Aim: Accurate fit of the abutment to the implant is required for the uniform load distribution throughout the assembly. The study aims to compare the marginal misfit of titanium stock abutments with the cobalt-chromium (CoCr) customized abutments fabricated with the different manufacturing processes in internal hex implant-abutment connection using an appropriate scanning technique. Implants were mounted in a clear epoxy resin block and the abutments were then fitted onto the implants with a torque of 30Ncm. The marginal discrepancy at implant-abutment connections was measured with confocal laser scanning microscope. Statistical Analysis Used: One-way ANOVA and Tukey's post hoc test was done for statistical analysis. Results: One-way ANOVA revealed a significant difference in marginal misfit of abutments. The mean marginal misfit was lowest for stock abutments (0.35 ± 0.009 μm). Among the customized abutments, the mean marginal misfit was highest for cast abutments (2.44 ± 0.445 μm) followed by sintered abutments (1.67 ± 0.232 μm) and least for milled abutments (0.65 ± 0.041 μm). A significant difference was found in marginal misfit with cast abutments and sintered abutments when compared to stock abutments (P < 0.001). The difference in marginal misfit was insignificant between stock abutments and milled abutments (P = 0.052). Conclusion: Difference in marginal misfit exists between the titanium stock abutments and customized CoCr abutments. Among the customized abutments, milled CoCr abutments have the least marginal discrepancy and cast CoCr abutments have a maximum marginal discrepancy. Milled CoCr abutments can be used as an alternative to titanium stock abutments.

A marginal fit of all-ceramic crowns is a prerequisite for the long-term clinical success of a dental restoration. Few in vivo studies have investigated the effect of the film thickness of various luting agents on vertical discrepancy. This in vivo study evaluated the influence of three luting cements on the occlusal vertical discrepancy of milled crowns using a complete digital workflow. Forty-three patients treated in a students' clinic in Tel-Aviv University with 45 single posterior digitally prepared monolithic crowns were included in the study. The crowns were randomly divided into three groups using different resin luting agents: self-adhesive resin cement, resin-modified glass ionomer cement and adhesive resin cement. The crowns were intra-orally scanned before and after cementation. The two standard tessellation language (STL) files for each crown were superimposed using digital software, and between four and six measurements were made at the occlusal surface to demonstrate the occlusal and marginal discrepancies. One-way ANOVA (α = 0.05) was used. The vertical occlusal discrepancy ranged from 2 to 38 μm. The mean vertical discrepancy values were (µm): self-adhesive resin = 12.93 ± 4.74, resin-modified glass ionomer = 19.05 ± 4.60 and adhesive resin = 13.69 ± 5.17. There were significant differences between resin-modified and self-adhesive cement groups (p = 0.004), and between resin-modified and adhesive resin cement groups (p = 0.013). Distal marginal ridge measurements were significantly different between resin-modified glass ionomer cement and self-adhesive resin cement group (p < 0.001) and the adhesive resin cement group (p = 0.021). There were no significant differences between the discrepancy values at the two measurement points in the self-adhesive cement group (p = 0.377), nor the resin-modified glass ionomer group (p = 0.388), or the adhesive resin cement group (p = 0.905). The cementation procedure with various resin cements results in occlusal vertical discrepancies within standard clinical acceptability. The aim is to assess the influence of fabrication techniques—conventional (CN), CAD-CAM (CC), and 3D printing (3D)—on the marginal fit, adaptation, surface roughness, and wear of interim restorations of crowns. Thirty interim crowns were fabricated using CN, CC, and 3D techniques. Sixty discs were fabricated to evaluate surface wear and surface roughness properties, with 10 disc samples in each group (n = 10). Internal adaptation and marginal integrity of interim crowns were analyzed with micro CT to detect microgaps at selected points. Average surface micro-roughness (Ra) was calculated in micrometers (μm) using an optical non-contact surface microscope under cyclic loading. Surface wear was evaluated by surface area measurements (mm3) using a micro CT. Analysis of variance (ANOVA) and Tukey's post hoc comparison tests with Pearson correlation were