Objectives: To compare marginal gap width and depth with different cementation systems, excess removal, and after polishing. Method and materials: In total, 80 composite crowns were milled, divided into ten groups, and cemented on identical artificial teeth. Eight crowns per group were fixed with (i) zinc phosphate cement (ZnOPh), (ii) glass-ionomer cement (GIC), (iii) resin-reinforced glass-ionomer cement (GIC mod), (iv) dual-curing adhesive composite (Comp dual), or (v) dual-curing self-adhesive composite (Comp SE dual). Excess removal was performed with a scaler after brief light-cure (tack-cure), final light-cure, during rubber or gel phase or by wiping with foam pellet. Curing was completed in chemical, dark cure, or light-curing modus. The specimens were polished and stored in water (37°C). The margins were digitized using a 3D laser-scanning microscope (VK-X100 series, Keyence). The width and the depth of the marginal gap were measured at 10 points between the crown margin and the preparation margin. Results: The width after excess removal varied between 65.1 ± 15.7 µm (Comp dual, wipe, with polishing) and 208.6 ± 266.7 µm (Comp SE dual, dark cure, without polishing). The depth varied between 29.8 ± 22.2 µm (Comp dual, wipe, with polishing) and 89.5 ± 45.2 µm (Comp SE dual, dark cure, without polishing). The impact on gap width and depth was detected for fixation material, excess removal, and polishing. Conclusion: The gap depth and width depend on the luting material and the mode of access removal. Objectives: To evaluate the effect of sonic oscillation on penetration depth and marginal adaptation of resin-based fissure sealants. Method and materials: Thirty-six extracted human third molars were randomized into three groups: in the HF group, teeth were sealed with a filled-resin-based sealant (Helioseal F); in the Sonic-HF group, teeth were sealed with Helioseal F and subjected to sonic oscillation (Compothixo Device, 140 Hz vibration frequency and ± 150 µm amplitude) before curing; in the CO group, teeth were sealed with an unfilled-resin-based sealant (Clinpro). All teeth were thermocycled for 1,500 cycles at 5°C and 55°C, then sectioned buccolingually into four sections (mesial, two middle, and distal). Sections were evaluated under SEM for marginal adaptation and scored. For penetration depth, three independent measurements from each tooth were recorded under SEM, and data were subjected to statistical analysis. Results: The group subjected to sonic oscillation (Sonic HF) resulted in significantly superior penetration depth and marginal adaptation compared with the HF group, but with no significant difference compared with the CO group. Conclusions: Sonic activation of resin-based sealant increased penetration depth into fissures and marginal adaptation to the enamel wall without decreasing filler load or compromising sealant mechanical properties. Purpose: This study investigated and compared the bond strengths, microleakage, microgaps, and marginal adaptation of self-adhesive resin composites (SAC) to dentin with or without universal adhesives. Materials and Methods: Dentin surfaces of 75 molars were prepared for shear and microtensile bond strength testing (SBS and μTBS). Silicon molds were used to build up direct restorations using the following materials to form 5 groups: 1. Surefil One; 2. Prime&Bond active Universal Adhesive + Surefil One; 3. Vertise Flow; 4. OptiBond Universal + Vertise Flow; 5. Scotchbond Universal + Filtek Z500 (control group). Bonded specimens were thermocycled 10,000x before being tested either for SBS or μTBS using a universal testing machine at a crosshead speed of 0.5mm/min. Direct mesial and distal class-II cavities were created on 100 sound premolars, with the gingival margin of distal cavities placed below CEJ and restored according to the five groups. After thermocycling, microleakage scores were assessed following immersion of restored premolars in 2% methylene blue dye for 24h, while marginal gaps and adaptation percentages were investigated on epoxy resin replicas under SEM at magnifications of 2000X and 200X, respectively. Results were statistically analyzed with parametric and non-parametric tests as applicable, with a level of significance set at a = 0.05. Results: Bond strengths, microleakage scores, microgaps, and percent marginal adaptation of Surefil One and Vertise Flow were significantly (p<0.001) inferior to the control group. Dentin preconditioning with universal adhesives significantly increased the study parameter outcomes of Surefil One and Vertise Flow, yet they were still significantly below the performance of the control group. Background: Long-term durability of a restoration relies on the marginal integrity and its ability to withstand the occlusal forces. Fiber-reinforced composites (FRCs) exhibited superior properties in terms of fracture toughness, flexural strength, and wear resistance. Aim: The aim of this study was to assess and compare marginal adaptation and microtensile bond strength (µTBS) of posterior FRCs comparing with a condensable composite. Materials and Methods: Standardized mesio- and disto-occlusal cavities were prepared on 60 maxillary molars with 3.5 mm pulpal floor depth, 3 mm buccolingual width, and 1.5 mm axial depth. The mesial cavities were restored with everX Posterior and Build-It FRCs, while distal cavities of all teeth were restored with Alert composite and were thermomechanically aged. Half of the specimens from each group (n = 15) were evaluated for marginal adaptation under scanning electron microscope (SEM), and the other half were sectioned for μTBS testing with Instron testing machine. Bond failure mode was examined under SEM. Results: Marginal microgaps were significantly high for the Alert group than FRC groups. Gingival surface showed more gaps. Mean µTBS values were significantly high for FRCs. Cohesive failures are more among FRCs, and adhesive failures are more in condensable composite. Aims: This pilot study aimed to compare the marginal adaptation of composite resin at the tooth-restoration interface, before and after radiation. Subjects and Methods: Fifteen extracted premolars were divided into 2 experimental groups (based on the timing of irradiation) and 1 control group of 5 teeth each. In Group I (control group), teeth were restored but not exposed to radiation at any stage, Group II: teeth were irradiated before cavity preparation and restoration, and Group III: after cavity preparation and restoration employing selective etch technique, teeth were exposed to radiation. The samples were then sectioned buccolingually to analyze the extent of the marginal gap under scanning electron microscopy and compositional alteration of dental hard tissues by X-ray diffraction study. The data collected were analyzed statistically. Statistical Analysis Used: The statistical software used was IBM SPSS version 23 New York, USA, and analysis was done using two-way ANOVA followed by Turkey's post hoc test, this difference in the mean marginal gap between all three groups was nonsignificant (P ≥ 0.05). Results: In the control group (Group I), a minimum gap (4.203 µm ± 0.533) was observed at the tooth-restoration interface, indicating the highest level of adaptation as compared to Group II (5.816 µm ± 0.762) and Group III (4.862 µm ± 1.018). This suggests that radiation adversely affected the bonding between composite materials and both enamel and dentin, attributed to the alterations induced by radiotherapy in the chemical, physical, and morphological properties of both tooth structure and composite resin. Conclusions: Ionizing radiations adversely affect the bonding between enamel, dentin, and composite resin. Hence, restorative procedures should be performed before undergoing radiotherapy. The purpose of this study was to investigate the vertical marginal discrepancy (VMD) and residual excess cement (REC) of cementable and screwmentable monolithic zirconia crowns cemented with different types of cement. Abutments were attached to 40 implant analogues. Crowns were created using computer-aided design/computer-aided manufacturing technology from monolithic zirconia blocks, either with or without a screw access hole (SAC). Crowns created both ways were split into two groups and cemented with resin and zinc polycarboxylate cement under a 5-kg weight. VMD and REC values were evaluated using an X20 zoom stereomicroscope. Data were analysed using two-way ANOVA and the Bonferroni test. According to the two-way ANOVA results, REC measurements differed significantly in the crown design and cement groups. However, whilst VMD values were significantly different in both crown design groups, there was no significant difference in the cement groups. According to the Bonferroni test results, the highest REC (157.241 ± 44.29 µm) and VMD (68.052 ± 16.19 µm) values were found in the crowns without SAC and cemented with zinc polycarboxylate. Screwmentable crowns are more effective than cementable crowns in reducing REC and VMD. Whilst polycarboxylate cement reduces VMD in screwmentable crowns, resin cement is more suitable for cementable crowns. [ABSTRACT FROM AUTHOR]

