Objective To analyze the distribution bio-relationship of loci related antihypertensive drug efficacy in patients with essential hypertension (EH) in Guiyang region to support the development of clinical guidance for individualized medication of hypertension patients in this region. Methods A total of 406 EH patients who visited the Cardiology Department of Guiyang Hospital from December 2020 to December 2021 were collected as the study subjects and ligase sequencing method was applied to detect 7 gene loci of antihypertensive drugs. The distribution of gene loci in EH patients of different genders and from different geographic regions was evaluated. Results The mutation frequencies of the seven genes, CYP2D6*10(c.100 C＞T),CYP2C9*3(c.1075 A＞C),ADRB1(c.1165 G＞C),AGTR1(c.1166 A＞C),ACE(I/D),NPPA(T2238C) and CYP3A5*3(A6986G) were 47.29%, 5.91%, 73.15%, 6.65%, 34.24%, 0.49%, and 69.70% respectively; There was no significant difference in distribution frequency of hypertension drug related gene polymorphisms among different genders. Conclusions The distribution frequency of alleles in 7 antihypertensive drug efficacy related loci is not related to gender of patients.EH patients in Guiyang region are more sensitive to β-blockers and calcium antagonists, but less sensitive to other types of antihypertensive drugs. Therefore, the dose administered should be adjusted appropriately when using β-blockers and calcium antagonists.

Introduction: In pharmacotherapy for inflammatory bowel disease (IBD), good medication adherence is necessary to control the condition. However, some patients show poor adherence. Pharmacists need to provide appropriate medication guidance to improve medication adherence. Community pharmacists often have to provide medication guidance in case of insufficient patient information because of varying affiliations. Therefore, to help improve medication adherence in patients with IBD and establish cooperation between community and hospital pharmacies, we investigated the awareness of IBD among pharmacists and the actual status of pharmacist-led medication guidance for patients with IBD. Methods: This study comprised a knowledge test for IBD and a survey of medication guidance for IBD in the form of questionnaires, which were administered to pharmacists using web forms. Results: Community pharmacy affiliation (p < 0.01) and having no experience in medication guidance for IBD (p < 0.01) contributed to low scores in the IBD knowledge test. There was a difference in the correct answer rate for interactions or screening tests prior to medication administration between community and hospital pharmacists. Medication guidance consultations involving residual drug adjustment (p < 0.01), confirmation of symptoms (p < 0.01), prescription from other hospitals (p = 0.04), therapeutic effects (p = 0.04), and confirmation of medication adherence were more common among community pharmacists than among hospital pharmacists. Cooperation between community and hospital pharmacies was most commonly achieved through tracing reports or personal medication handbooks. Conclusion: Improving pharmacists’ awareness of IBD and sharing information are important to facilitate cooperation between community and hospital pharmacists to improve medication adherence.

