11. Lawsuits after dermal filler injection registered in the state Court of São Paulo

The search for aesthetic procedures in the health sector have increased in the last years. The increase in the number of procedures brought together a higher number of lawsuits, especially when it comes to the injection of dermal fillers. This study aimed to revisit the jurisprudence of the State Court of São Paulo, Brazil, to investigate the risk factors behind the lawsuits and convictions of health care professionals after the injection of dermal fillers. The sample consisted of 46 lawsuits published between 2000 and 2022 in the State Court of São Paulo, Brazil. Data extracted from each lawsuit were: the year of publication, the reason behind the lawsuit, the defendant (individual or legal entity), sex of the plaintiff, type of dermal filler, whether there was a technical legal/forensic examination of the case and conviction. Most of the lawsuits were published between 2020 and 2022 (n = 26, 56.52%). The presence of intercurrences was the most common reason behind the lawsuits (n = 27, 58.69%). The defendants were mostly health care providers (n = 16, 34.78%), individually, and their combination with legal entities (n = 16, 34.78%), separately (the rest was legal entities alone). Female plaintiffs were predominant (n = 36, 78.26%), as well as the use of hyaluronic acid as dermal filler (n = 35, 76.08%). Technical legal/forensic examination was registered in 34 lawsuits (73.91%). Most of the defendants were convicted (n = 25, 54.34%). The chance of conviction was 3,9 times higher if the case was judged between 2017-2019 compared to 2020-2022 (p = 0.05); and 3,4 times higher if the case was based on intercurrences (p = 0.05). It is estimated that convictions in lawsuits after dermal filler injections might decrease in more recent cases. However, higher chances of conviction might be expected in the presence of intercurrences (e.g. accidents or complications) in the State São Paulo, Brazil.

Although the acquisition of Terrace from patients in the right method does not require a high level of expertise, the interpretation of this Terrace needs adequate knowledge, proficiency, and experience. The purpose of this study was to examine the effect of using the checklist in the ECG interpretation by medical interns.Methods: The present cross-section descriptive study was carried out on medical interns of Mashhad University of Medical Sciences in 2015. 40 students who were attending a one-month emergency medicine course were randomly divided into two groups of 20. In one group, 9 standard tracings classified with equal difficulty level (easy, medium, and hard) with a standard checklist form and a questionnaire for each were completed and in the other group, the same tracings of the first group were first handed without checklists and then handed with checklists for the second time. Finally, the scores of completing the checklists and the correctness of tracing interpretations were recorded in both groups. Data analysis was done using descriptive and inferential statistical tests.Results: There was no statistically significant difference between the two groups in terms of baseline variables. The first group identified 41.6% of the terraces correctly using the checklist. The second group, without using the checklist, correctly identified 25.5% of the terraces; and after using the checklist, this indicator increased to 32.7% in this group. Considering at least three correct responses in the ECG interpretation as “desirable response”, it was found that 50% of the subjects in the first group (using the checklist) (n=10) and only 15% (n=3) of the second group (without using the checklist) had desirable responses (p = 0.531). On the other hand, the comparison of responses before and after the use of the checklist in the second group showed a significant improvement in the number of desirable responses (15% (n=3) versus 25% (n=5), p = 0.009).Conclusion: The use of a checklist for the ECG interpretation by interns of emergency medicine did not affect improving the accuracy of the interpretation than the object-oriented system, but was effective in the diagnostic review and confirmation step.Keywords: checklist, electrocardiogram, medicine

Due to the importance of solving the low academic performance of medical students, a constant problem in higher education, and which is a widely researched multifactorial variable, the following research work was proposed to determine the effectiveness of academic advising to improve the learning of Biochemistry and Molecular Biology in students of the medical career. An observational, analytical, prolective and longitudinal study was designed, which included a total of 64 first-semester students who studied the subject of Biochemistry and Molecular Biology in the medical career who decided to participate in the subject's consultancies. As a result, it was observed that there was a statistically significant difference between the averages compared before and after the assessments, in favor of the final evaluation in which an average of 71.52 was obtained. It was evidenced that the total of students that accredited and the Coefficient of determination was calculated, which indicated that the learning of Biochemistry and Molecular Biology depends on 53.2% of the academic advising. Therefore, it can be concluded that academic advising is effective to improve the learning of Biochemistry and Molecular Biology in medical students.