As a current debate is existing about the refrigeration of resin composite restorative materials, this study was designed to assess the effect of refrigeration of 3 types of resin composite restorative materials with different matrix systems on their marginal adaptation in Class II restorations. Methods: Forty-two sound maxillary molars, each with two separated Class II cavities, were used in this study. The teeth were assigned into 3 main groups (n = 14) according to the restorative /adhesive system used; an Ormocer-based composite (Admira Fusion/Futurabond M+, Voco GmbH, Cuxhaven, Germany), a methacrylate modified Ormocer-based (Ceram.X SphereTEC One/Prime&Bond Universal, Dentsply Sirona GmbH Konstanz, Germany), and a methacrylate-based (Tetric N-Ceram/Tetric N-Bond Universal, Ivoclar Vivadent AG, Schaan, Liechtenstein). Each group was then divided into 2 subgroups (n = 14) according to the gingival margin location; 1 mm above and 1 mm below the cemento-enamel junction (C.E.J). Each subgroup was further divided into 2 categories (n = 7) according to the storage temperature; stored at room temperature or stored in refrigerator at 4°- 5° C. Epoxy resin replicas were observed under scanning electron microscope (SEM) to examine the marginal gaps. A gab scoring system was used to assess the marginal adaptation of each restoration by giving scores on the basis of measurements of the maximum marginal gaps. The data obtained were statistically analyzed using the Chi-square test at a significance level of p < 0.05. Results: None of the tested groups exhibited 100% gap-free margins irrespective of margin location or storage temperature. For both storage temperatures, no statistically significant difference was observed among all tested groups either with margins located above or below C.E.J (p > 0.05). As well, there was no statistically significant difference when comparing both marginal locations for each material (p > 0.05). Regarding the effect of storage temperature, statistically significant difference was only observed between the room-temperature stored groups with margins located above C.E.J and their corresponding groups stored in refrigerator (p < 0.05). Conclusion: The refrigeration of resin composite restorative materials prior to the restorative procedures revealed a deleterious effect on marginal adaptation of the restorations with margins located in enamel regardless the type of material used. Aim of the study: This in vitro study aimed to evaluate the effect of vertical preparation and modified vertical preparation designs on the marginal adaptation and fracture resistance of two types of ceramic crowns using CAD/CAM technology. Materials and methods: Two typodont maxillary first premolars were embedded in acrylic resin. Forty positive replicas of epoxy resin dies were used that were divided into two groups depending on the preparation design (n = 20); Group V (Vertical): dies with feather edge finish line and Group MV (Modified vertical): dies with feather edge finish line, where a reverse shoulder of 1 mm depth was placed on the buccal surface 1.5 mm from the occlusal surface. Each group was further subdivided into two subgroups according to the type of ceramic material (n = 10): Subgroup Va and subgroup MVa for lithium disilicate (e.max CAD) and subgroup Vb and subgroup MVb for zirconia (zolid ht+). Crown restorations were made with CAD-CAM technology. The marginal adaptation was assessed using a stereomicroscope both prior to cementation and after cementation and aging. Fracture resistance was tested with a universal testing machine, and the data were statistically analyzed. Results: Marginal adaptation showed no significant differences between subgroups before or after cementation and aging. Three-way ANOVA indicated that preparation design (p = 0.516) and material (p = 0.269) had no significant effect, but cementation had a significant effect (p < 0.0001) on the marginal adaptation. According to two-way ANOVA test, Subgroup (MVb) showed the highest result followed by subgroup (Vb) and subgroub (MVa) and the least was subgroub (Va). Fracture modes showed no significant differences among the subgroups (p = 0.982). Conclusions: Marginal adaptation of lithium disilicate and zirconia crowns remained clinically acceptable regardless of preparation design. While the modified vertical preparation with a reverse shoulder notably enhanced the fracture resistance of both materials, with zirconia demonstrating superior fracture resistance compared to lithium disilicate with average values exceeding premolar biting force. (1) Background: Restoring decayed teeth in young patients can be challenging. This calls for a simplification of the protocols through new biomaterials. Therefore, the aim of this study was to compare the marginal adaptation delivered by restorative materials applied on class II cavities by using a simplified protocol, before and after fatigue test, followed by the assessment of the internal adaptation. (2) Methods: Forty-eight human teeth were divided into six groups (n = 8). Dentinal fluid simulation was performed before restoring the class II cavities: Gr 1—adhesive (Clearfil Universal Bond Quick) and nanohybrid flowable composite (Clearfil Majesty ES Super Low Flow), Gr 2—adhesive (Clearfil Universal Bond Quick) and nanohybrid composite (Clearfil Majesty ES standard), Gr 3—bulk fill self-adhesive composite (Surefil One), Gr 4—bioactive powder-liquid filling material (Cention Forte), Gr 5—universal adhesive (Adhese Universal) and nanohybrid composite resin (Tetric Powerfill); and control group (CT)—high-viscosity glass ionomer (Equia Forte). Marginal adaptation was observed with scanning electron microscopy (SEM) and compared before and after a fatigue test consisting of repeated thermal and mechanical cycles. The specimens were then cut mesio-distally, and internal adaptation was undertaken using SEM again. Repeated measures and one way ANOVA followed by a Fisher's LSD test and Fisher's LSD post hoc test were used in order to compare the statistically significant differences among groups. (3) Results: As for the marginal adaptation after loading, Cention Forte (58%) and Equia Forte HT (53%) were statistically equivalent and presented the highest results, followed by Clearfil Majesty ES Standard (32%) and Tetric Powerfill (27%), with Surefil One (8%) and Clearfil Majesty ES Flow Super Low (7%) showing the worst results. In terms of internal adaptation, Cention Forte (85%) and Clearfil Majesty ES Standard (74%) had the highest percentages of continuous margins. Tetric powerfill (56%) and Equia Forte HT (44%) showed statistically significantly lower results, followed by Clearfil Majesty ES Flow Super Low (33%) and eventually Surefil One (17%). (4) Conclusions: This in vitro study showed promising results for the marginal and internal adaptation of alkasite dual cured Cention Forte in the restoration of class II cavities. Methodology A total of 40 extracted maxillary and mandibular premolars were treated endodontically, and each type was subdivided according to the remaining axial height (remaining walls on all surfaces; 2–4 mm) and 2 mm inside the pulp chamber. Specimens were immersed in coffee for 14 days, ΔE* was determined, marginal adaptation was observed, fracture forces test was conducted, and the samples were examined visually at 10× magnification to evaluate failure type and identify fracture origin. The data were entered and analyzed using Statistical Package for Social Sciences, and significance between and within groups was evaluated through ANOVA. Objective To evaluate the impact of ultra-high-molecular-weight polyethylene (UHMWPE)-Ribbond fibers, when combined with different restorative materials, on fracture resistance and marginal adaptation of isolated primary molar defects, to provide a reference for clinical practice. Methods This study was approved by the Ethics Review Committee. A total of 72 extracted primary molars with complete crowns were collected, and 66 primary molars were randomly assigned as experimental groups for the fracture resistance and microleakage tests. The molars were divided into six groups (n = 11) based on the type of restorative materials and the application of Ribbond fibers: Group A1, 3M Filtek Z250 + Ribbond; Group A2, 3M Filtek Z250; Group B1, Beautifil II LS + Ribbond; Group B2, Beautifil II LS; Group C1, 3M Filtek Bulk Fill + Ribbond; and Group C2, 3M Filtek Bulk Fill. Groups A1, B1 and C1 received the fiber-reinforcing technique, whereas Groups A2, B2 and C2 received the direct restorative technique; the remainings were in Group D (blank control group), which did not receive treatment for the fracture resistance test. The fracture resistance test was divided into six experimental groups and one blank control group (n = 6). Primary molar teeth in each experimental group were prepared with Class II cavities and filled. The fracture load of all samples was detected, and the fracture mode was analyzed after thermal cycling. The microleakage test was divided into six experimental groups, with five in each group. Class I cavities with a diameter of 3 mm and depth of 2.5 mm were prepared within the mesial and