performed for data analysis. The highest adaptation means were for CN (269.94 ± 64 μm), and the lowest mean value was observed for 3D (197.82 ± 11.72 μm) crowns. CN and CC specimens showed comparable adaptation (p > 0.05), which were significantly higher (p < 0.05) than 3D crowns. CN crowns showed significantly higher marginal misfit compared to 3D (p < 0.05) and CC (p < 0.05) crowns. The highest mean surface roughness was observed in the 3D crowns (5.61 ± 0.33 µm), whereas the lowest was found in CC crowns (3.28 ± 0.34 µm). Better restoration Ra was observed using the CC method followed by CN. CN had the highest wear (17.79 ± 2.78 mm3), and the lowest wear was observed in the 3D (10.81 ± 2.00 mm3) specimen. Low surface wear was observed using 3D printing, followed by CN and CC techniques. The printed specimen showed comparable outcomes to CAD-CAM restoration; however, they were better than CN restoration. Group 1, Casting-Lost wax technique (Cast-LWT), Group 2, CAD-CAM, Group 3, Selective laser melting (SLM), and Group 4, Digital light processing-Cast (DLP-Cast). Ra was measured using laser profilometry and marginal misfit was analyzed with Micro-CT. Analysis of variance (ANOVA), Tukey multiple comparison, and correlation coefficient tests were applied (p < 0.05). SLM technique showed the highest Ra (2.251 ± 0.310 μm) and the Cast-LWT group presented the lowest Ra (1.055 ± 0.184 μm). CAD-CAM copings showed statistically lower Ra compared with SLM samples (p = 0.028), but comparable Ra to DLP-Cast (p > 0.05). CoCr copings fabricated from the DLP-Cast technique demonstrated the highest marginal misfit (147.746 ± 30.306 μm) and the lowest misfit was established by SLM copings (27.193 ± 8.519 μm). The SLM technique displayed lower marginal misfit than DLP-Cast and CAD-CAM (p = 0.001), but comparable misfit to Cast-LWT copings. Ra influenced the marginal misfit in CAD-CAM, SLM, and DLP-Cast technique-fabricated copings. (p < 0.01). Objective: The aim of this study was to evaluate marginal fit of yttrium tetragonal zirconia polycrystals (Y-TZP)' copings with different finish line designs fabricated with various digital scanners and milling systems. Methods: Three model plastic teeth were prepared with three finish line designs: Design-1, continuous chamfer; Design-2, chamfer with shallow depression; Design-3, chamfer with deep depression. The "master models" were replicated using elastomeric polyvinyl siloxane impression material and poured in type IV stone generating 90 dies, 30 dies for each design. Dies were scanned and copings were milled utilizing three digital scanners and computer-aided design/computer-aided manufacturing (CAD/CAM) systems: System-1, InEos Red Scan (Sirona Dental Systems, Germany), Vitablocks® Mark II (VITA, Germany) copings milled by Cerec® inLab (Sirona Dental Systems, Germany); System-2, Cerec® AC Connect with BlueCam (Sirona Dental Systems, Germany), Vitablocks® Mark II (VITA, Germany) copings milled by Cerec® inLab (Sirona Dental Systems, Germany); and System-3, NobleProcera™ Optical Scanner (NobleBiocare™), procera zirconia coping milled by a Noble Procera™ milling machine (NobleBiocare™). Copings were seated on their respective "master models" and secured with uniform force. Eight measurements per coping were performed at pre-established points, with a metallurgical microscope (Zeiss, Germany) connected to a high precision digital video-micrometer (Javelin JV6000, California, USA) at 200 × magnification. Results: The tested systems demonstrated marginal gaps ranging from 12.4 to 26.6 µ m. Results for marginal fit of milled copings fabricated using three systems with different finish line designs differed significantly (p < 0.05). Procera zirconia copings scanned and milled with NobleProcera™ exhibited significantly lower marginal gaps compared to other specimen groups. However, InEos Red Scan/Vitablocks® Mark II/Cerec® inLab showed maximum marginal gaps among the study specimens. Conclusions: CAD-CAM manufactured Y-TZP' copings exhibited marginal gaps ranging from 12.49 to 26.6 µ m. The CAD-CAM fabrication system was a significant factor influencing the marginal misfit of Y-TZP' copings. Margin design exhibited system dependent influence on the marginal misfit. PURPOSE. The purpose of this in vitro study was to evaluate the marginal misfits of three-unit frameworks fabricated with conventional and digital impressions techniques. MATERIALS AND METHODS. Thirty brass canine and second premolar abutment preparations were fabricated by using a computer numerical control machine and were randomly divided into 3 