Background: Facebook, as the world’s most popular social media platform, has been playing various important roles throughout the COVID-19 pandemic, allowing users to produce and share health-related information that both eases and complicates public health communication. However, the characteristics of vaccine-related Facebook content and users’ reaction to the vaccine issue has been an unexplored area to date. Methods: To fill the previous knowledge-gap, this exploratory study wants to understand the communication climate of Facebook on the COVID-19 vaccine issue, including the nature of dominant content and users’ engagement patterns with them. Therefore, the study analyzes the 10,000 most popular Facebook posts with the highest interactions on the vaccine issue. Results: The results show that Facebook users prioritize more vaccine-related news links (71.22%) over other content. The declining interactions on the issue suggests that interaction growth mainly depends on positive news on the vaccine. Finally, users’ reaction to the vaccine issue is dominantly positive, though they may show a highly negative attitude toward vaccine misinformation. Conclusions: A few limitations and strengths of this study are discussed along with values and implications. This study for the first time analyzes Bangla language-based Facebook content related to the COVID-19 vaccine issue, which is largely overlooked in global academic research.

Published 3D printing activity in general thoracic surgery is scarce and has been mostly limited to case reports. The aim of this report was to reflect on the results and lessons learned from a newly created multidisciplinary and multicenter 3D unit of the Spanish Society of Thoracic Surgery (SECT).Methods: This is a pilot study to determine the feasibility and usefulness of printing 3D models for patients with thoracic malignancy or airway complications, based on real data. We designed a point-of-care 3D printing workflow involving thoracic surgeons, radiologists with experience in intrathoracic pathology, and engineers with experience in additive manufacturing.Results: In the first year of operation we generated 26 three-dimensional models out of 27 cases received (96.3%). In 9 cases a virtual model was sufficient for optimal patient handling, while in 17 cases a 3D model was printed. Per pathology, cases were classified as airway stenosis after lung transplantation (7 cases, 25.9%), tracheal pathology (7 cases, 25.9%), chest tumors (6 cases, 22.2%) carcinoid tumors (4 cases, 14.8%), mediastinal tumors (2 cases, 7.4%) and Pancoast tumors (one case, 3.7%).Conclusion: A multidisciplinary 3D laboratory is feasible in a hospital setting, and working as a multicenter group increases the number of cases and diversity of pathologies thus providing further opportunity to study the benefits of the 3D printing technology in general thoracic surgery.Keywords: 3d printing, thoracic surgery, multidisciplinary group, preoperative study

Education modernization in the healthcare of Ukraine has formed new approaches to the training of the young professionals. The article describes the methods of the active teaching in the medical university clinical departments. Traditional lecturing used in teaching has the lowest retention rate; the use of simulation technique education as part of an instruction method may enhance retention and reinforce learning by creating a dynamic educational environment. The aim of the study was to analyze, generalize existing points of view in the education system, which affect the issue of necessity, effectiveness and features of the simulation programs use for medical students in the example of «Standardized patient» methodic. Object and methods. It were held an overview of modern literature on the implementation of simulation method in way of the «Standardized patient» methodic in medical education. Seven databases were used in the search: ERIC, Education Research Complete, Medline, Medline Complete, Academic Search Complete, The Cochrane Library and PubMed in period of 2007-2018. Using key words: standardized patient, students, active teaching method, professional training, medicine. Results and discussion. Students of medical universities often feel lack the experience of interacting with real patients. By mastering realistic scenarios with a standardized patient (SP), they accumulate experience, and teachers gain an opportunity to evaluate how students apply new knowledge in practice. Interaction with SP develops communication and professional skills, enabling students to get their first clinical experience. Using SP, students learn to collect anamnesis from a patient, conduct a physical examination on structured and effective principles, learn to ask questions related to medical, surgical and social anamnesis, make objective examination, estimate additional methods of investigation. SPs also help students of the senior courses to ensure integration of curriculum content in practical activity. There is a possibility apply their theoretical knowledge in practice, using them as tools in achieving a specific goal helping a sick person. This method of simulation training is developed to involve university staff as «standardized patients» for modeling clinical situations of differentiation and assistance in various diseases. The approximate structure and rules for writing a clinical scenario for SP are proposed. Conclusion. SP methodic is one of the effective ways to learn how to manage errors and how to work in accordance with modern algorithms. The advantage of this technique is that the student can really assess the level of his training, identify gaps in knowledge and skills, and make timely attempts to eliminate them. The inclusion of SP methodic in an educational program is a very effective teaching method.