distal marginal ridges on the occlusal surface and filled for primary molars in each group. The maximum fracture force was observed in maxillary premolars with 2 mm axial height (2248.15 ± 134.74 N), and no statistically significant difference (p = 0.07) was observed between maxillary and mandibular premolars at 4 mm axial height. Conclusion: The recorded ΔE* values of the ECs were within clinically acceptable values or slightly higher, and the marginal adaption values were within acceptable and recommended clinical values in µm. EC preparation with 2 mm axial height in both arches recorded the highest fracture forces. Objectives: This in vitro study examined the marginal integrity of experimental composite materials doped with bioactive glass (BG). Materials and methods: Class-II MOD cavities were prepared and restored with one of the following composite materials: a commercial composite material as a reference (Filtek Supreme XTE), an experimental composite doped with BG 45S5 (C-20), and an experimental composite doped with a fluoride-containing BG (F-20). Six experimental groups (n = 8) were used, as each of the three composites was applied with (+) or without (-) a universal adhesive (Adper Scotchbond Multipurpose). All specimens were subjected to thermocycling (10,000 x, 5–55 °C) and then additionally stored in artificial saliva for eight weeks. Scanning electron micrographs of the mesial and the distal box were taken at three time points (initial, after thermocycling, and after eight weeks of storage in artificial saliva). The margins were classified as “continuous” and “non-continuous” and the percentage of continuous margins (PCM) was statistically analyzed (α = 0.05). Results: In most experimental groups, thermocycling led to a significant decrease in PCM, while the additional 8-week aging had no significant effect. F-20 + performed significantly better (p = 0.005) after 8 weeks storage in artificial saliva than the reference material with adhesive, while no statistically significant differences were observed at the other two time points. C-20 + exhibited significantly better PCM than the reference material with adhesive after thermocycling (p = 0.026) and after 8 weeks (p = 0.003). Conclusions: Overall, the experimental composites with BG showed at least as good marginal adaptation as the commercial reference, with an indication of possible re-sealing of marginal gaps. Clinical relevance: Maintaining or improving the marginal integrity of composite restorations is important to prevent microleakage and its likely consequences such as pulp irritation and secondary caries. This study aimed to evaluate the quality of dental restorations using laboratory microscopic techniques, focusing on the positioning of restorations relative to cavity edges and identifying common errors, such as incomplete or excessive coronal restorations. A total of 41 extracted lateral teeth, previously treated in vivo with Class I or II composites, were analyzed. Thirty-three of these teeth were sectioned medio-distally or buccal-orally and examined under a research laboratory microscope. Marginal areas were measured using eyepieces with a graduated scale and calibration slide, and a frequency statistical analysis was conducted. The analysis revealed that the teeth had fractious edges, deficient marginal closure, excess composite, incorrectly restored occlusal cusps, and marginal adaptation errors, with approximately half of these errors involving excess material. It was observed that restoration procedures in distal areas are particularly challenging, and clinical errors with excess material occur more frequently than those with a deficit. Background: In everyday dentistry, monolithic single crowns can be cemented with self-adhesive resin cements. The aim of this in vitro study was to evaluate how the marginal adaptation of full monolithic zirconia-reinforced lithium silicate (ZLS) single crowns is influenced by three different self-adhesive resin cements. Methods: Forty-five typodont teeth fully prepared for full monolithic crowns were divided into three groups (fifteen each) for the use of three different self-adhesive resin cements. A fourth control group (Temp-bond) was created by taking five teeth from each group before cementation with self-adhesive resin cements. All forty-five abutments were scanned using a Primescan intra-oral scanner (IOS), followed by computer-aided design (CAD) and computer-aided manufacturing (CAM) of zirconia-reinforced lithium silicate (ZLS) full crowns using a four-axis machine. Initially, the crowns of the control group were fixed to the abutments using Temp-bond, and the marginal gap was evaluated using a scanning electron microscope (SEM). After removing the control group crowns from the abutments, fifteen crowns in each group were cemented using a different self-adhesive resin cement and observed under SEM for evaluation of the marginal gap. A Kolmogorov–Smirnov test was performed, indicating no normal distribution (p < 0.05), followed by Mann–Whitney tests (α = 0.05). Results: The total mean marginal gap of the temp-bond control group was significantly lower compared to all three groups of self-adhesive resin cement (p < 0.0005). The total mean marginal gap of the G-cem ONE group was significantly lower compared to the TheraCem group (p < 0.026) and RelyX U200 group (p < 0.008). The total mean marginal gap of the TheraCem group was significantly higher than the G-cem ONE group (p < 0.026) but showed no significant difference with the RelyX U200 group (p > 0.110). Conclusions: All four groups showed a clinically acceptable marginal gap (<120 microns). Although all three groups of self-adhesive resin cement showed a significant increase in the marginal gap compared to the temp-bond control group, they were within the limits of clinical acceptability. Regarding the marginal gap, in everyday dentistry, it is acceptable to use all three self-adhesive resin cements, although the G-cem ONE group exhibited the lowest marginal gap for ZLS single crowns. Objectives: To evaluate surface microhardness and gingival marginal adaptation of three different bulk‐fill flowable resin composites (FB‐RBCs) in Class V cavities with enamel or dentin margins before and after thermocycling (TMC). Materials and Methods: Three available FB‐RBCs were used; Palfique Bulk Flow (PaBF) (Tokuyama Dental), SDR Flow+ Bulk Fill Flowable (Dentsply Sirona), and I‐Flow Bulk Fill (I‐Dental). Thirty discs were prepared from each type of FB‐RBCs. The discs were subjected to Vickers microhardness tester machine. Class V cavities were prepared on 180 molars. Gingival margins of half the specimens were prepared above CEJ and the other half below CEJ. Cavities were restored with FB‐RBCs for gingival marginal adaptation test. Two‐way ANOVA was used in microhardness, while three‐way ANOVA was used for marginal adaptation. Results: The used materials showed statistically significant differences in microhardness and marginal adaptation. Conclusions: Regarding microhardness, PaBF showed the highest value before TMC and SDR was the highest after TMC. Regarding marginal adaptation, SDR revealed the best marginal adaptation than PaBF and I‐flow, either subjected to TMC or not. Clinical Significance: The longevity of cervical restorations depend on both the location of the cavity margin and the material used. The microfractures in resin composite surface due to low surface microhardness as well as marginal gaps seen in laboratory studies could be considered as an indicative parameter for clinical problem associated with marginal differences. Due to innumerable confounding factors and a high number of types and brands of dental restorative materials, the clinical performance of restorative materials are sought predicted by various in vitro tests. However, only few such tests have been found to correlate well with clinical findings. Thus, the present study determined the in vitro dentin bond strength and marginal adaptation of Class II restorations and correlated the results to their clinical outcomes. Dentin bond strength (µTBS and µSBS) and marginal gap formation of Class II restorations (replica technique and SEM) were measured after 24 h and 6 m water storage using eight combinations of adhesive and resin composite. Clinical outcomes (mean survival time, Hazard Ratio, annual failure rate; n = 10.695) were gained from a data set of a retrospective multicenter study of direct restorations. Significant differences were found for dentin bond strength and marginal gap formation between the restorative material groups, and negative effects of long-term storage were observed. µTBS correlated significantly with certain clinical outcomes of Class I restorations, while µSBS correlated with certain clinical outcomes of Class II, III, IV and V restorations. Marginal gap formation in enamel and number of paramarginal fractures correlated with certain clinical outcomes of Class II restorations. Background and objective: The aim of this randomized split-mouth study-controlled clinical trial was to compare the 2-year clinical performance of resin composite restorations placed at non-caries cervical lesions (NCCL) with one-step self-etch, total-etch, and selective enamel etch and self-etch adhesive techniques. Materials and methods: Thirty-two patients received three resin composite restorations each at NCCLs (Tetric EvoCeram/Ivoclar/Vivadent), bonded with a total-etch adhesive agent (ExciTE F/Ivoclar/Vivadent) and a self-etch (AdheSE One F/Ivoclar/Vivadent) without and with selective enamel etching. All restorations were evaluated by two examiners at baseline, 6-, 12-, 18-, and 24-months with FDI clinical criteria (post-operation regarding retention, caries occurrence, marginal adaptation, and marginal staining). A logistic regression analysis, a Cohen's kappa statistic, a multifactorial analysis, and X2 were performed with generalized estimating equations. Results: After 2 years, the retention rate was 86.8% for total etch, 92.26% for self-etch, and 93.63% for selective enamel etching and self-etch. No caries was detected on the restorations. Concerning marginal adaptation, the clinically perfect restorations were 26.9% for the total-etch technique, 16% for self-etch, and 25.9% for selective enamel etch and self-etch. The logistic regression model revealed that only time reduced the probability of perfect marginal adaptation. Conclusions: All three adhesive strategies provided restorations with no significant differences in the retention rate or marginal adaptation, whereas the total etch yielded better performance for marginal staining. The stainless-steel crown (SSC) is a durable restoration and has several indications for use in primary teeth such as following a pulpotomy/pulpectomy, fractured teeth, teeth with developmental defects, or large multi-surface caries lesions where amalgam is likely to fail. Due to its durability and a lifespan like the primary tooth, it could well be the gold standard in restorative care. SSCs protect the crown from fracture, reduce the possibility for leakage, and ensure a biological seal. However, the placement of the SSC should follow a meticulous technique. There are some clinical situations where the SSC may fail, leading to plaque accumulation and gingivitis. This could be secondary to improper crimping of crown margins, which lead to poorly adapted SSC. In some clinical situations, ledge formation under the crown or failure to clean excess cement can contact the gingiva and cause gingival inflammation. This study was carried out on 41 children between the ages of 4 and 10 in Al Qassim region to study the effects of SSCs on gingiva and oral hygiene. The study also aims to establish the correlation between SSC adaptation and post-insertion inflammation. The plaque and gingival index were recorded at 3 months’ post SSC insertion. Clinical examination was undertaken, and gingival index (Loe and Silness 1967) and plaque index (Silness and Loe 1967) were used to record gingival health and plaque accumulation, respectively. The result for post-inflammation and SSC adaptation showed that there was no statistically significant difference in post-insertion inflammation and crown adaptation (P value = 0.216). The result for pre-operative inflammation and post-operative inflammation shows that there is no significant difference in post-inflammation and adaptation (P value = 0.47). We found that oral hygiene care had a heightening effect and oral hygiene maintenance plays a key role in preventing gingival inflammation irrespective of the SSC adaptation over short periods of time (3 months).

This study aimed to evaluate the effects of different resin-coating technique strategies and dual-cure resin luting materials on proximal marginal adaptation and the microtensile bond strengths (μTBSs) of CAD/CAM hybrid ceramic inlays. Extracted human molars were classified into four groups, depending on the coating technique: No coating (None), single coating (1-coating), double coating (2-coating), and flowable resin-coating (Combination). The inlays were bonded with one of the three materials: Panavia V5 (V5), Rely X Ultimate (RXU), and Calibra Ceram (CC). The differences with regard to adaptation were not significant. In the case of μTBS data for V5, no significant differences were observed, whereas for RXU, μTBS values for Combination statistically exceeded those for None and 1-coating. Purpose: To evaluate the marginal adaptation and fracture resistance of three computer-aided design/computer-assisted manufacturing hybrid dental materials with different occlusal thicknesses. Methods: Ninety single-molar crowns were digitally fabricated using a milled hybrid nanoceramic (Cerasmart, CE), polymer-infiltrated ceramic network (PICN, Vita Enamic, VE), and 3D-printed materials (Varseosmile, VS) with occlusal thicknesses of 0.8, 1, and 1.5 mm (10 specimens/group). Anatomical 3D-printed resin dies (Rigid 10K) were used as supporting materials. A CEREC MCX milling unit and a DLP-based 3D printer, Freeform Pro 2, were utilized to produce the crown samples. Before cementation, the marginal adaptation, absolute marginal discrepancy (AMD), and marginal gap (MG) were assessed using micro-CT scanning. After cementation with self-adhesive resin cement, fracture resistance was evaluated using a universal testing machine. The number of fractured crowns and the maximum fracture values (N) were recorded. Data were statistically analyzed using both one- and two-way ANOVA, followed by Tukey's honestly significant difference (HSD) test. Results: For all occlusal thicknesses, the VS crowns demonstrated the lowest AMD and MG distances, significantly different from those of the other two milling groups (P < 0.05), whereas CE and VE did not differ significantly (P > 0.05). All VS crowns were fractured using the lowest loading forces (1480.3±226.1 to 1747.2±108.7 N). No CE and 1 and 1.5 mm VE crowns fractured under a 2000 N maximum load. Conclusions: All hybrid-material crowns demonstrated favorable marginal adaptation within a clinically acceptable range, with 3D printing yielding superior results to milling. Featured Application: This is the first investigation to quantitatively assess the porosity of a novel MTA brand, Harvard MTA, in terms of all three of its aspects (interfacial, internal, and total) through the use of micro-computed tomography. When applied as a retrofilling, the experimental results demonstrated excellent marginal adaptability and filling capacity for this calcium silicate cement. These findings could be beneficial to clinicians in selecting the most appropriate root-end filling biomaterial for endodontic surgery. Furthermore, they could be helpful for other endodontic applications of Harvard MTA Universal. Marginal adaptation and internal porosity characteristics of root-end filling materials are important factors determining their clinical performance. The aim of this study was to quantitatively evaluate the marginal adaptation to radicular dentin (interfacial void volume) and internal porosity volume of a novel mineral trioxide aggregate brand using micro-CT analysis. Ten extracted upper central incisors were selected, instrumented, and obturated. Roots were resected at the apical 3 mm, and root-end cavities were prepared ultrasonically and filled with Harvard MTA. SkyScan 1272 micro-CT equipment was used to scan the specimens at a resolution of 6 μm, and three-dimensional images were reconstructed. All volumetric porosity parameters of the tested material were calculated in absolute (mm3) and relative values (%), as follows: open porosity volume (OPV), closed porosity volume (CPV), and total porosity volume (TPV). The mean OPV and OPV% found for Harvard MTA were 0.0268 mm3 and 0.91%, respectively. The mean CPV and CPV% were 0.0283 mm3 and 0.94%, respectively. The TPV and TPV% were 0.0569 mm3 and 1.85%. There was no significant difference between the OPV% and CPV% (p < 0.05). In conclusion, when applied as a retrofilling material, Harvard MTA exhibited excellent marginal adaptation to the dentin with minimal interfacial voids and internal microporosity. Objective: This research is meant to evaluate the effectiveness of different bonding agents in minimally invasive cavity preparations at a tertiary care center. Materials and Methods: A prospective research was conducted involving patients requiring minimally invasive cavity preparations. Patients were randomly assigned to groups based on the bonding agent used. Standardized cavity preparations were performed, and bonding agent application followed manufacturer instructions. Outcome measures included bond strength and marginal adaptation. Data were analyzed using appropriate statistical methods. Results: Bonding Agent A exhibited the highest mean bond strength (25.6 MPa) and superior marginal adaptation (mean score: 4.2) compared to Bonding Agents B and C. These differences were statistically significant (P < 0.001 for bond strength, P < 0.05 for marginal adaptation). Conclusion: Bonding Agent A demonstrated superior performance in achieving durable and esthetically pleasing restorations in minimally invasive cavity preparations. These findings underscore the importance of selecting appropriate bonding agents for conservative dental treatments to optimize treatment outcomes and patient satisfaction.

