as a better candidate for both segmentation and classification tasks and hence, we termed our framework as MICaps. The training algorithm explores both minimisation of Dice Loss and Focal Loss and makes a comparative study between the two. The proposed framework is validated with our in-house dataset consisting of 290 skin biopsy images. Two different experiments are considered. Under the first protocol, only 3-fold cross-validation is done to directly compare the current results with the state-of-the-art ones. Next, the performance of the system on a held-out data set is reported. The experimental results show that MICaps improves the state-of-the-art diagnosis performance by 3.27% (maximum) and reduces the number of model parameters by 50%.

Mast cells in the brain are associated with increased inflammation during the acute period following exposure to infection; these cells are important for destroying the infectious agents. We investigated the relation between histopathological lesions and mast cells in sheep brains infected with Listeria. Pons and medulla regions from 17 infected and eight normal sheep brains were examined. Microabscesses and perivascular infiltration were assessed for histopathology. Mast cells were identified using toluidine blue and Listeria monocytogenes were investigated immunohistochemically. We found a significant increase in mast cells in infected sheep brains that was related directly to the extent of brain lesions. A strong correlation was found between mast cells and microabscess formation. A correlation between bacteria level and brain lesions also was observed, but not between bacteria level and mast cells. Our findings indicate that mast cells are increased following Listeria infection in sheep in proportion to the severity of brain lesions; the increase may contribute to acute inflammatory reactions and also may destroy bacteria directly.

Background: In the pandemic era of Coronavirus disease 19 (COVID-19), one of the most important issues is the nature of real pathological events that occur during disease course in different parts of the body. There are several ways to know more about COVID-related histopathological events,such as tissue sampling which means biopsy from the tissues of either livepeople or necropsy/autopsy of people who died from COVID-19. Methods: We conducted an original study for assessing histopathological findings of lung necropsy samples collected from 15 Iranian patients.The continuous variables were presented as mean and standard deviation, and for the qualitative data on histopathological findings, the percentage or qualitative scores (0 to +3) were used. Results: We found similar presentations of COVID-related histopathologic events regarding percentage and severity in pulmonary tissue, includinglymphocytic infiltrations, inflammatory infiltrations of septal and perivascular areas, desquamated type2 pneumocytes, hyaline membrane changes, fibrin material depositions, abnormal changes of alveolar capillaries, presence of megakaryocytes, PMN infiltrations, septal necrosis, microabscess formation and bacterial colony formation. Also, we found few interesting features which were not completely compatible with previous similar studies or newly reported by ours asa high percentage of anthracosis (86%: 13 patients) that was not clearly reported in other previous studies, also a lower percentage of microthrombotic vascular lung injuries (20%: 3 patients), and a higher percentage of viral cytopathic effects (27%: 4 patients). Conclusion: This article suggests a greater need for evaluatingthe autopsy samples of COVID-19 patients to provide better management strategies and propose the question of whether anthracosismay be a mortality risk factor in COVID-19 patients.

Ascariasis, which is caused by Ascaris lumbricoides, is the most common gastrointestinal parasitic infection worldwide, with occasional invasion of the biliary tract leading to a variety of complications. In rare cases, pathogens carried on the surface of A. lumbricoides can complicate the course of the disease and lead to superimposed bacterial infections. In this article, we present a case of ascariasis-induced cholangitis complicated with Klebsiella pneumoniae bacteremia and multiple hepatic microabscesses. This article, which shows an association that was not reported in the literature before, aims to increase the awareness of clinicians for the possibility of the association between ascariasis and superimposed bacterial infection, specifically with K. pneumoniae.