groups (n=10) as follows: conventional impression group (Group Ci), Cerec Omnicam (Group Cdi), and 3shape TRIOS-3 (Group Tdi) digital impression groups. The laser-sintered metal frameworks were designed and fabricated with conventional and digital impressions. The marginal adaptation was assessed with a stereomicroscope at ×30 magnification. The data were analyzed with 1-way analysis of variances (ANOVAs) and the independent simple t tests. RESULTS. A statistically significant difference was found between the frameworks fabricated by conventional methods and those fabricated by digital impression methods. Multiple comparison results revealed that the frameworks in Group Ci (average, 98.8 ± 16.43 μm; canine, 93.59 ± 16.82 μm; premolar, 104.10 ± 15.02 μm) had larger marginal misfit values than those in Group Cdi (average, 63.78 ± 14.05 μm; canine, 62.73 ± 13.71 μm; premolar, 64.84 ± 15.06 μm) and Group Tdi (average, 65.14 ± 18.05 μm; canine, 70.64 ± 19.02 μm; premolar, 59.64 ± 16.10 μm) (P=.000 for average; P=.001 for canine; P<.001 for premolar). No statistical difference was found between the marginal misfits of canine and premolar abutment teeth within the same groups (P>.05). CONCLUSION. The three-unit frameworks fabricated with digital impression techniques showed better marginal fit compared to conventional impression techniques. The purpose of this in vitro study was to measure the vertical, positive-horizontal, and negative-horizontal misfit (VM, PHM, and NHM, respectively) of the zirconia three-element prosthetic framework, fabricated using different methods, and compare them with conventional fabrication methods (lost-wax casting). Furthermore, this study aimed to evaluate the influence of the misfit values on the biomechanical behavior of the 3-unit fixed prosthetic frameworks using three-dimensional finite element analysis (3D-FEA). Forty frameworks (n = 10) were fabricated as follow: G1, Cerec Bluecam; G2, iTero; G3, 3Series; and G4, conventional method. The samples were randomized to measure marginal misfit using a high-precision three-dimensional (3D)-optical microscope. The results were submitted to analysis of variance (ANOVA), with the significance level set at 5%. The mean VM values of each group were used in creating the models by 3D-FEA with the misfit found in optical microscopy. The programs used were the InVesalius, Rhinoceros, SolidWorks, FEMAP and NEiNastran. The von Mises map was plotted for each model. The G4 showed the lowest mean VM value (16.73 μm), followed by G3 (20.71 μm), G2 (21.01 μm), and G1 (41.77 μm) (p < 0.001). G2 was more accurate than G1 (p < 0.05) and similar to G3 (p = 0.319). For PHM, G4 was the most accurate and did not present overextended values. With regard to NHM, the computer-aided design and computer-aided manufacturing (CAD-CAM) systems were more accurate (−61.91 μm) than G4 (−95.36 μm) (p = 0.014). In biomechanical analysis, stress concentration caused by oblique loading is greater than caused by axial loading. In axial loading, G4 was the most favorable while G1 was the least favorable, biomechanically, in oblique loading, similar stress patterns were observed in all the models. The prosthetic screw was the most overloaded structure, but the material did not influence the stress distribution. The misfit prostheses showed a greater degree of stress than the controls (without misfit). The manufacturing method influenced the marginal misfit of the frameworks, with the conventional method being the most accurate and the Cerec Bluecam System (closed system) the least accurate. The precision of fit at the implant-abutment connection is an important criterion for the clinical success of restorations and implants. Several factors are involved among which are the abutment materials and manufacturing techniques. The aim of this study was to investigate the effect of two materials and methods of manufacturing implant abutments, milled titanium versus laser sintered Co-Cr, on the marginal misfit at the implant-abutment interface. Scanning electron microscopes (SEM) were used to geometrically measure the marginal vertical discrepancy of a total of 80 specimens, classified into eight categories, according to the implant system and abutment. The data were statistically analyzed by Student’s paired t test, one-way and two-way ANOVA with the Bonferroni-Holm correction at the significance level of p = 0.05. Milled titanium abutments demonstrated the lowest misfit values in the implant systems analyzed. The marginal fit of all the groups was within the clinically acceptable range for implant prostheses.