Aims: The present study aimed to compare the fracture resistance and marginal adaptation of endocrowns fabricated using lithium disilicate (LDS) and zirconia-reinforced lithium silicate 9ZLS) ceramics by the computer-aided design and computer-aided manufacturing (CAD/CAM) technology. Materials and Methods: In this in vitro experimental study, 24 extracted mandibular first molars received standard endocrown preparation and were randomly assigned to two groups (n = 12) for the fabrication of endocrowns with ZLS (Suprinity) and LDS (IPS e.max CAD) ceramics. After scanning the teeth with a CAD scanner, endocrowns were designed by inLab Software version 15 (inLab SW 15) and prepared by an imes-icore 350i milling machine. The vertical marginal gap of endocrowns was measured under a stereomicroscope at three steps before and after cementation and after thermomechanical cycles. The fracture resistance of specimens was then measured by load application at a 45° angle. Mode of failure was also determined as reparable or irreparable. Data were analyzed using Pearson’s correlation test and t-test. Results: The mean fracture resistance of ZLS endocrowns was significantly higher than that of LDS endocrowns (P = 0.000). Purpose: Indirect restorations can be performed using different impression techniques (conventional, laboratory, or intraoral scanners). Their accuracy determined their longevity. This study compares the marginal and internal adaptation of milled CAD/CAM composite inlays fabricated by conventional, hybrid, and digital methods. Material and methods: Thirty human premolars were prepared for MOD composite inlays(CI). They were divided into three equal groups depending on the process of fabrication: Group 1 (conventional group)— impression and laboratory-fabricated CI, Group 2 (hybrid group)— impression, laboratory scanner and milled CAD/CAM CI, and Group 3(digital group)— digital impression (intraoral scanner) and milled CAD/CAM CI. The marginal gap (MG), absolute marginal discrepancy (AMD), and internal gap (IG) were measured at 120 different points per CI using X‐ray microtomography. The data were analyzed using Kolmogorov–Smirnov and Mann–Whitney U tests. Results: The conventional and digital composite inlays present significantly smaller marginal and internal gaps than the CAD/CAM group manufactured by hybrid methods. Conclusions: Within the limitations of this in vitro study, we concluded that the CAD-CAM composite inlays fabricated by digital method exhibited statistically better marginal and internal adaptation results than composite CAD-CAM inlays by hybrid methods. The composite CAD-CAM inlays made by digital methods are an alternative to those made by a conventional methodology.

(1) Background: Restoring decayed teeth in young patients can be challenging. This calls for a simplification of the protocols through new biomaterials. Therefore, the aim of this study was to compare the marginal adaptation delivered by restorative materials applied on class II cavities by using a simplified protocol, before and after fatigue test, followed by the assessment of the internal adaptation. (2) Methods: Forty-eight human teeth were divided into six groups (n = 8). Dentinal fluid simulation was performed before restoring the class II cavities: Gr 1—adhesive (Clearfil Universal Bond Quick) and nanohybrid flowable composite (Clearfil Majesty ES Super Low Flow), Gr 2—adhesive (Clearfil Universal Bond Quick) and nanohybrid composite (Clearfil Majesty ES standard), Gr 3—bulk fill self-adhesive composite (Surefil One), Gr 4—bioactive powder-liquid filling material (Cention Forte), Gr 5—universal adhesive (Adhese Universal) and nanohybrid composite resin (Tetric Powerfill); and control group (CT)—high-viscosity glass ionomer (Equia Forte). Marginal adaptation was observed with scanning electron microscopy (SEM) and compared before and after a fatigue test consisting of repeated thermal and mechanical cycles. The specimens were then cut mesio-distally, and internal adaptation was undertaken using SEM again. Repeated measures and one way ANOVA followed by a Fisher’s LSD test and Fisher’s LSD post hoc test were used in order to compare the statistically significant differences among groups. (3) Results: As for the marginal adaptation after loading, Cention Forte (58%) and Equia Forte HT (53%) were statistically equivalent and presented the highest results, followed by Clearfil Majesty ES Standard (32%) and Tetric Powerfill (27%), with Surefil One (8%) and Clearfil Majesty ES Flow Super Low (7%) showing the worst results. In terms of internal adaptation, Cention Forte (85%) and Clearfil Majesty ES Standard (74%) had the highest percentages of continuous margins. Tetric powerfill (56%) and Equia Forte HT (44%) showed statistically significantly lower results, followed by Clearfil Majesty ES Flow Super Low (33%) and eventually Surefil One (17%). (4) Conclusions: This in vitro study showed promising results for the marginal and internal adaptation of alkasite dual cured Cention Forte in the restoration of class II cavities. This material could be considered an interesting restorative alternative for the restoration of deciduous teeth.