In a 68-year-old male patient with cholangitis microabscess in the liver, cavitary lung cancer of the right lower lobe was incidentally diagnosed. The patient's medical history comprised totally laparoscopic distal gastrectomy (TLDG) and cholecystectomy 9 years ago. Before TLDG, endoscopic retrograde cholangiopancreatography (ERCP) was performed because of a common bile duct stone and cholangitis. Three months ago, he was readmitted with cholangitic microabscess, and a cavitary lesion of right lower lobe was detected incidentally. Hepatobiliary microabscess was improved with ERCP and antibiotic treatment. Video-assisted thoracoscopic surgery (VATS) right lower lobectomy and mediastinal lymph node dissection were undergone uneventfully. Pneumonic infiltration in the right lower lung field and hydropneumothorax on the right side were developed on postoperative day 10. Chest computed tomography showed pneumobilia and micro-bronchopleural fistula (BPF) was suspected with bronchoscopic examination. Despite antibiotics usage and thoracic suction, air leakage could not be improved. VATS Talc pleurodesis was performed on postoperative day 38. Contralateral pneumonia developed 1 week following talc pleurodesis. Furthermore, bilirubin was detected in the repeated bronchial washing and pleural fluid and they showed the same color and nature. Though ERCP and endoscopic nasobiliary drainage, the patient's condition deteriorated and the patient passed away on postoperative day 60. Bronchopleurobiliary fistula is an extremely rare complication after thoracic surgery for which surgical treatment is challenging, early recognition of a developing fistula and the aggressive treatment at an early stage can reduce the associated morbidity and mortality from the sequelae of this rare disease entity.

Background Amiodarone is an antiarrhythmic drug that has been recognized to induce hepatotoxicity. We report a case of acute-on-chronic liver failure (ACLF) in a patient who was receiving amiodarone for more than 2 years. The patient developed cirrhosis and suppurative microabscesses of the liver and died of progressive liver failure. Case presentation A 69-year-old woman with risk factors for nonalcoholic fatty liver disease (NAFLD) was treated with oral amiodarone at a daily dose of 400 mg for more than 2 years, until she developed epigastralgia and vomiting. Initial laboratory findings included leukocytosis and elevated liver enzymes. Images of abdominal computed tomography scan revealed diffusely increased hepatic attenuation density (in contrast to decreased density in NAFLD), hepatomegaly, periportal edema, and ascites. Liver biopsy targeting the hotspot identified through positron emission tomography confirmed the diagnosis of amiodarone-associated chronic steatohepatitis and superimposed microabscesses. The patient died of progressive ACLF despite intensive supportive care. Conclusion Accumulation of amiodarone can result in chronic liver disease and pose an additional risk of ACLF following infection.

Extranodal natural killer/T-cell lymphoma is a rare Epstein-Barr virus-associated aggressive lymphoma commonly involving the nasal and upper aerodigestive tract and, less commonly, extranodal sites such as skin and gastrointestinal tract. There is only one case report of paraneoplastic myositis associated with this type of lymphoma. Here in we present a second case of paraneoplastic eosinophilic myositis seen in a patient with extranodal NK/T-cell lymphoma. Biopsy of one of the cutaneous nodules showed necrosis of the soft tissue and skeletal muscle with eosinophilic microabscess formation, along with peripheral blood eosinophilia. These nodules and the peripheral blood eosinophilia resolved with treatment of the cutaneous lymphoma.

Background The widespread administration of the Haemophilus influenzae type b vaccine has led to the predominance of non-typable H. influenzae (NTHi). However, the occurrence of invasive NTHi infection based on gynecologic diseases is still rare. Case presentation A 51-year-old Japanese woman with a history of adenomyoma presented with fever. Blood cultures and a vaginal discharge culture were positive with NTHi. With the high uptake in the uterus with 67Ga scintigraphy, she was diagnosed with invasive NTHi infection. In addition to antibiotic administrations, a total hysterectomy was performed. The pathological analysis found microabscess formations in adenomyosis. Conclusions Although NTHi bacteremia consequent to a microabscess in adenomyosis is rare, this case emphasizes the need to consider the uterus as a potential source of infection in patients with underlying gynecological diseases, including an invasive NTHi infection with no known primary focus.

Tuberculosis (TB) is a major public health problem in developing countries. It can affect any organ of the body, and can have a multitude of clinical presentations. We present the case of a 22-year-old male who presented with fever, abdominal pain, and weight loss, and was found to have enlarged liver and spleen, both studded with multiple microabscesses. He had deranged liver functions, mild anemia, and elevated acute phase reactants. Examination of the aspirate from the liver did not reveal any organism on microscopy or culture. Based on the demographics, history of contact with a TB patient, positive Mantoux test, and clinical and radiological features, the patient was successfully managed with empirical treatment for TB.

