Trench foot is considered the most common non-combat related military injury and we have known of its existance since Napoleon's time. However, this vasoneuropathy remains a poorly understood condition and, therefore, a diagnostic challenge. With the report of 2 clinical cases treated in our hospital during Filomena storm, which occurred in Spain in January 2021, with Madrid as one of the most affected areas, we intend to contribute a little more knowledge, in order to improve cases identification and the correct management that, surely will improve our patients prognosis, without forgetting preventive measures. In our experience, a rigorous diagnostic study (vascular and electroneurophysiological) and a vasodilatador treatment with sciatic popliteal block, 5-phosphodiesterase inhibitors and dihydropyridine calcium antagonists have been decisive. Introducción: la Medicina de desastres tiene como objetivo la adquisición de conocimientos para ejecutar acciones en eventos de emergencia.Objetivo: determinar las deficiencias en la impartición de la asignatura Medicina de desastres a los estudiantes extranjeros que afectan el proceso enseñanza-aprendizaje, en el Hospital Docente Clínico Quirúrgico «Joaquín Albarrán».Método: estudio descriptivo de corte trasversal, con 1034 estudiantes extranjeros durante 3 cursos. Se aplicó una encuesta, una vez finalizado cada rotación de estudiantes por la asignatura de medicina de desastre. Se empleó estadística descriptiva.Resultado: 233 estudiantes (22,1%) estaban desmotivados, 801(77,4 %) la consideró necesaria, 174 (16,8 %) estudiantes respondieron que no guardaban relación con las asignaturas del ciclo clínico. Primero auxilios y atención al paciente politraumatizado fueron los temas más importantes (99 y 98 % respectivamente). El 50 % se autocalificaron con excelente preparación.Conclusión: no se ha logrado aún la interrelación de la asignatura con el resto de las materias, ni la motivación y preparación adecuada de los estudiantes, para el desplegamiento de acciones instructivas, educativas, valorativas y desarrollador en el enfrentamiento de desastres masivos.

Foundation: in response of the health system before a hurricane, municipalities and establishments have a protagonist role. Objective: to describe the Aguada de Pasajeros municipal health system at the Irma hurricane’s path across its territory. Methods: prospective cross study, in the health and service system, developed in the Aguada de Pasajeros Municipality between September the 7th and 30th, 2017. The universe was the territory total of workers and institutions, all involved in the response to Irma’s hurricane. A documentary review was included (plans, instructions from the Defense Councils, and the Minister of Health) and participating observation. Indicators of possible impact and municipal health situation. Results: a total of 88 actions by the municipal health system; 50 during the informative phase and 9 in alert. The plan for disaster reduction was totaly achieved, to which 9 new tasks were added. There were no deaths due to the hurricane nor morbidity after the event. There was maintained assistance coverage in sites of difficult access and shelters. All the institutions maintained vitality. Conclusion: the actions developed in the municipality allowed maintaining the health situation, the comprehensive care to the people who demanded medical services during the path of the natural phenomena across the territory institutional vitality.

Background: Hurricane Denis (July 2005) affected Cienfuegos province, causing great economic lost, no life was lost though. Health sector put in practice the previewed plans, including the levelled unfolding of 11 Integral Health Brigades up to the most affected places since the beginnings of the recovering phase. Objectives: to analyze the impact of the Integral Health Brigades in the affected areas. Method: For the article it were revised the statistics, enquiries to the population and workers of the health sector carried out by each Brigade, as well as the conclusions of the experience, analyzed in a provincial meeting for this purpose. Results: There was no disease outbreak, not even changes in morbidity. The psychological affection was very little. Population expressed its satisfaction for the immediate attention offered by health system. It allowed the direct work with people of greatest vulnerability and the stratification of their management in a more integral way. The labour bettered communitarian education and health promotion. Conclusions: in the most affected areas it was a necessary and positive element to elevate integral health attention to the community as part of the support to the civil defence sector.