Purpose: To investigate marginal integrity of restorations applied with preheated and non-preheated composite, flowable composite, sonically activated composite, and a new thermo-viscous bulk-fill composite using near-infrared technology for preheating, in class V cavities of human molars. Materials and Methods: Standardized cavities were prepared on the buccal surfaces of 60 human mandibular molars and restored with one of the following resin composite materials after application of an etch-and-rinse adhesive (Opti-Bond FL, Kerr): non-preheated or preheated conventional composite (Filtek Supreme XTE, 3M Oral Care), preheated thermo-viscous composite (VisCalor bulk, Voco), soncially activated composite (SonicFill 3, Kerr), or flowable composite (Filtek Supreme XTE Flowable, 3M Oral Care) applied in bulk or as a lining material using the snow-plow technique. After light curing and polishing, the percentage of continuous margins (PCM) of the restorations in enamel and dentin was assessed using SEM both before and after thermomechanical loading (TML). TML was carried out with 3000 thermal cycles (5°C-50°C) and a simultaneous mechanical stress application with 1.2 million load-cycles (1.7 Hz, 49 N) in a computer-controlled masticator. Non-parametric statistical analysis was performed using Wilcoxon, Kruskal- Wallis, and Mann-Whitney U-tests (2= 0.05). Results: All groups revealed a significant decline in marginal integrity after TML in both enamel and dentin. Although the flowable group in enamel and the snow-plow group in dentin showed the highest PCM before TML, the differences between the groups were compensated after TML. Conclusion: All of the tested composites and application methods showed similar marginal integrities after thermomechanical loading and can be recommended for clinical implementation. After scanning the teeth with a CAD scanner, endocrowns were designed by inLab Software version 15 (inLab SW 15) and prepared by an imes-icore 350i milling machine. The vertical marginal gap of endocrowns was measured under a stereomicroscope at three steps before and after cementation and after thermomechanical cycles. The fracture resistance of specimens was then measured by load application at a 45° angle. Mode of failure was also determined as reparable or irreparable. Data were analyzed using Pearson's correlation test and t -test. The mean fracture resistance of ZLS endocrowns was significantly higher than that of LDS endocrowns (P = 0.000). The reparability of ZLS endocrowns was zero, while that of LDS endocrowns was 83.33 %. The vertical marginal gap was significantly smaller in ZLS than in LDS endocrowns at all three time points (P < 0.05). Also, the marginal gap increased by cementation and thermomechanical cycles in both groups. ZLS and LDS endocrowns both showed acceptable vertical marginal adaptation. Background: The emergence of new hydraulic calcium silicate cements has revolutionized endodontics, addressing the limitations of mineral trioxide aggregate (MTA). The aim of this study is to assess and compare the marginal adaptation of two calcium silicate-based cements (White ProRoot MTA® and TotalFill® BC RRM Fast Set PuttyTM), when creating apical plugs for teeth with open apices. Methods: twenty-four single-rooted teeth were divided into two groups—GMTA (plug with MTA®) and GTBC (plug with TotalFill® BC RRM Fast Set PuttyTM)—and were sectioned at 1 mm and 2 mm from the apex. The transverse sections were analyzed with scanning electron microscopy and the marginal adaptation of the cements was measured with ImageJ® 1.3 software. Statistical analysis (IBM® SPSS® statistics software version 27) was performed and statistical significance was set at 0.05 (p < 0.05). Results: There was a significant difference in the apical region, favoring White ProRoot MTA®, with a lower percentage of marginal adaptation failure (1.32 ± 4.47), presenting a statistically significant difference in the apical region (p = 0.029) but not in the cervical region of the apical plugs (p = 0.774). Background: Several approaches were introduced in placing the composite resin into the prepared cavity. Some researchers recommend using an incremental technique. The material is gradually placed in 2 mm or less thick layers, resulting in better curing by adequate light penetration and better composite resin polymerization. However, disadvantages like voids and the time taken to complete the procedure are lengthier due to the time consumption in placement and polymerization of each increment. Time-saving restorative materials are an ongoing demand for posterior restorations. A newer resin-based composite, bulk-fill composites, have been introduced to speed up the restoration process by enabling up to 4-5 mm thick increments to be cured in one step, thus skipping the time-consuming layering process. Materials and Methods: The randomized clinical trial was conducted on 40 patients with 80 restorations. Randomization was followed by using a random number table. Patients with at least two class I or class II restorations were selected. One tooth is restored with bulk-fill followed by capping with ceram x, and the other with bulk-fill followed by Filtek Z250. All the patients were recalled after 6 months and 12 months. Two examiners evaluated restorations using Federation Dentaire International (FDI) criteria at baseline, 3, 6, and 12 months. Results: The collected data from the participants were analyzed using SPSS software. There was no statistically significant difference observed between the two restorations placed with two nanohybrid composites (ceram x and Filtek Z250). Conclusion: The study concludes that bulk-fill resin composites capped with nanohybrid composites show excellent clinical performance equal to conventional composites and are beneficial to the clinician in reducing the time. Dental crown marginal adaptation is a matter of the success of dental restoration treatment. Nowadays, there are many technological ways for crown manufacturing, such as tridimensional printing of an exactly desired shape through CAD-assisted systems and the appropriate shape milling of a predesigned bulk crown. Both methods are developed for patient benefits. The current research aims to investigate the marginal adaptation of E-Max crowns manufactured by printing-pressed and milling methods. The in vitro cementation procedures were effectuated on healthy teeth extracted for orthodontic purposes according to the standard procedures and the marginal adaptation was investigated with SEM microscopy. The restoration overview was inspected at a magnification of 100× and the microstructural details at 400×. The integrity of marginal adaptation was properly inspected in identical samples on segments of 2 mm from each buccal, palatal, distal and mesial side. The obtained results reveal a good marginal adaptation for all samples, with some particularities. The statistical analysis shows that the best values of the marginal adaptation were obtained for vestibular/buccal and palatal sides of the teeth being situated around 90–95%, while the values obtained for distal and mesial sides are slightly lower such as 80–90%. Furthermore, it was observed that the milled crowns presents better marginal adaptations than the printed-pressed ones, sustained by the statistical p < 0.05. This indicates that the milling process allows a better fit of the crown to the tooth surface and preserves the integrity of the bonding cement layer. [ABSTRACT FROM AUTHOR]

Abstract Background The successful integration of resin-based dental adhesives significantly impacts restorative dentistry, providing efficient and aesthetically pleasing caries treatments. Among various adhesives, one-step self-etching adhesives (1-SEAs) have gained popularity due to their simplicity and short application time. However, concerns have been raised regarding their bonding performance and marginal adaptation characteristics, which differ from two-step self-etching adhesives (2-SEAs) and three-step etch-and-rinse adhesives. Additionally, light-cured bonding materials may encounter challenges in deep cavities and inaccessible areas, necessitating extended light irradiation time. Autocured bonding materials are a potential solution, but limited comparative studies have been conducted on their performance. Methods In this in vitro study, we evaluated a new autocured universal bonding material (Bondmer Lightless 2) and compared the results with recent light-cured bonding materials. Microshear bond strength (μSBS) tests were performed on 25 human molars using five different combinations of adhesives and composite resins: Bondmer Lightless 2 with Estelite Bulk Fill Flow (BE group), Bondmer Lightless 2 with a prototype composite resin (BO group), Prime&Bond Universal with SDR flow + (PS group), Scotchbond Universal with Filtek Bulk Fill (SF group), and G-Premio Bond with Gracefil BulkFlo (GG group). The bond strengths and failure modes were assessed using a universal testing machine and scanning electron microscope (SEM), respectively. Marginal adaptation was evaluated using swept-source optical coherence tomography (SS-OCT) and confocal laser scanning microscopy (CLSM) on 40 sound bovine maxillary incisors. Results The μSBS test showed no significant differences in bond strength among the tested groups. Most failure modes were observed at the bond interface between the adhesive and the dentin. The autocured bonding material demonstrated significantly higher marginal adaptation (SI%) than PS, SF, and GG. The CLSM images corresponded with gaps observed in the SS-OCT images, indicating improved marginal sealing in the autocured group. Conclusions The new autocured universal bonding material exhibited comparable bond strength to a conventional light-cured material while demonstrating a superior marginal adaptation level. This finding suggested that the autocured material could be a valuable alternative, especially when extended light irradiation would pose a challenge. Further clinical studies would be warranted to evaluate the performance of the autocured bonding material in actual restorative dental practice.