Flos Magnoliae is one of the important medicinal plants in different traditional medicine, including Chinese herbal medicine. Lignans and neolignans, including tetrahydrofurofuran, tetrahydrofuran, and aryltetralin, are present in the Flos Magnoliae species. A wide range of pharmacological activity of Flos Magnoliae has been reported in medicine. Fargesin has been isolated from Magnolia fargesii and it is a lignan-class phytochemical. Fargesin has numerous pharmacological activities in medicine, including its effectiveness on lipid and glucose metabolism, oxidative stress, myocardial apoptosis, etc. In the present work, we have summarized the detailed scientific information of fargesin concerning its medicinal properties and pharmacological activities. Numerous biological and chemical aspects of fargesin are discussed here, including the detailed pharmacological activities and analytical aspects of fargesin. In this review, we have also compiled analytical data on fargesin based on available scientific literature. Ethnopharmacological information on fargesin was gathered by a literature survey on PubMed, Science Direct, Google, and Scopus using the terms fargesin, Flos Magnoliae, phytochemical, and herbal medicine. The present review paper compiled the scientific data on fargesin in medicine for its pharmacological activities and analytical aspects in a very concise manner with proper citations. The present work signified the biological importance of fargesin in medicine due to its significant impact on bone disorders, lung injury, colon cancer, atherosclerosis, neurological disorders, ischemia, sars-cov-2, allergy, lipid and glucose metabolism, melanin synthesis, and different classes of enzymes. Furthermore, fargesin also has anti-inflammatory, antihypertensive, antiprotozoal, antimycobacterial, and antifeedant activity. However, analytical methods used for the separation, identification and isolation of fargesin in different biological and non-biological samples were also covered in the present review. Objectives: Educational programs based on high-fidelity simulation training aim to promote students' acquisition of nontechnical competencies such as understanding crisis resource management (CRM). This study evaluated the efficacy of a CRM course for students in their last year of university studies in health sciences. The course was developed by the Spanish Society of Emergency Medicine (SEMES)., Material and Methods: Quasi-experimental study of a high-fidelity simulation course to teach emergency CRM (E-CRM) using preand postcourse measures of achievement in a single student cohort. A total of 209 students completed 2 selfadministered self-efficacy evaluations of their acquisition of nontechnical competencies and resilience. External observers also assessed the students' nontechnical competencies with objective measurement scales., Results: Scores on resilience and self-efficacy assessments improved through the intervention (F = 25.90 and F = 68.02, respectively; P .001, for both pre-post comparisons). Statistically significant differences were found between students in different health sciences at baseline (t = 2.67; P = .008). Scores improved significantly on the Mayo High Performance Teamwork Scale (F = 6.18, P .001, eta2 = 0.20) and the Ottawa CRM Global Rating Scale (F = 5.58; P .005, eta2 = 0.19)., Conclusion: The E-CRM course developed by a coordinated multiprofessional team based on high-fidelity simulations improved self-efficacy assessments of resilience and all nontechnical competencies.