Background and Purpose: Fungal renal abscesses are rare entities associated with significant morbidity and mortality. Affected kidneys can have microabscess, pyelonephritis, pyonephrosis, or papillary necrosis. Case report: Herein, we reported an unusual case of a large renal abscess cause by Candida tropicalis in a diabetic patient. The entity presented as a lump in the abdomen and later was diagnosed to be an abscess on computed tomography scan. Candida tropicalis was confirmed on the culture of the aspirate. The abscess was successfully treated by percutaneous drainage and administration of amphotericin B deoxycholate. Conclusion: Candida tropicalis is now a global concern because of its rising prevalence and high virulence. The growing resistance of this Candida species to azoles, as in our case, calls for a more judicious usage of antifungal agents. Empirical therapy with either amphotericin or echinocandins is an option in case of high azole resistance. This case highlights the importance of timely diagnosis and implementation of aggressive management in cases suffering from fungal abscesses.

Endometritis is subdivided into two categories. Acute endometritis is symptomatic and characterized by microabscess formation and neutrophil invasion in the endometrial superficial epithelium, gland lumina, and uterine cavity. Chronic endometritis is rather silent and recognized as unusual plasmacyte infiltration in the endometrial stromal areas. Over the last decade, studies have disclosed the potential association between poor reproductive outcomes and endometritis, particularly chronic endometritis. The aim of this review is to address the current literature surrounding chronic endometritis and highlight recent advances in the research of this long-neglected gynecologic disease.

Background Various pathological findings have been reported in pigmented purpuric dermatosis, but their clinical significance remains unclear. Methods We retrospectively reviewed demographics, clinical presentations, pathological patterns and concurrent systemic diseases among biopsy-confirmed cases of pigmented purpuric dermatosis. Results A total of 107 cases were ascertained. Five major pathological patterns were identified: lichenoid (45/107, 42.1%), perivascular (40/107, 37.4%), interface (11/107, 10.3%), spongiotic (7/107, 6.5%) and granulomatous (4/107, 3.7%). Lymphocytic vasculitis was present in 17 patients (15.9%), and Langerhans cell microabscess was seen in 4 (3.7%). Nine patients had partial features mimicking mycosis fungoides but none were confirmed. The lichenoid, perivascular and spongiotic patterns correlated to lichen aureus, Schamberg and eczematoid clinical variants, respectively. The interface pattern was associated with a higher risk of coincident autoimmune diseases (18.2%, P = .0280) and gout (27.3%, P = .0180). Conclusions This study described the wide pathological spectrum of pigmented purpuric dermatosis among Asians. Physicians should be aware about the clinical and pathological variations to facilitate diagnosis.

Patients with eosinophilic pustular folliculitis (EPF), a sterile eosinophilic infiltration of hair follicles, often present with papulopustules that tend to form annular plaques. Histopathologic examination revealed eosinophilic infiltration around the pilosebaceous units and eosinophilic microabscess formation. Although the pathogenesis of EPF is unknown, T-helper type 2 immune responses were suggested to be important based on their stimulating effect on the sebaceous glands. Here, we report the first case of EPF associated with herpes zoster, indicating that herpes zoster and EPF are correlated with T-helper type 2 immune responses.

Acne keloidalis (AK) is one of the primary cicatricial alopecias and predominantly affects men of African descent. Reports in Asians are scant. This study aimed to retrospectively review the clinical and histopathological features of AK patients in southern Taiwan and identify the pathognomonic features of AK. There were 15 patients with histopathologically confirmed AK in National Cheng Kung University Hospital between 1988 and 2018. The median onset age was 24 years (range, 14-71). The male : female ratio was 14:1. In the acute stage of AK, the lymphocytic and neutrophilic peri-infundibular inflammatory infiltrates with microabscess formation and edema corresponded to the clinical finding of isolated papules or pustules. Subsequently, the inflammatory infiltrates involved the mid-dermis and the isthmus of hair follicles. The "spade sign", a thin and dilated space resembling the shape of a balloon or spade symbol of playing cards at the level of lower isthmus, was identified in eight biopsies from five patients and may be a pathognomonic sign in the subacute stage of AK. At the chronic stage, the segments of hair shafts remained in the upper to mid-dermis and induced chronic inflammation and extensive fibrosis, resulting in the clinical keloid-like appearance. The restriction of inflammation and fibrosis in the upper to mid-dermis was another unique and pathognomonic feature of AK.