Abstract Background Treated dentin matrix hydrogel (TDMH) has been introduced as a novel injectable direct pulp capping material. In this regard, this study aimed to evaluate its marginal adaptation, physicochemical and rheological properties for the development of clinically feasible TDMH. Methods TDMH was applied to the pulp floor of prepared Class I cavities (n = 5), marginal adaptation was assessed by SEM at 1000 X magnification to detect gap between dentin and filling material. Five syringes were filled with TDMH and placed between the compression plates of a universal testing machine to evaluate injectability and gelation time was also evaluated by test vial inverting method. The microstructures of lyophilized TDMH were observed by SEM. Moreover, TDMH discs (n = 5) were prepared and the water uptake (%) was determined based on the equilibrium swelling theory state of hydrogels. Its solubility was measured after one week by the ISO standard method. Rheological behaviours of TDMH (n = 5) were analysed with a rotational rheometer by computing their complex shear modulus G* and their associated storage modulus (G′) and loss modulus (G′’). Statistical analysis was performed using F test (ANOVA) with repeated measures and Post Hoc Test (p = 0.05). Results TDMH presented an overall 92.20 ± 2.95% of continuous margins. It exhibited gelation during the first minute, and injectability mean was 66 ± 0.36%. TDMH showed a highly porous structure, and the pores were interconnected with an average diameter about 5.09 ± 3.17 μm. Swelling equilibrium gradually reached at 6 days up to 377%. The prepared hydrogels and maintained their shape after absorbing over three times their original weight of water. TDMH fulfilled the requirements of ISO 6876, demonstrating a weight loss of 1.98 ± 0.09% and linear viscoelastic behaviour with G` 479.2 ± 12.7 and G`` 230.8 ± 13.8. Conclusions TDMH provided good marginal adaptation, appropriate physicochemical and viscoelastic properties support its use as a novel direct pulp capping material in future clinical applications.

Abstract Objective The aim of this in vitro study was to compare the effect of printing layer thickness on the marginal and internal fit of interim crowns. Material and methods A maxillary first molar model was prepared for ceramic restoration. Thirty‐six crowns were printed with three different layer thicknesses using a digital light processing‐based three‐dimensional printer (25, 50, and 100 µm [LT 25, LT 50, and LT 100]). The marginal and internal gaps of the crowns were measured with replica technique. An analysis of variance was conducted to determine if there were significant differences between the groups (ɑ = .05). Results The marginal gap of LT 100 group was significantly higher than that LT 25 (p = .002) and LT 50 groups (p ≤ .001). The LT 25 group has significantly larger axial gaps than LT 50 group (p = .013); however, there were no statistically significant differences between other groups. The LT 50 group showed the smallest axio‐occlusal gap. The mean occlusal gap differed significantly by printing layer thickness (p ≤ .001), with the largest gap occurring for LT 100. Conclusions Provisional crowns printed with 50 µm layer thickness provided the best marginal and internal fit. Clinical significance It is recommended that provisional crowns be printed with a 50 µm layer thickness to ensure optimal marginal and internal fit.

Background and objective: The aim of this randomized split-mouth study-controlled clinical trial was to compare the 2-year clinical performance of resin composite restorations placed at non-caries cervical lesions (NCCL) with one-step self-etch, total-etch, and selective enamel etch and self-etch adhesive techniques. Materials and methods: Thirty-two patients received three resin composite restorations each at NCCLs (Tetric EvoCeram/Ivoclar/Vivadent), bonded with a total-etch adhesive agent (ExciTE F/Ivoclar/Vivadent) and a self-etch (AdheSE One F/Ivoclar/Vivadent) without and with selective enamel etching. All restorations were evaluated by two examiners at baseline, 6-, 12-, 18-, and 24-months with FDI clinical criteria (post-operation regarding retention, caries occurrence, marginal adaptation, and marginal staining). A logistic regression analysis, a Cohen’s kappa statistic, a multifactorial analysis, and X2 were performed with generalized estimating equations. Results: After 2 years, the retention rate was 86.8% for total etch, 92.26% for self-etch, and 93.63% for selective enamel etching and self-etch. No caries was detected on the restorations. Concerning marginal adaptation, the clinically perfect restorations were 26.9% for the total-etch technique, 16% for self-etch, and 25.9% for selective enamel etch and self-etch. The logistic regression model revealed that only time reduced the probability of perfect marginal adaptation. Conclusions: All three adhesive strategies provided restorations with no significant differences in the retention rate or marginal adaptation, whereas the total etch yielded better performance for marginal staining. All restorations were assessed as clinically acceptable after 2 years.

Abstract Background Straight preparable abutments and titanium bases (ti-base) can be used to support single-unit screw-retained lithium disilicate implant-supported restorations. The choice between using both abutments depends on many factors. The purpose of this in vitro study was to compare the masking ability, marginal adaptation, and fracture resistance of screw-retained lithium disilicate implant-supported crowns cemented to straight preparable abutments and ti-bases. Methods Twenty laboratory implant analogs (Straumann Bone Level; Straumann AG) were randomly divided into 2 groups (n = 10 each) according to the type of the abutment used. Preparable abutment group and ti-base group. Lithium disilicate crowns were used to restore the specimens. All specimens were subjected to thermocycling (from 5 to 55 °C for 2000 cycles) followed by cyclic loading (120 000 cycles). The vertical marginal gap between the abutment finish line and the most apical part of the crown was measured in (µm) by using a stereomicroscope after cementation and after thermocycling and cyclic loading. A spectrophotometer was used to evaluate the masking ability of the specimens after cementation. The load required to fracture the crowns was measured in Newtons (N) by using a universal testing machine after thermocycling and cyclic loading. The Shapiro-Wilk test of normality was used. The appropriate statistical test was used. Results Regarding the masking ability, the color difference (∆E) showed no statistically significant difference between the ti-base group (2.6 ± 0.2) and the preparable abutment group (2.6 ± 0.3) (P = .888). The average of the microgap values (µm) was greater in ti-basegroup after cementation (13.9 ± 9.2) than preparable group (7.63 ± 1.78) with no statistically significant difference between the 2 groups (P = .49). After cyclic loading and thermocycling, the average microgap values (µm) was significantly greater in the ti base group (21.3 ± 7.4) than in preparable group (13.3 ± 1.5) (P = .02). The load required to fracture the specimens was greater in the preparable group (1671.5 ± 143.8) than in the ti-base group (1550.2 ± 157.5) with no statistically significant difference between the 2 groups (P = .089). Conclusion The abutments used in the present study did not compromise the masking ability of the screw-retained lithium disilicate implant supported crowns. Moreover, the crowns cemented to preparable abutments had better marginal adaptation and higher fracture resistance when compared to those cemented to ti-bases. Clinical implications Straight preparable abutments are considered as an alternative to the ti-bases when restoring single screw-retained lithium disilicate implant-supported crowns with comparable fracture resistance, marginal adaptation, and masking ability.