Background: The majority of cardiovascular disease patients require life-long treatment and regular monitoring. The objective of the study was to evaluate the hospital care in cardiovascular disease patients in Faisalabad Institute of Cardiology, Faisalabad. Material & Methods: This cross-sectional study was conducted in the Faisalabad Institute of Cardiology (FIC), Faisalabad, Pakistan from January to June 2016 after approved by ethical committee. A total of 150 patients were selected through convenient sampling technique. Adult admitted cardiac patients of both genders were included. Emergency cases were excluded. Demographic variables were sex, age groups, residence and economic status. Research variables were diagnosis, mode of payment, lab investigation and staff’s communication. Study instruments was a pre validated standardized questionnaire. All the variables being categorical were analyzed by frequency and percentages by using SPSS version 21. Results: Out of 150 patients, 9.3% were 65 years of age. Mostly were male 54.7% from rural area 44.7% and 73.3% were having less monthly income. Myocardial Infarctions cases were 28%, Coronary Artery Bypass Grafting (CABG) were 26%, Mitral Valve Regurgitation 26% and Acute Coronary Syndromes (ACS) were 20%. Mostly payment for treatment paid by the government funds 75.3%. Conclusion: Most of the cardio vascular disease patients were urban males, 45-55 years of age and of lower income group. Majority were suffering from myocardial infarction. Common mode of payment was through government. Communication of hospital staff with the patients was good.

Introduction: In addition to precise physical examination and objective observations, it is necessary for students and interns to be knowledgeable about the rules and religious orders in medicine. This study was performed to investigate Yazd interns' knowledge on religious orders and legal rules in medicine. Methods: In this descriptive study performed in educational health centers of Yazd, 120 interns selected through census sampling were asked about religious orders and legal rules in medicine using a researcher made questionnaire. Data was analyzed by SPSS software using descriptive statistics and chi² test. Results: Fourteen point three percent of interns had knowledge about religious orders and legal rules Among which, 11.8% had knowledge about legal rules and 14.6 about religious orders. Seventy two percent had faced with cases about which did not know the related rules. Eighty three point two percent believed that academic courses of medical education did not provide enough information to resolve many problems. Level of their knowledge showed no significant relationship with demographic features. Conclusion: Interns' knowledge about religious orders and legal rules was in an average level. It is recommended to increase the content of medical curriculum regarding medical ethics and related issues.