Buschke-Lowenstein tumor (BLT) is a rare sexually transmitted disease, mostly described in clinical literature as case reports or small series. Here, we investigated the clinicopathologic features of BLT in a total of 38 cases retrieved from multiple academic institutions. The average age was 47.6 ± 12.8 (mean ± SD) years old at diagnosis. The male to female ratio was 4.4:1. Common presenting symptoms were pain/discomfort, bleeding, mass lesion, and discharge. It was frequently linked to smoking and positive human immunodeficiency virus status. The tumor size and thickness were 8.5 ± 6.6 cm and 1.5 ± 1.3 cm, respectively. Histologically, 19 (50%) cases had an invasive squamous cell carcinoma component and were associated with high-risk human papillomavirus infection. There was no lymphovascular or perineural invasion, or nodal metastasis at initial diagnosis. BLTs with invasion had higher frequency of dyskeratosis, neutrophilic microabscesses, and abnormal mitoses, but lower frequency of pushing border compared with BLTs without invasion. All patients underwent wide excision, and some also received chemoradiation therapy. After a median follow-up of 23 months (range 1-207), the recurrence rate was 23.7% and disease-specific mortality was 2.6%. In summary, we presented the largest case series of BLT to date to characterize its unique clinicopathologic features. Our study indicated that certain histologic features such as dyskeratosis, neutrophilic microabscess, and abnormal mitosis in the non-invasive portion may be important clues on lesional biopsy to predict the presence of underlying invasive carcinoma.

Idiopathic granulomatous mastitis (IGM) is a rare chronic inflammatory condition of the breast which although benign can mimic carcinoma. Establishing a diagnosis can be challenging and requires a high index of suspicion with exclusion of infective and autoimmune breast diseases. IGM is characterized histologically by noncaseating granulomas which are of a lobulo-centric pattern and often associated with microabscess formation. Management of confirmed cases remains controversial with proponents of initial surgical or medical therapies – each has its associated problems which can be worse than the original symptoms of IGM. However, many patients require more than one modality of treatment to completely resolve IGM lesions and careful judgment is necessary to ensure optimal type and sequencing of treatments.

Nous rapportons le cas d’une patiente de 13 ans d’origine rurale qui présentait une lésion papulo-nodulaire de 4/3cm du tiers inférieur de la jambe droite, évoluant depuis deux années. La biopsie cutanée montrait une hyperplasie papillomateuse avec des micro-abcès à polynucléaires neutrophiles et des spores. L’étude mycologique objectivait des corps fumagoides à l’examen direct et isolait Fonsecaea Pedrosoi à la culture. La patiente a bénéficié d’un traitement médico-chirurgical (terbinafine 250mg/j pendant 6 mois + exérèse puis greffe cutanée) avec une bonne évolution, le recul actuel étant de deux années sans récidive. La chromomycose est une infection fongique chronique de la peau, fréquente en zones tropicales et subtropicales et rare au nord de l’Afrique. Au Maroc, jusqu'à juin 2014, sept cas seulement ont été rapportés dans la littérature. Elle est contractée par inoculation du germe après contact avec le sol ou les matières organiques. Les agents responsables sont des champignons pigmentés du groupe des dématiés. Compte tenu de sa rareté, elle peut mimer d’autres dermatoses comme la leishmaniose ou la tuberculose. Malgré la rareté de cette infection, il faut y penser devant des lésions cutanées chroniques (verruqueuses, végétantes, nodulaires, en plaque) surtout si elles sont localisées au niveau des zones exposées à d’éventuels traumatismes végétaux comme les membres inférieurs. Ceci dit, la place de l’étude mycologique parait indéniable pour confirmer le diagnostic. Le traitement de choix est la chirurgie ou l’association chirurgie et antifongiques systémiques car l’utilisation des antifongiques seuls peut se solder par des résistances ou des récidives.