Abstract Background Insufficient research has been conducted in the literature assessing the performance of zirconia and polyetheretherketone (PEEK) crowns in relation to the essential requirements of successful restorations, such as fracture resistance or margin adaptation. The purpose of this study was to evaluate the effect of the coping materials zirconia or PEEK with different fabrication techniques on the vertical marginal gap and fracture resistance of posterior crowns with composite veneering. Methods Ceramic copings (n = 18) restoring mandibular first molar were fabricated from zirconia (Zircon.x, Presidentdental, Germany), milled PEEK (PEEK CAD) (breCAM.BioHPP, Bredent, Germany) and pressed PEEK (PEEK Press) (BioHPP Granules, Bredent, Germany) six specimens each (n = 6). The copings were veneered with high impact polymer composite (HIPC) material (breCAM.HIPC, Bredent, Germany). The vertical marginal gap was captured under a magnification of 40X. Five equidistant marks on each surface of the die distinguished the points of measurement for a total of 20 readings per sample. The analysis was completed using an image analysis system (ImageJ 1.53t, National Institute of Health, USA). The specimens were loaded to failure at a crosshead speed of 1 mm/min and the load at failure was recorded to measure the fracture resistance. Results The marginal gap was analyzed using one-way ANOVA followed by Tukey’s post hoc test. Fracture resistance was analyzed using Welch one-way ANOVA followed by the Games-Howell post hoc test. Purpose This in vitro study evaluated the internal and marginal adaptability of different composite resins in class V cavities. Materials and methods Thirty human maxillary premolars were selected. Trapezoidal class V were prepared on buccal surface. Flowable, self-adhering flowable and nanohybrid composites were applied following their manufacture instructions, in groups I, II and III respectively. Specimens were subjected to 500 thermal cycles and then examined under a stereomicroscope to determine the marginal gap width. All specimens were then sectioned buccolingually into two sections to detect and measure the internal gap width. Results The highest mean value of occlusal gap was recorded for Group II followed by Group I while the lowest value was recorded in Group III. Similar results were found regarding the cervical margins, with no statistically significant difference between the groups. Comparing occlusal versus cervical marginal gap a highly significance difference was recorded in each group. The highest internal mean gap width was recorded in Group III, followed by Group II while the lowest in Group I, with no significant difference between them. Conclusion All tested materials showed satisfactory marginal adaptation with no significant benefits of the self-adhesive composite. Aim The aim of this in-vitro study was to evaluate the Marginal Accuracy of lithium di-silicate Monolithic Veneers when they are milled by Different CAD/CAM systems. Materials and methods A maxillary ready-made scannable model containing central incisor tooth with laminate veneer preparation was used to generate the design of the virtual master die and using CEREC in lab software the final design of the laminate veneer restoration was made and then was exported to the 2 tested milling machines MCX5 and Ceramill motion 2, the milled monolithic lithium di-silicate veneers was then cemented to their corresponding three-dimensional printed dies and was examined under Digital microscope for values of vertical marginal gap. Results There was statistically significant difference in marginal gaP values between the two studied groups (P < 0.05). Conclusion Veneers produced by both milling machines were with clinically acceptable marginal adaptation greater than 120 μm, However there was statistically significant difference in the vertical marginal gap values of the produced veneer between the two test groups CEREC MCX5 and Amangerbach Ceramill motion 2, with CEREC MCX5 milling machine having the advantage of milling veneer restoration with better marginal adaptation and less vertical marginal gap values. Objective: Testing the influence of various restorative materials (conventional or bulkfill composites) and filling techniques (single- or multi-layered techniques) on the in vitro marginal adaptation of large class II direct composites with supra and sub-gingival margins subjected to thermomechanical loading (TML). Methods: A total of 40 prepared teeth were attributed randomly to five experimental groups. In Group 1, restorations were made of multi-layered high-viscosity conventional composite (Tetric EvoCeram); in Groups 2 and 3, restorations were made of a high viscosity bulkfill composite (Tetric EvoCeram Bulk Fill) applied in one (Group 2) or three layers (Group 3); in Groups 4 and 5, restorations were made of a flowable bulkfill composite (SDR flow) applied in one (Group 4) or two layers (Group 5), covered with a layer of high-viscosity conventional composite (Ceram-X Universal). A single adhesive (OptiBond FL) was used in all groups. All specimens were submitted to a staged TML comprising three phases (2 × 500,000 and 1,000,000 cycles) at 50 N with 3350 thermal cycles (5 to 55 °C) every 500,000 cycles. The tooth–restoration interfaces on proximal surfaces were evaluated quantitatively by scanning electron microscopy, before and after each TML phase, hence at three timepoints (T0, T1, T2 and T3). The following segments were considered for evaluation: proximal, vertical enamel margins (assessed individually on both restoration sides), cervical enamel (restoration side above CEJ) and cervical dentin margin (restoration side below the CEJ). Results: TML induced a significant reduction in continuous adaptation at both enamel and dentin margins in all groups, with percentages of continuous margins ranging from 75.2 to 91.8% at T0, and decreasing to values ranging from 21.3 to 73.9% at T3. Both composite systems and layering protocols had a significant influence on marginal adaptation of the restorations, with statistical associations depending on the restoration group and the timepoint considered. Defective margins in enamel were all of a cohesive nature with micro-fractures, while in dentin, interfacial gaps were the main defect observed. Conclusions/Clinical significance: The present results highlighted significant degradation of marginal adaption after long-term in vitro fatigue test using materials even with high-viscosity conventional resin composites, applied with a proper layering approach in medium–large sub-gingival cavities. While no significant differences were observed at the dentin cervical margins, there was a tendency for better adaptation at the enamel margin when using a higher modulus material with a multi-layered technique. Objectives: Marginal adaptation is considered one of the key factors influencing the success of indirect restorations. This study aimed to estimate the marginal fit of lithium disilicate overlays with three distinct preparation designs before and after cementation. Methods: Thirty maxillary first premolars were divided into the hollow chamfer design (HCD11 HCD: Hollow Chamfer Design.) group, butt-joint design (BJD22 BJD: Butt-Joint Design.) group, and conventional occlusal box design (COD33 COD: Conventional Occlusal Design.) group (n = 10 each). The samples were scanned using an intra-oral scanner, and overlays were fabricated using computer-assisted design and milled on a computer-assisted machine. The finished restorations were luted using a self-adhesive resin RelyX Ultimate. The marginal gap was assessed using a digital microscope with 230X magnification power. Statistical analysis was conducted using analysis of variance and post hoc (Bonferroni correction) tests, assuming a significance level of 5%. Results: The HCD and BJD groups recorded significantly lower marginal gap, (11.39 ± 0.72, 16.29 ± 0.75) and (11.59 ± 0.75, 16.93 ± 0.65) respectively, than the COD group (24.57 ± 1.18, 34.45 ± 1.09) both pre- and post-cementation. Conclusion: This study demonstrated that modification of tooth preparation plays a significant role in the marginal adaptation of the lithium disilicate overlays. The gap was smaller with the HCD and BJD than with the COD, with a statistically significant difference.

Introduction: Successful periapical surgery requires appropriate root resection, preparation, and adequate sealing.Aim: The aim of the present study was to assess the marginal adaptation of MTA and Biodentine after apical resection with an Er:YAG laser and a diamond turbine bur using a scanning electron microscope (SEM).Materials and methods: The crown part of forty-eight extracted single-root human teeth was removed, and the root canal length of 15 mm was standardized. The root canals were prepared using rotary Ni-Ti Revo-S files up to an apical stop – AS40 and filled with MTA Fillapex and gutta-percha points (cold lateral condensation). The teeth are divided into 2 main groups: group 1 (n=24) after apical resection with a turbine bur, ultrasonic preparation of the retrograde cavity at 3 mm depth and retrograde obturation with Biodentine and MTA; group 2 (n=24) after apical resection with an Er:YAG laser, ultrasonic preparation of the retrograde cavity at a depth of 3 mm and retrograde obturation with MTA and Biodentine. A SEM was used for assessment of the marginal adaptation of the material to the root dentin. The data was entered into and analyzed with IBM SPSS Statistics 22.0.Results: In the group with apical resection with a turbine bur, a statistically significant difference in the gap size between the material and dentin was found in both materials we studied (MTA and Biodentine). The higher mean value was in MTA (1.72 µm), in Biodentine it was 1.08 µm. In the group with apical resection with Er:YAG laser, no statistically significant difference in the gap size between the material and dentin was found in both studied materials: MTA – 1.88 µm, Biodentine – 1.32 µm.Conclusions: In the present study, MTA and Biodentine showed good sealing capabilities after apical resection. Biodentine displayed better marginal adaptation when resecting the root tip using a turbine bur. The Er:YAG laser-assisted apical resection shows sealing of the open dentinal tubules around the resected root surface.