Purpose The aim of the study was to evaluate the effect of a prosthetic cylinder and casting on the misfit and loosening torque of screw-retained multiple-unit implant-supported dental prostheses under masticatory simulation. Materials and Methods Screw-retained, three-unit fixed dental prostheses (FDP) and screw-retained full-arch FDP frameworks were waxed using calcinable (plastic cylinders) or overcasted (premachined cast-on cylinders) on the dental implant abutments. The cylinders were cast in Co-Cr alloy to obtain four groups according to cylinder type and prosthesis type (n = 10). The screws were tightened with 20 N/cm (abutment) and 10 N/cm (prosthetic) torque according to the manufacturer's recommendation. After 24 hours, the initial loosening torque was analyzed. The initial misfit measurements were performed according to the Schiffleger test. The screws were retightened, and the specimens were submitted to 106 mechanical cycles (2 Hz/280 N). Loosening torque and misfit were reevaluated (final measurements), and data were submitted to ANOVA, Tukey's HSD, and Pearson's correlation tests (α = 0.05). Results The calcinable three-unit FDP demonstrated greater misfit (initial: 107.53 ± 40.36 μm; final: 99.00 ± 40.85 μm) than did the overcasted three-unit FDP frameworks (initial: 51.50 ± 22.98 μm; final: 44.33 ± 14.14 μm) (initial: p = 0.0005; final: p = 0.0007). No difference was noted between the calcinable and overcasted full-arch FDP frameworks ( p > 0.05). Masticatory simulation did not affect the misfit ( p > 0.05). The overcasted full-arch FDP presented a lower abutment screw loosening torque (12.05 ± 1.80 N/cm) than did the calcinable ones (14.75 ± 1.72 N/cm) in the final measurement ( p = 0.0024). The calcinable groups presented a lower prosthetic screw loosening torque than did the overcasted groups in the final evaluation ( p < 0.05). After masticatory simulation, the prosthetic screw loosening torque of the calcinable three-unit FDP decreased (initial: 5.49 ± 1.07 N/cm; final: 3.73 ± 1.15 N/cm; p = 0.0044). Correlation between misfit and loosening was observed only for the prosthetic screws ( p < 0.05). Conclusions The overcasted components provided a better fit in three-unit FDPs but did not influence the fit of full-arch FDPs. Prosthetic screws of overcasted frameworks presented higher stability, whereas masticatory simulation did not influence misfit but did reduce the prosthetic screw loosening torque of calcinable three-unit FDP frameworks. The influence of casting and masticatory simulation on marginal misfit and strain in multiple implant-supported prostheses was evaluated. Three-unit screw retained fixed dental prosthesis (FDP) and screw retained full-arch fixed dental prosthesis (FAFDP) frameworks were made using calcinable or overcasted cylinders on conical dental implant abutment. Four groups were obtained according to the cylinder and prosthesis type (n = 10). Frameworks were casted in CoCr alloy and subjected to strain gauge analyses and marginal misfit measurements before and after 10 6 mechanical cycles (2 Hz/280 N). Results were submitted to ANOVA, Tukey's HSD and Pearson correlation test (α = 0.05). No difference was found on misfit among all groups and times (p > 0.05). Overcasted frameworks showed higher strain than the calcinable ones (FDP — Initial p = 0.0047; Final p = 0.0004; FAFDP — Initial p = 0.0476; Final p = 0.0115). The masticatory simulation did not influence strain (p > 0.05). No correlation was observed between strain and misfit (r = 0.24; p > 0.05). In conclusion, the marginal misfit value in the overcasted full-arch frameworks was higher than clinical acceptable data. It proved that overcasted method is not an ideal method for full-arch prosthesis. Overcasted frameworks generate higher strain upon the system. Abstract: An inappropriate prosthetic fit could cause stress over the interface implant/bone. The objective of this study