The white-tailed deer (Odocoileus virginianus) is an important resource for rural communities for the goods it contributes to peasant and indigenous families. The objective of the research was to analyze the traditional knowledge and nutritional value of O. virginianus in the community of Pitzotlán, Morelos, Mexico. In-depth interviews were applied to eleven hunters, participant observation and guided tours. For the nutritional analysis of venison, techniques from the Official Association of Analytical Chemists (AOAC International) were used. Local hunters recognize different aspects of the biology of the species such as feeding habits, sexual dimorphism, breeding season and social behavior. Three hunting techniques are recorded: "arreadas" (100 %), "espiadas" (100 %) and "lampareadas" (54 %), which are practiced in agricultural and forest areas of low deciduous forest. The "arreadas" as a collective practice for hunting deer, are conceived as spaces for recreation and community socialization. The categories of use registered were in the following proportions: food, medicinal and ornamental (100 %), tool (45 %), amulet (10 %) and sale (10 %). The main forms of preparation were "guaxmole", barbecue, "asadura", dried meat, fried meat, and steaks. Venison meat (dry basis) reported 75.14 g / 100 g of protein and 7.88 g / 100 g of total minerals. The white-tailed deer is an important wildlife resource for the rural families of Pitzotlán because the traditional knowledge about its appropriation has a role in their diet, health, worldview, and community life., El venado cola blanca (Odocoileus virginianus), es un recurso importante para las comunidades rurales por los bienes que aporta a las familias campesinas e indígenas. El objetivo de la investigación fue analizar el conocimiento tradicional y el valor nutricional de O. virginianus en la comunidad de Pitzotlán, Morelos, México. Se aplicaron entrevistas a profundidad a once cazadores, observación participante y recorridos guiados. Para el análisis nutricional de la carne de venado se emplearon técnicas de la Asociación Oficial de Químicos Analíticos (AOAC International). Los cazadores locales reconocen diferentes aspectos de la biología de la especie como hábitos alimentarios, dimorfismo sexual, época de reproducción y comportamiento social. Se registran tres técnicas de cacería: “arreadas” (100 %), “espiadas” (100 %) y “lampareadas” (54 %), las cuales se practican en áreas agrícolas y forestales de selva baja caducifolia. Las “arreadas” como practica colectiva para la cacería del venado, son concebidas como espacios de recreación y socialización comunitaria. Las categorías de uso registradas fueron en las siguientes proporciones: alimentario, medicinal y ornamental (100 %), herramienta (45 %), amuleto (10 %) y venta (10 %). Las principales formas de preparación fueron el “guaxmole”, barbacoa, “asadura”, carne seca, carne frita y bistecs. As a new type of probe, the heavy isotope of water has been widely used in the study of cell development, metabolism, tissue homeostasis, aging, and tumor heterogeneity. Womens’ representation in most of the professions in India was been dismally low before the country gained independence. This can be attributed to the customs and traditions which have been in vogue through the centuries. Women’s accepted role within the confines of the home and seclusion from males other than the family members were two reasons which contributed to women not being part of many professions. Medicine as a profession reflected this and thus the representation of women in medicine was insignificant in the 19th and the first half of the 20th centuries. However certain measures during the period began the process of entry of women into the profession. Opening of medical colleges and schools was an important step, although women were allowed to join in meagre numbers. Later, medical colleges only for women opened, which forced other medical colleges to admit women. This was because of the setting aside of special funds by the government for this cause and the role of women missionaries who believed it a moral duty to induct women into the medical profession in order to better serve women patients. These factors were helpful for women trying to get a foothold in the profession. The pioneering efforts of a few women doctors added impetus to this activity. It can be said with pride that there may now be more women than men enrolled in medical colleges. There are still challenges such as the unequal representation of women in all disciplines, but to change this will require a change in the mind set of Indian society rather than the enacting of any law . Empowerment of women is a widely researched topic, and this study differs country wise and profession wise from other studies. It can be presumed that the medical profession, because of its inherent characteristics, is looked upon as a noble profession and thus it might be expected to lead other professions in empowerment. Drawing on our shared experience of a mixed-methods evaluation of a rehabilitation scheme for older people in six residential care homes in Nottingham, this paper examines the challenges and rewards of interprofessional research. A social anthropologist, social work academic and medical academic respectively, we explore understandings and misunderstandings that arose during the two-year research process as we sought to integrate quantitative and qualitative findings from a randomized controlled trial (RCT) and an ethnographic study conducted in parallel. We identify ways in which it was possible to overcome the danger of fragmentation, which we argue may be inherent in interprofessional research. This includes a recognition that researchers from different disciplinary backgrounds may use different “key symbols” to view the world but that professional knowledge and expertise may enable researchers to transcend such disciplinary boundaries and to hear what each is saying to the other for the benefit of improved collaboration. Indigenous knowledge points to the fact that Africa has been able to generate, test and apply knowledge through its own methodologies and approaches. This knowledge sustained communities for years prior to colonialism which somehow downplayed the value of indigenous knowledge and promoted western knowledge systems. Colonialism created dependence on western knowledge systems and Africa has over the years neglected its own rich indigenous knowledge. This paper sought tohighlight challenges faced in the quest to incorporate IK into developmental goals and projects. Furthermore, the study also sought to recommend ways through which Africans can promote IK in its developmental projects. A desk research was carried out to show the challenges faced in African indigenous knowledge. The results of the study showed that there was a need for policies, legislation, standards, research capacitation, investment and local inclusion in indigenous knowledge research, documentation and use. The study also pointed to the fact that indigenous knowledge could be tapped into for developmental goals and projects.