Mammary disorders in mares are rare and usually only one animal per paddock is affected. In this report, two mares with 7 and 9 years old, were concomitantly diagnosis of chronic pyogranulomatous mastitis, associated with the Splendore-Hoeppli reaction, indicative of botryomycosis a rare, chronic suppurative disease with microabscess formation, characterized by in vivo formation of eosinophilic materials around microorganisms or biologically inert material. Various bacteria can cause botryomycosis in horses, and the most frequently isolated one is Staphylococcus sp., particularly S. aureus. This report confirms the role of Staphylococcus sp.; however, specifically S. hyicus and S. cohnii that prior to the current report, had not been associated with cases of botryomycosis. RESUMO Enfermidades mamárias em éguas são raras e, normalmente, apenas um animal é afetado. Neste relato, duas éguas, de sete e nove anos, foram diagnosticadas concomitantemente com mastite crônica piogranulomatosa, associada com reação de Splendore - Hoeppli, indicativa de botriomicose, uma doença crônica supurativa rara, com formação de microabscessos caracterizados pela presença de material eosinofílico em torno dos microrganismos ou de material biologicamente inerte. Várias bactérias podem causar botriomicose em cavalos, sendo Staphylococcus sp., particularmente S. aureus, as mais frequentemente isoladas. Este relato confirma o papel do Staphylococcus sp, no entanto este é o primeiro relato em que S. hyicus e S. cohnii foram identificados em lesões relacionadas à botriomicose.

This paper presents an approach for automatic detection of Munro’s Microabscess in stratum corneum (SC) of human skin biopsy in order to realize a machine assisted diagnosis of Psoriasis. The challenge of detecting neutrophils in presence of nucleated cells is solved using the recent advances of deep learning algorithms. Separation of SC layer, extraction of patches from the layer followed by classification of patches with respect to presence or absence of neutrophils form the basis of the overall approach which is effected through an integration of a U-Net based segmentation network and a capsule network for classification. The novel design of the present capsule net leads to a drastic reduction in the number of parameters without any noticeable compromise in the overall performance. The research further addresses the challenge of dealing with Mega-pixel images (in 10X) vis-a-vis Giga-pixel ones (in 40X). The promising result coming out of an experiment on a dataset consisting of 273 real-life images shows that a practical system is possible based on the present research. No members of the freshwater ascomycetes family Lindgomycetaceae have been associated with human infections. We isolated a mould (HKU35(T)) from the biopsy specimen of a patient with invasive foot infection and underlying immunoglobulin G4-related sclerosing disease. Histology showed florid, suppurative, granulomatous inflammation in the dermis, with central microabscess formation surrounded by epithelioid histiocytes, scattered giant cells, and a small number of lymphocytes. A Grocott stain revealed fungal elements in the center of the lesion. On Sabouraud glucose agar, HKU35(T) grew as gray and velvety colonies. Among the members of the family Lindgomycetaceae, HKU35(T) was the only strain that grew at 37°C. Microscopically, only sterile mycelia, but no fruiting bodies, were observed. HKU35(T) was susceptible to itrazonazole, voriconazole, and posaconazole, which was in line with the patient's clinical response to itraconazole treatment. Internal transcribed spacer and partial 18S nuclear rDNA (nrDNA), 28S nrDNA, β-tubulin gene, and EF1α gene sequencing showed that HKU35(T) occupied a unique phylogenetic position, most closely related to but distinct from members of the genera Clohesyomyces and Lindgomyces. We propose a new genus and species, Hongkongmyces pedis gen. et sp. nov., to describe this fungus, which belongs to the family Lindgomycetaceae in the orderPleosporales of class Dothideomycetes. This case also represents the first report of human infection associated with the family Lindgomycetaceae.