was to compare stresses transmitted to implants from frameworks cast using different materials and to investigate a possible correlation between vertical misfits and these stresses. Fifteen one-piece cast frameworks simulating bars for fixed prosthesis in a model with five implants were fabricated and arranged into three different groups according to the material used for casting: CP Ti (commercially pure titanium), Co–Cr (cobalt–chromium) or Ni–Cr-Ti (nickel–chromium–titanium) alloys. Each framework was installed over the metal model with all screws tightened to a 10Ncm torque and then, vertical misfits were measured using an optical microscope. The stresses transmitted to implants were measured using quantitative photoelastic analysis in values of maximum shear stress (τ), when each framework was tightened to the photoelastic model to a 10Ncm standardized torque. Stress data were statistically analyzed using one-way ANOVA and Tukey''s test and correlation tests were performed using Pearson''s rank correlation (α=0.05). Mean and standard deviation values of vertical misfit are presented for CP Ti (22.40±9.05μm), Co–Cr (66.41±35.47μm) and Ni–Cr–Ti (32.20±24.47μm). Stresses generated by Co–Cr alloy (τ=7.70±2.16kPa) were significantly higher than those generated by CP Ti (τ=5.86±1.55kPa, p=0.018) and Ni–Cr–Ti alloy (τ=5.74±3.05kPa, p=0.011), which were similar (p=0.982). Correlations between vertical misfits and stresses around the implants were not significant as for any evaluated materials. [Copyright &y& Elsevier]

The precision of fit at the implant-abutment connection is an important criterion for the clinical success of restorations and implants. Several factors are involved among which are the abutment materials and manufacturing techniques. The aim of this study was to investigate the effect of two materials and methods of manufacturing implant abutments, milled titanium versus laser sintered Co-Cr, on the marginal misfit at the implant-abutment interface. Scanning electron microscopes (SEM) were used to geometrically measure the marginal vertical discrepancy of a total of 80 specimens, classified into eight categories, according to the implant system and abutment. The data were statistically analyzed by Student's paired t test, one-way and two-way ANOVA with the Bonferroni-Holm correction at the significance level of p = 0.05. Milled titanium abutments demonstrated the lowest misfit values in the implant systems analyzed. The marginal fit of all the groups was within the clinically acceptable range for implant prostheses. Objective: the aim of this study was to evaluate the marginal misfit at the interface between a ceramic coping and its abutment., Methods: Twenty-four specimens were made with solid abutments. The specimens were divided into 3 groups according to the ceramic system (n = 8): Lava (zirconia), IPS e.max Press (lithium disilicate), and IPS Empress Esthetic (leucite). All copings were cemented with resin luting agent (RelyX U200) and the marginal misfit were evaluated at 3 different times: initial, after cementation, and after mechanical cycling using a linear measuring microscope (Measuring Microscope STM-Olympus) at a magnification of 40x. All specimens were subjected to mechanical cycling (1 million cycles) by an universal testing machine (Instron 8800). The results were statistically analyzed using Analysis of Variance and Student's t-test (α = 0.05)., Results: all groups showed an increase in the marginal misfit after cementation. The lithium disilicate group demonstrated the lowest interacial gap values at each evaluation (p = 0.001). The zirconia and leucite groups showed similar interfacial gap values (initial, p = 0.244; and post cementation, p = 0.751)., Conclusions: the cementation increase the marginal misfit, but the mechanical cycling did not influence the marginal misfit of the ceramics systems evaluated., (Copyright© 2017 Dennis Barber Ltd.)