The incidence of malignant tumors has steadily increased year by year in China in recent decades. The emergence of innovative anti-cancer drugs has bolstered the prognosis of cancer patients, making it a chronic disease. Kidney injuries are frequent complications of cancer treatment clinically manifested as acute kidney injury (AKI), chronic kidney disease (CKD), proteinuria, and electrolyte disorders, etc. Although kidney injuries affect up to 50% of the patients, there is a lack of professional management guidance for anti-cancer drug dosage adjustment. The clinical pharmacology branch of the Chinese Medical Association has initiated the "Chinese expert consensus on the guidance of anti-tumor drugs for patients with tumor combined chronic kidney disease (2024 edition)". We unified national multidisciplinary experts to analyze clinical evidence with the Delphi method, and conducted expert interviews and seminars focusing on kidney function assessment, anti-tumor drug dosage adjustment for kidney disease patients. We make recommendations on selection and dose adjustment of anti-tumor drugs (chemotherapy, small molecules and antibodies) according to chemotherapeutic drug classifications, providing practical and feasible scientific guidance for clinicians. Hepatocellular carcinoma (HCC) is a highly detrimental cancer type and has limited therapeutic options, posing significant threats to human health. The development of HCC has been associated with a disorder in bile acid (BA) metabolism. In this study, we employed an integrative approach, combining various datasets and omics analyses, to comprehensively characterize the tumor microenvironment in HCC based on genes related to BA metabolism. Our analysis resulted in the classification of HCC samples into four subtypes (C1, C2a, C2b, and C3). Notably, subtype C2a, characterized by the highest bile acid metabolism score (BAMS), exhibited the highest survival probability. This subtype also demonstrated increased immune cell infiltration, lower cell cycle scores, reduced AFP levels, and a lower risk of metastasis compared to subtypes C1 and C3. Subtype C1 displayed poorer survival probability and elevated cell cycle scores. Importantly, the identified subtypes based on BAMS showed potential relevance to the gene expression of drug targets in currently approved drugs and those under clinical research. Genes encoding VEGFR (FLT4 and KDR) and MET were elevated in C2, while genes such as TGFBR1, TGFB1, ADORA3, SRC, BRAF, RET, FLT3, KIT, PDGFRA, and PDGFRB were elevated in C1. Additionally, FGFR2 and FGFR3, along with immune target genes including PDCD1 and CTLA4, were higher in C3. This suggests that subtypes C1, C2, and C3 might represent distinct potential candidates for TGFB1 inhibitors, VEGFR inhibitors, and immune checkpoint blockade treatments, respectively. Significantly, both bulk and single-cell transcriptome analyses unveiled a negative correlation between BA metabolism and cell cycle-related pathways. In vitro experiments further confirmed that the treatment of HCC cell lines with BA receptor agonist ursodeoxycholic acid led to the downregulation of the expression of cell cycle-related genes. Our findings suggest a plausible involvement of BA metabolism in liver carcinogenesis, potentially mediated through the regulation of tumor cell cycles and the immune microenvironment. This preliminary understanding lays the groundwork for future investigations to validate and elucidate the specific mechanisms underlying this potential association. Background: Time and space constraints have often hindered the provision of optimal pharmaceutical care, limiting medication therapy management. Social media tools have gained significant popularity in the field of pharmaceutical care. This study aimed to develop a WeChat-based intelligent medication manager platform that facilitates online pharmaceutical care and encourages selfmanagement. Methods: We developed a WeChat-based Internet pharmacy service platform called Xiang Medicine Guidance (XMG). Through the analysis of surveys and user access data, we evaluated the demand and utilization of the XMG platform and assessed patients' satisfaction with its services. Patients' adherence before and after the XMG platform intervention was also investigated. Results: The XMG platform was launched in November 2022, offering medication guidance, reminders, and consultation services through the WeChat mini-program. By the end of April 2023, the platform had attracted 141.2 thousand users, accumulating 571.0 thousand visits. Moreover, 1,183 clients sought online medication consultations during this period. Six months after the launch of XMG, an impressive 91.02% of users expressed their satisfaction with the platform. The medication reminders and consultations provided by XMG significantly contributed to medication adherence, with 56.02% of users categorized as having good adherence, better than the previous 47.26%. Conclusion: Through its services and features, XMG empowers patients to better manage their medications, seek professional advice, and adhere to their prescribed treatment plans. This study is to test the technologies related to natural language processing (NLP) and text analysis to solve the problem of symptom analysis and symptomatic treatment for healthcare workers and patients after large-scale outbreaks of infectious diseases. Using the keyword extraction tool based on NLP technology combined with text analysis, the symptom description of the infected population obtained from the questionnaire survey was analyzed, and the efficacy of the symptomatic treatment drug was analyzed, and finally a pre-experimental system of online symptomatic treatment support drug selection system was produced. Through natural language processing (NLP) and text analysis of the symptoms of infected people, we found that high-frequency symptoms were mainly reflected in: nasal congestion, sore throat, fever and other high-frequency keywords, and through the analysis of alternative drug libraries, we also found that the drugs were mainly concentrated in: cold medicines, antipyretic drugs, and traditional Chinese medicines. In the early stage of infectious disease outbreak, it is difficult to use the existing drug library to make a prompt system under the premise of insufficient data collection, and a relatively complete symptomatic treatment support medication system can be formed after accumulating certain samples. Background: Sepsis is a severe and acute disease associated with high mortality, for those who survive, long-term morbidity. In-time detection of pathogen for proper and optimized treatment in the early stage of sepsis is crucial. Methods: We performed droplet digital PCR (ddPCR) on two cases of sepsis. The clinical information, laboratory test results, and the therapeutic regimen was detailed recorded. Results: ddPCR showed its clinical value of sepsis ultra-early diagnosis and the guidance of medication in both cases we provided. Conclusions: ddPCR has potential for rapid and precise diagnose in ultra-early stage of sepsis for its high sensitivity and short turn-around-time, thus benefiting in optimal and timely treatment of sepsis.

The Respiratory Syncytial Virus (RSV) is one of the main causes of respiratory diseases in children up to 2 years old. The monthly administration of Palivizumab during the seasonality of the RSV reduces the hospitalization rate from 40 to 55%, however, the non-attendance rate of children is worrying. This work aims to assess the importance of pharmaceutical guidance in treatment adherence. This is a descriptive and retrospective study, analyzing data between the years 2015 and 2020 and the year 2021, where the application of the pharmaceutical guideline took place. A total of 470 children were analyzed for the number of absences and administration of all doses. It was observed that in 2021, after pharmaceutical guidance, there was a significant decrease in the number of absences compared to previous years, with only 8.2% of the total, while the average for previous years was 24.76%. There was an increase in the number of children who took all the doses, reaching 86.89% of the total. It was evaluated that 68.9% of the parents were unaware of the medication and 60% believed that it was a vaccine. Pharmaceutical guidance provided users with greater clarification and consequently resulted in better treatment adherence compared to previous years. [ABSTRACT FROM AUTHOR]

The mobile APP for medication guidance related to pharmacogenomic is developed to solve various practical problems, such as inconvenient reading of English database, slow updating of paper reference books and lack of shortcut for access. We extracted the medication guidance information related to the pharmacogenomic from 'Dosing Guidelines' (http://www.pharmgkb.com), 'Table of Pharmacogenomic Biomarkers in Drug Labeling' (http://www.fda.gov/drugs/scienceresearch) and relevant authoritative books. SQLite was used to build the medication guidance information database. We designed and implemented a mobile APP for medication guidance by JavaScript programming language. The APP contained 197 drugs that have been extensively studied and have high levels of evidence. It covered 25 categories, such as anticoagulant and antiplatelet drugs, general antitumor, immunosuppressant drugs, targeted antitumor drugs, antipsychotic drugs, antiepileptic drugs, and proton pump inhibitors and so on. Users can obtain clinical significance and guidance information related to the genotype of the drug by entering the pinyin initials of the generic name of the drug.