Se revisan los últimos trabajos publicados sobre la conciencia que se tiene acerca de la producción y diseminación de las lecciones aprendidas en desastres para la gestión del conocimiento en materia de medicina y desastres. Se destaca la influencia de las lecciones aprendidas como elemento importante en la toma de decisiones ante el manejo de los desastres. Se expone la inquietud que para el Centro Latinoamericano de Medicina de Desastres constituye la necesidad de rescatar e incentivar la producción de las lecciones aprendidas por el personal de la salud en su enfrentamiento a catástrofes y en el desempeño de sus funciones en Cuba y en Latinoamérica.This paper reviewed the last papers published about the awareness of the production and dissemination of lessons learned on disasters for the knowledge management in terms of medicine and disaster. It underlined the effect of the lessons learned as an important element in the decision-taking on disaster management. It showed the concern of the Latin-American Center of Disaster Medicine about the need of rescuing and encouraging the production of the lessons learned by the health personnel in facing up to catastrophes and in performing its work in Cuba and Latin America

Evaluar la capacidad de respuesta ante emergencias y desastres de instituciones hospitalarias de I y II nivel, de carácter público y privado del distrito de Buenaventura. Métodos: Este estudio es una investigación evaluativa de tipo cualitativo. Se aplicaron entrevistas semiestructuradas, y fue a través de la comprensión de los discursos y el diálogo con el personal de las instituciones hospitalarias y organismos de socorro, que se analizaron los Planes de Emergencia Hospitalaria de ambas instituciones de salud, al igual que el Plan Distrital de Gestión del Riesgo. La unidad de análisis del presente estudio se concentró en la Red de Atención en Salud en Emergencias y Desastres en el Distrito de Buenaventura, identificada gracias al método de Georreferenciación. Los participantes de esta investigación fueron seleccionados por conveniencia dada su experiencia y trayectoria en el hospital Luis Ablanque de la Plata y en la Clínica Santa Sofía del Pacífico, en temas relativos de Emergencias y Desastres. Resultados: Los hospitales deben responder de forma coordinada y articulada ante las necesidades de salud de la población y ante situaciones de Emergencia o Desastre deben atender a la población de forma inmediata de acuerdo a las directrices del Sistema Nacional para la Prevención y Atención de Desastres. Se observa que en los contenidos desarrollados de los Planes Hospitalarios de Emergencia no se tiene en cuenta los escenarios de riesgo previstos por el Consejo Distrital de Gestión del Riesgo de Buenaventura el cual es el órgano que tiene a su cargo la gestión del riesgo de desastres en este territorio. El no tener en cuenta estos escenarios impide que la planeación del Plan Hospitalario de Emergencia y la consecuente articulación interinstitucional con organismos de socorro Secretaria de Salud y Comité Distrital de Gestión del Riesgo no sea efectiva ya que se demuestra que cada uno hace esfuerzos no articulados. Conclusiones: En el estudio realizado se observa que ambas instituciones evaluadas poseen sus Planes Hospitalarios de Emergencia sin embargo no los articulan con los organismos de socorro Secretaría de Salud y de Gestión del Riesgo los cuales integran la respuesta en salud del Distrito de Buenaventura. Objectives: We reviewed the literature on mass gatherings published worldwide to determine event types and topics or epidemiologic aspects covered. Articles using the term mass gatherings indexed in the Scopus database between 2000 and 2015 were reviewed. Of the 518 returned, we selected 96 with relevant information. The main event types studied were related to sports (46%), music (25%) or religious/social content (23%), and the most commonly studied locations were the United States (n=21), the Kingdom of Saudi Arabia (n=17), Australia (n=11), and the United Kingdom (n=10). The four most often studied events were the Hajj (n=17), the Olympic games (n=13), World Youth Day (n=8), and the FIFA World Cup (n=6). The main topics studied were models of health care (n=55), health care evaluation by means of rates of patients presenting for care or transferred to hospitals (n=21), respiratory pathogens (n=18), syndromic surveillance (n=10), and the global spread of diseases (n=10). Mass gatherings are an emerging area of study addressed by various medical specialties that have focused on studying the health care models used at such events. Emergency medicine is particularly involved with this research topic.