This volume presents a set of essays that discuss the development and plasticity of the vertebrate auditory system. The topic is one that has been considered before in the Springer Handbook of Auditory Research (volume 9 in 1998, and volume 23 in 2004) but the field has grown substantially and it is appropriate to bring previous material up to date to reflect the wealth of new data and to raise some entirely new topics. At the same time, this volume is also unique in that it is the outgrowth of a symposium honoring two-time SHAR co-editor Professor Edwin W Rubel on his retirement. This fully updated and revised second edition address specifically some of the significant advances made in our understanding of the pathophysiology of penile cancer including discussion of the suspected role of the human papilloma virus in penile carcinogenesis. Similarly, the molecular pathways implicated in precancerous and penile cancer will be discussed as these are the present targets of novel therapeutic approaches to this disease. The text details the recent national and international diagnostic and treatment guidelines that have been developed by both the National Comprehensive Cancer Network and the European Urological Association establishing evidence based standards upon which the care of penile cancer should be based. The text also covers significant advances made in the diagnostic and therapeutic approach to both primary tumors and inguinal nodal metastases resulting from penile cancer such as penile sparing surgery and minimally invasive inguinal lymph node surgery. The text will similarly highlights the improvement in cancer specific outcome in the management of loco-regionally advanced (N2/3) penile cancer managed by upfront systemic chemotherapy followed by consolidative surgery in appropriately selected patients based on recent prospective phase II data. It also covers the role of radiotherapy in the management of penile cancer for both primary tumors and/or at sites of regional or distant metastases. - Abstract
This book is an informal autobiography by John West MD PhD. He obtained his medical degree in Adelaide, Australia and then spent 15 years mainly at the Royal Postgraduate Medical School, Hammersmith Hospital in London where he, with others, used radioactive oxygen-15 to make the first description of the uneven regional distribution of blood flow in the lung. In 1960-1961, he was a member of the Himalayan Scientific and Mountaineering Expedition led by Sir Edmund Hillary who had made the first ascent of Mt Everest 7 years before. During the expedition about 6 scientists spent up to three months at an altitude of 5800 m studying the effects of this very high altitude on human physiology. Because of his interests in the effects of gravity on the lung, Dr. West spent a year at the NASA Ames Research Center in Mountain View, California in 1967-1968. While there he submitted a proposal to NASA to measure pulmonary function of astronauts in space, and this was funded. Later, in 1981 he organized the American Medical Research Expedition to Everest during which the first measurements of human physiology on the summit, altitude 8848 m, were obtained. In the 1990’s, Dr. West’s team made the first comprehensive measurements of pulmonary function of astronauts in space using SpaceLab which was taken up in the Shuttle.

This volume describes the pathogenesis and pathophysiology of several pulmonary diseases as well as their treatment. It also discusses the underlying genetic and molecular biological basis, which opens the way for new treatments for these conditions. It focuses on the treatment of cystic fibrosis including CFTR (cystic fibrosis transmembrane-conductance regulator) modulator therapies, drug therapies that augment airway surface liquid as well as anti-inflammatory and anti-infective therapies. Further topics include long-term, low-dose macrolide therapy for diffuse panbronchiolitis; novel agents for previously untreatable idiopathic pulmonary fibrosis; possible new treatments for pulmonary alveolar proteinosis (PAP); and multiple novel therapeutic targets for treating lymphangiomyomatosis. Research into these conditions has led to major advances in our understanding of the underlying genetic and molecular basis of this disease, and to dramatic improvements in survival and quality of life for affected individuals.

This book is designed to meet the need for a practically oriented textbook on the rapidly growing field of continuous EEG (cEEG) monitoring. A wide range of key clinical aspects are addressed, with explanation of status epilepticus classification, criteria for institution of monitoring, seizure patterns and their recognition, quantitative EEG analysis, and neuroimaging in patients undergoing cEEG monitoring. The value of cEEG and the nature of cEEG findings in various special situations are then reviewed, covering particular pathologies, critical care considerations, and prognostication. Treatments of nonconvulsive status epilepticus (NCSE) and nonconvulsive seizures (NCS) are discussed. The concluding section is devoted to important administrative issues including billing, staffing issues, comparison of EEG machines, and information technology (IT) issues. Continuous EEG monitoring offers the only reliable means of detecting seizures that are not clinically obvious in critically ill patients. Such seizures are common: approximately 20% of patients undergoing cEEG monitoring in hospital have NCSE or NCS. Against this background, many hospitals have started to offer cEEG monitoring as a basis for delivery of appropriate treatment.
