Introduction: Effective management of medical records is essential for delivering high-quality treatment. The location, architecture, and personnel of a medical records department (MRD) can considerably impact its operational efficiency. This study seeks to examine hospital MRD and establishes whether its current configuration is suitable for effective medical record management. Materials and Methods: The descriptive method was utilized to evaluate the MRD of the hospital. The review analyzed the department's physical layout, personnel, workload, training programs, and available equipment. The study also evaluated the influence of government and business restrictions on MRDs operations. Results: It was determined that the MRD of the hospital had an appropriate physical layout, with divisions positioned in optimal locations. The department's personnel levels were adequate, with twenty employees managing the patient population's workload. The analysis determined that the department required extra photocopiers and scanners to boost operational efficiency. In addition, the study underlined the significance of adhering to policies, protocols, and established processes in ensuring efficient workflow. Conclusion: The analysis concludes that the hospital MRD has an adequate physical layout, staffing levels, and task management. However, the present equipment could be enhanced to increase operational efficiency. The study also emphasizes the importance of adhering to policies, protocols, and written processes to ensure the department's efficient workflow. The outcomes of this study may inform future decisions on MRD management in other health-care organizations, especially those subject to comparable government and commercial regulations.

Introduction: Depression is a common mood disorder, whose recurrence alters its management and prognosis. The aim of the paper is to describe the hospital discharge rate (HED) for recurrent depressive episode (RDE) between 2018-2021 in Chile, according to sex, age and severity. Materials and methods: Observational and cross-sectional study considering hospital discharges due to DRE in the period 2018-2021 in Chile (N=1551). The databases of the Department of Health Statistics and Information and the National Institute of Statistics were used. The variables studied were sex, age group and severity. Ethics committee approval was not required. Results: The lowest HTE was observed in 2020 with 1.91 discharges per 100,000 inhabitants. Females stood out during the entire period with an HTE of 3.68 discharges per 100,000 population. The 15-19 years age group stood out with the highest HTE in both sexes with 5.3 admissions per 100,000 inhabitants. In terms of severity, hospitalizations were concentrated in patients with unspecified diagnosis. Discussion: The COVID 19 pandemic could explain the decrease in HTE in 2020, with a reduction in diagnosis and hospitalization for RDE, presumably due to the bed conversion phenomenon. HTE is higher in the female sex, which is consistent with the literature. The predominance of the 15 to 19 years age group disagrees with the evidence, which indicates that it tends to be concentrated in individuals aged 25 to 64 years. Conclusion: HTE due to DRE was concentrated in young women. It is important to know the local epidemiology in order to focus resources on the timely detection of risk factors to avoid serious episodes and reduce their recurrence. [ABSTRACT FROM AUTHOR]

Aim of Study: A study of the medical records department of a multi super specialty secondary care hospital in NCR. Materials and Methods: Primary data was collected through direct observation and retrospective study of documents maintained in MRD. Secondary data was collected from quality control department books, journals, scholarly articles, and internet. Results and Conclusion: Sample sizes of 350 retrospective and current medical records were thoroughly scrutinized. Conclusion revealed the hospital has published as exhaustive medical records manual listing and the scope, objective, hierarchy chart, job description, policies, procedures, and processes. The MRD has a well-documented flow process of medical records, but on checking the flow of patient records between Nov 2016 to Feb 2017; it was revealed that in month of Nov 2016, out of the total 278 patients discharged only 276 files were received in MRD and 0.72% files were not received. Moreover, it took over 31 days for 71 patients (23.67%) to receive files in MRD. In Jan 2017, out of 286 patients discharged, only 237 files were received in MRD contrasting to 10.14% files not received. Moreover, it took over 31 days for 28 patients (9.80%) to receive files in MRD. In Feb 2017, out of 268 patients discharged, only 206 files were received in MRD and 22.39% files were not received as on 11 March 2017. This study concluded that there is no effective system in place to monitor/track files from ward/billing section to MRD once the patient is discharged. Clinical Significance: Medical records are valuable to patients, physicians, healthcare institutions, researchers, National Health agencies, and International health organizations. Memories fade, people lie, witnesses die; however, medical records live forever. A thorough system of flow process of monitoring/tracking files is to be in place to ensure accountability, smooth functioning, and quality of care being provided without violating basic patient sight of confidentiality of information.

Introduction: Technology Acceptance Model is used to describe human behavior in relation to the information systems. Concentrating on the model, the present research tries to recognize and examine the factors affecting the hospital information system in the teaching hospitals of Tehran University of Medical Sciences. Methods: This paper was a cross- sectional and correlation study conducted in 1390 AH (March 19, 2011 to March 20, 2012). The research population covered 185 respondents including the medical records personnel of the teaching hospitals of Tehran University of Medical Sciences. The data were gathered using a questionnaire (developed by the author comparing the similar studies), which validity was confirmed by a committee of experts and the reliability was calculated (before the study began) using a Cronbach α (α=0.93) in a 50 people sample. The descriptive statistics and non-parametrical analysis of the Spearman rank correlation was applied to the data analysis using an SPSS18. As an area of medical informatics, the introduction of a HIS into a hospital was purported to reduce the time spent on administrative and clinical activities by electronic data processing. However, adoption has been slow, and a key concern has been that staffs will require more time to complete their work using HIS. And also Most of available HIS have lots of deficiencies in data gathering, so hospital managers decided to keep their paper records and same time use HIS. Most previous studies addressing this issue have been done with concentration on clinical staff activities. Methods: Comparative observational of the impact of a HIS on Medical Records Department time in four units: Admission Discharge and Transfer/ADT, Medical Filing and Retrieval, Medical coding/billing, and Medical Statistics. Measurements: We used a time-motion method to measure staff time spent in predefined activities. Medical Records staff monitored while using HIS and paper based system. 15 clerical medical records staff were observed treating 300 documents while still using paper-based records, and same persons were observed treating 400 patients’ records with HIS. Results: Following HIS implementation, the use of the computerized system decreased the average total workload by 61.35% (from 66.41 to 23.93 m). Conclusion: Hospital Information System (HIS) reduces three nonclinical activities time noticeably which is vital to decision making and plays a crucial role in the success of the organization.

The article presents a study which evaluated the performance of Medical Records Departments (MRD) of several hospitals in Isfahan, Iran in 2008 through the use of the Analytical Hierarchy Process (AHP). Also cited are the other proposed performance measurement systems like the Balanced Score Card approach, the Smart System, and the Performance Measurement Questionnaire. Among the focus of the study are customer and information management.

It can be predicted that a substantial number of patients will seek medical care during a possible disaster, placing an increased strain on hospital resources, including health information services. With medical records playing a vital role in the identification of patients and documentation of patient care, the ability of the health information system to cope with this projected surge in demand needs to be addressed. This study was designed to investigate the expected use of specialised health information systems for disasters in Victorian hospitals during such contingencies. Specifically, this study investigated what type of specialised systems hospitals had in place at the time and whether a standard for specialised health information systems for disasters was needed. While 79% of responding hospitals reported having a specialised health information system for disasters, 91% of all responding hospitals reported that specialised health information systems for disasters were necessary. All specialised systems were paper-based, and 94% were based on the standard medical record format and content.
